Resumen: Este artículo estimó la prevalencia de diabetes gestacional en Brasil. Se realizó una revisión sistemática y metaanálisis con artículos publicados del 2010 al 2021 en las bases de datos PubMed, Scopus, Google Scholar, SciELO, LILACS y Biblioteca Virtual en Salud, además de literatura gris. Los datos se extrajeron usando un instrumento estandarizado junto con el instrumento de evaluación del riesgo de sesgo de Hoy et al. Posteriormente, se desarrolló un metaanálisis con varianza robusta y efectos aleatorios. La heterogeneidad se verificó mediante el I2, y el sesgo de publicación se evaluó por medio del gráfico en embudo y la prueba de Egger. Se realizaron análisis de subgrupos para determinar la prevalencia según el riesgo de sesgo, criterios diagnóstico y regiones del país. En total, se incluyeron 32 estudios en este metaanálisis, con un total de 21.942 mujeres. La prevalencia combinada de diabetes gestacional en Brasil fue del 14% (IC95%: 11,0; 16,0), considerablemente más alta que las estimaciones de estudios anteriores. Con relación al riesgo de sesgo, los estudios con riesgo bajo, medio y alto mostraron una prevalencia combinada del 12%, del 14% y del 14%, respectivamente. En cuanto a la certeza de la evidencia (enfoque GRADE), la clasificación general fue baja. La mayoría de los estudios utilizó los criterios del Grupo de Estudio de la Asociación Internacional de Diabetes en el Embarazo (IADPSG) y del IADPSG adaptado, lo que muestra una prevalencia combinada del 15% y del 14%, respectivamente. Con relación a las regiones, la prevalencia combinada fue mayor en el Sudeste (14%) y menor en el Centro-Oeste (9%). Esta fue la primera revisión sistemática que proporcionó evidencias sobre la prevalencia de diabetes gestacional en el ámbito nacional, lo que demuestra una considerable heterogeneidad entre los artículos y la influencia de la región, los criterios diagnósticos y la calidad de los estudios sobre este indicador.

ABSTRACT This study aimed to identify factors associated with colonization by community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) in adult patients admitted to a Brazilian hospital. This is a cross-sectional study, in which patients underwent a nasal swab and were asked about hygiene behavior, habits, and clinical history. Among the 702 patients, 180 (25.6%) had S. aureus and 21 (2.9%) MRSA. The factors associated with MRSA colonization were attending a gym (OR 4.71; 95% CI; 1.42 – 15.06), smoking habit in the last year (OR 2.37; 95% CI; 0.88 – 6.38), previous hospitalization (OR 2.18; CI 95%; 0.89 – 5.25), and shared personal hygiene items (OR 1.99; 95% CI; 0.71 – 5.55). At the time of admission, colonization by CA-MRSA isolates was higher than that found in the general population. This can be an important public health problem, already endemic in hospitals, whose factors such as those associated with habits (smoking cigarettes) and behaviors (team sports practice and activities in gyms) have been strongly highlighted. RESUMO A desnutrição/sarcopenia é frequente em pacientes com doenças inflamatórias intestinais (DII), resultando em catabolismo muscular, com impacto nas respostas aos tratamentos, complicações cirúrgicas e na qualidade de vida. Objetivo: Este estudo tem como objetivo, avaliar se o ângulo de fase (AF) é um parâmetro para a predição de redução de massa muscular em pacientes com DII. Métodos: Pacientes adultos com DII foram incluídos neste estudo transversal. A estimativa da massa muscular e o cálculo do AF foram realizados a partir do exame de bioimpedância elétrica (BIA). As atividades da doença de Crohn e retocolite ulcerativa foram definidas pelo índice Harvey-Bradshaw e escore parcial de Mayo, respectivamente. A área de curva ROC foi calculada para identificar o ponto de corte do AF para a massa muscular reduzida. Resultados: A amostra foi composta por 145 pacientes, sendo 39 (26.9%) com DII em fase ativa. Houve correlação do AF com massa muscular esquelética (MME) (rs 0.35, P<0.001) e com o índice de massa muscular esquelética (IMME) (rs 0.427, P<0.001), mantendo-se as associações na forma mais ativa (moderada ou grave) da DII. A análise da curva ROC indicou que os pontos de corte de AF ≤5.042 para mulheres e ≤6.079 para homens podem ser usados para prever a redução da massa muscular. ABSTRACT Malnutrition/sarcopenia is frequent in patients with inflammatory bowel diseases (IBD), and results in muscle catabolism, impacting treatment response, postoperative complications, and quality of life. Objective: This study aims to assess whether the phase angle (PhA) is a parameter for predicting reduced muscle mass in patients with IBD. Methods: Adult patients with IBD were included in this cross-sectional study. For the estimation of muscle mass and the calculation of the PhA, we used bioelectrical impedance analysis (BIA). Crohn’s disease (CD) and ulcerative colitis (UC) activity scores were defined using the Harvey-Bradshaw index and partial Mayo score, respectively. The area under the ROC curve was calculated to identify the PhA cut-off point for reduced muscle mass. Results: The sample consisted of 145 