Abstract The article aims to analyze the application of consensuality in the context of criminal execution, particularly the introduction of the term and adjustment of conduct, through the Ordinance of the former National Penitentiary Department nº 275/2016 and has the hypothesis that such a procedure could bring improvements to, at least, the speed of the internal processes of penitentiary establishments. It is based on a pragmatic perspective of Sanctioning Administrative Law, which has the mission of protecting the public interest projected in the legal system in contexts marked by the complexity and dynamism of social relations. The methodology used was the promotion of a dialectical debate between fundamental texts on this area of law and consensus, current legislation and methods of using consensus in the context of criminal execution, in particular the Term of Adjustment of Conduct. As a starting point, it is argued that disciplinary sanctions should be understood as instruments for achieving public purposes that require state action that goes beyond the licit/illicit binary logic, seeking solutions that produce the best results for society. It is argued that the system of punishments and incentives that permeates the execution of the custodial sentence must take into account the potential double function of disciplinary sanctions: guaranteeing order and discipline while promoting the social integration of the convict. The text concludes that the introduction of consensuality by means of the aforementioned ordinance can be an effective instrument available to the penitentiary administration, but it imposes caution in its application, in order not to generate arbitrariness. Furthermore, it concludes that Ordinance nº 275/2016 demands improvement in terms of densification and detailing, so that the TAC can be justified as an exercise of disciplinary sanctioning powers capalbe of promoting the public interest in the conformity of conduct.
Resumo O artigo objetiva analisar a aplicação da consensualidade no âmbito da execução penal, notadamente a introdução do termo de ajustamento de conduta (TAC), por meio da Portaria do antigo Departamento Penitenciário Nacional nº 275/2016, e tem como hipótese que tal procedimento poderia representar um avanço em termos de celeridade dos processos internos dos estabelecimentos penitenciários, além de se traduzir em importante ferramenta para dissuasão e prevenção de novos ilícitos disciplinares. Fundamenta-se em uma perspectiva pragmática do Direito Administrativo Sancionador, o qual tem a missão de tutelar o interesse público projetado no ordenamento jurídico em contextos marcados pela complexidade e dinamicidade das relações sociais. A metodologia utilizada foi a promoção de um debate dialético entre textos fundamentais sobre essa área do Direito e a consensualidade, as legislações vigentes e os métodos de utilização da consensualidade no contexto da execução penal, em especial o Termo de Ajustamento de Conduta. Como ponto de partida, defende-se que as sanções disciplinares devem ser compreendidas como instrumentos para a consecução das finalidades públicas que reclamam uma atuação estatal que supere a lógica binária lícito/ilícito, buscando soluções que produzam os melhores resultados para a sociedade. Argumenta-se que o sistema de punições e incentivos que perpassa a execução da pena privativa de liberdade deve levar em conta a potencial dupla função das sanções disciplinares: garantir ordem e disciplina sem deixar de promover a integração social do sentenciado. O texto conclui que a introdução da consensualidade, por meio da citada portaria, pode ser um instrumento efetivo à disposição da administração penitenciária, mas impõe cautelas em sua aplicação, a fim de não gerar espaço para arbitrariedades. Ademais, conclui que a Portaria nº 275/2016 demanda melhor densificação e detalhamento, para que se justifique o TAC como um exercício de potestades sancionadoras disciplinares apto a promover o interesse público na conformidade de condutas.

