Abstract This research analyzes the relationship of curricular coherence between the tasks of an eighth-grade textbook on the subject of linear equations of the mathematics subject and the detailed guidelines in the study programs in Costa Rica. Since it seeks to address a particular situation in Costa Rican education, the analysis consists of a case study with a qualitative approach on a specific topic in order to achieve a better degree of understanding about the design of tasks that guide the learning of mathematics at the secondary level. As a result, the relationship that the tasks in the subject of equations show with the basis and foundations of the study program is detailed, including the active role of the student in their learning process, as well as elements such as the use of history and technology, skills and others. The results lead us to the recommendation to strengthen some elements to achieve greater curricular coherence between classroom activities and the mathematics study program in eighth grade, such as the inclusion of playful activities for learning equations. eighthgrade grade Rica education level result process technology others

Resumen Esta investigación analiza la coherencia curricular existente entre las tareas de un libro de texto de octavo año en el tema de ecuaciones lineales de la asignatura de matemáticas y el currículo normativo en Costa Rica. El análisis conforma un estudio de caso de enfoque cualitativo, dado que aborda una situación particular de la educación costarricense, de manera profunda en una temática específica, y busca una mayor comprensión sobre el diseño de tareas que orientan el aprendizaje de las matemáticas a nivel de secundaria. Como resultados, se detalla la relación que evidencian las tareas en el tema de ecuaciones con los ejes y fundamentos del programa de estudio, entre los cuales se encuentra el papel activo del estudiante en su proceso de aprendizaje, además de elementos como el uso de la historia y la tecnología, habilidades y otros. Los resultados nos conducen a la recomendación de reforzar elementos curriculares señalados en el plan de estudios costarricense, a la hora de planificar y diseñar tareas escolares, para lograr una mayor coherencia curricular. Abstract Background: Cardiovascular diseases are the leading cause of mortality worldwide and Mexico is no exception. The epidemiological data obtained in 1990 showed that cardiovascular diseases represented 19.8% of all causes of death in our country. This figure increased significantly to 25.5% for 2015. Some national surveys suggest that more than 60% of the adult population has at least one risk factor for cardiovascular disease (obesity or overweight, hypertension, smoking, diabetes, dyslipidemias). On the other hand, data from the Pan American Health Organization have linked the process of atherosclerosis as the first cause of premature death, significantly reducing life expectancy, which has enormous social repercussions. Objective: This document constitutes the Clinical Practice Guide (CPG) prepared at the initiative of the Mexican Society of Cardiology in collaboration with the Mexican Society of Nutrition and Endocrinology, AC, National Association of Cardiologists of Mexico, AC, Mexican Association for the Prevention of Atherosclerosis and its Complications, AC, National Normative Committee of General Medicine, AC, National College of Geriatric Medicine, AC, College of Internal Medicine of Mexico, AC, Mexican Society of Angiology and Vascular and Endovenous Surgery, AC, Mexican Institute of Research Nephrological, AC and the Mexican Academy of Neurology, A.C.; with the methodological support of the Ibero-American Agency for the Development and Evaluation of Health Technologies, in order to establish recommendations based on the best available evidence and agreed upon by an interdisciplinary group of experts. The objective of this document is to provide evidence-based recommendations to help decision makers in the diagnosis and treatment of dyslipidemias in our country. Material and methods: This document complies with international quality standards, such as those described by the Institute of Medicine of the USA, the Institute of Clinical Excellence of Great Britain, the Scottish Intercollegiate Guideline Network and the Guidelines International Network. A multidisciplinary group of clinical experts and methodologists with experience in systematic reviews of the literature and the development of clinical practice guidelines was formed. A scope document was agreed upon, relevant clinical questions were established, the best available evidence critically evaluated in systematic literature reviews was exhaustively identified, and clinical recommendations were developed. The modified Delphi Panel methodology was used to achieve an adequate level of consensus in each of the recommendations contained in this CPG. Results: 23 clinical questions were agreed upon which gave rise to their respective clinical recommendations. Conclusions: We consider that this document contributes to better clinical decision-making and becomes a point of reference for clinicians and patients in the management of dyslipidemias and this contributes to reducing the morbidity and mortality derived from atherosclerotic cardiovascular events in our country.

