Occurrence of Cryptosporidium oocysts and Giardia cysts in public water supplies in Vitória, ES, Brazil Vitória ES

RESUMO Este estudo teve como objetivo investigar a ocorrência de oocistos de Cryptosporidium e cistos de Giardia em águas brutas, filtradas e cloradas, coletadas de duas estações de tratamento de água potável (ETA A e ETA B). A ETA A utiliza filtração direta ou flotação-filtração dependendo da turbidez da água bruta. A ETA B possui duas linhas de tratamento independentes, uma de filtração direta e outra de tratamento convencional. Os oocistos de Cryptosporidium e cistos de Giardia foram concentrados por floculação, identificados por microscopia de imunofluorescência direta e confirmados por coloração com DAPI e microscopia de contraste de fase. Na água bruta, a ocorrência de cistos variou de 75% (ETA A) a 100% (ETA B) das amostras, e de oocistos de 66,6% (ETA A) a 83,3% (ETA B). Ambos os protozoários foram detectados na água tratada por filtração direta (cistos: ETA A e B < 0,27 a 20,0 cistos L-1; oocistos: < 0,48 a 22,5 oocistos L-1) e flotação-filtração (cistos: < 0,27 a 5,0 cistos L-1; oocistos: < 0,48 a 17,5 oocistos L-1) na ETA A. A ausência de cistos e oocistos na água clorada não exclui riscos, pois as limitações das técnicas de concentração e identificação devem ser consideradas, dados os baixos índices de recuperação, especialmente em água com baixa turbidez (15,5 – 72,7% de Giardia; 3,6 – 38,5% de Cryptosporidium). Nas amostras de água bruta da ETA A, foi observada uma correlação moderada entre os protozoários e destes com os parâmetros convencionais de qualidade da água. Nas amostras de água bruta da ETA B, a correlação foi insignificante. ABSTRACT This study aimed to investigate the occurrence of Cryptosporidium oocysts and Giardia cysts in raw, filtered, and chlorinated waters collected from two drinking water treatment plants (WTP A and WTP B). WTP A uses either direct filtration or flotation-filtration depending on the turbidity of raw water. WTP B has two independent treatment lines, a direct filtration and a conventional treatment line. Cryptosporidium oocysts and Giardia cysts were concentrated by flocculation, identified by direct immunofluorescence microscopy and confirmed by DAPI staining and phase-contrast microscopy. In raw water, the occurrence of cysts was from 75 (WTP A) to 100% (WTP B) of the samples, and of oocysts from 66.6 (WTP A) to 83.3% (WTP B). Both protozoa were detected in water treated by direct filtration (cysts: < 0.27 to 20.0 cysts L-1; oocysts: < 0.48 to 22.5 oocysts L-1) and flotation-filtration (cysts: < 0.27 to 5.0 cysts L-1; oocysts: < 0.48 to 17.5 oocysts L-1). The absence of cysts and oocysts in chlorinated water does not exclude risks, as the limitations of concentration and identification techniques must be considered, given the low recovery rates, especially in water with low turbidity (15.5 – 72.7% of Giardia; 3.6 – 38.5% of Cryptosporidium). In the raw water samples from WTP A, a moderate correlation was observed between the protozoa, and these with the conventional parameters of water quality. In the raw water samples from WTP B, the correlation was insignificant. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT The genus Bartonella encompasses 38 validated species of Gram-negative, facultative intracellular bacteria that colonize the endothelial cells and erythrocytes of a wide spectrum of mammals. To date, 12 Bartonella species have been recorded infecting humans, causing diseases of long historical characterization, such as cat scratch fever and trench fever, and emerging bartonellosis that mainly affect animal health professionals. For this reason, this study aimed to report a documented case of Bartonella bovis infecting a veterinarian from Mexico by the amplification, sequencing and phylogenetic reconstruction of the citrate synthase (gltA) and the RNA polymerase beta-subunit (rpoB) genes, and to report the natural course of this infection. Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

