Inflammatory markers in prepubertal children and their associations with abdominal fat

Abstract Objective To evaluate the association between inflammatory markers and abdominal fat assessed by ultrasound in prepubertal children with and without excess weight. Methods A cross-sectional study involving 241 prepubertal children, 156 with obesity, 37 with overweight, and 48 with normal weight, aged five to ten years, who were followed at a research unit on Childhood Obesity from a teaching hospital belonging to a public health system. The concentration of interleukin-6, tumor necrosis factor-α and C-reactive protein were assessed and regression analyses, considering outcome variables such as abdominal wall and intra-abdominal fat thickness measured by ultrasound, were performed. Results The findings highlighted an association between abdominal fat and inflammatory markers, even in children at this young age group. Subcutaneous fat showed a stronger association with inflammatory biomarkers compared to intra-abdominal fat when performing logistic regression, with a positive association between tumor necrosis factor-α and abdominal wall thickness equal to or greater than the 75th percentile in adjusted logistic regression (OR: 18.12; CI 95 %: 1.57: 209.55). ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. RESUMO Objetivo: Compreender as repercussões da COVID-19 no cotidiano de mulheres em um assentamento rural. Métodos: Estudo de abordagem qualitativa, realizado num assentamento rural do Movimento dos Trabalhadores Rurais Sem Terra, em um município no Nordeste do Brasil, entre janeiro e março de 2021. Participaram 48 mulheres por meio de entrevista semiestruturada. Os dados coletados foram analisados pelo método do Discurso do Sujeito Coletivo, à luz do referencial dos processos pandêmicos. Resultados: O agrupamento dos discursos desvelou sentidos semelhantes e/ou complementares acerca das estratégias de enfrentamento e dos sentimentos gerados como consequência da pandemia. Foram organizadas quatro Ideias Centrais: Negação ao progressivo reconhecimento; Percepção do problema, aceitação e explicação da realidade; Negociação; e Retrospecção/reflexão. Conclusão: As repercussões da pandemia estão intrinsicamente relacionadas a uma inóspita realidade da perspectiva da vivência de mulheres que são cotidianamente esquecidas, marginalizadas e silenciadas.
ABSTRACT Objective: To understand the repercussions of COVID-19 on women’s daily lives in a rural settlement. Methods: A qualitative study was conducted in a rural settlement of the Landless Workers’ Movement (MST) in a municipality in Northeastern Brazil between January and March 2021. Forty-eight women participated through semi-structured interviews. The data collected were analyzed by the Collective Subject Discourse method in light of the referential of pandemic processes. Results: The grouping of the speeches unveiled similar and/or complementary meanings about the coping strategies and the feelings generated due to the pandemic. Four Central Ideas were organized: denial to progressive awareness; Perception of the problem, acceptance, and explanation of reality; Negotiation; and Retrospection/reflection. Conclusion: The pandemic repercussions are intrinsically related to an inhospitable reality from the perspective of the experience of women daily forgotten, marginalized, and suppressed.

RESUMO Objetivo: Identificar as relações dos enunciados de diagnósticos de enfermagem para crianças com doenças renais elaborados segundo a Classificação Internacional para a Prática de Enfermagem (CIPE®) com os diagnósticos da NANDA Internacional (NANDA-I). Métodos: Estudo metodológico operacionalizado pelas etapas: 1) Levantamento dos achados clínicos por meio de entrevista e exame físico com crianças; 2) Elaboração dos diagnósticos de enfermagem mediante o julgamento clínico de Gordon; 3) Mapeamento cruzado dos enunciados de diagnósticos entre os sistemas de classificação NANDA-I e CIPE®; 4) Validação de conteúdo utilizando a técnica Delphi, em duas rodadas, com enfermeiros especialistas. Resultados: Participaram 90 crianças. Foram elaborados 151 diagnósticos, dos quais 66,3% (n= 100) da terminologia da CIPE® e 33,7% (n= 51) da NANDA-I; 55 diagnósticos apresentaram equivalência de significados. Conclusões: O mapeamento cruzado de diagnósticos foi alcançado partindo-se da realidade de crianças, com uso de raciocínio clínico e validação por enfermeiros especialistas.

Methods: Methodological study operationalized by the steps: 1) Survey of clinical findings through interviews and physical examination with children; 2) Elaboration of nursing diagnoses through Gordon’s clinical judgment; 3) Cross-mapping of diagnostic statements between the NANDA-I and ICNP® classification systems; 4) Content validation using the Delphi technique, in two rounds, with specialist nurses. Results: 90 children participated. A total of 151 diagnoses were made, of which 66.3% (n=100) used ICNP® terminology and 33.7% (n=51) used NANDA-I; 55 diagnoses showed equivalence of meanings. Conclusions: Cross-mapping of diagnoses was achieved starting from the reality of children, using clinical reasoning and validation by specialist nurses.

