Resumo Fundamento Dados robustos sobre a curva de aprendizagem (LC) da substituição da válvula aórtica transcateter (TAVR) são escassos nos países em desenvolvimento. Objetivo Avaliar a LC da TAVR no Brasil ao longo do tempo. Métodos Analisamos dados do registro brasileiro de TAVR de 2008 a 2023. Pacientes de cada centro foram numerados cronologicamente em número sequencial de caso (NSC). A LC foi realizada usando um spline cúbico restrito ajustado para o EuroSCORE-II e o uso de próteses de nova geração. Ainda, os desfechos hospitalares foram comparados entre grupos definidos de acordo com o nível de experiência, com base no NSC: 1º ao 40º caso (experiência inicial), 41º ao 80º caso (experiência básica), 81º ao 120º caso (experiência intermediária) e 121º caso em diante (experiência alta). Análises adicionais foram conduzidas de acordo com o número de casos tratados antes de 2014 (>40 e ≤40 procedimentos). O nível de significância adotado foi p <0,05. Resultados Foram incluídos 3194 pacientes de 25 centros. A idade média foi 80,7±8,1 anos e o EuroSCORE II médio foi 7±7,1. A análise da LC demonstrou uma queda na mortalidade hospitalar ajustada após o tratamento de 40 pacientes. Um patamar de nivelamento na curva foi observado após o caso 118. A mortalidade hospitalar entre os grupos foi 8,6%, 7,7%, 5,9%, e 3,7% para experiência inicial, básica, intermediária e alta, respectivamente (p<0,001). A experiência alta foi preditora independente de mortalidade mais baixa (OR 0,57, p=0,013 vs. experiência inicial). Centros com baixo volume de casos antes de 2014 não mostraram uma redução significativa na probabilidade de morte com o ganho de experiência, enquanto centros com alto volume de casos antes de 2014 apresentaram uma melhora contínua após o caso de número 10. Conclusão Observou-se um fenômeno de LC para a mortalidade hospitalar do TAVR no Brasil. Abstract Background Robust data on the learning curve (LC) of transcatheter aortic valve replacement (TAVR) are lacking in developing countries. Objective To assess TAVR’s LC in Brazil over time. Methods We analyzed data from the Brazilian TAVR registry from 2008 to 2023. Patients from each center were numbered chronologically in case sequence numbers (CSNs). LC was performed using restricted cubic splines adjusted for EuroSCORE-II and the use of new-generation prostheses. Also, in-hospital outcomes were compared between groups defined according to the level of experience based on the CSN: 1st to 40th (initial-experience), 41st to 80th (early-experience), 81st to 120th (intermediate-experience), and over 121st (high-experience). Additional analysis was performed grouping hospitals according to the number of cases treated before 2014 (>40 and ≤40 procedures). The level of significance adopted was <0.05. Results A total of 3,194 patients from 25 centers were included. Mean age and EuroSCORE II were 80.7±8.1 years and 7±7.1, respectively. LC analysis demonstrated a drop in adjusted in-hospital mortality after treating 40 patients. A leveling off of the curve was observed after case #118. In-hospital mortality across the groups was 8.6%, 7.7%, 5.9%, and 3.7% for initial-, early-, intermediate-, and high-experience, respectively (p<0.001). High experience independently predicted lower mortality (OR 0.57, p=0.013 vs. initial experience). Low-volume centers before 2014 showed no significant decrease in the likelihood of death with gained experience, whereas high-volume centers had a continuous improvement after case #10. Conclusion A TAVR LC phenomenon was observed for in-hospital mortality in Brazil. Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

Resumo A dinâmica da serapilheira é um dos processos fundamentais para o crescimento e manutenção dos fragmentos florestais nativos, sendo considerada a principal via de ciclagem de nutrientes nas florestas. Portanto, estudos sobre a serapilheira acumulada e conteúdo de nutrientes subsidiam informações para uma melhor compreensão da dinâmica dos nutrientes. O objetivo do estudo foi avaliar o estoque de serapilheira e nutrientes em diferentes estações do ano e a taxa instantânea de decomposição em um fragmento de Floresta Atlântica ao longo de dois anos. A amostragem da serapilheira foi realizada em 12 parcelas permanentes com dimensões de 20 m x 50 m. Determinou-se a massa seca da serapilheira e o teor dos nutrientes. O acúmulo médio anual de serapilheira foi de 5.269 kg ha-1. Não houve diferença estatística na quantidade de serapilheira entre as estações do ano. Houve diferença estatística nos teores de N, P, K Ca e Mg entre as diferentes estações do ano. Os estoques de nutrientes foram de 123.3, 92.8, 13.2, 11.8, 9.6 e 3.0 kg ha-1 para Ca, N, Mg, S, K e P, respectivamente. O total de nutrientes na serapilheira acumulada foi de 253,7 kg ha-1 ano-1. O tempo de renovação da serapilheira foi de 281 dias. O tempo para a decomposição de 50% e 95% da mesma foi de 196 e 850 dias, respectivamente. A produção média de serapilheira, o estoque de nutrientes e a decomposição estão de acordo com outros estudos realizados, indicando que o fragmento de Floresta Atlântica sazonal semidecidual apresenta indicadores de ciclagem de nutrientes. Esses resultados encontrados mostram que o ecossistema se encontra sustentável sob ponto de vista da ciclagem e liberação dos nutrientes.

