ABSTRACT Cardiovascular diseases stand as the leading cause of mortality among adults globally. For decades, comprehensive evidence has underscored the correlation between infections, particularly those involving the respiratory system, and an elevated risk of cardiovascular and cerebrovascular events, as well as all-cause mortality. The mechanisms through which infections heighten cardiovascular events are intricate, encompassing immune system activation, systemic inflammation, hypercoagulable states, sympathetic system activation, and increased myocardial oxygen demand. Respiratory infections further contribute hypoxemia to this complex interplay. These mechanisms intertwine, giving rise to endothelial dysfunction, plaque ruptures, myocardial depression, and heart failure. They can either instigate de novo cardiovascular events or exacerbate pre-existing conditions. Compelling evidence supports the safety of influenza, pneumococcal, herpes zoster, COVID-19 and respiratory syncytial virus vaccines in individuals with cardiovascular risk factors or established cardiovascular disease. Notably, the influenza vaccine has demonstrated safety even when administered during the acute phase of a myocardial infarction in individuals undergoing angioplasty. Beyond safety, these vaccinations significantly reduce the incidence of cardiovascular events in individuals with an augmented cardiovascular risk. Nevertheless, vaccination rates remain markedly suboptimal. This manuscript delves into the intricate relationship between infections and cardiovascular events. Additionally, we highlight the role of vaccinations as a tool to mitigate these occurrences and reduce residual cardiovascular risk. Finally, we emphasize the imperative need to optimize vaccination rates among individuals with heart diseases.

La neumonía adquirida en la comunidad debida a virus, neumococo, además de otras bacterias y microorga nismos “atípicos” afecta tanto a adultos sanos como enfermos. Resumen Los adultos fumadores con comorbilidades, y los ancianos, corren mayor riesgo de contraer infecciones y sus complicaciones. Las infecciones respiratorias comu nitarias por virus, neumococo y otras bacterias afectan tanto a adultos sanos como enfermos. Existen vacunas que el neumonólogo debe conocer y prescribir. Las ce pas blanco de la vacuna contra influenza son definidas por la OMS para el hemisferio sur considerando a las implicadas en la temporada precedente de influenza en el hemisferio norte. Su efectividad depende de la virulencia, la concordancia entre las cepas circulantes y las vacunales y la cobertura de la población. La vacuna anti-neumocócica polisacárida disponible desde 1983 está siendo reemplazada por vacunas conjugadas más eficaces para prevenir infecciones relacionadas a sero tipos presentes en las mismas. La inmunización contra SARS-CoV-2 redujo el contagio, la gravedad y la letalidad de COVID-19. La vacuna acelular contra Bordetella pertussis para adultos está presente para situaciones puntuales en el calendario para adultos, vacunarlos fortalece el control del contagio infantil. Las vacunas doble bacte riana (difteria + tétanos), y triple (doble + pertussis), y contra sarampión, varicela, rubeola, virus del papiloma humano, Haemophylus influenzae, meningococo, herpes zóster, fiebre hemorrágica argentina y fiebre amarilla, son de uso más limitado. Pronto contaremos con nuevas vacunas, como la recientemente aprobada por la FDA contra el virus sincicial respiratorio. Revisamos a través de un consenso de expertos en infecciones respiratorias las nuevas evidencias acerca de la inmunización de adultos que consultan al neumonólogo, y actualizamos así las recomendaciones sobre vacunación realizadas ocho años atrás.