Abstract Objective: Women with ST-segment elevation myocardial infarction (STEMI) have worst outcomes than men. The objective of the study was to determine gender differences in mortality in patients with STEMI. Methods: Cohort study including patients with STEMI. We recorded demographic and clinical data, laboratory tests, and in-hospital mortality in patients who underwent primary angioplasty and pharmacoinvasive strategy. Kaplan-Meier analysis was used to assess mortality differences between both genders. Results: A total of 340 patients were analyzed, 296 males and 44 females. Mean age of the female group was 64.3 ± 12.3 years. About 98% of females were among Killip-Kimball Class I-II. They had higher risk scores compared to man, longer ischemic time and first medical contact with a difference in comparison to man of 47 and 60 min, respectively. Mortality was 9.1% (4) in the female group. Conclusions: Although the proportion of women had higher mortality than man, we did not found any difference with statistical significance probably due to the lack of representation. We need more awareness in the female population about STEMI, since longer first medical contact time and longer total ischemic time might be one possible explanation of a higher mortality.
Resumen Objetivo: Las mujeres con infarto agudo al miocardio con elevación del segmento ST (SICA CEST) tienen peor pronóstico que los hombres. El objetivo fue determinar las diferencias de mortalidad en género en pacientes con SICA CEST. Metodos: Estudio de cohorte de pacientes con SICA CEST. Se recolectaron datos demográficos, clínicos, de laboratorio y mortalidad intrahospitalaria. Se realizó un análisis de Kaplan-Meier para valorar la mortalidad y determinar diferencias de género. Resultados: Se analizaron 340 pacientes, 296 hombres y 44 mujeres. Se observó que las mujeres tuvieron mayores puntajes en los scores de riesgo, mayor tiempo de primer contacto y tiempo total de isquemia encontrando una diferencia con el grupo de hombres de 47 y 60 minutos respectivamente. La mortalidad intrahospitalaria fue de 9.1% (4) sin diferencia estadísticamente significativa en comparación con los hombres. Conclusiones: Aunque la proporción de mujeres tuvo mayor mortalidad no se encontró diferencia estadísticamente significativa en comparación con los hombres probablemente por el tamaño de la muestra debido a baja representación del grupo femenino. Se necesita mayor conciencia en relación al infarto en el grupo de mujeres, ya que mayor tiempo de primer contacto y mayor tiempo total de isquemia pueden ser una posible explicación de una mayor mortalidad.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.

Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract Background Insects can be found in numerous diverse environments, being exposed to pathogenic organisms like fungi and bacteria. Once these pathogens cross insect physical barriers, the innate immune system operates through cellular and humoral responses. Antimicrobial peptides are small molecules produced by immune signaling cascades that develop an important and generalist role in insect defenses against a variety of microorganisms. In the present work, a cecropin B-like peptide (AgCecropB) sequence was identified in the velvetbean caterpillar Anticarsia gemmatalis and cloned in a bacterial plasmid vector for further heterologous expression and antimicrobial tests. Methods AgCecropB sequence (without the signal peptide) was cloned in the plasmid vector pET-M30-MBP and expressed in the Escherichia coli BL21(DE3) expression host. Expression was induced with IPTG and a recombinant peptide was purified using two affinity chromatography steps with Histrap column. The purified peptide was submitted to high-resolution mass spectrometry (HRMS) and structural analyses. Antimicrobial tests were performed using gram-positive (Bacillus thuringiensis) and gram-negative (Burkholderia kururiensis and E. coli) bacteria. Results AgCecropB was expressed in E. coli BL21 (DE3) at 28°C with IPTG 0.5 mM. The recombinant peptide was purified and enriched after purification steps. HRMS confirmed AgCrecropB molecular mass (4.6 kDa) and circular dichroism assay showed α-helix structure in the presence of SDS. AgCrecropB inhibited almost 50% of gram-positive B. thuringiensis bacteria growth. Conclusions The first cecropin B-like peptide was described in A. gemmatalis and a recombinant peptide was expressed using a bacterial platform. Data confirmed tertiary structure as predicted for the cecropin peptide family. AgCecropB was capable to inhibit B. thuringiensis growth in vitro.

