Abstract The aim was to analyse and improve the Primary Health Care (PHC) response to domestic violence against women (DVAW) by developing, implementing and evaluating an intervention. A pilot study evaluating the before and after of intervention implementation, using mixed methods and carried out in three phases - formative, intervention and evaluation - between August 2017 and March 2019 in two Basic Health Units (UBS) in the city of São Paulo. In this paper, we present the details and evaluation of the intervention, carried out six to twelve months after its implementation. The intervention was developed based on the findings of the formative phase and in line with the health policy that establishes the Violence Prevention Nucleus (NPV) and consisted of stablishing a care pathway; general training for all workers and specific training for the NPV; drawing up educational material and monthly case discussions over 6 months. The evaluation showed acceptability among the workers, increased identification and repertoire for caring for cases of DVAW, strengthening internal referral and the intersectoral network. We identified obstacles to the full implementation and sustainability of the intervention.

Resumo O objetivo foi analisar e aprimorar a resposta da Atenção Primária à Saúde (APS) ao cuidado dos casos de violência doméstica contra a mulher (VDCM), desenvolvendo, implementando e avaliando uma intervenção. Pesquisa piloto de avaliação do antes e depois da implementação de uma intervenção, utilizando métodos mistos e realizada em três fases - linha de base, intervenção e avaliação - entre agosto/2017 e março/2019 em duas Unidades Básicas de Saúde (UBS) do Município de São Paulo. Apresentamos neste artigo o detalhamento e a avaliação da intervenção, realizada 6-12 meses após implementação. A intervenção foi desenvolvida com base nos achados da primeira fase e em consonância com a política de saúde que estabelece os Núcleos de Prevenção à Violência (NPV), consistindo em: elaboração de fluxo assistencial; capacitação geral para todos os trabalhadores e específica para o NPV; elaboração de material educativo e discussões mensais de caso durante 6 meses. A avaliação mostrou aceitabilidade entre os trabalhadores, aumentou a identificação e repertório para o cuidado dos casos de VDCM, fortalecendo o encaminhamento interno e à rede intersetorial. Identificamos obstáculos para plena implementação e sustentabilidade da intervenção.

Resumo Objetivo Identificar os fatores clínico-funcionais associados ao risco de quedas, avaliado pelo Mini-BESTest, em idosos com diabetes mellitus tipo 2 (DM2). Método Trata-se de um estudo transversal. Um total de 145 idosos com idade =60 anos foram avaliados por meio das variáveis sociodemográficas (sexo, faixa etária, estado civil, nível de educação e percepção geral da saúde, audição e visão) Mini-BESTest, Mini-Mental State Examination (MMSE), Escala de Depressão Geriátrica (GDS-15) e o teste Timed Up and Go (TUG) (dupla tarefa). Foi utilizado um modelo de regressão logística múltipla. Resultados O domínio de orientação sensorial apresentou a pontuação média mais elevada, seguido pelos domínios estabilidade na marcha, ajustes posturais antecipatórios e respostas posturais. Os fatores associados ao risco de quedas em idosos foram: percepção visual ruim/muito ruim OR 3.40 (1,50-7,72); presença de doenças respiratórias OR 8.00 (1,32-48,46); sensação de tontura OR 2.53 (1,10-5,80); e tempo do teste Timed Up and Go (TUG) (dupla tarefa) igual ou superior a 13,5 segundos OR 3.31 (1,03-10,64). Conclusão Os idosos deste estudo apresentaram um equilíbrio postural comprometido, principalmente no domínio das respostas posturais. O conhecimento dos fatores associados ao risco de quedas em idosos com DM2 permite uma orientação mais eficaz na avaliação, prevenção e intervenção, visando minimizar a ocorrência de quedas e preservar ou otimizar o equilíbrio postural. Abstract Objective Identify clinical-functional factors associated to the risk of falls, assessed by Mini-BESTest in older adults with type 2 diabetes mellitus (T2DM). Method This cross-sectional study. A total of 145 older adults aged ≥60 years