RESUMO: As penas são subprodutos que são gerados em quantidades significativas pela indústria avícola. A bioconversão microbiana tem sido investigada como uma estratégia promissora para o processamento de penas, uma vez que, juntamente com a degradação desses materiais queratinosos, o bioprocessamento pode resultar em produtos de valor agregado. Assim, do ponto de vista da microbiologia industrial, as penas de frango podem ser consideradas matéria-prima para a obtenção de proteases microbianas. Dentro deste contexto, o objetivo deste trabalho foi investigar e caracterizar a produção de proteases extracelulares por Aspergillus sp., isolados de solo da floresta Amazônica. A produção enzimática foi avaliada utilizando diversos substratos de crescimento (penas inteiras, farinha de penas, cabelo humano, caseína, gelatina, peptona e bicos de frango). Com maior produção de enzima foi obtida a farinha de penas (FP) e peptona. Após 48 h de fermentação, a degradação da FP foi de 15.82%. A protease bruta mostrou atividade ótima em pH 5.0 e 37 0C e a atividade enzimática foi aumentada com a adição de 1 e 5 mM de CaCl2, MnSO4, KCl, MgSO4 e CuSO4. Os detergentes Tween 20 e Triton x-100, nas concentrações 0.5 e 1% (v/v), tenderam a estimular a atividade. A presença de 0.5 e 1% (v/v) dos solventes orgânicos (metanol, acetona, butanol, acetonitrila, isopropanol e DMSO), mantiveram a atividade enzimática. O β-mercaptoetanol estimulou a atividade proteolítica nos ensaios enzimáticos. ABSTRACT: Feathers are by-products that are generated in significant quantities by the poultry industry. Microbial bioconversion has been investigated as a promising strategy for the processing of feathers, since, along with the degradation of these keratinous materials, bioprocessing can result in value-added products. Thus, from the perspective of industrial microbiology, chicken feathers can be considered a raw material for obtaining microbial proteases. Within this context, this research investigated and characterized the production of extracellular proteases by Aspergillus sp., isolated from soil of the Amazon Rainforest. The enzymatic production was evaluated using several growth substrates (whole feathers, feather meal, human hair, casein, gelatin, peptone and chicken beaks). With highest enzyme production was obtained the feather meal (FM) and peptone. After 48 h of fermentation, FM degradation was 15.82%. The crude protease showed optimal activity at pH 5.0 and 37 °C and enzymatic activity was enhanced with the addition of 1 and 5 mM of CaCl2, MnSO4, KCl, MgSO4 and CuSO4. The detergents Tween 20 and Triton x-100, at concentrations 0.5 and 1% (v/v), tended to stimulate activity. VALIDATION OF THE SCALE FOR THE ENVIRONMENT EVALUATION OF PROFESSIONAL NURSING PRACTICE FOR BRAZIL

RESUMO Objetivo: realizar a adaptação linguística, cultural e validação da Scale for the Environment Evaluation of Professional Nursing Practice (SEE - Nursing Practice) para o Brasil. Método: estudo metodológico utilizando uma amostra de 291 enfermeiros, atuantes em oito hospitais brasileiros. A coleta de dados ocorreu no período de julho a outubro de 2021. A consistência interna do instrumento foi avaliada pelo coeficiente alfa de Cronbach e a validade da estruturação da escala por domínios foi avaliada pela Análise Factorial Exploratória com extração por componentes principais e rotação Varimax e medidas de adequação e de esfericidade. Resultados: na subescala da dimensão Estrutura uma solução de 6 fatores explicou 63,1% da variância total, ficando constituída por 40 itens, distribuídos em seis fatores. Na subescala da dimensão Processo, a análise factorial exploratória de cinco fatores, explicou 62% da variância total e ficou constituída por 33 itens. A análise factorial exploratória da subescala da dimensão Resultado apontou uma solução de dois fatores que explicaram 67,7% da variância total e constituída por 13 itens. As subescalas da SEE - Nursing Practice - Estrutura, Processo e Resultado obtiveram uma consistência interna de 0,956, 0,929 e 0,937, respectivamente. Conclusão: a versão brasileira da SEE - Nursing Practice é uma ferramenta válida e confiável utilizada para avaliar se os ambientes de prática profissional de enfermagem são promotores de qualidade de atendimento. ABSTRACT Objective: to perform the linguistic, cultural and validation adaptation of Scale for the Environment Evaluation of Professional Nursing Practice (SEE - Nursing Practice) for Brazil. Method: a methodological study with a sample of 291 nurses working in eight Brazilian hospitals. Data collection occurred from July to October 2021. Internal consistency of the instrument was evaluated by means of the Cronbach's alpha coefficient, and validity of the structure of the scale by domains was evaluated via exploratory factor analysis with extraction by principal components and Varimax rotation, as well as adequacy and measureability measures. Results: in the subscale of the Structure dimension, a 6-factor solution explained 63.1% of the total variance, consisting of 40 items, distributed in six factors. In the subscale of the Process dimension, the exploratory five-factor analysis explained 62% of the total variance and consisted of 33 items. The exploratory factor analysis of the Outcome dimension subscale indicated a two-factor solution that explained 67.7% of the total variance and consisted of 13 items. Bactericidal Effectiveness of 3D Printed PLA Surfaces Coated with Sealant Containing Silver Nitrate D

