Resumo As síndromes poliglandulares autoimunes (PAS) são um grupo complexo e heterogéneo de doenças caracterizado pela combinação de duas ou mais disfunções endócrinas autoimunes. É apresentado o caso de uma adolescente de 16 anos de idade com história de astenia, prostração e perda de peso, que foi diagnosticada com hipotiroidismo e anemia ferropénica. Duas semanas após ter iniciado tratamento com levotiroxina e ferro, desenvolveu insuficiência suprarrenal com náuseas e vómitos. Os níveis de cortisol baixo e hormona adrenocorticotrópica elevada e presença de anticorpos anti-suprarrenal e antiperoxidase tiroideia positivos confirmaram o diagnóstico de síndrome poliglandular autoimune tipo 2 (PAS-2). O tratamento com hidrocortisona, fludrocortisona e levotiroxina levou à remissão dos sintomas A PAS-2 é uma condição rara, sendo necessária uma elevada suspeita clínica para a mesma, dado que a maioria dos sintomas são inespecíficos. O diagnóstico precoce pode prevenir complicações potencialmente fatais.
Abstract Autoimmune polyglandular syndromes (PAS) are a complex and heterogeneous group of diseases characterized by the combination of two or more endocrine or non-endocrine organ dysfunctions. Herein is presented the case of a 16-year-old female with a history of asthenia, prostration, and weight loss who was diagnosed with hypothyroidism and iron deficiency anemia. Two weeks after starting therapy with levothyroxine and iron, she developed an adrenal crisis with nausea and vomiting. Low cortisol, elevated adrenocorticotropic hormone, and positive antiadrenal and antithyroid peroxidase antibodies confirmed the diagnosis of autoimmune polyglandular syndrome type 2 (PAS-2). Symptoms resolved with hydrocortisone, fludrocortisone, and levothyroxine. PAS-2 is a rare but potentially serious disorder. A high clinical suspicion is required, as most symptoms are nonspecific. Early diagnosis may prevent life-threatening complications.

ABSTRACT Purpose Varicocele is a condition known to cause damage to seminal parameters and sperm function. Furthermore, it has been hypothesized that the varicocele effect on fertility is time-dependent; however, little is known about the consequences of its establishment time on reproductive organs and/or sperm function. This study aimed to evaluate the effect of the duration of experimental varicocele on reproductive organs, sperm parameters, and sperm function. Materials and Methods Varicocele induction surgeries were performed in Wistar rats aged 40 or 100 days old. At 160-day-old, analyses were performed, including biometry of reproductive organs (prostate, seminal vesicles, epididymis, and testis), sperm parameters (vitality, morphology, and motility), and sperm function tests (nuclear DNA integrity, acrosome integrity, and mitochondrial activity). Results The analysis of the biometry of reproductive organs showed no differences between distinct ages in which varicocele was induced. The total abnormal sperm morphology was bigger in animals with varicocele induced to 100 days old than in animals with varicocele induced to 40 days old. Regarding nuclear DNA integrity, animals of varicocele induced to 100 days old showed worse results compared to animals of varicocele induced to 40 days old. Other parameters analyzed showed no differences between varicocele groups. Conclusion In this study conducted on rats, we conclude that varicocele adversely affects sperm, particularly its function. Abstract Creating artificial caries-like lesions that mimic the complex changes observed in natural caries is essential for properly evaluating new strategies, dental materials, and devices designed to arrest their progression and avoid more costly and invasive treatments. Objective This study compared three protocols for developing artificial white spot lesions (WSL) using biofilm models. Methodology In total, 45 human enamel specimens were sterilized and allocated into three groups based on the biofilm model: Streptococcus sobrinus and Lactobacillus casei (Ss+Lc), Streptococcus sobrinus (Ss), or Streptococcus mutans (Sm). Specimens were incubated in filter-sterilized human saliva to form the acquired pellicle and then subjected to the biofilm challenge consisting of three days of incubation with bacteria (for demineralization) and one day of remineralization, which was performed once for Ss+Lc (four days total), four times for Ss (16 days total), and three times for Sm (12 days total). After WSL creation, the lesion fluorescence, depth, and chemical composition were assessed using Quantitative Light-induced Fluorescence (QLF), Polarized Light Microscopy (PLM), and Raman Spectroscopy, respectively. Statistical analysis consisted of two-way ANOVA followed by Tukey’s post hoc test (α=0.05). WSL created using the Ss+Lc protocol presented