RESUMO Este estudo buscou expor como as interações entre os atores de saúde da tríplice fronteira Brasil, Colômbia e Peru influenciam a oferta de atendimento a residentes fronteiriços daquela localidade. Como método, foi realizado um caso único, explicativo, integrado e qualitativo em município da tríplice fronteira Brasil, Colômbia e Peru. As fontes de evidências utilizadas foram: dados documentais, entrevistas e observações diretas. Para organização dos dados, análise, proposições teóricas e construção de explicações como estratégia analítica e técnica de análise foi utilizado o software MaxQDA12®. Os dados coletados evidenciaram que questões financeiras são o principal desafio para a oferta de atendimento de saúde ao residente fronteiriço. No entanto, o atendimento de saúde ofertado a esta população se baseia em valores éticos, como o igualitarismo; respeito à liberdade individual e a vida humana; solidariedade e altruísmo. Neste contexto, o atendimento de saúde de residentes fronteiriços, na região estudada, é reconhecido pelos gestores de saúde locais como um direito.

ABSTRACT This study sought to explain how interactions between health actors on the triple border of Brazil, Colombia and Peru influence the provision of care to border residents in that location. As a method, a single, explanatory, integrated, qualitative case was carried out in a municipality on the triple border of Brazil, Colombia, and Peru. The sources of evidence used were: documentary data, interviews, and direct observations. MaxQDA12® software was used to organize the data and for analysis, theoretical propositions, and construction of explanations as an analytical strategy and analysis technique. The data collected showed that financial issues are the main challenge in offering healthcare to border residents. However, the healthcare offered to this population is based on ethical values, such as egalitarianism; respect for individual freedom and human life; solidarity and altruism. In this context, healthcare for border residents in the studied region is recognized by local health managers as a right.

Resumen: Este ensayo aporta una reflexión teórica acerca de los desafíos para lograr las metas de los Objetivos de Desarrollo Sostenible de la Agenda 2030, teniendo en cuenta su lema “no dejar a nadie atrás”. Para ejemplificar estos desafíos, tomamos como telón de fondo las enfermedades crónicas no transmisibles (ECNT), uno de los principales temas de la agenda de salud global antes de la pandemia de COVID-19, discutiendo las dimensiones políticas y económicas que determinan su presencia y avance global. Después de un breve panorama de las ECNT, el texto busca responder tres preguntas: En “¿Sin dejar a nadie atrás?” enumeramos algunos temas para reflexionar sobre cómo y quién ha quedado históricamente atrás, profundizando un poco más los ejemplos al centrarse en “¿Quién ha quedado atrás en el mundo?” y “¿Quién ha quedado atrás en Brasil?”. Con base en datos de la literatura más relevante y reciente sobre el tema, presentamos los desafíos y algunas maneras de no dejar a nadie atrás en un mundo en el que el modo de producción ha vulnerabilizado históricamente a algunos grupos sociales, con énfasis en la población negra y en la población indígena. En nuestras consideraciones finales, nos inspiramos en el ideograma de Sankofa para recordar que las respuestas al desarrollo sostenible que buscamos pueden estar en algún lugar de nuestro pasado más original y tradicional. Y es necesario apostar por la construcción de nuevos caminos basados en otras epistemologías y cosmovisiones presentes al otro lado de la línea abisal.

Resumo: Este ensaio traz uma reflexão teórica sobre os desafios para alcançar as metas dos Objetivos do Desenvolvimento Sustentável da Agenda 2030, considerando seu lema de “não deixar ninguém para trás”. Para exemplificar esses desafios, apresenta-se como pano de fundo as doenças crônicas não transmissíveis (DCNT), um dos principais temas da agenda da saúde global antes da pandemia de COVID-19, discutindo as dimensões políticas e econômicas que determinam sua presença e avanço global. Após um breve panorama sobre as DCNT, busca-se responder a três perguntas: em “Sem deixar ninguém para trás?”, elencamos alguns temas para refletir sobre como e quem tem ficado historicamente para trás, aprofundando um pouco mais os exemplos ao adentrar em “Quem tem ficado para trás no mundo?” e “Quem tem ficado para trás no Brasil?”. A partir de dados da literatura mais relevante e recente sobre o tema, apresentamos os desafios e alguns caminhos para não deixar ninguém para trás em um mundo em que o modo de produção tem historicamente vulnerabilizado alguns grupos sociais, com destaque para a população negra e a população indígena. Trazemos nas considerações finais a inspiração do ideograma Sankofa para lembrar que as respostas para o desenvolvimento sustentável que buscamos podem estar em algum lugar de nosso passado mais originário e tradicional, e que é preciso apostar na construção de novos caminhos a partir de outras epistemologias e cosmovisões presentes do outro lado da linha abissal.

