RESUMO Objetivo: Descrever as etapas de um protocolo de decanulação e os resultados da aplicação desse protocolo em crianças hospitalizadas. Métodos: Trata-se de um estudo observacional retrospectivo. Os dados foram coletados de prontuários de pacientes decanulados acompanhados em um hospital pediátrico de Belo Horizonte, Minas Gerais, entre 2011 e 2021. Resultados: Foram analisadas 120 crianças com idade entre dois meses e 16 anos, com média de idade de quatro anos, 69% das quais eram do sexo masculino. Cerca de 75% dos pacientes apresentam traqueostomia por obstrução de vias aéreas superiores e 60% destes por estenose subglótica adquirida. No início do protocolo de decanulação, 5,5% dos pacientes apresentavam disfagia orofaríngea moderada, enquanto 80,4% apresentavam deglutição normal. A correção da pré-decanulação das vias aéreas superiores foi realizada em 39,5% dos pacientes, a dilatação em 63,8% e a correção endoscópica em 55,3%. Após a realização da decanulação, nenhum paciente apresentou complicações. Conclusões: O protocolo de decanulação descrito é seguro, pois a taxa de complicações como óbito e a necessidade de recanulação foi ausente. ABSTRACT Objective: The aim of this study was to describe the phases of a decannulation protocol and the results from its application in hospitalized children. Methods: This is a retrospective, observational study. Data were collected from medical records of decannulated patients followed up in a pediatric hospital in Belo Horizonte, Minas Gerais between 2011 and 2021. Results: Among the children followed up in the service (n=526), 23% (n=120) were successfully decannulated. Children aged between 2 months and 16 years, with a mean age of 4 years, 69% of whom were male, were evaluated. About 75% of the patients have tracheostomy due to upper airway obstruction and 60% of these due to acquired subglottic stenosis. At the beginning of the decannulation protocol, 5.5% of the patients had moderate oropharyngeal dysphagia, while 80.4% had normal swallowing. Correction in the upper airway pre-decannulation was performed in 39.5% of the patients, dilation in 63.8%, and endoscopic correction was performed in 55.3%. After performing the decannulation, none of the patients had complications. Conclusions: The described decannulation protocol is safe, since no complications such as death and need for recannulation happened. Resumen Este texto reflexiona sobre el uso de abordajes metodológicos colaborativos en proyectos de extensión específicos en el área de la comunicación, más específicamente para la comunicación estratégica en contextos de desastres socioambientales. Para ello, aborda las acciones del proyecto de extensión de la carrera de Relaciones Públicas de la Universidad Federal de Alagoas, Brasil, el Laboratorio de Comunicación Colaborativa. Se parte de una comprensión de la extensión universitaria como el intercambio de conocimientos —académicos y populares— que resulta en la democratización del conocimiento, la participación efectiva de la comunidad en las actividades universitarias y una producción resultante de enfrentarse con la realidad. Se sostiene que los procesos colaborativos, distintos de los cooperativos, generan aprendizajes para todos los involucrados, allanan el camino para la apropiación de acciones, lenguajes y tecnologías, además de posibilitar la problematización colectiva de cuestiones sociales.

Abstract This paper discusses the use of collaborative methodological approaches in university extension projects in the area of communication, more specifically for strategic communication in contexts of socio-environmental disasters. To this end, see the actions of the extension project for the Public Relations course at the Federal University of Alagoas, Brazil, the Collaborative Communication Laboratory. It starts from an understanding of university extension as the exchange of knowledge —academic and popular— resulting in the democratization of knowledge, the effective participation of the community in the university’s activities and a production resulting from the confrontation with reality. It is argued that collaborative processes, which are distinct from cooperatives, generate learning for everyone involved, paving the way for the appropriation of actions, languages and technologies, in addition to enabling collective problematization of social issues.

Resumo O presente texto faz uma reflexão sobre a utilização de abordagens metodológicas colaborativas em projetos de extensão voltados para a área da comunicação, mais especificamente para a comunicação estratégica em contextos de desastres socioambientais. Para tanto, olha para as ações do projeto de extensão do curso de Relações Públicas da Universidade Federal de Alagoas, Brasil, o Laboratório Colaborativo de Comunicação. Parte-se de uma compreensão de extensão universitária como a troca de saberes –acadêmico e popular– tendo como consequência a democratização do conhecimento, a participação efetiva da comunidade na atuação da universidade e uma produção resultante do confronto com a realidade. Defende-se que os processos colaborativos, que são distintos dos cooperativos, geram aprendizados para todos os envolvidos, abrindo caminho para a apropriação dos fazeres, linguagens e tecnologias, além de possibilitar problematizações coletivas das questões sociais.

