RESUMO Objetivo: Avaliar as doenças autoinflamatórias (DAI) de acordo com sexo e idade no momento do diagnóstico e a resposta terapêutica em uma grande população. Métodos: Este é um estudo observacional transversal de um registro latino-americano que usou um sistema de dados coletados entre 2015 e 2018. Quaisquer achados alterados ao longo do acompanhamento foram registrados. Os formulários foram traduzidos para os idiomas português e espanhol, incluindo características demográficas, clínicas, laboratoriais, genéticas e de tratamento. Resultados: Incluímos 152 pacientes, sendo 51,3% do sexo masculino e 75% da raça branca. A média de idade de início da doença foi de 2,1 anos (0–15,6 anos) e a média de idade de diagnóstico 6,9 anos (0–21,9 anos); 111 (73%) eram crianças (0–9 anos) e 41 (27%) adolescentes/adultos jovens (10–21 anos). A síndrome de febre periódica, estomatite aftosa, faringite e adenite (PFAPA) ocorreu em 46/152 (30%), osteomielite não bacteriana crônica (CNO) em 32/152 (21%) e febre familiar do Mediterrâneo (FMF) em 24/152 (15,7%). A PFAPA foi significativamente maior em crianças pequenas (38,7 vs. 7,3%, p<0,001), e a CNO, em adolescentes/adultos jovens (13,5 vs. 41,5%, p<0,001). A frequência do sexo feminino foi significativamente maior na CNO (28,4 vs. 14,1%, p=0,031) e menor na FMF (8,1 vs. 23,1%, p=0,011). Os medicamentos mais utilizados foram glicocorticoides, anti-inflamatórios não esteroidais (AINE) e colchicina. O tratamento com glicocorticoides e colchicina foi usado em todas as DAI com resposta boa a moderada. No entanto, as síndromes periódicas associadas à criopirina (CAPS) pareciam não responder aos glicocorticoides. AINE e metotrexato foram os principais medicamentos utilizados no tratamento da CNO. Conclusões: Diferenças de pacientes com DAI foram observadas na população latino-americana em pacientes agrupados por sexo e idade ao diagnóstico da doença. ABSTRACT Objective: To evaluate autoinflammatory diseases (AID) according to age at diagnosis and sex, and response to therapy in a large population. Methods: This is a cross-sectional observational study of a Latin American registry using a designed web system for data storage, collected between 2015 and 2018. Any altered findings during follow-up were recorded. The forms were translated into Portuguese and Spanish, including demographic, clinical, laboratory, genetic and treatment characteristics. Results: We included 152 patients, 51.3% male and 75% Caucasian. The median age at disease onset was 2.1 years (0–15.6 years) and median age at diagnosis 6.9 years (0–21.9 years); 111 (73%) were children (0–9 years old), and 41 (27%) were adolescents and young adults (AYA) (10–21 years old). Periodic fever, aphthous stomatitis, pharyngitis, and adenitis syndrome (PFAPA) occurred in 46/152 (30%), chronic non-bacterial osteomyelitis (CNO) in 32/152 (21%), and familial Mediterranean fever (FMF) in 24/152 (15.7%). PFAPA was significantly higher in young children than in AYA (38.7% vs. 7.3%, p<0.001), while CNO were lower (13.5% vs. 41.5%, p<0.001). The frequency of females was significantly higher in CNO (28.4% vs. 14.1%, p=0.031) and lower in FMF (8.1% vs. 23.1%, p=0.011). The most used drugs were glucocorticoids, non-steroidal anti-inflammatory drugs (NSAID), and colchicine. Glucocorticoids and colchicine treatment were used in all AID with good to moderate response. This was a qualitative study carried out in a public maternity hospital with 25 women in the city of Salvador, Bahia, Brazil. Data were collected through semi-structured interviews and participant observation from November 2021 to February 2022. Content analysis was used to organize the data obtained through the interviews. The results were analyzed through the theoretical contributions of intersectionality, focusing on the interaction between obstetric violence and obstetric racism. The narratives discuss issues of obstetric violence, institutional racism, and how these experiences are permeated by issues of race, gender, and class. Questions related to the feelings of these women regarding the experience of violence at the time of childbirth care were also highlighted. Obstetric racism denies reproductive rights and hinders access to respectful and equitable care for black women.

