Abstract Introduction: Autoimmune hepatitis (AIH) has a spectrum of symptoms ranging from asymptomatic disease to acute severe hepatitis, chronic hepatitis, and decompensated cirrhosis. The acute presentation is not rare and could represent genuine acute AIH (GAAIH) or acute exacerbation of chronic autoimmune hepatitis. We aimed to identify the prevalence, clinical features, and prognostic factors associated with GAAIH and compare these cases with acute exacerbation of chronic AIH. Methods: This cross-sectional observational study evaluated patients with acute AIH presentation, defined as total bilirubin >5 times the upper limit of normality (xULN) and/or alanine aminotransferase >10 xULN, and no prior history of liver disease. Histology findings of acute disease defined GAAIH. Bivariate analyses were performed to identify factors associated with the GAAIH, when compared with acute exacerbation of chronic AIH. Results: Seventy-two patients with acute presentation of AIH were included and six (8.3%) of them presented GAAIH. Comparative analysis between patients with GAAIH and patients with acute exacerbation of chronic AIH revealed that prothrombin activity (96% [74-100] vs. 61% [10-100]; p = 0.003) and albumin levels (3.9 ± 0.2 g/dL vs. 3.4 ± 0.5 g/dL; p < 0.001) were higher in patients with GAAIH. The International Autoimmune Hepatitis Group score was higher in patients with acute exacerbation of chronic AIH (18.5 [8-23] vs. 16.5 [15-17]; p = 0.010). Compared to 15.2% of acute exacerbation of chronic AIH, complete therapeutic response to treatment was achieved in 67.7% of cases with GAAIH (p = 0.018). Conclusions: GAAIH was rare (8.3%), and patients with this presentation exhibited more preserved liver function tests, suggesting that most cases presenting with loss of function are acute exacerbation of chronic AIH. Additionally, patients with GAAIH had a better complete therapeutic response, suggesting a more preserved liver function at presentation, and early diagnosis has a positive therapeutic implication.
Resumo Introdução: A hepatite autoimune (HAI) apresenta um espectro de sintomas que varia de doença assintomática a hepatite aguda grave, hepatite crónica e cirrose descompensada. A apresentação aguda não é rara e pode representar hepatite autoimune aguda genuína (HAIAG) ou exacerbação aguda de hepatite autoimune crónica (EAHAIC). O nosso objetivo foi identificar a prevalência, caraterísticas clínicas e fatores prognósticos associados à HAIAG, e comparar esses casos com EAHAIC. Métodos: Estudo observacional, transversal, incluindo doentes com apresentação aguda de HAI, definida como bilirrubina total > 5 vezes o limite superior da normalidade (xLSN) e/ou ALT > 10 xLSN, e sem história prévia de doença hepática. HAIAG foi definida pela presença de achados histológicos de doença aguda. Análises bivariadas foram realizadas para identificar fatores associados à HAIAG, quando comparado com o EAHAIC. Resultados: Foram incluídos setenta e dois doentes com apresentação agu-da de HAI, dos quais seis (8.3%) com HAIAG. A análise comparativa entre doentes com HAIAG e doentes com EAHAIC mostrou que a atividade de protrombina (96%(74-100) versus 61% (10-100; p=0.003) e os níveis de albumina (3,9 ± 0,2 g/dL vs. 3,4 ± 0,5 g/dL; p < 0,001) foram significativamente mais elevados em pacientes com HAIAG. O score do Grupo Internacional de Hepatite Autoimune foi mais elevado em doentes com EAHAIC (18.5 (8-23) versus 16.5 (15-17); p=0.010). A resposta terapêutica completa ao tratamento foi alcançada em 66.7% dos casos de HAIAG (vs. 15,2% na EAHAIC, p=0,018). Conclusões: A HAIAG é rara (8.3%), e os doentes com esta apresentação mostraram testes de função hepática mais preservados, sugerindo que a maioria dos casos com perda de função são EAHAIC. Além disso, os doentes com HAIAG tiveram maior taxa de resposta terapêutica completa, sugerindo que uma função hepática mais preservada na apresentação e o diagnóstico precoce tem uma implicação terapêutica positiva.

