Resumo Introdução: Fatores socioeconômicos e demográficos dos municípios podem afetar os serviços da atenção primária, influenciando na satisfação dos usuários. Objetivo: Verificar a relação entre a satisfação do usuário com a atenção primária e as condições socioeconômicas e demográficas dos municípios brasileiros. Método: Estudo transversal baseado nas entrevistas de usuários das equipes de atenção básica participantes do 2º Ciclo do Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica, realizado em 2013 e 2014. A análise da satisfação do usuário considerou questões organizacionais e aspectos assistenciais. A avaliação foi distribuída em seis estratos municipais, considerando fatores socioeconômicos e demográficos. Realizou-se análise de variância objetivando comparar as médias de satisfação dos usuários entre os estratos. Resultados: Usuários de municípios com pior condição socioeconômica e população menor que 10 mil habitantes apresentaram maior satisfação que a média em relação à maioria dos aspectos organizacionais. Nas questões assistenciais, participantes de municípios com melhor condição socioeconômica ou com população maior que 500 mil habitantes se mostraram mais satisfeitos em relação ao cuidado do diabetes e da hipertensão arterial. Conclusões: A satisfação dos usuários é maior nas questões organizacionais do que nas subdimensões assistenciais, ensejando a necessidade de investimento para melhorar a prestação do cuidado. Abstract Background: Socioeconomic and demographic factors in municipalities can affect primary care services, influencing user satisfaction. Objective: To verify the relationship between user satisfaction with primary care and socioeconomic and demographic conditions in Brazilian municipalities. Method: Cross-sectional study based on interviews with users of primary care teams participating in the 2nd Cycle of the National Program for Improving Access and Quality of Primary Care, in 2013 and 2014. The analysis of user satisfaction considered organizational issues and care aspects. The assessment was distributed into six municipal strata, considering socioeconomic and demographic factors. Analysis of variance was carried out in order to compare the averages of user satisfaction between the strata. Results: Users from municipalities with worse socioeconomic conditions and populations of less than 10 thousand inhabitants showed greater satisfaction than the average in relation to most organizational aspects. Regarding care issues, participants from municipalities with better socioeconomic conditions or with a population greater than 500 thousand inhabitants were more satisfied with regard to diabetes and hypertension care. Conclusions: User satisfaction is greater in organizational issues than in the sub-dimensions of care, implying the need for investment to improve the provision of care. Abstract: Mapping the submerged bottom is a hampered task when traditional vessels are inserted in shallow places that pose a danger to navigation. In this sense, current research has sought optical remote sensing to obtain bathymetry estimates in larger locations in less time. However, most studies employ a large sample of bathymetric points to estimate depth with orbital images. The need for a large amount of random bathymetric points can make these procedures less viable and unattractive. Thus, the present work proposes a methodology to estimate bathymetric depths from orbital images using sections of bathymetric points previously spaced in the study area, avoiding the need for a high amount of points collected through traditional bathymetric surveys. This work also compares this methodology using the NDWI index and the ANNs. Furthermore, the study showed that the points contained in the sections are quite efficient for extracting bathymetry with orbital images, especially through the implementation of neural networks, achieving a volume estimation accuracy within 4% of the actual volume of the reservoir in question. RESUMO: Objetivo: analisar o perfil e as características dos incidentes de segurança ocorridos entre 2015 e 2019 com pacientes pediátricos notificados no sistema informatizado de um hospital universitário. Método: estudo quantitativo, descritivo, transversal, com 2.558 notificações em instituição do sul do Brasil. Estatística descritiva e teste qui-quadrado foram utilizados para a análise dos dados. Resultados: ocorrência maior de incidentes de