Abstract Adult smokers, those with comorbidities, and the elderly, are at greater risk of contracting infections and their complications. Community acquired respiratory infections due to viruses, pneumococcus and other bac teria, affect both healthy and sick adults. There are vac cines that the pulmonologist must know and prescribe. The target strains of the influenza vaccine are defined by the WHO for the Southern hemisphere considering those involved in the previous influenza season in the Northern hemisphere. Its effectiveness depends on virulence, concordance between circulating and vaccine strains, and population coverage. The anti-pneumococcal polysaccharide vaccine available since 1983 is being replaced by more effective conjugate vaccines to pre vent infections related to serotypes present in them. Immunization against SARS-CoV-2 reduced the conta gion, severity, and lethality of COVID-19. The acellular vaccine against Bordetella pertussis for adults is present for specific situations in the adult calendar; vaccinating them strengthens the control of childhood contagion. The double (diphtheria + tetanus), and triple (double + pertussis) bacterial vaccines, and the vaccines against measles, chickenpox, rubella, human papillomavirus, Haemophilus influenzae, meningococcus, herpes zoster, Argentine hemorrhagic fever and yellow fever, are of a more limited use. Soon we will have new vaccines such as the one recently approved by the FDA against respi ratory syncytial virus. Through a consensus of experts in respiratory infections, we review the new evidence regarding the immunization of adults who consult a pulmonologist, and thus update the recommendations on vaccination made eight years ago.

Explorar la frecuencia e impacto de la violencia contra los trabajadores de salud de Argentina y compararlo con el resto de sus pares de Latinoamérica en el contexto de la pandemia por COVID-19. Materiales y métodos. Estudio de corte transversal a través de una encuesta electrónica al personal médico y no médico de Latinoamérica que desempeño tareas asistenciales desde marzo de 2020. Se utilizó una regresión de Poisson para estimar las Razones de Prevalencia crudas (RP) y ajustadas (RPa) con sus respectivos intervalos de confianza al 95%. Resultados. Un total de 3544 participantes de 19 países respondieron la encuesta; 1992 (56,0%) residían en Argentina. Entre los mismos, el 62,9% padeció al menos un hecho de violencia: 97,7% refirió violencia verbal y 11,8% violencia física. El 41,5% de los agredidos padecieron situaciones de violencia al menos una vez por semana. El personal de salud de Argentina vivenció más frecuentemente violencia que los de otros países (62,9% vs. 54,6%, p<0,001), siendo estos eventos más habituales y estresantes (p<0,05). Además, refirieron más frecuentemente haber considerado cambiar sus tareas asistenciales y/o deseos de abandonar su profesión (p<0,001). En la regresión de Poisson, los participantes de Argentina tuvieron una prevalencia de violencia mayor que los trabajadores de salud de la región (14,6%; p<0,001). Conclusiones. Existió una elevada prevalencia de violencia contra el personal de salud de Argentina durante la pandemia por COVID-19. Estos hechos tuvieron un alto impacto negativo entre quienes los padecieron. Nuestros datos sugieren que, esto podría haber sido más frecuente en Argentina respecto de otras regiones del continente.

Materials and methods. A cross-sectional study was conducted by applying an electronic survey on Latin American medical and non-medical personnel who carried out health care tasks since March 2020. We used Poisson regression to estimate crude (PR) and adjusted (aPR) Prevalence Ratios with their respective 95% confidence intervals. Results. A total of 3544 participants from 19 countries answered the survey; 1992 (56.0%) resided in Argentina. Of these, 62.9% experienced at least one act of violence; 97.7% reported verbal violence and 11.8% physical violence. Of those who were assaulted, 41.5% experienced violence at least once a week. Health personnel from Argentina experienced violence more frequently than those from other countries (62.9% vs. 54.6%, p<0.001), and these events were more frequent and stressful (p<0.05). In addition, Argentinean health personnel reported having considered changing their healthcare tasks and/or desired to leave their profession more frequently (p<0.001). In the Poisson regression, we found that participants from Argentina had a higher prevalence of violence than health workers from the region (14.6%; p<0.001). Conclusions. There was a high prevalence of violence against health personnel in Argentina during the COVID-19 pandemic. These events had a strong negative impact on those who suffered them. Our data suggest that violence against health personnel may have been more frequent in Argentina than in other regions of the continent.

