Ultra-Low-Dose Computed Tomography for Pneumonia in the Emergency Department: A Feasibility Study

Resumo Introdução: O papel da tomografia computorizada torácica de ultrabaixa dose (ULDCT) em contexto de urgência é desconhecido. Objetivo: Este estudo avaliou a viabilidade da ULDCT em adultos na urgência com suspeita de pneumonia. Foram medidos a dose de radiação, o tempo entre a solicitação e os relatórios do exame, prescrições de antimicrobianos também foram correlacionadas. Métodos: Este estudo avaliou a viabilidade da ULDCT em adultos na urgência com suspeita de pneumonia. Foram medidas a dose de radiação, o tempo entre a solicitação e a disponibilização dos relatórios do exame, e também foram correlacionadas as prescrições de antimicrobianos. Entre outubro de 2017 e dezembro de 2018, incluímos prospectivamente adultos com suspeita de pneumonia adquirida na comunidade, sem diagnóstico definitivo por avaliação clínica. Os critérios de exclusão abrangeram condições clínicas que pudessem afetar a interpretação da imagem. Resultados: Foram incluídos 131 pacientes elegíveis (idade média de 43,5 anos; 57% homens) encaminhados para ULDCT. Dezoito (13,7%) foram excluídos. A dose média de radiação da ULDCT foi de 0,14 mSv. O intervalo de tempo entre a solicitação médica e o resultado da ULDCT foi de 43 minutos. Conclusão: A ULDCT mostrou-se viável para pacientes adultos com suspeita de pneumonia em contexto de urgência. Os relatórios foram disponibilizados num intervalo razoável e os resultados negativos foram associados a uma baixa taxa de prescrição de antibióticos.
Abstract Background: The role of ultra-low-dose chest CT (ULDCT) in the emergency department is unknown. Purpose: This study sought to examine feasibility of ULDCT in adults presenting to the emergency department for whom pneumonia was suspected. Radiation dose and time interval between medical request and ULDCT results were measured, antibiotic prescriptions were correlated. Methods: Between October 2017 and December 2018, we prospectively enrolled consecutive healthy adult patients in a single-center for whom community-acquired pneumonia was suspected and for whom no definitive diagnosis was possible by clinical judgment. Exclusion criteria comprised all chronic conditions that could impact severity and interpretation of image. Results: 131 eligible patients (mean age 43.5 years; 57% men) were enrolled to ULDCT. Eighteen (13.7%) patients were excluded. Average CT radiation dose was 0.14 mSv. Time interval between medical request and ultra-low-dose chest CT result was 43 minutes. Only 15.9% of patients with negative CT received antibiotics. Conclusion: Ultra-low-dose chest CT was feasible for almost all patients in a group of healthy adults presenting to the emergency department with suspected community-acquired pneumonia. Results were made available in a reasonable interval, and negative ones were associated with a low antibiotic prescription rate.

ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT The genus Desmanthus presents wide ecogeographical distribution and complex taxonomy. Desmanthus pernambucanus (L.) Abstract Cultivation of species of the genus Eucalyptus is important for the Brazilian economy, with 6.97 million hectares planted. Glycaspis brimblecombei Moore (Hemiptera: Aphalaridae), detected in Brazil in 2003, has dispersed and now damages Eucalyptus crops in all regions of this country. The location and identification of entomopathogenic fungi isolates may increase the options for integrated pest management. The objective of this research was to evaluate the pathogenicity of Cordyceps cateniannulata and Cordyceps javanica isolates to G. brimblecombei. Ten nymphs of G. brimblecombei, with or without lerps, were placed per Eucalyptus leaf cut with one of its edges on hydroretentive gel inside Petri dishes. The fungi isolates were suspended in a solution of Tween 80 (0.1%) at the concentration of 1.0 x 108 conidia mL-1 and sprayed on the G. brimblecombei nymphs. The mortality of this insect was evaluated daily for seven days, and the dead individuals were transferred to humid chambers. The conidia viability of the isolates was greater than 93%. The mortality of G. brimblecombei nymphs, seven days after the application of the fungi, was 100%. This is the first report of the pathogenicity of C. cateniannulata and C. javanica isolates, occurring naturally in the field, to G. brimblecombei.