Resumen: Este estudio tuvo como objetivo investigar las asociaciones entre las percepcion del vecindario y los problemas de sueño en ancianos brasileños. Se realizó un estudio transversal con 5.719 ancianos de la comunidad (≥ 60 años) del Estudio Longitudinal de Salud de los Ancianos Brasileños (ELSI-Brasil, 2019-2021). Los desenlaces fueron los siguientes: problemas de sueño autoinformados: mala calidad del sueño, somnolencia diurna, quejas de insomnio primario, dificultad para permanecer dormido y despertarse durante la madrugada. Las variables de exposición fueron las siguientes: preguntas sobre la percepción de los participantes sobre el entorno físico y social del vecindario. En el análisis de datos se utilizó la regresión logística. La basura, la suciedad o el césped alto en las calles y el deseo de cambiar el lugar donde viven se asociaron con mayores probabilidades de tener una mala calidad del sueño. La preocupación por las caídas debido a aceras en mal estado, la preocupación por las dificultades para utilizar los medios de transporte y la preocupación por las dificultades para cruzar la calle se asociaron con mayores probabilidades de sufrir todos los problemas de sueño. El ruido producido por los autobuses y automóviles se asoció con una mayor probabilidad de sufrir algunos problemas de sueño. La percepción del vecindario como un buen lugar para vivir se asoció con menores probabilidades de sufrir somnolencia diurna y quejas de insomnio primario. La confianza en la mayoría de la gente del vecindario y la percepción de que los niños y jóvenes tratan a los adultos con respeto se asociaron con menores probabilidades de sufrir somnolencia diurna, quejas de insomnio primario y despertarse durante la madrugada. Un buen lugar para que los niños jugaran y para que los adolescentes crecieran se asoció con menores probabilidades de sufrir somnolencia diurna. Estos resultados pueden ayudar a los gestores públicos a crear políticas de planificación urbana dirigidas a mejorar los entornos vecinales como forma de promover la salud.
Resumo: Este estudo teve como objetivo investigar associações entre percepções da vizinhança e problemas de sono em idosos brasileiros. Foi realizado um estudo transversal com 5.719 idosos da comunidade (≥ 60 anos) do Estudo Longitudinal da Saúde dos Idosos Brasileiros (ELSI-Brasil, 2019-2021). Os desfechos foram problemas de sono autorreferidos: má qualidade do sono, sonolência diurna, queixas de insônia primária, dificuldade em manter o sono e acordar na madrugada. As variáveis de exposição foram: questões sobre a percepção dos participantes sobre o ambiente físico e social da vizinhança. Regressão logística foi utilizada na análise dos dados. Lixo, sujeira ou grama alta nas ruas e o desejo de mudar de onde mora foram associados a maiores chances de má qualidade do sono. Preocupação em cair devido a calçadas em má condição, preocupação com dificuldades para usar os meios de transporte e preocupação com dificuldades para atravessar a rua foram associadas a maiores chances de todos os problemas de sono. O som/ruído dos ônibus e carros foi associado a maiores chances de alguns problemas de sono. A percepção da vizinhança como um bom lugar para morar foi associado a menores chances de sonolência diurna e queixas de insônia primária. A confiança na maioria das pessoas da vizinhança e a percepção de que crianças e jovens tratam os adultos com respeito foram associados a menores chances de sonolência diurna, queixas de insônia primária e acordar na madrugada. Um bom lugar para as crianças brincarem e para os adolescentes crescerem foi associado a menores chances de sonolência diurna. Esses resultados podem ajudar os gestores públicos na criação de políticas de planejamento urbano voltadas a melhorias nos ambientes da vizinhança como forma de promoção da saúde.
Abstract: This study aimed to investigate associations between neighborhood perception and sleep problems in older Brazilian adults. A cross-sectional study was conducted with 5,719 community-dwelling older adults (≥ 60 years) from the Brazilian Longitudinal Study of Aging (ELSI-Brazil, 2019-2021). The outcomes were self-reported sleep problems: poor sleep quality, daytime sleepiness, primary insomnia complaints, difficulty staying asleep, and waking up at dawn. The exposure variables were questions about the perception of participants about the physical and social environment of the neighborhood. Logistic regression was used in data analysis. Garbage, rubbish, or tall grass on the streets and the desire to move were associated with higher odds of poor sleep quality. Concern about falling due to damaged sidewalks, concern about having difficulties taking transportation, and concern about having difficulties crossing the street were associated with higher odds of all sleep problems. Sound/noise of buses and cars was associated with higher odds of some sleep problems. Perceiving the neighborhood as a good place to live was associated with lower odds of daytime sleepiness and primary insomnia complaints. Trusting most people in the neighborhood and perceiving that kids and younger people treat adults with respect were associated with lower odds of daytime sleepiness, primary insomnia complaints, and waking up at dawn. Being a good place for kids to play and raise teenagers was associated with lower odds of daytime sleepiness. These results can assist public administrators in creating urban planning policies aimed at improving neighborhood environments as a means of health promotion.

