Estilo de vida y autocuidado en el paciente con riesgo de pie diabético: Revisión de la literatura

Abstract Introduction: Vascular and neuropathic complications of diabetes cause diabetic foot, being the main risk factor for non-traumatic amputations. Objective: To analyze the available scientific evidence on lifestyle and self-care of patients at risk of diabetic foot. Methodology: Literature review using the preferred reporting items for systematic reviews and meta-analyses methodology; published in EBSCO host, Google academic, PubMed, Scielo and Elsevier; in English and Spanish from 2016 to 2022. Nursing articles were included, mentioning two or more variables of interest: diabetes mellitus, lifestyle, diabetic foot, and self-care; duplicates, no access, and gray literature were eliminated. From a total of 1,080 articles, 14 were included that met the inclusion criteria. Checklists were used to assess the quality of the manuscripts used. Results: Systematic reviews, observational, experimental and qualitative studies showed evidence that self-care can be influenced by culture, context and knowledge deficit, being this a risk factor for the development of diabetic foot. An association was found between the risk of diabetic foot with years of diagnosis, inadequate self-care of the feet and advanced age. Conclusion: The association between the self-care deficit and the risk of diabetic foot in the elderly was evidenced. It is recommended to broaden the search and include people under 40 years of age in order to establish effective preventive measures and reduce the risk of diabetic foot.
Abstrato Introdução: As complicações vasculares e neuropáticas da diabetes causam o pé diabético, sendo o principal fator de risco para amputações não traumáticas. Objetivo: Analisar a evidência científica disponível sobre o estilo de vida e o autocuidado dos doentes em risco de pé diabético. Metodologia: Revisão da literatura utilizando a metodologia preferred reporting items for systematic reviews and meta-analyses; publicada no EBSCO host, Google Scholar, PubMed, Scielo e Elsevier; em inglês e espanhol de 2016 a 2022. Foram incluídos artigos de enfermagem que mencionavam duas ou mais variáveis de interesse: diabetes mellitus, estilo de vida, pé diabético e autocuidado; foram removidos os duplicados, sem acesso e literatura cinzenta. De um total de 1.080 artigos, 14 que preencheram os critérios de inclusão foram incluídos. Foram utilizadas listas de verificação para avaliar a qualidade dos manuscritos utilizados. Resultados: Revisões sistemáticas, estudos observacionais, experimentais e qualitativos mostraram evidências de que o autocuidado pode ser influenciado pela cultura, contexto e défices de conhecimento, que são factores de risco para o desenvolvimento da doença do pé diabético. Foram encontradas associações entre o risco de pé diabético e os anos de diagnóstico, o autocuidado inadequado dos pés e a idade avançada. Conclusão: Verificou-se a associação entre défices de autocuidado e o risco de doença do pé diabético em idosos. Recomendamos o alargamento da pesquisa a pessoas com menos de 40 anos de idade, de forma a estabelecer medidas preventivas eficazes e reduzir o risco de doença do pé diabético.
Resumen Introducción: Las complicaciones vasculares y neuropáticas de la diabetes causan el pie diabético, siendo el principal factor de riesgo para amputaciones no traumáticas. Objetivo: Analizar la evidencia científica disponible sobre el estilo de vida y autocuidado de los pacientes con riesgo de pie diabético. Metodología: Revisión de la literatura mediante la metodología preferred reporting items for systematic reviews and meta-analyses; publicada en EBSCO host, Google académico, PubMed, Scielo y Elsevier; en inglés y español desde 2016 a 2022. Se incluyeron artículos de enfermería, que mencionaran dos o más variables de interés: diabetes mellitus, estilo de vida, pie diabético y autocuidado; se eliminaron duplicados, sin acceso abierto y literatura gris. De un total de 1,080 artículos se incluyeron 14 que reunieron los criterios de inclusión. Se emplearon listas de verificación para evaluar la calidad de los manuscritos utilizados. Resultados: Las revisiones sistemáticas, estudios observacionales, experimentales y cualitativos mostraron evidencias de que el autocuidado puede verse influenciado por la cultura, contexto y déficit de conocimientos, siendo este factor de riesgo para el desarrollo de pie diabético. Se encontró asociación entre el riesgo de pie diabético con años de diagnóstico, autocuidado inadecuado de los pies y edad avanzada. Conclusión: Se evidenció asociación entre déficit de autocuidado y riesgo de pie diabético en el adulto mayor. Se recomienda ampliar la búsqueda e incluir personas menores de 40 años para establecer medidas preventivas eficaces y reducir el riesgo de pie diabético.

