Resumo Devido à sua eficácia e aos seus perfis de segurança, os anticoagulantes orais diretos (DOACs) tornaram-se o padrão de cuidado para a terapia aguda e de longo prazo de tromboembolismo venoso (TEV). As diretrizes da Sociedade Internacional de Trombose e Hemostasia de 2021 recomendam o uso de dosagens padrão de DOACs em pacientes com índice de massa corporal (IMC) > 40 kg/m2 ou peso > 120 kg. O uso de DOACs em pacientes com obesidade mórbida e TEV, incluindo embolia pulmonar aguda, ainda não foi esclarecido. Abstract Direct oral anticoagulants (DOACs) have become the standard of care for acute and long-term therapy for venous thromboembolism (VTE) due to their efficacy and safety profiles. The 2021 International Society on Thrombosis and Haemostasis guidelines recommend using standard DOAC dosages in patients with BMI >40 kg/m2 or weight >120 kg. Use of DOACs remains uncertain in morbidly obese patients with VTE, including acute PE. A morbidly obese woman in her 30s who presented with acute worsening of dyspnea was diagnosed with acute intermediate-high risk acute pulmonary embolism and concomitant proximal deep vein thrombosis, constituting a clinically challenging scenario for treating her with rivaroxaban. Standard doses of rivaroxaban for acute and extended phase treatment of venous thromboembolism in individuals with morbid obesity at BMI>70 kg/m2 may be effective, and safe. Abstract Severe coronavirus disease 2019 (COVID-19) is caused by the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) and predisposes to thrombotic complications. In this review, we address the close relationship between venous thromboembolism and COVID-19, providing practical aspects on epidemiology, risk factors, and thromboprophylaxis, as well as potential options of anticoagulation. The scientific data on COVID-19 is scarce; however, day by day we continue to learn, being aware of the evolving and dynamic changes in this emerging infectious and immunothrombotic disease.

Resumen La enfermedad grave por coronavirus 2019 (COVID-19) está causada por el Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) y predispone a complicaciones trombóticas. En esta revisión se aborda de manera práctica la estrecha relación entre la tromboembolia venosa y la COVID-19, enfatizando aspectos epidemiológicos, factores de riesgo y tromboprofilaxis, así como potenciales opciones de anticoagulación. Actualmente la evidencia científica es muy escasa, pero día a día seguimos aprendiendo, estando atentos a cambios novedosos y dinámicos en esta enfermedad infecciosa e inmunotrombótica emergente.

Abstract Acute pulmonary embolism represents a frequent cause of cardiovascular morbidity and mortality, only exceeded by acute coronary syndromes and cerebrovascular disease. The start-up and implementation of a designated pulmonary embolism response team is necessary to improve prognosis and minimize long-term sequelae in the subgroup of patients with significant pulmonary embolism. Herein, we describe and discuss an overview of the current and potential role of pulmonary embolism response teams, with a focus on the perioperative period.

Resumen La tromboembolia pulmonar aguda representa una causa frecuente de morbimortalidad cardiovascular, sólo rebasada por los síndromes coronarios agudos y la enfermedad cerebrovascular. El inicio y la intervención de un equipo multidisciplinario de respuesta rápida en la tromboembolia pulmonar son imperantes para mejorar el pronóstico y reducir al mínimo las posibles secuelas en el subgrupo de pacientes más graves. En este artículo de revisión se describe y revisa de manera general el papel actual y potencial que tienen dichos equipos de respuesta rápida, con un enfoque particular en el perioperatorio.

Platelets store more than 20 growth factors in their granules. Their secretion promotes wound healing and repair. The intricate wound healing process involves not only the angiogenic platelet factors but also de anti-angiogenic ones. The process of platelet preservation, such as freezing, disrupt the platelet membrane structure and their angiogenic properties causing platelet activation. In the field of wound healing and the management of recalcitrant wounds in debilitated and chronically ill patients, there have been several studies. These studies have focused on the use of the different platelet preparations such as fresh frozen plasma, frozen-dried platelet rich plasma, sonicated platelet rich plasma and the use of alogenic acelular dermis. Platelet preparations posses positive effects on wound healing with little alterations of the final scar tissue. In this article we will describe and discuss the studies that have involved the use of these platelet rich preparations in the filed of wound healing and we will also state the road blocks and the studies that will come in the near future regarding the use of platelets products in the field wound healing.

Las plaquetas poseen más de 20 factores de crecimiento en sus gránulos. Su secreción promueve la reparación de heridas. La cicatrización es un intrincado proceso en el cual intervienen no solo los factores plaquetarios proangiogénicos sino también factores antiangiogénicos. La preservación por congelación de las plaquetas empleada para su preservación, desestabiliza y activa la membrana plaquetaria alterando sus propiedades. En el campo del tratamiento de heridas recalcitrantes en pacientes debilitados o con enfermedades crónicas, el uso de preparaciones plaquetarias en forma de plasma fresco congelado, las preparaciones plaquetarias sonicadas, las preparaciones congeladas y deshidratadas y el uso de fragmentos de dermis alogénica, confiere efectos beneficiosos, logrando mejores tiempos de cierre en las heridas y menor alteración de la morfología de la cicatriz final. En el presente artículo, describiremos los estudios que han demostrado estos efectos, así como las encrucijadas actuales y los estudios que debemos esperar en un futuro no lejano.
