Abstract The aim of this paper is to investigate the extent of freedom to testify in the English legal system, with a view to assessing how recent court decisions have impacted on the testator's private autonomy to dispose of their assets after death. To this end, the deductive method is used, based on a literature review, analyzing legislation, doctrine, and case law. The hypothesis put forward is that the current English legislation, which makes it possible for the Judiciary to review the deceased's last will and testament without setting minimum objective parameters to guide the judge in cases where the will is contested by those entitled to apply for family provision, can cause great legal uncertainty for the courts, as well as representing a real affront to the principle of private autonomy. In the end, we conclude that it is necessary to establish minimum objective criteria in English inheritance law, which should be observed by the Judiciary for the purposes of minimum standardization of these decisions, thus aiming to promote the expected balance between the private autonomy of the testator and the protection of his/her dependents. This conclusion does not, in any way, contradict the legal system in force in England, insofar as, given the complexity of the legal situations that arise in contemporary times, there has been a real rapprochement between the common law and civil law systems, so that pure systems are no longer visible, and it is necessary to complement them in order to promote justice.

Resumo O presente trabalho tem por objetivo investigar a extensão da liberdade para testar no ordenamento jurídico inglês, visando aferir como as recentes decisões judiciais vêm impactando na autonomia privada do testador para dispor de seus bens para depois da morte. Para tanto, faz-se uso do método dedutivo, apoiado na revisão bibliográfica, com análise da legislação, da doutrina e da jurisprudência. A hipótese aventada é a de que a atual legislação inglesa, que possibilita a revisão das disposições de última vontade do falecido pelo Poder Judiciário sem a fixação de parâmetros objetivos mínimos para o direcionamento do julgador nos casos em que há contestação do testamento por parte dos legitimados para requererem uma provisão familiar, pode causar grande insegurança jurídica aos jurisdicionados, além de representar verdadeira afronta ao princípio da autonomia privada. Ao final, concluiu-se que é necessário o estabelecimento de critérios objetivos mínimos na Lei de herança inglesa, que deverão ser observados pelo Poder Judiciário para fins de mínima padronização dessas decisões, visando, assim, promover o esperado equilíbrio entre a autonomia privada do testador e a proteção de seus dependentes. A conclusão apresentada, em nada contraria o sistema jurídico vigente na Inglaterra, na medida em que, diante da complexidade das situações jurídicas que se apresentam na contemporaneidade, houve uma verdadeira aproximação entre os sistemas de common law e civil law, de maneira que não se vislumbram mais sistemas puros, sendo necessária uma complementação entre eles para fins de promoção de justiça.

ABSTRACT Objective: To measure the costs of preventive and therapeutic protocols of Photobiomodulation (PBM) for oral mucositis (OM) and their budgetary impact on Brazil’s Ministry of Health (BMH). Material and Methods: A partial economic analysis was performed to estimate the costs using a bottom-up approach from a social perspective. Monetary values were assigned in Brazilian reais (BRL). The costs of the preventive protocol were calculated for five, 30, and 33 consecutive PBM sessions, depending on the antineoplastic treatment instituted. The costs of the therapeutic protocol were calculated for 5 or 10 sessions. The annual financial and budgetary impact was calculated considering the groups of oncologic patients with a higher risk of development of OM, such as those with head and neck and hematological cancer and pediatric patients. Results: The cost of a PBM session was estimated at BRL 23.75. The financial impact of providing one preventive protocol per year for all oncologic patients would be BRL 14,282,680.00, 0.030% of the estimated budget for hospital and outpatient care of the BMH in 2022. The financial and budgetary impacts of providing one treatment for OM for all patients in one year would be BRL 2,225,630.31 (0.005%, most optimistic scenario) and