RESUMEN Objetivo. Describir el patrón temporal y espacial, y determinar los factores asociados a la incidencia de infección por el VIH/sida en jóvenes en Brasil. Método. Estudio ecológico en jóvenes brasileños de 15 a 24 años con diagnóstico de infección por el VIH/sida en el período 2001-2021. Para el análisis temporal se utilizó el método de regresión de puntos de inflexión (joinpoint). Los conglomerados espaciales se detectaron con métodos Bayesianos y de autocorrelación espacial, Gi* de Getis-Ord y escaneo. Se utilizaron cuatro modelos de regresión espacial y no espacial para detectar los factores asociados al resultado. En todos los análisis estadísticos se estableció un valor de p < 0,05 como umbral de significación. Resultados. En Brasil, la incidencia media fue de 12,29 por 100 000 habitantes, con un aumento del 7,3% anual en el período 2007-2014 y una reducción posterior del 3,4% en el período 2014-2021. Se observó un patrón alto/alto y la presencia de puntos calientes, principalmente en municipios del Sur, Sudeste, Centro-Oeste y Norte. El principal conglomerado se localizó en 572 municipios de Rio Grande do Sul y Santa Catarina, y los riesgos relativos más altos se observaron en Manaus (Amazonas) y Rondonópolis (Mato Grosso). La tasa de analfabetismo (β = -0,08), el índice de Gini (β = -3,74) y la cobertura de la estrategia de salud familiar (β = -0,70) mostraron una asociación negativa con el resultado. En cambio, el índice de Firjan de desarrollo municipal (β = 2,37), el índice de vulnerabilidad social (β = 6,30), el porcentaje de personas que reciben ayuda del programa de bienestar social Bolsa Família (β = 0,04) y los ingresos per cápita (β = 0,008) mostraron una asociación positiva. Conclusión. Hubo una tendencia al aumento de la incidencia de infección por el VIH/sida hasta el 2014, con una reducción posterior hasta el 2021. Los conglomerados de tasas elevadas se concentraron especialmente en los municipios de las regiones Norte, Sur, Sudeste y Centro-Oeste. Los indicadores de vulnerabilidad socioeconómica tienen una influencia positiva o negativa en el resultado, según el territorio investigado.
ABSTRACT Objective. To describe temporal and spatial patterns and identify the factors associated with the incidence of HIV/AIDS among young people in Brazil. Method. Ecological study of young Brazilians aged 15-24 years with reported HIV/AIDS, from 2001 to 2021. The Joinpoint method was used for the temporal analysis. Spatial clusters were detected using Bayesian methods, spatial autocorrelation, Getis-Ord Gi*, and scan techniques. Four non-spatial and spatial regression models were used to identify factors associated with the result. All statistical analyses considered p < 0.05. Results. In Brazil, the average incidence was 12.29 per 100 000 inhabitants, with an annual increase of 7.3% in the period 2007-2014 and a subsequent 3.4% decrease in 2014-2021. A high-high pattern and hotspots were observed, mainly in municipalities in the South, Southeast, Central-West, and North regions. The primary cluster was located in 572 municipalities in Rio Grande do Sul and Santa Catarina, with the highest relative risks in Manaus (Amazonas) and Rondonópolis (Mato Grosso). The illiteracy rate (β = -0.08), GINI Index (β = -3.74) and Family Health Strategy coverage (β = -0.70) were negatively associated with the result. In contrast, the Firjan Municipal Development Index (β = 2.37), Social Vulnerability Index (β = 6.30), percentage of Bolsa Família recipients (β = 0.04), and per capita income (β = 0.008) showed a positive association. Conclusion. There was an upward trend in the incidence of HIV/AIDS until 2014, followed by a decline until 2021. High-rate clusters were concentrated in municipalities in the North, South, Southeast and Central-West regions in particular. Indicators of socioeconomic vulnerability had positive or negative effects on the result, depending on the territory investigated.