The aim of this work was to extract and characterize pectin from lyophilized noni pulp (Morinda citrifolia Linn.) at three different pHs using different extractor agents and to evaluate anti-viral activity. The extraction of high yield (acid extraction) was subjected to sulfation procedure. Fourier transform infrared spectroscopy and proton nuclear magneticresonance analysis were used to calculate the degree of methylation of pectin from acid extraction and its sulfated derivative. The data obtained for the degree of methylation of noni pectin and sulfated pectin were 30.3 and 30.7, respectively. Pectin was subjected to acute oral toxicity testing in mice. The results showed that pectin had a pro-inflammatory effect, and sulfated pectin had an anti-inflammatory effect. Both pectins were also evaluated for herpes simplex virus activity in vitro, and although they successfully inhibited virus replication, the sulfated pectins were cytotoxic. Morinda Linn. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT Introduction: Violence has become a public health problem worldwide and Cuba is not exempt, there are few studies without exploring the associated variables. In Cuba, there are no validated instruments available to explore violence in its different scenarios and types of violence in children based on their exposure to it. The objective was to design and validate an instrument for the identification of exposure to violence in children. Methods: Technology development research, 480 sixth grade students from 11 to 13 years old participated who answered the questionnaire in test-retest mode with 30 days between them. The content validity was evaluated by a group of experts using the criteria of Moriyama; construct by confirmatory factor analysis; internal consistency by Cronbach's alpha coefficient and temporal stability by Wilcoxon signed rank mean comparison test and associated p. Results: Instrument validated for the study of exposure to violence as victims and observers of violence with the dimensions: physical violence, psychological violence, sexual violence and violence due to negligence and abandonment at home, school and community. All items met Moriyama's criteria, the construct structure was justified. The dimensions achieved acceptable to good coefficients of internal consistency (0.60 <= α < = 0.82), temporal stability was met (p > 0.05). The conclusions were that the instrument designed and validated to explore the exposure to violence as a victim and observers in the home, school and community scenarios in the variables; physical, psychological, sexual and negligence violence met its objectives for which it was designed.
RESUMEN Introducción: La violencia se ha convertido en un problema para la salud pública a nivel mundial y Cuba no está exenta, existen estudios aislados sin que se exploren las variables asociadas. En Cuba no se dispone de instrumentos validados para explorar la violencia en sus diferentes escenarios y tipos de violencia en niños a partir de su exposición a la misma. El objetivo fue diseñar y validar un instrumento para la identificación de la exposición a la violencia en niños. Métodos: Investigación de desarrollo de tecnología, participaron 480 alumnos de sexto grado de 11 años a 13 años quienes respondieron el cuestionario en modalidad test-retest con 30 días entre ellas. Se evaluó la validez de contenido por medio de un grupo de expertos utilizando los criterios de Moriyama; de constructo por análisis factorial confirmatoria; la consistencia interna por coeficiente alfa de Cronbach y estabilidad temporal por prueba de comparación de media de rangos con signos de Wilcoxon y p asociada. Resultados: Instrumento validado para el estudio de la exposición a la violencia como víctimas y observadores de violencia con las dimensiones: violencia física, violencia psicológica, violencia sexual y violencia por negligencia y abandono en la casa, escuela y comunidad. Todos los ítems cumplieron con criterios de Moriyama, se justificó la estructura de constructo. Las dimensiones lograron coeficientes aceptables a buenos de consistencia interna (0,60 <= α <= 0,82), se cumplió con la estabilidad temporal (p > 0,05). Las conclusiones fueron que el instrumento diseñado y validado para explorar la exposición a la violencia como víctima y observadores en los escenarios casa, escuela y comunidad en las variables; violencia física, psicológica, sexual y por negligencia cumplió sus objetivos para el cual fue diseñado.

