This study analyzed federal government management of the National Policy for Comprehensive Men's Health Care (PNAISH) from a critical historical perspective. We analyzed documents addressing the policy implementation process between 2009 and 2024 contained in the archives of the National Office for the Coordination of Men’s Healthcare (COSAH). The content analysis resulted in the following thematic categories: COSAH organization and management; the PNAISH review process; and challenges and key strategic areas for implementing the PNAISH. We found weaknesses in the institutionalization of coordination, in addition to a policy review in 2021 marked by neoconservative discourse excluding gender and sexual and reproductive rights. These epistemological principles of the PNAISH were revisited in 2023, stemming from actions to improve the quality of primary care addressing the effects of machismo and patriarchy on men’s, women’s and children’s health. Este artículo analizó la gestión federal de la Política Nacional de Atención Integral Salud del Hombre (PNAISH), bajo una perspectiva histórico-crítica. Se realizó un análisis documental del proceso de implementación de la política entre 2009 y 2024, contenida en el archivo de la Coordinación de Atención a la Salud del Hombre (COSAH). El análisis de contenido resultó en las siguientes categorías temáticas: organización y gestión de la COSAH; proceso de revisión de la PNAISH; y desafíos y frentes estratégicos para la implementación de la PNAISH. Se encontraron fragilidades en la institucionalización de la coordinación, además de una revisión de la política, en 2021, marcada por el discurso neoconservador que excluyó las menciones a género y derechos sexuales y reproductivos. Esos principios epistemológicos de la PNAISH se retomaron en 2023 a partir de acciones de calificación de la atención primaria para el abordaje de las repercusiones del machismo y del patriarcado sobre la salud de hombres, mujeres y niños. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT Purpose: To investigate the effects of chronic stress in the prostate of prepubertal and adult rats. Methods: Thirty-two male rats were assigned into four groups depending on the type of treatment (control or stressed) and the age at which stress was initiated (prepubertal or adult). Restraint stress stimuli were applied for six weeks. Stressed prepubertal and adult rats evaluated immediately after the last stress stimuli were named SP and SA groups, respectively. Age-matched rats were used as control groups (CP and CA). At the end of the experiment, the rats were euthanized, and prostate morphological parameters were evaluated and statistically compared. Results: Application of stress stimuli to the SP group resulted in reduced body weight, but no prostate morphological modification was noted. The SA group showed reduced testosterone level and prostatic epithelium surface density, in comparison to CA group. Further, the prostatic lumen surface density was increased in adult stressed animals, in comparison to adult controls. Conclusions: The stress stimuli promoted changes in hormonal and morphological parameters in the prostate of adult stressed rats. Abstract Objective: The present study aimed to perform a morphological and morphometric analysis of cochlear structures of C57BL/6J mice receiving oral melatonin for a 12-month period. Methods: 32 male C57BL/6J were divided into control and melatonin groups. Control received saline and ethanol solution and melatonin group, 50 μL of 10 mg of melatonin/kg/day orally for a 12-month period. After de experiment the animals were sacrificed into a 40% concentration of CO2 chamber, and the blades were morphological and morphometrically analyzed. Results: The melatonin group revealed a higher median density of viable cells (45 ± 10.28 cells/100 μm2, 31–73, vs. 32 ± 7.47 cells/100 μm2, 25–48). The median area of stria vascularis was 55.0 ± 12.27 cells/100 μm2 (38–80) in the control, and 59.0 ± 16.13 cells/100 μm2 (40–134) in the melatonin group. The morphometric analysis of the spiral ligament reveals a higher median of total viable neurons in the melatonin (41 ± 7.47 cells/100 μm2, 27–60) than in the control group (31 ± 5.68 cells/100 μm2, 21–44). Conclusion: Although melatonin is a potent antioxidant, it does not completely neutralize the occurrence of presbycusis; however, it may delay the onset of this condition. Level of evidence: 3. Resumo Introdução O controle da disseminação do coronavírus em Instituições de Longa Permanência para Idosos (ILPI) é considerado um desafio, uma vez que os idosos fazem parte do grupo de risco e apresentam prognóstico bastante desfavorável e também alta letalidade. Objetivo Conduzir uma rapid review para mapear e sintetizar a literatura sobre medidas de enfrentamento de Coronavirus Disease-2019 (COVID-19) em