Estudo ecológico que incluiu jovens brasileiros de 15 a 24 anos notificados com HIV/AIDS de 2001 a 2021. Utilizou-se o método joinpoint para a análise temporal. Aglomerados espaciais foram detectados pelos métodos Bayesiano, autocorrelação espacial, Getis-Ord Gi* e Varredura Scan. Quatro modelos de regressão não espacial e espacial foram usados para identificar fatores associados ao desfecho. Todas as análises estatísticas consideraram p < 0,05. Resultados. No Brasil, a incidência média foi de 12,29 por 100 000 habitantes, com aumento de 7,3% ao ano no período 2007-2014 e posterior decréscimo de 3,4% em 2014-2021. Observou-se padrão Alto/Alto e hotspots, principalmente em municípios do Sul, Sudeste, Centro-Oeste e Norte. O cluster primário localizou-se em 572 municípios do Rio Grande do Sul e Santa Catarina e os maiores riscos relativos em Manaus (Amazonas) e Rondonópolis (Mato Grosso). A taxa de analfabetismo (β = -0,08), Índice de GINI (β = -3,74) e Cobertura da Estratégia de Saúde da Família (β = -0,70) apresentaram relação negativa com o desfecho. Em contrapartida, o Índice Firjan de Desenvolvimento Municipal (β = 2,37), o Índice de Vulnerabilidade Social (β = 6,30), o percentual de pessoas que recebem o Bolsa Família (β = 0,04) e renda per capita (β = 0,008) apresentaram associação positiva. Conclusão. Houve tendência de aumento da incidência de HIV/AIDS até 2014 com posterior declínio até 2021. Aglomerados de altas taxas concentraram-se, especialmente, em municípios das regiões Norte, Sul, Sudeste e Centro-Oeste. Indicadores de vulnerabilidade socioeconômica influenciam o desfecho positivamente ou negativamente, dependendo do território investigado.

Resumo Objetivo Analisar as evidências de validade de conteúdo e processo de resposta de um banco de itens sobre vulnerabilidade em saúde. Métodos Estudo psicométrico, subsidiado pelo polo teórico de Pasquali. Foram selecionados 62 especialistas com características sociodemográficas e acadêmicas distintas. Desses, 15 responderam ao questionário de validade de conteúdo. Após os ajustes, o banco passou para fase de validade de processo de resposta e foi avaliado por 12 usuários dos serviços de saúde. Utilizou-se coeficiente de validade de conteúdo para avaliação dos itens e do banco, cuja confiabilidade foi verificada pelo teste binomial, considerando percentual de 80% e p-valor >0,05. Resultados O banco apresentou boa evidência de validade de conteúdo pelos especialistas, cujos coeficientes foram 0,87 para o elemento sujeito, 0,86 para o elemento social e 0,865 para o geral. O público-alvo realizou sugestões na fase de validade de processo de resposta para melhor compreensão dos itens (coeficiente total: 0,89), sendo considerado, claro, preciso e compreensível. Conclusão A versão final do banco de itens possui 535 itens, sendo 238 para o sujeito e 297 para o social. Abstract Objective To analyze the evidence of content validity and response process of a health vulnerability item bank. Methods Psychometric study with support of Pasquali’s theoretical pole in which 62 specialists with different sociodemographic and academic characteristics were selected and 15 of them responded to the content validity questionnaire. After adjustments, the bank moved to the response process validity phase and was evaluated by 12 health service users. A content validity coefficient was used to evaluate the items and the bank, which had the reliability assessed by the binomial test, considering a percentage of 80% and p-value >0.05. Results The bank presented good evidence of content validity by experts. The coefficients were 0.87 for the subject element, 0.86 for the social element and 0.865 for the general element. The target audience made suggestions in the response process validity phase to better understand the items (total coefficient: 0.89), which were considered clear, accurate and understandable. Conclusion The final version of the item bank has 535 items, 238 for the subject and 297 for the social element. It presents evidence of validity of content and response process and is suitable for testing the validity of internal structure. Abstract Sjogren’s disease (SjD) is an autoimmune disease that is characterized not only by the sicca symptoms it causes but also by its systemic nature, which is capable of several and not yet fully understood extraglandular manifestations. To gain a clearer understanding of these manifestations as well as a better practical approach, a panel of experts from the Brazilian Society of Rheumatology conducted a systematic review and meta-analysis on the identification of epidemiologic and clinical features of the extraglandular manifestations present in ESSDAI (EULAR Sjogren´s syndrome disease activity index), followed by a voting panel with recommendations for clinical practice. This publication is complementary to others already published and covers cutaneous and hematological