BRL 4,451,355.63 (0.009%, most pessimistic scenario). Abstract Diabetes mellitus (DM) is a major cause of chronic kidney disease (CKD) and changes in lipoprotein. Literature data suggest that cholesterol associated with high-density lipoprotein (HDLc) plays a protective role against albuminuria and diabetic nephropathy. These factors are related with changes in serum apolipoproteins such Apo A-I. This work aims to evaluate the correlation of apolipoprotein A-I with renal function in diabetic outpatients. Samples were collected from 281 outpatients for analyses of glycated hemoglobin (HbA1c), serum creatinine (SCr), urea (BUN), cholesterol (TC), triglycerides (TG), HDL-cholesterol (HDLc) and estimation of glomerular filtration rate (eGFR). Urinary samples were obtained to assess urine albumin creatinine ratio (UACR). Triglyceride-glucose index (TyG) was calculated to estimate insulin resistance. The mean age was 61,5 ± 16,4 years. Most patients were female, Afro-Mestizo and over 50 years of age. Pearson's revealed negative correlation of creatinine with HDLc and Apo A-I. There was a correlation of eGFR with HbA1c, TG and HDLc. TyG presented a negative correlation with Apo A-I and HDLc. The highest quartiles of UACR presented the highest TyG and the lowest Apo A-I. In conclusion, Apo A-I indicated an association with insulin resistance and changes in renal function parameters, especially as a factor associated with the onset of albuminuria, with a better predictive potential than the HDLc. Resumo Valores do trabalho representam o que os indivíduos consideram importante no ambiente laboral, suas motivações, seus comportamentos e suas atitudes. Pesquisas sobre esse tema alcançam diversos grupos, inclusive os de economia criativa, cujos produtos e serviços refletem criatividade, talento e cultura local. Nesse cenário, o setor de cosmética natural artesanal, especificamente a saboaria, propõe uma produção pautada pela sustentabilidade social, ambiental e econômica. Assim, este artigo busca analisar como os valores do trabalho se manifestam entre as artesãs-empreendedoras do setor de cosmética natural. O lócus é Fortaleza, capital do Ceará, chancelada em 2019 como cidade criativa pela Organização das Nações Unidas para a Educação, a Ciência e a Cultura (Unesco). Na coleta de dados, foram realizadas entrevistas baseadas numa adaptação qualitativa da Escala Revisada de Valores Relativos ao Trabalho (EVT-R). Abstract Work values represent what individuals consider important in the work environment, such as motivation, behaviors, and attitudes. Research on this topic reaches different groups of workers, including those in the creative economy, whose products and services reflect creativity, talent, and local culture. In this scenario, the artisanal natural cosmetics sector, specifically soapmaking, proposes a production guided by social, environmental, and economic sustainability. Thus, this work aims to analyze how work values manifest among female entrepreneurs in the artisanal natural cosmetics sector. The locus is the Brazilian city Fortaleza, which, in 2019, was recognized by Unesco as a Creative City. Data was collected through interviews based on a qualitative adaptation of the Revised Work Values Scale. Content analysis was conducted using Atlas.ti software. The results show that professional fulfillment and social relations are the most present values, especially in terms of creative work and collaboration for the development of society. The existence of a collaborative network was demonstrated, and the lack of specific regulation was found to be the main obstacle to growth. ABSTRACT Hematopoietic stem cell transplant (HSCT) recipients are at -increased risk for severe COVID-19. The aim of this study was to evaluate the burden of COVID-19 in a cohort of HSCT recipients. This retrospective study evaluated a cohort of adult hospitalized HSCT recipients diagnosed with COVID-19 in two large hospitals in São Paulo, Brazil post-HSCT, from January 2020 to June 2022. The primary outcome was all-cause mortality. Of 49 cases, 63.2% were male with a median age of 47 years. Allogeneic-HSCT (51.2%) and autologous-HSCT (48.9%) patients were included. The median time from HSCT to COVID-19 diagnosis was 398 days (IQR: 1211-134), with 22 (44.8%) cases occurring within 12 months of transplantation. Most cases occurred during the first year of the pandemic, in non-vaccinated