segurança relacionada a medicações/fluidos intravenosos (n=643; 25,1%), dieta/alimentação (n=448; 17,5%), processo/procedimento clínico (n=384; 15,0%), dispositivos/equipamentos médicos (n=304; 11,9%), acidentes do paciente (n=273; 10,7%). Unidades de internação e terapia intensiva estão mais relacionadas à ocorrência de incidentes; turno de trabalho não esteve relacionado ao tipo de incidente notificado. Conclusão: o panorama dos incidentes de segurança pode nortear estratégias para a qualificação do cuidado na instituição em estudo e colaborar com outros serviços de saúde na identificação de situações de risco para a criança hospitalizada. ABSTRACT Objective: To analyze the profile and characteristics of safety incidents that occurred between 2015 and 2019 with pediatric patients reported in the computerized system of a university hospital. Method: quantitative, descriptive, cross-sectional study with 2,558 notifications at an institution in southern Brazil. Descriptive statistics and the chi-square test were used to analyze the data. ABSTRACT Introduction: Immune checkpoints are regulators of the immune system response that allow self-tolerance. Molecules such as Programmed Cell Death Protein 1 (PD-1) and its Ligand (PD-L1) participate in the immune checkpoint by signaling co-inhibition of lymphocyte responses. In cancers, PD-L1 expression is associated with the immune evasion mechanism, which favors tumor growth. The use of anti-PD-1/PD-L1 drugs is already well described in solid tumors, but still not fully understood in hematologic malignancies. Myelodysplastic neoplasms (MDSs) are heterogeneous bone marrow disorders with an increased risk of progression to Acute Myeloid Leukemia (AML). The MDS affects hematopoietic stem cells and its pathogenesis is linked to genetic and epigenetic defects, in addition to immune dysregulation. The influence of the PD-L1 on the MDS remains unknown. Methods: In this study, we evaluated the mRNA expression of the PD-L1 in 53 patients with MDS, classified according to the WHO 2016 Classification. Results: Patients with dyserythropoiesis presented significantly higher PD-L1 expression than patients without dyserythropoiesis (p = 0.050). Patients classified as having MDS with an excess of blasts 2 (MDS-EB2) presented a significant upregulation in the mRNA expression of the PD-L1 compared to the MDS with an excess of blasts 1 (MDS-EB1) (p = 0.050). Furthermore, we detected three patients with very high levels of PD-L1 expression, being statistically classified as outliers. Conclusion: We suggested that the high expression of the PD-L1 is associated with a worse prognosis in the MDS and functional studies are necessary to evaluate the possible use of anti-PD-L1 therapies for high-risk MDS, such as the MDS-EBs. ABSTRACT Objective This study aimed to evaluate inflammatory biomarkers in patients undergoing peritoneal dialysis and investigate their association with all-cause mortality or transfer to hemodialysis. Methods This prospective cohort study included 43 patients undergoing peritoneal dialysis. Plasma levels of cytokines were measured using flow cytometry and capture enzyme-linked immunosorbent assay. Biomarkers were categorized based on their respective median values. Survival analysis was conducted using the Kaplan-Meier estimator, considering two outcomes: all-cause mortality and transfer to hemodialysis. Results After adjusting for confounding factors, plasma levels above the median of the levels of CCL2 and plasma, as well as below the median of TNF-α, and the median of dialysate IL-17 levels, were associated with an increased risk of experiencing the specified outcomes after approximately 16 months of follow-up. The amino-quinazoline scaffold stands out as a privileged structure in Medicinal Chemistry, due to its pleiotropic pharmacological profile. It has been explored in the design and development of novel drug candidates for several therapeutic applications, including antihypertensive, anti-inflammatory, antipsychotic, anti-Alzheimer, anticancer, antiviral, antibiotic, and antiparasitic treatments, among others. The therapeutic value of amino-quinazoline drugs was first demonstrated with the use of alpha 1-adrenoceptor antagonists, such as prazosin, doxazosin and terazosin. These drugs were initially approved for the treatment of hypertension and later for the treatment of benign prostatic hyperplasia. Several amino-quinazoline kinase inhibitors were introduced into the clinic as innovative therapeutic alternatives for cancer treatment after the U.S. Food and Drug Administration approved gefitinib in 2003 as a novel epidermal growth factor receptor inhibitor drug for non-small cell lung cancer. More recently, in 2021, the kinase inhibitor belumosudil was launched as a third-line therapy for adult and pediatric patients with chronic graft-versus-host disease. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. RESUMO Fundamentos Pacientes críticos têm maior risco de infecções relacionadas à assistência à saúde devido ao uso de vários dispositivos (infecção da corrente sanguínea associada a cateter central, infecção do trato urinário associada ao cateter e pneumonia associada à ventilação mecânica), o que representa uma ameaça significativa para essa população. Entre as várias estratégias, o banho diário com digluconato de clorexidina, um antisséptico solúvel em água, foi estudado como intervenção para diminuir a incidência de infecções relacionadas à assistência à saúde na unidade de terapia intensiva; no entanto, sua capacidade de reduzir todas as infecções relacionadas à assistência à saúde devido a vários dispositivos não está clara. Desenhamos o estudo Daily Chlorhexidine Bath for Health Care Associated Infection Prevention (CLEAN-IT) para avaliar se o banho diário com digluconato de clorexidina reduz a incidência de infecções relacionadas à assistência à saúde em pacientes críticos em comparação com o banho de água e sabão. Métodos O estudo CLEAN-IT é um estudo clínico multicêntrico, aberto, randomizado e em cluster com cruzamentos. Todos os pacientes adultos admitidos nas unidades de terapia intensiva participantes serão incluídos no estudo. Cada grupo (unidade de terapia intensiva) será randomizado para realizar o banho inicial com digluconato de clorexidina ou banho de água e sabão com cruzamento por um período de 3 a 6 meses, dependendo do momento de entrada de cada centro no estudo, com um período de washout de 1 mês entre os banhos com digluconato de clorexidina e as transições para os banhos de água e sabão. O desfecho primário é a incidência de infecções relacionadas à assistência à saúde devido a dispositivos. ABSTRACT Background Critically ill patients are at increased risk of health care-associated infections due to various devices (central line-associated bloodstream infection, catheter-associated urinary tract infection, and ventilator-associated pneumonia), which pose a significant threat to this population. Among several strategies, daily bathing with chlorhexidine digluconate, a water-soluble antiseptic, has been studied as an intervention to decrease the incidence of health care-associated infections in the intensive care unit; however, its ability to reduce all health care-associated infections due to various devices is unclear. We designed the Daily Chlorhexidine Bath for Health Care Associated Infection Prevention (CLEAN-IT) trial to assess whether daily chlorhexidine digluconate bathing reduces the incidence of health care-associated infections in critically ill patients compared with soap and water bathing. Methods The CLEAN-IT trial is a multicenter, open-label, cluster randomized crossover clinical trial. All adult patients admitted to the participating intensive care units will be included in the trial. Each cluster (intensive care unit) will be randomized to perform either initial chlorhexidine digluconate bathing or soap and water bathing with crossover for a period of 3 to 6 months, depending on the time of each center’s entrance to the study, with a 1-month washout period between chlorhexidine digluconate bathing and soap and water bathing transitions. The primary outcome is the incidence of health care-associated infections due to devices. Resumo O kefir é uma bebida funcional com potencial crescimento de mercado e diversas propriedades benéficas ao organismo humano, como equilíbrio da microbiota intestinal e propriedades anti-inflamatórias. Ele é produzido a partir de grãos que se multiplicam de uma cultura inicial de microrganismos que fermentam sacarose, resultando em uma bebida levemente ácida, gaseificada e com baixo teor alcóolico. O objetivo deste trabalho foi utilizar três diferentes substratos para a fermentação (açúcar de coco, mascavo e refinado) e realizar análises de pH, acidez titulável e teor de sólidos solúveis durante 48 h do processo. Nos grãos, foram