Abstract Introduction: The COVID-19 pandemic has posed challenges to entities responsible for qualifying healthcare professionals worldwide, needing adaptation to offer distance learning courses in the face of implications such as social distancing. However, despite these courses, there are still gaps in understanding their effects. Objective: To analyse the feeling of professional managers on the effects of a distance learning course, implemented for the development of public health competencies during the COVID-19 pandemic. Methodology: Qualitative research based on data collected from the virtual course environment, which included responses to a question posed to manager-professionals one month after completion: "Which tasks do you routinely perform that have undergone any modification due to your participation in the course?" Data analysis used Descending Hierarchical Classification with the aid of the IRAMUTEQ® software, focusing on Simple Segments of Text (SST). SSTs were classified based on their respective vocabularies of higher frequency and words with significant association (p<0.001). Finally, the software produced a dendrogram illustrating the hierarchical class scheme. Results: 256 responses were processed with a utilization rate of 76.3% of SSTs, resulting in a dendrogram with four classes, revealing modifications in (1) Execution of daily work tasks; (2) Expansion of knowledge about work in the Unified Health System; (3) Coordination between areas for the development of actions in Public Health; (4) Support and guidance for health surveillance actions in collaboration with municipalities. Final Considerations: The use of IRAMUTEQ® software eased the processing and analysis of a large amount of qualitative data, yielding findings that showed significant changes in the professional practice of managers and, so, a positive impact on the development of public health competencies even during a pandemic.

Resumo Introdução: a pandemia COVID-19 trouxe desafios às instâncias responsáveis pela qualificação dos profissionais de saúde no mundo todo, que necessitaram se adaptar para oferta de cursos na modalidade à distância, frente às implicações impostas, como o distanciamento social. Entretanto, a despeito desses cursos, ainda persistem lacunas na produção de conhecimentos dos seus efeitos. Objetivo: analisar a percepção de profissionais-gestores sobre os efeitos de um curso na modalidade à distância, implementado para desenvolvimento de competências em saúde pública em tempos de pandemia COVID-19. Metodologia: pesquisa qualitativa realizada a partir de dados coletados do ambiente virtual do curso que, dentre outras informações, possuía respostas de uma pergunta realizada para os profissionais-gestores um mês após a sua conclusão: quais tarefas você executa habitualmente que sofreram algum tipo de modificação em consequência de sua participação no curso? A análise dos dados ocorreu por Classificação Hierárquica Descendente com auxílio do software IRAMUTEQ®, a partir de segmentos simples de texto (SST). Os SST foram classificados em função dos respectivos vocabulários de maior frequência e das palavras com associação significativa (p<0,001). Por fim, o software produziu um dendrograma com a ilustração do esquema hierárquico de classes. . Resultados: foram processadas 256 respostas com aproveitamento de 76,3% dos SST, que resultaram em um dendrograma de quatro classes, sendo possível constatar modificações na/o: (1) Execução de tarefas do cotidiano laboral; (2) Ampliação do conhecimento acerca do trabalho no Sistema Único de Saúde; (3) Articulação entre áreas para desenvolvimento das ações em Saúde Pública; (4) Apoio e orientações de ações de vigilância em saúde junto aos municípios. Considerações Finais: o uso do software IRAMUTEQ® facilitou o processamento e a análise de grande número de dados qualitativos. Destarte, foram obtidos achados que apontaram mudanças significativas na prática profissional dos gestores e, consequentemente, efeito positivo no desenvolvimento de competências.

This work takes a qualitative approach, anchored in the Paradigm of Complexity, conducted with twenty families involved in negligence against children and adolescents in a municipality in the countryside of the state of São Paulo, Brazil. Data collection took place through minimal maps of the personal social network and semi-structured interviews in January 2021. The networks were limited, with little to no interaction among the different services and sectors, and were predominantly homogeneous. Because they have many weakened ties, they are relatively unsupportive, pointing out difficulties in access to work, education, and health. Due to the characteristics of the network, the complexity of the phenomenon of neglect was identified, in which elements condition and perpetuate experiences of absence and fragility. Interprofessional and intersectoral views and actions are requested and recommended.