Abstract Objective: The objective of PHASE-MX registry is to validate the efficacy and safety of the pharmacoinvasive strategy in comparison with percutaneous coronary intervention (PCI) in patients with acute myocardial infarction with ST segment elevation (STEMI) in a metropolitan region of Mexico. The primary outcome will consist of the composite of cardiovascular death, re-infarction, stroke and cardiogenic shock. Methods: The PHASE-MX registry will include a prospective cohort of patients with STEMI who received reperfusion treatment (mechanical of pharmacological) in the first 12 h after the onset of symptoms. The registry is designed to compare the efficacy and safety of primary PCI and pharmacoinvasive strategy. The simple size was calculated in 344 patients divided into two groups, with an estimated loss rate of 10%. Patients included in the PHASE-MX cohort will be followed for up to one year. Conclusion: In Mexico, only 5 out of 10 patients with STEMI have access to reperfusion therapy. Pharmacoinvasive strategy is takes advantage of the accessibility of fibrinolysis and the effectiveness of PCI. The present research protocol aims to provide information that serves as a link between information derived from controlled clinical trials and records derived from real world experience.
Resumen Objetivo: El objetivo del registro PHASE-MX es validar la eficacia y seguridad de la estrategia farmacoinvasiva en comparación con la angioplastia coronaria transluminal percutánea primaria (ACTPp) en pacientes con infarto agudo de miocardio con elevación del segmento ST (IAMCEST) en una región metropolitana de México. El desenlace primario es el compuesto de muerte cardiovascular, reinfarto, accidente vascular cerebral y choque cardiogénico. Métodos: El registro PHASE-MX es una cohorte prospectiva de pacientes con IAMCEST que recibieron tratamiento de reperfusión (mecánico o farmacológico) en las primeras 12 horas desde el inicio de los síntomas, atendidos en el Instituto Nacional de Cardiología Ignacio Chávez. El análisis estadístico se basa en la no inferioridad de la estrategia farmacoinvasiva en comparación con la ACTPp. Se calcula un tamaño de muestra de 344 pacientes divididos en dos grupos (angioplastia primaria y estrategia farmacoinvasiva), considerada una tasa de pérdidas de 10%. Los pacientes incluidos en la cohorte PHASE-MX se seguirán durante un año. Discusión: En México, sólo 5 de cada 10 pacientes con IAMCEST tienen acceso al tratamiento de reperfusión. La estrategia farmacoinvasiva aprovecha la accesibilidad de la fibrinólisis y la efectividad de la ACTPp, por lo que podría resultar el método de elección en el tratamiento del IAMCEST en la mayoría de los casos. El presente protocolo de investigación pretende aportar información que sirva como enlace entre la información derivada de los estudios clínicos controlados y los registros derivados de la experiencia del mundo real.