avaliados biomassa microbiana, teor de massa seca e microscopia eletrônica de varredura (MEV). Foram feitas cinco fermentações para cada açúcar. Os resultados apresentaram diferenças na utilização dos açúcares, concluindo maior diferença em relação ao açúcar refinado e os demais. Abstract Kefir is a functional drink with potential market growth and several beneficial properties to the human body such as intestinal microbiota balance and anti-inflammatory properties. It is produced from grains that multiply from an initial culture of microorganisms that ferment sucrose, resulting in a slightly acidic, carbonated and low-alcohol beverage. The objective of this work was to use three different substrates for the fermentation (coconut, brown, and refined sugar) and to carry out analyzes of pH, titratable acidity (TA), and soluble solids content during 48 h of the process. In the grains, microbial biomass, dry mass content, and scanning electronic microscopy (SEM) were evaluated. Five fermentations were made for each sugar. The results showed differences in the use of sugar, concluding that there was a greater difference in relation to refined sugar and the others. Drinks fermented in coconut and brown sugar showed pH and acidity values within the ranges found in the consulted literature, with grain structures that provided good adhesion of microorganisms, which was not noticed in refined sugar grains. This may be due to the white sugar’s refining process involving chemical compounds, causing the sugar to have a high sucrose content but a low concentration of minerals and nutrients that favor grain growth. However, grains did not show a high mass gain, which may have been caused by the proportion of sugar/grain (40 g/26 g), temperature (27 °C), and time (48 h) used. ABSTRACT Objective: to reveal the experiences and consequences of workplace moral harrassment among nursing professionals. Method: this is an exploratory study with a qualitative approach, conducted between August and December 2023, with 14 nursing professionals. Resumo Os objetivos deste estudo foram apresentar informações sobre: volume de jogo, facilidades ou dificuldades para atacar ou defender no voleibol e quantificar os gestos técnico-táticos em jogos com diferentes alturas de rede, para fornecer informações sobre a quantidade de execuções técnicas em cada situação. Foram realizados três jogos, o primeiro com a rede a 2,10 metros de altura, o segundo a 2,00 metros e o terceiro a 2,20 metros. O Jogo 1 apresentou quantidade significativamente menor de tentativas de bloqueio e mais contatos realizados com a bola, o Jogo 2 apresentou quantidade significativamente maior de tentativas de bloqueio e o Jogo 3 apresentou menor quantidade de contatos com a bola. Concluímos que alterações de 10 cm na altura da rede não causam mudanças significativas na capacidade de manter a bola no ar, na dificuldade ou facilidade ofensiva e defensiva e na ocorrência de gestos técnico-táticos. As únicas variáveis que apresentaram influência foram as tentativas de bloqueio e a quantidade de contatos com a bola. Portanto, a altura de rede se torna uma variável interessante de ser modificada no ensino do gesto técnico-tático de bloqueio e da quantidade de contatos com a bola para iniciantes entre 10 e 12 anos.
Abstract The objective of this study was to present information about: volume of play, ease or difficulties to attack or defend in volleyball and to quantify the technical-tactical gestures in games with different net heights, in order to provide information about the number of technical executions in each situation. Three games were played, the first with the net at 2.10 meters high, the second at 2.00 meters high and the third at 2.20 meters high. Game 1 had significantly fewer block attempts and more contacts with the ball; game 2 had significantly more block attempts; and game 3 had fewer contacts with the ball. In conclusion, changes of 10 cm in the net height do not cause significant changes in the ability to keep the ball in the air, in offensive and defensive difficulty or ease, and in the occurrence of technical-tactical gestures. The only variables that showed influence were block attempts and the number of contacts with the ball. Therefore, net height becomes an interesting variable to modify in the teaching of the technical-tactical block gesture and the number of contacts with the ball for beginners between 10 and 12 years old.