were evaluated through sociodemographic variables (sex, age group, married, education level, general health status hearing and vision), Mini-BESTest, Mini-Mental State Examination (MMSE), Geriatric Depression Scale (GDS-15) and dual-task Timed Up and Go Test (TUG) Multiple logistic regression model was used. Results The sensory orientation domain presented the highest average score, followed by the gait stability, anticipatory postural adjustments and postural responses domains. Factors associated to the risk of falls in older adults are: poor/very poor visual perception OR 3.40 (1.50-7.72); have respiratory diseases OR 8.00 (1.32-48.46); feeling dizzy OR 2.53 (1.10-5.80); and TUGT (dual task) time equal to or greater than 13.5 seconds OR 3.31 (1.03-10.64). Conclusion Older adults in this study presented impaired postural balance, mainly in the postural responses domain. The knowledge of the factors associated with the risk of falls in older adults with T2DM allows for better guidance in prevention, assessment and intervention, in order to minimize the occurrence of falls and maintain or optimize postural balance. ABSTRACT Objective To determine whether low muscle strength and low gait speed increase the risk of disability related to basic activities of daily living in community-dwelling older adults. Methods A longitudinal study (9 years of follow-up) was conducted with 390 older adults who were independent in basic activities of daily living at baseline and answered the Katz Index at follow-up. Associations were determined using Pearson’s χ2 test with a 5% significance level and logistic regression analysis. Results Increases in prevalence between baseline and follow-up were observed for low muscle strength (17.5%-38.2%), slow gait speed (26.0%-81.1%), and functional dependence (10.8%-26.6%). At follow-up, 11.7% of the participants had concomitant functional dependence, slow gait speed, and low muscle strength. Slow gait speed remained a predictor of dependence in basic activities of daily living (odds ratio=1.90; 95% confidence interval=1.06-3.41). ABSTRACT Purpose: Shock, cardiovascular problems, and respiratory failure constitute the main causes of death in patients cared in medical emergency rooms. Patients commonly require orotracheal intubation (OTI), a fact that has been intensified by diseases that generate important and fatal hemodynamic and respiratory problems in the affected patient. Methods: Although etomidate (ETO) is a highly used anesthetic for OTI, its use remains controversial in several scenarios. Some studies refer to an increase in mortality with its use in critically patients, while others do not refer to a difference. Therefore, we evaluated the mortality of patients submitted to OTI in the public hospital of a public federal university, with the use of ETO and other sedative-hypnotic drugs used in the induction of the performance of OTI, with the in-hospital mortality of patients cared in hospital. Results: The results demonstrate that the use of ETO as a hypnotic for OTI in the emergency room is not associated with a significant difference in morbidity or early mortality, within 30 days of hospitalization, compared with other hypnotics. Conclusions: There was no difference in mortality between patients intubated in the emergency department who used ETO and those who used non-ETO hypnotic within 72 hours and 30 days. RESUMO Objetivo: Atualizar as recomendações para embasar as decisões para o tratamento farmacológico de pacientes hospitalizados com COVID-19 no Brasil. Métodos: A elaboração desta diretriz foi feita por especialistas, incluindo representantes do Ministério da Saúde e metodologistas. O método utilizado para o desenvolvimento rápido de diretrizes baseou-se na adoção e/ou adaptação de diretrizes internacionais existentes (GRADE ADOLOPMENT) e contou com o apoio da plataforma e-COVID-19 RecMap. A qualidade das evidências e a elaboração das recomendações seguiram o método GRADE. Resultados: Chegaram-se a 21 recomendações, incluindo recomendações fortes quanto ao uso de corticosteroides em pacientes em uso de oxigênio suplementar e recomendações condicionais para o uso de tocilizumabe