Abstract This research characterized the morphological and chemical characteristics, as well as the biocidal effectiveness of polylactic acid (PLA) plate surfaces, created through additive manufacturing, both with and without an additional sealing coating of a polymer infused with silver nitrate crystals. In acknowledgement of the well-accepted principle of silver-based materials as antibacterial agents, this study delves deeper into the underlying mechanism responsible for the bactericidal effect of the sealant, offering a thorough characterization of the system and its behavior. To assess the bactericidal effect of the system, a suite of techniques including visual examination, optical microscopy (OM), digital image processing (DIP), scanning electron microscopy (SEM), semi-quantitative chemical analyses using energy dispersive spectroscopy (EDS), average roughness (Ra) computations, and in vitro tests were employed. The goal was to assess the impact of the presence or absence of the sealant on the proliferation of Escherichia coli American Type Culture Collection (ATCC) 11229 and Staphylococcus aureus ATCC 25923 bacteria. Results showed that the sealant had a significant antimicrobial effect, reducing the number of bacteria on the coated surfaces by at least 64 times within 30 minutes, in comparison with uncoated surfaces. This study also revealed the presence of micropores in the cross-section of the sealant layer. The microporous structure likely served as a conduit for silver ions from the encapsulated silver nitrate to exert their bactericidal effect on bacterial colonies. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Lateral-Torsional Buckling (LTB) is an important limit state that must be considered in the design of steel members subjected to bending. During the process of revising of the Brazilian Standard ABNT NBR 8800:2008, it was questioned whether the present formulation would really lead to good results vis-à-vis the experimental tests. To this end, a comprehensive review of the technical literature was carried out, looking for test results that could be compared with the ones using the equations provided in the Brazilian Standard. Then, a meta-analysis was performed by comparing the test results with the ones of the ABNT NBR 8800:2008 methodology. It was observed that, for beams subjected to uniform bending moments and simple supports, the formulation proved to be adequate. However, for the case of beams with variable bending moments, for which the factor Cb (modification factor for non-uniform bending moment diagram) is used, it was realized that the formulation could lead to unsafe results, especially for high values of Cb. A modified formulation was then proposed, with the introduction of Cb only in the elastic range of the equations (only in the buckling moment equation) and in the equation for the slenderness parameter corresponding to the beginning of yielding, λΓ. The proposed modified formulation, as well as the EN 1993-1-1:2022 methodology, were subsequently included in the meta-analysis. Structural reliability analyses were also carried out for both the ABNT NBR 8800:2008 and the modified methodologies. The analyses showed that the reliability indexes of the modified formulation were higher than those of the ABNT NBR 8800:2008 one and closer to the recommended "target value". ABSTRACT Objective: To assess the 30-day unplanned readmission rate and its association with overlapping probable sarcopenia and malnutrition after major oncological surgery. Methods: A prospective bicentric observational cohort study performed with adult oncological patients undergoing major surgery. The primary outcome was unplanned readmission within 30 days after discharge and the association with probable sarcopenia and malnutrition. Nutritional status and probable sarcopenia were assessed just prior to surgery. Patients classified using subjective global assessment, as B and C were malnourished. Probable sarcopenia was defined using SARC-F (strength, assistance with walking, rise from a chair, climb stairs, falls) questionnaire ≥4 points and low HGS (handgrip strength) <27kg for males and <16kg for females. Results: Two hundred and thirty-eight patients (51.7% female) with a median age of 60 years were included. The 30-day readmission rate was 9.0% (n=20). Univariate analysis showed an association of malnutrition (odds ratio (OR) = 4.84; p=0.024) and probable sarcopenia (OR = 4.94; p=0.049) with 30-day readmission. Furthermore, when both conditions were present, the patient was almost nine times more likely to be readmitted (OR = 8.9; p=0.017). Multivariable logistic regression analysis showed that overlapping probable sarcopenia and malnutrition was an independent predictor of 30-day unplanned readmission (OR = 8.10, 95% confidence interval (95%CI) 1.20-0.55; p=0.032). Conclusion: The 30-day unplanned readmission rate was 9.0%, and the overlap of probable sarcopenia and malnutrition is an independent predictor for the 30-day unplanned readmission after major oncologic surgery. Abstract Background: The number of people suffering from chronic kidney disease undergoing hemodialysis in Portugal has been increasing over the last decade, highlighting the need to assess the quality of life of these patients. Objective: To assess the quality of life of patients with chronic kidney disease undergoing regular hemodialysis. Methodology: A quantitative, cross-sectional descriptive study was conducted using the Kidney Disease Quality of Life Instrument (KDQOL-SF Version 1.3). The non-probability convenience sample consists of 268 patients aged 25 to 90 years undergoing regular hemodialysis in seven clinics in Portugal. Results: The Dialysis staff encouragement (82.46) and social support (77.49) dimensions had the highest scores. Male patients had higher scores in the physical functioning (p = 0.023),bodily pain (p = 0.011), and emotional well-being (p = 0.020) dimensions. This sample had statistically significant lower scores in all domains of the 36-Item Health Survey (SF-36) than the general Portuguese population. Conclusion: It is essential to develop strategies to improve the quality of life of patients with chronic kidney disease.