patients, with 39 (26.9%) with IBD in the active phase. There was a correlation of the PhA with skeletal muscle mass (SMM) (rs 0.35, P<0.001) and with the skeletal muscle mass index (SMI) (rs 0.427, P<0.001), and the associations remained in the most active form (moderate or severe) of IBD. The ROC curve analysis indicated that the cut-offs points of the PhA ≤5.042 for female and PhA ≤6.079 for male can be used to predict muscle mass reduction. Conclusion: The PhA can be considered a predictor of muscle mass reduction in IBD patients, and we can use it for screening and monitoring the evolution of malnutrition. Abstract Agricultural research and extension are key to promoting innovation and rural development processes. In this article, we analyze the conceptions of research, extension and their link present in different institutional documents of INTA, the main Argentine institution of agricultural innovation and rural development. The analysis shows that the conceptions of research, extension and the link among them present in the institutional documentation are organized around two different models of innovation. The first is based on positivism. We named it ‘contextualized transfer of technologies’. In it, although the needs and feedback of farmers are valued, the knowledge is produced by specialists and transferred to the producers. In the ‘territorial constructivist’ model, innovation is related to social learning resulting from the horizontal interaction among multiple actors. It prioritizes knowledge for action and the local adaptation of knowledge in the context of use. The analyzed documents fail to integrate both models, since they oscillate between making the differences invisible and considering them from an evolutionary perspective that denies the legitimacy of contextualized transfer of technologies. Resumen La investigación agropecuaria y la extensión resultan clave para impulsar procesos de innovación y desarrollo rural. En este artículo nos proponemos analizar las concepciones de investigación, extensión y de su vinculación presentes en diversos documentos institucionales del INTA, principal institución argentina de innovación agropecuaria y desarrollo rural. El análisis muestra que las concepciones de investigación, extensión y del vínculo entre ellas presentes en los documentos institucionales se organizan a partir de dos modelos de innovación. Al primero, que se apoya en el positivismo, lo denominamos ‘difusionista contextualizado’. En él, si bien se valoran las necesidades y la retroalimentación de los productores, el conocimiento es producido por especialistas y transferido a los productores. En el modelo ‘territorial constructivista’ la innovación está relacionada con el aprendizaje social resultante de la interacción horizontal entre múltiples actores. En él se prioriza el conocimiento para la acción y la adaptación local de los conocimientos en el contexto de uso. ABSTRACT The objective of this study was to evaluate the effects of prebiotics, alone and in combination with antibiotics, on broiler performance, carcass yield, intestinal permeability, and intestinal morphometry. A total of 1440 day-old male Cobb 500 chicks were distributed in a completely randomized design, with four treatments and 10 replications per treatment. The treatments used were a basal diet without prebiotics and antibiotics (BD), a basal diet with antibiotics (AB), a basal diet with prebiotics (PRE), and a basal diet with antibiotics and prebiotics (AB+PRE). To perform the treatment, the antibiotic zinc bacitracin and a prebiotic of S. cerevisiae-derived mannan-oligosaccharides and beta-glucans were used. The treatments did not affect the performance of birds from 1 to 7 and 1 to 21 days of age. From 1 to 35 and 1 to 42 days, birds fed the AB+PRE treatment had higher feed intake than those fed the AB treatment. From 1 to 35 days, birds fed the AB+PRE treatment showed a worse feed conversion ratio. However, there was no difference in carcass yield between treatments. A higher number of goblet cells was observed in the duodenum and ileum of birds fed AB+PRE, but lower counts were obtained in birds fed AB. Intestinal histomorphometry displayed similar responses between both ages regarding antibiotic and prebiotic treatments, except in the ileum at 42 days, in which the antibiotic elicited a better response. Intestinal permeability analysis showed no significant difference between diets. ABSTRACT Asbestos was largely used in Brazil. It is a mineral that induces pleural and pulmonary fibrosis, and it is a potent carcinogen. Our objective was to develop recommendations for the performance of adequate imaging tests for screening asbestos-related diseases. We searched peer-reviewed publications, national and international technical documents, and specialists’ opinions on the theme. RESUMO Introdução: A implementação da Política Nacional de Saúde Integral de Lésbicas, Gays, Bissexuais, Travestis e Transexuais (PNSI-LGBTT) em 2011 e de outros marcos legais destaca a importância da inclusão da identidade de gênero e da orientação sexual na formação em saúde. Objetivo: Nesse contexto, o presente estudo tem como objetivo apresentar a presença da diversidade sexual e de gênero nas Diretrizes Curriculares Nacionais (DCN) dos cursos de graduação em saúde. Método: A coleta de dados foi realizada por meio de documentos de acesso público disponíveis no site do MEC. Examinaram-se 22 DCN estabelecidas nas últimas duas décadas, com foco nos registros de direitos humanos, identidade de gênero e orientação sexual. Resultado: Houve um aumento progressivo na inclusão das minorias sexuais nas DCN ao longo das duas últimas décadas, sobretudo nos últimos dez anos. ABSTRACT Introduction: The implementation of the National Policy for Comprehensive Health for Lesbians, Gays, Bisexuals, Transvestites and Transsexuals (PNSI-LGBTT) in 2011, together with other legal frameworks, highlights the importance of including Gender Identity and Sexual Orientation in health training. In this context, the present study aims to demonstrate the presence of sexual and gender diversity in the National Curricular Guidelines (DCN) of undergraduate health courses. Method: Data collection was carried out through publicly accessible documents available on the MEC website. Twenty-two DCNs established in the last two decades were assessed, focusing on records of Human Rights, Gender Identity and Sexual Orientation. Results: There has been a progressive increase in the inclusion of sexual minorities in the DCNs over the last two decades, especially in the last ten years. A notable growth in records of Human Rights, Gender Identity and Sexual Orientation stands out, reflecting a significant change in the panorama of curricular guidelines. Discussion: The results show a positive trend towards the inclusion of topics related to sexual and gender diversity in the DCNs in the health area. This advance suggests greater awareness and recognition of the importance of inclusive training for health professionals, promoting equity and social justice. Abstract The present study aimed to investigate the contextual inequalities of specialized public dental care (SPDC) in Brazil. The outcome was the trajectory of dental specialized production in municipalities with SPDC (from 2015 to 2017) obtained by group-based trajectory modeling. A Poisson regression model was used to analyze the factors associated with the high trajectory of SPDC production. The inequality indicators for SPDC production were the slope index and the concentration index according to contextual factors. The study included 954 SPDC units distributed across 893 municipalities. Among the municipalities evaluated, 62.9% had a low trajectory of SPDC. Large-sized municipalities had the highest production (IRR = 2.84, 95%CI: 1.94–4.14) and the southern region had the lowest production (IRR = 0.73, 95%CI: 0.58–0.92). Municipalities presenting a very high human development index (HDI) showed the greatest SPDC production (IRR = 3.34, 95%CI: 1.09–10.24), as well as municipalities with the highest tertile of schooling rate (IRR = 1.23, 95%CI: 1.00–1.50). The absolute inequality was 52.1 percentage points for the average monthly wage (p < 0.001), 61.0 percentage points for the HDI (p < 0.001), -22.1 for infant mortality rate (p <0.001), and 14.8 for the schooling rate (p = 0.012). ABSTRACT This study aimed to evaluate the effect of including an emulsifier (soybean lecithin) and two lipid sources (soybean oil and chicken fat) on the performance and intestinal morphology of broiler chickens. A total of 576 one-day-old Cobb® 500 male chicks were housed in a completely randomized design, with treatments divided in a 2 × 2 factorial arrangement (with and without emulsifier and two lipid sources – soybean oil or chicken fat), totaling four treatments with eight replications and 18 chicks per plot. The experimental diets were formulated to be isoenergetic and isoproteic. The use of chicken fat improved the live weight at the starter rearing stage. However, the use of the emulsifier improved the live weight at the grower and finisher phases. No interaction effect was observed between treatments for performance. The intestinal morphology showed an increase in duodenal villus height with the use of chicken fat and emulsifier. The ileum had an increase in villus height in chickens fed the diet with soybean oil and emulsifier. ABSTRACT. The objective of this study was to determine the agronomic characteristics, chemical composition and in situ degradability of different sorghum genotypes. The experiment was conducted at the National Maize and Sorghum Research Center - Sete Lagoas, MG. For agronomic and nutritional characteristics, a randomized block design was used, and for in situ degradability, a split-plot design was applied. Genotypes 13F2006, BRS655 and Volumax showed lower dry matter productivity compared to 12F02006 and 12F03033. Genotype BRS655 had the lowest levels of neutral detergent-insoluble fiber (NDF), acid detergent-insoluble fiber (ADF) and lignin. Furthermore, no difference was found for potential degradability (PD), indigestible fraction (IF), and effective degradability (ED) at 2; 5 and 8% hour-1 DM among the studied