e baricitinibe, em pacientes com oxigênio suplementar ou ventilação não invasiva, e de anticoagulantes, para prevenção de tromboembolismo. Devido à suspensão da autorização de uso, não foi possível fazer recomendações para o tratamento com casirivimabe + imdevimabe. Foram feitas recomendações fortes contra o uso de azitromicina em pacientes sem suspeita de infecção bacteriana, hidroxicloroquina, plasma convalescente, colchicina e lopinavir + ritonavir, além de recomendações condicionais contra o uso de ivermectina e rendesivir. Conclusão: Foram criadas novas recomendações para o tratamento de pacientes hospitalizados com COVID-19, como as recomendações de tocilizumabe e baricitinibe. Ainda são recomendados corticosteroides e profilaxia contra tromboembolismo, esta em caráter condicional. Vários medicamentos foram considerados ineficazes e não devem ser usados, no intuito de proporcionar o melhor tratamento segundo os princípios da medicina baseada em evidências e promover a economia de recursos. ABSTRACT Objective: To update the recommendations to support decisions regarding the pharmacological treatment of patients hospitalized with COVID-19 in Brazil. Methods: Experts, including representatives of the Ministry of Health and methodologists, created this guideline. The method used for the rapid development of guidelines was based on the adoption and/or adaptation of existing international guidelines (GRADE ADOLOPMENT) and supported by the e-COVID-19 RecMap platform. The quality of the evidence and the preparation of the recommendations followed the GRADE method. Results: Twenty-one recommendations were generated, including strong recommendations for the use of corticosteroids in patients using supplemental oxygen and conditional recommendations for the use of tocilizumab and baricitinib for patients on supplemental oxygen or on noninvasive ventilation and anticoagulants to prevent thromboembolism. Due to suspension of use authorization, it was not possible to make recommendations regarding the use of casirivimab + imdevimab. Strong recommendations against the use of azithromycin in patients without suspected bacterial infection, hydroxychloroquine, convalescent plasma, colchicine, and lopinavir + ritonavir and conditional recommendations against the use of ivermectin and remdesivir were made. Conclusion: New recommendations for the treatment of hospitalized patients with COVID-19 were generated, such as those for tocilizumab and baricitinib. Corticosteroids and prophylaxis for thromboembolism are still recommended, the latter with conditional recommendation. Several drugs were considered ineffective and should not be used to provide the best treatment according to the principles of evidence-based medicine and to promote resource economy. Resumo Objetivo: Descrever o comportamento da fosfatase alcalina total (tALP) em pacientes com carcinoma de próstata metastático resistente a castração, submetidos a terapia com rádio-223 em um cenário do mundo real, e a sobrevida global (SG) desses pacientes. Materiais e Métodos: Estudo retrospectivo envolvento 97 pacientes, no período de fevereiro/2017 a setembro/2020. Os pacientes foram estratificados de acordo com a tALP basal (normal/elevada). A resposta à tALP foi definida como uma redução em relação à linha de base de ≥ 30% na semana-12. Para pacientes com tALP basal elevada, também foi avaliada a resposta ao tratamento como uma redução de ≥ 10% de tALP após o primeiro ciclo. A SG foi definida como o tempo entre o primeiro ciclo e a data do óbito. Resultados: A redução da tALP média após cada ciclo foi significativa (p < 0,05). A tALP na semana 12 estava disponível para 71 dos 97 pacientes. Desses 71 pacientes, 26 (36,6%) responderam. Dezenove (40,4%) dos 47 pacientes com tALP elevada apresentaram resposta. Foi observada uma SG mais longa nos pacientes com tALP basal normal, nos pacientes com tALP basal elevada que apresentaram resposta ao tratamento (redução de ≥ 10%) e nos pacientes que receberam 5-6 ciclos. Conclusão: A tALP pode ser usada para prever parte dos pacientes que se beneficiarão do tratamento com um maior número de ciclos e uma SG mais longa. Abstract Objective: To describe the behavior of total alkaline phosphatase (tALP) in patients with metastatic castration-resistant prostate cancer receiving radium-223 therapy, in a real-world scenario, and to describe overall survival (OS) among such patients. Materials and Methods: This was a retrospective study involving 97 patients treated between February 2017 and September 2020. Patients were stratified by the baseline tALP (normal/elevated). A tALP response was defined as a ≥ 30% reduction from baseline at week 12. For patients with elevated baseline tALP, we also evaluated treatment response as a ≥ 10% reduction in tALP after the first cycle of treatment. We defined OS as the time from the first treatment cycle to the date of death. Results: There was a significant reduction in the median tALP after each cycle of treatment (p < 0.05 for all). Data for tALP at week 12 were available for 71 of the 97 patients. Of those 71 patients, 26 (36.6%) responded. Elevated baseline tALP was observed in 47 patients, of whom 19 (40.4%) showed a response. Longer OS was observed in the patients with normal baseline tALP, in those with elevated baseline tALP that showed a response to treatment (≥ 10% reduction), and in those who received 5-6 cycles of therapy. Conclusion: The tALP may be used to predict which patients will benefit from treatment with a greater number of cycles of radium-223 therapy and will have longer OS. Resumo Objetivos Os objetivos deste estudo foram analisar a segurança da artroplastia total bilateral de joelho simultânea (ATJBS) e investigar a satisfação dos pacientes com o procedimento realizado de forma concomitante. Métodos Em um estudo prospectivo, avaliamos 45 pacientes submetidos à ATJBS feita por duas equipes cirúrgicas. A idade média dos pacientes foi 66,9 anos, sendo 33 do gênero feminino (73,3%) e 12 (26,6%) do gênero masculino. No período intra- e pós-operatório foi seguido um protocolo de medidas visando a segurança do procedimento. Avaliamos o tempo de cirurgia, a perda sanguínea pelo hematócrito (Ht) e hemoglobina (Hb), obtidos no primeiro dia de pós-operatório, a porcentagem de pacientes que receberam transfusão de concentrado de hemácias e quantidade de unidades que foram necessárias. Verificamosascomplicaçõesnoperíodo periopera-tório e, ao final de três meses, inquirimos aos pacientes sobre a preferência entre o procedimento simultâneo ou estagiado. Resultados O tempo médio de cirurgia foi de 169 minutos; no pós-operatório houve um decréscimo médio de 28,2% do Ht e de 27,0% da Hb. Ao todo, 16 pacientes (35,5%) receberam transfusão de concentrado de hemácias (média de 1,75 unidades por paciente que precisou de reposição). Tivemos 12 complicações menores (26,6%) e 2 complicações maiores (4,4%); além disso, nenhum paciente teve o diagnóstico clínico de trombose venosa profunda e não houve mortes. Conclusões A ATJBS pode ser considerada um procedimento seguro, se realizada em pacientes selecionados e com um protocolo de cuidados para prevenir complicações. Esse procedimento teve a aprovação unânime dos pacientes. Abstract Objectives This study aims to analyze the safety of a simultaneous bilateral total knee arthroplasty (SBTKA) and to investigate patients' satisfaction with the procedure performed concomitantly. Methods In a prospective study, we evaluated 45 patients undergoing SBTKA performed by two surgical teams. The mean age of the patients was 66.9 years; 33 subjects were female (73.3%) and 12 (26.6%) were male. We also recorded perioperative complications, and, at the end of 3 months, we asked patients about their preference between the simultaneous or staged procedure. Results The mean surgery time was 169 minutes. In the postoperative period, there was an average decrease of 28.2% in Htc and 27.0% in Hgb. A total of 16 patients (35.5%) received a transfusion of packed red blood cells (mean, 1.75 units per patient requiring a transfusion). There were 12 minor complications (26.6%) and 2 major complications (4.4%); furthermore, no patient had a clinical diagnosis of deep vein thrombosis, and there were no deaths. Conclusions The SBTKA procedure may be considered safe if performed in selected patients