Abstract Background Systemic sclerosis (SSc) is a rare chronic autoimmune disease with heterogeneous manifestations. In the last decade, several clinical trials have been conducted to evaluate new treatment options for SSc. The purpose of this work is to update the recommendations of the Brazilian Society of Rheumatology in light of the new evidence available for the pharmacological management of SSc. Methods A systematic review including randomized clinical trials (RCTs) for predefined questions that were elaborated according to the Patient/Population, Intervention, Comparison, and Outcomes (PICO) strategy was conducted. The rating of the available evidence was performed according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE) methodology. To become a recommendation, at least 75% agreement of the voting panel was needed. Results Six recommendations were elaborated regarding the pharmacological treatment of Raynaud’s phenomenon, the treatment (healing) and prevention of digital ulcers, skin involvement, interstitial lung disease (ILD) and gastrointestinal involvement in SSc patients based on results available from RCTs. New drugs, such as rituximab, were included as therapeutic options for skin involvement, and rituximab, tocilizumab and nintedanib were included as therapeutic options for ILD. Recommendations for the pharmacological treatment of scleroderma renal crisis and musculoskeletal involvement were elaborated based on the expert opinion of the voting panel, as no placebo-controlled RCTs were found. Conclusion These guidelines updated and incorporated new treatment options for the management of SSc based on evidence from the literature and expert opinion regarding SSc, providing support for decision-making in clinical practice. Abstract In the pediatric population, complex regional pain syndrome (CRPS) is a challenging condition that leads to chronic psychosocial dysfunction. This case is of a 12-year-old male patient, 50 kg, who, after twisting his right ankle, started to present intense pain. Without adequate diagnosis and treatment, his family sought a pain specialist. During the evaluation, a change in sensitivity and temperature was observed, besides hyperalgesia, allodynia, redness, edema, and motor dysfunction, being diagnosed with CRPS. ABSTRACT The Caatinga biome in Brazil comprises the largest and most continuous expanse of the seasonally dry tropical forest (SDTF) worldwide; nevertheless, it is among the most threatened and least studied, despite its ecological and biogeographical importance. The spatial distribution of volumetric wood stocks in the Caatinga and the relationship with environmental factors remain unknown. Therefore, this study intends to quantify and analyze the spatial distribution of wood volume as a function of environmental variables in Caatinga vegetation in Bahia State, Brazil. Volumetric estimates were obtained at the plot and fragment level. The multiple linear regression techniques were adopted, using environmental variables in the area as predictors. Spatial modeling was performed using the geostatistical kriging approach with the model residuals. The model developed presented a reasonable fit for the volume m3 ha with r2 of 0.54 and Root Mean Square Error (RMSE) of 10.9 m3 ha–1. The kriging of ordinary residuals suggested low error estimates in unsampled locations and balance in the under and overestimates of the model. The regression kriging approach provided greater detailing of the global wood volume stock map, yielding volume estimates that ranged from 0.01 to 109 m3 ha–1. Elevation, mean annual temperature, and precipitation of the driest month are strong environmental predictors for volume estimation. This information is necessary to development action plans for sustainable management and use of the Caatinga SDTF in Bahia State, Brazil. RESUMO As árvores transportam água do subsolo para a atmosfera através do processo de evapotranspiração. As mudanças climáticas podem comprometer significativamente esse processo devido a mudanças no uso da terra decorrentes, por exemplo, do desmatamento. O objetivo deste estudo foi caracterizar os atributos hidráulicos e anatômicos de Jacaranda copaia (Aubl.) D. Don (Bignoniaceae) no sudoeste da Amazônia brasileira. Para tanto, foi descrita a curva de vulnerabilidade do xilema desta espécie. A frequência e o diâmetro dos vasos do xilema e a densidade dos estômatos também foram medidos. Finalmente, um atributo hidráulico de Jacaranda copaia foi comparado a outras espécies em nível global, tropical e amazônico. Os achados mostram que, na região estudada, a espécie Jacaranda copaia possui lenhos de porosidade difusa e vasos numerosos (vasos médios variando de 8 a 14 nº/mm2) com diâmetros pequenos (<50µm) a médios (entre 100 e 200 µm). A densidade estomática média variou de 289 a 309 estômatos/mm2. A resistência hidráulica do xilema à embolia (Ψ50) variou de -0.814 a -2.400 MPa, com margens de segurança hidráulica relativamente estreitas (MSH50 variando de -0.312 a 1.122; MSH88 variando de 0.204 a 1.709). Em geral, os valores médios de Ψ50 detectados foram semelhantes a uma grande porcentagem de espécies arbóreas em nível global, tropical e amazônico. ABSTRACT Trees transport water from underground to the atmosphere through the evapotranspiration process. Climate change can significantly compromise this process due to changes in land use, such as deforestation. This study aimed to characterize the hydraulic and anatomical attributes of Jacaranda copaia (Aubl.) D. Don (Bignoniaceae), in the Southwestern Brazilian Amazon. For this purpose, the xylem vulnerability curve of this species was described. The frequency and diameter of the xylem vessels and the stomata density were also measured. Finally, a hydraulic attribute of Jacaranda copaia was compared to other species at global, tropical, and Amazonian levels. The findings show that, in the region studied, the species Jacaranda copaia has diffuse-porous woods and numerous vessels (average vessel ranging from 8 to 14 nº/mm2) with small (<50µm) to medium (between 100 and 200 µm) diameters. The average stomatal density ranged from 289 to 309 stomata/mm2. The xylem hydraulic resistance to embolism (Ψ50) ranged from -0.814 to -2.400 MPa, with relatively narrow hydraulic safety margins (HSM50 ranging from -0.312 to 1.122; HSM88 ranging from 0.204 to 1.709). The average values of Ψ50 detected were similar to a large percentage of arboreal species at global, tropical, and Amazonian levels. Resumo Contexto: Tendo por base os estudos do Modelo Multifatorial Teresa Lluch (1999), foi criado e validado um Programa Promotor Saúde Mental Positiva (SM+) para adolescentes e aplicado numa turma piloto. Após 3 meses do final do programa, procedeu-se ao follow-up na mesma turma, dando origem a novos dados. O trabalho integra-se Projeto Saúde Mental Positiva: CINTESIS/NursID: Centro de Investigação em Tecnologias e Serviços de Saúde/ Escola Superior de Enfermagem do Porto. Objetivo(s): Avaliar o impacto do programa de SM+ em adolescentes após 3 meses do seu término, analisar a eficácia do programa SM+ na promoção da saúde mental dos adolescentes em ambiente escolar e avaliar a vulnerabilidade psicológica dos adolescentes. Métodos: Estudo quantitativo, observacional-descritivo e longitudinal. A amostra foi constituída por 23 alunos com idades entre os 14-16 anos, e como instrumento foi aplicado novamente Escala de Vulnerabilidade Psicológica (EVP) e o Questionário de Saúde Mental Positiva (QSM+). Resultados: Antes do programa o QSM+ da turma era de 94,35, após programa passou a 92,61. Após 3 meses, QSM+ passou para 91,17. Cotações baixas no QSM+ correspondem a níveis mais elevados de SMP, variando entre 39 e 156. Na EVP registou-se alterações, passando de 14,26 após o programa para 15,39 no follow-up. Conclusões: Apesar das diferenças observadas não terem significado estatístico, que nos permita afirmar que o Programa de SM+ teve eficácia na saúde mental dos adolescentes e na sua vulnerabilidade psicológica, foram observadas mudanças a nível comportamental, com maior interajuda entre eles, melhor comunicação e mais ativos nas atividades escolares.