Resumo Introdução: As técnicas endoscópicas são atualmente consideradas abordagens de primeira linha no manejo das fístulas associadas a cirurgia bariátrica. O uso off-label de dispositivos de oclusão do septo cardíaco (CSDO) é uma técnica nova que tem demonstrado resultados favoráveis no encerramento de defeitos extra-vasculares, incluindo fístulas gastrointestinais. Relatos de caso prévios reportaram resultados semelhantes com o CSDO Amplatzer™ para o tratamento de fístulas gastrointestinais pós cirurgia bariátrica. No entanto, o uso de dispositivos alternativos semelhantes para esse fim ainda não foi descrito. Relato de Caso: Este relato de caso apresenta o primeiro uso reportado do CSDO Occlutech® para tratamento de fístula gastrocutânea crônica após cirurgia bariátrica revisional. Apesar do aparente sucesso inicial - nenhum extravasamento de contraste através do dispositivo na fluoroscopia após a colocação do CSDO, houve recorrência da drenagem fistulosa devido ao deslocamento parcial do dispositivo. A colocação de um segundo dispositivo entre os discos do primeiro acabou por encerrar o orifício fistuloso. Discussão: Nas fístulas gastrointestinais crônicas, o trajeto epitelizado muitas vezes não é passível de aplicação dos métodos endoscópicos tradicionais, levando a múltiplas tentativas fracassadas de encerramento. A nova aplicação de Occlutech® CSDO pode evitar o risco de uma laparotomia de alto risco nesses casos. Equipamentos endoscópicos adequados, bem como o envolvimento de equipe multidisciplinar são condições primordiais para garantir o sucesso do tratamento.

Abstract Introduction: Endoscopic techniques are now considered the first-line approach for the management of bariatric surgery-related fistulas. The off-label use of cardiac septal defect occluders (CSDO) is an emerging technique that has demonstrated favorable outcomes for the closure of extra-vascular defects, including gastrointestinal (GI) disruptions. Previous case reports have reported similar results with the CSDO Amplatzer™ for the management of GI disruptions following bariatric surgery. However, the use of similar alternative devices for this purpose has not yet been described. Case Presentation: This case report presents the first reported use of the Occlutech® CSDO for the treatment of a chronic gastrocutaneous fistula after bariatric revisional surgery. Despite apparent initial success - no extravasation of contrast material through the device in the contrast study after the CSDO placement - fistula closure failed due to partial dislodgement of the device. The placement of a second device between the discs of the former one ultimately sealed the fistulous orifice. Discussion: In chronic GI fistulas, the mature tract is often not liable to the application of standard endoscopic methods, leading to failed closure attempts. A new application of Occlutech® CSDO can obviate the clinical burden of a high-risk laparotomy in these cases. Appropriate endoscopic equipment as well as the involvement of a multidisciplinary team are prime conditions to ensure successful patient outcomes.

ABSTRACT Introduction. Different environmental hazards associated with water converge in Santiago de Cuba, which constitutes a permanent threat to human life and to the socioeconomic development of the province (heavy rains, floods, tropical cyclones, among others). Its study in terms of disaster reduction is a priority for the Cuban State. Objective: Doing a group of research and development actions in order to assess the hazard, vulnerability and risk caused by water in nature, and in water supply systems of Santiago de Cuba province, in the face of the impact of environmental phenomena. Methods. Investigations of a multidisciplinary group structured in three parts are integrated: Studies of hazard, vulnerability and risks (HVR) in water systems of Santiago de Cuba; studies of HVR in watersheds and vulnerability behavior due to the impact of capillarity humidity in buildings from the historical center of Santiago de Cuba city. Results. Tools and solutions to strengthen forecasting and early warning systems for the population and institutions involved. Design of a new procedure for risk management based on the theory of processes, vulnerability studies in water supply systems, and water as a phenomenon of capillarity humidity in buildings. Watersheds diagnostics and flood modeling; the monthly rainfall series for the province was updated, as well as the design of professional software for early warning of extreme hydrometeorological events and community actions to improve risk culture.