patients (n=35; 71.4%). Most patients developed severe (24.4%) or critical (40.8%) disease; 67.3% received some medication for COVID-19, primarily corticosteroids (53.0%). The probable invasive aspergillosis prevalence was 10.2%. All-cause mortality was 40.8%, 51.4% in non-vaccinated patients and 14.2% in patients who received at least one dose of the vaccine. In the multiple regression analyses, the variables mechanical ventilation (OR: 101.01; 95% CI: 8.205 – 1,242.93; p = 0.003) and chest CT involvement at diagnosis ≥50% (OR: 26.61; 95% CI: 1.06 – 664.26; p = 0.04) remained associated with all-cause mortality. RESUMO Contexto: Nas últimas décadas, a ecoendoscopia evoluiu de uma ferramenta diagnóstica para uma plataforma para diversas intervenções terapêuticas. Vários avanços tecnológicos surgiram desde o último Consenso Brasileiro, demandando uma revisão e atualização das recomendações baseadas nas melhores evidências científicas. Métodos: Um grupo de 32 ecoendoscopistas renomados selecionou oito tópicos relevantes para serem discutidos a fim de gerar questões clínicas. Em seguida, foi realizada uma revisão da literatura para responder a essas perguntas com base nas evidências mais atualizadas. Resultados: Trinta e três tópicos foram formulados e votados pelos especialistas para alcançar um consenso. O Sistema de Oxford foi utilizado para classificar o nível de evidência. ABSTRACT Background: In the past decades, endoscopic ultrasound has developed from a diagnostic tool to a platform for many therapeutic interventions. Various technological advancements have emerged since the last Brazilian Consensus, demanding a review and update of the recommendations based on the best scientific evidence. Methods: A group of 32 renowned echoendoscopists selected eight relevant topics to be discussed to generate clinical questions. After that, a literature review was conducted to answer these questions based on the most updated evidence. Results: Thirty-three statements were formulated and voted on by the experts to reach a consensus. The Oxford System was used to grade the level of evidence. Abstract This study explored the effects of L-PRF on pain, soft tissue healing, periodontal condition, and post-extraction bone repair of mandibular third molars (3Ms). A randomized, prospective, triple-blind, split-mouth clinical trial was conducted with 34 volunteers. Eligible patients were randomly allocated into two treatments: G1 (without L-PRF), G2 (alveoli filled with L-PRF), in which the removal of bilateral 3Ms was performed at the same surgical time. Outcomes were assessed according to a visual analogue scale (pain), soft tissue scoring system (wound healing), periodontal probing of mandibular second molar. Bone repair was determined by volumetric analysis (ITK-SNAP software) and fractal analysis (ImageJ software). An intention-to-treat approach to Statistical analysis was used. L-PRF reduced pain in the 7-day postoperative follow-up (p = 0.019) and not only improved soft tissue healing after 1 month of follow-up (p = 0.021), but also probing depth (distal face) in 3 months postoperatively (p = 0.011). Significant alveolar reduction occurred in 3 months after surgery in both treatments (p < 0.05), however, this was more significant in G1 (p = 0.016). The fractal dimension showed no statistical differences. RESUMO Há suficientes evidências de que os resultados de um programa de restauração dependem em grande parte do contexto da paisagem. Os usos e coberturas prévios da terra e a paisagem devem ser considerados nos projetos de restauração, pois influenciam todo o processo, especialmente na dispersão de sementes e na regeneração natural das áreas. Neste trabalho verificou-se o efeito da paisagem (uso do solo) no sucesso da restauração em áreas de plantio total, semeadura direta e de regeneração natural no bioma Mata Atlântica, sudeste do Brasil. A metodologia baseou-se em unidades demonstrativas de restauração, com foco nos indicadores de riqueza e abundância de regenerantes, juntamente com dados de uso e cobertura da terra das áreas entre os anos de 2010, 2015 e 2020, obtidos das Coleções MapBiomas. Aplicou-se modelos lineares generalizados e análise de correlação entre os dados. Nossa hipótese era que uma paisagem com mais cobertura florestal e de regeneração natural (mosaico de agricultura e pastagem) afetariam positivamente os regenerantes em locais