Abstract To date, hydroxyurea is the only effective and safe drug that significantly reduces morbidity and mortality of individuals with Sickle cell disease. Twenty years of real-life experience has demonstrated that hydroxyurea reduces pain attacks, vaso-occlusive events, including acute chest syndrome, the number and duration of hospitalizations and the need for transfusion. The therapeutic success of hydroxyurea is directly linked to access to the drug, the dose used and adherence to treatment which, in part, is correlated to the availability of hydroxyurea. RESUMO: O parvovírus canino 2 (CPV-2) é um vírus entérico altamente contagioso que causa alta morbidade e mortalidade, principalmente em cães com menos de seis meses de idade. A recuperação desta doença é dependente de vários fatores, incluindo a determinação do prognóstico do paciente para terapia adequada. O objetivo deste estudo foi avaliar os fatores de risco associados ao desfecho óbito em cães com CPV-2 em um estudo de caso-controle realizado no Hospital Veterinário da Universidade Federal de Lavras (HV-UFLA), em Lavras, Minas Gerais. Vinte e seis cães com sintomas de CPV-2 que chegaram entre 2017 e 2018 ao HV-UFLA foram avaliados para inclusão no estudo. Dados sobre histórico médico, sinais clínicos, hemograma e teste rápido de parvovirose e fezes para reação em cadeia da polimerase (PCR) foram coletados para todos os animais. Todos os cães receberam tratamento estabelecido pelo HV-UFLA e a taxa geral de letalidade por parvovirose canina foi de 30,77%. Análise descritiva e análise estatística univariada e multivariada (regressão logística) foram utilizadas para avaliar variáveis possivelmente associadas a um prognóstico desfavorável (óbito). Na análise univariada e multivariada, a Síndrome da Resposta Inflamatória Sistêmica (SIRS) foi observada como fator de risco significativo para prognóstico desfavorável na parvovirose canina, pois aumentou 12,96 vezes o risco de morte (95% IC 1,85-133,70; P < 0,01) em comparação com aqueles pacientes que não apresentaram SIRS. Assim, a SIRS foi fortemente associada com um prognóstico desfavorável, sugerindo que pode ser utilizada como indicador prognóstico na parvovirose canina na prática veterinária. ABSTRACT: Canine parvovirus 2 (CPV-2) is a highly contagious enteric virus that causes high morbidity and mortality, especially in dogs under six months of age. Recovery from this illness is dependent on several factors, including the patient’s prognosis for adequate therapy. The aim of this study was to evaluate the risk factors associated with the death outcome in CPV-2 positive dogs in a case-control study conducted at the Veterinary Hospital of the Universidade Federal de Lavras (HV-UFLA) in Lavras, Minas Gerais, Brazil. Twenty-six dogs with CPV-2 symptoms that arrived at the HV-UFLA between 2017 and 2018 were evaluated for inclusion in the study. Data on medical history, clinical signs, blood count and rapid test of parvovirus and faecal test for polymerase chain reaction (PCR) were collected for all the animals. All the dogs received treatment at the HV-UFLA, and the overall fatality rate due to canine parvovirus was 30.77%. Descriptive analysis and univariate and multivariate statistical analyses (logistic regression) were performed to assess the variables that were possibly associated with an unfavourable prognosis (death). In the univariate and multivariate analyses, Systemic Inflammatory Response Syndrome (SIRS) was observed to be a significant risk factor for an unfavourable prognosis in canine parvovirus, as it increased the risk of death by 12.96 times (95% CI 1.85-133.70; P < 0.01) compared with patients who did not exhibit SIRS. Thus, SIRS was strongly associated with an unfavourable prognosis, suggesting that it can be used as a prognostic indicator for canine parvovirus in veterinary practice. ABSTRACT BACKGROUND: Precisely determining the aspects related to an instrument's validity and reliability measures allows for greater assurance of the quality of the results. OBJECTIVES: To analyze the psychometric properties of The Providers Survey in the Brazilian context of mental health services. DESIGN AND SETTING: The instrument validation study was conducted in Montes Claros, Minas Gerais, Brazil. METHODS: The validation study was conducted using the Consensus-based Standards for the Selection of Health Measurement Instruments checklist to analyze its validity and reliability. RESULTS: A committee of expert judges performed content validation after which the Content Validity Index was calculated. Construct