statistically significant higher fluorescence loss (ΔF) and integrated fluorescence (ΔQ) in comparison to the other two protocols (p<0.001). Results In addition, Ss+Lc resulted in significantly deeper WSL (137.5 µm), followed by Ss (84.1 µm) and Sm (54.9 µm) (p<0.001). While high mineral content was observed in sound enamel surrounding the WSL, lesions created with the Ss+Lc protocol showed the highest demineralization level and changes in the mineral content among the three protocols. RESUMO Objetivos: Analisar a percepção dos enfermeiros sobre os métodos de trabalho em enfermagem no contexto hospitalar. Métodos: Estudo descritivo com abordagem qualitativa, realizado num hospital do norte de Portugal, com participação de 17 enfermeiros. Como instrumento de coleta de dados, recorreu se à entrevista semiestruturada. Os dados, recolhidos entre maio e junho de 2023, passaram por análise de conteúdo, com suporte do software Atlas.ti. Resultados: Emergiram três áreas temáticas: “Métodos de trabalho dos enfermeiros em contexto hospitalar”, destacando-se a concepção e componentes dos métodos de trabalho e os métodos em uso; “Implementação dos métodos de trabalho”, salientando-se fatores influenciadores e desafios à implementação; e “Impacto dos métodos de trabalho dos enfermeiros para os pacientes, enfermeiros e instituições”. Considerações Finais: Os métodos de trabalho constituem-se como a estrutura da assistência de enfermagem. Existem fatores que influenciam e desafios que se colocam à implementação desses métodos, produzindo impacto nos pacientes, enfermeiros e instituições. ABSTRACT Objectives: To analyze nurses’ perspectives on nurses’ work methods in the hospital context. Methods: A descriptive study with a qualitative approach was conducted in a hospital in northern Portugal, involving 17 nurses. Semi-structured interviews were used for data collection. Data collected between May and June 2023 underwent content analysis, supported by Atlas.ti software. Resumo O presente trabalho teve como objetivo avaliar a eficiência de taninos quimicamente modificados como coagulantes para tratamento de água. Os taninos das cascas de Anacardium occidentale (cajueiro) e Mimosa caesalpiniifolia (sabiá) e dos frutos sem sementes de Anadenanthera colubrina (angico) e Pityrocarpa moniliformis (angico de bezerro) foram avaliados após cationização para remover a turbidez da água. Após a extração, os extratos de taninos tiveram o teor de sólidos totais (TST), o índice de Stiasny (IS) e o teor de taninos totais condensados (TCT). Os taninos extraídos foram convertidos em pó e cationizados. Em seguida, avaliou-se a eficácia dos taninos cationizados na remoção da turbidez e o tempo de sedimentação além do efeito no pH da água. Para avaliar a eficiência dos taninos cationizados como floculantes foram utilizadas três concentrações de cada tipo de material: 50, 100 e 150 mg L-1. Embora o extrato de P. moniliformis tenha apresentado maior valor de TST (56,06%), apresentou os menores valores de IS (0,42%) e TCT (0,23%). Os taninos da casca de M. caesalpiniifolia apresentou o menor TST (9,18%) e o maior IS (91,27%), enquanto os derivados de A. occidentale se destacaram pelo maior TCT (19,83%). Todas as espécies demonstraram potencial para produção de taninos e foram eficientes em remover a turbidez da água em todas as concentrações. Abstract This research assessed the efficiency of chemically modified tannins as coagulants for water treatment. Tannins from Anacardium occidentale (cashew tree) and Mimosa caesalpiniifolia (‘sabiá’) barks and seedless fruits of Anadenanthera colubrina (‘angico’) and Pityrocarpa moniliformis (‘angico de bezerro’) were evaluated after cationization to remove turbidity from water. After extraction, the total solids content (TSC), Stiasny index (SI), and total condensed tannins content (TCC) of the tannins extracts were determined. Extracted tannins were converted into powders and cationized. Then, the cationized tannins' effectiveness on water turbidity removal, sedimentation time, and effect on pH were assessed. Three concentrations of each type of tannin were used to evaluate flocculant efficiency: 50, 100, and 150 mg L-1. Although the P. moniliformis extract had a higher TSC value (56.06%), it presented the lowest SI (0.42%) and TTC (0.23%) values. M. caesalpiniifolia had the lowest TSC (9.18%) and the highest SI (91.27%), while Ac occidentale stood out with the highest TCC (19.83%). All species have shown potential for tannin production and efficiently removed turbidity at all concentrations. RESUMO Objetivo Verificar a associação entre a restrição à participação decorrente de perda auditiva e a autopercepção da saúde, do suporte social e da qualidade de vida em pessoas idosas. Método Estudo com delineamento transversal, observacional, descritivo