RESUMEN Introducción. En Santiago de Cuba convergen distintos peligros ambientales asociados al agua que constituyen una amenaza permanente a la vida humana y al desarrollo socioeconómico de la provincia (intensas lluvias, inundaciones, ciclones tropicales, entre otros.). Su estudio en función de la reducción de desastres constituye una prioridad para el Estado cubano. Con el objetivo de realizar un conjunto de acciones de investigación y desarrollo encaminadas a determinar el peligro, la vulnerabilidad y el riesgo que causa el agua tanto en la naturaleza, como en las obras de infraestructura de la provincia Santiago de Cuba, ante el posible impacto de distintos fenómenos ambientales. Métodos. Se integran investigaciones de un grupo multidisciplinario estructurado en 3 partes: Estudios de peligro, vulnerabilidad y riesgos (PVR) en obras hidráulicas de Santiago de Cuba; estudios de PVR en cuencas hidrográficas de Santiago de Cuba y comportamiento espacial de la vulnerabilidad por el impacto de la humedad por capilaridad en edificaciones del centro histórico de la ciudad de Santiago de Cuba. Resultados. Herramientas y soluciones para fortalecer los sistemas de pronóstico y alerta temprana a la población e instituciones involucradas. Diseño de un nuevo procedimiento para la gestión de riesgos basado en la teoría de procesos, estudios de vulnerabilidad en obras hidráulicas y el agua como fenómeno de humedad capilar en las construcciones. Diagnósticos en cuencas hidrográficas y modelaciones de inundaciones; se actualizó la serie de precipitaciones mensuales de la provincia, así como diseño de software profesionales para la alerta temprana ante el impacto de eventos hidrometeorológicos extremos y acciones comunitarias para mejorar la cultura de riesgos.

Resumo As restrições, decorrentes da pandemia de coronavírus de 2019, estabeleceram um contexto prolongado de implementação forçada do teletrabalho em diversas organizações e instituições. O presente estudo analisa o impacto exercido pela intensidade na adoção do referido regime laboral na satisfação dos colaboradores do setor do ensino superior. Para isso, foi aplicado um instrumento transversal a uma amostra de 154 colaboradores de uma universidade colombiana, cujos dados foram analisados por meio de modelagem de equações estruturais. Os resultados obtidos sugerem a existência de associação significativa entre as variáveis em questão. Observa-se também que tanto os indivíduos com mais de 40 anos quanto os que possuem um local isolado para o teletrabalho tendem a favorecer essa relação.

Abstract The restrictions derived from the pandemic by the coronavirus 2019 created a prolonged context of forced implementations of telework in several organizations and institutions. This study examines the impact exerted by the intensity of the adoption of this work scheme on the satisfaction of employees in the higher education sector. To do this, the study applies a cross-sectional instrument to a sample of 154 employees of a Colombian university, and the data obtained was subsequently analyzed by structural equation modeling. Results suggest the existence of a significant association between the variables in question. It is also observed that both individuals over 40 years of age and those who have an isolated place to telework tend to favor this relationship.

Resumen Las restricciones derivadas de la pandemia por el coronavirus 2019 instauraron un contexto prolongado de implementación forzosa de teletrabajo en diversas organizaciones e instituciones. El presente estudio examina el impacto ejercido por la intensidad en la adopción de dicho esquema laboral sobre la satisfacción de los colaboradores del sector de la educación superior. Para ello, se aplicó un instrumento transversal sobre una muestra de 154 colaboradores de una universidad colombiana, cuyos datos fueron analizados mediante un modelamiento de ecuaciones estructurales. Los resultados obtenidos sugieren la existencia de una asociación significativa entre las variables en cuestión. Se observa además que tanto los individuos mayores de 40 años como aquellos que cuentan con un lugar aislado para teletrabajar tienden a favorecer dicha relación. Clasificación JEL: J28.

