Medicines for reduced ejection fraction heart failure in Central region of Portugal’s primary care: a 2022 observational study

Abstract Objetivo: Avaliar a concordância da terapêutica com medicamentos modificadores de prognóstico (MMP) em pessoas com insuficiência cardíaca crónica com fração de ejeção reduzida (P-CICFEr), em cuidados de saúde primários (CSP) na região Centro de Portugal, com as diretrizes da Sociedade Europeia de Cardiologia (ESC) de agosto de 2021. Métodos: Estudo observacional transversal de uma amostra representativa do tamanho, intervalo de confiança de 95% e margem de erro de 8%, da população com CICFEr em 31/dezembro/2021. Foram estudados idade, sexo, ano de diagnóstico da introdução da Classificação Internacional para Cuidados Primários-2 (ICPC-2, K-77), anos desde o último ecocardiograma, MMPs para P-CICFEr e outras terapêuticas para P-CICFEr. Participaram, convidadas por conveniência, 11 Unidades de Saúde Familiar após parecer positivo da Comissão de Ética. Resultados: Num universo de n=2.381 pessoas com insuficiência cardíaca (PCIC) encontraram-se n=453 (19,0%) (P-CICFEr) estudando-se uma amostra representativa de n=133 (5,6%), n=95 homens (71,4%). Verificou-se prescrição de medicamentos inibidores do sistema renina-angiotensina-aldosterona (iSRAA) 91,0%, de betabloqueadores em 75,2%, de antagonistas mineralocorticoides em 40,6%, de inibidores da SGLT2 em 44,4%, de inibidores da neprilisina em 38,3% e de diuréticos de ansa em 72,2%. A terapêutica farmacológica quádrupla verificou-se em n=28 (21,1%) P-CICFEr e a tripla em n=38 (28,6%). Verificou-se idade média de 74,3±11,6 anos, 75,1±4,0 no homem e 76,6±6,0 na mulher, p=0,088. A mediana de tempo desde o diagnóstico ICPC-2 foi de 6,0 anos no homem e de 4,0 anos na mulher, p=0,031. O tempo mediano desde o último ecocardiograma foi de três anos, p=0,750. Conclusão: Os medicamentos modificadores de prognóstico, segundo as diretrizes mais recentes da ESC para a CICFEr, estavam subutilizados em 50,4% dos casos. Apesar dos resultados encontrados é necessária reflexão acerca das barreiras que os médicos possam encontrar para aplicar as diretrizes em P-CICFEr.
Resumo Objective: To evaluate the accordance of prognostic medicines prescription (PMP), for persons with chronic heart failure with reduced ejection fraction (P-CHFrEF), attended in primary health care units in Central Portugal, with the European Society of Cardiology (ESC) 2021 August, guidelines. To know about mean age, median time since the diagnosis and last echocardiogram performance in P-CHFrEF. Methods: Observational, cross-sectional study of a random size representative sample, 95% confidence interval, and 8% error margin of the P-CHFrEF of a population on 31st December 2021. Age, gender, year of International Classification for Primary Care-2 (ICPC-2, K-77) problem classification input data, years since the last echocardiogram, ongoing PMP and the use of other chronic heart failure (CHF) drugs were gathered. Data were obtained from 11 Portugal Primary Health Care Units, invited by convenience, after an Ethics Committee approval. Results: From a universe of n=2,381 persons with CHF, n=453 (19.0%) P-CHFrEF was found. A size representative sample of P-CHFrEF of n=133 (5.6%), n=95 males (71.4%) was studied. Prescription of a renin-angiotensin system inhibitor (RAAS-I) was ongoing in 91.0%, beta-blocker (BB) in 75.2%, mineralocorticoid antagonists (MRA) in 40.6%, inhibitors of the sodium-glucose cotransporter-2 (SGLT2i) in 44.4%, neprilisine inhibitors 38.3% and loop diuretics in 72.2%. The optimized medical quadruple therapy was established in 28 patients (21.1%) and the triple one in 38 (28.6 %). Mean age was of 74.3±11.6 years, males 73.4±11.0 and females 76.6±12.9, p=0.088. The median time since the diagnosis was 6.0 for men and 4.0 for women, p=0.031. The median time since the last echocardiogram was 3.0, p=0.750 between genders. Conclusion: According to the 2021 ESC guidelines, prognostic modifying medicines were underused for 50.4% of P-CHFrEF. Even with these results, a reflection must be made, on the barriers physicians encounter, to comply with the ESC guidelines in such patients.