restaurados, o que observamos apenas em áreas de tratamento de regeneração natural. ABSTRACT There is good evidence that the results of a restoration program depend largely on the landscape context. In restoration projects, it is crucial to consider the previous land uses and covers and landscape configuration as they have a significant impact on the entire process, especially seed dispersal and natural regeneration of sites. Thus, the objective of this work was to verify the effect of landscape (i.e., land use) on restoration success of total planting, seed sowing and natural regeneration sites in the Atlantic Forest biome, southeast Brazil. The methodology employed was based on demonstration units of restoration. The focus was on regenerant richness and abundance indicators, along with land use and land cover data (class area) from 2010, 2015, and 2020, obtained from the MapBiomas Project. Generalized linear models and correlation analysis were used for the study. We hypothesize that a landscape with more forest and natural regeneration cover (mosaic of agriculture and pasture) would positively affect regenerants in restored sites, which we only observed for natural regeneration treatment sites. In active restoration treatment sites, mosaic of agriculture and pasture was negatively and farming was positively associated to regenerants. We also found greater density and richness of regenerants in natural regeneration treatment sites than in seed sowing and seedling planting. The influence of the surrounding landscape, particularly mosaic, played a crucial role in this success. In addition, land use cover history, as we observed in classes in the last five and ten years, did explain regenerants in studied sites. Serum levels of total cholesterol (TC), high-density lipoprotein cholesterol (HDL-c), low-density lipoprotein cholesterol (LDL-c), non-high-density lipoprotein cholesterol (non-HDL-c), triglycerides (TG), apolipoproteins (Apo A-I and B), and vascular cell adhesion molecule-1 (VCAM-1) were determined. COVID-19 outpatients showed increases in TC (187.0±48.3 vs . 160.8±32.0 mg/dL), LDL-c (110.3±41.5 vs . 98.8±28.2 mg/dL), non-HDL-c (138.3±45.8 vs . 115.6±31.6 mg/dL), TG (139.7±80.3 vs . 84.9±35.1 mg/dL), and Apo A-I (149.5±40.0 vs . 133.3±20.9 mg/dL), along with a high frequency of hypercholesterolemia and hypertriglyceridemia. VCAM-1 levels were double those of the control (682.7±231.8 vs. 299.3±102.9 ng/mL) and a strong predictor of COVID-19 (AUC=0.946). Moreover, VCAM-1 correlated with TC (r = -0.223) and HDL-c (r = -0.225). The decrease in TG and VCAM-1 is predicted by the dominance of the T allele (rs670) and its codominance with the A (rs693) allele in COVID-19 outpatients. Abstract Introduction Low-Level Red-Light (LLRL) Therapy is a safe and natural way to promote healing and reduce inflammation in the body. When it comes to treating myopia in children, LLRL therapy is recent, and its efficacy and safety still are not clear. Methods A systematic review and meta-analysis of the literature for LLRL was conducted in accordance with the PRISMA guidelines on November 5, 2022. Databases, including PUBMED, Cochrane Library, Web of Science, and Embase were queried. A meta-analysis of random effects was conducted. Inclusion criteria included Randomized Controlled Trials (RCTs) or observational studies where LLRL therapy was used in children (3‒15 years old) with myopia. Exclusion criteria were studies with other ocular abnormalities. Efficacy was evaluated through the mean change in Axial Length (AL) and cycloplegic Spherical Equivalent Error (SER), while safety was evaluated by monitoring adverse effects. Results A total of 5 final studies were included (4 RCTs, and 1 observational), in which 685 total patients were analyzed. The mean age was 9.7 ± 0.66 years, with 48,2% female patients. The number of eyes in the LRLL arm is 714 and, in the control, arm is 656. LLRL showed better results in SER and AL mean change (OR = 0.58; 95% CI 0.33 to 0.83; p < 0.00001, and MD -0.33; 95% CI -0.52 to -0.13; p = 0.001, respectively), in comparison to the control group. There was no significant difference in adverse effects between groups (MD = 5.76; 95% CI 0.66 to 50.14; p = 0.11). Conclusion LLRL therapy is a non-invasive, effective, and safe short-term treatment option; however, long-term evaluation, particularly in comparison to other therapies, requires additional