Abstract: This essay provides a theoretical reflection on the challenges of meeting the Sustainable Development Goals of the 2030 Agenda, considering its motto of “leave no one behind”. To exemplify these challenges, we discuss noncommunicable diseases (NCDs), one of the main issues on the global health agenda before the COVID-19 pandemic, and the political and economic dimensions that determine their presence and global spread. After a brief overview of NCDs, the text seeks to answer three questions: In “Leaving no one behind?” we list some themes to reflect on how and who has historically been left behind, delving a little deeper into the examples in “Who has been left behind in the world?” and “Who has been left behind in Brazil?”. Using data from the most relevant and recent literature on the subject, we discuss the challenges and some ways to leave no one behind in a world where the mean of production has historically made some social groups vulnerable, especially black and Indigenous populations. In our final remarks, we draw inspiration from the Sankofa ideogram to remember that the answers to the sustainable development we seek may lie somewhere in our most primordial and traditional past. And that it is necessary to invest on building new paths from different worldviews and approaches to epistemology on the other side of the abyssal line.

Resumo Objetivo Analisar as evidências de validade de conteúdo e processo de resposta de um banco de itens sobre vulnerabilidade em saúde. Métodos Estudo psicométrico, subsidiado pelo polo teórico de Pasquali. Foram selecionados 62 especialistas com características sociodemográficas e acadêmicas distintas. Desses, 15 responderam ao questionário de validade de conteúdo. Após os ajustes, o banco passou para fase de validade de processo de resposta e foi avaliado por 12 usuários dos serviços de saúde. Utilizou-se coeficiente de validade de conteúdo para avaliação dos itens e do banco, cuja confiabilidade foi verificada pelo teste binomial, considerando percentual de 80% e p-valor >0,05. Resultados O banco apresentou boa evidência de validade de conteúdo pelos especialistas, cujos coeficientes foram 0,87 para o elemento sujeito, 0,86 para o elemento social e 0,865 para o geral. O público-alvo realizou sugestões na fase de validade de processo de resposta para melhor compreensão dos itens (coeficiente total: 0,89), sendo considerado, claro, preciso e compreensível. Conclusão A versão final do banco de itens possui 535 itens, sendo 238 para o sujeito e 297 para o social. Abstract Objective To analyze the evidence of content validity and response process of a health vulnerability item bank. Methods Psychometric study with support of Pasquali’s theoretical pole in which 62 specialists with different sociodemographic and academic characteristics were selected and 15 of them responded to the content validity questionnaire. After adjustments, the bank moved to the response process validity phase and was evaluated by 12 health service users. A content validity coefficient was used to evaluate the items and the bank, which had the reliability assessed by the binomial test, considering a percentage of 80% and p-value >0.05. Results The bank presented good evidence of content validity by experts. The coefficients were 0.87 for the subject element, 0.86 for the social element and 0.865 for the general element. The target audience made suggestions in the response process validity phase to better understand the items (total coefficient: 0.89), which were considered clear, accurate and understandable. Conclusion The final version of the item bank has 535 items, 238 for the subject and 297 for the social element. Resumo O objetivo deste artigo é verificar como o ensino de ética/bioética está inserido nas grades curriculares dos programas de pós-graduação stricto sensu na área medicina III, conforme classificação da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior, nas regiões Norte e Nordeste. Trata-se de estudo documental, descritivo e exploratório, de natureza qualiquantitativa. Utilizou-se a Plataforma Sucupira para buscar as seguintes informações sobre os programas: estado, instituição, nome do programa, categoria (acadêmico/profissional), nota, obrigatoriedade e nome da disciplina, carga horária e site do programa. Foram encontrados nove programas na área medicina III, sobretudo mestrados profissionais, e a maioria deles oferece disciplinas ligadas a ética como obrigatórias. Verificou-se que disciplinas ligadas a ética/ bioética estão