Abstract Litter dynamics is one of the fundamental processes for the growth and maintenance of native forest fragments, being considered the main pathway for nutrient cycling in forests. Therefore, studies on accumulated litter and nutrient content provide information for a better understanding of nutrient dynamics. The aim of the study was to evaluate leaf litter and nutrient stock in different seasons and the instantaneous rate of decomposition in an Atlantic Forest Fragment over two years. Litter sampling was carried out in 12 permanent plots with dimensions of 20 m x 50 m. Litter dry mass and nutrient concentration were determined. The average annual accumulation of litter was 5269 kg ha-1. There was no statistically significant difference in the amount of litter between seasons. There was a statistically significant difference in the contents of N, P, K Ca and Mg between the different seasons. Nutrient stocks were 123.3, 92.8, 13.2, 11.8, 9.6, 3.0 kg ha-1 year-1 for Ca, N, Mg, S, K and P respectively. The total of nutrients in the accumulated litter was 253.7 kg ha-1 year-1. The litter renewal time was 281 days. The times required for 50% and 95% litter decomposition were 196 and 850 days. The average litter stocks, nutrients and decomposition are in line with other studies, indicating that the Atlantic Forest fragment seasonal semi-deciduous presents indicators of nutrient cycling. These results show that the ecosystem is sustainable from the point of view of cycling and nutrient release.

SUMMARY OBJECTIVE: This study aims to analyze the risk factors for in-hospital mortality in a cohort of patients admitted to a newly adapted intensive care unit in a public hospital in Rio de Janeiro. METHODS: This was an observational, retrospective, and descriptive study. Data were obtained from electronic medical records. Coronavirus disease 2019 (COVID-19) was diagnosed by detecting viral ribonucleic acid using reverse transcription polymerase chain reaction. Factors associated with the risk/protection from death were determined using the odds ratio and adjusted odds ratio. RESULTS: Fifty-one patients were admitted to the hospital. The median age of the patients was 63 years, 60% were male patients, and 54% were white patients. Sixty-seven percent of the patients were diagnosed with COVID-19. Sepsis at admission increased the chance of in-hospital death by 21 times (adjusted odds ratio=21.06 [0.79–555.2]; p=0.06). The strongest risk factor for death was the development of septic shock during hospitalization (adjusted odds ratio=98.56 [2.75–352.5]; p=0.01), and one in four patients had multidrug-resistant bacteria. Mechanical ventilation, vasopressors, neuromuscular blockers, and sedatives were also the risk factors for in-hospital mortality. The in-hospital mortality rate was 41%, and the mortality rate of patients on mechanical ventilation was 60%. The diagnosis of COVID-19 was not statistically related to the adverse outcomes. CONCLUSIONS: In this cohort, the strongest risk factor for in-hospital death was the development of nosocomial septic shock. Healthcare-associated infections have a significant impact on mortality rates. Therefore, to have a better outcome, it is important to consider not only the availability of beds but also the way healthcare is delivered.

Resumo As lavanderias de Jeans geram grandes vazões de efluentes com elevada carga orgânica, cor e outros poluentes, o que dificulta sua adequação para lançamentos dentro dos limites legais. Filtros lentos de areia (FLA) com fluxo descendente foram testados durante 7 dias, em escala de bancada, a fim de propor um pós-tratamento de efluentes de uma Estação de Tratamento de Efluentes de lavanderia de jeans. Nesta pesquisa, foi avaliada a remoção dos seguintes parâmetros: cor, turbidez, demanda química e bioquímica de oxigênio, condutividade, amônia, fósforo total e salinidade. O experimento contou com quatro FLA: o primeiro alimentado com água destilada e os outros três alimentados com o efluente têxtil (triplicatas). Os FLA removeram 91% de amônia, 61,24% de cor, 89,43% de turbidez e 83,54% de fósforo. Quanto à remoção de matéria orgânica, em média houve remoção de 98,11% para DBO e 81,17% para DQO, demonstrando que os FLA foram eficientes na remoção de materiais particulados e dissolvidos.