ILPI. Método Foi realizada uma Rapid review, e as buscas foram conduzidas nas bases de dados eletrônicas Biblioteca Cochrane, Web of Science, Scopus, Scielo, Medline/Pubmed e Google Scholar. Foram incluídas publicações a partir de 2019, nos idiomas português, inglês ou espanhol. A seleção das publicações ocorreu em duas etapas: leitura dos títulos/resumos; e leitura na íntegra de todas as publicações selecionadas. As recomendações para enfrentamento da COVID-19 em Instituições de Longa Permanência para Idosos foram extraídas e agrupadas de acordo com o conteúdo. Resultados Foram selecionadas 59 publicações que descreviam recomendações referentes aos temas: controle da disseminação do vírus; formação / educação continuada dos trabalhadores responsáveis pelo cuidado ao idoso; bem como o cuidado no contexto da pandemia: residentes, trabalhadores e familiares, e planejamento e gerenciamento de ações para o enfrentamento. Conclusão As recomendações para o enfrentamento da COVID-19 demandam comportamentos para evitar a disseminação do vírus, adaptações nas dinâmicas de cuidado e de convivência nas instituições, planejamento de ações específicas e suporte familiar, institucional e do Estado para assegurar a proteção da saúde física e psicossocial dos idosos e trabalhadores.
Abstract Background Controlling the spread of the coronavirus in Long-Term Care Facilities for older adults is considered a challenge, since this group have a very unfavorable prognosis and also high lethality. Objective To conduct a rapid review of guidelines to manage COVID-19 in Long-Term Care Facilities for older adults. Method A Rapid review was carried out, searches were conducted in the electronic databases Cochrane Library, Web of Science, Scopus, Scielo and Medline/Pubmed. Publications from 2019 were included, in Portuguese, English, or Spanish. The selection of publications took place in two stages: reading the titles/abstracts and reading in full all selected publications by two independent researchers. Guidelines for managing COVID-19 in LCTFs were extracted and grouped according to content. Results 59 publications were selected describing guidelines regarding control of the spread of the virus; training/continuing education of staff responsible for caring for the elderly residents; care addressing residents, staff and family during the pandemic and planning and management of actions to manage the disease. Conclusion Guidelines for managing COVID-19 demand for behaviors to prevent the spread of the virus and adaptations in the dynamics of care and the coexistence inside facilities. They also require planning for specific actions that include family, institutional and State support so the protection of physical and psychosocial health of the elderly residents and staff is ensured.

Resumo: Objetivo As amebas testáceas são protistas de vida livre reconhecidas como bioindicadores, capazes de revelar alterações ambientais apresentando variação de riqueza, diversidade e abundância de espécies. Para avaliar a produção científica relativa ao uso de amebas testáceas como bioindicadores, foi realizada uma análise cienciométrica no período de 1960 a 2020. Métodos A seleção de artigos científicos foi feita nas plataformas Google Acadêmico, Scielo, Science Direct e Online Library. Todos os artigos selecionados foram categorizados por cronologia, periódico de publicação, país, afiliação dos autores, se o estudo era empírico ou experimental, se incluía apenas amebas testáceas e aspectos do meio ambiente. Resultados Um total de 215 artigos de noventa e dois periódicos revelou um aumento notável no número de publicações nas últimas décadas. Os dois periódicos que mais publicaram dados sobre o tema foram Microbial Ecology e Ecological Indicators. Embora o maior número de trabalhos tenha sido publicado no continente europeu, os países que mais contribuíram para o tema foram o Canadá na região temperada e o Brasil nos trópicos. Edward A.D. Mitchell publicou o maior número de estudos e em termos de instituição, a Universidade Franche-Comté. A maioria dos trabalhos que associaram amebas testáceas como bioindicadores foram empíricos, realizados tecamebas habitantes de musgos e, como tal, dissociadas de outros grupos. Conclusões Apesar do aumento verificado nas publicações com testáceas como bioindicadores, nossos resultados indicam uma concentração de estudos em regiões limitadas do globo. Sob outro aspecto, um grande número de estudos avaliou a comunidade de testáceas como um todo, mostrando uma escassez de conhecimento aprofundado sobre espécies e grupos funcionais. Com isso, nosso levantamento aponta para uma grande variedade de ecossistemas aquáticos desprovidos de estudos sobre o assunto, indicando a necessidade de fomentar a pesquisa sobre o papel das amebas testáceas como bioindicadoras, em especial nas regiões tropicais.