Resumen La precocidad y adaptación del maíz nativo de Tamaulipas en ambientes restrictivos, permite considerarlo fuente de variabilidad para programas de mejoramiento genético, por ello es necesario conocer los efectos genéticos que controlan su variación. El objetivo fue determinar la acción génica que controla la variación de la precocidad de poblaciones de maíz nativo de Tamaulipas y la relación de la precocidad con el rendimiento de grano. En Güémez, Tamaulipas, en otoño-invierno 2019-2020 y primavera-verano 2020 con aplicación y no aplicación de insecticida para combatir S. frugiperda, se evaluaron los efectos genéticos de días a floración masculina y femenina de seis poblaciones de maíz, sus cruzas directas y reciprocas, mediante un análisis dialélico con el método de Griffing, se analizó la correlación entre la floración y el rendimiento de grano. Para floración masculina y femenina no existieron efectos de aptitud combinatoria general y existieron efectos de aptitud combinatoria especifica, recíprocos y maternos: la variación entre cultivares se explicó en 53% para floración masculina y 45% para femenina por efectos de aptitud combinatoria especifica, para floración masculina 38% de la variación entre cultivares y 43% para femenina fue debida a efectos recíprocos. La variación entre cultivares, dependió de efectos no aditivos y existió herencia citoplasmática involucrada en su expresión. La precocidad se correlacionó positivamente con el rendimiento de grano en el ciclo primavera-verano sin aplicación de insecticida. Con aptitud combinatoria especifica alta sobresalieron TML3S3×VHA y Cam×VHA, para aprovechar los efectos recíprocos en Cam×VHA hay que considerar el orden de los progenitores.
Abstract The earliness and adaptation of native corn of Tamaulipas in restrictive environments allows considering it a source of variability for genetic improvement programs, so it is necessary to know the genetic effects that control its variation. The objective was to determine the gene action that controls the variation of the earliness of native corn populations of Tamaulipas and the relationship of earliness with grain yield. In Güémez, Tamaulipas, in the agricultural cycles of autumn-winter 2019-2020 and spring-summer 2020 with application and non-application of insecticide to combat S. frugiperda, the genetic effects of days to male and female flowering of six corn populations, their direct and reciprocal crosses were evaluated, through a diallel analysis with the Griffing method, the correlation between flowering and grain yield was analyzed. For male and female flowering, there were no effects of general combining ability and there were effects of specific combining ability, reciprocal and maternal: the variation between cultivars was explained in 53% for male flowering and 45% for female flowering by effects of specific combining ability, for male flowering 38% of the variation between cultivars and 43% for female flowering was due to reciprocal effects. The variation between cultivars depended on non-additive effects and there was cytoplasmic inheritance involved in their expression. Earliness was positively correlated with grain yield in the spring-summer cycle without insecticide application. With high specific combining ability, TML3S3×VHA and Cam×VHA stood out, to take advantage of the reciprocal effects in Cam×VHA, the order of the parents must be considered.

Identificación de proteínas en Candidatus Liberibacter asiaticus para desarrollar un método de detección inmunoenzimático

Resumen El objetivo de este trabajo fue identificar en el genoma de Candidatus Liberibacter asiaticus (CLas), proteínas de membrana externa con potencial para el desarrollo y optimización de un método de detección inmunoenzimático. El estudio se realizó durante 2019 y se utilizó el servidor web Predict Protein, así como las bases de datos HhPred/HhSearch y Pfam. Se detectaron 52 proteínas de membrana externa en el genoma completo de CLas, de las cuales, 11 no habían sido caracterizadas previamente. Los análisis predictivos realizados en la proteína B8Y674 generaron ocho posibles epítopos y cuatro de ellos evaluados experimentalmente en células B, mostraron porcentajes de identidad entre 80 a 90%. Se detectó a CLas mediante PCR-punto final a partir del ADN extraído de limón mexicano con síntomas de Huanglongbing utilizando iniciadores diseñados sobre la secuencia del gen Omp que codifica para la proteína B8Y674 y se registró 95% de identidad entre las secuencias generadas y secuencias de CLas previamente reportadas. Los resultados obtenidos nos permiten inferir que la proteína B8Y674 es un candidato potencial para ser utilizada en la detección inmunoenzimática de CLas.