Molecular Detection of Medically Important Candida species from Droppings of Pigeons (Columbiformes) and Captive Birds (Passeriformes and Psittaciformes)

Abstract Passeriformes and Psittaciformes birds and pigeons (Columba livia) are known to be reservoirs of microorganisms, and their stool allows fungi development. Since accumulated avian excreta can interfere with public health, this study aimed to perform a molecular screening of medically important Candida species in pigeon droppings in public places and birds raised in captivity. Excreta collected from captive birds (3 residences) and pigeons (4 districts) were inoculated on Sabouraud dextrose agar with chloramphenicol for Gram staining and subculture on Hicrome® Candida. Three DNA extraction methods were performed for comparison (commercial kit, in-house and by boiling) and PCR to screen 6 clinically important Candida species among the isolates. The correlation between phenotypic and molecular methods was calculated by kappa/K. Only 6 C. parapsilosis (20%) were identified from captive birds’ feces among 30 isolates (80% not identified), while pigeons’ feces harbored a greater diversity, with the 6 pathogenic species confirmed among 41 isolates: C. albicans (31.70%/13), C. krusei (14.63%/6), C. tropicalis (14.63%/6), C. parapsilosis (17.10%/7), C. glabrata (14.63%/6) and C. guilliermondii (7.31%/3); 100% correlation between tested methods (K = 1) for the first 3 species. Boiling DNA extraction method was fast and efficient to obtain viable DNA from Candida spp. for PCR. Our results indicate that pigeon droppings harbor more potentially pathogenic species than birds in residential captivity, which probably have non-albicans Candida less frequently isolated in infectious processes. The greater availability of nutrients may have contributed to a diversity of Candida spp. in feces from public environments.

RESUMO Os beta-lactâmicos constituem a causa mais frequente de reações de hipersensibilidade a fármacos mediadas por mecanismo imunológico específico. As reações imediatas ocorrem em 1 até 6 horas após a administração do beta-lactâmico, sendo geralmente IgE-mediadas. Elas se traduzem clinicamente por urticária, angioedema e anafilaxia. As reações não imediatas ou tardias ocorrem após 1 hora da administração. São as reações mais comuns, sendo geralmente mediadas por células T. O tipo mais frequente é o exantema maculopapular ou morbiliforme. A maioria dos indivíduos que refere alergia aos beta-lactâmicos pode tolerar esse grupo de antibióticos. No diagnóstico, uma história clínica detalhada é fundamental para verificar se a reação foi do tipo imediato ou não imediato. A partir daí, podem ser realizados testes in vivo e/ou in vitro para investigação. O teste de provocação é considerado o método padrão-ouro no diagnóstico de hipersensibilidade aos beta-lactâmicos. A primeira conduta diante da suspeita de uma reação ao beta-lactâmico é suspender a exposição ao medicamento, e o único tratamento específico é a dessensibilização, que possui indicações bem precisas. O diagnóstico equivocado de alergia à penicilina afeta o sistema de saúde, pois o rótulo de “alergia à penicilina” está associado a aumento da resistência bacteriana, maior índice de falha terapêutica, hospitalizações prolongadas, readmissões e aumento dos custos. Assim, torna-se fundamental elaborar estratégias com o objetivo de auxiliar na prescrição de antibióticos em pacientes com rótulo de “alergia aos beta-lactâmicos” nos hospitais e melhorar a educação dos pacientes e seus responsáveis, além de médicos não especialistas.
ABSTRACT Betalactams are the most frequent cause of hypersensitivity reactions to drugs mediated by a specific immune mechanism. Immediate reactions occur within 1 to 6 hours after betalactam administration, and are generally IgE-mediated. They clinically translate into urticaria, angioedema and anaphylaxis. Non-immediate or delayed reactions occur after 1 hour of administration. These are the most common reactions and are usually mediated by T cells. The most frequent type is the maculopapular or morbilliform exanthematous eruption. Most individuals who report allergies to penicillin and betalactams can tolerate this group of antibiotics. To make diagnosis, a detailed medical history is essential to verify whether it was an immediate or non-immediate reaction. Thereafter, in vivo and/or in vitro tests for investigation may be performed. The challenging test is considered the gold standard method for diagnosis of betalactam hypersensitivity. The first approach when suspecting a reaction to betalactam is to discontinue exposure to the drug, and the only specific treatment is desensitization, which has very precise indications. The misdiagnosis of penicillin allergy affects the health system, since the “penicillin allergy” label is associated with increased bacterial resistance, higher rate of therapeutic failure, prolonged hospitalizations, readmissions, and increased costs. Thus, it is essential to develop strategies to assist the prescription of antibiotics in patients identified with a label of “betalactam allergy” at hospitals, and to enhance education of patients and their caregivers, as well as of non-specialist physicians.

In face of the pandemic of the novel coronavirus disease 2019 (COVID-19), the management of patients with cardiovascular risk factors and/or disease is challenging. The cardiovascular complications evidenced in patients with COVID-19 derive from several mechanisms, ranging from direct viral injury to complications secondary to the inflammatory and thrombotic responses to the infection. The proper care of patients with COVID-19 requires special attention to the cardiovascular system aimed at better outcomes.