Resumen Antecedentes. La identificación de pólipos gástricos de manera incidental en endoscopias de vías digestivas altas ha aumentado su incidencia, que varía entre el 0.5% y el 23%. El 10% de estos pólipos presentan síntomas y el 40% son hiperplásicos. Nos permitimos proponer una técnica laparoscópica para el manejo de los pólipos hiperplásicos gigantes asociados a síndrome pilórico no susceptibles de resección endoscópica. Método. Serie de pacientes llevados a polipectomía transgástrica laparoscópica por hallazgo de pólipos gástricos gigantes asociados a síndrome pilórico, en Bogotá, Colombia, de enero de 2015 a diciembre de 2018. Resultados. Un total de siete pacientes, el 85% de sexo femenino, con edad promedio de 51 años, ingresaron por síndrome pilórico y fueron llevados a manejo laparoscópico, con un tiempo quirúrgico promedio de 42 minutos, sangrado intraoperatorio de 7-8 cc, tolerancia a la vía oral a las 24 horas, no conversión, sin mortalidad. Conclusiones. La polipectomía transgástrica para el manejo de pólipos gástricos gigantes benignos que no pueden ser resecados por vía endoscópica resulta ser un método factible, con una baja tasa de complicaciones y sin mortalidad.

Abstract Background. The identification of gastric polyps incidentally in endoscopies of the upper digestive tract has increased its incidence, varying between 0.5% and 23%. 10% of these polyps have symptoms, 40% are hyperplastic. We allow ourselves to propose a laparoscopic technique for the management of giant hyperplastic polyps associated with a pyloric syndrome, not susceptible to endoscopic resection. Method. A series of patients approached by laparoscopic transgastric polypectomy due to the giant gastric polyps associated with pyloric syndrome, in Bogotá, Colombia, from January 2015 to December 2018. Results. Seven patients, 85% female, with an average age of 51 years, who were admitted for pyloric syndrome and were taken to laparoscopic management, with an average surgical time of 42 min, intraoperative bleeding 7-8 cc, tolerance to the oral route 24 hours, no conversion, without mortality. Conclusions. Transgastric polypectomy for the management of benign giant gastric polyps that cannot be resected endoscopically turns out to be a feasible method, with a low rate of complications and without mortality.

Resumen El suicidio es una problemática de salud mental de alto impacto en la sociedad actual, tanto en el mundo como en Colombia. Dentro de las conceptualizaciones modernas como el modelo socioecológico de la prevención del suicidio, se ha reconocido que los factores familiares son determinantes muy importantes del riesgo y la protección. El presente estudio tuvo como propósito evaluar un modelo explicativo del riesgo por ideación suicida en adolescentes que involucre las dimensiones de socialización parental y la funcionalidad familiar percibida, así como establecer la influencia del estilo de socialización de padres y madres. Para tales objetivos, se desarrolló un estudio cuantitativo, de alcance explicativo y de corte transversal. A una muestra de 268 adolescentes de 12 a 16 años, el 51.9 % de sexo femenino y 48.1 % masculino, ubicados en la ciudad de Valledupar, Colombia, se aplicó la Escala de socialización parental en la adolescencia (ESPA-29), el APGAR Familiar y la Escala de Ideación Suicida BECK-HF. Mediante SPSS 25 y AMOS 25 se evaluaron cinco modelos explicativos diferentes. El modelo de mejores indicadores de bondad de ajuste (χ. = 22.2, p = .022, CMINDF = 2.023, CFI = .968, TLI = .919, NFI = .942, RMSEA = .061) establece una influencia de la aceptación/implicación de ambos padres sobre el funcionamiento familiar, lo que reduce el nivel de ideación suicida. En este último, también inciden directamente la edad y la coerción/imposición del padre. Como conclusión, se demuestra la importancia del funcionamiento familiar, de las conductas de apoyo y afecto de la dimensión aceptación/implicación, pues presentan un efecto protector ante el riesgo suicida. De igual manera, los datos ponen de relieve que las conductas de coerción/imposición del padre, mas no de la madre, fueron un factor de riesgo. Como limitante de este estudio se presenta la no inclusión de otros factores en el modelo que podrían influir en el riesgo suicida, por lo que se sugiere realizar investigaciones que tengan en cuenta estos otros factores a nivel social e intrapersonales, pero con evaluaciones desde el modelo explicativo, que poco se evidencia en la literatura científica.