Abstract The objective of this work was to identify outer membrane proteins in the genome of Candidatus Liberibacter asiaticus (CLas) with potential for the development and optimization of an immunoenzymatic detection method. The study was conducted during 2019, and the Predict Protein web server, as well as the HhPred/HhSearch and Pfam databases, were used. Fifty-two outer membrane proteins were detected in the complete genome of CLas, of which 11 had not been previously characterized. Predictive analyses performed on the protein B8Y674 generated eight possible epitopes and four of them, experimentally evaluated in B cells, showed percentages of identity between 80 to 90%. CLas was detected by endpoint PCR from DNA extracted from Mexican lime with symptoms of Huanglongbing using primers designed on the sequence of the Omp gene encoding the protein B8Y674 and 95% identity was recorded between the generated sequences and sequences of CLas previously reported. The results obtained allow us to infer that the protein B8Y674 is a potential candidate to be used in the immunoenzymatic detection of CLas.

Abstract Background: In mezcal industry, Agave salmiana leaves are a crop residue and a potential source of fructans. Agave fructans are a soluble fiber that can act as prebiotic in gut microbiota. Questions and Hypotheses: The molecular weight distribution of agave fructans extracted from leaves of Agave salmiana depends on the region where agaves growth and on the subspecies. Studied species: Agave salmiana Otto ex Salm Dyck, spp. salmiana and Agave salmiana spp. crassispina. Study site: Twelve municipalities from Guanajuato México were sampled: Ocampo, San Felipe, San Diego de la Unión, Victoria, Xichú, Atarjea, Dolores Hidalgo, Doctor Mora, Santa Catarina, Tierra Blanca, San Miguel Allende and Comonfort. Methods: The base of the leaf close to the stem of Agave salmiana ssp. salmiana and A. salmiana ssp. crassispina plants, six years old, were harvested. Water soluble carbohydrates were extracted, the content and molecular weight distribution of fructans was determined by HPLC-SEC. Results: An average of 0.7 % of soluble carbohydrates was recovered from the leaves. The molecular weight distribution of Agave salmiana fructans was: number average molecular weight: 3,209 g/mol; average molecular weight: 5,046 g/mol; number average degree of polymerization: 19; weight average degree of polymerization: 30. High polymerization degree fructans content was greater in Agave salmiana spp. salmiana (55 %) than in Agave salmiana spp. crassispina (47 %). Conclusions: The leaves of Agave salmiana contain a low proportion of high-molecular-weight fructans, compared to commercial agave fructans. The results demonstrated the technical feasibility to obtain fructans from Agave salmiana leaves.