validation took place through Exploratory Factor Analysis using the Kaiser-Meyer-Olkin Test criterion and Bartlett's Sphericity Test. Reliability was verified using test-retest reliability. The significance level adopted for the statistical tests was 5% (P < 0.05). The final instrument comprised 54 questions. The Content Validity Index was 97%. Exploratory Factor Analysis identified a Kaiser-Meyer-Olkin index of 0.901 and Bartlett's Sphericity Test with P < 0.001. We obtained a Cronbach's alpha coefficient of 0.95 and an intraclass correlation coefficient of 0.849. CONCLUSIONS: The Providers Survey, translated and adapted into Portuguese, was named the Work Assessment Instrument for the Recovery of Mental Health. It presented adequate psychometric properties for evaluating work-related practices for the recovery of psychosocial care network users. Resumo O sarampo é uma das principais causas de morbidade e mortalidade na população pediátrica e pode ser prevenido com 100% de eficácia pela vacinação. No entanto, a doença permanece ativa no território brasileiro. O objetivo do artigo é avaliar a adesão da população à vacinação e a possível relação com hospitalização e mortalidade em relação ao sarampo no Brasil. Trata-se de um estudo ecológico realizado a partir de dados secundários de mortalidade e internações acerca do sarampo e da cobertura vacinal contra a doença no Brasil nos anos de 2013 a 2022. O ápice de adesão ao calendário vacinal contra o sarampo se deu nos três anos que precederam a erradicação da doença no país, ocorrida em 2016. Nesse intervalo, tem-se as menores taxas de internação, com a mortalidade zerada de 2014 a 2017. Em contrapartida, verifica-se, desde então, queda na taxas de vacinação, acentuadas a partir de 2019, quando a doença reaparece no Brasil. Concomitantemente, as taxas de internação e mortalidade atingem os valores mais altos registrados. A adesão populacional ao calendário vacinal completo contra o sarampo, essencial ao controle da doença e dos óbitos relacionados, está insuficiente, o que se reflete nas taxas de internações e mortalidade.

Abstract Measles is one of the main causes of morbidity and mortality in the pediatric population and it can be prevented with 100% effectiveness by vaccination. However, the disease remains active in throughout Brazil. The scope of this article is to evaluate the population’s adherence to vaccination and the potential connection with hospitalizations and mortality in relation to measles in Brazil. This is an ecological study based on secondary data on mortality and hospitalizations due to measles and vaccination coverage against the disease in Brazil from 2013 to 2022. The peak of adherence to the measles vaccination schedule occurred in the 3 years that preceded the eradication of the disease in the country, which occurred in 2016. In this interval, there are the lowest hospitalization rates, with zero mortality from 2014 to 2017. On the other hand, there has been a marked drop in vaccination rates since 2019, when the disease resurfaced in Brazil. Concomitantly, hospitalization and mortality rates reach the highest recorded values. Population adherence to the complete measles vaccination schedule, which is essential to control the disease and related deaths, is insufficient, which is reflected in hospitalization and mortality rates.

Abstract Objectives To evaluate outcomes of oral food challenge (OFC) test to assess tolerance in infants with non-IgE-mediated cow's milk allergy (CMA) with gastrointestinal manifestations and explore clinical data predictive of these outcomes. Methods Single-center retrospective study including infants (age < 12 months) who were referred for CMA between 2000 and 2018 and underwent OFC on follow-up. A univariate logistic regression test was performed to evaluate variables associated with the outcomes of the follow-up OFC test. Results Eighty-two patients were included, 50% were male. Eighteen patients had a positive OFC test (22%). Most patients had presented with hematochezia (77%). The median age of symptom onset was 30 days. Two-thirds of the patients were on appropriate infant formula (extensively hydrolyzed or amino acid-based formula), exclusively or in association with breastfeeding. The median time on an elimination diet before the OFC test was 8 months (Q1 6 - Q3 11 months). All cases with positive follow-up OFC tests (n= 18) had been exposed to cow's milk-based formula before the first clinical manifestation of CMA. Five out of eight cases with Food Protein-Induced Enterocolitis Syndrome (FPIES) had