Abstract Tongue cancer is a pathology with significant morbidity and impact on the quality of life of the patients. It affects chewing, swallowing, communication, and taste. Therefore, a multidisciplinary team is required to establish adequate rehabilitation. Extensive resections are needed to ensure negative margins. If the surgeon considers that there is insufficient remaining volume to obliterate the oral cavity, flap surgery will be carried out to maintain the swallowing function. We present a case report of a 76-year-old male patient with a history of smoking cessation of 23 years. He presents a painful ulcerated lesion on the right edge of the mobile tongue. The biopsy reports an epidermoid carcinoma, staged T2N0M0 which led to perform a right hemiglosectomy, cervical dissection, and reconstruction with free radial forearm flap.

Resumen El cáncer de lengua es una patología con una importante morbilidad e impacto en la calidad de vida del paciente, afectando la masticación, deglución, comunicación y gusto, por lo que requiere de un equipo multidisciplinario para establecer la rehabilitación adecuada. Se necesita realizar resecciones amplias para asegurar márgenes negativos, y si el cirujano considera que existe un volumen remanente insuficiente para obliterar la cavidad oral, la realización de un colgajo será necesario para mantener la función deglutoria. Se presenta el caso de un paciente de 76 años, masculino, con antecedentes de tabaquismo suspendido hace 23 años, que presenta una lesión ulcerada dolorosa en el borde derecho de la lengua móvil, la biopsia informa un carcinoma epidermoide, etapificado T2N0M0, por lo que se decide realizar hemiglosectomía derecha más disección cervical y reconstrucción con colgajo libre radial.

Abstract Otalgia is a common reason for consultation in otolaryngology and can be classified into two types: primary or secondary otalgia. Primary otalgia is a consequence of an otological disease while secondary otalgia arises from pathological processes that originate in other structures different than the ears, but share common neuronal pathways. These pathways involve cranial nerves V, VII, IX and X and cervical nerves C2 and C3. The most common causes of secondary otalgia are musculoskeletal pathology such as temporomandibular joint dysfunction and myofascial syndrome, dental pathology and oropharyngeal infections. To find the etiology of secondary otalgia, in most cases a medical history and physical examination will be sufficient, however there is a group of patients with risk factors that may require additional studies. The objective of this review is to give the reader tools to understand the different processes involved in secondary otalgia, know the most relevant related pathologies and thus give the clinician a guide to deal with this problem.

Resumen La otalgia es un motivo de consulta frecuente en otorrinolaringología que se puede clasificar en dos tipos: primaria y secundaria. La otalgia primaria es una consecuencia de una enfermedad otológica mientras que la secundaria o referida, surge de procesos patológicos que se originan en estructuras distintas del oído, pero que comparten vías neuronales comunes. Estas vías la componen los nervios craneales V, VII, IX y X y los nervios cervicales C2 y C3. Las causas más comunes de otalgia secundaria son la patología musculoesquelética como la disfunción de la articulación temporomandibular y el síndrome miofascial, la patología dental y las infecciones orofaríngeas. Para encontrar la etiología de la otalgia secundaria, en la mayoría de los casos, una historia clínica y examen físico acucioso resultarán suficientes, sin embargo, existe un grupo de pacientes con factores de riesgo que pudiesen requerir estudios adicionales. El objetivo de esta revisión es otorgar herramientas al lector para comprender los distintos procesos involucrados en la otalgia secundaria, conocer las patologías más relevantes relacionadas y de esta forma entregar al clínico una pauta para enfrentar este motivo de consulta.

Cet article présente une étude de cas unique et n’a donc pas l’intention de fournir une explication généralisée. Cependant, il représente un cas exemplaire qui pourrait évoquer chez le lecteur d’autres cas cliniques similaires et il permet, à la fois, d’illustrer le développement de théories existantes. Cette étude décrit le rapport entre les problèmes de séparation et l’expérience de ce rapport dans la vie quotidienne. Il contribue d’ailleurs à la compréhension de ce rapport à partir de la théorie des phénomènes transitionnels de Winnicott.