Abstract The total cost of processes that use lipases can be reduced by using the mycelium or not purified crude extract with lipolytic activity. These catalysts can be used without complex purification and can show desirable properties to emerging applications like hydrolysis and biotransformation of vegetable oils. To explore the potential of multiple forms of lipases (produced by the same filamentous fungus species), this work investigated intracellular and extracellular lipases produced by Trichoderma koningii (intracellular lipases were never studied), Penicillium spp. F02 (isolated from industrial grease trap) and Aspergillus niger. For this, batch fermentations were conducted for 96h using olive oil as a carbon source. The submerged culture was interrupted with vacuum filtration to isolate the mycelium and crude extract. The lipolytic activities were determined by the hydrolysis method of olive oil. The best results were obtained for intracellular activity with T. koningii (81.5 U/g of dry biomass) and for extracellular activity with A. niger (10.6 U/mL of crude extract). Both were attained in 96h of fermentation. T. koningii showed an accumulation of intracellular lipases and a depreciated extracellular activity in the initial 24 hours, which can be reasonable indications that there is strong retention of lipases in the mycelium. Statistical analysis within the range of 37°C to 44°C associated with pH from 7.8 to 8.0 favored the activity of lipases from T. koningii. SUMMARY OBJECTIVE: The objective of this study was to assess the predictors of survival among patients with coronavirus disease 2019 who underwent tracheal intubation, as part of a hospital cohort study. METHODS: This retrospective cohort study in the Rio do Sul County Hospital, Santa Catarina, Brazil, from April 2020 to May 2021, focused on patients aged 18 years or older intubated for coronavirus disease 2019. We assessed the 90-day survival of intubated patients by estimating the hazard ratio using a Cox proportional hazards regression model. RESULTS: The study included 132 participants, with an average age of approximately 60 years. Tracheal intubation was successfully accomplished in 97% of cases within two attempts. The overall mortality rate was 62.9%. Notably, mortality rates were significantly higher in patients aged over 60 years (hazard ratio=2.57; 95%CI 1.54–4.29; p<0.001), those with blood oxygen saturation below 85% (hazard ratio=1.92; 95%CI 1.03–3.57; p=0.04), instances where tracheal intubation was carried out using a conventional laryngoscope (hazard ratio=2.59; 95%CI 1.22–5.48; p=0.013), and when performed by emergency physicians (hazard ratio=3.96; 95%CI 1.51–10.4; p=0.005). CONCLUSION: Our analysis unveiled that the risk of death in intubated coronavirus disease 2019 patients is four times higher when an emergency physician, as opposed to an anesthesiologist, leads the tracheal intubation team. Abstract At low concentrations used for in-office bleaching gels, such as 6% HP, gingival barrier continues to be performed. If we take into account that, in the at-home bleaching technique, no barrier is indicated, it seems that the use of a gingival barrier fails to make much sense when bleaching gel in low concentration is used for in-office bleaching. Objective This double-blind, split-mouth, randomized clinical trial evaluated the gingival irritation (GI) of in-office bleaching using 6% hydrogen peroxide (HP) with and without a gingival barrier in adolescents, as well as color change and the impact of oral condition on quality of life. Methodology Overall, 60 participants were randomized into which side would or would not receive the gingival barrier. In-office bleaching was performed for 50 minutes with 6% HP in three sessions. The absolute risk and intensity of GI were assessed with a visual analogue scale. Color change was assessed using a digital spectrophotometer and color guides. The impact of oral condition on quality of life was assessed using the Brazilian version of the Oral Health Impact Profile (α=0.05). Results The proportion of patients who presented GI for the “with barrier” group was 31.6% and for the “without barrier” group, 30% (p=1.0). There is an equivalence for the evaluated groups regarding GI intensity (p<0.01). Color change was detected with no statistical differences (p>0.29). There was a significant impact of oral condition on quality of life after bleaching (p<0.001). RESUMO Contexto: Avaliar a relação entre a razão de linfonodos (RLA) acometidos e variáveis clínicas e anatomopatológicas em portadores de adenocarcinoma de reto submetidos ou não à quimiorradioterapia neoadjuvante. Métodos: A RLA foi determinada dividindo-se o número total de linfonodos (LFNs) dissecados no espécime cirúrgico pelo número de comprometidos. Os doentes foram divididos em dois grupos: com QRT e sem QRT. Em cada grupo foi avaliada a relação entre a RLA e as seguintes variáveis: grau de diferenciação celular, profundidade de invasão na parede retal, invasão angiolinfática/perineural, grau de regressão tumoral e ocorrência de metástases. Avaliou-se a RLA em pacientes com mais do que 12 LFNs (RLA>12) ou menos (RLA<12) na peça