Abstract Suicide is a high-impact mental health problem in today’s society, both in the world and in Colombia: in fact, epidemiological data shows that there has been an upward trend in the suicide rate both in the country and in the city of Valledupar, which is the local context of interest. On the other hand, within modern conceptualizations such as the socio-ecological model of suicide prevention, it has been recognized that family factors are very important determinants of risk and protection. The different investigations reviewed as antecedents show congruently that family dysfunction as well as coercive paternal socialization behaviors are risk factors for suicide. Likewise, adequate family functionality and parental acceptance, and involvement behaviors are protective factors. Similarly, different studies have shown that older adolescents and females are at greater risk of suicidal behavior. The purpose of this study was to evaluate an explanatory model of the risk of suicidal ideation, which involved the dimensions of parental socialization and perceived family functionality, as well as to establish the influence of the parents’ socialization style, based on the findings of previous studies. For this purpose, a quantitative, explanatory and cross-sectional study was developed. The ESPA-29, the APGAR and the BECK-HF Suicidal Ideation Scale were applied to a sample of 268 adolescents from the city of Valledupar, Colombia. A simple random sampling was used for the selection of participants within a neighborhood of the city of Valledupar. The sample consisted of 51.9 % female and 48.1 % male adolescents. The approval of the parents was obtained through an informed consent of the legal representative, as well as an informed consent for the adolescents, which complies with the characteristics requested by the Colegio Colombiano de Psicólogos. Using SPSS 25 and AMOS 25, five different explanatory models were evaluated, with a SEM modeling-Path Analysis. The model of best goodness of fit indicators (χ. = 22.2, p = .022, CMINDF = 2.023, CFI = .968, TLI = .919, NFI = .942, RMSEA = .061) establishes an influence of acceptance/involvement of both parents on family functioning, which reduces the level of suicidal ideation; age and coercion/imposition of the father also directly affect the latter. The data demonstrate the importance of family functioning and of supportive and affective behaviors of the acceptance/involvement dimension, since they present a protective effect against suicidal risk. In the same way, the data shows that the coercion/imposition behaviors of the father, but not the mother, were a risk factor. Although the father’s authoritarian style did predict a greater risk of suicidal ideation, it was the indulgent style, and not the authoritative style, that presented the lowest risk. Although the measurements obtained by this research do not allow us to unambiguously explain why this difference occurs between the study’s findings and what is suggested by the scientific literature regarding maternal and paternal styles of socialization in the face of suicide, there are possible determining factors for the results. The possibility is raised that, at a cultural level, beliefs about the maternal role in families in the Colombian Caribbean region, which arise from a formation of matrilocal families, influence the perception of greater acceptability of punishment by the mother towards their children. On the other hand, there is the lack of evidence of significant differences in terms of suicide risk according to sex. Some directions are suggested for future research, mainly to broaden the research spectrum on the role of the family in suicide prevention and to corroborate in the context of the Colombian Caribbean region the finding about the absence of effect of coercion/imposition behaviors carried out by the mother.

ABSTRACT The aim of this study was to evaluate the usefulness of the measurement of glucose and serum fructosamine as possible indicative biomarkers for the diagnosis of canine diabetes mellitus (CDM). An analytical, descriptive, cross-sectional study was developed with a sample of 30 canines from a shelter in Cundinamarca, Colombia. The animals were without clinical signs of CDM. Data on sex, breed, age, diet, reproductive history, symptoms related to CDM and vaccination status of each animal were taken. A sample was collected from the cephalic vein while fasting for 8-12 hours and were immediately centrifuged to determine the concentration of glucose and serum fructosamine in an automated chemistry equipment. The results showed that 60% of the sampled population was male, 46.7% was 5 years old, 33.3% between 6 and 10 years old, and 20% more than 10 years old. All were consuming an exclusive diet of concentrate and all were sterilized. The results showed that 30% of canines without clinical signs of CMD presented higher glucose concentrations and fructosamine concentrations below the established biological reference interval.