This research is based on a unique case report, therefore it does not intend to provide a generalizable explanation, but it contributes with an exemplary case that could stimulate readers to remember similar cases, also helping to illustrate the theoretical development. This research shows the relation between separation difficulties and the transitional experience in the maternal-infant relationship, and contributes to the comprehension of this relationship based on Winnicott’s transitional phenomena theory.

Dieser Artikel analysiert eine Einzelfallstudie und dessen Schlussfolgerungen können daher nicht verallgemeinert werden. Ziel ist es, eine Parallele zu ähnlichen Fällen zu ziehen und seine theoretische Entwicklung zu beschreiben. Der Artikel beschreibt das Verhältnis zwischen Trennungsschwierigkeiten und die Erfahrung dieser Beziehung im Alltag. Er trägt somit zum Verständnis dieser Beziehung bei, basierend auf Winnicotts Theorie der Übergangsphänomene.

Esta pesquisa é de um estudo de caso único. Portanto, não pretende dar uma explicação generalizada. Contribui, porém, como um caso exemplar que pode estimular a memória dos leitores que tenham casos similares, e também, ao mesmo tempo, ajuda a ilustrar o desenvolvimento teórico. Esta pesquisa demonstra a relação entre os problemas da separação e a experiência dessa relação no dia a dia e contribui para a compreensão dessa relação a partir da teoria dos fenômenos transicionais de Winnicott.

Poultry is a main reservoir and source of human infection in campylobacteriosis. Three hundred and forty one stool samples (291 human, 50 avian) were analyzed. In the human group, 220 samples were collected from children with acute diarrheal disease (183 inpatients, 37 outpatients) and 71 from healthy children. Erythromycin and ciprofloxacin agar dilution MIC tests, Penner serotyping and RAPD-PCR genotyping were performed on 23 strains isolated. C. jejuni was reported only in patients with acute diarrhea (5.4% inpatients, 2.2% outpatients). Campylobacter prevalence in poultry was 34%. Cross-resistance to nalidixic acid and ciprofloxacin was found in 33.3% of human samples and 11.8% of animal samples. Human samples could not be typed using the Penner method. F serotype was the most expressed in poultry. We obtained a total of 14 genotypes (4 / 5 human and 10/15 avian). In conclusion, the predominant species in poultry and humans was C. jejuni, a significant amount of quinolone-resistant human and avian samples were obtained, and avian genotypes and serotypes were not found in human samples. The latter would mean that another source of infection could exist; therefore other reservoirs must be studied.

Las aves de consumo constituyen uno de los principales reservorios y fuente de infección humana de la campilo-bacteriosis. Se analizaron 341 muestras de deposiciones, 291 humanas y 50 aviares. De las muestras, 220 de niños con síndrome diarreico agudo-SDA (183 hospitalizados y 37 consultantes ambulatorios) y 71 niños sanos. A las 23 cepas obtenidas se les realizó CIM por dilución en agar a eritromicina y ciprofloxacina, serotipificación de Penner y genotipiicación por RAPD-PCR. Se encontró Campylobacterjejuni sólo en pacientes con SDA, de ellos 5,4% ambulatorios y 2,2% hospitalizados. En aves, la prevalencia de Campylobacter spp., fue de 34%. Hubo resistencia cruzada a ácido nalidixico y ciprofloxacina en 33,3% cepas de origen humano y 11,8% animal. Las cepas humanas fueron no tipiicables por el método de Penner. Predominó entre las aves el serotipo F. Se obtuvo un total de 14 genotipos (4/5 humanos y 10/15 aviares). En conclusión, la especie predominante en aves de corral y en humanos fue C. jejuni, existiendo una alta prevalencia de cepas de origen humano y aviar resistentes a quinolonas. Los genotipos y serotipos aviares no fueron encontrados en cepas de origen humano, lo que indica que podría existir otra fuente de infección, por lo que se requiere estudiar otros reservorios.