manifestations, with prevalence data generated by a meta-analysis of 13 clinical or laboratory manifestations and 6 clinical management recommendations. Abstract Introduction Takayasu’s arteritis (TAK) patients are at an elevated risk of metabolic syndrome and cardiovascular diseases (CVD). Currently, there are no well-validated biomarkers to assess this risk in this population. Previous research in different cohorts has linked serum levels of osteoprotegerin (OPG) and its polymorphisms to accelerated atherosclerosis and a marker of poor prognosis in CVD. Thus, we assessed this protein as a potential biomarker of CVD in TAK patients. Objectives To evaluate the serum levels of OPG and its SNPs (single nucleotide polymorphisms) in TAK patients and healthy controls, and to associate these parameters with clinical data. Methods This bicentric cross-sectional study included TAK patients who were compared with healthy individuals (control group). The serum levels of OPG and the frequency of OPG SNPs [1181G > C (rs2073618), 245 A > C (rs3134069), 163T > C (rs3102735), and 209 C > T (rs3134070)] were compared between the both groups and associated with clinical data. Results In total, 101 TAK patients and 93 controls were included in the study. The serum levels of OPG (3.8 ± 1.9 vs. 4.3 ± 1.8pmol/L, respectively; P = 0.059), and its four polymorphisms were comparable between both groups. In an additional analysis of only TAK patients, serum OPG levels and its four genes were not associated with any CVD parameters, except for higher OPG levels among patients without dyslipidemia. Conclusion No significant differences were observed in serum OPG levels or in the genotype frequencies of OPG SNPs between the patient and control groups. Resumo Objetivo: descrever sentimentos expressos por profissionais de saúde no atendimento a vítimas infantojuvenis da violência sexual na perspectiva teórica do Interacionismo Simbólico. Método: pesquisa qualitativa realizada com 30 mulheres profissionais da saúde. Foi aplicado instrumento composto de questões fechadas para dados sociodemográficos e roteiro com perguntas abertas para a entrevista. Os dados foram organizados e analisados no software Nvivo versão 12, de acordo com a proposta de Bardin, sob a ótica do Interacionismo Simbólico na obra de Charles Morris. O projeto foi aprovado por Comitê de Ética em Pesquisa. Resultados: emergiram cinco categorias temáticas que revelaram sentimentos de empatia, medo, indignação, sofrimento e consternação. Tais sentimentos permaneceram na memória das entrevistadas, tornando o atendimento a vítimas infantojuvenis da violência sexual uma experiência tocante, difícil e que marca profundamente a vida do profissional de saúde. Conclusão: há necessidade de adotar estratégias de apoio à saúde mental de profissionais que atuam em serviços que prestam atendimento geral a crianças e adolescentes, considerando que existe a possibilidade de estes atenderem a vítimas infantojuvenis da violência sexual em respeito às políticas públicas preexistentes. Abstract Objective: to describe the feelings expressed by health professionals when caring for child and adolescent victims of sexual violence from the theoretical perspective of Symbolic Interactionism. Method: qualitative research carried out with 30 female health professionals. An instrument was used consisting of closed questions for sociodemographic data and a script with open questions for interviews. The data was organized and analyzed using Nvivo software version 12, according to Bardin’s proposal, from the perspective of Symbolic Interactionism in the work of Charles Morris. The project was approved by the Research Ethics Committee. Results: five thematic categories emerged, revealing feelings of empathy, fear, indignation, suffering, and consternation. These feelings remained in the interviewees’ memories, making caring for child and adolescent victims of sexual violence a moving and difficult experience that deeply marks the life of the health professional. Conclusion: there is a need to adopt strategies to support the mental health of professionals who work in services that provide general care to children and adolescents, considering that there is a possibility that they will provide care to child and adolescent victims of sexual violence in compliance with pre-existing public policies. Resumo: Este estudo teve como objetivo investigar associações entre percepções da vizinhança e problemas de sono em idosos brasileiros. Foi realizado um estudo transversal com 5.719 idosos da comunidade (≥ 60 anos) do Estudo Longitudinal da Saúde dos Idosos Brasileiros (ELSI-Brasil, 2019-2021). Os desfechos foram problemas de sono autorreferidos: má qualidade do sono, sonolência diurna, queixas de insônia primária, dificuldade em manter o sono e acordar na madrugada. As variáveis de exposição foram: questões sobre a percepção dos participantes sobre o