RESUMO A eficiência do nitrogênio na aveia associada às condições climáticas são fundamentais no desenvolvimento de manejos sustentáveis com produtividade e qualidade. Objetivou-se neste estudo definir a eficiência agronômica do nitrogênio pela relação dose fornecida e produto obtido, estimar a máxima eficiência técnica do nutriente na produtividade de grãos, e pela dose ideal, simular a expressão da produtividade de palha e de indústria, proteína e fibra total em diferentes condições do ano agrícola no sistema soja/aveia. O estudo foi conduzido de 2011 a 2016, em Augusto Pestana, RS, em delineamento de blocos casualizados com quatro repetições em esquema fatorial 4 x 2 referente a doses de nitrogênio (0, 30, 60 e 120 kg ha-1) e cultivares de aveia (Barbarasul e Brisasul) no sistema soja/aveia. O nitrogênio incrementa a produtividade de grãos, de palha, de indústria e o teor de proteína dos grãos, com eficiência agronômica de 7,8, 19,7 e 3,3 kg ha-1 e 0,10 g kg-1, respectivamente, com redução na fibra total de grãos de 0,05 g kg-1 por kg de N fornecido. A dose de máxima eficiência técnica na expressão da produtividade de grãos depende das condições de cultivo. Em geral, a máxima eficiência de produtividade de grãos é obtida com 86 kg ha-1 de N, com equações lineares que mostram acréscimos da produtividade da palha e de indústria e a proteína total, e redução do teor de fibras dos grãos de aveia pelo uso do nitrogênio.
ABSTRACT The efficiency of nitrogen use by oats in association with climatic conditions is fundamental to the development of more sustainable managements with yield and quality. The objectives of this study were to define the agronomic efficiency of nitrogen by the ratio of the dose provided and product obtained, estimate the maximum technical efficiency of the nutrient on grain yield; and for the optimum dose, simulate the expression of the straw and industry yields, protein and total fiber in different conditions of the agricultural year in a soybean/oat system. The study was conducted from 2011 to 2016, in Augusto Pestana, RS, Brazil, in a randomized block design with four repetitions in a 4 x 2 factorial referring to nitrogen doses (0, 30, 60 and 120 kg ha-1) and oat cultivars (Barbarasul and Brisasul) in a soybean/oat system. Nitrogen increased grain, straw, and industry yields and total grain protein, with agronomic efficiency of 7.8, 19.7 and 3.3 kg ha-1 and 0.10 g kg-1, respectively, with reduction of the total fiber in 0.05 g kg-1 per kg of N supplied. The dose of maximum technical efficiency in the expression of grain yield is dependent on the weather conditions during cultivation. In general, the maximum efficiency of grain productivity was obtained with 86 kg ha-1 of N, with linear equations showing increased productivity of straw and industry yield, total protein, and reduction of the fiber content of oat grains by nitrogen use.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