cirúrgica com a sobrevida global (SG) e sobrevida livre de doença (SLD). Os resultados foram expressos pela média com o respectivo desvio padrão. As variáveis qualitativas foram analisadas utilizando-se o teste exato de Fisher, enquanto as quantitativas pelos testes de Kruskal-Wallis e Mann-Whitney. O nível de significância foi de 5%. ABSTRACT Background: To evaluate the relationship between the ratio of affected lymph nodes (LNR) and clinical and anatomopathological variables in patients with rectal adenocarcinoma submitted or not to neoadjuvant chemoradiotherapy. Methods: The LNR was determined by dividing the number of compromised LNR by the total number of LNR dissected in the surgical specimen. Patients were divided into two groups: with QRT and without QRT. In each group, the relationship between LNR and the following variables was evaluated: degree of cell differentiation, depth of invasion in the rectal wall, angiolymphatic /perineural invasion, degree of tumor regression and occurrence of metastases. The LNR was evaluated in patients with more than 1, LNR (LNR >12) or less (LNR<12) in the surgical specimen with overall survival (OS) and disease-free survival (DFS). The results were expressed as the mean with the respective standard deviation. Qualitative variables were analyzed using Fisher’s exact test, while quantitative variables were analyzed using the Kruskal -Wallis and Mann-Whitney tests. The significance level was 5%. Results: We evaluated 282 patients with QRT and 114 without QRT, between 1995-2011. In the QRT Group, LNR showed a significant association with mucinous tumors (P=0.007) and degree of tumor regression (P=0.003). In both groups, LNR was associated with poorly differentiated tumors (P=0.001, P=0.02), presence of angiolymphatic invasion (P<0.0001 and P=0.01), perineural (P=0.0007, P=0.02), degree of rectal wall invasion (T3>T2; P<0.0001, P=0.02); Compromised LNR (P<0.0001, P<0.01), metastases (P<0.0001, P<0.01). Abstract Laterite gravels have been successfully adopted as pavement materials in the field. Despite that, they are often rejected in material selection processes because of its base on traditional gradation and consistency limits requirements. To allow a deeper evaluation of such material found abundantly in tropical countries, along to characterizing traditional parameters, repeated load triaxial tests were performed in a Laterite Gravel from Acre/Brazil to evaluate its elastic and plastic behavior through Resilient Modulus (RM) and Permanent Deformation (PD), respectively. ABSTRACT: Hops are grown mainly for the brewing market due to the rich chemical composition that gives beer aroma and bitterness. Brazil is one of the largest beer producers and consumers in the world and one of the largest hop importers. There is growing demand and interest in producing hops for domestic supply; however, as this crop originates from a temperate climate and the Northern Hemisphere, it is important to carry out research that describes the development of hops in a tropical climate to provide information on its cultivation in Brazil, as well as to determine its behavior in organic cultivation. Resumo: O tratamento químico de sementes com produtos fitossanitários sob condições de déficit hídrico, podem comprometer a mobilização das reservas das sementes para as plântulas. Objetivou-se avaliar a qualidade fisiológica, o desenvolvimento inicial de plântulas e a mobilização de reservas em sementes de soja tratadas com produtos fitossanitários sob déficit hídrico. O ensaio foi instalado em delineamento inteiramente casualizado em esquema fatorial 4 x 4: tratamentos químicos (Controle; Thiametoxam; Fludioxonil + Metalaxil-M e Fipronil + Piraclostrobina + Tiofanato-metílico) x potenciais osmóticos (0; -0,1; -0,2 e -0,3 MPa). Foram analisadas as variáveis: germinação, comprimento do hipocótilo, da raiz e total das plântulas, índice de vigor do crescimento, peso de matéria seca de cotilédones, rendimento de massa seca de plântulas, redução de reservas, rendimento relativo de matéria seca, taxa de redução de reservas e eficiência de conversão das reservas de sementes em matéria seca de plântulas. Abstract: Seed treatments with chemical phytosanitary products can compromise mobilization of seed reserves for the seedlings under water deficit conditions. The aim of this study was to evaluate the physiological quality, the initial seedling development, and the mobilization of reserves in soybean seeds treated with phytosanitary products under water deficit. The trial was set up in a completely randomized design in a 4 × 4 factorial arrangement: chemical treatments (control, Thiamethoxam, Fludioxonil + Metalaxyl-M, and Fipronil + Pyraclostrobin + Tiophanate-methyl) × osmotic potentials (0, -0.1, -0.2, and -0.3 MPa). We analyzed the following variables: germination; hypocotyl length, root length, and total seedling length; seedling length vigor index; cotyledon dry matter weight; seedling dry matter yield; seed reserve reduction; relative dry matter yield; seed reserve reduction rate; and conversion efficiency of seed reserves into seedling dry matter. Seed quality and seedling development decline under