Abstract Background: Based on studies of Teresa Lluch's Multifactorial Model (1999), a Positive Mental Health Promoting Program for adolescents was created and validated and applied to a pilot class. Three months after the end of the program, we followed up in the same class, giving rise to new data. The work is part of the Positive Mental Health Project of CINTESIS / NursID of the Research Center on Health Technologies and Services and the Porto School of Nursing. Aim: To assess the impact of the positive mental health (PMH) program on adolescents 3 months after its end; analyze the effectiveness of the PMP program in promoting adolescent mental health in the school environment and assess the psychological vulnerability of adolescents. Methods: The study was quantitative, observational, descriptive and longitudinal. The sample consisted of 23 students aged 14 to 16 years, and as instrument was applied again the psychological vulnerability scale and the Positive Mental Health Questionnaire (QSM +). Results: Before the program, the class's QSM + was 94.35, after the program it went to 92.61. After 3 months, the QSM + increased to 91.17. Low quotations on the QSM + correspond to higher levels of SMP, ranging from 39 to 156. In EVP, there were changes, going from 14.26 after the program to 15.39 in the follow-up. Conclusions: Although the observed differences have no statistical significance, which allows us to affirm that the PMP Program was effective in the mental health of adolescents and their psychological vulnerability, changes were observed at the behavioral level, with greater mutual help between them, better communication and more active in school activities.