RESUMO: A transferência nuclear de células somáticas ou clonagem é uma técnica que permite produzir um indivíduo geneticamente igual a um outro indivíduo adulto. Esta técnica abre inúmeras possibilidades para a medicina e para a reprodução animal. Porém, existem inúmeros relatos de problemas associados à clonagem. A taxa de perda nos períodos embrionário e fetal ainda é muito alta quando comparada a outras biotécnicas; além disso, há uma maior incidência de hidropsias e distocias, diminuindo a eficiência e aumentando o custo da técnica. Os animais que vem a termo frequentemente apresentam uma síndrome chamada de macrossomia, e apresentam dificuldades de adaptação à vida extrauterina e, por isso, o óbito é um desfecho comum. No presente trabalho realizou-se necropsia e coleta de fragmentos de órgãos para avaliação histopatológica de 41 bezerros com óbito neonatal. As lesões macroscópicas mais importantes foram encontradas no fígado (hepatomegalia, congestão e coloração amarelada), rins (coloração amarronzada na superfície e ao corte, e cistos), pulmões (atelectasia, parênquima consolidado, e secreções nos brônquios e bronquíolos), e coração (hipertrofia concêntrica e excêntrica, cistos hemáticos e persistência de ducto arterioso). As principais alterações microscópicas observadas foram presença de pigmento acastanhado no interior dos túbulos corticais renais (aproximadamente 90% dos animais), degeneração hepática (85% das amostras avaliadas) e lesões características de pneumonia (55% dos animais). A pigmentação acastanhada no interior dos túbulos corticais é uma alteração que ainda não havia sido relatada anteriormente em animais clonados. As alterações observadas nestes órgãos foram determinantes para o óbito, e devem ter ocorrido sobretudo devido a problemas na adaptação ao ambiente extrauterino e em decorrência de infecções adquiridas no período neonatal. Os achados encontrados no presente trabalho denotam a necessidade de investigação anatomopatológica detalhada de animais clonados inviáveis, na tentativa de mapear as anormalidades apresentadas por eles.
ABSTRACT: Somatic-cell nuclear transfer is a cloning technique that enables the creation of a viable embryo from a donor adult to produce a genetically identical individual. This technique opens numerous potential possibilities for medicine and animal reproduction. However, several reports have documented cloning-related issues. Embryo and fetal losses remain significantly higher than in other techniques, and there is a high incidence of dystocia and hydrops, which decreases efficiency and increases costs. Animals delivered at term often exhibit a syndrome known as macrosomia and experience difficulties in adapting to life outside the uterus, and death is a common outcome. In the present study, 41 cloned calves that died in the neonatal period were subjected to gross and histopathological examination. Most important gross lesions were found in the liver (enlargement, congestion, yellowish color), kidneys (brownish color at surface and cut, and cysts), lungs (atelectasis, parenchymal consolidation, and secretions in bronchi and bronchioles), and heart (concentric and eccentric hypertrophy, hematic cysts, persistence of ductus arteriosus). Primary microscopic findings were seen in the liver, kidneys, and lungs from neonatal calves. In the liver, 85% of the animals exhibited hepatic degeneration. The presence of a brownish pigment within the cortical tubules of the kidneys was found in approximately 90% of the samples; the presence of this pigment has not been previously reported in cloned calves. In the lungs, a large number of animals exhibiting lesions characteristic of pneumonia (55%). These changes were the pivotal causes of death, mainly due to problems in adapting to life outside the uterus and opportunistic infections in the neonatal period. Further investigation focusing on pathological anatomical changes is necessary to map these abnormalities in cloned animals.

The recent emergence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has led society to live with a serious public health problem. In this sense, repositioning of antiretrovirals has captured the attention of the scientific community. Tenofovir disoproxil fumarate (TDF) is an antiretroviral compound that is used to treat acquired immune deficiency syndrome (AIDS) and hepatitis B. In this short report, we present a scale-up investigation of TDF by in situ infrared spectroscopy monitoring and a forced degradation study to describe a new degradation product. Finally, we have evaluated TDF in vitro for SARS-CoV-2 for the first time foreseeing the using of this medicine in pre-clinical and clinical investigations for the COVID-19 (coronavirus disease 2019) treatment.