Resumen Antecedentes: Las hojas de Agave salmiana, como residuo de la industria mezclaera, podrían ser fuente de fructanos. Los fructanos de agave son fibras solubles prebióticas. Hipótesis: La distribución de tamaños de fructanos extraídos de hojas de Agave salmiana será distinta en función de la región y de la subespecie. Especies estudiadas: Agave salmiana Otto ex Salm Dyck, spp. salmiana and Agave salmiana spp. crassispina. Lugar de estudio: Doce municipios de Guanajuato: Ocampo, San Felipe, San Diego de la Unión, Victoria, Xichú, Atarjea, Dolores Hidalgo, Doctor Mora, Santa Catarina, Tierra Blanca, San Miguel Allende and Comonfort. Métodos: Se muestreó la base de hojas de Agave salmiana ssp. salmiana y A. salmiana ssp. crassispina de seis años. Se extrajeron los carbohidratos solubles en agua y se determinó contenido y distribución de tamaños de los fructanos por HPLC-SEC. Resultados: Se extrajeron en promedio 0.7 % de carbohidratos solubles. La distribución de tamaños de los fructanos de Agave salmiana fue: peso molecular promedio en número: 3,209 g/mol; peso molecular promedio en masa: 5,046 g/mol; grado de polimerización promedio en número: 19; grado de polimerización promedio en masa: 30. El contenido de fructanos con alto grado de polimerización fue mayor en Agave salmiana spp. salmiana (55 %) que en Agave salmiana spp. crassispina (47 %). Conclusiones: Las hojas de Agave salmiana contienen baja cantidad de fructanos de alto peso molecular comparados con fructanos de agave comerciales. Se demostró la factibilidad técnica de obtener fructanos a partir de hojas de Agave salmiana.

Abstract Objective: To evaluate the health systems’ response capacity according to the perception of chronic patients, and the factors related to that perception. Method: Source of data: patients diagnosed with at least one chronic disease who visited primary care centers during June and July 2015 in a basic health area of La Rioja. Design: cross-sectional descriptive study based on interviews to over 18s who visited primary care centers. The dependent variable was the health systems’ response capacity and independent variables were sociodemographic and health related. In order to collect data, trained interviewers conducted a short questionnaire in Spanish from the World Health Organization Multi-country Survey Study with 403 subjects. Descriptive statistics, bivariate and multivariate logistic regression were performed. Results: The overall health systems’ response capacity was considered good by 87.10%. The domains that scored highest were: confidentiality (99.3%), dignity (98.3%) and communication (97.3%). Those evaluated worst were: rapid service (38,6%) and quality of basic services (31.8%). Low social class was the most important factor associated with the responsiveness, mainly with autonomy and rapid service. Sex, educational level, and occupation were related to communication domain, and patients with worse perceived health rated the general response worse. The domains considered most important were dignity (33.5%) and rapid service (30.5%). Conclusions: The domains best evaluated were those related to respect for people. Rapid service has a low health systems’ response capacity, but a high importance, and therefore requires priority action.

La clase social baja fue el factor más importante asociado a la capacidad de respuesta, concretamente con la autonomía y la atención rápida. Sexo, educación y ocupación estuvieron relacionados con comunicación, y los pacientes con peor salud percibida valoraron peor la respuesta general. Los dominios considerados más importantes fueron dignidad (33,5%) y atención rápida (30,5%). Conclusiones: Los dominios mejor evaluados fueron los relacionados con el respeto a las personas. In order to collect data, trained interviewers conducted a short questionnaire in Spanish from the World Health Organization Multi-country Survey Study with 403 subjects. Descriptive statistics, bivariate and multivariate logistic regression were performed. Results: The overall health systems’ response capacity was considered good by 87.10%. The domains that scored highest were: confidentiality (99.3%), dignity (98.3%) and communication (97.3%). Those evaluated worst were: rapid service (38,6%) and quality of basic services (31.8%). Low social class was the most important factor associated with the responsiveness, mainly with autonomy and rapid service. Sex, educational level, and occupation were related to communication domain, and patients with worse perceived health rated the general response worse. The domains considered most important were dignity (33.5%) and rapid service (30.5%). Conclusions: The domains best evaluated were those related to respect for people. Rapid service has a low health systems’ response capacity, but a high importance, and therefore requires priority action.