e com abordagem quantitativa dos dados. Foi utilizado um banco de dados com informações coletadas em um município brasileiro de médio porte, sendo incluídas no estudo 235 pessoas cadastradas em cinco unidades de Estratégia de Saúde da Família. Utilizou-se informações sociodemográficas, de saúde e os resultados dos questionários: avaliação à restrição à participação (Hearing Handicap Inventory for the Elderly Screening Version - HHIE-S), Suporte Social (Escala de Apoio Social do MOS), Avaliação Subjetiva de Saúde e a Qualidade de Vida (Short-Form 6 dimensions- SF-6D). Comparou-se os grupos com restrição e sem restrição à participação segundo as variáveis sociodemográficas, de saúde, suporte social e qualidade de vida. Um método de regressão logística binária multivariado foi utilizado para avaliar as associações entre as variáveis independentes e a restrição à participação. Resultados O grupo com restrição à participação é mais velho, possui menor qualidade de vida e pior autopercepção de saúde. Esta se mostrou ser o único preditor da restrição à participação relacionada à perda auditiva. Conclusão A restrição à participação está associada a uma pior autopercepção de saúde. ABSTRACT Purpose To verify the association between participation restriction due to hearing loss and self-perception of health, social support, and quality of life in elderly people. Methods This is a cross-sectional, observational, and descriptive study with a quantitative data approach. A database with information collected in a medium-sized Brazilian municipality was used. The study was conducted with 235 elderly people registered in five Family Health Strategy Units. Sociodemographic and health information and the results of the following questionnaires were used: Hearing Handicap Inventory for the Elderly – Screening Version (HHIE-S), Medical Outcomes Study (MOS) Social Support Survey, Subjective Health Assessment, and Short-Form 6-Dimension (SF-6D) Health and Quality of Life Index. Groups with and without participation restriction were compared according to sociodemographic, health, social support, and quality of life variables. A multivariate binary logistic regression method was employed to evaluate the associations between the independent variables and participation restriction. Results The group with participation restriction is composed of older individuals with lower quality of life and poorer self-perception of health. Poorer self-perception of health was the only predictor of participation restriction related to hearing loss. Conclusion Participation restriction is associated with poorer self-perception of health. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. RESUMO A modificação térmica da madeira é um processo que altera sua composição química, visando melhorar propriedades tecnológicas como estabilidade dimensional, redução do teor de umidade de equilíbrio, homogeneidade ou cor. Neste contexto, este estudo teve como objetivo avaliar a colorimetria e as propriedades químicas da madeira de Parkia pendula (angelim-saia) e Simarouba amara (marupá) modificada termicamente por diferentes métodos. Foram avaliados três tratamentos: T1 (pré-tratamento em estufa por uma hora a 120 °C e tratamento em estufa a 180 °C por três horas); T2 (pré-tratamento em autoclave a 125 °C por três horas sob 1,2 kgf cm-2 e tratamento em estufa a 180 °C por três horas); T3 (sem pré-tratamento, com tratamento da amostra em óleo vegetal). A madeira termicamente modificada foi avaliada em relação à madeira não tratada quanto à composição química estrutural (extrativos, lignina, holocelulose e teor de cinzas), parâmetros colorimétricos definidos através do sistema CIELAB (Commission Internationale de l´Eclairage) e variação total de cores. Detectamos um aumento significativo no teor de extrativos e uma diminuição no teor de holocelulose e lignina em T3 para ambas espécies, o que pode ser explicado pela impregnação de óleo nas amostras de madeira. A modificação térmica provocou o escurecimento superficial da madeira de ambas espécies, que foi mais pronunciado em P. pendula. ABSTRACT The thermal modification of wood is a process that alters its chemical composition, aiming to improve technological properties such as dimensional stability, reduction of equilibrium moisture content, homogeneity, or color. RESUMO Objetivo: analisar a taxa e as causas de cancelamento em unidade de cirurgia ambulatorial em uma policlínica universitária integrante do Sistema Único de Saúde e situada no município do Rio de Janeiro. Método: estudo transversal, desenvolvido em centro cirúrgico ambulatorial de uma policlínica universitária da cidade do Rio de Janeiro, Brasil. Dados do período de agosto de 2021 a julho de 2022 foram extraídos do banco de dados da instituição via formulário contendo variáveis