Resumo Dentre as ecorregiões dulcícolas neotropicais, a Nordeste Médio-Oriental (MNCE) é uma lacuna de conhecimento sobre peixes. Suas drenagens temporárias são receptoras do projeto de transposição do São Francisco (SFR-IWT) no semiárido brasileiro. Fornecemos uma linha de base abrangente da riqueza de peixes das cinco bacias do SFR-IWT. Riqueza de espécies, espécies compartilhadas, endêmicas, ameaçadas e não-nativas foram obtidas através de amostragem, coleções ictiológicas, literatura e repositórios online (306 localidades). No total, foram registradas 121 espécies, 111 nativas e 16 (14,41%) listadas para todas as bacias. A maior riqueza de espécies nativas (78, 70,27%) foi registrada no trecho sub-médio do São Francisco (SFRE), incluindo 23 endêmicas e 61 (54,95%) nas bacias da MNCE (13 endêmicas), e 28 (25,23 %) compartilhadas entre ambas ecorregiões. Na MNCE, foram registradas 50 espécies na Jaguaribe (JAG), 39 na Piranhas-Açu, 36 na Paraíba do Norte e 32 na Apodi-Mossoró (APO). O número de espécies compartilhadas entre SFRE e cada bacia receptora variou de 24 (23,08%, JAG) a 20 (22,22%, APO). JAG contém 81,97% das espécies das bacias receptoras. Sua maior riqueza e endemismo merecem atenção especial em relação às mudanças hidrológicas em curso. Este estudo ajudará a detectar possíveis modificações na ictiofauna das principais drenagens da MNCE.
Abstract Among Neotropical freshwater ecoregions, the Mid-Northeastern Caatinga (MNCE) is a fish knowledge gap. Its temporary drainages are receptors of the São Francisco interbasin water transfer project (SFR-IWT) in the Brazilian semiarid. We provide a comprehensive baseline of fish richness of the five SFR-IWT basins. Species richness, shared, endemic, threatened and non-native species were obtained using sampling, ichthyologic collections, literature and online repositories (306 localities). In total 121, species were recorded, 111 of them native, and 16 (14.41%) listed for all basins. Higher richness of native species (78, 70.27%) was recorded in the São Francisco lower-middle stretch (SFRE), including 23 endemic, 61 (54.95%) in MNCE basins (13 endemic), and 28 (25.23%) shared between both ecoregions. In the MNCE, 50 species were recorded in Jaguaribe (JAG), 39 in Piranhas-Açu, 36 in Paraíba do Norte, and 32 in Apodi-Mossoró (APO). The number of species shared between the SFRE and each receptor basin varied from 24 (23.08%, JAG) to 20 (22.22%, APO). JAG contains 81.97% of the receptor basins’ species. Its higher richness and endemism deserve special attention regarding the ongoing hydrological changes. This study will help detect possible modifications in the ichthyofauna of the main MNCE drainages.

ABSTRACT This study investigated the frequency of infection by Anaplasma platys and Ehrlichia canis in dogs submitted to animal health centers in Campo Grande, state of Mato Grosso do Sul, Brazil. E. canis and A. platys showed infection frequencies of 55.75% and 16.96%, respectively. The identity of the two species was confirmed by DNA sequencing.