positive OFC tests. Exposure to cow's milk-based formula before diagnosis, a history of other food allergies, hematochezia and diarrhea were predictors of a positive OFC test. Conclusions In infants with non-IgE-mediated CMPA with gastrointestinal manifestations, the use of cow's milk-based formula, a history of other food allergies, and hematochezia and diarrhea upon initial presentation were associated factors for the later achievement of tolerance. Silver(I) complexes of amantadine (atdH), memantine (mtnH) and rimantadine (rtdH), named Ag-atd, Ag-mtn and Ag-rtd, respectively, were recently synthesized and described in the literature as promising antibacterial agents. In the present study, the cytotoxicity of such complexes was evaluated against cultures of primary epidermal keratinocytes (HaCaT) and murine melanoma tumor cells (B16-F10), and mutagenicity was studied by the Ames test to investigate their abilities to induce gene mutations. The Ames test was performed using Salmonella Typhimurium strains (TA98, TA100, TA102 and TA97a) capable of detecting frameshift and base pair substitution gene mutations, in experiments with and without metabolic activation (microsomal fraction S9). This study revealed significant cytotoxic activity against tumor cells and selectivity of Ag-atd and Ag-rtd complexes when compared to non-tumor human keratinocyte cells. Moreover, the Ag(I) complexes did not induce a significant growth in the number of revertant colonies when comparing with the negative control, both in the experiments without and with metabolic activation, indicating absence of mutagenic activity. The results are encouraging and collaborate in the genotoxicological investigations necessary for understanding the interaction and ability of the silver complexes to induce mutations and contribute to ensure their uses as future antibacterial or antitumor agents. ABSTRACT Objective: To identify internal structure validity evidence of a dysphagia screening questionnaire for caregivers of older adults with Alzheimer’s disease dementia and/or vascular dementia. Methods: The 24-question Dysphagia Screening in Older Adults with Dementia − Caregiver Questionnaire (RaDID-QC) was administered by interviewing 170 caregivers of older people with dementia, selected by convenience at the Outpatient Reference Center for Older People. Exploratory Factor Analysis (EFA) was used to assess the internal structure validity of the questionnaire, and Cronbach’s alpha was used to analyze reliability. Questions with factor loadings lower than 0.45 in magnitude were removed from the final questionnaire. Multivariate multiple linear regression was used to assess the percentage of variance explained by the remaining questions. Results: Kayser-Meyer-Olkin (KMO) and Bartlett’s tests suggested that the questionnaire was adequate for EFA. Principal Component Analysis (PCA) suggested that 12 components captured at least 75 % of the total variance. The corresponding 12-factor EFA model showed a statistically significant fit, and 15 out of the 24 questions had factor loadings greater than 0.45. Cronbach’s alpha was 0.74 for the 15 questions, which explained 71 % of the total variance in the complete dataset. The questionnaire has adequate internal structure validity and good reliability. Based on EFA, RaDID-QC decreased from 24 to 15 questions. Other internal validity and reliability parameters will be obtained by administering the questionnaire to larger target populations. Conclusion: The RaDID-QC applied to caregivers of older adults with dementia due to Alzheimer’s disease and/or vascular dementia produced valid and reliable responses to screen dysphagia signs and symptoms. Abstract Objectives To present the Brazilian Psychiatric Association’s Consensus on the Management of Acute Intoxication. Methods A group of experts selected by the Brazilian Psychiatric Association searched for articles on the MEDLINE (by PubMed) and Cochrane databases, limited to human studies and acute intoxication. Working groups reviewed these materials for appropriateness to the topic and the quality of the work. A survey was conducted using the Delphi method to produce a table of agreed recommendations presented at the end of the systematic review. Three survey rounds were held to reach consensus. Results Support for intoxication should start with Initial Management: Resuscitation/Life Support/Differential Diagnosis. For this, the group proposed the following sequence of assessments: A (airway), B (breathing), C (circulation), D.1 (disability), D.2 (differential