Resumen Marco contextual: En la última década ha aumentado el número de personas con insuficiencia renal crónica que se someten a un programa de hemodiálisis, lo que plantea la necesidad de evaluar la calidad de vida de estos pacientes. Objetivo: Evaluar la calidad de vida de los pacientes renales crónicos en programa regular de hemodiálisis. Metodología: Se trata de un estudio cuantitativo, transversal y descriptivo que utiliza el KDQOL-SF. Muestra de conveniencia no probabilística, con 268 pacientes en programa regular de hemodiálisis con edades comprendidas entre los 25 y los 90 años, en siete clínicas de Portugal. Resultados: La motivación del personal de diálisis (82,46) y el apoyo social (77,49) fueron las dimensiones con las mejores puntuaciones. Los individuos del sexo masculino presentan mejores puntuaciones en calidad de vida, en las dimensiones: función física (p = 0,023), dolor (p = 0,011) y bienestar emocional (p = 0,020). El análisis del SF-36 demostró que todos los dominios presentaban una puntuación peor y con significación estadística en comparación con la población portuguesa. Conclusión: Es esencial idear estrategias que puedan mejorar la calidad de vida de las personas con insuficiencia renal.

Amostra não probabilística de conveniência, com 268 doentes em programa regular de hemodiálise com idades entre 25 e 90 anos, em sete clínicas de Portugal. Resultados: O encorajamento do pessoal da diálise (82,46) e o apoio social (77,49) foram as dimensões com melhores scores. Os indivíduos do sexo masculino apresentam melhores pontuações na qualidade de vida, nas dimensões: função física (p = 0,023), dor (p = 0,011) e bem-estar emocional (p = 0,020). A análise do SF-36 demostrou que todos os domínios apresentavam pior score e com significado estatístico quando comparados com a população portuguesa. Conclusão: É fundamental arranjar estratégias que possam melhorar qualidade de vida da pessoa com insuficiência renal.

ABSTRACT Introduction: Embedded electronic systems on reconfigurable hardware devices make it possible to carry out hybrid hardware/software implementations that allow higher latency functions to be speeded up by hardware, which is why the development of parameterizable hardware architectures that allow their use in various applications is a current field of research. The objective of this work is to expose different contributions in this field related with the acceleration of image processing functions and cryptographic functions, as well as on the detection of possible vulnerabilities against different types of attacks. Methods: They are presented different development methodologies due to the diversity of the contributions that are staged. It was used for the implementation of the image processing functions, a model-based methodology, making use of the Xilinx System Generator tool together with Matlab/Simulink, as well as a new automatic configuration procedure for the hardware components of the developed library, while they were developed the implementations of the cryptographic functions in hardware description language. For the detection of vulnerabilities, they were used content manipulation and collateral channel attacks for power consumption of the SPA type. Results: The use of hardware components of the library developed in a vehicle license plate identification application and hardware components of cryptographic functions, duly protected against different attacks, in the ARCANO cryptographic system, as well as the validation that the Binary Euclidean Algorithm is vulnerable to partial knowledge of its execution flow. Conclusions, they are emphasized the fundamental contributions of the research results, as well as their practical utility in real applications.