Resumen Este trabajo tiene un doble objetivo; por un lado, analizar las definiciones que brindan estudiantes universitarios sobre el concepto de límite en un punto de una función y, por otro, tanto diseñar como validar un sistema de categorías para llevarlo a cabo. Se estudiaron las definiciones de límite proporcionadas por 38 estudiantes universitarios de Biología e Ingeniería en Química Industrial, matriculados en un curso de Cálculo Diferencial e Integral, durante el primer semestre del 2018, en la Universidad Nacional, Costa Rica. Para ello, se utilizaron categorías a priori y fue necesario complementarlas con otras de naturaleza inductiva, surgidas durante el análisis de contenido de las respuestas. Estas categorías fueron ratificadas mediante un análisis de fiabilidad. Los estudiantes evidenciaron la categoría límite como objeto en la mitad de sus respuestas, aproximadamente, y la categoría límite como proceso en más de tres cuartas partes. Además, constataron las categorías términos de posición relativa y descoordinación general de los procesos, en un estimado de la mitad de sus respuestas. El sistema de categorías creado permitió examinar unidades de información de una manera ordenada, simple y replicable. Finalmente, los estudiantes muestran una concepción dual del límite ya sea como un objeto, noción fija y estática, o como un proceso, noción procesual y dinámica. Asimismo, consideramos que el análisis de contenido descrito en la metodología puede permitir a futuros investigadores crear un sistema de categorías similar o usar este para indagaciones en otros contextos.

Resumo Este trabalho tem um objetivo duplo: por um lado, analisar as definições que estudantes universitários oferecem sobre o conceito de limite em um ponto de uma função e, por outro lado, desenhar e validar um sistema de categorias para realizá-lo. Foram estudadas as definições de limite proporcionadas por 38 estudantes universitários de Biologia e Engenharia em Química Industrial, matriculados em uma cadeira de Cálculo Diferencial e Integral, durante o primeiro semestre de 2018, na Universidade Nacional, Costa Rica. Para isso, foram utilizadas categorias a priori e foi preciso complementá-las com outras de natureza indutiva, surgidas durante a análise de conteúdo das respostas. Estas categorias foram ratificadas por médio de uma análise de fiabilidade. Os estudantes demonstraram a categoria limite como objeto na metade de suas respostas, aproximadamente, e a categoria limite como processo em mais de três quartos. Também, constataram as categorias termos de posição relativa e descoordenação geral dos processos, em uma média da metade de suas respostas. O sistema de categorias criado permitiu examinar unidades de informação de uma maneira estruturada, simples e replicável. Finalmente, os estudantes mostram uma concepção dual do limite, seja como um objeto, noção fixa e estática, ou como um processo, noção processual e dinâmica. Igualmente, consideramos que a análise de conteúdo descrito na metodologia pode permitir que futuros pesquisadores criem um sistema de categorias semelhante ou usem este para indagações em outros contextos.

Abstract This paper serves two objectives: analyze the definitions provided by college students for the concept of limit at a point in a function as well as design and validate a category system to carry it out. The definitions of limit were provided by 38 university students in the Biology and Engineering in Industrial Chemistry program, enrolled in a Differential and Integral Calculus course during the first semester of 2018, at Universidad Nacional, Costa Rica. A priori categories were used and complemented with other categories of an inductive nature, which emerged during the analysis of the responses. These categories were validated through a reliability analysis. Approximately half of the students’ answers showed the limit category as an object and over three-quarters showed the limit category as a process. In addition, approximately half of their answers showed the categories terms of relative position and general lack of coordination of the processes. The category system created allowed for the analysis of information units in an organized, simple, and replicable way. Finally, students showed a dual conception of limit either as an object, fixed and static notion, or as a process, processual and dynamic notion. Furthermore, the content analysis described in the methodology may allow future researchers to create a similar category system or use this one for research in other contexts.