diagnosis), D.3 (decontamination), D.4 (drug antidotes), E (enhanced elimination). The group of experts then presented specific interventions for the main drugs of abuse. Conclusions Management of intoxication with drugs of abuse is complex and requires systematic protocols. The group suggests adoption of the A-B-C-D-E technique first, with constant investigation. Then, specific conduct and support until remission of intoxication. The literature is still scarce in evidence on the subject. Therefore, this consensus was necessary. We believe that at present this document can help psychiatric, general, and emergency physicians deal with emergency psychiatric episodes due to acute intoxication. This work could stimulate future studies on the topic. RESUMO: Objetivo: descrever o perfil da violência em relacionamentos homoafetivos entre adolescentes no estado de Pernambuco-Brasil, notificados no período de 2017 a 2021. Método: a amostra foi composta por adolescentes de 10 a 19 anos (n=925), selecionados de acordo com a orientação sexual. Dados extraídos das fichas de notificação individual de violência interpessoal/autoprovocada no Sistema de Informação de Agravos de Notificação. Efetuou-se a análise estatística descritiva e as proporções pelo qui-quadrado de Pearson. Resultados: vítimas com faixa etária entre 15-19 anos (61,8%), sexo masculino (93,9%), raça/cor parda (77,5%), até oito anos de estudo (51,7%) e residiam na zona urbana (77,2%). A maioria dos agressores era do sexo masculino, era namorado (a) e/ou cônjuge. Predominou as violências: sexual, física e psicológica. Conclusão: traçou-se o perfil da violência em relações homoafetivas apontando que o público masculino foi o mais propenso a sofrer e praticar a violência. ABSTRACT Objective: to describe the profile of violence in same-sex relationships among adolescents in the state of Pernambuco-Brazil, reported between 2017 and 2021. ABSTRACT Introduction: Blood transfusion is one of the most common medical practices worldwide. However, current scientific literature has shown that the immunomodulatory effects of blood transfusion are associated with an increased likelihood of infection, prolonged hospitalization, and morbimortality. Also, it means high costs for healthcare systems. Methods: In this context, acknowledging that blood transfusions are essentially heterologous cell transplantations, the use of therapeutic options has gained strength and is collectively known as the patient blood management (PBM) program. PBM is an approach based on three main pillars: (1) treating anemias and coagulopathies in an optimized manner, especially in the preoperative period; (2) optimizing perioperative hemostasis and the use of blood recovery systems to avoid the loss of the patient's blood; (3) anemia tolerance, with improved oxygen delivery and reduced oxygen demand, particularly in the postoperative period. Results: Current scientific evidence supports the effectiveness of PBM by reducing the need for blood transfusions, decreasing associated complications, and promoting more efficient and safer blood management. Thus, PBM not only improves clinical outcomes for patients but also contributes to the economic sustainability of healthcare systems. Conclusion: The aim of this review was to summarize PBM strategies in a comprehensive, evidence-based approach through a systematic and structured model for PBM implementation in tertiary hospitals. RESUMO Finalidade o objetivo geral do estudo apresentado neste artigo foi descrever e analisar como uma instituição hospitalar de alta complexidade implementou a prática do home office em sua operação em meio à pandemia. Metodologia adotou-se uma abordagem qualitativa e de natureza exploratória. A coleta dos dados ocorreu por meio de entrevistas semiestruturadas realizadas com seis gestores do Hospital Gama e aplicação de questionário com questões de múltipla escolha e dissertativas preenchidos de forma online por colaboradores das esquipes desses gestores. Constatações os resultados apontaram que: (1) para a migração de modalidade de trabalho houve a aplicação de elementos facilitadores proporcionados pelo hospital, gestores e colaboradores; (2) identificaram-se vários desafios à nova modalidade, cuja superação, poderá fomentar a transformação da cultura do hospital em médio e longo prazo; (3) o hospital não estava preparado quanto à tecnologia e aos recursos humanos para o processo de transição, mas houve um conjunto de rápidas ações para tornar possível a migração do trabalho presencial para o home office; (4) as vantagens indicadas