RESUMO: O objetivo do presente trabalho foi o de criar uma ferramenta para mensurar imagens ultrassonográficas da área do olho de lombo em ovinos. Foram capturadas 121 imagens ultrassonográficas de ovinos, a partir das quais foram segmentadas regiões de interesse utilizando o algoritmo u-net. As métricas adotadas para avaliar as segmentações automáticas foram Dicescore e Interseção sobre União. Por fim, foi realizada análise de regressão utilizando os algoritmos AdaBoost Regressor e Random Forest Regressor e o ajuste dos modelos foi avaliado por meio de quadrado médio dos resíduos, erro absoluto médio e coeficiente de determinação (R²). Os valores obtidos para a métrica Dice foram 0,94, e para Interseção sobre União foi 0,89, demonstrando uma alta similaridade entre os valores reais e previstos, variando de 0 a 1. Os valores de quadrado médio dos resíduos, erro absoluto médio e coeficiente de determinação do modelo de regressores indicaram o melhor ajuste para o Random Forest Regressor. O algoritmo U-Net segmentou com eficiência as imagens ultrassonográficas do músculo Longissimus Dorsi, com maior precisão que as medidas realizadas pelo especialista. Essa eficiente segmentação permitiu a padronização das medidas de área de olho de lombo e, consequentemente, a fenotipagem de ovinos de corte em larga escala. ABSTRACT: The present research created a tool to measure ultrasound images of the rib eye area in sheep. One hundred twenty-one ultrasound images of sheep were captured, with regions of interest segmented using the U-Net algorithm. The metrics adopted to evaluate automatic segmentations were Dicescore and intersection over union. Finally, a regression analysis was performed using the AdaBoost Regressor and Random Forest Regressor algorithms and the fit of the models was evaluated using the Mean Square Residuals, mean absolute error and coefficient of determination. The values obtained for the Dice metric were 0.94, and for Intersection over Union it was 0.89, demonstrating a high similarity between the actual and predicted values, ranging from 0 to 1. RESUMO O câncer de pulmão (CP) é uma das neoplasias mais comuns e letais no Brasil, e apenas 15% dos pacientes são diagnosticados nos estágios iniciais. O tabagismo persiste como o responsável por mais de 85% de todos os casos. O rastreamento do CP (RCP) por meio da TC de baixa dosagem de radiação (TCBD) reduz a mortalidade do CP em 20%, e, quando combinado com a cessação do tabagismo, essa redução chega a 38%. Na última década, diversos países adotaram o RCP como recomendação de saúde populacional. No Brasil, embora ainda incipiente, a discussão sobre o tema é cada vez mais ampla e necessária. Com o intuito de aumentar o conhecimento e estimular o debate sobre o RCP, a Sociedade Brasileira de Cirurgia Torácica, a Sociedade Brasileira de Pneumologia e Tisiologia e o Colégio Brasileiro de Radiologia e Diagnóstico por Imagem constituíram um painel de especialistas para elaborar as recomendações para o RCP. As recomendações aqui apresentadas foram baseadas em revisão narrativa da literatura, com ênfase em grandes estudos populacionais, em revisões sistemáticas e em recomendações de diretrizes internacionais, sendo construídas após ampla discussão pelo grupo de especialistas. ABSTRACT Although lung cancer (LC) is one of the most common and lethal tumors, only 15% of patients are diagnosed at an early stage. Smoking is still responsible for more than 85% of cases. Lung cancer screening (LCS) with low-dose CT (LDCT) reduces LC-related mortality by 20%, and that reduction reaches 38% when LCS by LDCT is combined with smoking cessation. In the last decade, a number of countries have adopted population-based LCS as a public health recommendation. Albeit still incipient, discussion on this topic in Brazil is becoming increasingly broad and necessary. With the aim of increasing knowledge and stimulating debate on LCS, the Brazilian Society of Thoracic Surgery, the Brazilian Thoracic Association, and the Brazilian College of Radiology and Diagnostic Imaging convened a panel of experts to prepare recommendations for LCS in Brazil. The recommendations presented here were based on a narrative review of the literature, with an emphasis on large population-based studies, systematic reviews, and the recommendations of international guidelines, and were developed after extensive discussion by the panel of experts. The following topics were reviewed: reasons for screening; general considerations about smoking; epidemiology of LC; eligibility criteria; incidental findings; granulomatous lesions; probabilistic models; minimum requirements for LDCT; volumetric acquisition; risks of screening; minimum structure and role of the multidisciplinary team; practice according to the Lung CT Screening Reporting and Data System; costs versus benefits of screening; and future perspectives for LCS. ABSTRACT In 2000, Cleide Costa published a paper presenting the state of knowledge of the Neotropical Coleopte ra, with a focus on the Brazilian fauna. Twenty-four years later, thanks to the development of the