Abstract: Class III skeletal malformations generally present a concave profile, maxillary retrusion, and protrusion. Maxillary protrusion in combination with rapid expansion of the maxilla is a very well used treatment in patients with maxillary deficiency. The objective of this work is to demonstrate through clinical cases, the treatment of this skeletal anomaly with the use of the Petit Mask seen in 3 patients between 3 to 8 years old who presented concave profile with maxillary deficiency and anterior crossbite; A modified Hass type circuit breaker with hooks is placed at the level of the canines and the circuit breaker is immobilized, and the Petit Mask is installed; finally, the patients presented correction of the skeletal problem.

Resumen: Las malformaciones esqueléticas tipo Clase III presentan generalmente un perfil cóncavo, retrusión maxilar, y protrusión. La protrusión maxilar en combinación con la expansión rápida de la maxila es un tratamiento muy bien utilizado en pacientes con deficiencia maxilar. El objetivo de este trabajo es demostrar por medio de casos clínicos, el tratamiento de esta anomalía esquelética con el uso de la Máscara de Petit visto en 3 pacientes entre 3 a 8 años que presentaron perfil cóncavo con deficiencia maxilar y mordida cruzada anterior; se coloca un disyuntor tipo Hass modificado con ganchos a nivel de los caninos y el disyuntor se inmoviliza, y se instala la Máscara de Petit; finalmente, los pacientes presentaron la corrección del problema esquelético.

Abstract: Bruxism is characterized by tooth clenching, which affects dental structures and causes pain. Treatment so that it does not negatively interfere with the growth of children must be conservative and reversible. This work aims to report a clinical case of a girl with grinding teeth, headache, tooth wear, TMJ noises and pain in the chewing muscles. As treatments, dental procedures were performed and an interocclusal acetate plate was installed, which after 30 days a decrease in symptoms was observed. It is concluded that an early diagnosis of bruxism in children is necessary to treat them in advance and avoid damage caused at the dental level.

Resumen: El Bruxismo es caracterizado por el apretamiento dental, que afecta las estructuras dentales y causa dolor. El tratamiento para que no interfiera negativamente en el crecimiento de los niños debe ser conservador y reversible. Este trabajo tiene como objetivo relatar un caso clínico de una niña con rechinamiento dental, dolor de cabeza, desgastes dentarios, ruidos en la ATM y dolor en músculos masticadores. Como tratamientos se realizaron los procedimientos odontológicos y fue instalada una placa interoclusal de acetato que luego de 30 días se observó disminución de los síntomas. Se concluye, que es necesario el diagnóstico p recoz del bruxismo en niños para tratarlos anticipadamente y evitar daños ocasionados a nivel dental.

Abstract Due to the importance of textbooks within the processes of teaching and learning in Mathematics, this article focuses on the tasks proposed in five textbooks of 1st of Bachillerato for this topic. The goal is to identify meanings of derivative in the textbooks through the proposed tasks. It is a quantitative research in which, by means of a cluster analysis, the tasks were grouped according to similarity. The results show that the books emphasize three meanings of the derivative: one procedural-algebraic, one algorithmic, and finally another conceptual-geometric meaning, all of them dominated by the symbolic representation system and that exclusively show a mathematical context.

Resumen Dada la importancia de los libros de texto dentro del proceso de enseñanza y aprendizaje de la matemática, este artículo se centra en las tareas propuestas en cinco libros de texto de 1º de Bachillerato para este tema. El objetivo es identificar los significados de derivada que ponen de manifiesto los libros de texto escolares a través de las tareas que proponen. Se trata de una investigación cuantitativa en la que mediante un análisis clúster se agruparon las tareas según similitud. Los resultados muestran que los libros destacan tres significados de la derivada: uno procedimental-algebraico, un segundo algorítmico y otro conceptual-geométrico; dominados por el sistema de representación simbólico y que manifiestan un sentido de aplicación exclusivamente matemático.