pelos Interviewee s sobre a nova modalidade corrobora a literatura que serviu como fundamento teórico para esta pesquisa; e (5) o nível de engajamento dos colaboradores durante o processo de mudança parece ter se mantido igual ou aumentado, de acordo com as percepções dos participantes da pesquisa. ABSTRACT Purpose the general objective of the study presented in this article was to describe and analyze how a highly complex hospital institution implemented the practice of home office in its operations during the pandemic. Methodology a qualitative and exploratory approach was adopted. RESUMO O envelhecimento populacional traz consigo inúmeros desafios, especialmente no que diz respeito à saúde cognitiva. Nesse contexto, fatores socioeconômicos como a educação têm recebido atenção especial em razão do seu papel na saúde cerebral. Objetivo: Este estudo teve como objetivo descrever o desempenho das funções executivas, variáveis de saúde mental e qualidade de vida entre idosos saudáveis em relação ao seu nível de educação. Métodos: Foi realizado um estudo descritivo e transversal com participantes de 60 anos ou mais. O protocolo de avaliação incluiu o Exame Cognitivo de Addenbrooke e Teste de Fluência Verbal (FAS), Testes de Trilhas A e B, Escala de Controle, Autonomia, Autorrealização e Prazer (CASP-19), bem como a Escala de Depressão, Ansiedade e Estresse. Resultados: Foram encontradas diferenças significativas no desempenho das funções executivas entre idosos com níveis mais elevados de educação. No entanto, variáveis de saúde mental e qualidade de vida estavam relacionadas apenas à idade dos participantes. ABSTRACT Population aging brings about numerous challenges, particularly concerning cognitive health. In this context, socioeconomic factors such as education have received special attention due to their role in brain health. Objective: The aim of this study was to describe the performance of executive functions, mental health variables, and quality of life among healthy older adults in relation to their level of education. Methods: A descriptive, cross-sectional study was conducted with participants aged 60 or older. Resumo Antecedentes Programas informatizados de treinamento cognitivo podem ter beneficiado a autoavaliação da memória, a qualidade de vida e o humor entre os idosos durante a pandemia de doença do coronavírus 2019 (coronavirus disease 2019, COVID-19, em inglês). Objetivo Determinar os impactos subjetivos do treinamento cognitivo computadorizado no humor, na frequência de esquecimento, nas queixas de memória, e na qualidade de vida em idosos utilizando uma plataforma online. Métodos Ao todo, 66 idosos participantes do programa USP 60 +, oferecido à terceira idade pela Universidade de São Paulo, e inscritos voluntariamente no estudo, foram selecionados e randomizados em uma razão de 1:1 em 2 grupos: grupo treinamento (n = 33) e grupo controle (n = 33). Após assinarem o termo de consentimento livre e esclarecido, os participantes responderam a um protocolo que incluía um questionário sociodemográfico, o Questionário de Queixas de Memória (Memory Complaints Questionnaire, MAC-Q), A Escala de Frequência de Esquecimento de McNair e Kahn, a Escala de Depressão Geriátrica (Geriatric Depression Scale, GDS-15), o Inventário de Ansiedade Geriátrica (Geriatric Anxiety Inventory, GAI), e –o questionário de Controle, Autonomia, Autorrealização e Prazer (Control, Autonomy, Self-Realization, and Pleasure, CASP-19). A plataforma de jogos cognitivos de treinamento visou estimular diversos aspectos cognitivos, incluindo memória, atenção, linguagem, funções executivas (raciocínio, raciocínio lógico) e habilidades visuais e espaciais. Abstract Background Computerized cognitive training programs may have benefited the self-assessment of memory, quality of life, and mood among older adults during the coronavirus disease 2019 (COVID-19) pandemic. Objective To determine the subjective impacts of computerized cognitive training on mood, frequency of forgetfulness, memory complaints, and quality of life in the elderly using an online platform. Methods In total, 66 elderly participants of USP 60 +, a program for the elderly offered by Universidade de São Paulo, who voluntarily enrolled in the study were selected and randomized with an allocation ratio of 1:1 into 2 groups: the training group (n = 33) and the control group (n = 33). After signing the free and informed consent form, they answered a protocol which included a sociodemographic questionnaire, the Memory Complaints Questionnaire (MAC-Q), the McNair and Kahn’s