ambiente físico e social da vizinhança. Regressão logística foi utilizada na análise dos dados. Lixo, sujeira ou grama alta nas ruas e o desejo de mudar de onde mora foram associados a maiores chances de má qualidade do sono. Preocupação em cair devido a calçadas em má condição, preocupação com dificuldades para usar os meios de transporte e preocupação com dificuldades para atravessar a rua foram associadas a maiores chances de todos os problemas de sono. O som/ruído dos ônibus e carros foi associado a maiores chances de alguns problemas de sono. A percepção da vizinhança como um bom lugar para morar foi associado a menores chances de sonolência diurna e queixas de insônia primária. A confiança na maioria das pessoas da vizinhança e a percepção de que crianças e jovens tratam os adultos com respeito foram associados a menores chances de sonolência diurna, queixas de insônia primária e acordar na madrugada. Abstract: This study aimed to investigate associations between neighborhood perception and sleep problems in older Brazilian adults. A cross-sectional study was conducted with 5,719 community-dwelling older adults (≥ 60 years) from the Brazilian Longitudinal Study of Aging (ELSI-Brazil, 2019-2021). The outcomes were self-reported sleep problems: poor sleep quality, daytime sleepiness, primary insomnia complaints, difficulty staying asleep, and waking up at dawn. The exposure variables were questions about the perception of participants about the physical and social environment of the neighborhood. Logistic regression was used in data analysis. Garbage, rubbish, or tall grass on the streets and the desire to move were associated with higher odds of poor sleep quality. Concern about falling due to damaged sidewalks, concern about having difficulties taking transportation, and concern about having difficulties crossing the street were associated with higher odds of all sleep problems. Sound/noise of buses and cars was associated with higher odds of some sleep problems. Perceiving the neighborhood as a good place to live was associated with lower odds of daytime sleepiness and primary insomnia complaints. Trusting most people in the neighborhood and perceiving that kids and younger people treat adults with respect were associated with lower odds of daytime sleepiness, primary insomnia complaints, and waking up at dawn. Being a good place for kids to play and raise teenagers was associated with lower odds of daytime sleepiness. These results can assist public administrators in creating urban planning policies aimed at improving neighborhood environments as a means of health promotion. Resumen: Este estudio tuvo como objetivo investigar las asociaciones entre las percepcion del vecindario y los problemas de sueño en ancianos brasileños. Se realizó un estudio transversal con 5.719 ancianos de la comunidad (≥ 60 años) del Estudio Longitudinal de Salud de los Ancianos Brasileños (ELSI-Brasil, 2019-2021). Los desenlaces fueron los siguientes: problemas de sueño autoinformados: mala calidad del sueño, somnolencia diurna, quejas de insomnio primario, dificultad para permanecer dormido y despertarse durante la madrugada. Las variables de exposición fueron las siguientes: preguntas sobre la percepción de los participantes sobre el entorno físico y social del vecindario. En el análisis de datos se utilizó la regresión logística. La basura, la suciedad o el césped alto en las calles y el deseo de cambiar el lugar donde viven se asociaron con mayores probabilidades de tener una mala calidad del sueño. La preocupación por las caídas debido a aceras en mal estado, la preocupación por las dificultades para utilizar los medios de transporte y la preocupación por las dificultades para cruzar la calle se asociaron con mayores probabilidades de sufrir todos los problemas de sueño. El ruido producido por los autobuses y automóviles se asoció con una mayor probabilidad de sufrir algunos problemas de sueño. La percepción del vecindario como un buen lugar para vivir se asoció con menores probabilidades de sufrir somnolencia diurna y quejas de insomnio primario. La confianza en la mayoría de la gente del vecindario y la percepción de que los niños y jóvenes tratan a los adultos con respeto se asociaron con menores probabilidades de sufrir somnolencia diurna, quejas de insomnio primario y despertarse durante la madrugada. Temporal Pattern and Spatial Distribution of Tuberculosis Prevalence Associated with Multimorbidity in Brazil