RESUMEN El objetivo del estudio fue evaluar la utilidad de la medición de glucosa y fructosamina sérica como posibles biomarcadores indicativos para diagnóstico de diabetes mellitus canina (DMC). Se desarrolló un estudio analítico, descriptivo de corte transversal con una muestra de 30 caninos de un refugio en Cundinamarca, Colombia. Los animales se encontraban sin signos clínicos de DMC. Se tomaron datos de sexo, raza, edad, dieta, historia reproductiva, sintomatología relacionada con DMC y estado de vacunación de cada animal. Se recolectaron muestras en ayuno de 8-12 horas de la vena cefálica y se centrifugaron de inmediato para determinar la concentración de glucosa y fructosamina sérica en un equipo de química automatizado. El 60% de la población muestreada era macho, 46.7% tenía 5 años, 33.3% entre 6 y 10 años y 20% más de 10 años. Todos consumían una dieta exclusiva de concentrado y estaban esterilizados. Los resultados indicaron que 30% de caninos sin signos clínicos de DMC presentaron concentraciones de glucosa mayores y de fructosamina por debajo del intervalo biológico de referencia establecido.

Resumen Introducción: Los tumores del estroma gastrointestinal (GIST) son las neoplasias mesenquimales más comunes del tracto gastrointestinal. El tratamiento estándar para los GIST localizados es la resección quirúrgica completa. Para este procedimiento, el abordaje cooperativo laparo-endocopico (ACLE) se ha propuesto como una alternativa segura y eficaz. Queremos mostrar nuestra experiencia con la técnica ACLE para el manejo de GIST y unzaa revisión de la literatura. Material y métodos: Se realizó un estudio retrospectivo, transversal, que incluyó pacientes con diagnóstico de GIST localizado tratados con la técnica ACLE entre enero de 2011 y diciembre de 2018. Resultados: Durante el periodo de estudio se manejaron 21 pacientes con técnica de ACLE, con una tasa hombre-mujer de 3:1. El tiempo quirúrgico promedio fue de 98.5 min, con 100% de bordes negativos en todos los pacientes. El sangrado intraoperatorio fue de 30.7 cc y todos los pacientes tuvieron tolerancia oral en las primeras 24H. Ninguno de ellos requirió manejo en UCI, no se reportó mortalidad. Conclusiones: La técnica ACLE ha demostrado ser una técnica viable, segura y eficaz para el manejo de los GIST gástricos. Mostrando superioridad en la preservación de la función orgánica y en los magines oncológicos. Son necesarios estudios prospectivos para conocer el resultado de los pacientes manejados mediante la técnica ACLE.

Abstract Introduction: Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract. The standard management for localized GIST is the complete surgical resection. For this procedure, laparoendoscopic cooperative surgery (LECS) has been proposed as a safe and effective alternative. We want to show our experience with LECS technique for the management of GIST and a literature review. Materials and methods: A retrospective, cross-sectional study was carried out, which included patients with a diagnosis of localized GIST treated with LECS technique between January 2011 and December 2018. Results: During the period of the study, 21 patients were managed by LECS technique, with a male-female rate 3:1. Average surgical time was 98.5 min, 100% negative borders in all patients. Intraoperative bleeding was 30.7 cc and all patients had orally tolerance in the first 24 h. None of them required ICU management, no mortality was reported. Conclusions: The LECS technique has demonstrated to be a viable, safe, and effective technique for the management of gastric GIST's, showing superiority in organ function preservation and in the range of oncological margins. Prospective studies are necessary to obtain knowledge about the outcome of patients managed through LECS technique.