and with a care protocol to prevent complications. RESUMO O pirarucu, Arapaima gigas é um peixe de grande importância econômica e social na Amazônia, onde é comumente produzido em viveiros. A produção de peixes em tanques-rede tem crescido no Brasil, e este sistema poderia ser uma alternativa para a produção do pirarucu, reduzindo custos e melhorando os parâmetros produtivos. Nós avaliamos a contribuição do alimento natural na produção em sistema semi-intensivo (viveiros) e intensivo (tanques-rede) e seus efeitos sobre os aspectos produtivos e econômicos do pirarucu na fase de recria. Quatro viveiros (300 m2) e quatro tanques-rede (4,0 m3) foram estocados, respectivamente, com 120 (0,4 peixes m-2) e 160 (40 peixes m-3) juvenis de pirarucu (28,03 ± 6,34 g e 11,75 ± 0,80 cm). O estudo foi conduzido por 105 dias e os peixes foram alimentados com ração comercial extrusada. Crescimento e ingestão de plâncton pelos peixes foram avaliados quinzenalmente. Sobrevivência, comprimento padrão, ganho de peso e peso final foram maiores nos peixes cultivados em viveiros. A conversão alimentar nos viveiros (0,96 ± 0,06) foi menor que nos tanques-rede (1,20 ± 0,11). Mesmo sendo ofertada ração, os pirarucus consumiram alimento natural nos dois sistemas de produção. A abundância relativa de zooplâncton e insetos nos estômagos foi diretamente proporcional ao ganho de peso nos viveiros, e inversamente proporcional nos tanques-rede. Um índice de eficiência econômica mais alto e custo médio de produção mais baixo foram calculados para os viveiros escavados. Nossos resultados indicam que o custo-benefício do cultivo de A. gigas durante a fase de recria é melhor em viveiros. ABSTRACT Pirarucu, Arapaima gigas is a fish species of great social and economic importance in the Amazon region, where they are often farmed in earthen ponds. Intensive cage aquaculture has been growing in Brazil, which could be an alternative means to farm pirarucu, reducing costs and increasing productivity. We evaluated the contribution of natural food organisms in semi-intensive (ponds) and intensive (cages) production systems, and their effect on pirarucu growth and economic performance during the first phase of grow-out. Four ponds (300 m2) and four cages (4.0 m³) were stocked, respectively, with 120 (0.4 fish m-²) and 160 (40 fish m-³) juvenile pirarucu (28.03 ± 6.34 g, 11.75 ± 0.80 cm). The study was conducted for 105 days and fish were fed with commercial feed. Fish growth and plankton intake were evaluated every two weeks. Survival rate, standard length, weight gain and final weight were higher in fish reared in earthen ponds than in cages. Feed conversion of fish kept in ponds was lower (0.96 ± 0.06) than in cages (1.20 ± 0.11). The consumption of natural food organisms was observed, despite artificial feed being fed in both systems. The relative abundance of zooplankton and insects in stomachs was directly proportional to fish weight gain in ponds, and inversely proportional in cages. Higher economic efficiency rate and lower average production cost were calculated for earthen ponds. Our results indicate that the cost-benefit of the first phase grow-out of A. gigas is better in earthen ponds. Abstract The aim of this paper was to present a summary of the process of developing and preparing the final documents of the national consensus for teaching undergraduate Brazilian dental students the dental caries curriculum in the Portuguese language. The final document was developed in three steps: a) The ABENO and LAOHA cariology group invited experts from all five regions of Brazil to participate in the discussion. The theoretical support for crafting the first draft of the consensus was based on two publications: National Curriculum Guidelines of the Dentistry graduation in Brazil, Ministry of Education (2021) and the competences described in the European Core Curriculum for Cariology (ORCA-ADEE, 2011); b) The group of experts was divided into 5 working groups: G1-Domain, Main and Specific Competences, G2-Essential knowledge, G3-Life course perspective, G4-Social determinants and dental caries, G5- Glossary. The document was finalized by thoroughly reviewing the