ABSTRACT Background: Four main chronic conditions may be involved in the tuberculosis pathogenic process and/or clinical evolution: HIV/AIDS, diabetes mellitus, mental illness, and Chronic Obstructive Pulmonary Disease. This study aimed to determine the spatiotemporal pattern of tuberculosis (TB) associated with multimorbidity in Brazil. Methods: Ecological study use data from the Notifiable Diseases Information System (SINAN), collected from the electronic portal of the Department of Informatics of the SUS (DATASUS). These data included TB cases that were reported between 2007 and 2021 and were associated with two or more chronic clinical health conditions (multimorbidity). Results: A total of 7,795 cases of TB associated with multimorbidity were recorded, with an average growth trend of 4.6% per year (95% Confidence Interval (CI): 3.3-5.9; p<0.001) and higher growth in the first temporal segment (2007 to 2011) (8.9%; 95%CI: 4.2-13.9; p=0.002). The spatial analysis, after Bayesian smoothing, highlighted the main municipalities states of the study, namely: São Paulo (19.8%; n = 297), Porto Alegre (23.6%; n = 354), and Rio de Janeiro (44.8%; n = 672). The proportion of extremely poor (β=-0.002), the bolsa família program (β=0.002), the average per-person income (β=0.001), and the percentage of the population living in households with a density of more than 2 people per bedroom (β=0.001) were related to chronic health conditions. Conclusions: These findings will stimulate public action to manage this situation. However, as this is still a recent topic in the literature, we encourage the development of studies on the synergistic characteristics of TB and other clinical conditions. Abstract Background There is a remarkable variability in the frequency of HLA-B27 positivity in patients with spondyloarthritis (SpA), which may be associated with different clinical presentations worldwide. However, there is a lack of data considering ethnicity and sex on the evaluation of the main clinical and prognostic outcomes in mixed-race populations. The aim of this study was to evaluate the frequency of HLA-B27 and its correlation with disease parameters in a large population of patients from the Brazilian Registry of Spondyloarthritis (RBE). Methods The RBE is a multicenter, observational, prospective cohort that enrolled patients with SpA from 46 centers representing all five geographic regions of Brazil. The inclusion criteria were as follow: (1) diagnosis of axSpA by an expert rheumatologist; (2) age ≥18 years; (3) classification according to ASAS axial. The following data were collected via a standardized protocol: demographic data, disease parameters and treatment historical. Results A total of 1096 patients were included, with 73.4% HLA-B27 positivity and a mean age of 44.4 (±13.2) years. Positive HLA-B27 was significantly associated with male sex, earlier age at disease onset and diagnosis, uveitis, and family history of SpA. Conversely, negative HLA-B27 was associated with psoriasis, higher peripheral involvement and disease activity, worse quality of life and mobility. Conclusions Our data showed that HLA-B27 positivity was associated with a classic axSpA pattern quite similar to that of Caucasian axSpA patients around the world. Furthermore, its absence was associated with peripheral manifestations and worse outcomes, suggesting a relevant phenotypic difference in a highly miscegenated population. RESUMO Objetivo: Relatar a coleta de dados via telefone realizada em pesquisa multicêntrica sobre avaliação do cuidado de enfermagem durante a pandemia da COVID-19. Método: Relato de experiência sobre o uso do telefone para coleta de dados quantitativos e qualitativos com participantes de dez Hospitais Universitários Brasileiros, de outubro de 2020 a dezembro de 2021. A experiência foi apresentada segundo etapas: 1) Operacionalização da coleta de dados via telefone; 2) Capacitação da equipe de entrevistadores; 3) Acompanhamento e ajustes da coleta de dados; e 4) Resultados do contato telefônico com o paciente. Resultados: O planejamento e a organização da coleta de dados envolveram a construção de manuais de orientação para guiar os coletadores, os quais passaram por validação quanto à clareza e concordância. Para acompanhamento e ajustes, realizou-se reunião semanal com os entrevistadores e pesquisadores responsáveis. Foram incluídos dados de 539 respondentes do instrumento Patient Measure of Safety, de 643 do instrumento Care Transitions Measure e de 56 entrevistas abertas. Conclusão: A utilização de manuais de orientação para coleta de dados via telefone, realização de treinamentos e reuniões de acompanhamento são estratégias que podem potencializar essa estratégia em pesquisas multicêntricas, quando da impossibilidade de coleta face-a-face. ABSTRACT Objective: to report data collection via telephone carried out in multicenter research on nursing care assessment during the COVID-19 pandemic. Method: this is an experience report on using the telephone to collect quantitative and qualitative data with participants from ten Brazilian university hospitals from October 2020 to December 2021. The experience was presented in stages: 1) Operationalization of data collection via telephone; 2) Interviewing team training; 3) Monitoring and adjustments to data collection; and 4) Results of telephone contact