RESUMO Objetivo: Analisar a prevalência e os fatores associados às doenças crônicas não transmissíveis em pessoas surdas. Métodos: Estudo transversal com 110 pessoas surdas, em Maringá-Paraná, selecionadas com a técnica snowball sampling. Coletaram-se dados de fevereiro a agosto de 2019, mediante aplicação de instrumento estruturado; e, na análise, utilizou-se regressão logística múltipla. Resultados: A prevalência autorreferida de doenças crônicas foi de 43,6%, sendo as mais frequentes: hipertensão arterial (12,7%), depressão (6,4%), diabetes mellitus (5,4%), doença respiratória (5,4%) e hipotireoidismo (4,5%). Utilizar os serviços de saúde para consulta de rotina apresentou associação significante com ser bilíngue bimodal. O único comportamento de risco associado significantemente com doenças crônicas foi consumo excessivo de alimentos doces Conclusão: A prevalência de doenças crônicas nessa população pode ser maior do que o encontrado, pois há a possibilidade do subdiagnóstico decorrente da pouca procura por consultas de rotina e da dificuldade de comunicação com os profissionais de saúde.
ABSTRACT Objective: To analyze the prevalence and factors associated with non-communicable chronic diseases in deaf people. Methods: Cross-sectional study with 110 deaf people in Maringá-Paraná, selected using the snowball sampling technique. Data were collected from February to August 2019, using a structured instrument; and, in the analysis, multiple logistic regression was used. Results: The self-reported prevalence of chronic diseases was 43.6%, the most frequent being: arterial hypertension (12.7%), depression (6.4%), diabetes mellitus (5.4%), respiratory disease (5.4%) and hypothyroidism (4.5%). Using health services for routine consultations was significantly associated with being bimodal bilingual. The only risk behavior significantly associated with chronic disease was excessive consumption of sweet foods. Conclusion: The prevalence of chronic diseases in this population may be higher than that found, as there is a possibility of underdiagnosis due to the low demand for routine consultations and the difficulty of communication with health professionals.

Recolectados datos de febrero a agosto de 2019, mediante aplicación de instrumento estructurado; y, en el análisis, utilizado regresión logística múltiple. Resultados: La prevalencia autorreferida de enfermedades crónicas fue de 43,6%, siendo las más frecuentes: hipertensión arterial (12,7%), depresión (6,4%), diabetes mellitus (5,4%), enfermedad respiratoria (5,4%) e hipotiroidismo (4,5%). Utilizar los servicios de salud para consulta rutinaria presentó relación significante con ser bilingüe bimodal. La única conducta de riesgo relacionado significativamente con enfermedades crónicas fue consumo excesivo de alimentos dulces. Conclusión: La prevalencia de enfermedades crónicas en esa población puede ser mayor que lo encontrado, pues hay la posibilidad del sobrediagnóstico resultante de la escasa búsqueda por consultas rutinaria y de la dificultad de comunicación con los profesionales de salud.

ABSTRACT Introduction: Brazilian borreliosis (BB) disease is an infectious disease transmitted by ticks that mimics Lyme disease (LD) from the Northern Hemisphere. The BB clinical picture is characterized by a pathognomonic skin lesion (migratory erythema) and joint, neurological, cardiac and psychiatric symptoms. Innate and Th1/Th17 adaptive immunity seem to play an important role in the pathogenesis of Lyme disease. Objective: The aim of this study was to characterize the role of innate and Th1/Th17 adaptive immunity in BB patients with acute (<3 months) and convalescent (>3 months) disease. Methods: Fifty BB patients (28 with acute and 22 with convalescent disease) without treatment and 30 healthy subjects were evaluated. Levels of 20 cytokines or chemokines associated with innate and Th1/Th17 adaptive immunity were analyzed using Luminex (Millipore Corp., Billerica, MA). Results: Overall, BB patients had increased levels of IL-8 (6.29 vs 2.12 p = 0.002) and MIP-1α/CCL3 (5.20 vs 2.06, p = 0.030), associated with innate immunity, and MIP3B/CCL19 (Th1; 297.86 vs 212.41, p = 0.031) and IL-17A (Th17; 3.11 vs 2.20, p = 0.037), associated with adaptive immunity, compared with the levels of healthy controls. When comparing acute BB vs. convalescent BB subjects vs. healthy controls, IL-1β, IL-8 and MIP-1α/CCL3 (innate mediators) levels were highest in patients in the acute phase of disease (p < 0.05). TNF-α was associated with disseminated symptoms and with humoral reactivity against Borrelia burgdorferi. IL-10 was significantly correlated with IL-6 (r = 0.59, p = 0.003), IL-8 (r = 0.51, p < 0.001), MIP-1α/CCL3 (r = 0.42, p < 0.001) and MIP-3β/CCL19 (r = 0.40, p = 0.002) in all BB patients. Conclusions: This is the first study describing that innate and Th1/Th17 adaptive immunity play a crucial role in BB disease. Furthermore, innate mediators are particularly important in acute BB disease, and TNF-α is associated with evolution of BB symptoms.