presentes em todos os programas, embora com diferentes cargas horárias. Abstract The objective of this article is to examine how ethics/bioethics teaching is included in the curricula of stricto sensu graduate programs in the field of medicine III, according to the classification of the Coordination for the Improvement of Higher Education Personnel, in the North and Northeast regions of Brazil. This is a documentary, descriptive and exploratory study, of a qualitative and quantitative nature. The Sucupira Platform was used to search for the following information about the programs: state, institution, name of the program, category (academic/professional), score, compulsoriness and name of the course, workload and website of the program. We found nine programs in the field of medicine III, mainly professional master’s degree programs, most of which offer ethics-related courses as compulsory courses. We found that ethics/ bioethics-related courses are present in all programs, although with different workloads. Resumen El objetivo de este artículo es comprobar la manera en que se incluye la enseñanza de la ética/bioética en los planes de estudio de los programas de posgrado stricto sensu en el campo de la medicina III, según la clasificación de la Coordinación de Perfeccionamiento de Personal de Nivel Superior, en las regiones Norte y Nordeste. Se trata de un estudio documental, descriptivo y exploratorio, de carácter cualicuantitativo. Se utilizó la Plataforma Sucupira para buscar la siguiente información sobre los programas: estado, institución, nombre del programa, categoría (académico/profesional), nota, obligatoriedad y nombre de la asignatura, horas lectivas y página web del programa. Abstract Oviduct fluid extracellular vesicles (oEV) are essential for periconceptional events. The presence of EV has already been identified in the oviduct fluid (OF) from mammalian species, except in caprine. Therefore, this study aimed to isolate and characterize the caprine oEV (coEV). Initially, in Experiment 1, coEV were isolated from the OF of either each animal individually or from a pool of three animals. In experiment 2, coEV were isolated during the follicular or luteal phases of the estrous cycle. The coEV were characterized by size distribution, polydispersity index (PDI), and zeta potential using dynamic light scattering (DLS) analysis, as well as, by transmission electron microscopy (TEM) and dot blotting (DB). Our results indicated that the physicochemical characteristics of the coEV were similar (P > 0.05), regardless of the isolation method (individual or pool). However, coEV collected during the luteal phase were larger (P < 0.05) than those during the follicular phase. The TEM showed spherical and cup-shaped particles, characteristic of exosomes. The DB revealed the presence of exosomal proteins involved in the biogenesis of coEV. In conclusion, it is possible to isolate and characterize coEV from a single caprine female and the estrous cycle phase influences the vesicles average size and PDI. ABSTRACT The objective of this study was to evaluate, through principal component analysis, the effect of different feedlot structures on climatic conditions, nutrient intake, and performance of Nellore bulls during the feeding period in the semi-arid region. Data from 225 non-castrated Nellore bulls with initial mean body weight (BW) of 294 ± 19 kg and age of 24 months were used. This was done in a completely randomized design with three feedlot structures (no shading, natural shade, and artificial shade with 50% light interception), resulting in 75 repetitions (each animal serving as an experimental unit). The feeding period for the animals in the feedlot was 107 days. The first two components explained 99.65 (no shade), 97.32 (artificial shade), and 99.89 (natural shade) of total data variation. The variables, namely the black globe temperature index (0.88), radiant heat load (0.96, 0.99, 0.76), black globe temperature (0.96), and hot carcass weight (0.64) exerted the greatest influence on the feedlot structures. Climatic and intake variables were identified as having the most significant impact on feedlot structures. ABSTRACT Purpose: To investigate the effects of chronic stress in the prostate of prepubertal and adult rats. Methods: Thirty-two male rats were assigned into four groups depending on the type of treatment (control or stressed) and the age at which stress was initiated (prepubertal or adult). Restraint stress stimuli were applied for six weeks. Stressed prepubertal