RESUMEN Introducción: Los sistemas electrónicos embebidos sobre dispositivos de hardware reconfigurable posibilitan la realización de implementaciones híbridas hardware/software que permiten acelerar mediante hardware las funciones de mayor latencia, por lo que constituye un campo de investigación actual el desarrollo de arquitecturas hardware parametrizables que posibiliten su utilización en diversas aplicaciones. El objetivo de este trabajo es presentar diferentes contribuciones en este campo relacionadas con la aceleración de funciones de procesamiento de imágenes y funciones criptográficas, así como en la detección de posibles vulnerabilidades ante diferentes tipos de ataques. Métodos: Se presentan diferentes metodologías de desarrollo dada la diversidad de las contribuciones que se exponen. Para la implementación de las funciones de procesamiento de imágenes se utilizó una metodología basada en modelos haciendo uso de la herramienta Xilinx System Generator junto con Matlab/Simulink así como un novedoso procedimiento de configuración automática de los componentes hardware de la biblioteca desarrollada; mientras que las implementaciones de las funciones criptográficas fueron desarrolladas en lenguaje de descripción de hardware. Para la detección de las vulnerabilidades se utilizaron ataques de manipulación de contenidos y de canal colateral por consumo de potencia del tipo SPA. Resultados: Se expone la utilización de componentes hardware de la biblioteca desarrollada en una aplicación de identificación de matrículas de vehículos y de componentes hardware de funciones criptográficas, debidamente protegidas contra diferentes ataques, en el sistema criptográfico ARCANO, así como la validación de que el algoritmo binario de Euclides es vulnerable ante el conocimiento parcial de su flujo de ejecución. En Conclusiones se enfatizan las contribuciones fundamentales de las investigaciones realizadas, así como su utilidad práctica en aplicaciones reales.

RACIONAL: Para reduzir o risco de regurgitação durante a anestesia para procedimentos eletivos, os volumes gástricos residuais (VRG) têm sido tradicionalmente minimizados com jejum noturno. O jejum pré-operatório prolongado tem algumas consequências adversas e tem sido abandonado para a maioria dos procedimentos cirúrgicos, com exceção de pacientes obesos e/ou diabéticos.
OBJETIVOS: O objetivo deste estudo foi avaliar o VRG em pacientes diabéticos obesos mórbidos após jejum tradicional ou abreviado.
MÉTODOS Este estudo foi aprovado pelo Comitê de Ética em Pesquisa em Seres Humanos da Universidade Federal de Mato Grosso, sob o número 179.017/2012. Este é um projeto prospectivo, randomizado e cruzado em 8 pacientes diabéticos tipo II com obesidade mórbida. O VRG foi medido endoscopicamente após jejum noturno tradicional (pelo menos 8 horas) ou após jejum abreviado (6 horas para sólidos e 3 horas para uma bebida contendo água mais 25 g (12,5%) de maltodextrina). Os dados são expressos como média e variação e as diferenças foram comparadas com testes t pareados em p<0,05.
RESULTADOS: A população estudada tinha 41,5 (28-53) anos de idade, peso médio de 135 (113-196) kg, índice de massa corporal (IMC) de 48,2 (40-62,4) kg/m2 e diabetes tipo II de 4,5 (1-10) anos. O VRG após o jejum abreviado foi de 21,5 (5-40) ml versus 26,3 (7-65) ml após o jejum tradicional. Essa diferença não foi significativa (p=0,82).
CONCLUSÕES: O esvaziamento gástrico em pacientes diabéticos obesos mórbidos é semelhante após jejum tradicional ou abreviado com uma bebida com carboidrato.
BACKGROUND: To reduce the risk of regurgitation during anesthesia for elective procedures, residual gastric volumes (RGV) have traditionally been minimized with overnight fasting. Prolonged preoperative fasting have some adverse consequences and has been abandoned for most surgical procedures, with the exception of obese and/or diabetic patients.
AIMS: The aim of this study was to assess the RGV in morbidly obese diabetics patients after traditional or abbreviated fasting.
METHODS: This study was approved by the Committee of Ethics in Research in Human Beings from the Federal University of Mato Grosso, number 179.017/2012. This is a prospective, randomized, and cross-over design in 8 morbidly obese type II diabetic patients. RGV was measured endoscopically after either traditional overnight fasting (at least 8 hours) or after abbreviated fasting (6 h for solids and 3 hours for a drink containing water plus 25 g (12.5%) of maltodextrin). Data are expressed as mean and range and differences were compared with paired t tests at p<0.05.