RESUMO Introdução: Mastócitos podem participar da inflamação e contribuir para fibrose na nefrite lúpica (NL). Objetivo: Correlacionar mastócitos em biópsias renais (BR) de pacientes pediátricos com NL com índices de atividade (IA) e cronicidade (IC), avaliando sua efetividade como fator prognóstico. Metodologia: Foram estudados 40 pacientes, entre 6 e 18 anos, diagnosticados com NL pelo Serviço de Nefropatologia da UFTM entre 1996 e 2015. Dados laboratoriais e epidemiológicos foram correlacionados com IA, IC e contagem de mastócitos (CM). Resultados: Encontramos correlação positiva e significativa entre contagem de mastócitos (CM) e IA (p = 0,003; r: 0,66) e entre CM e IC (p = 0,048; r: 0,48). Conforme a curva Roc, os mastócitos têm alta sensibilidade e especificidade na diferenciação de pacientes com IA menor ou maior do que 12. A creatinina sérica foi mais elevada na classe IV em relação à classe V [1,50 (0,40 - 20,90) versus 0,70 (0,62 - 0,90), p = 0,04]. Ureia sérica apresentou correlação positiva e significativa com CM (p = 0,002; r: 0,75). Observou-se tendência à correlação negativa entre CM e albumina sérica (p = 0,06; r: -0,5459). BR de pacientes com síndrome nefrótica apresentaram maior CM [2,12 (0,41 - 5,140) versus 0,53 (0,0 - 3,94), p = 0,07]. Conclusão: Não apenas o infiltrado inflamatório como também a diferenciação morfológica dos tipos celulares que o constituem são importantes para a avaliação da NL. Isso indica que a análise dos mastócitos, juntamente com a dos IA e IC, pode ajudar os nefrologistas a definirem o prognóstico de pacientes pediátricos.
ABSTRACT Introduction: Mast cells may be involved in inflammation and contribute to the onset of fibrosis in lupus nephritis (LN). Objective: This study aimed to correlate the presence of mast cells in kidney biopsy specimens of pediatric patients with LN with activity (AI) and chronicity (CI) indices and assess how effectively mast cells may be used as a prognostic factor. Method: The study included 40 patients aged 6-18 years diagnosed with LN at the Renal Disease Service of the Federal University of Triângulo Mineiro between 1996 and 2015. Workup and epidemiological data were evaluated vis-à-vis AI, CI, and mast cell counts (MCC). Results: Significant positive correlations were found between mast cell counts (MCC) and AI (p = 0.003; r: 0.66) and MCC and CI (p = 0.048; r: 0.48). The ROC curve showed that mast cells were highly sensitive and specific in the differentiation of patients with an AI > 12 from individuals with an AI ≤ 12. Serum creatinine levels were higher in individuals with class IV LN than in patients with class V disease [1.50 (0.40-20.90) vs. 0.70 (0.62-0.90), p = 0.04]. Blood urea nitrogen had a positive significant correlation with MCC (p = 0.002; r: 0.75). A trend toward a negative correlation was observed between MCC and serum albumin (p = 0.06; r: -0.5459). Kidney biopsies of patients with nephrotic syndrome had higher MCC [2.12 (0.41-5.140) vs. 0.53 (0.0-3.94), p = 0.07]. Conclusion: Inflammatory cell infiltration and morphological differences between cell types in the inflammatory infiltrate are relevant factors in the assessment of the LN. Mast cell analysis and AI/CI assessment may be relevant prognostic indicators for pediatric patients with LN.

