RESUMO Objetivo: Este estudo investigou a presença de resposta inflamatória na polpa dentária de ratos apresentando hipersensibilidade dentinária induzida por episódios erosivos. Métodos: Dezesseis ratos Wistar foram alimentados com ração peletizada isenta de sacarose por 12 horas; enquanto a dieta estava ausente no restante do período diário, os animais receberam água mineral ou um refrigerante dietético à base de limão, de acordo com o grupo ao qual pertenciam. Oito animais consumiram o refrigerante para a indução do quadro de hipersensibilidade, enquanto os demais receberam água mineral (controle). Após seis semanas, realizou-se a eutanásia dos animais, os quais tiveram suas mandíbulas removidas e seccionadas medialmente no plano sagital, a fim de serem obtidas hemimandíbulas direita e esquerda. Lâminas coradas com hematoxilina-eosina foram analisadas sob microscopia óptica. Resultados: A avaliação histológica dos grupos controle e experimental revelou ausência de processo inflamatório no tecido pulpar e também não foi observada a presença de células inflamatórias como linfócitos, plasmócitos, eosinófilos ou macrófagos. Em adição, não houve edema ou vasos sanguíneos dilatados e congestos. O teste de Mann-Whitney demonstrou inexistência de diferença significativa (p = 1,000) entre os grupos experimental e controle. Conclusão: No modelo animal adotado, a hipersensibilidade dentinária não desencadeou resposta inflamatória na polpa dentária.

ABSTRACT Objective: This study investigated the presence of inflammatory response in the dental pulp of rats showing hypersensitive dentin, induced by erosive episodes. Methods: Sixteen Wistar rats were fed with commercial sucrose-free pellet diet for 12 hours; whereas the food was removed during the remainder of the day, and the animals received mineral water or a lemon-based sucrose-free soft drink, according to the group to which they belonged. Eight animals consumed the soft drink to induce hypersensitivity, while the other 8 animals received mineral water (control). After six weeks, the animals were euthanized, the mandible was removed and subjected to a median incision in the sagittal plane, to obtain right and left hemimandibles. The slides stained with hematoxylin-eosin were analyzed using light microscopy. Results: Histological evaluation of the control and experimental groups revealed no inflammatory process in the pulp tissue, and the presence of inflammatory cells, such as lymphocytes, plasma cells, eosinophils and macrophages, was not observed. In addition, there was no edema or dilated and congested blood vessels. The Mann-Whitney test showed no significant difference (p = 1.000) between the experimental and the control groups. Conclusion: In the animal model used, dentin hypersensitivity does not trigger dental pulp inflammatory response.

A sialolitíase, ou cálculo salivar é uma doença de glândulas salivares, caracterizada pela formação de estrutura mineralizada no interior dos ductos salivares excretores ou no próprio parênquima glandular. A glândula submandibular é acometida em aproximadamente 80% dos casos seguida das glândulas parótida e sublingual. Embora raro o cálculo salivar também pode ocorrer em glândulas salivares menores. O lábio superior e a mucosa oral são os sítios de envolvimento de maior frequência, embora possam desenvolver-se em qualquer região da cavidade oral que contenha glândulas salivares menores. O tratamento baseia-se na remoção cirurgia, com prognóstico favorável. Este artigo apresenta três casos raros de sialolitíase de glândula salivar menor localizados em lábio superior e inferior, abordando as características clínicas e sua relação com os achados microscópicos, diagnóstico e formas de tratamento. A etiologia da lesão e fatores que devem ser considerados para o diagnóstico, tratamento adequado e um melhor prognóstico são discutidos neste estudo.

Sialolithiasis, or salivary calculus, is a disease of the salivary glands, characterized by the formation of mineralized structures within the excretory salivary ducts or the glandular parenchyma. Approximately 80% occur in the submandibular gland, followed by the parotid and sublingual glands. Although rare, sialolithiasis can also occur in the minor salivary glands. Although the oral cavity and upper lip are the most common sites, they can develop in any region of the oral cavity that contains minor salivary glands. Treatment is based on surgical excision of the lesion, with a favorable prognosis. This study presents three cases of sialolithiasis of minor salivary glands of the upper and lower lip, addresses their clinical characteristics and correlation with microscopic findings, diagnosis and treatment plans. Etiology and factors that should be considered during diagnosis, appropriate treatment and improved prognosis are discussed in this study.

Cúspide em garra é uma anomalia dental do desenvolvimento relativamente rara caracterizada por uma estrutura acessória em forma de cúspide projetando-se da área do cíngulo ou junção amelocementária. Ela é frequentemente encontrada na face palatina dos dentes maxilares anteriores na dentição permanente. A etiologia é controversa, mas é sugerido que essa condição tem causa multifatorial, incluindo ambos os fatores, genéticos e ambientais. Radiograficamente cúspides em garra são estruturas radiopacas, nas quais o esmalte, dentina e ocasionalmente a polpa podem ser vistos. A alteração é geralmente assintomática, mas pode causar problemas clínicos eventualmente. O diagnóstico precoce é importante para prevenir interferência oclusal, irritação na língua, necrose pulpar, cáries e alteração periodontal. O manejo clínico das cúspides em garra pode ser tanto conservativo quanto radical, dependendo do tamanho e forma do dente afetado. Nós relatamos um raro caso de cúspides em garra envolvendo todos os 6 dentes maxilares anteriores em um paciente de 20 anos de idade.

A talon cusp is a relatively rare dental developmental anomaly characterized by the presence of accessory cusp-like structure projecting from the cingulum area or cementoenamel junction. It is frequently found on the palatal surface of the maxillary anterior teeth in the permanent dentition. The etiology is controversial, but it is suggested that this condition has a multifactorial cause, including both genetic and environmental factors. Radiographically talon cusps are radiopaque structures, in which the enamel, dentin and occasionally the pulp can be seen. The alteration is generally asymptomatic, but it may cause clinical problems eventually. Early diagnosis is important to prevent occlusal interference, irritation of the tongue, pulpal necrosis, caries and periodontal alterations. Clinical management of talon cusps may be either conservative or radical, depending on the size and shape of the affected tooth. We report a rare case of talon cusps involving all six maxillary anterior teeth in a 20-year-old patient.

Fibrosarcoma is a malignant mesenchymal neoplasm of fibroblasts that rarely affects the oral cavity and can cause local recurrences or metastasis. In this article we describe a case of primary fibrosarcoma in the mandible in a 16-year-old girl. Microscopically the tumor showed an intense proliferation of spindle-shaped cells, varying little in size and shape and arranged in parallel bands, partly crossing each other, the mitotic activity was increased and there was nuclear pleomorphism. Immunohistochemically the cells only showed immunoreactivity for vimentin and negativity for S-100 protein, CD 68, cytokeratin cocktail, HMB-45, CD34, pan actin HHF 35, desmin, smooth muscle actin and epithelial membrane antigen (EMA). Based on clinical, radiological, histological and immunohistochemical findings the final diagnosis was high-grade intra-osseous fibrosarcoma. The treatment choice was radical surgery with mandibular reconstruction. After one year and nine months of the treatment the patient displayed multiple metastases. Radiation therapy and chemotherapy were used as adjuvant treatment. Unfortunately, the girl died two years after initial diagnosis.