RESULTS: The study population was 41.5 (28-53) years old, with a mean weight of 135 (113-196) kg, body mass index (BMI) 48.2 (40-62.4) kg/m2, and having type II diabetes for 4.5 (1-10) years. The RGV after abbreviated fasting was 21.5 (5-40) ml vs 26.3 (7-65) ml after traditional fasting. This difference was not significant (p=0.82).
CONCLUSIONS: Gastric emptying in morbidly obese diabetics patients is similar after either traditional or abbreviated fasting with a carbohydrate drink.

Abstract Objective To assess the prevalence of GERD exclusively by means of multichannel intraluminal impedanciometry associated with pH monitoring (MIIpH) and compare it with respiratory symptoms in children with CF. To compare MIIpH with pHmetry alone to perform GERD diagnosis. Methods An analytical cross-sectional study was conducted with children diagnosed with CF who underwent MIIpH. Clinical and laboratory markers, including respiratory and digestive symptoms, were used for comparative analyses. High-resolution chest computed tomography was performed on patients with symptoms of chronic lung disease. Severity was classified according to the Bhalla score. Results A total of 29 children < 10 yo (18 girls) were evaluated; 19 of whom with physiological GER and 10 with GERD. Of the children with GERD, seven had predominantly acid GER, two acid+non-acid GER, and one non-acid GER. Three patients had GERD diagnosed only by MIIpH. Bhalla scores ranged from seven to 17.75 with no significant relationship with GERD. The number of pulmonary exacerbations was associated with a decrease in esophageal clearance regardless of the position in pHmetry and MIIpH. Conclusions The prevalence of GERD was 34% in children with CF. There was no association between respiratory disease severity and GER types. MIIpH detected 30% more patients with GERD than pHmetry. Treating Advanced Hepatocellular Carcinoma with Sorafenib: A 10-Year Single Center Experience

Abstract Introduction: Sorafenib was the first therapy used for sys-temic treatment of unresectable hepatocellular carcinoma (HCC). Multiple prognosis factors associated with sorafenib therapy have been described. Objectives: The aim of this work was to evaluate survival and time to progression (TTP) on HCC patients treated with sorafenib, and check for predictive factors of sorafenib benefit. Materials and Methods: Retrospectively, data from all HCC patients treated with sorafenib in a Liver Unit from 2008 to 2018 were collected and analyzed. Results: Sixty-eight patients were included; 80.9% were male, the median age was 64.5 years, 57.4% had Child-Pugh A cirrhosis and 77.9% were BCLC stage C. Macro-vascular invasion (MVI) was present in 25% of the patients and 25% of the subjects had other extrahepatic metastasis. The median survival was 10 months (IQR 6.0-14.8) and median TTP was 5 months (IQR 2.0-7.0). Survival and TTP were similar between Child-Pugh A and B patients: 11.0 months (IQR 6.0-18.0) for Child-Pugh A and 9.0 months (IQR 5.0-14.0) for Child-Pugh B (p = 0.336). In univariate analysis, larger lesion size (LS >5 cm), higher alpha-fetoprotein (AFP >50 ng/mL), and no history of locoregional therapy were statistically associated with mortality (HR 2.17, 95% CI 1.24-3.81; HR 3.49, 95% CI 1.90-6.42; HR 0.54, 95% CI 0.32-0.93, respectively), but only LS and AFP were independent predictive factors, as shown in multivariate analysis (LS: HR 2.08, 95% CI 1.10-3.96; AFP: HR 3.13, 95% CI 1.59-6.16). MVI and LS >5 cm were associated with TTP shorter than 5 months in univariate analysis (MVI: HR 2.80, 95% CI 1.47-5.35; LS: HR 2.1, 95% CI 1.08-4.11), but only MVI was an independent predictive factor of TTP shorter than 5 months (HR 3.42, 95% CI 1.72-6.81). Regarding safety data, 76.5% of patients reported at least one side effect (any grade), and 19.1% presented grade III-IV adverse effects leading to treatment discontinuation. Conclusions: We observed no significant difference in survival or TTP in Child-Pugh A or Child-Pugh B patients treated with sorafenib, as compared to more recent real-life studies. Lower primary LS and AFP were associated with a better out-come, and lower AFP was the main predictor of survival. The reality of systemic treatment for advanced HCC has recently changed and continues to evolve, but sorafenib remains a viable therapeutic option.