Resumo Introdução: Alguns casos de nefropatia membranosa (NM) apresentam glomeruloesclerose segmentar e focal (GESF) tipicamente associada a progressão da doença. Contudo, relatamos o caso de uma paciente que parece ter NM e GESF, ambas primárias. Apresentação do caso: Uma jovem branca de 17 anos de idade com edema de membros inferiores associado a episódios de urina espumosa e hipertensão apresentou-se com achados físicos e laboratoriais sugestivos de síndrome nefrótica. Foi realizada biópsia renal. GESF foi observada por microscopia de luz em alguns glomérulos que apresentavam lesões de ponta, enquanto em outros o achado era acompanhado por hipertrofia podocitária e descolamento de podócitos no espaço urinário, compatíveis com podocitopatia GESF. Além disso, as alças capilares estavam espessadas com irregularidades na membrana basal devido a "espículas" compatíveis com NM estágio II. Imunofluorescência revelou depósitos finamente granulares de IgG, IgG4 e PLA2R nas alças capilares. Microscopia eletrônica exibiu depósitos subepiteliais e apagamento de pedicelos. Tais achados morfológicos são compatíveis com GESF e NM estágio II. Conclusões: No presente caso, as características clínicas e morfológicas revelaram uma possível sobreposição de GESF primária e NM, uma vez que a glomeruloesclerose segmentar e focal não parece estar relacionada com a progressão da NM, mas com a podocitopatia GESF.
Abstract Introduction: Some cases of membranous nephropathy (MGN) present focal segmental glomerulosclerosis (FSGS) typically associated with disease progression. However, we report a case of a patient who seemed to have MGN and FSGS, both primary. Case presentation: A 17-year-old female, Caucasian, presenting lower extremity edema associated with episodes of foamy urine and high blood pressure, had physical and laboratorial exams indicating nephrotic syndrome. A renal biopsy was performed and focal and segmental glomerulosclerosis were observed under light microscopy in some glomeruli presented as tip lesion, and in others it was accompanied by podocyte hypertrophy and podocyte detachment in urinary space, compatible with podocytopathy FSGS. Besides, there were thickened capillary loops with basement membrane irregularities due to "spikes" compatible with MGN stage II. Immunofluorescence showed finely granular IgG, IgG4, and PLA2R deposits in capillary loops and, in electron microscopy, subepithelial deposits and foot process effacement. These morphological findings are compatible with FSGS and MGN stage II. Conclusions: In the present case, clinical and morphological characteristics showed a possible overlap of primary FSGS and MGN as focal and segmental glomerulosclerosis does not seem to be related with MGN progression but with the podocytopathy FSGS.

Resumo Objetivo avaliar as contribuições da Terapia Comunitária sobre a ansiedade e a depressão entre usuários de drogas psicoativas. Métodos estudo quase-experimental, realizado com 21 homens residentes em três instituições de saúde mental voltadas à recuperação da dependência química, submetidos a seis rodas de terapia comunitária como processo de intervenção em 2018. A coleta de dados ocorreu em três etapas a partir do uso de um questionário semiestruturado e dois inventários de ansiedade e depressão de Beck. Utilizou-se método estatístico não-paramétricos na comparação dos resultados. Resultados a depressão esteve presente entre 76% dos usuários e a ansiedade entre 48%. Dentre os participantes das rodas, houve uma redução nos níveis de depressão durante e após o processo de intervenção (p=0,016; p=0,004) quando comparado ao estado inicial e para manter a média dos escores de ansiedade no T1 e T2 (9,90; 9,95) se comparado ao T0 (13,10). Conclusão e implicações para a prática o uso da terapia comunitária demonstrou resultados positivos sobre a ansiedade e a depressão, sendo então considerada uma importante ferramenta de cuidado em saúde mental a ser utilizada por enfermeiros, com vistas a ampliar o seu cuidado as pessoas em situação de dependência química, contribuindo ainda para adesão ao tratamento.