investigation. Resumo Objetivos Este estudo avaliou a intensidade da dor em idosos acometidos por fratura do quadril internados no setor de emergência e submetidos ao Pericapsular Nerve Group (PENG) block no pré-operatório. Ademais, o grau de flexão tolerável do quadril foi avaliado. Métodos Ensaio clínico, prospectivo, aleatorizado e controlado em grupos paralelos. O grupo controle consiste em idosos com fratura do quadril, submetidos à analgesia sistêmica endovenosa padronizada. O grupo intervenção consiste em idosos com fratura do quadril submetidos ao PENG block e analgesia sistêmica padronizada. Os grupos foram avaliados em repouso e durante o movimento pela escala de dor Pain Assessment in Advance Dementia (PAINAD). Aferiram-se intensidade da dor e redução álgica, assim como o grau de flexão tolerável do quadril fraturado. Todos os pacientes foram avaliados previamente à administração de medicação ou bloqueio e aos 45 minutos, 12, 24 e 36 horas pós-medicação ou bloqueio. Resultados No pré-operatório e 24 horas após o PENG block, idosos com fratura do quadril apresentaram redução significativa da dor em repouso ou movimento em comparação com o controle (p< 0,05), com 60% dos pacientes avaliados em repouso, demonstrando a redução álgica desejável de ≥ 50% e apenas 13,3% do grupo controle com redução álgica desejável. Durante o movimento, após o PENG block, 40% demonstraram redução álgica desejada e nenhum paciente do grupo controle apresentou a redução desejada. Abstract Objectives This study evaluated pain intensity in elderly subjects with hip fractures admitted to the emergency sector and undergoing preoperative pericapsular nerve group (PENG) block. Additionally, the degree of tolerable hip flexion was assessed. Methods A prospective, randomized, and controlled clinical trial with parallel groups. The control group consisted of elderly subjects with hip fractures undergoing standardized intravenous systemic analgesia. The intervention group consisted of elderly patients with hip fractures undergoing PENG block and standardized systemic analgesia. The groups were evaluated at rest and during movement using the Pain Assessment in Advanced Dementia (PAINAD) scale. We determined pain intensity and reduction, in addition to the degree of tolerable flexion of the fractured hip. All patient assessments occurred before the medication or block administration and at 45 minutes, 12, 24, and 36 hours postmedication or block. Results Preoperatively and 24 hours after PENG block, elderly subjects with hip fracture showed a significant reduction in pain at rest or movement compared to control patients (p< 0.05), with 60% of patients assessed at rest demonstrating desirable pain reduction (≥50%) and only 13.3% of the control group achieving the desired pain reduction. During movement, after undergoing PENG block, 40% of subjects demonstrated the desired pain reduction and no patient from the control group. The intervention group also showed a significant improvement in the tolerable hip flexion group (p< 0.05). ABSTRACT Objective To evaluate deaths, hospitalizations, and persistence of symptoms in patients with COVID-19 after infection in an outpatient setting during the first COVID-19 wave in Brazil. Methods This prospective cohort was between April 2020 and February 2021. Hospitalized or non-hospitalized COVID-19 patients until five days after symptom onset were included. The outcomes measured were incidence of death, hospitalization, and persistence of more than two symptoms 60 days after discharge. Results Out of 1,198 patients enrolled in the study, 66.7% were hospitalized. A total of 289 patients died (1 [0.3%] non-hospitalized and 288 [36%] hospitalized). At 60 days, patients non-hospitalized during admission had more persistent symptoms (16.2%) compared to hospitalized (37.1%). The COVID-19 severity variables associated with the persistence of two or more symptoms were increased age (OR= 1.03; p=0.015), respiratory rate at hospital admission (OR= 1.11; p=0.005), length of hospital stay of more than 60 days (OR= 12.24; p=0.026), and need for intensive care unit admission (OR= 2.04; p=0.038). Conclusion COVID-19 survivors who were older, tachypneic at admission, had a hospital length of stay >60 days, and were admitted to the intensive care unit had more persistent symptoms than patients who did not require hospitalization in the early COVID-19 waves.
