RESUMO: O estudo de adaptabilidade, estabilidade e produtividade é importante para selecionar e recomendar genótipos superiores. Esse fato é particularmente válido para a introdução de cultivares de feijão comum no Rio de Janeiro, Brasil, cuja produção é ínfima e não atende a demanda interna. Assim, este trabalho teve como objetivo estimar parâmetros genéticos e ganhos de seleção e realizar uma seleção simultânea para adaptabilidade, estabilidade e produtividade de genótipos de feijão preto através de modelos mistos. O estudo foi realizado em três municípios do estado do Rio de Janeiro e em três anos agrícolas. Os ensaios foram instalados em delineamento de blocos casualizados com 11 genótipos e três repetições. Observou-se alta herdabilidade média (81%), acurácia seletiva (90%) e boas perspectivas de seleção. Também foram observados ganhos satisfatórios para a característica entre 1,03 e 9,49%. A seleção simultânea foi eficiente e permitiu selecionar duas linhagens de feijão preto CNFP (15290 e 15361) como sendo as mais produtivas, adaptáveis e estáveis e, portanto, têm potencial para serem lançadas como novas cultivares de feijão preto para o estado do Rio de Janeiro. ABSTRACT: The study of adaptability, stability, and productivity is essential for selecting and recommending superior genotypes. This fact is particularly the case for the introduction of common bean cultivars in Rio de Janeiro, Brazil, whose production is negligible and does not meet the internal demand. Thus, this study estimated genetic parameters and selection gains and undertake a simultaneous selection for adaptability, stability, and productivity in black bean genotypes via mixed models. The investigation was carried out in three municipalities in the state of Rio de Janeiro during three crop years. The trials were set up in a randomized block design with 11 genotypes and three replications. High mean heritability (81%) and selection accuracy (90%), as well as good selection prospects, were observed. Gains between 1.03 and 9.49% were achieved for grain yield. Simultaneous selection was efficient, indicating two black bean lines (CNFP 15290 and CNFP 15361) as the most productive, adaptable, and stable. ABSTRACT BACKGROUND: Stroke is a major cause of mortality worldwide. Renal dysfunction is an important risk factor for stroke. Brazilian studies on stroke knowledge are generally population based. Studies stratifying stroke knowledge according to comorbidities are rare. Scientific data are essential to guide the awareness of stroke. OBJECTIVE: To assess stroke knowledge in patients with chronic kidney disease (CKD) on hemodialysis. DESIGN AND SETTING: Cross-sectional analytical study of patients with CKD on hemodialysis in north-eastern Brazil. METHODS: A self-administered questionnaire survey on stroke awareness was administered to patients with CKD on hemodialysis between April and November 2022. The chi-square test and other descriptive statistics were used. Univariate and multivariate analyses were performed using logistic regression. RESULTS: A total of 197 patients were included in the analysis. The Brazilian acronym for stroke was used by 53.5% of the participants. Less than 10.0% of the sample showed optimal decision-making ability regarding stroke. Of the participants, 29.9% knew at least one risk factor and one symptom; however, this was considered as having below the minimum capacity because they did not know the emergency service call number. In the analysis adjusted for income and education, females (odds ratio [OR], 0.40%; 95% confidence interval [CI], 0.20-0.82), older patients (OR, 0.24%; 95% CI, 0.09-0.63) and having at most one comorbidity (OR, 0.48%; 95% CI, 0.23-0.98) were factors for lower levels of knowledge or ideal decision-making capacity against stroke. CONCLUSIONS: Patients on hemodialysis, especially women and older people, have little knowledge about stroke. Resumo Os padrões espaciais e temporais e a dinâmica da estrutura da comunidade biológica podem ser compreendidos por meio das lentes da teoria de metacomunidades, no qual o efeito dos processos locais (processos determinísticos, teoria do nicho ecológico) e regionais (processos estocásticos, teoria neutra) são destacados como os principais preditores do fitoplâncton. O objetivo deste trabalho foi avaliar o efeito de preditores ambientais locais, espaciais e de paisagem sobre a comunidade fitoplanctônica em lagos da planície de inundação do Rio Araguaia. Avaliamos as seguintes questões: (i) Qual é a importância específica das características físicas e químicas da água (preditores ambientais locais), dos processos dispersivos (preditores espaciais) e do uso e ocupação do solo (preditores de paisagem) na estrutura da metacomunidade fitoplanctônica, tanto para grupos taxonômicos quanto funcionais? (ii) O tamanho do buffer usado na medição do uso e da ocupação do solo ao redor das unidades de amostragem mostra diferenças na previsão da comunidade fitoplanctônica? Todos os preditores explicaram a estrutura do fitoplâncton, mas os espaciais foram os mais importantes. Os buffers mostraram diferentes