sociodemográficas e clínica dos pacientes, mês, especialidade cirúrgica, causas e período do cancelamento. Para variável faixa etária utilizou-se o teste de Wilcoxon-Mann-Whitney, e para variáveis mês e especialidade, o teste qui-quadrado de Pearson, adotado nível de significância de 5%. Resultados: dentre as 2.147 cirurgias ambulatoriais agendadas, 334 foram canceladas, com taxa de cancelamento cirúrgico anual de 15,55%. Houve diferença estatística significativa nos cancelamentos ocorridos no mês de dezembro (p= 0,010), na especialidade cirúrgica vascular (p= 0,001) e em pacientes idosos (p= 0,007). Foram encontradas dez causas de cancelamento, sendo as mais frequentes relacionadas à falta do paciente (n=117; 35,03%), às condições clínicas desfavoráveis (n=92; 27,54%) e à inconformidade no preparo pré-operatório (n=30; 8,98%). Conclusão: a taxa de cancelamento cirúrgico foi elevada, sobretudo pela falta e pelas condições clínicas do paciente no dia da cirurgia. Espera-se que os dados possam contribuir para subsidiar e sensibilizar a participação ativa de todos os profissionais envolvidos em cirurgia ambulatorial, de modo a evitar cancelamento. ABSTRACT Objective: to analyze the rate and causes of cancellations in an outpatient surgery unit at a university polyclinic that is part of the Unified Health System and located in the city of Rio de Janeiro. Method: a cross-sectional study carried out in the outpatient surgical center of a university polyclinic in the city of Rio de Janeiro, Brazil. The data for the period August 2021 to July 2022 was extracted from the institution's database using a form containing the patients' sociodemographic and clinical variables, month, surgical specialty, causes and period of cancellation. The Wilcoxon-Mann-Whitney test was used for the age group variable, and Pearson's chi-square test was used for the month and specialty variables, with a significance level of 5%. Results: of the 2,147 outpatient surgeries scheduled, 334 were canceled, with an annual surgical cancellation rate of 15.55%. There was a statistically significant difference in cancellations in December (p= 0.010), in the vascular surgery specialty (p= 0.001) and in older adults (p= 0.007). A total of ten causes of cancellation were found, the most frequent being patient absence (n=117; 35.03%), unfavorable clinical conditions (n=92; 27.54%) and non-compliance with preoperative preparation (n=30; 8.98%). Conclusion: the surgical cancellation rate was high, mainly due to the patient's absence and clinical conditions on the day of surgery. It is hoped that the data will help to subsidize and raise awareness of the active participation of all professionals involved in outpatient surgery, in order to avoid cancellations. RESUMO Objetivos: adaptar e validar o conteúdo da Healthy Work Environment Assessment Tool para a cultura brasileira, e avaliar os aspectos práticos de sua aplicação. Métodos: estudo metodológico que seguiu seis estágios: tradução, síntese, retrotradução, validação de conteúdo por um grupo de especialistas, pré-teste e aprovação do processo pelo autor do instrumento original. Resultados: os três primeiros estágios foram realizados por empresas contratadas. No comitê, dois itens e o título de uma subescala foram avaliados em uma segunda rodada, na qual se alcançou um consenso entre os especialistas. No pré-teste, mais de 93% dos profissionais concordaram com a facilidade de compreensão da ferramenta. O tempo médio de preenchimento foi de 8,53 minutos. A American Association of Critical-Care Nurses autorizou a publicação dos resultados. Conclusões: a adaptação da ferramenta para a cultura brasileira foi concluída seguindo o referencial adotado. ABSTRACT Objectives: to adapt and validate the content of the Healthy Work Environment Assessment Tool for Brazilian culture, and evaluate the practical aspects of its application. ABSTRACT Background: Cases of coronavirus disease 2019 (COVID-19) requiring hospitalization continue to appear in vulnerable populations, highlighting the importance of novel treatments. The hyperinflammatory response underlies the severity of the disease, and targeting this pathway may be useful. Herein, we tested whether immunomodulation focusing on interleukin (IL)-6, IL-17, and IL-2, could improve the clinical outcomes of patients admitted with COVID-19. Methods: This multicenter, open-label, prospective, randomized controlled trial was conducted in Brazil. Sixty hospitalized patients with moderate-to-critical COVID-19 received in addition to standard of care (SOC): IL-17 inhibitor (ixekizumab 80 mg SC/week) 1 dose every 4 weeks; low-dose IL-2 (1.5 million IU per day) for 7 days or until discharge; or indirect IL-6 inhibitor (colchicine) orally (0.5 mg) every 8 hours for 3 days, followed by 4 weeks at 0.5 mg 2x/day; or