Resumen ANTECEDENTES: La dehiscencia de la cúpula vaginal es una complicación rara de la histerectomía, secundaria a factores de riesgo que alteran la cicatrización. Su incidencia y prevalencia son bajas y cuando ocurre es una urgencia quirúrgica. OBJETIVO: Reportar el caso clínico de una paciente con evisceración intestinal transvaginal y revisar el procedimiento diagnóstico y terapéutico y compararlo con lo registrado en la bibliografía. CASO CLÍNICO: Paciente de 67 años, con un cuadro clínico de cuatro horas de dolor abdominal asociado con sensación de masa vaginal. Antecedente de cirugía oncológica. En el examen físico se identificó una evisceración intestinal, sin necrosis. Durante el procedimiento quirúrgico se evidenció un defecto en la cúpula vaginal de tres centímetros. Por acceso abdominal y vaginal se efectuó la reparación, sin complicaciones. En el posoperatorio cursó con desaturación de oxígeno multifactorial, se trató, controló y se dio de alta del hospital. Acudió a los controles posoperatorios en los que se la encontró con adecuada evolución clínica. CONCLUSIÓN: La evisceración transvaginal es poco frecuente, cuando ocurre requiere diagnosticarse y tratarse de inmediato, para contrarrestar las complicaciones asociadas. En este caso, la paciente recibió atención multidisciplinaria con adecuada evolución posoperatoria. En virtud de la baja incidencia no se dispone de evidencia suficiente para recomendar una vía de acceso ideal para el tratamiento quirúrgico.

Abstract BACKGROUND: Vaginal vault dehiscence is a rare complication of hysterectomy, secondary to risk factors that alter healing. Its incidence and prevalence are low and when it occurs it is a surgical emergency. OBJECTIVE: To report the clinical case of a patient with transvaginal intestinal evisceration and to review the diagnostic and therapeutic procedure and compare it with that reported in the literature. CLINICAL CASE: A 67-year-old female patient with a four-hour clinical picture of abdominal pain associated with vaginal mass sensation. History of previous oncologic surgery. Physical examination revealed intestinal evisceration without necrosis. During the surgical procedure a three centimeter defect of the vaginal vault was evidenced. The repair was performed by abdominal and vaginal access, without complications. Postoperatively she presented with multifactorial oxygen desaturation, was treated, monitored and discharged from the hospital. She attended postoperative controls in which she was found with adequate clinical evolution. CONCLUSION: Transvaginal evisceration is rare, when it occurs it needs to be diagnosed and treated immediately, to counteract associated complications. In this case, the patient received multidisciplinary care with adequate postoperative evolution. Due to the low incidence, there is insufficient evidence to recommend an ideal access route for surgical treatment.

Abstract Introduction and objectives: Although cardiologists frequently assist patients who suffer damage from smoking, the degree of training they receive to manage this problem during their residency is unknown. Because of this, we’d proposed to evaluate the preferences and practices of cardiology residents for smoking cessation of the attending patients. Materials and methods: Closed, prefixed, voluntary and anonymous survey among doctors who carried out the specialty of cardiology in 5 countries of Latin America and Spain. Results: 716 residents were surveyed; 62.4% from Argentina, 19% from Mexico, 6.8% from Spain, 6.7% from Chile, 3.2% from Uruguay, and 1.9% from Paraguay. When asked about the importance they assigned to this problem (using a scale of 1-10), 85.8% assigned this question a score of 8 or higher. While 80.5% of the participants expressed giving short anti-tobacco advice routinely, only 27.7% used pharmacological therapy for this purpose. Among those who did not use pharmacological therapy, 58.3% said that the reason was not being familiar with the treatments; 62.9% of the surveyed said they had not received any type of training in this problem. Those residents who received some type of training reported feeling more prepared for this (p < 0.0001). Conclusion: We found that cardiology residents have a low knowledge of pharmacological treatment and relatively low confidence to provide assistance in smoking cessation. This topic should be included in the training of future cardiologists in order to achieve a more comprehensive cardiovascular prevention.