Resumo O uso de coagulantes à base de plantas (coagulante natural) em tratamentos de efluentes tem potencial contra coagulantes inorgânicos utilizados comercialmente. Este trabalho teve como objetivo avaliar os coagulantes orgânicos hemiceluloses catiônicas (CH) - sintetizadas a partir da casca de amendoim - associadas ao tanino catiônico (TSG) - comercial - para coagulação/floculação como tratamento primário, seguido de separação sólido-líquido via sedimentação/flotação por ar (DAF). O ensaio foi realizado em jar test em efluente de uma indústria multinacional do setor de beneficiamento de grãos, localizada na cidade de Uberlândia-MG. Os diagramas de coagulação foram determinados pelo método de interpolação espacial de dados do modelo de regressão de Kringing e o teste de Tukey foi utilizado para avaliar a diferença nos resultados obtidos. Os pontos ótimos de eficiência de remoção de turbidez (TRE), superiores a 98%, foram alcançados para a associação TSG/CH com 200 mg L-1 (pH 10,72), 350 mg L-1 (pH 9,72), 500 mg L-1 (pH 9,56) na sedimentação. Para a separação por DAF, a associação de TSG/CH resultou em valores de TRE superiores a 95% nas dosagens de 350 mg L-1 (pH 9,59) e 500 mg L-1 (pH 7,92). Além disso, os resultados obtidos indicam que o uso associado de TSG/CH como auxiliar de coagulação, favoreceu o acoplamento bolha-partícula, resultando em menor volume de lodo, além disso, o CH expandiu a ação do TSG para a região básica.

Abstract The use of plant-based coagulants (natural coagulants) in wastewater treatments has potential advantages over the inorganic coagulants used commercially. This study evaluated organic coagulants cationic hemicelluloses (CH) synthesized from peanut shell and associated with commercial cationic tannin (TSG) for use as the primary coagulation/flocculation treatment, followed by solid-liquid separation via sedimentation/flotation by dissolved air (DAF). The assay was carried out in a jar test on effluent from a multinational industry in the grain processing sector, located in the city of Uberlândia-MG. Coagulation diagrams were determined using the data spatial interpolation method of the Kringing regression model and the Tukey test was used to assess the difference in the results obtained. The optimum removal points of turbidity removal efficiency (TRE), greater than 98%, were achieved for the TSG/CH association with 200 mg L-1 (pH 10.72), 350 mg L-1 (pH 9.72), 500 mg L-1 (pH 9.56) in sedimentation. For the separation by DAF, the association of TSG/CH resulted in TRE values greater than 95% at dosages of 350 mg L-1 (pH 9.59) and 500 mg L-1 (pH 7.92). Furthermore, the results indicate that the associated use of TSG/CH, a coagulation aid, favored the coupling of the DAF bubble-particle, resulting in a smaller volume of sludge. In addition, CH expanded the action of TSG to the basic region.

RESUMO Objetivo Verificar se a estimulação das habilidades auditivas de fonoaudiólogos com e sem dificuldades no processamento auditivo central (PAC) interfere no desempenho da avaliação perceptivo-auditiva (APA) do desvio vocal predominante. Métodos Estudo prospectivo, quase-experimental, realizado com 26 fonoaudiólogos, desenvolvido em cinco etapas: Etapa1: Pré-intervenção - primeira APA, composta por 30 amostras da vogal sustentada /ε/, acrescidas de 20% de repetição (seis amostras repetidas), apresentadas randomicamente, na qual os participantes deveriam identificar o desvio vocal predominante; Etapa 2: Triagem das habilidades do PAC com nove testes da plataforma AudBility; Etapa 3: Treinamento perceptivo-auditivo do desvio vocal predominante e segunda APA, idêntica à anterior; Etapa 4: Intervenção - estimulação das habilidades auditivas (EHA), realizada por 14 fonoaudiólogos, sendo oito que não passaram na triagem da Etapa 2 (Grupo1) e seis que passaram na triagem e, espontaneamente, realizaram a EHA (Grupo G2a). Os outros 12 participantes não realizaram EHA e formaram o Grupo G2b; Etapa 5: Pós-intervenção - realização da terceira APA, idêntica às anteriores, por todos os fonoaudiólogos. A acurácia das respostas e confiabilidade intra-avaliadores foram verificadas ao longo das três APAs. Resultados Os três grupos apresentaram desempenhos semelhantes nas três APAs. G1 e G2a não apresentaram melhora no desempenho da análise do desvio vocal predominante pós-EHA. A confiabilidade intra-avaliador pré-intervenção no G1 foi inferior a do G2a e G2b nas APA1 e APA2, e similar a eles na APA3, pós-intervenção. Conclusão EHA proposto não impactou a acurácia da APA, mas influenciou a confiabilidade intra-avaliador dos fonoaudiólogos com dificuldades nas habilidades auditivas do PAC.