Resumo Introdução: O sorafenib foi o primeiro fármaco usado em primeira linha na terapêutica sistémica do carcinoma he-patocelular (CHC) em estadio avançado. Têm sido descritos múltiplos factores modificadores de prognóstico associados à sua utilização. Objectivos: Caracterizar um grupo de doentes com CHC que realizaram terapêutica com sorafenib, estudar a sobrevivência e o tempo até progressão (TAP), e avaliar os factores preditores de benefício. Material e Métodos: Estudo retrospectivo com recol-ha e análise dos dados relativos a todos os doentes com CHC tratados com sorafenib numa Unidade de Hepatologia, entre 2008 e 2018. Resultados: Foram incluídos no estudo sessenta e oito doentes; 80.9% do sexo masculino, com mediana de idades de 64.5 anos, 57.4% tinham cirrose em estadio A de Child-Pugh e 77.9% apresentavam CHC em estadio C do Barcelona Clínic Liver Cancer (BCLC). A invasão macrovascular (IMV) estava presente em 25% dos doentes, e também 25% dos doentes tinha metastização extra-hepática (que não a IMV). A mediana de sobrevivência foi de 10 meses (IQR 6.0-14.8) e a mediana de TAP foi de 5 meses (IQR 2.0-7.0). A sobrevivência e o TAP foram similares nos doentes Child-Pugh A e B: 11.0 meses (IQR 6.0-18.0) para Child-Pugh A e 9.0 meses (IQR 5.0-14.0) para Child-Pugh B (p = 0.336). Na análise univariada, o tamanho da lesão >5 cm (TL), alfa-fetoproteína > 50 ng/mL (AFP) e a ausência de terapêuticas locorregionais prévias (TLP) tiveram relação estatisticamente significativa com a mortalidade (TL: HR 2.17, 95% CI 1.24-3.81; AFP: HR 3.49, 95% CI 1.90-6.42; TLP: HR 0.54, 95% CI 0.32-0.93), mas apenas o TL e AFP foram fatores preditores independentes, como mostrou a análise multivariada (TL: HR 2.08, 95% CI 1.10-3.96; AFP: HR 3.13, 95% CI 1.59-6.16). A IMV e o TL >5 cm estiveram associados com o TAP <5 meses na análise univariada (IMV: HR 2.80, 95% CI 1.47-5.35; TL: HR 2.1, 95% CI 1.08-4.11), mas apenas a IMV foi um fator preditor independente de TAP <5 meses (HR 3.42, 95% CI 1.72-6.81). Relativamente aos dados de segurança, 76.5% dos doentes relataram pelo menos um efeito lateral (qualquer grau), e 19.1% apresentaram efeitos adversos de grau III-IV, que levaram à suspensão do fármaco. Conclusões: Não foi observada diferença significativa na sobrevivência ou no tempo até progressão nos doentes Child-Pugh A ou Child-Pugh B tratados com sorafenib, quando comparado com estudos real-life recentes. Menor TL e AFP estiveram associados a melhor outcome e um valor de AFP baixo mostrou-se o principal preditor de sobrevivência. A realidade da terapêutica sistémica para o CHC avançado alterou-se recentemente e continua em mudança, mas o sorafenib permanece uma alternativa terapêutica viável.

Abstract Background: prone Position is a well-established evidence-based clinical practice in patients with Acute Respiratory Distress Syndrome undergoing invasive mechanical ventilation. Little evidence in non-intubated patients is known. Due to the increasing rate of acute respiratory distress syndrome, the need to know the effectiveness in non-intubated patients has emerged. Objective: to know the effectiveness of prone position in the treatment of acute respiratory distress syndrome in non-intubated adults. Methodology: Systematic reviews based on the manual of Joanna Briggs Institute and carried out using PICO strategy. Research conducted in August 2021 in the access platforms Web of Science, PubMed, and EBSCO Host. Selection was done after elimination of duplicates, title reading, reading of abstracts, and full texts according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses diagram. Results: 741 results were identified, and seven were included in the review. Conclusion: Early prone position is advantageous in non-intubated patients with mild and moderate acute respiratory distress syndrome. A risk intervention in non-intubated patients with Severe Acute Respiratory Distress Syndrome. Mortality/survival, physiological/clinical, adverse events/effects and functioning outcomes were identified as predictors of success/failure of prone position.