process using Delphi methodology; c) The 5-chapter document (one from each working group) was submitted to three open public consultations in 2022 (May-June, August, and October) using Google-forms. The suggestions (content/wording) were discussed within the group as: totally accepted, partially accepted, and rejected. A total of 192 suggestions were registered from 31 dental schools in all regions of Brazil. The number of suggestions received per Group were: 84, 28, 26, 24, 30 suggestions for G1, G2, G3, G4 and G5, respectively. The majority of suggestions were totally accepted by the group of experts (n = 172, 89.6%), 15 were partially accepted (7.8%), and 5 were rejected. RESUMEN Objetivo: averiguar las necesidades de salud de los adultos mayores que tuvieron Covid-19 prolongado y detalles del acceso al sistema de salud para suplir estos requerimientos. Método: estudio cualitativo y exploratorio realizado con 41 adultos mayores que tuvieron Covid-19 en 2020 y presentaron síntomas residuales 18 meses después de la infección. La recolección de datos tuvo lugar entre febrero y julio de 2022 por medio de entrevistas telefónicas semiestructuradas; en el análisis se utilizaron las técnicas analíticas de codificación inicial y focalizada y la base conceptual se fundamentó en el atributo “Accesibilidad” de la Atención Primaria de la Salud. Resultados: surgieron cuatro categorías en el análisis de los resultados, a saber: Comprender la necesidad de atención profesional; Reconocer los requerimientos que llevaron a los adultos mayores a procurar un servicio de salud; Percibir la disponibilidad de los servicios; y Analizar la capacidad de pago. Conclusión: la población anciana desarrolló requerimientos de la salud específicos relacionados con Covid-19 prolongado y los servicios de salud públicos y privados presentan cierta heterogeneidad en relación al enfoque de esta nueva condición, puesto que la atención basada en directrices propuestas por órganos oficiales no es unánime en los servicios públicos, privados y de Operadoras de Planes de Salud. ABSTRACT Objective: to understand the health needs of aged people who had long Covid-19 and details about access to the health system to meet these demands. RESUMO Objetivo: conhecer as necessidades de saúde dos idosos que tiveram a Covid longa e o acesso ao sistema de saúde para atender essas demandas. Método: estudo qualitativo exploratório, realizado com 41 idosos que tiveram Covid-19 no ano de 2020 e apresentaram sintomas residuais após 18 meses da infecção. A coleta de dados ocorreu entre fevereiro e julho de 2022 por meio de entrevistas semiestruturadas via telefone. Na análise foram utilizadas as técnicas de codificação inicial e focalizada e a base conceitual se fundamentou no atributo “Acessibilidade” da Atenção Primária à Saúde. Resultados: na análise dos resultados emergiram quatro categorias: Compreendendo a necessidade de atendimento profissional; Reconhecendo as demandas que levaram os idosos a buscar o serviço de saúde; Percebendo a disponibilidade dos serviços; e Analisando a capacidade de pagamento. Conclusão: a população idosa desenvolveu demandas específicas de saúde relacionadas à Covid longa, e os serviços de saúde público e privado possuem heterogeneidade quanto à abordagem dessa nova condição, uma vez que o atendimento pautado em diretrizes propostas por órgãos oficiais não é unânime nos serviços públicos, privados e Operadoras de Plano de Saúde. Abstract This study aimed to evaluate the contribution of oral and maxillofacial pathology laboratories (OMPLs) in Brazilian public universities to the diagnosis of lip, oral cavity, and oropharyngeal squamous cell carcinoma (SCC). A cross-sectional study was performed using biopsy records from a consortium of sixteen public OMPLs from all regions of Brazil (North, Northeast, Central-West, Southeast, and South). Clinical and demographic data of patients diagnosed with lip, oral cavity, and oropharyngeal SCC between 2010 and 2019 were collected from the patients’ histopathological records. Of the 120,010 oral and maxillofacial biopsies (2010-2019), 6.9% (8,321 cases) were diagnosed as lip (0.8%, 951 cases), oral cavity (4.9%, 5,971 cases), and oropharyngeal (1.2%, 1,399 cases) SCCs. Most cases were from Brazil’s Southeast (64.5%), where six of the OMPLs analyzed are located. The predominant profile of patients with lip and oral cavity SCC was Caucasian men, with a mean age over 60 years, low schooling level, and a previous history of heavy tobacco consumption. In the oropharyngeal group, the majority were non-Caucasian men, with a mean age under 60 years, had a low education level, and were former/current tobacco and alcohol users. According to data from the Brazilian National Cancer Institute, approximately 9.9% of the total lip, oral cavity, and oropharyngeal SCCs reported over the last decade in Brazil may have been diagnosed at the OMPLs included in the current study. Therefore, this data confirms the contribution of public OMPLs with respect to the important diagnostic support they provide to the oral healthcare services extended by the Brazilian Public Health System. Resumo O trabalho em rede tem papel central na assistência a mulheres em situação de violência. Este estudo analisa as diferentes perspectivas desse trabalho para profissionais da Atenção Primária e profissionais de serviços especializados nas áreas de assistência social, assistência jurídica e segurança pública, na cidade de São Paulo, Brasil. Realizaram-se entrevistas semi-estruturadas com 16 profissionais dos serviços especializados e 46 da saúde. Os eixos para a análise temática foram: o que os profissionais sabem e pensam sobre os demais serviços; sua atuação a partir disso; e suas expectativas. Os dados revelaram conhecimento insuficiente sobre os distintos serviços, resultando em dificuldades comunicativas, bem como em encaminhamentos equivocados pautados em idealizações sobre como deveria atuar o outro serviço. Concluímos que cada setor é bastante autônomo e seus serviços partem de seu próprio campo de atuação para definir aquilo que seria melhor para a mulher. Abstract Networking plays a central role in assisting women in situations of violence. This study analyzes how different the work perspectives are for Primary Care professionals and specialized services professionals in the areas of social and law assistance, and public security in the city of São Paulo, Brazil. Semi-structured interviews were carried out with 16 professionals from specialized services and 46 from the health sector. The axes for a thematic analysis were: what professionals know and think about services other than their own; their performance based on that; and their expectations. The findings revealed insufficient knowledge of the different services, resulting in communication difficulties as well as wrong referrals to other services, based on how other services would ideally work. We concluded that each sector is autonomous and its services start from its own field of action to define what would be best for women. Resumo Introdução Alterações na composição corporal do idoso podem ser rapidamente identificadas por profissionais de saúde na Atenção Básica, reduzindo o impacto sobre a saúde, o estado nutricional e a capacidade funcional. Objetivo Identificar relações entre massa e força muscular com o estado nutricional e a capacidade funcional em idosos da comunidade. Método Estudo transversal, envolvendo 323 idosos. A avaliação da composição corporal foi realizada por absorciometria (DEXA) e antropometria. A capacidade funcional foi avaliada pelas Atividades Instrumentais de Vida Diária, pela velocidade da marcha (VM) e Time Up and Go (TUG). Resultados Nesta amostra, o índice de massa magra (IMM) se mostrou associado aos indicadores do estado nutricional (IMC, CP e CC), além de ter apresentado importante correlação com a força de preensão palmar (FPP). A FPP esteve relacionada à VM e ao TUG. A análise de regressão identificou associação entre o IMC e o IMM (p<0,001), bem como entre a VM e a FPP (p=0,003). Conclusão A FPP se apresentou como importante indicador da capacidade funcional e se mostrou correlacionada com o IMM. Consequentemente, o IMM está associado aos indicadores do estado nutricional. Logo, na impossibilidade de avaliação do IMM, recomendamos a avaliação da FPP, CP e IMC