Resumo A pandemia de COVID-19 tem desafiado pesquisadores e gestores a encontrar medidas de saúde pública que evitem o colapso dos sistemas de saúde e reduzam os óbitos. Esta revisão narrativa buscou sistematizar as evidências sobre o impacto das medidas de distanciamento social na epidemia de COVID-19 e discutir sua implementação no Brasil. Foram triados artigos sobre o efeito do distanciamento social na COVID-19 no PubMed, medRXiv e bioRvix, e analisados atos do poder público nos níveis federal e estadual para sumarizar as estratégias implementadas no Brasil. Os achados sugerem que o distanciamento social adotado por população é efetivo, especialmente quando combinado ao isolamento de casos e à quarentena dos contatos. Recomenda-se a implementação de medidas de distanciamento social e de políticas de proteção social para garantir a sustentabilidade dessas medidas. Para o controle da COVID-19 no Brasil, é imprescindível que essas medidas estejam aliadas ao fortalecimento do sistema de vigilância nos três níveis do SUS, que inclui a avaliação e uso de indicadores adicionais para monitorar a evolução da pandemia e o efeito das medidas de controle, a ampliação da capacidade de testagem, e divulgação ampla e transparente das notificações e de testagem desagregadas.
Abstract The COVID-19 pandemic has challenged researchers and policy makers to identify public safety measures forpreventing the collapse of healthcare systems and reducingdeaths. This narrative review summarizes the available evidence on the impact of social distancing measures on the epidemic and discusses the implementation of these measures in Brazil. Articles on the effect of social distancing on COVID-19 were selected from the PubMed, medRXiv and bioRvix databases. Federal and state legislation was analyzed to summarize the strategies implemented in Brazil. Social distancing measures adopted by the population appear effective, particularly when implemented in conjunction with the isolation of cases and quarantining of contacts. Therefore, social distancing measures, and social protection policies to guarantee the sustainability of these measures, should be implemented. To control COVID-19 in Brazil, it is also crucial that epidemiological monitoring is strengthened at all three levels of the Brazilian National Health System (SUS). This includes evaluating and usingsupplementary indicators to monitor the progression of the pandemic and the effect of the control measures, increasing testing capacity, and making disaggregated notificationsand testing resultstransparentand broadly available.

Our study aimed at the development of a method based on ultra-performance liquid chromatography coupled to mass spectrometry (UPLC-ESI-MS) using selected ion monitoring for the simultaneous quantitation of the alkaloids galantamine, pseudolycorine, sanguinine and narciclasine in Amaryllidaceae species (Hippeastrum elegans, Habranthus cf. irwinianus, Hymenocallis littoralis and Griffinia nocturna). The alkaloids were extracted from dried and ground bulbs (100 mg) using liquid-liquid microextraction followed by solid phase extraction in cation exchange cartridges. The quantification method showed good linearity (correlation coefficient, R ≥ 0.9968) and selectivity for a run time of 10 min. All values were within the acceptable limits for recovery (87.5-96.2%), interday (coefficient of variation, CV%, 1.3-8.4%) and intraday precision (CV% 5.7-8.1%), except narciclasine (70%). The limits of detection and quantification were 5-100 and 20-350 ng mL-1, respectively. Our method demonstrated to be rapid, sensitive and reliable, therefore appropriate for being employed in the prospection of new source of galantamine and other alkaloids.

ABSTRACT Leishmaniasis is one of the most neglected tropical diseases worldwide caused by the parasite Leishmania. In Paraguay the species responsible for cutaneous leishmaniasis (CL) is L. (Viannia) braziliensis. Here we report a case diagnosed with Leishmaniasis, and molecular analyses using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) demonstrate that the case was caused by L. (V.) lainsoni. This is the first record of this species for Paraguay, with which we extend the distribution range of the parasite 1,430 km southwards from the southernmost previous known locality. More studies are needed to know the actual incidence of this species in cases of CL in Paraguay, and to identify natural reservoirs in the wild.
RESUMEN La leishmaniasis es una de las enfermedades tropicales más desatendidas, causada por el parásito Leishmania. En Paraguay, la especie responsable de la leishmaniasis cutánea es (LC) es L. (Viannia) braziliensis. Aquí se reporta un caso diagnosticado de Leishmaniasis, y análisis moleculares empleando reacción en cadena de la polimerasa - polimorfismos de restricción de fragmentos de restricción (PCR-RFLP) demostraron que el caso fue causado por L. (V.) lainsoni. Este es el primer registro de esta especie para Paraguay, con lo cual se extiende el rango de distribución conocido de la especie, unos 1.430 km al sur de localidades previamente conocidas. Se necesitan más estudios para conocer la incidencia real de esta especie en casos de LC en Paraguay, y para identificar reservorios naturales del parásito en la naturaleza.