SOC alone. The primary outcome was accessed in the “per protocol” population as the proportion of patients with clinical improvement, defined as a decrease greater or equal to two points on the World Health Organization’s (WHO) seven-category ordinal scale by day 28. Results: All treatments were safe, and the efficacy outcomes did not differ significantly from those of SOC. Interestingly, in the colchicine group, all participants had an improvement of greater or equal to two points on the WHO seven-category ordinal scale and no deaths or patient deterioration were observed. Conclusions: Ixekizumab, colchicine, and IL-2 were demonstrated to be safe but ineffective for COVID-19 treatment. These results must be interpreted cautiously because of the limited sample size. ABSTRACT Background: Complications of leprosy neuritis are considered serious and apparent, with the potential to disable and/or limit individuals. These complications affect not only a patient’s physical functioning, but also their family and social lives, while directly impacting the ability to work and/or maintain financial independence, subsequently interfering with their overall quality of life. The present review, therefore, aimed to analyze the effectiveness of neurolysis as an alternative treatment for the complications associated with leprosy neuritis. Methods: The present review was performed based on the Joanna Briggs Institute methodology, in an effort to answer the following research question: what is the effectiveness of neurolysis as a treatment for leprosy neuritis complications? This research question was defined using the patient-intervention-outcome (PIO) framework, where leprosy represents ‘P’, neurolysis for ‘I’, and neuropathic pain/motor function/sensorial function/physical disability/quality of life for ‘O’. Randomized and non-randomized clinical trials and prospective observational cohort studies were included in the present review, with no time or date restrictions. Results: The present review included 1 randomized clinical trial and 10 prospective studies, published between 1976 and 2020. All of the outcomes showed improvement, with relief from neuropathic pain being the primary finding. Conclusions: The evidence obtained in the present review suggested that neurolysis is an effective alternative for the treatment of physical disabilities, the recovery of sensory and motor function, the restoration of quality of life, and neuropathic pain relief. Método: es un revisión de alcance siguiendo las pautas de Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviewse. Las búsquedas se realizaron en bases de datos y portales de tesis y disertaciones, en septiembre de 2021, y se incluyeron artículos, tesis y disertaciones. Se realizó una búsqueda independiente en las tiendas online de aplicaciones en sistemas operativos Androide e iOS, en octubre y diciembre de 2021, y aplicaciones seleccionadas con contenido para apoyar a los padres de los recién nacidos. Resultados: Se identificaron 5238 estudios y 757 aplicaciones, y de estos, 16 y 150 conformaron la muestra, respectivamente. Los temas discutidos en los estudios fueron: cuidados, lactancia materna, fiebre, identificación de enfermedades neonatales, crecimiento y desarrollo infantil. En las aplicaciones, los temas encontrados fueron: cuidado, lactancia, crecimiento, inmunización, desarrollo, sueño, consejos y orientaciones. Tuberculosis preventive treatment in children and adolescents: an observational study of secondary data adolescents

Abstract Objective To analyze the effectiveness, safety, outcomes, and associated factors of tuberculosis preventive treatment (TPT) in children and adolescents in Paraná, southern Brazil. Method This was an observational cohort study with a retrospective collection of secondary data from the TPT information systems of the state of Paraná from 2009 to 2016, and tuberculosis in Brazil from 2009 to 2018. Results In total, 1,397 people were included. In 95.4% of the individuals, the indication for TPT was a history of patient-index contact with pulmonary tuberculosis. Isoniazid was used in 99.9% of the cases with TPT, and 87.7% completed the treatment. The TPT protection was 98.7%. Among the 18 people who had TB, 14 (77.8%) became ill after the second year of treatment, and four (22.2%) in the first two years (p < 0.001). Adverse events were reported in 3.3% of cases, most of them were gastrointestinal and medication was discontinued in only 2 (0.1%) patients. No risk factors associated with the illness were observed. Conclusions The authors observed a low rate of illness in pragmatics routine conditions in TPT for children and adolescents, especially within the first two years after the end of treatment, with good tolerability and a good percentage of adherence to the treatment.