Resumo: No Brasil, a leishmaniose visceral é causada pela Leishmania infantum. É uma zoonose crônica e, frequentemente, fatal, sendo considerada um grave problema de saúde pública no mundo e reconhecida pela Organização Mundial de Saúde como uma das dezessete "Doenças Tropicais Negligenciadas". O presente estudo foi realizado em 2012, na zona rural do município de Patos, semiárido paraibano, com o objetivo de estimar a prevalência, os fatores de risco da leishmaniose visceral canina (LVC) na região e avaliar os testes de diagnóstico sorológico. Foram coletadas 362 amostras de sangue e a prevalência da infecção foi determinada através de três técnicas sorológicas (ELISA, RIFI e Teste Imunocromatográfico DPP - Dual Path Plataform), sendo consideradas positivas as amostras que apresentaram pelo menos dois testes reagentes. As amostras positivas na sorologia foram testadas na PCR e qPCR. Aplicou-se um questionário epidemiológico aos proprietários para identificação dos possíveis fatores de risco. A taxa de prevalência foi de 11,33% (41/362). Considerou-se o ELISA como padrão ouro, já que é o teste confirmatório para a LVC pelo Ministério da Saúde. O DPP (Dual Path Plataform) apresentou sensibilidade de 58% (95% CI: 0,43-0,72) e especificidade de 96% (95% CI: 0,93-0,98), com índice Kappa de 0,57. A RIFI apresentou sensibilidade de 85% (95% CI: 0,72-0,94), especificidade de 99% (95% CI: 0,98-1) e valor Kappa de 0,88. O fator de risco identificado na análise multivariada por regressão logística múltipla foi o sexo (OR=2,15 [1,09-4,21]) e os cães machos apresentaram 2,15 vezes mais chances de serem infectados, que pode ser justificado por serem mais utilizados para a caça e como cão de guarda, dois fatores que aumentam a exposição do animal ao flebótomo. Em função da taxa de prevalência encontrada, indica-se a aplicação de medidas de controle no município. Os resultados da comparação dos ensaios sorológicos demonstraram que o DPP (Dual Path Plataform) não é a melhor técnica para triagem dos animais positivos em função da baixa sensibilidade do método, sugerindo que o mesmo seja substituído pelo ELISA.
Abstract: Visceral leishmaniasis is caused by Leishmania infantum in Brazil. It is a chronic disease and often a fatal zoonosis and considered as a serious worldwide public health problem and recognized by the World Health Organization as one of seventeen "Neglected Tropical Diseases". This study was conducted in 2012 in the county of Patos, in Paraíba State semiarid region. The main purposes were to estimate the prevalence and the risk factors of canine visceral leishmaniasis (CVL), and to evaluate the performance of serological tests. Serological prevalence was determined using three techniques (ELISA, IFA and DPP chromatographic immunoassay - Dual Path Platform). ELISA was the golden standard, since it is the confirmatory test for CVL as established by the Brazilian Ministry of Health. In total, 362 blood samples were collected and were considered positive if positive by two serological assays. Positive samples were tested by PCR and qPCR. An epidemiological questionnaire was applied to the dog owner to identify risk factors associated with CVL. The seroprevalence was 11.33% (41/362). Chromatographic immunoassay - DPP showed a sensitivity of 58% (95%, CI: 0.43 to 0.72) and specificity of 96% (95%, CI: 0.93-0.98) , with a Kappa index of 0.5. The IFA had a sensitivity of 85% (95%, CI: 0.72 to 0.94), specificity of 99% (95%, CI: 0.98 to 1) and Kappa value of 0.8. The risk factor identified in the multivariate analysis was gender (OR=2.15 [1.09 to 4.21]), male dogs having 2.15 times more chance of being infected. The increased likelihood can possibly be explained by the use of male dogs for hunting and as guard dogs, two factors that increase dog contact to infected sandflies. Due to the prevalence rates, it should be recommended control measures against canine visceral leishmaniasis. The results demonstrated that the Chromatographic immunoassay (DPP-Dual Path Platform) is not the best technique for screening positive animals due to its low sensitivity, suggesting its replacement by ELISA.

Hydroxylation of 1,8-cineole by Mucor ramannianusand Aspergillus niger

The monoterpenoid 1,8-cineole is obtained from the leaves of Eucalyptus globulus and it has important biological activities. It is a cheap natural substrate because it is a by-product of the Eucalyptuscultivation for wood and pulp production. In this study, it was evaluated the potential of three filamentous fungi in the biotransformation of 1,8-cineole. The study was divided in two steps: first, reactions were carried out with 1,8-cineole at 1 g/L for 24 h; afterwards, reactions were carried out with substrate at 5 g/L for 5 days. The substrate was hydroxylated into 2-exo-hydroxy-1,8-cineole and 3-exo-hydroxy-1,8-cineole by fungi Mucor ramannianus and Aspergillus niger with high stereoselectivity. Trichoderma harzianum was also tested but no transformation was detected. M. ramannianus led to higher than 99% of conversion within 24 h with a starting high substrate concentration (1 g/L). When substrate was added at 5 g/L, only M. ramannianuswas able to catalyze the reaction, but the conversion level was 21.7% after 5 days. Both products have defined stereochemistry and could be used as chiral synthons. Furthermore, biological activity has been described for 3-exo-hydroxy-1,8-cineol. To the best of our knowledge, this is the first report on the use of M. ramannianus in this reaction.