Resumen Introducción y objetivos: Si bien los cardiólogos asisten cotidianamente a pacientes que sufren daño por el tabaquismo, no se conoce el grado de formación que reciben sobre esta problemática durante su residencia. Debido a ello nos propusimos evaluar las preferencias y prácticas de los residentes de cardiología para la cesación tabáquica de los pacientes que asisten. Materiales y métodos: Encuesta cerrada, prefijada, voluntaria y anónima entre médicos que realizaban la especialidad de cardiología en cinco países de Latinoamérica y España. Resultados: Se encuestaron 716 residentes: un 62.4% de Argentina, un 19% de México, un 6.8% de España, un 6.7% de Chile, un 3.2% de Uruguay y un 1.9% de Paraguay. Con respecto a la importancia que asignaban a esta problemática (empleando una escala de 1-10), el 85.8% le asignó a esta pregunta una puntuación de 8 o mayor. Mientras el 80.5% de los participantes expresó dar consejo breve antitabáquico sistemáticamente, solamente un 27.7% empleaban terapia farmacológica con este fin. Entre quienes no empleaban terapia farmacológica, el 58.3% manifestó que el motivo era no encontrarse familiarizados con los tratamientos. El 62.9% de los encuestados dijo no haber recibido ningún tipo de formación en esta problemática. Aquellos residentes que recibieron algún tipo de formación manifestaron sentirse más preparados (p < 0.0001). Conclusión: Encontramos un bajo conocimiento sobre el tratamiento farmacológico y relativamente poca seguridad por parte de los residentes de cardiología para brindar asistencia en cesación tabáquica. Consideramos esencial incluir este tópico en la formación de los futuros cardiólogos a fin de lograr una prevención cardiovascular más integral.

Resumen Antecedentes: Las complicaciones tras la colangiopancreatografía retrógrada endoscópica (CPRE) que necesitan manejo quirúrgico son limitadas y aproximadamente el 0.09-1.8% corresponden a perforación duodenal. Este tipo de lesiones requieren un diagnóstico temprano y un manejo adecuado, ya que los desenlaces pueden ser catastróficos, con una mortalidad reportada de hasta el 20%. Objetivo: Presentar nuestra experiencia en el manejo quirúrgico de estas lesiones. Método: Estudio observacional, retrospectivo y descriptivo, de pacientes que requirieron manejo quirúrgico por perforación duodenal tras CPRE en tres centros de Bogotá, Colombia, entre enero de 2013 y diciembre de 2018. Resultados: Fueron llevados a manejo quirúrgico 13 pacientes, 7 (53.8%) por vía laparoscópica, con una estancia hospitalaria promedio de 10.7 días y una mortalidad del 0%, y 6 (46.1%) por vía abierta, con una estancia hospitalaria promedio de 33 días y una mortalidad del 50%, con seguimiento mínimo a 6 meses. Conclusiones: La perforación duodenal tras una CPRE, pese a su baja incidencia, se debe sospechar por su alta morbimortalidad. La decisión del abordaje quirúrgico está condicionada por el estado clínico del paciente, y por ende está ligada al pronóstico de este.

Abstract Background: Complications after endoscopic retrograde cholangiopancreatography (ERCP) are rare, approximately 0.09% to 1.8% are duodenal perforation. This type of injury requires early diagnosis and proper management since the outcomes can be catastrophic, leading to death in 20% of cases. Objective: To show our experience in the management of these injuries in order to establish if there is benefit from minimally invasive management versus conventional management. Method: Observational, retrospective and descriptive study comparing patients who required surgical management after post-ERCP duodenal perforation in three centers in Bogotá, Colombia, between January 2013 and December 2018. Results: 13 patients were taken to surgical management after duodenal perforation post ERCP, 7 laparoscopically (53.8%), with an average hospital stay of 10.7 days and mortality of 0% and 6 by open route (46.1%), with an average hospital stay of 33 days and mortality of 50%, with a minimum follow-up of 6 months. Conclusions: Post ERCP duodenal perforation, despite its low incidence, should be suspected due to its high mortality. Laparoscopic surgery is a good tool to address duodenal perforations after ERCP, because it presents a lower rate of complications and mortality.