habilidades preditivas, com a classificação taxonômica sendo relacionada a tamanhos maiores de buffers e grupos funcionais, o oposto. A grande influência dos preditores espaciais pode ser explicada pela dinâmica fonte-sumidouro, em que a dispersão é tão forte que pode diminuir os efeitos dos preditores locais e garantir um grande fluxo de organismos para as comunidades de sumidouros. Abstract The spatial and temporal patterns and dynamics of biological community structure can be better understood through the lens of metacommunity theory, in which the effect of local (deterministic processes, ecological niche theory) and regional (stochastic processes, neutral theory) processes are evaluated as the main predictors of phytoplankton. The objective of this paper was to evaluate the effect of local environmental characteristics, spatial, and landscape predictors on the phytoplankton community in lakes of the Araguaia River floodplain. We evaluated the following questions: (i) What is the specific importance of physical and chemical water characteristics (local environmental predictors), dispersive processes (spatial predictors), and land use and occupancy (landscape predictors) in the phytoplankton metacommunity structure, both for taxonomic and functional groups? (ii) Does the buffer size used in land use and land cover measurement around the sampling units show differences in phytoplankton community prediction? All the predictors could explain the phytoplankton structure but the spatial were the most important. The buffers showed different predictive abilities, with taxonomic classification being related to larger sizes of buffers and functional groups the opposite. The great influence of spatial predictors can be explained by source-sink dynamics, where dispersal is so strong that it can diminish the effects of local predictors and guarantee a large flux of organisms to sink communities. In this study, electrospun nanofibers of poly(vinylidene fluoride) (PVDF) were produced with the addition of [6,6]-phenyl-C61-butyric acid methyl ester (PCBM) and poly(3-hexylthiophene) (P3HT), for application as a nanostructured active layer of bulk heterojunction polymer with fullerene and polythiophene derivatives in third-generation photovoltaic devices. The electrospinning technique was used to favor the formation of the β phase, exhibiting ferroelectric properties, improving the electrical performance of the device. The morphological, structural and electrical characteristics of the nanofibers were investigated using scanning electron microscopy (SEM), Fourier transform infrared spectroscopy (FTIR) and DC electrical measurements. Abstract The study of adaptability and yield stability is essential for the selection and recommendation of new cultivars. Thus, this study determined the adaptability and yield stability of black bean genotypes for cultivar recommendation using a GGE biplot. To this end, nine trials were carried out at different locations in the state of Rio de Janeiro. Eleven genotypes were evaluated in three growing seasons, in a randomized block design, with three replications. Adaptability and stability of grain yield were analyzed by the GGE biplot method. The effects of genotypes, environments and of the genotype - environment interaction were significant, which explained the different responses of the genotypes to the tested environments. RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. ABSTRACT BACKGROUND: Stroke is one of the main causes of death worldwide. Educational interventions on stroke are potentially effective in reducing the period between the onset of symptoms and the initial emergency medical assistance. OBJECTIVES: To assess high school students’ knowledge of stroke. DESIGN AND SETTING: Cross-sectional study conducted in high schools in northeastern Brazil. METHODS: A self-structured questionnaire survey regarding stroke awareness was applied among high school students in northeastern Brazil. Data were collected between 2018 and 2019. The chi-square test and other descriptive statistics were used. Univariate and multivariate analyses were performed using logistic regression. RESULTS: A total of 1,788 students were analyzed. Eighty percent (n = 1430) of them did not have the minimum knowledge on how to act in a stroke situation. Only 10% (n = 179) presented the ideal knowledge on how to act. Males presented lower levels of knowledge on risk factors (odds ratio, OR: 0.62%; 95% confidence interval, CI: 0.49-0.79) and signs and symptoms of stroke (OR: 0.63%; 95% CI: 0.52-0.77). Students with ≥ 10 years of schooling (OR: 1.64%; 95% CI: 1.30-2.07) demonstrated greater knowledge of signs and symptoms of stroke. Students aged 18 years (OR: 1.70%; 95% CI: 1.14-2.52) demonstrated greater knowledge than other ages regarding the telephone number of the emergency medical services. CONCLUSIONS: There was a knowledge deficit with regard to recognizing stroke and activating the emergency medical services. The findings apply to the sample investigated and suggest that there is a need for stroke educational interventions, starting in high school.