ABSTRACT Purpose To verify if the stimulation of auditory skills of speech therapists with and without difficulty in auditory processing (CAP) interferes in performance of auditory-perceptual assessment (APA) regarding predominant vocal deviation. Methods Prospective, quasi-experimental study, conducted with 26 speech therapists and developed in five stages: Stage1: Pre-intervention - APA, composed of 30 samples of sustained vowel emissions /ε/, plus 20% repetition (six repeated samples) in which participants should identify the predominant vocal deviation. Stage2: Screening of CAP skills through nine tests of AudBility web platform. Stage3: Perceptual-auditory training of predominant vocal deviation and, later, second APA, identical to previous one. Stage4: Intervention - Stimulation of auditory skills (EHA) performed by 14 participants, eight of which failed the screening from Stage2 and six who passed th e screening and spontaneously performed EHA (Group G2a ). The other 12 participants did not perform EHA and formed the G2b Group. Stage5: Post-intervention - Third execution of APA, identical to previous, by all participants. Accuracy of analysis and intra-rater reliability were assessed in all APAs. Results The three groups showed similar performances in the three APAs. G1 and G2a showed no improvement in the performance of the analysis of predominant vocal deviation after EHA. Pre-intervention intra-rater reliability in G1 was lower than in G2a and G2b in APA1 and APA2, and similar to them in APA3, post-intervention. Conclusion Proposed EHA had no impact on APA accuracy, but influenced in intra-rater reliability of speech therapists with difficulty in the hearing skills of CAP.

Introduction: Previous works have reported an association between human papilloma virus (HPV) infection and the development of colorectal cancer, and although controversy regarding this association exists. Materials and Method: This was an observational, cross-sectional, descriptive, retrospective unblinded study. Fifty pathology samples embedded in paraffin with a diagnosis of colorectal adenocarcinoma were used to isolate DNA from the tissue. DNA was extracted according to established protocols for extraction, lysis and sample rehydration. Viral DNA was identified and genotypified to amplify and detect oncogenic subtypes among 35 different subtypes included in the study, sequencing positive samples with established protocols of purification and sample analysis using microarrays. Results: Fourteen of 50 (28%) samples were identified as positive for human papilloma virus; of these 11 (22%) included one or more high-risk subtypes for neoplasia. Statistically significant differences were not found between the groups regarding age, sex, tumor location, degree of differentiation, infiltration, affected lymph nodes, metastasis and number of pack years. Conclusion: The detection of high-risk VPH subtypes in a high percentage of positive samples, suggests an association between infection and the development of colorectal cancer.

Resumen Introducción: Previos trabajos han reportado una asociación entre la infección del virus del papiloma humano (VPH) y el desarrollo de cáncer colorrectal, aunque existe controversia al respecto. Materiales y Método: Estudio observacional, transversal, descriptivo, retrospectivo, no ciego. Se utilizaron 50 muestras de patología con diagnóstico de adenocarcinoma colorrectal, incluidas en parafina, para aislar ADN de las muestras. Se realizó la extracción de ADN mediante protocolos establecidos para extracción, lisis y rehidratación de muestra. Se identificó y genotipicó el ADN del virus para amplificar y detectar subtipos oncogénicos de entre 35 subtipos diferentes incluidos en la prueba, secuenciando las muestras positivas, utilizando protocolos ya establecidos de purificación y análisis de muestra, mediante microarreglos. Resultados: Se identificaron 14 muestras de 50 (28%) estudiadas positivas para el virus de papiloma humano de las cuales 11 (22%) incluyen uno o más subtipos de alto riesgo para neoplasia. No se identificaron diferencias estadísticamente significativas entre grupos en cuanto a edad, sexo, localización del tumor, grado de diferenciación, infiltración, ganglios afectados, metástasis o número de paquetes/año. Conclusión: La detección de los subtipos de VPH de alto riesgo en un alto porcentaje de las muestras positivas, sugiere una asociación entre la infección con el desarrollo de cáncer colorrectal.