Frequency of Forgetfulness Scale, the Geriatric Depression Scale (GDS-15), the Geriatric Anxiety Inventory (GAI), and the Control, Autonomy, Self-Realization, and Pleasure (CASP-19) questionnaire. The training cognitive game platform aimed to stimulate various cognitive aspects, including memory, attention, language, executive functions (reasoning, logical thinking), and visual and spatial skills. Results The participants of the training group showed a reduction in the MAC-Q, MacNair and Kahn, and GAI scores in the pre- and posttest comparison. Significant differences were identified between the groups regarding the total scores of the MAC-Q in the post-test, which was also evidenced by the logistic regression. Resumo Fundamento As complicações cardiovasculares da COVID-19 são aspectos importantes da patogênese e do prognóstico da doença. Evidências do papel prognóstico da troponina e da lesão miocárdica em pacientes hospitalizados com COVID-19 na América Latina são ainda escassos. Objetivos Avaliar a lesão miocárdica como preditor independente de mortalidade hospitalar e suporte ventilatório mecânico em pacientes hospitalizados, do registro brasileiro de COVID-19. Métodos Este estudo coorte é um subestudo do registro brasileiro de COVID-19, conduzido em 31 hospitais brasileiros de 17 cidades, de março a setembro de 2020. Os desfechos primários incluíram mortalidade hospitalar e suporte ventilatório mecânico invasivo. Os modelos para os desfechos primários foram estimados por regressão de Poisson com variância robusta, com significância estatística de p<0,05. Resultados Dos 2925 pacientes [idade mediana de 60 anos (48-71), 57,1%], 27,3% apresentaram lesão miocárdica. A proporção de pacientes com comorbidades foi maior nos pacientes com lesão miocárdica [mediana 2 (1-2) vs. 1 (0-20)]. Os pacientes com lesão miocárdica apresentaram maiores valores medianos de peptídeo natriurético cerebral, lactato desidrogenase, creatina fosfoquinase, N-terminal do pró-peptídeo natriurético tipo B e proteína C reativa em comparação a pacientes sem lesão miocárdica. Como fatores independentes, proteína C reativa e contagem de plaquetas foram relacionados com o risco de morte, e neutrófilos e contagem de plaquetas foram relacionados ao risco de suporte ventilatório mecânico invasivo. Os pacientes com níveis elevados de troponina apresentaram um maior risco de morte (RR 2,03, IC95% 1,60-2,58) e suporte ventilatório mecânico (RR 1,87;IC95% 1,57-2,23), em comparação àqueles com níveis de troponina normais. Conclusão Lesão cardíaca foi um preditor independente de mortalidade hospitalar e necessidade de suporte ventilatório mecânico em pacientes hospitalizados com COVID-19. Abstract Background Cardiovascular complications of COVID-19 are important aspects of the disease’s pathogenesis and prognosis. Evidence on the prognostic role of troponin and myocardial injury in Latin American hospitalized COVID-19 patients is still scarce. Objectives To evaluate myocardial injury as independent predictor of in-hospital mortality and invasive mechanical ventilation support in hospitalized patients, from the Brazilian COVID-19 Registry. Methods This cohort study is a substudy of the Brazilian COVID-19 Registry, conducted in 31 Brazilian hospitals of 17 cities, March-September 2020. Primary outcomes included in-hospital mortality and invasive mechanical ventilation support. Models for the primary outcomes were estimated by Poisson regression with robust variance, with statistical significance of p<0.05. Results Of 2,925 patients (median age of 60 years [48-71], 57.1% men), 27.3% presented myocardial injury. The proportion of patients with comorbidities was higher among patients with cardiac injury (median 2 [1-2] vs. 1 [0-2]). Patients with myocardial injury had higher median levels of brain natriuretic peptide, lactate dehydrogenase, creatine phosphokinase, N-terminal pro-brain natriuretic peptide, and C-reactive protein than patients without myocardial injury. As independent predictors, C-reactive protein and platelet counts were related to the risk of death, and neutrophils and platelet counts were related to the risk of invasive mechanical ventilation support. Patients with high troponin levels presented a higher risk of death (RR 2.03, 95% CI 1.60-2.58) and invasive mechanical ventilation support (RR 1.87, 95% CI 1.57-2.23), when compared to those with normal troponin levels. Conclusion Cardiac injury was an independent predictor of in-hospital mortality and the need for invasive mechanical ventilation support in hospitalized COVID-19 patients.