Abstract: Aim The free-living protists testate amoebae are recognized as being bioindicators, able to reveal environmental alteration via remodifying richness, diversity and abundance of species. To assess the scientific production on the use of testate amoebae as bioindicators, a scientometric analysis was undertaken from 1960 to 2020. Methods The sourcing of scientific articles derived from Google Academic, Scielo, Science Direct and Online Library research platforms. All selected papers were factored according to chronology, journal of publication, country, authors’ affiliation, whether the study was empirical or experimentation, if it included solely testate amoebae or other groups of organisms, and aspects of the environment. Results A total of 215 papers from ninety-two journals revealed a notable increase in publication numbers over the last decades. The two journals that most published data on this theme were Microbial Ecology and Ecological Indicators. Whilst the largest number of papers was published in the European Continent, the countries that most contributed to the subject were Canada in the temperate region and Brazil in the tropics. Edward A.D. Mitchell published the largest number of studies and regarding institutions the Franche-Comté University. The majority of papers that associated testaceans as bioindicators were empirical, conducted with Thecamoebian inhabiting moss and as such, dissociated from other groups. Conclusions Despite a suggested uptick in research of testaceans as bioindicators, our results indicate a concentration of studies to limited regions of the globe. In another aspect, a great number of studies assess testaceans’ community as-a-whole, showing a shortage of in-depth knowledge into species and functional groups. Therefore, our survey points to a wide variety of aquatic ecosystems devoid of study matter, indicating the need to foster the research on testate amoeba's role as bioindicators especially in the tropical regions.

Resumo Neste presente estudo, objetivou-se avaliar as alterações induzidas pela infecção natural de Schistosoma mansoni nas concentrações de glicose do sangue periférico de roedores Holochilus sciureus, reservatório silvestre do parasito. A dosagem da concentração de glicose foi realizada no plasma de amostras de sangue por meio de teste enzimático colorimétrico. Parâmetros biológicos e carga parasitária de S. mansoni também foram verificados em cada roedor e correlacionados com as concentrações de glicose. Um total de 76 H. sciureus foram capturados, dentre os quais 20 (26%) estavam infectados por S. mansoni (n=13 machos e n=7 fêmeas). Apesar da carga parasitária ter sido semelhante entre os sexos, as concentrações de glicose no sangue foram menores em machos naturalmente infectados e quase inalteradas em fêmeas. Além disso, os dados histopatológicos mostraram uma maior reação inflamatória granulomatosa hepática em roedores machos, quando comparados com as fêmeas. Adicionalmente, também foi confirmado que o peso e o comprimento total dos animas analisados não interferiram nos valores de glicose. Portanto, a infecção natural por S. mansoni em H. sciureus pode induzir menos impacto na homeostase glicêmica em fêmeas, o que pode ajudar a melhor entender o papel desses roedores como reservatórios de S. mansoni.
Abstract The present study aimed to evaluate the changes in peripheral blood glucose concentrations induced by Schistosoma mansoni infection in Holochilus sciureus rodents, a wild reservoir of the parasite. Glucose concentration was measured in the plasma of blood samples using a colorimetric enzymatic test. Biological parameters and S. mansoni burden in each rodent were also verified and correlated with glucose concentrations. A total of 76 H. sciureus were captured, out of which 20 (26%) were infected with S. mansoni (n=13 males and n=7 females). Although the parasite burden was comparable between the sexes, blood glucose concentration was lower in infected males and almost unchanged in females. Furthermore, histopathological data revealed that male rodents had a greater hepatic granulomatous inflammatory reaction than females. In addition, we also confirmed that the weight and total length of the analyzed animals had no effect on glucose levels. Therefore, natural infection with S. mansoni in H. sciureus may have a lower impact on glycemic homeostasis in females, which will help us understand the role of these rodents as reservoirs of S. mansoni.