Abstract Jean laundering generates significant effluent flows with a high organic load, color, and other pollutants, making it difficult to adjust effluent releases within legal limits. Slow Sand Filters (SSF) with downflow were tested for seven days (bench scale), to propose an after-treatment of effluents from an Effluent Treatment Plant of a jean laundry. The research evaluated the removal of the following parameters: color, turbidity, chemical and biochemical oxygen demand, conductivity, ammonia, total phosphorus and salinity of the textile effluent. The experimental apparatus had four SSF: the first filter was fed with distilled water, while the other three filters (triplicates) were fed with effluent. The filters had, on average, the following removal efficiencies: 91% for ammonia, 61.24% color, 89.43% turbidity, and 83.54% for phosphorus. Regarding the removal of organic matter, 98.11% for BOD and 81.17% for COD, demonstrating that SSFs were efficient in removing particulate, dissolved materials and organic matter.

BACKGROUND Biomphalaria glabrata is the major species used for the study of schistosomiasis-related parasite-host relationships, and understanding its gene regulation may aid in this endeavor. The ubiquitin-proteasome system (UPS) performs post-translational regulation in order to maintain cellular protein homeostasis and is related to several mechanisms, including immune responses. OBJECTIVE The aims of this work were to identify and characterise the putative genes and proteins involved in UPS using bioinformatic tools and also their expression on different tissues of B. glabrata. METHODS The putative genes and proteins of UPS in B. glabrata were predicted using BLASTp and as queries reference proteins from model organism. We characterised these putative proteins using PFAM and CDD software describing the conserved domains and active sites. The phylogenetic analysis was performed using ClustalX2 and MEGA5.2. Expression evaluation was performed from 12 snail tissues using RPKM. FINDINGS 119 sequences involved in the UPS in B. glabrata were identified, which 86 have been related to the ubiquitination pathway and 33 to proteasome. In addition, the conserved domains found were associated with the ubiquitin family, UQ_con, HECT, U-box and proteasome. The main active sites were lysine and cysteine residues. Lysines are responsible and the starting point for the formation of polyubiquitin chains, while the cysteine residues of the enzymes are responsible for binding to ubiquitin. The phylogenetic analysis showed an organised distribution between the organisms and the clades of the sequences, corresponding to the tree of life of the animals, for all groups of sequences analysed. The ubiquitin sequence was the only one with a high expression profile found in all libraries, inferring its wide range of performance. MAIN CONCLUSIONS Our results show the presence, conservation and expression profile of the UPS in this mollusk, providing a basis and new knowledge for other studies involving this system. Due to the importance of the UPS and B. glabrata, this work may influence the search for new methodologies for the control of schistosomiasis.

Although cryptococcal infections begin in the lungs, meningoencephalitis is the most frequently encountered manifestation of cryptococcosis among individuals with advanced immunosuppression. As the infection progresses along the Virchow-Robin spaces, these structures may become dilated with mucoid material produced by the capsule of the organism. We report a case of a 24-year-old man with cryptococcal meningoencephalitis in which magnetic resonance imaging showed clusters of gelatinous pseudocysts in the periventricular white matter, basal ganglia, mammillary bodies, midbrain peduncles and nucleus dentatus with a soap bubble appearance.

A peptide (SmB2LJ; r175-194) that belongs to a conserved domain from Schistosoma mansoni SmATPDase 2 and is shared with potato apyrase, as predicted by in silico analysis as antigenic, was synthesised and its immunostimulatory property was analysed. When inoculated in BALB/c mice, this peptide induced high levels of SmB2LJ-specific IgG1 and IgG2a subtypes, as detected by enzyme linked immunosorbent assay. In addition, dot blots were found to be positive for immune sera against potato apyrase and SmB2LJ. These results suggest that the conserved domain r175-194 from the S. mansoni SmATPDase 2 is antigenic. Western blots were performed and the anti-SmB2LJ antibody recognised in adult worm (soluble worm antigen preparation) or soluble egg antigen antigenic preparations two bands of approximately 63 and 55 kDa, molecular masses similar to those predicted for adult worm SmATPDase 2. This finding strongly suggests the expression of this same isoform in S. mansoni eggs. To assess localisation of SmATPDase 2, confocal fluorescence microscopy was performed using cryostat sections of infected mouse liver and polyclonal antiserum against SmB2LJ. Positive reactions were identified on the external surface from the miracidium in von Lichtenberg's envelope and, in the outer side of the egg-shell, showing that this soluble isoform is secreted from the S. mansoni eggs.