ABSTRACT Light is a determining factor in plant morphophysiology, as it influences the growth and development of agricultural crops. Thus, the objective was to investigate the effects of light sources on the growth and development of “biquinho” pepper cultivars. The experiment was performed in two stages (Exp1 and Exp2), evaluating two cultivars (BRS Moema; Airetama biquinho) in five light sources (white LEDs, red LEDs, blue LEDs, red/blue LEDs, and fluorescent lamps). In Exp1 the plants were kept in a controlled condition, with a completely randomized design, in a 2x5 factorial scheme (cultivars x light sources) until 76 days after emergence (DAE), in which growth variables and photosynthetic pigments were evaluated. In Exp2, the plants were removed from the above conditions and transplanted in pots, being kept in greenhouse for more 76 DAE. At 152 DAE, the same variables as Exp1 were evaluated, as well as gain of shoot fresh and dry mass, and gain of root fresh and dry mass. Resumo Objetivo Avaliar os efeitos do colágeno hidrolisado e do peptídeo de colágeno no tratamento de lesões condrais superficiais de ratos. Método Foram utilizados 18 Rattus norvegicus nesta pesquisa. O dano articular foi induzido por uma única infiltração intra-articular de iodoacetato de sódio (solução 2 mg), injetada através do ligamento patelar da articulação dos animais previamente anestesiados. Os animais foram distribuídos em três grupos: grupo controle, grupo peptídeo de colágeno e grupo colágeno hidrolisado. O tratamento foi realizado por 30 dias com a administração via oral do peptídeo de colágeno ou do colágeno hidrolisado. Posteriormente, foi realizada a eutanásia dos experimentos e seguiu-se para o estudo das alterações condrais articulares. Os resultados foram avaliados conforme contagem de condrócitos por cluster e através da avaliação histológica segundo Pritzker et al. Resultados Ao observar os estágios de lesão, não foi observada significância estatística entre os grupos controle, colágeno hidrolisado e peptídeo de colágeno (p= 0,11). Ao observar os escores, houve significância estatística na comparação do grupo tratado com colágeno hidrolisado e o grupo peptídeo colágeno (p< 0,05), porém sem diferença estatística em relação ao grupo controle. Abstract Objective To evaluate the effects of hydrolyzed collagen and collagen peptide in the treatment of superficial chondral lesions in rats. Method This research employed 18 Rattus norvegicus. A single intraarticular infiltration of sodium iodoacetate (2 mg solution) through the patellar ligament induced joint damage in previously anesthetized animals. We divided the animals into three groups: a control group, a collagen peptide group, and a hydrolyzed collagen group. Treatment consisted of oral administration of collagen peptide or hydrolyzed collagen for 30 days. Afterwards, we euthanized the animals and studied the joint chondral changes. We evaluated the results according to the chondrocyte clusters count and a histological evaluation, as per Pritzker et al. Results There was no statistical significance in injury stages between the control, hydrolyzed collagen, and collagen peptide groups (p= 0.11). Regarding scores, there was a statistical significance between the groups treated with hydrolyzed collagen and collagen peptide (p< 0.05), but not in comparison with the control group. Resumen Objetivo: evaluar los efectos de un suplemento especializado en el control de la glucosa (GTSS) frente a un desayuno estándar sobre la glucemia posprandial (PPG). Metodología: es un estudio cruzado, prospectivo, aleatorizado en el que se incluyeron a pacientes con diabetes tipo 2 (T2D) con control subóptimo de la glucemia (HbA1c entre 6,5 y 8,5 %) utilizando monoterapia con metformina. El desayuno estandarizado fue isocalórico en comparación con el GTSS y solamente la distribución calórica fue diferente. Ambas intervenciones se utilizaron una vez al día por la mañana, reemplazando cada una el desayuno durante 7 días consecutivos (14 días de observación). Se utilizó el sistema de monitoreo continuo de glucosa (isCGM) para determinar las diferencias entre las intervenciones con respecto al área bajo la curva (iAUC) de glucosa postprandial (PPG) (3 horas después de la intervención) como variable principal o primaria; las variables secundarias fueron la diferencia entre las intervenciones con respecto al pico glicémico, la excursión de glucosa posprandial (PPGE), la glucosa media en sangre (MBG) y el rango de tiempo (TIR). Resultado: se incluyeron treinta y un pacientes con T2D con edades entre 39 y 69 años. El grupo del GTSS tuvo un área bajo la curva (iAUC) significativamente más baja de la PPG en comparación con el grupo del desayuno estandarizado (33,3 [15,0 a 54,0] frente a 46,8 [27,3 a 75,1] mg/dL), mientras que también presentó una PPG significativamente más baja (26,4 + 17,2 vs. 44,8 + 24,4 mg/dL). No hubo diferencia entre los períodos de intervención con respecto a la MBG, el TIR y eventos hipoglucémicos. Conclusión: el nuevo GTSS, como sustituto del desayuno, produjo una reducción de la iAUC de la PPG del 25 %, según el isCGM, en comparación con un desayuno isocalórico normalizado.