Neste trabalho foi efetuada uma revisão da literatura no que respeita à interação da atividade física com o Câncer, bem como com intervenções nutricionais passiveis de promoverem um melhor prognóstico da doença. Ficou evidente na literatura que a atividade física é um fator associado ao estilo de vida que se relaciona com a prevenção e o tratamento do câncer, principalmente por exercer efeitos anti-inflamatórios e imunoestimuladores. O exercício aeróbio regular de intensidade moderada está relacionado com a melhora da capacidade funcional bem como ao aumento da população e atividade de células imunológicas envolvidas no combate aos tumores. A literatura sugere que os exercícios podem ser realizados com segurança após a quimioterapia para prevenir perda de desempenho físico, favorecer a manutenção da massa muscular e, consequentemente, contribuir com o prognóstico. Seus efeitos podem ser potencializados quando associados à prescrição nutricional, com a adição de vitamina D, BCAA, HMB e resveratrol, para fornecer ao corpo e aos músculos substratos adequados para o seu funcionamento e para a sua melhor capacidade em responder ao tratamento.

This work presents a review of the literature concerning the interaction of physical activity and cancer, as well as with nutritional interventions designed to promote a better prognosis. It was evident in the literature that physical activity is a factor that is associated with lifestyle and with prevention and treatment of cancer, mainly because of its anti-inflammatory and immuno-stimulating effects. Aerobic exercise with average intensity performed regularly is related with improvement of functional capacity as well as with increases in the population and in the activity of immune cells fighting cancer. The literature suggests that exercise can be performed safely post-chemotherapy to prevent the loss f physical ability, to favor maintenance of muscle mass and, subsequently, to improve general prognosis. All these effects can be potentially enhanced by nutritional interventions including, adding of D vitamin, BCAA, HMB and resveratrol; in order to supply muscles with appropriate nutrients to their function and to improve the capacity to tolerate the treatment.

Abstract: In Mexico, around 200,000 enterprises are created every year, however, only 35% of them survive after two years of operations. Among the main causes of this problem, we can find the lack of access to financing, labor force with no skills and deficient information about market opportunities. Two strategies commonly used by enterprises are training of its associates and the participation in associations like the National Network of Sustainable Rural Development (Rendrus). The main objective of this study was to identify the success factors of rural microenterprises that produce cactus leaves processed products in the State of Tlaxcala, Mexico, which participated in meetings with Rendrus. The research technique of "Case Studies" was applied to interview to participants in three enterprises. The results show that enterprises have survived in the market because of their social capital, as the main factor of success, manifested trough their organizational process, compromise, dedication, and solidarity that allowed them to face limitations in availability of technology, problems of marketing and commercialization, low levels of sales and limited schemes of innovation.

Resumen: En México alrededor de 200,000 empresas se crean cada año, sin embargo, únicamente el 35% de ellas sobreviven después de dos años de operación. Entre las causas más importantes de este problema se encuentran la falta de acceso al financiamiento, fuerza de trabajo carente de habilidades y deficiente información sobre oportunidades de mercado. Dos estrategias comúnmente adoptadas por las empresas son la capacitación de sus integrantes y la participación en asociaciones, como la Red Nacional de Desarrollo Rural Sustentable (Rendrus). El objetivo de este estudio fue identificar los factores de éxito de microempresas rurales que se dedican a la producción de productos procesados de nopal, en el estado de Tlaxcala, México, que han participado en las reuniones de la Rendrus. Metodología. Se utilizó la técnica de investigación de "Estudios de casos", entrevistando a diversos integrantes de tres empresas. Los resultados indican que las empresas han sobrevivido en el mercado gracias a su capital social, como el principal factor del éxito, manifestado a través de sus procesos organizativos, compromiso, dedicación y solidaridad, les ha permitido enfrentar las limitantes en relación a la disponibilidad de tecnología, problemas de mercadeo y canales de comercialización limitados, niveles bajos de ventas y esquemas limitados de innovación.