with patients. Results: data collection planning and organization involved creating guidance manuals to guide the collectors, which were validated for clarity and agreement. For monitoring and adjustments, a weekly meeting was held with the interviewers in charge and researchers. Data from 539 respondents from the Patient Measure of Safety instrument, 643 from the Care Transitions Measure instrument and 56 from open interviews were included. Conclusion: using guidance manuals for data collection via telephone, training and follow-up meetings are strategies that can enhance this strategy in multicenter research when in-person data collection is impossible. RESUMEN Objetivo: informar la recolección de datos vía telefónica realizada en una investigación multicéntrica sobre la evaluación de los cuidados de enfermería durante la pandemia de COVID-19. Método: informe de experiencia sobre el uso del teléfono para la recolección de datos cuantitativos y cualitativos con participantes de diez hospitales universitarios brasileños, de octubre de 2020 a diciembre de 2021. La experiencia fue presentada en etapas: 1) Operacionalización de la recolección de datos por teléfono; 2) Capacitación del equipo entrevistador; 3) Monitoreo y ajustes a la recolección de datos; y 4) Resultados del contacto telefónico con el paciente. Validating evidence for the knowledge, management and involvement of dentists in a dental approach to sickle-cell disease knowledge sicklecell sickle cell

Abstract This study validated the content of an instrument designed to assess the knowledge, involvement (attitudes) and management (practice) of dentists relative to sickle-cell disease (KAPD-SCD). The instrument consisted of five domains composed of a total of thirteen items: I. Dentist’s self-assessment relative to sickle-cell disease; II. Dentist’s knowledge of the repercussions of sickle-cell disease on the stomatognathic system; III. Dentist’s knowledge of the complications of sickle-cell disease in the stomatognathic system; IV. Dentist’s knowledge concerning the dental management of sickle-cell disease patients; and V. Dentist’s involvement in an approach to sickle-cell disease. Twelve experts assigned scores to each item of the instrument. The criteria were clarity, understanding and appropriateness, leaving open fields for comments. Descriptive and content analyses of the data were made. Each expert analyzed 39 assessment units. The percentages considered for agreement were high (>80%), medium (70%-80%), or low (<70%), and each item was maintained or revised according to the percentage observed. There was high consensus in 74% of the assessment units (the corresponding items were maintained), medium consensus in 24% of them (the corresponding items were revised), and disagreement in 2% of them, namely as regards the “appropriateness” of item 5 (“Are there oral complications in sickle-cell disease?”), which was revised. The final version of the instrument had 16 items for different applications such as in the clinical care program, teaching program, or research program, with different cut-off scores for each application. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Objectives: To identify somatic mutations in tumors from young women with triple-negative or luminal breast cancer, through targeted sequencing and to explore the cancer driver potential of these gene variants. Methods: A customized gene panel was assembled based on data from previous sequencing studies of breast cancer from young women. Triple-negative and luminal tumors and paired blood samples from young breast cancer patients were sequenced, and identified gene variants were searched for their driver potential, in databases and literature. Additionally, the authors performed an exploratory analysis using large, curated databases to evaluate the frequency of somatic mutations in this gene panel in tumors stratified by age groups (every 10 years). Results: A total of 28 young women had their tumoral tissue and blood samples sequenced. Using a customized panel of 64 genes, the authors could detect cancer drivers in 11/12 (91.7 %) TNBC samples and 11/16 (68.7 %) luminal samples. Among TNBC patients, the most frequent cancer driver was TP53, followed by NF1, NOTCH1 and PTPN13. In luminal samples, PIK3CA and GATA3 were the main cancer drivers, and other drivers were GRHL2 and SMURF2. CACNA1E was involved in both TN and luminal BC. The exploratory analysis also indicated a role for SMURF2 in luminal BC development in young patients. Conclusions: The data further indicates that some cancer drivers are more common in a specific breast cancer subtype from young patients, such as TP53 in TNBC and PIK3CA and GATA3 in luminal samples. These results also provide additional evidence that some genes not considered classical cancer-causing genes, such as CACNA1E, GRHL2 and SMURF2 might be cancer drivers in this age group.