Resumo Enquadramento: o decúbito ventral é uma prática clínica baseada na evidência que se encontra bem estabelecida em pacientes com Acute Respiratory Distress Syndrome submetidos a ventilação mecânica invasiva. Porém, existe parca evidência em pacientes não intubados. Face ao aumento da taxa de Acute Respiratory Distress Syndrome, emergiu a necessidade de conhecer a sua eficácia em pacientes não intubados. Objetivo: conhecer a efetividade do decúbito ventral no tratamento da Acute Respiratory Distress Syndrome em adultos não intubados. Metodologia: revisão sistemática fundamentada no manual de Joanna Briggs Institute, com recurso à estratégia PICO. Pesquisa realizada em agosto de 2021, nas plataformas de acesso Web of Science, PubMed e EBSCO Host. Seleção realizada após eliminação de duplicados, leitura do título, de resumos e textos integrais de acordo com o diagrama Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Resultados: identificaram-se 741 resultados, sendo sete os incluídos na revisão. Conclusão: O decúbito ventral precoce revelou-se vantajoso em pacientes não intubados com Acute Respiratory Distress Syndrome ligeira e moderada. É uma intervenção de risco em pacientes não intubados com Acute Respiratory Distress Syndrome grave. Foram identificados indicadores de mortalidade/ sobrevida, fisiológicos/ clínicos, de eventos/ efeitos adversos e indicadores funcionais como preditores de sucesso/ insucesso do decúbito ventral.

Hay poca evidencia en pacientes no entubados. La creciente tasa de síndrome de distrés respiratorio agudo originó la necesidad de desarrollar esfuerzos para conocer su efectividad en pacientes no intubados. Objetivo: conocer la efectividad del decúbito prono en el tratamiento del síndrome de distrés respiratorio agudo en adultos no intubados. Metodología: revisión sistemática basadas en el manual del Instituto Joanna Briggs y mediante la estrategia PICO. La búsqueda ocurrió en agosto 2021 en las plataformas de acceso Web of Science, PubMed y EBSCO Host. Selección realizada por previa eliminación de duplicados, lectura de títulos, resúmenes y textos completos de acuerdo con el diagrama Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Resultados: se identificaron 741 resultados, siete incluyeron la revisión. Conclusión: la posición prona temprana demostró ser ventajosa en pacientes no intubados con Síndrome de Dificultad Respiratoria Aguda leve y moderado. Intervención riesgosa en pacientes no intubados con Síndrome de Dificultad Respiratoria Aguda grave. Indicadores de mortalidad/supervivencia, fisiológicos/clínicos, de eventos/efectos adversos e indicadores funcionales, fueron identificados como predictores de éxito/fracaso de la posición prono.

Nonalcoholic fatty liver disease in adolescents with phenylketonuria

Abstract Objective: This study aimed to investigate non-alcoholic fatty liver disease (NAFLD) occurrence and factors associated with the disease in phenylketonuria (PKU) patients undergoing exclusive dietary treatment. Method: This cross-sectional study included 101 adolescents 10 to < 20 years of age with PKU, who were undergoing exclusive dietary treatment and monitored since early diagnosis at a single reference service. Anthropometric and biochemical assessments were performed and food intake was documented, and an ultrasound diagnosis of NAFLD was established. Data were evaluated using the Student’s t-test for continuous variables, the chi-square for categorical variables, and logistic regression using the Wald chi-squared test; differences with p < 0.05 were considered to be statistically significant. Results: NAFLD was detected in 26 (25.7%) teenagers. There was no difference in prevalence between the sexes or nutritional status. The final logistic regression model revealed low sensitivity (26.1%) and high specificity (94.7%). The specificity suggested a lower likelihood of NAFLD in older adolescents, in the presence of normal or high levels of alkaline phosphatase, lower carbohydrate intake, and adequate protein and lipid intake. Conclusions: The prevalence of NAFLD in adolescents with PKU was higher than that found in healthy Brazilian adolescents and similar to that found in obese Brazilian children, suggesting a higher risk for NAFLD in patients with PKU treated exclusively by dietary modification.