ABSTRACT BACKGROUND: Lung transplantation (LTx) has been discussed as an option for treating irreversible lung fibrosis post-coronavirus disease 2019 (COVID-19), in selected cases. OBJECTIVES: To report on the initial experience and management of end-stage lung disease due to COVID-19 at a national center reference in Brazil. DESIGN AND SETTING: Cohort study conducted at a national reference center for lung transplantation. METHODS: Medical charts were reviewed regarding patients’ demographics and pre-COVID-19 characteristics, post-LTx due to COVID-19. RESULTS: Between March 2020 and September 2021, there were 33 cases of LTx. During this period, we evaluated 11 cases of severe COVID-19-related acute respiratory distress syndrome (ARDS) that were potentially candidates for LTx. Among these, LTx was only indicated for three patients (9.1%). All of these patients were on venovenous extracorporeal membrane oxygenation (ECMO), and the procedure that they underwent was central venoarterial ECMO. All three patients were still alive after the first 30 postoperative days. However, patient #1 and patient #2 subsequently died due to fungal sepsis on the 47th and 52nd postoperative days, respectively. Patient #3 was discharged on the 30th postoperative day. CONCLUSIONS: LTx is feasible among these complex patients. Survival over the first 30 days was 100%, and this favors surgical feasibility. Nonetheless, these were critically ill patients.

RESUMO Objetivos: identificar e sintetizar evidências científicas sobre prevenção de lesões de pele relacionadas a adesivo médico nos pacientes adultos em terapia intensiva. Métodos: trata-se de uma scoping review desenvolvida com base nas recomendações do PRISMA-ScR e no método proposto por Joanna Briggs Institute. Realizaram-se buscas nas bases de dados PubMed, CINAHL, Web of Science, Scopus, LILACS e Embase, utilizando os descritores “Injuries AND Adhesives AND Skin AND Medical”. Resultados: identificaram-se 1.329 estudos; após análise, a amostra final foi composta por nove artigos. Quanto ao tipo de estudos, obtiveram-se dois consensos de especialistas, três estudos de caso, dois estudos transversais, um de coorte prospectivo e uma revisão de literatura. Considerações Finais: as evidências sintetizadas possibilitaram elencar cuidados para prevenção de lesões de pele relacionadas a adesivos médicos. É importante que o profissional saiba identificar tanto as lesões de pele relacionadas aos adesivos médicos quanto as principais estratégias para prevenção destas.

ABSTRACT Objectives: to identify and synthesize scientific evidence on preventing medical adhesive-related skin injuries in adult intensive care patients. Methods: this is a scoping review based on PRISMA-ScR recommendations and the technique proposed by Joanna Briggs Institute. PubMed, CINAHL, Web of Science, Scopus, LILACS, and Embase databases were searched using “Injuries AND Adhesives AND Skin AND Medical” descriptors”. Results: 1,329 studies were identified, and after analysis, the final sample consisted of nine articles. We obtained two experts’ consensus, three case studies, two cross-sectional studies, one prospective cohort study, and one literature review regarding the type of studies. Final Considerations: the synthesized evidence allowed us to list health care measures to prevent medical adhesive-related skin injuries. The professional must know how to identify the skin injuries associated with medical adhesives and the main strategies for their prevention.

Consideraciones Finales: las evidencias sintetizadas posibilitaron enumerar cuidados para prevención de lesiones cutáneas relacionadas con adhesivos médicos. Es importante que el profesional sepa identificar tanto las lesiones cutáneas relacionadas con adhesivos médicos cuanto las principales estrategias para prevención de estas.

RESUMO Objetivo: refletir acerca da utilização da telessaúde no contexto da pandemia da COVID-19 para as mulheres no período gravídico-puerperal. Método: trata-se de uma reflexão alicerçada na literatura com abordagem sobre a telessaúde como ferramenta para os atendimentos virtuais em saúde, no contexto da pandemia. Para tanto, esta reflexão faz uma abordagem sobre a utilização da telessaúde demonstrando as particularidades da adoção de atendimentos remotos, com a sua aplicabilidade e suas limitações, considerando os aspectos socioeconômicos e raciais que estão intrinsecamente associados às questões de saúde pública. Resultados: a telessaúde é uma ferramenta para o enfrentamento à COVID-19, pois minimiza a exposição de gestantes e puérperas durante as consultas nas unidades de saúde. No Brasil, observa-se que esta estratégia ainda é incipiente. Isso se intensifica na medida em que relacionamos as questões socioeconômicas e raciais às taxas de mortalidade materna, sobretudo entre mulheres pretas e pardas. Conclusão: esta reflexão demonstra que a telessaúde é uma importante estratégia de cuidado para as mulheres no período gravídico-puerperal e, nessa perspectiva, necessita ter sua viabilidade reconhecida e valorizada para que sejam criadas políticas públicas que garantam o acesso aos recursos digitais, além da necessidade de investimentos em capacitações de profissionais e aprofundamento desta temática pela comunidade acadêmica. Tais iniciativas promoverão acesso ampliado ao atendimento virtual para as mulheres, para além das questões socioeconômicas e raciais.