and adult rats evaluated immediately after the last stress stimuli were named SP and SA groups, respectively. Age-matched rats were used as control groups (CP and CA). At the end of the experiment, the rats were euthanized, and prostate morphological parameters were evaluated and statistically compared. Results: Application of stress stimuli to the SP group resulted in reduced body weight, but no prostate morphological modification was noted. The SA group showed reduced testosterone level and prostatic epithelium surface density, in comparison to CA group. Further, the prostatic lumen surface density was increased in adult stressed animals, in comparison to adult controls. Conclusions: The stress stimuli promoted changes in hormonal and morphological parameters in the prostate of adult stressed rats. Abstract This study aimed to evaluate the contribution of oral and maxillofacial pathology laboratories (OMPLs) in Brazilian public universities to the diagnosis of lip, oral cavity, and oropharyngeal squamous cell carcinoma (SCC). A cross-sectional study was performed using biopsy records from a consortium of sixteen public OMPLs from all regions of Brazil (North, Northeast, Central-West, Southeast, and South). Clinical and demographic data of patients diagnosed with lip, oral cavity, and oropharyngeal SCC between 2010 and 2019 were collected from the patients’ histopathological records. Of the 120,010 oral and maxillofacial biopsies (2010-2019), 6.9% (8,321 cases) were diagnosed as lip (0.8%, 951 cases), oral cavity (4.9%, 5,971 cases), and oropharyngeal (1.2%, 1,399 cases) SCCs. Most cases were from Brazil’s Southeast (64.5%), where six of the OMPLs analyzed are located. The predominant profile of patients with lip and oral cavity SCC was Caucasian men, with a mean age over 60 years, low schooling level, and a previous history of heavy tobacco consumption. In the oropharyngeal group, the majority were non-Caucasian men, with a mean age under 60 years, had a low education level, and were former/current tobacco and alcohol users. According to data from the Brazilian National Cancer Institute, approximately 9.9% of the total lip, oral cavity, and oropharyngeal SCCs reported over the last decade in Brazil may have been diagnosed at the OMPLs included in the current study. Therefore, this data confirms the contribution of public OMPLs with respect to the important diagnostic support they provide to the oral healthcare services extended by the Brazilian Public Health System. Abstract Fluoride (F) has been widely used to control dental caries, and studies suggest beneficial effects against diabetes when a low dose of F is added to the drinking water (10 mgF/L). Objectives This study evaluated metabolic changes in pancreatic islets of NOD mice exposed to low doses of F and the main pathways altered by the treatment. Methodology In total, 42 female NOD mice were randomly divided into two groups, considering the concentration of F administered in the drinking water for 14 weeks: 0 or 10 mgF/L. After the experimental period, the pancreas was collected for morphological and immunohistochemical analysis, and the islets for proteomic analysis. Results In the morphological and immunohistochemical analysis, no significant differences were found in the percentage of cells labelled for insulin, glucagon, and acetylated histone H3, although the treated group had higher percentages than the control group. Moreover, no significant differences were found for the mean percentages of pancreatic areas occupied by islets and for the pancreatic inflammatory infiltrate between the control and treated groups. Proteomic analysis showed large increases in histones H3 and, to a lesser extent, in histone acetyltransferases, concomitant with a decrease in enzymes involved in the formation of acetyl-CoA, besides many changes in proteins involved in several metabolic pathways, especially energy metabolism. The conjunction analysis of these data showed an attempt by the organism to maintain protein synthesis in the islets, even with the dramatic changes in energy metabolism. Conclusion Our data suggests epigenetic alterations in the islets of NOD mice exposed to F levels comparable to those found in public supply water consumed by humans. ABSTRACT Nasturtium (Tropaeolum majus) is an ornamental and medicinal plant that has edible flowers. Drought and salt stress decrease flower production and quality, including affecting sugar metabolism and ethylene production. Ethylene can accelerate the nasturtium senescence