materials. RESUMO A qualidade da distribuição da água e os coeficientes que expressam a eficiência do uso da água são avaliados a partir da distribuição da lâmina de irrigação (I). No entanto, foi proposta a hipótese de que pode ser mais eficaz basear essas análises na lâmina de água armazenada no perfil do solo (H), após a irrigação. Para avaliar esta hipótese, em solo muito argiloso, o feijão foi cultivado sob irrigação por aspersão convencional, com quatro cenários de uniformidade de aplicação de água. Durante o ciclo da cultura foram realizadas irrigações programadas de acordo com um limite inferior do potencial hídrico do solo. ABSTRACT The quality of water distribution and the coefficients that express water use efficiency are evaluated from the distribution of the irrigation depth (I). However, a hypothesis has been proposed that it may be more effective to base these analyses on the water depth stored in the soil profile (H), after irrigation. To evaluate this hypothesis, in a very clayey soil, bean was cultivated under irrigation by conventional sprinkling, with four scenarios of water application uniformity. During the crop cycle, irrigations were carried out, scheduled according to a lower limit of soil-water potential. The four scenarios were associated with 64, 144, 192 and 256 experimental points, which were characterized as having excellent (S1), adequate (S2), inadequate (S3) and unacceptable (S4) uniformity, according to obtained values of Christiansen’s uniformity coefficient (CUC) and distribution uniformity coefficient (DUC). ABSTRACT Adjustments in the nitrogen fertilization recommendation in wheat fields are essential to promote an increase in nitrogen (N) use efficiency by the crop. The real-time estimation of the nutritional demand of plants considering the spatial variability is one of the efficient ways to perform this adjustment. This study aimed to develop a model for the indication of topdressing nitrogen fertilization as a function of nutritional N demand using NDVI. Data from field experiments conducted in four regions of the State of Rio Grande do Sul, Brazil, during the 2015, 2017, 2018, and 2019 agricultural years using cultivars with significant sowing areas in the state, were used to construct the model. The analysis of the relationship between the amounts of N accumulated in the shoot and NDVI values allowed establishing a single model for different cultivars. The proposed model is sensitive to capturing regional edaphoclimatic differences and variability in N dynamics in cultivated areas. A consistent relationship was found between NDVI values and biomass and N content, which supported the generation of the topdressing N indication model for wheat. Hematological problems are associated with Coronavirus disease 2019 (COVID-19). Respiratory impairment is the higher point studied, although without experimental studies related to the oxygen transport performed by erythrocytes. Therefore, we decided to investigate if erythrocytes from COVID-19 patients have their functionality changed. The case-control study included hospitalized patients with a positive real-time polymerase chain reaction (RT-PCR) result admitted to University Hospital. Volunteers (negative RT-PCR results) were recruited as a control group. Thus, we assessed different erythrocytes parameters, oxidative stress markers, and biophysical studies using whole blood and isolated hemoglobin. We found a decrease of 51% in oxygen uptake and reduced antioxidant enzyme activity in COVID-19 patients compared to controls. Raman spectrometry showed structural changes in the hemoglobin and lipids of the erythrocytes from COVID-19 patients; thus, these results were consolidated with an increase in Young’s modulus in erythrocytes followed by morphology changes. Besides, isolated hemoglobin from COVID-19 patients has a distinct interaction profile using a ligand model compared to the control. RESUMO A piramutaba, Brachyplatystoma vaillantii é um bagre de água doce que representa o recurso pesqueiro mais abundante no estuário amazônico. O arrasto da piramutaba é feito em escala industrial, caracterizado pela presença de muitas espécies de água doce e marinha capturadas de forma incidental, com e sem valor comercial. Aqui descrevemos a captura incidental da pesca industrial de piramutaba no estuário amazônico e avaliamos a relação de dois fatores ambientais, profundidade e salinidade, com a captura incidental de espécies de água doce e marinhas nas estações chuvosa e seca. ABSTRACT The piramutaba, Brachyplatystoma vaillantii is a freshwater catfish that is the most abundant fishery resource in the Amazon estuary. Piramutaba trawling is done on industrial fishing scale and is characterized by the presence of many freshwater and marine bycatch species, with and without commercial value. Here we describe the bycatch of the industrial fishery of piramutaba in the Amazon estuary and evaluate the relationship of two important environmental factors, depth and salinity, with the accidental capture of freshwater and marine fishes in the Amazon estuary in the rainy and dry seasons.