RESUMO O presente estudo apresenta a adaptação e validação transcultural do Questionário Saúde Docente (QSD) para o contexto brasileiro nas variáveis de bem-estar (satisfação e autoeficácia) e mal-estar (disfunções musculoesqueléticas, disfunções cognitivas, esgotamento e disfunções da voz). Foram realizadas distintas etapas para assegurar a qualidade da tradução e da adaptação do QSD no que se refere à equivalência semântica, idiomática, experiencial e conceitual entre os itens originais e traduzidos. A amostra final do estudo piloto foi constituída por 697 professores, sendo 168 do sexo masculino e 529 do feminino, com idades entre 18 e 52 anos, atuantes em 03 centros de Educação Infantil, 09 escolas do nível Ensino Fundamental de 1º ao 5º ano e 04 dos níveis Ensino Fundamental de 6º ao 9º ano e Ensino Médio. Para a análise da estabilidade temporal dos escores foi empregado o teste de Kappa e para a consistência interna das dimensões e da avaliação global do instrumento o teste de alfa de Cronbach, os quais apresentaram correlações satisfatórias. Como instrumento de medida, o questionário é considerado confiável para avaliar a saúde docente, além disso contém diretrizes para futuros estudos das variáveis associadas ao bem-estar e mal-estar de professores brasileiros.
ABSTRACT The present study aimed at showing the cross-cultural adaptation and validation of the Spanish health questionnaire so-called Cuestionario de Salud Docente (CSD) for the Brazilian context with regard to the following variables: welfare (satisfaction and self-efficacy), and discomfort (musculoskeletal disorders, cognitive disorders, exhaustion and vocal cord disorders). Different steps were taken in order to ensure the quality of the translation and adaptation of the CSD concerning semantic, idiomatic, experiential and conceptual equivalence between the original items and the translated ones. The final sample of the pilot study consisted of 697 teachers, 168 men and 529 women, aged between 18 and 52 years who worked in 3 Early Childhood Education Centers, 9 Elementary Schools from the 1st to 5th grade, 04 Elementary Schools from the 6th to 9th grade, and 4 High Schools. Cohen’s Kappa coefficient was used for assessing the temporal stability of the scores, and Cronbach's alpha test for the internal consistency of the dimensions and overall evaluation of the instrument, which showed a significant correlation. As a measuring instrument, the questionnaire is considered reliable to evaluate teacher’s health, in addition to containing guidelines for further studies on welfare and discomfort variables of Brazilian teachers.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

ABSTRACT Purpose: To evaluate the low-level laser therapy (LLLT) on the membrane induced by the Masquelet technique in rabbits. Methods: Twelve Norfolk rabbits at approximately 3 months of age were used. A 1-cm segmental defect was induced in both radii, which were filled with polymethylmethacrylate cylinder. LLLT was used postoperatively in the bone defect of one of the forelimbs every 48 hours for 15 days. Six rabbits were euthanatized on third and sixth postoperative weeks. Results: In both forelimbs, radiographs showed new bone growth from radius cut ends on the third postoperative week and more advanced stage on the sixth postoperative week. Ultrasound showed induced membrane one week after the surgery. Histologically, there were no significant differences in the semi-quantitative score of inflammation intensity, total number of blood vessels, bone metaplasia, and collagen. The average thicknesses were 2,050.17 and 1,451.96 μm for control membranes and 2,724.26 and 2,081.03 μm for irradiated membranes, respectively, on third and sixth postoperative weeks. Vascular endothelial growth factor A (VEGF-A) and platelet derived growth factor (PDGF) expression were present in the induced membranes of control and irradiated forelimbs, but there was no significant difference. Conclusions: Based on assessment methods, it was not possible to demonstrate the effect of LLLT on the induced membrane.