Prior to 2007, efforts to monitor and conserve hawksbill turtles (Eretmochelys imbricata) in the eastern Pacific Ocean were opportunistic and records were virtually non-existent. The first abundance estimates were published in 2010, but contained limited data on the species. Ongoing research since that time has led to the identification of several rookeries, including sites containing large proportions of the overall hawksbill nesting currently known to occur in the region. Monitoring projects were established at several sites and have since provided substantial nesting data on the species. Here we summarize data collected between 1983 and March 2016 from all sites (n = 9) confirmed to host >10 nests in any given season to provide an update on hawksbill nesting in the eastern Pacific. We documented a total of 3,508 hawksbill nests, 265,024 hatchlings and 528 individual nesting females in the region. The vast majority of these records (99.4%, 99.9% and 99.6%, respectively) were generated subsequent to 2007, coinciding with the discovery of eight of the nine rookeries included in this study and the organization of monitoring efforts at those sites, which led to the increased documentation conferred here. Our findings should not be misconstrued as increases in actual nesting or signs of recovery, which could diminish the ongoing need for conservation actions, but rather as optimism, that there is still an opportunity to restore the species in the eastern Pacific. The top three sites in terms of average annual number of nests were Estero Padre Ramos (Nicaragua; 213.2 ± 47.6 nests), Bahia de Jiquilisco (El Salvador; 168.5 ± 46.7 nests) and Aserradores (Nicaragua; 100.0 ± 24.0 nests), and all three sites are located in mangrove estuaries in Central America, highlighting the importance of these rookeries/habitats for the survival and recovery of hawksbills in the region. The remaining six sites received between 6.9 ± 7.3 nests (Costa Careyes, Mexico) and 59.3 ± 17.7 nests (Los Cobanos, El Salvador) annually. By integrating data collected on nesting hawksbills with local conservation realities at the most important known hawksbill rookeries in the eastern Pacific, we provide a more holistic view of the conservation status and management needs of the species in this ocean region.

Objetivo Avaliar o potencial da angiotomografia computadorizada multidetectores (ATCM) na determinação etiológica precoce do acidente vascular encefálico (AVE) e correlacionar o mecanismo causal com o padrão de infarto. Método De Julho de 2011 a Julho de 2013, foram selecionados os pacientes com suspeita clínica de AVE hiperagudo. Os achados da ATCM dos vasos intracranianos e cervicais foram correlacionados com a etiologia final do evento. Resultados AVE foi confirmado em 50/106 pacientes (47,2%). Estes apresentaram alterações angiográficas estatisticamente mais relevantes. Aterosclerose dos grandes vasos intracranianos esteve presente em 34% destes pacientes. O padrão radiológico topográfico de infarto mais comum foi o infarto territorial parcial (86%). A embolização arterio-arterial foi o mecanismo mais prevalente (52%). Conclusão A utilização da ATCM traz benefícios na detecção etiológica precoce dos pacientes com suspeita de AVE hiperagudo, além de possibilitar o entendimento do padrão radiológico topográfico de acordo com o mecanismo causal do evento isquêmico.

Objective The potential of computed tomography angiography (CTA) was assessed for early determination of stroke subtypes in a Brazilian cohort of patients with stroke. Method From July 2011 to July 2013, we selected patients with suspected hyperacute stroke (< 6 hours). Intracranial and cervical arteries were scrutinized on CTA and their imaging features were correlated with concurrent subtype of stroke. Results Stroke was documented in 50/106 selected patients (47.2%) based on both clinical grounds and imaging follow-up (stroke group), with statistically significant arterial stenosis and vulnerable plaques on CTA. Intracranial large artery disease was demonstrated in 34% of patients in the stroke group. Partial territorial infarct prevailed (86%) while artery-to-artery embolization was the most common stroke mechanism (52%). Conclusion Multidetector CTA was useful for the etiologic work-up of hyperacute ischemic stroke and facilitated the knowledge about the topographic pattern of brain infarct in accordance with its causative mechanism.