Abstract Objective: to evaluate the effects of a new glycemia targeted specialized supplement (GTSS) compared to a standard breakfast on postprandial blood glucose (PPG). Methods: patients with type 2 diabetes (T2D) and suboptimal control (A1C between 6.5 and 8.5 %) in monotherapy with metformin were included to this prospective, randomized, crossover trial. The standardized breakfast was isoenergetic compared to the GTSS, differing on macronutrients distribution. Both interventions were used once a day in the morning, each replacing breakfast for 7 consecutive days (14 days of observation). Intermittent scanning continuous glucose monitoring system (isCGM) determined the difference between the interventions regarding the incremental area under the curve (iAUC) of the PPG (3 hours after intervention), as a primary endpoint; secondary endpoints were the difference between the interventions regarding the glycemic peak, postprandial glucose excursion (PPGE), mean blood glucose (MBG) and time in range (TIR). Results: thirty-one T2D patients with ages between 39 and 69 years-old were enrolled. GTSS group had significantly lower iAUC of the PPG compared to standardized breakfast (33.3 [15.0 to 54.0] vs 46.8 [27.3 to 75.1] mg/dL), while also presenting a significantly lower PPG excursion (26.4 ± 17.2 vs 44.8 ± 24.4 mg/dL). There was no difference between the intervention periods regarding MBG, TIR and hypoglycemic events. Conclusion: the new GTSS, as a meal replacement in the breakfast, produced a 25 % reduction in the iAUC of the PPG, as accessed by isCGM, in comparison with an isocaloric-standardized meal.

ABSTRACT The Scientists of Tomorrow/ Cientistas do Amanhã project is an immersive science training program developed by the Program of Post-Graduation in Health Sciences at Hospital Israelita Albert Einstein. This program was conducted in partnership with Volunteering and Escola Municipal de Ensino Fundamental Professor Paulo Freire in Paraisópolis, São Paulo, Brazil. The Scientists of Tomorrow Program comprised a short training period conducted in May 2022 involving 37 students, and a long training period from August to December 2022, which included 15 students. It aimed to popularize science through practical activities; transfer knowledge to young students; sensitize and guide them to pursue academic-scientific careers; reduce stereotypes about scientific work and scientists; and help students understand the social, political, and ethical roles of science within society. All activities were led by postgraduate students and professors from our postgraduate program, physicians, nurses, physiotherapists, biomedicals, and veterinarians from Hospital Israelita Albert Einstein, as well as medical students from Faculdade Israelita de Ciências da Saúde Albert Einstein . Activities in the short training included lectures on cinema and science, strategies to combat fake news, non-violent communication, innovation, design-thinking framework, and developing a scientific project. During the long training period, discussions were focused on nanotechnology, animal research, big data, bioinformatics, meditation, blood and bone marrow donation, telemedicine, sex and sexually-transmitted infections, rehabilitation, career opportunities, and scientific integrity. In addition, practical activities were further expanded using optical and confocal microscopy, cytometry, and basic concepts regarding the structure and function of living cells. The program also included the launching of the open-air outreach Education E-natureza activity, which turned students into ambassadors of nature. In conclusion, the Scientists of Tomorrow Program was innovative and enabled young students to learn that science is a collective activity that can enhance public health.