Abstract: For a long time, various agencies, such as the National Council of Teachers of Mathematics, have advocated promoting a meaningful teaching of mathematics. As mathematics teachers trainers, we show in this work our proposal of teacher training model in which we develop the mathematical sense through: the identification of capabilities in solving school mathematical tasks, as well as in the identification of specific learning objectives for these tasks. In this model, we review the mathematical capabilities by relating them to different mathematical senses- number, spatial, measurement and stochastic-and exemplify tasks that allow their development in future primary school teachers. Likewise, these tasks develop some capabilities that future teachers could use in their profession, due to the model goes beyond the memorization of the senses and their capabilities, and aims to highlight a functional knowledge of mathematics.

Resumen: Desde hace tiempo, diversos organismos, como el National Council of Teachers of Mathematics, promueven una enseñanza de las matemáticas con sentido. Desde nuestra perspectiva de formadores de profesores de matemáticas y basándonos en investigaciones de reconocido prestigio, mostramos en este trabajo un modelo de formación de profesores en el que desarrollamos el sentido matemático a través de tareas matemáticas escolares y de la identificación de objetivos de aprendizaje para dichas tareas. En este modelo, repasamos las capacidades matemáticas necesarias para enseñar matemáticas con sentido, relacionándolas con distintos sentidos matemáticos -numérico, espacial, de la medida y estocástico- y ejemplificamos con tareas de formación que permiten su desarrollo en futuros maestros de educación primaria. Asimismo, estas tareas desarrollan las capacidades que los futuros maestros podrán utilizar en su labor docente, pues se trata de un modelo que va más allá de la memorización de los componentes de los sentidos matemáticos, pretendiendo lograr un conocimiento funcional de las matemáticas.

Prevalence of psychotropic medication use and associated factors in the elderly

Background and Objectives: The object of our study was to estimate the prevalence of psychotropic drug use among non-institutionalised elderly people 65 years or older and identify possible conditioning factors. Methods: A cross-sectional, observational study was carried out of 1161 subjects aged 65 years and older. Participants were randomly selected from health care registry listings. The main outcomes of interest were based on self-reporting and included consumption of psychotropic and other drugs (filed by the ATC Classification System), mood (based on the GDS-VE), cognitive status (Pfeiffer Questionnaire), physical-functional assessment based on Katz score, health problems (ICPC-2 classification WONCA) and socio-demographic variables. Results: The prevalence of self-reported psychotropic drug consumption was 20.5% (95% CI: 18.2 to 22.8). Anxiolytics and hypnotics were consumed by 16.6% of the elderly, antidepressants by 7.0% and antipsychotics by only 2.1%. In multivariate logistical analyses, the use of psychotropics was independently associated with other drug consumption (non-psychotropics), (OR: 4.0, 95% CI: 1.7 to 9.5), presence of established depression (OR: 3.2, 95% CI: 1.3 to 7.6), presence of 4 or more comorbidities (OR: 2.7, 95% CI: 1.9 to 3.8), being female (OR: 2.1, 95% CI: 1.5 to 3.0) and being dependent for basic activities of daily living (OR: 1.7, 95% CI: 1.0 to 2.6). Conclusions: The prevalence of psychotropic drug use among the elderly from Albacete is high. Several factors were identified as potential determinants of psychotropic drug use. It will be important to evaluate the misuse of these drugs in order to inform effective, efficient and safe prescription strategies.

Cardiac shotgun injuries affecting the heart, pericardium and great vessels are occasionally encountered in clinical practice. Specifically in Spain, pellet wounds from hunting accidents have increased. The treatment of these injuries is not standardized due to the small number of cases attended in each particular center. We present two consecutive cases of cardiac pellet-gun related injuries treated in our institution with a conservative approach, with favorable outcomes.

Esporádicamente se presentan en la práctica clínica casos de heridas por arma de fuego que afectan el corazón, el pericardio y los grandes vasos. En el caso concreto de España, la afición a la caza hace que se atiendan pacientes con impacto de perdigones por accidente. El tratamiento de este tipo de traumatismo no está estandarizado, debido fundamentalmente a los pocos afectados tratados en cada centro particular. En esta presentación se describen dos casos consecutivos atendidos en nuestra institución por impacto de perdigón sobre estructuras cardíacas y cuya evolución fue satisfactoria con tratamiento conservador.