water deficit conditions. The treatment with Fipronil + Pyraclostrobin + Thiophanate-methyl results in lower phytotoxicity. ABSTRACT Objectives: to identify mothers’ perceptions about caring for newborns in the home environment, from the perspective of complexity thinking. Methods: qualitative, exploratory and descriptive research, carried out between November/2022 and February/2023. Data were collected through individual interviews with 21 mothers from southern Brazil who cared for newborns at home and analyzed using the thematic analysis technique. Results: the four thematic axes resulting from the data analysis: Living amidst order and disorder; embracing singularities; dealing with the certain and the uncertain; support network in the (re)organizing process demonstrate that the mother caring for a newborn in their home environment experiences a distinct and plural adaptive process, which must be welcomed and understood by health professionals who work within the family environment. Final Considerations: the care of newborns in a home environment, in the perception of mothers, requires differentiated attention and a formal or informal support network that considers the unique specificities of each woman/mother in the personal, family and social spheres. Abstract The study aimed to investigate oral potentially malignant disorders (OPMDs) diagnosed in an Oral Pathology service in southern Brazil over a span of 56 years and to assess the factors influencing their severity and outcomes. A retrospective analysis of histopathological records from 1965 to 2021 was performed. Lesions diagnosed as leukoplakia, erythroplakia, leukoerythroplakia, or actinic cheilitis were included. Data on age, sex, race, tobacco smoking, alcohol drinking, clinical characteristics, biopsy type, and histopathological diagnosis were collected. Among 32.698 histopathological reports, 2.10% were classified as OPMD. The study included 689 specimens from 602 patients, with a predominance of cases affecting men aged over 51 years, particularly in anatomical sites considered of low risk. White lesions, with leukoplakia as the most common clinical diagnosis were observed. Over time, there was an increase in the incidence of OPMDs among females and in high-risk anatomical sites. The mean follow-up period was 30.28 ± 29.86 months, during which 9.6% of cases exhibited malignant transformation. Lesions diagnosed as leukoerythroplakia and those presenting epithelial dysplasia exhibited a higher risk of malignant transformation, particularly when located in high-risk sites. This underscores the importance of identifying clinical and microscopic features, as well as lesion’s anatomical location, as crucial factors in determining the risk of malignant transformation. These findings provide valuable insights for clinicians managing OPMDs and contribute to the improvement of oral cancer prevention. Abstract Rural extension plays a significant role in rural development, acting as an instrument of economic, social, and environmental leverage. Aware of this importance, Ceará has restructured its extension services by implementing the Rural Agent Program in 2012. In this sense, this study aims to evaluate the impact of this policy on agricultural sustainability and the generation of employment and income for family farmers in the municipality of Crato, Ceará, in 2021. Abstract Background Strategies aimed at improving the diagnosis and treatment of cardiovascular diseases (CVD) are of utmost importance. Clinical decision support systems (CDSS) are guided by updated guidelines and may be capable of benefiting screening and early diagnosis initiatives in remote regions. Objective To develop a CDSS for screening hypertension and diabetes mellitus (DM), as well as to assess its feasibility and usability in the context of a primary care setting in a resource-constrained region. Methods This research focused on the strategy of screening hypertension and DM based on CDSS. A software was developed and implemented in 10 municipalities in the Northeast of the state of Minas Gerais, Brazil, from April 2017 to October 2018. Patients of 30 to 69 years of age were attended by primary care practitioners using the software. A Likert-scale questionnaire with 15 questions to assess usability (System Usability Scale) and satisfaction was applied to the professionals at the end of study. Groups were compared by applying the chi-squared test for categorical variables. Statistical significance was considered as p < 0.05. Results In the field study, 13,775 individuals were assessed; 185 patients were diagnosed with hypertension; 35 were diagnosed with DM, and 5 were diagnosed with both diseases. For the usability and satisfaction assessment, 258 healthcare professionals participated. Fifty-one percent considered their prior knowledge for the use of technologies to be good; 53.7% reported a desire to use the application frequently, and 78.4% would recommend the platform. Conclusion The implementation of a CDSS developed to assist in the screening of hypertension and DM was feasible in the context of a primary care setting in a resource-constrained region, with good user satisfaction.