Abstract Preparation practices and physicochemical characteristics of moin-moin commercialized in Salvador (Brazil) as a street food by typically clothed women called baianas de acarajé were studied. Four moin-moin samples were collected from each of the thirty sales points included in the study and analyzed for moisture, protein, fat, ash, carbohydrates, fiber, total carotenoids, weight, color and texture. The ingredients used for the preparation of moin-moin were as follows: grated dry shrimp heads (100.0%), grated onions (96.7%), salt (73.3%), Crude Palm Oil (53.3%) or Crude Palm Olein (46.7%) and garlic (16.7%). Peanut, codfish, cashew nuts, wheat flour, ginger and tomatoes are further ingredients frequently used. Texture analysis revealed that moin-moin is a soft, fragile and brittle food product. The content of total carotenoids ranged from 6.99 to 64.88 ppm with an average value of 28.49 ± 4.00 ppm, giving the product an ocher-orange/yellow-orange color. An average sized moin-moin ball (186.6 g) contained 14.08% protein, 13.08% lipids, 30.83% carbohydrates, 4.01% fiber and 297.42 kcal, corresponding to 18.77%, 23.78%, 10.28% and 14.87% of the daily requirements of an adult for protein, lipids, carbohydrates and energy, respectively.

Resumo Objetivo: Avaliar taxas de sucesso e de complicações de biópsias renais guiadas por ultrassonografia em um hospital terciário. Materiais e Métodos: Análise retrospectiva de 97 biópsias renais realizadas entre 1º de março de 2017 e 31 de outubro de 2019, guiadas por ultrassonografia e executadas por um único médico radiologista. Resultados: Oitenta e sete biópsias apresentaram diagnóstico anatomopatológico definitivo. Cinco biópsias foram inconclusivas (5,4%) e precisaram de um segundo procedimento. Houve complicações em sete procedimentos (7,6%), todas devidamente solucionadas. Conclusão: A biópsia renal guiada por ultrassonografia demonstrou ser um método efetivo e seguro para o diagnóstico de nefropatias, com elevada taxa de sucesso em sua execução.