As complicações cardiovasculares evidenciadas nos pacientes com COVID-19 resultam de vários mecanismos, que vão desde lesão direta pelo vírus até complicações secundárias à resposta inflamatória e trombótica desencadeada pela infecção. O cuidado adequado do paciente com COVID-19 exige atenção ao sistema cardiovascular em busca de melhores desfechos.
Abstract In face of the pandemic of the novel coronavirus disease 2019 (COVID-19), the management of patients with cardiovascular risk factors and/or disease is challenging. The cardiovascular complications evidenced in patients with COVID-19 derive from several mechanisms, ranging from direct viral injury to complications secondary to the inflammatory and thrombotic responses to the infection. The proper care of patients with COVID-19 requires special attention to the cardiovascular system aimed at better outcomes.

Greek yogurt with added sorghum flours: antioxidant potential and sensory acceptance

ABSTRACT The aim of this study was to develop and evaluate the antioxidant characteristics and sensory acceptance of Greek yogurt with added whole sorghum flours (WSF) from genotypes BRS305 (tannin) and BR501 (tannin-free). Five formulations were elaborated: 1) Control (without WSF); 2) 2% BR501; 3) 4% BR501; 4) 2% BRS305 and 5) 4% BRS305 and evaluated for antioxidant capacity, total anthocyanins, total phenols, condensed tannins, sensory acceptance and purchase intention. The 4% BRS305 formulation presented higher antioxidant properties, but lower sensory acceptance. No significant difference was observed for: color, flavor and overall acceptability between the control and 2% BR501; nor for texture and overall acceptability between the control and 4% BR501. Principal component analysis explained that all variables associated with antioxidant properties were positively correlated with the first major component (PC1: 82.7%). The 2% and 4% BR501 formulations were more highly correlated with PC1 (92.7%), as well as the control in relation to sensorial characteristics. The addition of WSF BRS305 improved the antioxidant properties of yogurts and the addition of WSF BR501 did not interfere with the sensory acceptance of the formulations. The similarity of the sensory acceptance of the yogurt containing WSF from the BR 501 genotype to the control, opens perspectives for the insertion of sorghum in human food, using dairy products as suitable matrices, adding potential functionality to this type of product.
RESUMEN El trabajo tuvo como propósito desarrollar y evaluar las características antioxidantes y la aceptación sensorial de yogurt griego con adición de harinas integrales de sorgo (HIS) de los genotipos BRS305 (con taninos) y BR501 (sin taninos). Cinco formulaciones fueron elaboradas: 1) Control (sin HIS); 2) 2% BR501; 3) 4% BR501; 4) 2% BRS305 y 5) 4% BRS305. Las formulaciones fueron evaluadas en función de su actividad antioxidante, antocianinas totales, fenólicos totales, taninos condensados, aceptación sensorial e intención de compra. La formulación 4% BRS305 presentó más propiedades antioxidantes, pero menor aceptación sensorial que las otras formulaciones. No se observó diferencia significativa entre el control y el 2% BR501 para el color, sabor e impresión global, ni tampoco para textura e impresión global en relación al 4% BR501. El análisis de componente principal ha explicado que todas las variables asociadas a las propiedades antioxidantes han sido positivamente correlacionadas con el primer componente principal (PC1: 82,7%). Las formulaciones 2% y 4% BR501 se han correlacionado más con el PC1 (92,7%), así como el control con las características sensoriales. La adición de HIS BRS305 ha mejorado las propiedades antioxidantes de los yogurts y la adición de HIS BR501 no ha interferido en la aceptación sensorial de las formulaciones. La similitud de la aceptación sensorial del yogurt que contiene el HIS del genotipo BR501 con el control, abre perspectivas para la inserción del sorgo en la alimentación humana, utilizando productos lácteos como matrices adecuadas, agregando funcionalidad potencial a este tipo de producto.

Abstract Although the conservation of tropical biodiversity depends on protected areas, there is still a very large ‘gap’ of knowledge on the flora of Brazilian reserves, especially in the Northeast region of Brazil. Field and herbarium surveys of the phanerogamic flora of the Ubajara National Park, located on the Brazilian Northeast, were made and analyses on phenology and dispersal syndromes were performed. 418 taxa (213 trees and shrubs, 100 terrestrial herbs, 68 climbing plants, 33 sub-shrubs, two epiphytes, one hemiparasite and one aquatic herb) were recorded. The most representative families were: Fabaceae, Malvaceae, Asteraceae, Rubiaceae and Euphorbiaceae. The annual flowering / fruiting peak hypothesis was not fully confirmed, therefore, the forest may be an important food resource for the fauna all year long (especially in the moister region). Zoochory was the predominant dispersal syndrome in the moister area, whereas, autochory and anemochory together, predominated in the drier area.