Coleoptera section of the Taxonomic Catalog of the Brazilian Fauna (CTFB - Catálogo Taxonômico da Fauna do Brasil) through the collaboration of 100 coleopterists from all over the globe, we can build on Costa’s work and present an updated overview of the state of knowledge of the beetles from Brazil. There are currently 35,699 species in 4,958 genera and 116 families known to occur in the country, including representatives of all extant suborders and superfamilies. Our data show that the Brazilian beetle fauna is the richest on the planet, concentrating 9% of the world species diversity, with some estimates accounting to up to 15% of the global total. The most diverse family in numbers of genera is Cerambycidae (1,056 genera), while in number of species it is Chrysomelidae (6,079 species). Conotrachelus Dejean, 1835 (Curculionidae) is the most species-rich genus, with 570 species. The French entomologist Maurice Pic is the author who has contributed the most to the naming of species recorded from Brazil, with 1,794 valid names in 36 families, whereas the Brazilians Ubirajara R. Martins and Maria Helena M. Galileo are the only ones among the top-ten authors to have named species in the 21st century. Abstract This study explored the effects of L-PRF on pain, soft tissue healing, periodontal condition, and post-extraction bone repair of mandibular third molars (3Ms). A randomized, prospective, triple-blind, split-mouth clinical trial was conducted with 34 volunteers. Eligible patients were randomly allocated into two treatments: G1 (without L-PRF), G2 (alveoli filled with L-PRF), in which the removal of bilateral 3Ms was performed at the same surgical time. Outcomes were assessed according to a visual analogue scale (pain), soft tissue scoring system (wound healing), periodontal probing of mandibular second molar. Bone repair was determined by volumetric analysis (ITK-SNAP software) and fractal analysis (ImageJ software). An intention-to-treat approach to Statistical analysis was used. L-PRF reduced pain in the 7-day postoperative follow-up (p = 0.019) and not only improved soft tissue healing after 1 month of follow-up (p = 0.021), but also probing depth (distal face) in 3 months postoperatively (p = 0.011). Significant alveolar reduction occurred in 3 months after surgery in both treatments (p < 0.05), however, this was more significant in G1 (p = 0.016). The fractal dimension showed no statistical differences. L-PRF improved postoperative clinical parameters of pain, soft tissue healing, and periodontal condition, suggesting that it has a beneficial effect on preserving the alveolar ridge and accelerating the initial repair process. ABSTRACT This paper evaluates the mechanical behavior of nanocarbon modified asphalt binder. The nanocarbon was produced by a pyrolysis process of lignin. Three samples of asphalt binder were used as base for modification, a Pen 30/45, a Pen 50/70, and a SBS-polymer-modified 55/75. Different conditions of temperature, stirring speed, and modification time periods were evaluated to identify the best operational conditions. The characterization of nanocarbon was performed by different methodologies. Performance tests were conducted in a dynamic shear rheometer (DSR) to determine dynamic shear modulus, phase angle, and parameters of the Linear Amplitude Sweep test (LAS) and Multiple Stress Creep and Recovery test (MSCR) on the modified and unmodified asphalt samples. Furthermore, tests were performed in a bending beam rheometer (BBR) to measure the flexural creep stiffness and m-value at low temperatures, as well as the ∆Tc parameter. The samples were submitted to short- and long-term aging, and then master curves were constructed from frequency sweep dynamical shear tests. The results indicated that small amounts (0.02% to 0.05%, w/w) of nanocarbon adopted in the research were enough to improve rutting, fatigue, and aging resistance. Resumo Fundamento Não existem estudos randomizados comparando a manutenção do ritmo sinusal após ablação por cateter (AC) em relação ao tratamento com fármacos antiarrítmicos (AA) em pacientes idosos portadores fibrilação atrial (FA) paroxística. Objetivos Comparar os resultados clínicos do isolamento das veias pulmonares (VPs) com o cateter PVAC Gold de segunda geração com o uso de AA em idosos com FA paroxística sintomática, recorrente, apesar do uso de fármacos AA. Métodos Sessenta pacientes com FA paroxística ≥ 65 anos e sem cardiopatias estruturais foram randomizados para duas formas de tratamento: grupo 1: AC e grupo 2: AA. O desfecho primário foi a taxa livre de recorrência de FA após pelo menos um ano de seguimento. Os desfechos secundários foram: progressão para formas persistentes de FA, impacto na qualidade de vida (QVFA) e complicações. O nível de significância adotado na análise estatística foi de 5% (p<0,05). Resultados A taxa livre de recorrência de FA foi de 80% (10% com amiodarona) no grupo AC, após 1,3 procedimentos por paciente e de 65% no grupo AA (60% com amiodarona), (p = 0,119) num seguimento médio de 719 dias (Q1: 566; Q3: 730). A taxa livre de FA