Objetivou-se avaliar a resposta tecidual, após implantação de hidroxiapatita sintética 30% (HAP-91®), em dois estados físicos, para preenchimento cutâneo. Foram utilizados 18 coelhos da raça Nova Zelândia, distribuídos aleatoriamente em dois grupos (HAP-91® fluida e viscosa) e, posteriormente, divididos em três subgrupos, de acordo com o período pós-implante, em oito, 21 e 49 dias. Em cada grupo foi implantado 1,0 mL de HAP-91®, fluida ou viscosa, no tecido subcutâneo, 1 cm cranial à crista da escápula direita. Foi realizada a mensuração da espessura cutânea, imediatamente antes e durante 15 dias após a implantação do compósito. Avaliações de sensibilidade dolorosa foram realizadas, atribuindo-se os escores: 0 - quando o animal permitiu o toque e não manifestou reação; 1 - quando permitiu o toque, mas manifestou reação de dor, como aumento da frequência respiratória ou tentativa de fuga; 2 - quando não permitiu o toque. Aos oito, 21 e 49 dias, foi realizada a biópsia da região implantada. Não foi observada diferença entre a espessura cutânea dos animais (p > 0,05) e todos os animais receberam escore 0 para sensibilidade dolorosa. Na análise histológica, não foi notado processo inflamatório evidente. Houve predomínio de células mononucleares, nas amostras de oito dias, e uma organização tecidual ao redor do biomaterial, com tendência ao encapsulamento. A HAP-91®, tanto fluida quanto viscosa, é biocompatível e indicada para o preenchimento cutâneo.

The aim of this study was to evaluate the response to the implantation of synthetic hydroxyapatite 30% (HAP-91®) in different physical states as dermal filler. Eighteen New Zealand rabbits were used, distributed randomly into two equal groups and then divided into three groups according to the postoperative period at 8, 21 and 49 days. One mL of HAP-91®, fluid and viscous, was implanted in the subcutaneous tissue, 1 cm proximal to the cranial crest of the right scapula. The thickness of the skin was measured before and after implantation and for the following 15 days. Pain sensitivity assessment was conducted, assigning the following scores: 0 - when the animal allowed the touch of the implant area and expressed no signs of pain; 1 - when the animal allowed the touch, but pain reaction occurred, like increase of the respiratory rate or attempt to escape; 2 - when the animal did not allow the touch to the implanted area. At 8, 21 and 49 days, biopsy of the implanted area was performed. No difference was observed between the thickness of the skin (p>0.05) and all animals received a score 0 for soreness. Histological analysis did not reveal any obvious inflammatory process, showing a predominance of mononuclear cells in samples of eight days and tissue organization around the biomaterial with a tendency to encapsulation. The results indicate that HAP-91®, both viscous and fluid, is biocompatible and suitable for dermal filling.

A tomografia computadorizada multidetectores (TCMD) permitiu o diagnóstico precoce de isquemia cerebral hiperaguda. O presente estudo objetivou validar a interpretação e a descrição padronizada de um protocolo de TCMD multiparamétrica em uma série de pacientes adultos. A concordância entre os examinadores foi testada, e seus resultados confrontados com uma leitura padrão. Não foram observados resultados falso-positivos, e foi documentado um elevado grau de concordância (Kappa>0,81) quando os dados da angiotomografia (ATC) e dos mapas de perfusão cerebral por TC (PCTC) foram adicionados à análise da TC sem contraste (TCSC). A concordância interobservador foi superior para os leitores melhor treinados, corroborando a necessidade de formação específica para a interpretação dos exames. Os autores recomendam acrescer a interpretação da ATC e da PCTC à análise da TCSC, visando à análise global das anormalidades cerebrais estruturais e hemodinâmicas. O presente protocolo é adequado como um roteiro reprodutível para a análise da TCMD de pacientes com suspeita de acidente vascular cerebral isquêmico.

Multidetector computed tomography (MDCT) scanning has enabled the early diagnosis of hyperacute brain ischemia. We aimed at validating a standardized protocol to read and report MDCT techniques in a series of adult patients. The inter-observer agreement among the trained examiners was tested, and their results were compared with a standard reading. No false positives were observed, and an almost perfect agreement (Kappa>0.81) was documented when the CT angiography (CTA) and cerebral perfusion CT (CPCT) map data were added to the noncontrast CT (NCCT) analysis. The inter-observer agreement was higher for highly trained readers, corroborating the need for specific training to interpret these modern techniques. The authors recommend adding CTA and CPCT to the NCCT analysis in order to clarify the global analysis of structural and hemodynamic brain abnormalities. Our structured report is suitable as a script for the reproducible analysis of the MDCT of patients on suspicion of ischemic stroke.