Resumen Introducción. El aneurisma aórtico abdominal roto, tiene una mortalidad del 80 % al 90 %. Para su reparación existe una técnica abierta y otra endovascular, las cuales tienen diferencias entre sus beneficios y complicaciones. El método de elección en la actualidad para la corrección de esta alteración anatómica es endovascular, sin embargo, no es el más usado, porque no se cuenta todo el tiempo con el equipo humano de cirugía vascular para su realización. Caso clínico. Ingresa a urgencias un paciente en estado de shock de origen desconocido, con dolor abdominal de 24 horas de evolución. Se realiza una tomografía con contraste que demuestra un aneurisma aórtico abdominal infrarrenal roto. Debido a que no se contaba con el equipo de cirugía vascular, es llevado de urgencia a una corrección abierta que duró 153 minutos, con un sangrado intraoperatorio de 1754 cc. Fue dado de alta a los 12 días postoperatorios sin ninguna complicación. Discusión. La reparación endovascular del aneurisma aórtico abdominal roto es la mejor elección, ya que muestra mayores beneficios en comparación con la reparación abierta, sin embargo, no es el más utilizado, porque se necesita de un personal bien entrenado en cirugía endovascular, por lo que, dentro de la formación de los cirujanos generales, se tiene que incluir el aprendizaje de las técnicas abiertas.

Abstract Introduction. The ruptured abdominal aortic aneurysm has a mortality of 80% to 90%. There is an open and an endovascular techniques for its repair, which have differences between their benefits and complications. The method of choice for the correction of this anatomical alteration is endovascular; however, it is not the most frequently used, mainly because the vascular surgical team is not available all the time to perform it. Clinical case. A patient in a state of shock of unknown origin was admitted to the emergency room, with abdominal pain of 24 hours of evolution. A contrast-enhanced CT scan demonstrated a ruptured infrarenal abdominal aortic aneurysm. Due to the lack of a vascular surgery team, the patient was rushed for an open surgery that lasted 153 minutes, with an intraoperative bleeding of 1754 cc. He was discharged 12 days after surgery without any complications. Discussion. Endovascular repair of ruptured abdominal aortic aneurysm is the best choice, since it shows greater benefits compared to open repair. However, it is not the most widely used because it requires well-trained personnel in endovascular surgery. Therefore, learning of open techniques must be included in the training of general surgeons.

Resumen Objetivos: Presentar el manejo laparoscópico en un caso de bezoar atípico y una revisión de la literatura. Materiales y métodos: Se presenta el caso de un paciente de sexo masculino de 67 años con síndrome pilórico debido a una obstrucción intestinal por cuerpo extraño. Resultados: Se encuentra como hallazgo endoscópico un bezoar atípico (bezoar de dinero) impactado en la región prepilórica sin posibilidad de resolución por este medio, por lo cual se considera el manejo laparoscópico. Discusión: Los bezoares se definen como cualquier objeto el cual tuvo una ingesta voluntaria o involuntaria, que se aloja en alguna parte del tracto gastrointestinal superior, con mayor frecuencia a nivel gástrico, y no se puede digerir por los mecanismos fisiológicos del cuerpo; además, se clasifican según su composición. Conclusiones: En pacientes con obstrucción intestinal alta debido a cuerpos extraños en los cuales el manejo endoscópico falla, el manejo quirúrgico mínimamente invasivo con cirugía laparoscópica es viable y eficaz.

Abstract Objectives: To describe the laparoscopic management of an atypical bezoar case and present a literature review. Materials and methods: This is the case of a 67-year-old male patient with pyloric stenosis due to intestinal obstruction by a foreign body. Results: The endoscopic finding was an atypical bezoar (Money bezoar) in the prepyloric region with no possible resolution by this route, so laparoscopic treatment was considered. Discussion: Bezoars are defined as any object that was voluntarily or involuntarily swollen and is obstructing some part of the upper gastrointestinal tract, usually the stomach, and cannot be digested using the physiological mechanisms of the body. They are categorized based on their composition. Conclusions: When endoscopic treatment fails to relieve upper gastrointestinal tract obstruction caused by foreign bodies, minimally invasive surgical treatment with laparoscopic surgery is a viable and efficient option.