persistente foi de 83,4% no grupo AC e de 67,7% no grupo AA (p = 0,073). Ambas as estratégias apresentaram melhora no escore de QVFA durante o seguimento (p < 0,001), sem diferença entre os grupos. Embora sem repercussão clínica ou impacto no teste de avaliação intelectual, 25% dos pacientes do grupo PVAC apresentou sinais de embolização cerebral na RNM cerebral. Conclusões Ambas as estratégias para manutenção do ritmo sinusal promoveram melhora na qualidade de vida de pacientes idosos com FA sintomática, sem diferença estatística nos desfechos clínicos preconizados. Estudos adicionais usando tecnologias com melhor perfil de segurança são necessários para avaliar os benefícios da AC em pacientes idosos com FA. Abstract Background There are no randomized studies comparing the maintenance of sinus rhythm after catheter ablation (CA) concerning treatment with antiarrhythmic drugs (AA) in elderly patients with paroxysmal atrial fibrillation (AF). Objectives To compare the clinical results of pulmonary vein (PV) isolation with the second-generation PVAC Gold catheter against AA treatment in elderly people with recurrent symptomatic paroxysmal AF, refractory to at least one AA, and without structural heart disease. Methods Sixty patients with paroxysmal AF ≥ 65 years old were randomized to two forms of treatment: group 1: CA and group 2: AA drugs. The primary outcome was the AF recurrence-free rate after at least one year of follow-up. Secondary outcomes were: progression to persistent forms of AF, impact on quality of life (QOLF), and complications. The significance level adopted in the statistical analysis was 5% (p<0.05). Results The AF recurrence-free rate was 80% (10% with amiodarone) in the CA group, after 1.3 procedures per patient and 65% in the AA group (60% with amiodarone), (p = 0.119) in an average follow-up of 719 days (Q1: 566; Q3: 730). The persistent AF free rate was 83.4% in the AC group and 67.7% in the AA group (p = 0.073) Both strategies showed an improvement in the AFQoL score during follow-up (p < 0.001), with no difference between the groups. Conclusions Both strategies for maintaining sinus rhythm promoted an improvement in the quality of life of elderly patients with symptomatic AF, with no statistical difference in the clinical outcomes. ABSTRACT Background: Vectorial transmission through hematophagous triatomine insects remains the primary mode of Chagas Disease contagion. These insects have become increasingly common in urban environments. Therefore, this study aimed to report an encounter of triatomines with trypanosomatid infection in a vertical residential condominium in Rio Branco, the capital of the state of Acre, in the western Brazilian Amazon. Methods: Triatomines were collected from residents and sent to the municipality's Entomological Surveillance sector. Trypanosomatid positivity was evaluated using optical microscopy, followed by species and genotype identification using molecular biology techniques. Results: Twenty-five adult triatomine specimens were collected from two of three condominium buildings invading apartments from the 2nd to 13th floors. Six specimens were identified as Rhodnius sp. and 19 as R. montenegrensis. Among these, molecular tests were conducted on seven specimens, with five testing positive for Trypanosoma cruzi, all belonging to genotype TcI. Conclusions: These findings underscore the need for further studies to better understand the invasive capacity of these insects in these environments and the mechanisms involved in this process. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT Purpose: To develop and validate a trans oral endoscopic thyroidectomy vestibular approach (TOETVA) simulator. Methods: The first phase of the project consisted of designing and developing a transoral thyroid surgery simulator based on real surgeries. The product has the oral cavity for attaching the three trocars and the cervical part containing the thyroid and adjacent structures. In the second phase, the simulator was validated by specialists who performed an endoscopic thyroidectomy procedure. They all filled a questionnaire about the simulator and the simulation based on the Likert scale. Results: The simulator consists of a console similar to a human bust and a high-resolution camera system connected to a 22-inch monitor. The simulator had excellent results in the visual evaluation (face validity), with 100% of responses between good and excellent for the following characteristics: synthetic structures, design, visibility of the surgery field, resistance, resilience, fulcrum effect, ergonomics, surgical material, and practicality. The last three were rated higher, with more votes for excellent. For content validity, the items that received the best ratings were, precisely, the steps relating to the surgical procedure: opening the intermuscular midline, isthmotomy, and thyroidectomy. Conclusions: The thyroidectomy training box showed great ability to simulate a TOETVA, with satisfactory evaluations concerning its visual and content validation. RESUMO Este trabalho se insere no