Abstract Objective: To evaluate the success and complication rates of ultrasound-guided renal biopsy at a tertiary care hospital. Materials and Methods: This was a retrospective analysis of 97 ultrasound-guided renal biopsies, all performed by the same radiologist, between 1 March, 2017 and 31 October, 2019. Results: Of the 97 biopsies evaluated, 87 had a definitive pathological diagnosis. In five cases (5.4%), the biopsy results were inconclusive and a second procedure was required. In seven procedures (7.6%), there were complications, all of which were properly resolved. Conclusion: Ultrasound-guided renal biopsy has proven to be a safe, effective method for the diagnosis of nephropathies, with high success rates.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Resumo Fundamento A síndrome de takotsubo (takotsubo) é uma forma de cardiomiopatia adquirida. Dados nacionais sobre essa condição são escassos. O Registro REMUTA é o primeiro a incluir dados multicêntricos dessa condição no nosso país. Objetivo Descrever as características clínicas, prognóstico, tratamento intra-hospitalar e mortalidade hospitalar e em 1 ano de seguimento. Métodos Estudo observacional, retrospectivo, tipo registro. Incluídos pacientes internados com diagnóstico de takotsubo ou que desenvolveram esta condição durante internação por outra causa. Os desfechos avaliados incluíram fator desencadeador, análise dos exames, uso de medicações, complicações e óbito intra-hospitalar e em 1 ano de seguimento. O nível de significância adotado foi de 5%. Resultados Foram incluídos 169 pacientes, em 12 centros no Estado do Rio de Janeiro. A idade média foi de 70,9 ± 14,1 anos e 90,5% eram do sexo feminino; 63% dos casos foram de takotsubo primário e 37% secundário. Troponina I foi positiva em 92,5% dos pacientes e a mediana de BNP foi de 395 (176,5; 1725). Supradesnivelamento do segmento ST esteve presente em 28% dos pacientes. A fração de ejeção do ventrículo esquerdo teve mediana de 40 (35; 48)%. Observamos taxa de 25,7% de ventilação mecânica invasiva e 17,4% de choque. Suporte circulatório mecânico foi utilizado em 7,7%. A mortalidade intra-hospitalar foi de 10,6% e a mortalidade ao final de 1 ano foi de 16,5%. Takotsubo secundário e choque cardiogênico foram preditores independentes de mortalidade. Conclusão Os resultados do REMUTA mostram que takotsubo não se trata de patologia benigna como se pensava, especialmente no grupo de takotsubo secundário que acarreta elevada taxa de complicações e de mortalidade. Abstract Background Takotsubo syndrome (TTS) is an acquired form of cardiomyopathy. National Brazilian data on this condition are scarce. The Takotsubo Multicenter Registry (REMUTA) is the first to include multicenter data on this condition in Brazil. Objective To describe the clinical characteristics, prognosis, in-hospital treatment, in-hospital mortality, and mortality during 1 year of follow-up. Methods This is an observational, retrospective registry study including patients admitted to the hospital with diagnosis of TTS and patients admitted for other reasons who developed this condition. Evaluated outcomes included triggering factor, analysis of exams, use of medications, complications, in-hospital mortality, and mortality during 1 year of follow-up. A significance level of 5% was adopted. Results The registry included 169 patients from 12 centers in the state of Rio de Janeiro, Brazil. Mean age was 70.9 ± 14.1 years, and 90.5% of patients were female; 63% of cases were primary TTS, and 37% were secondary. Troponin I was positive in 92.5% of patients, and median BNP was 395 (176.5; 1725). ST-segment elevation was present in 28% of patients. Median left ventricular ejection fraction was 40 (35; 48)%. We observed invasive mechanical ventilation in 25.7% of cases and shock in 17.4%. Mechanical circulatory support was used in 7.7%. In-hospital mortality was 10.6%, and mortality at 1 year of follow-up was 16.5%. Secondary TTS and cardiogenic shock were independent predictors of mortality. Conclusion The results of the REMUTA show that TTS is not a benign pathology, as was once thought, especially regarding the secondary TTS group, which has a high rate of complications and mortality. RESUMO Objetivo: avaliar as características demográficas e clínicas associadas ao desenvolvimento de pneumonia em pacientes após acidente vascular encefálico internados em um hospital terciário do Vale do São Francisco que atende os estados de Pernambuco e Bahia, Brasil. Métodos: trata-se de estudo unicêntrico, observacional, analítico e transversal realizado a partir dos prontuários de pacientes com diagnóstico de acidente vascular encefálico incluídos no Registro de Acidente Vascular Encefálico (Estudo RAVESS). A análise estatística foi composta pelos testes qui-quadrado, exato de Fisher e a análise de variância com pós-teste de Bonferroni com nível de P≤0,05. Resultados: foram coletados dados de 69 pacientes com AVE agudo com média±desvio padrão de idade de 63,2±16,8 anos, sendo 37 (53,6%) do sexo feminino, com prevalência de pneumonia durante o internamento estimada em 31,9% (Intervalo de Confiança de 95%: 21,2-44,2%). Na análise univariada para preditores de pneumonia após acidente vascular encefálico foram identificados: idade mais avançada (72,6±17,9 vs. 58,8±14,5; P=0,001), menor sinal de resposta a escala de coma de Glasgow durante a admissão (11,3±1,8 vs. 13,3±2,1; P=0,001) e maior frequência de disartria na admissão (61,9% vs. 27,9%; P=0,009). Conclusão: a pneumonia foi uma complicação prevalente em pacientes após acidente vascular encefálico de um hospital terciário brasileiro, tendo sido relacionada com idade avançada do paciente e a severidade do evento encefálico.