OBJECTIVES: To compare demographic/clinical/laboratory/treatments and outcomes among children and adolescents with laboratory-confirmed coronavirus disease 2019 (COVID-19). METHODS: This was a cross-sectional study that included patients diagnosed with pediatric COVID-19 (aged <18 years) between April 11, 2020 and April 22, 2021. During this period, 102/5,951 (1.7%) of all admissions occurred in neonates, children, and adolescents. Furthermore, 3,962 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection samples were processed in patients aged <18 years, and laboratory-confirmed COVID-19 occurred in 155 (4%) inpatients and outpatients. Six/155 pediatric patients were excluded from the study. Therefore, the final group included 149 children and adolescents (n=97 inpatients and 52 outpatients) with positive SARS-CoV-2 results. RESULTS: The frequencies of sore throat, anosmia, dysgeusia, headache, myalgia, nausea, lymphopenia, pre-existing chronic conditions, immunosuppressive conditions, and autoimmune diseases were significantly reduced in children and adolescents (p<0.05). Likewise, the frequencies of enoxaparin use (p=0.037), current immunosuppressant use (p=0.008), vasoactive agents (p=0.045), arterial hypotension (p<0.001), and shock (p=0.024) were significantly lower in children than in adolescents. Logistic regression analysis showed that adolescents with laboratory-confirmed COVID-19 had increased odds ratios (ORs) for sore throat (OR 13.054; 95% confidence interval [CI] 2.750-61.977; p=0.001), nausea (OR 8.875; 95% CI 1.660-47.446; p=0.011), and lymphopenia (OR 3.575; 95% CI 1.355-9.430; p=0.010), but also had less hospitalizations (OR 0.355; 95% CI 0.138-0.916; p=0.032). The additional logistic regression analysis on patients with preexisting chronic conditions (n=108) showed that death as an outcome was significantly associated with pediatric severe acute respiratory syndrome (SARS) (OR 22.300; 95% CI 2.341-212.421; p=0.007) and multisystem inflammatory syndrome in children (MIS-C) (OR 11.261; 95% CI 1.189-106. 581; p=0.035). CONCLUSIONS: Half of the laboratory-confirmed COVID-19 cases occurred in adolescents. Individuals belonging to this age group had an acute systemic involvement of SARS-CoV-2 infection. Pediatric SARS and MIS-C were the most important factors associated with the mortality rate in pediatric chronic conditions with COVID-19.

ABSTRACT Efficient snail production is essential for the proper maintenance of the Schistosoma mansoni life cycle in the laboratory. In order to improve the breeding of Biomphalaria glabrata under laboratory conditions, this study aimed to demonstrate the effectiveness of a handmade shell-based substrate on the physiological performance of B. glabrata. The shells used to make the substrate were cleaned, sterilized and macerated until a powder was obtained (yield of 92.3%). B. glabrata specimens were randomly assigned to three treatment groups: negative control group (NCG) exposed to a clay-only substrate; a positive control group (PCG) containing clay, oyster flour and calcium carbonate; and the test group (TG) with the shell-based substrate and clay. B. glabrata bred in the test group showed improved growth, sexual maturity, fertility, mortality rate, and shell morphology when compared to the NCG, and similar to the PCG. Therefore, the shell-based substrate proved to be efficient and has a low cost for the breeding of B. glabrata.

Abstract INTRODUCTION: Curtailing the development of the aquatic immature stages of Aedes aegypti is one of the main measures to limit their spread and the diseases transmitted by them. The use of plant extracts is a promising approach in the development of natural insecticides. Thus, this research aimed to characterize the inhibitory effect of the hydroalcoholic extract of Caryocar brasiliense leaves on the emergence of adult A. aegypti and the main substances that constitute this extract. METHODS: C. brasiliense leaf extract was prepared by ethanol (70%) extraction. Bioassays using L3 larvae were performed at concentrations of 200, 300, 400, and 500 ppm. We identified the major secondary metabolites present in this extract, and performed toxicity tests on an off-target organism, Danio rerio. RESULTS: We observed a significant delay in the development of A. aegypti larvae mainly at a concentration of 500 ppm, and estimated an emergence inhibition for 50% of the population of 150 ppm. Moreover, the C. brasiliense leaf extracts exhibited low toxicity in D. rerio. The main compounds found in the extract were quercetin, violaxanthin, myricetin3-O-hexoside, methyl-elagic-3-arabinose acid, and isoquercitrin. CONCLUSIONS: Herein, we demonstrate the inhibition of mosquito development by the hydroalcoholic extract of C. brasiliense and suggest substances that may act as active principles.

Resumo O objetivo foi analisar a influência dos contextos sociais de treinadoras de Ginástica Rítmica, considerando as aprendizagens adquiridas ao longo da vida. Participaram do estudo cinco treinadoras. Os dados foram coletados a partir de uma entrevista semiestruturada. Este estudo baseia-se no pressuposto de que os contextos sociais são espaços e tempos de aprendizagem que se relacionam com as escolhas e experiências profissionais das treinadoras. Considera-se que a influência da família é significativa para a inserção e a manutenção no esporte, além do destaque no ambiente escolar e esportivo como contextos de relevância para a escolha profissional. Conclui-se que os contextos de aprendizagem tornam-se oportunidades de aquisição de conhecimentos, valores e crenças que influenciam a trajetória pessoal e profissional das treinadoras.