como preditores de comprometimento do estado nutricional e da incapacidade funcional do idoso. Abstract Background Changes in the body composition of the elderly can be quickly identified by health professionals in primary care, reducing the impact on health, nutritional status, and functional capacity. Objective To look for links between muscle mass and strength, nutritional status, and functional capacity in community-dwelling elderly. Method A cross-sectional study involving 323 elderly people was carried out. Body composition assessment was performed by absorptiometry (DEXA) and anthropometry. Functional capacity was assessed by Instrumental Activities of Daily Living and by gait speed (VM) and Time Up and Go (TUG). Results In this sample, the lean mass index (LMI) was associated with indicators of nutritional status (BMI, NC, and WC), in addition to having shown an important correlation with handgrip strength (HGS). HGS was associated with VM and TUG. Regression analysis identified an association between BMI and LMI (p < 0.001) and between GS and HGS (p = 0.003). Conclusion HGS was an important indicator of functional capacity and correlated with LMI. Consequently, the LMI is associated with indicators of nutritional status. Resumo Fundamento O Programa Boas Práticas em Cardiologia é uma iniciativa da Sociedade Brasileira de Cardiologia (SBC) destinada à melhoria do cuidado cardiovascular nos hospitais públicos brasileiros. Objetivos Descrever características dos pacientes internados com Síndrome Coronariana Aguda (SCA) e Insuficiência Cardíaca (IC) e avaliar os indicadores de desempenho alcançados nos braços (SCA e IC) em um hospital público terciário, com uma meta pré-estabelecida de 85% de aderência às recomendações da SBC. Métodos Estudo do tipo transversal descritivo realizado por meio da coleta de dados de pacientes que estiveram internados entre maio de 2016 e setembro de 2019. Resultados Foram incluídos 1036 pacientes, 273 pacientes no braço IC e 763 no braço SCA. A média de idade foi de 59,8 ± 12,0 anos na SCA e 57,0 ± 14,1 anos na IC, com predomínio do sexo masculino em ambos os grupos. Mais da metade dos pacientes não tinham ensino fundamental completo e mais de 90% declararam renda mensal inferior a cinco salários-mínimos. Na SCA, predominou o diagnóstico de SCA com supradesnivelamento do segmento ST (66,3%) e 2,9% dos pacientes foram a óbito. Na IC, a etiologia mais comum foi a Doença de Chagas (25,8%) e 17,9% dos pacientes foram a óbito. Na avaliação dos indicadores de desempenho, nove dos 12 indicadores tiveram taxas de aderência acima de 85%. Conclusão Programas de qualidade são essenciais à melhoria do cuidado e os indicadores de desempenho do hospital apontam para uma boa adesão às diretrizes assistenciais da SBC, particularmente no braço da SCA. Abstract Background The Good Practices in Cardiology Program is an initiative created by the Brazilian Society of Cardiology (SBC) to improve the quality of care of cardiovascular disease patients in Brazilian public hospitals. Objectives To characterize patients admitted to a tertiary public hospital with diagnosis of acute coronary syndrome (ACS) or heart failure (HF) and to evaluate performance indicators in both ACS and HF arms, with a pre-established target of 85% adherence to the SBC recommendations. Methods This was a descriptive cross-sectional study through data collection of patients hospitalized between May 2016 and September 2019. Results A total of 1,036 patients were included, 273 in the HF arm and 763 in the ACS arm. Mean age was 59.8 ± 12.0 years in the ACS and 57.0 ± 14.1 years in the HF, with a predominance of male patients in both groups. More than half of patients had some primary education and more than 90% reported a monthly income of less than five minimum wages. In ACS, the diagnosis of ACS with ST segment elevation was predominant (66.3%), and 2.9% of patients died. In HF, the most common etiology was Chagas disease (25.8%), and 17.9% died. Analysis of the performance indicators revealed an adherence rate higher than 85% to nine of the 12 indicators. Conclusion Quality programs are essential for improvement of quality of care.