ABSTRACT Purpose: to assess the demographic and clinical characteristics associated with the development of pneumonia in post-stroke patients hospitalized in a tertiary hospital, located in the Vale do São Francisco, that covers the states of Pernambuco and Bahia, Brazil. Methods: a unicentric, observational, analytical, cross-sectional study, based on the medical records of patients diagnosed with stroke and included in the Stroke Registry (RAVESS study). The statistical analysis was made with the chi-square test, Fisher’s exact test, and the analysis of variance, with the Bonferroni’s post-test, and P≤0.05. Results: data from 69 patients presented with acute stroke were collected, aged 63.2±16.8 years; 37 (53.6%) were females; the prevalence of pneumonia during hospital stay was estimated at 31.9% (95% confidence interval: 21.2-44.2%). In the univariate analysis of predictors for post-stroke pneumonia, the following were identified: older age (72.6±17.9 vs. 58.8±14.5; P = 0.001), lower response signal to the Glasgow Coma Scale at admission (11.3±1.8 vs. 13.3±2.1; P = 0.001), and higher frequency of dysarthria at admission (61.9% vs. 27.9%; P = 0.009). Conclusion: pneumonia was a prevalent complication in post-stroke patients at a Brazilian tertiary hospital. It was related to the patient’s older age and the severity of the cerebral event.

RESUMO Febre de origem indeterminada (FOI) é uma entidade desafiadora com presença marcante nos hospitais de todo o mundo. É definida como temperatura ≥37,8 ° C em várias ocasiões, com duração ≥3 semanas, na ausência de diagnóstico após três dias de investigação hospitalar ou três consultas ambulatoriais. As principais etiologias são de ordem infecciosa, neoplásica e doenças inflamatórias não infecciosas. O diagnóstico é baseado na história clínica e no exame físico minuciosos desses pacientes, com a finalidade de direcionar os exames complementares específicos a serem realizados em cada caso. A abordagem diagnóstica inicial do paciente com FOI deve incluir exames complementares inespecíficos. A terapia empírica não é recomendada (com poucas exceções) em pacientes com febre prolongada, uma vez que ela pode camuflar e retardar o diagnóstico e a conduta para tratar a etiologia específica. O prognóstico engloba uma mortalidade de 12-35%, variando de acordo com a etiologia de base.
SUMMARY Fever of undetermined origin (FUO) is a challenging entity with a striking presence in hospitals around the world. It is defined as temperature ≥ 37.8 ° C on several occasions, lasting ≥ three weeks, in the absence of diagnosis after three days of hospital investigation or 3 outpatient visits. The main etiologies are infectious, neoplastic, and non-infectious inflammatory diseases. The diagnosis is based on the detailed clinical history and physical examination of these patients, in order to direct the specific complementary tests to be performed in each case. The initial diagnostic approach of the FUO patient should include non-specific complementary exams. Empirical therapy is not recommended (with few exceptions) in patients with prolonged fever, as it may disguise and delay the diagnosis and conduct to treat the specific etiology. The prognosis encompasses mortality of 12-35%, varying according to the baseline etiology.