Resumen El objetivo fue analizar la influencia de los contextos sociales de los entrenadores de gimnasia rítmica, considerando el aprendizaje adquirido a lo largo de la vida. Cinco entrenadores participaron en el estudio. Los datos fueron recolectados de una entrevista semiestructurada. Este estudio se basa en el supuesto de que los contextos sociales son espacios y tiempos de aprendizaje relacionados con las elecciones y experiencias profesionales de los entrenadores. Se considera que la influencia de la familia es significativa para la inserción y el mantenimiento en el deporte, además del hecho destacado en el entorno escolar y deportivo como contextos de relevancia para la elección profesional. Se concluye que los diferentes contextos de aprendizaje se convierten en oportunidades para adquirir conocimientos, valores y creencias que influyen en la trayectoria personal y profesional de los entrenadores.

RESUMO Os países de baixa e media renda enfrentam orçamentos apertados na saúde, não somente devido aos novos recursos terapêuticos, mas relacionado ao custo oneroso do tratamento do acidente vascular cerebral. No entanto, poucos dados prospectivos sobre os custos do AVC, incluindo reperfusão cerebral de países de baixa e média renda estão disponíveis. Objetivo Mensurar os custos do atendimento ao AVC em um hospital público. Métodos Avaliamos prospectivamente todos os custos médicos e não médicos de pacientes internados com diagnóstico de acidente vascular cerebral ou AIT durante 1 ano, analisamos os custos por tipo de AVC e tratamento, tempo de permanência e comparamos os custos hospitalares com o reembolso governamental. Resultados Foram avaliados 274 pacientes. O custo total em um ano foi de US$ 1.307,114; o governo reembolsou o hospital no valor de US$ 1.095.118. Encontramos uma correlação linear significativa entre LOS e custos (r = 0,71). A mediana do custo do AVCI em 134 pacientes que não sofreram reperfusão cerebral (National Institutes of Health Stroke Scale [NIHSS] mediana = 3) foi de US$ 2.803; para pacientes submetidos a alteplase intravenosa (IV) (NIHSS 10), a mediana foi de US$ 5.099 e para os pacientes submetidos a trombectomia intra-arterial (IA) (NIHSS > 10), o custo mediano foi de US$ 10.997. A mediana do custo de uma hemorragia intracerebral primária, hemorragia subaracnóidea e AIT foram de US$ 2.436, US$ 8.031 e US$ 2.677, respectivamente. Conclusões Os tratamentos de reperfusão foram duas a quatro vezes mais caros do que o tratamento conservador. Estudo de custo-efetividade para o tratamento do AVC são necessários.
ABSTRACT Low- and middle-income countries face tight health care budgets, not only new resources, but also costly therapeutic resources for treatment of ischemic stroke (IS). However, few prospective data about stroke costs including cerebral reperfusion from low- and middle-income countries are available. Objective To measure the costs of stroke care in a public hospital in Joinville, Brazil. Methods We prospectively assessed all medical and nonmedical costs of inpatients admitted with a diagnosis of any stroke or transient ischemic attack over one year, analyzed costs per type of stroke and treatment, length of stay (LOS) and compared hospital costs with government reimbursement. Results We evaluated 274 patients. The total cost for the year was US$1,307,114; the government reimbursed the hospital US$1,095,118. We found a significant linear correlation between LOS and costs (r = 0.71). The median cost of 134 IS inpatients who did not undergo cerebral reperfusion (National Institutes of Health Stroke Scale [NIHSS] median = 3 ) was US$2,803; for IS patients who underwent intravenous (IV) alteplase (NIHSS 10), the median was US$5,099, and for IS patients who underwent IV plus an intra-arterial (IA) thrombectomy (NIHSS > 10), the median cost was US$10,997. The median costs of a primary intracerebral hemorrhage, subarachnoid hemorrhage, and transient ischemic attack were US$2,436, US$8,031 and US$2,677, respectively. Conclusions Reperfusion treatments were two-to-four times more expensive than conservative treatment. A cost-effectiveness study of the IS treatment option is necessary.