ABSTRACT Objective: to reflect on the use of Telehealth in the context of the COVID-19 pandemic for women in the pregnancy-puerperal period. Method: this is a reflection based on the literature with an approach on Telehealth as a tool for virtual health care in the pandemic context. Therefore, this reflection addresses the use of Telehealth, showing the particularities of adopting remote appointments, with their applicability and limitations, considering the socioeconomic and racial aspects that are intrinsically associated with public health issues. Results: Telehealth is a tool for coping with COVID-19, as it minimizes the exposure of pregnant and puerperal women during consultations in the health units. In Brazil, it is observed that this strategy is still incipient. This is intensified as we relate the socioeconomic and racial issues to the maternal mortality rates, especially among black- and brown-skinned women. Conclusion: this reflection shows that Telehealth is an important care strategy for women in the pregnancy-puerperal period and, in this perspective, it needs to have its viability recognized and valued so that public policies are created that guarantee access to the digital resources, in addition to the need for investments in training of the professionals and deepening of this theme by the academic community. Such initiatives will promote expanded access to virtual care for women, in addition to the socioeconomic and racial issues.

RESUMEN Objetivo: reflexionar sobre la utilización de la telesalud en el contexto de la pandemia de COVID-19 para mujeres que transitan el período grávido puerperal. Método: reflexión basada en la literatura con enfoque sobre la telesalud como herramienta para la atención virtual de la salud en el contexto de la pandemia. Para ello, aborda el uso de la telesalud y demuestra las particularidades de la adopción de la atención remota, su aplicabilidad y sus limitaciones, considerando los aspectos socioeconómicos y raciales intrínsecamente asociados a la salud pública. Resultados: la telesalud es una herramienta para el afrontamiento del COVID-19, dado que minimiza la exposición de embarazadas y puérperas en consultas en los centros de salud. En Brasil, se observa que esta estrategia es aún incipiente. Esto se intensifica a medida que se relacionan las cuestiones socioeconómicas y raciales con las tasas de mortalidad materna, especialmente entre las mujeres negras y morenas. Conclusión: esta reflexión demuestra que la telesalud es una estrategia de atención importante para las mujeres durante el período grávido puerperal y, en esta perspectiva, requiere que se reconozca y valore su viabilidad para que se creen políticas públicas que garantice el acceso a los recursos digitales, además de advertir la necesidad de realizar inversiones en capacitación profesional y ahondar el abordaje de esta temática desde la comunidad académica. Tales iniciativas promoverán un mayor acceso a la atención virtual para las mujeres, más allá de las cuestiones socioeconómicas y raciales.

Abstract Verruciform xanthoma (VX) is a rare benign lesion of unknown etiology, with a rough or papillary aspect, painless, sessile, well-defined, most lesions do not exceed 2 cm in their largest diameter, the degree of keratinization of the surface influences color, varying white to red, affecting mainly the gingiva and alveolar mucosa, and can also be seen in skin and genital. Herein, we present a report a clinical case of oral verruciform xanthoma in the buccal mucosa associated with the lichen planus lesion, as well as the morphological and immunohistochemical characteristics of the lesion. The clinical diagnostic hypothesis of oral lichen planus of the white reticular lesions on the buccal mucosa and on the tongue was confirmed by histopathology before a subepithelial connective tissue exhibiting intense inflammatory infiltrate in a predominantly lymphocytic band. In contrast, the hypothesis of the verrucous lesion in the left buccal mucosa was leukoplakia, with histopathological evidence showing exophytic and digitiform proliferations with parakeratin plugs between the papillary projections. Subepithelial connective tissue was characterized by macrophages with foamy cytoplasm (xanthoma cells). An immunohistochemical examination was performed, showing positivity for CD68, a macrophage marker, in addition to testing by Schiff's periodic acid (PAS) with diastasis, which was detected the presence of lipids inside these macrophages. The patient is free of recurrences of verruciform xanthoma and is being monitored due to the presence of lesions of oral lichen planus.