contexto de análise de imagens, visando a extração automática de informações complexas com alta precisão. Este estudo teve como objetivo avaliar o desempenho de redes neurais convolucionais na classificação de imagens de concreto em duas classes: (a) não fissuradas e (b) fissuradas. Para tanto, foram empregadas as redes profundas VGG16, VGG19 e ResNet50 com aprendizagem por transferência por meio de ajuste fino. As redes foram treinadas novamente e testadas usando um banco de dados de 40.000 imagens. Após o treinamento, as redes foram testadas, alcançando uma precisão impressionante entre 99,27% e 99,78%. Este alto nível de precisão inspira confiança no uso destes modelos preditivos. Para avaliar a robustez dos modelos, foram gerados gradientes visuais dos pontos focais de atenção das redes nas imagens, mostrando que os modelos focam e capturam aspectos das fotos que realmente caracterizam as fissuras. ABSTRACT This paper is inserted in the context of image analysis, aiming at the automatic extraction of complex information with high precision. This study aimed to evaluate the performance of convolutional neural networks in classifying concrete images into two classes: (a) non-cracked and (b) cracked. For this purpose, VGG16, VGG19, and ResNet50 deep networks were employed with transfer learning through fine-tuning. The networks were re-trained and tested using a database of 40,000 images. After training, the networks were tested, achieving an impressive accuracy between 99.27% and 99.78%. This high accuracy level inspires confidence in using these predictive models. To assess the robustness of the models, visual gradients of the networksʼ attention focal points on the images were generated, showing that the models focus on and capture aspects of the photos that truly characterize the cracks. Resumo A erosão hídrica é uma das principais causas da degradação das terras agricultáveis brasileiras, levando à diminuição da produtividade das culturas e à acidificação do solo, prejudicando sua capacidade de armazenar carbono, nutrientes e água. O manejo inadequado do solo é um dos principais responsáveis por esse tipo de degradação. A identificação da intensidade da erosão pode ser feita por meio da utilização de atributos indicadores que são altamente sensíveis as mudanças no ambiente edáfico. O estudo teve como objetivos: i) analisar os atributos físicos e químicos do solo em áreas de voçorocas com diferentes graus de degradação/estabilização; e ii) verificar a viabilidade de utilização desses atributos como indicadores de recuperação ambiental. O estudo foi realizado em quatro áreas com diferentes graus de formação de voçorocas: a) inicial, intermédia, madura e senil. As amostras foram coletadas nas faces externa e interna de cada voçoroca no final da estação seca. Entre os atributos físicos avaliados, a densidade do solo e a porosidade total foram os mais significativamente alterados. A avaliação dos microagregados permitiu conhecer a qualidade do solo através de frações como a argila dispersável em água, a argila refloculável em água e a argila não dispersível em água. O carbono total, o nitrogênio total e a relação C/N elucidaram a dinâmica da perda de solo e de nutrientes em diferentes fases dos processos de formação e estabilização da erosão, com valores mais baixos observados na face interna das voçorocas em comparação a externa. Observou-se que a vegetação influenciou os resultados dos atributos físicos e químicos. No geral, os valores tenderam ao equilíbrio entre as faces na voçoroca SE, indicando maior estabilização. Abstract Water erosion is one of the primary causes of agricultural soil degradation in Brazil, leading to diminished crop productivity and soil acidification, thereby impairing its ability to store carbon, nutrients, and water. Identifying the intensity of erosion can be achieved by utilizing indicator attributes that are highly sensitive to changes in the edaphic environment. The study analyzed the physical and chemical attributes of soil in areas with gullies exhibiting varying degrees of degradation/stabilization. The study was conducted across four areas with varying degrees of gully formation: a) initial, intermediate, mature, and senile. Samples were collected from both the external and internal sides of each gully at the end of the dry season. Among the physical attributes assessed, soil density and total porosity were found to be the most significantly altered. Evaluation of microaggregates provided insights into soil quality through fractions including water-dispersible clay, water-reflocculable clay, and non-water-dispersible clay. Total carbon, total nitrogen, and the C/N ratio elucidated the dynamics of soil and nutrient loss across different stages of erosion formation and stabilization processes, with lower values observed on the internal side of the gullies compared to the exterior side. Vegetation was observed to influence the results of the physical and chemical attributes. Overall, the values tended towards equilibrium between the faces at the senile stage, indicating greater stabilization.