process, decreasing its postharvest quality. The use of polyamines, mainly spermine, may be a strategy for reducing the harmful effects of these stresses on the metabolism of sugars and phenolic compounds and for decreasing the production of ethylene, which accelerates senescence, in nasturtium flowers. Therefore, the objective here was to evaluate the role of spermine application on sugar and phenolic compounds and on ethylene production in nasturtium flowers grown under drought and salt stress. Two experiments were performed in isolation and at the same time in order to achieve this. Spermine down-regulated ethylene production and up-regulated the content of sugars and phenolic compounds on nasturtium flowers grown under drought and saline stress. Sugars and phenolic compounds down-regulated ethylene production in nasturtium flowers. Spermine can be used to mitigate the harmful effects of drought and salt stress on nasturtium flowers by increasing sugar and phenolic compounds and decreasing ethylene production. Abstract Hydroxytyrosol (HT) is a natural phenolic compound found in leaves of olive trees. It is a photosensitive solid phytochemical constituent and an irritant substance to skin and mucosa as pure drug. HT is a lipophilic chemical constituent and shows low solubility in water, bitter taste, and instability to oxidizing atmosphere. Considering the limitations for its suitable therapeutic use, the aim of the present study was to obtain, to characterize, to quantify, and to evaluate the stability of HT-loaded poly(ԑ-caprolactone) nanocapsules for its further use in pharmaceutical and nutraceutical products. Nanocapsules were prepared by method of interfacial deposition of the preformed polymer. The formulations were characterized by morphological and spectroscopic methods. The UHPLC-PDA analytical method was developed and validated for quantifying the HT encapsulation. Physicochemical stability assay was performed for 120 days. Nanocapsules were successfully obtained by the proposed method. The morphological evaluation demonstrated the drug absence in the nanocapsule surface. Fourier-transform infrared spectroscopy demonstrated the drug nanoencapsulation. The analytical method was validated and confirmed a high HT encapsulation efficiency over 95%. Considering the physicochemical stability testing, the polydispersity index changed after 30 days for NC-2 (HT at 2 mg/mL) and after 90 days for NC-5 (HT at 5 mg/mL). Zeta potential changed after 60 days for both formulations (NC-2 and NC-5). pH values were modified after 60 days for NC-5 and after 90 days for NC-2. HT-loaded PCL nanocapsules demonstrate adequate physicochemical features and high drug encapsulation. The different formulations revealed stability changes from 30 to 90 days. Abstract Introduction Cortisol is a hormone involved in the response to stress. Attention is a function that can change due to exposure to stress. Objectives To verify the correlation between the level of salivary cortisol and sustained auditory attention in children with cleft lip and palate, as well as to compare the results of the variables analyzed between female and male patients. Methods In total, 103 children aged 6 to 11 years, were divided into 2 groups: those with cleft lip and palate (experimental group, EG; n = 69) and the control group (CG; n = 34). The Sustained Auditory Attention Ability Test (SAAAT) and salivary cortisol levels, measured by an enzyme immunoassay kit (Salimetrics, Stage College, PA, US), were calculated and compared regarding the two groups. The statistical tests used were the Mann-Whitney test and the Spearman correlation. Results The median level of salivary cortisol was of 0.03615 μl/dL and 0.18000 μl/dL respectively for the EG and CG, with a significant difference between the groups (p =0.000). Absence of statistical significance (total error score = 0.656; vigilance decrement = 0.051) was observed the for SAAAT among the EG (median total error score = 12.00; 25th percentile [25%] = 7.00; 75th percentile [75%] = 21.00; and vigilance decrement = 1.00; 25% = 0.00; 75% = 2.50) and the CG (median total error score = 12.00; 25% = 6.00; 75% = 24.00; and vigilance decrement = 0.00; 25% = -1.00; 75% = 2.00). Conclusion All children had median levels of salivary cortisol and scores for sustained auditory attention within normal parameters. A significant correlation between the level of salivary cortisol and the ability to sustain auditory attention was observed in children without cleft lip and palate.
