Objetivou-se identificar as percepções de cuidadores informais sobre motivações, necessidades e benefícios do cuidado ao idoso dependente. Estudo qualitativo realizado com 10 cuidadoras informais portuguesas, a partir de um instrumento com questões sobre o cuidado prestado ao idoso e suas percepções sobre o exercício dessa função. Os resultados revelaram como motivações para o cuidado: relação de proximidade e confiança, dever de cuidar, familiar mais disponível, proximidade da residência, inexistência de vagas e elevado custo das instituições de acolhimento e desejo do idoso permanecer na sua habitação. Os principais cuidados aos idosos são: hidratação, higiene, alimentação, administração terapêutica, companhia, apoio emocional, conforto, entretenimento, promoção da autonomia e dignidade. ABSTRACT The COVID-19 pandemic required the reorganization of PHC Units (UBS) to ensure greater laboratory diagnostic capacity timely, which demanded trained health professionals, availability of inputs/materials, and adequate sample management strategies at the Central Laboratory (LACEN). In 2020-2021, an analytical cross-sectional census study evaluated the UBS structure in the Federal District (DF). Data were collected remotely through structured telephone interviews and a self-completed questionnaire. Statistical analysis was carried out in Software R, comparing UBS-Sentinel with UBS-Traditional units. Nurse training in rapid testing or swab sample collection was almost universal (> 99%) and high among nursing technicians (70%). On the other hand, only 9% of doctors received any training. A defined flow was registered to forward samples to LACEN in 89% of UBS to diagnose SARS-CoV-2. The deadlines for returning laboratory results were met in 70% of cases. Inputs, materials, and equipment were available in sufficient amounts, especially at UBS-Sentinel units. In these UBS, 63% of the teams knew the MA-LACEN-0007 collection manual, compared to 35% at UBS-Traditional units (p < 0.001). Despite the challenges, the DF showed a satisfactory response capacity regarding the COVID-19 laboratory diagnosis.

RESUMO A pandemia de covid-19 exigiu reorganização das Unidades Básicas de Saúde (UBS) para garantir maior capacidade diagnóstica laboratorial em tempo oportuno, o que requereu profissionais de saúde capacitados, disponibilidade de insumos/materiais e estratégias adequadas de manejo das amostras no Laboratório Central (Lacen). Em 2020-2021, a estrutura das UBS do Distrito Federal (DF) foi avaliada por meio de estudo transversal analítico, censitário. A coleta de dados, remota, ocorreu por entrevista telefônica estruturada e questionário de autopreenchimento. Fez-se análise estatística no software R, comparando UBS-Sentinela com UBS-Tradicional. A capacitação no teste rápido e/ou na coleta da amostra por swab entre enfermeiros foi quase universal (> 99%), e entre técnicos de enfermagem, foi alta (70%); por outro lado somente 9% dos médicos receberam alguma capacitação. Registrou-se fluxo definido para encaminhar amostras para o Lacen em 89% das UBS, visando diagnosticar o Sars-CoV-2; os prazos de retorno dos resultados laboratoriais foram cumpridos em 70% dos casos. Insumos, materiais e equipamentos estavam disponíveis em quantidades suficientes, sobretudo nas UBS-Sentinela. Nestas, 63% das equipes conheciam o manual de coleta MA-LACEN-0007, comparado com 35% das equipes na UBS-Tradicional (p < 0,001). O Programa Mais Médicos atingiu o auge do provimento emergencial em 2016, com 18.088 médicos em 4.509 municípios, uma das maiores intervenções desse tipo no mundo. Realizou-se uma revisão sistemática para reunir evidências dos efeitos do Programa na Atenção Primária à Saúde e impactos na saúde da população atendida. Extraíram-se 570 estudos, e a seleção final incluiu 32 artigos. Quanto aos efeitos do Programa, verificaram-se rápida expansão na cobertura e melhoria na integralidade e humanização da Atenção Primária à Saúde e impacto significativo nas internações por condições sensíveis a essa atenção, reduzindo aproximadamente 23 mil internações em três anos e poupando R$ 30 milhões para o Sistema Único de Saúde. Abstract The distribution of doctors in Brazil is marked by inequalities, impairing integral and universal access to health, the basis of the Unified Health System. The Mais Médicos Program reached the peak of emergency provision in 2016, with 18,088 doctors in 4,509 municipalities, one of the most significant interventions of its kind in the world. A systematic review was conducted to gather evidence of the Program’s effects on Primary Health Care and impacts on the health of the population served. 570 studies were extracted, and the final selection included 32 articles. Regarding the effects of the Program, there was a rapid expansion in coverage and improvement in the comprehensiveness and humanization of Primary Health Care and a significant impact on hospitalizations due to conditions sensitive to this care, reducing approximately 23,000 hospitalizations in three years and saving R$30 million for the Unified Health System. Points that were detrimental to the potential impact of the Program were also identified: deviations in focus, changes in priority criteria, and improper replacement of doctors already hired by others from the Mais Médicos Program, in addition to the rupture caused by the departure of 8,500 Cuban doctors in November 2018. El Programa Más Médicos ha alcanzado el pico de prestación de emergencia en 2016, con 18.088 médicos en 4.509 municipios, una de las mayores intervenciones de este tipo en el mundo. Se llevó a cabo una revisión sistemática para reunir pruebas de los efectos del Programa de Atención Primaria y su repercusión en la salud de la población beneficiada. Se extrajeron 570 estudios y la selección final incluyó 32 artículos. Objective: To determine clinical outcomes and mortality risk factors related to mental disorders in a cohort of hospitalized patients with Covid-19 in Brazil. Methods: This retrospective cohort study used a Brazilian database called the Sistema de Vigilância Epidemiológica da Gripe (Influenza Epidemiological Surveillance System) to analyze patients aged ≥ 18 years who were hospitalized with Covid-19 between 2020 and 2022. The exposure of interest was mental disorders (anxiety, depression, schizophrenia, and bipolar disorder) identified through self-report. The primary outcome was in-hospital mortality. Covariates included demographic and clinical characteristics. Descriptive statistics, t-tests, chi-square tests, and binary logistic regression were used to analyze the data. Results: A cohort of 2,124,285 patients was included in the analysis, with 23,246 individuals (1.1%) self-reporting mental disorders, of which depression was the most prevalent (52.3%). The mortality rate of patients with mental disorders was 30.8%. Age, sex, region, dyspnea, low oxygen saturation, and comorbidities were associated with a higher mortality risk, as was schizophrenia (adjusted OR: 1.68; 95%CI 1.54-1.81). Conclusions: Individuals with schizophrenia had a greater likelihood of Covid-19-related death than those without mental health conditions. These findings underscore the significant effect of serious mental disorders on Covid-19 mortality. The publication Cardiovascular Statistics – Brazil aims to provide an annual compilation of data and research on the epidemiology of CVDs in Brazil.&nbsp; The report integrates official statistics from the Brazilian Ministry of Health and other governmental entities alongside data from the GBD project, coordinated by the IHME at the University of Washington. Additionally, it incorporates data derived from various sources and scientific studies, including cohorts and registries, that relate to CVDs and their associated risk factors. This publication is intended for a wide range of individuals, including researchers, clinicians, patients, healthcare policymakers, media professionals, the general public, and other interested parties seeking extensive national data about heart disease and stroke.&nbsp; Volunteer researchers from various Brazilian universities and research institutions carry out the project. The group is led by a five-member steering committee (ALPR, CAP, DCM, GMMO, and LCCB). The Brazilian Society of Cardiology fully supports this initiative, and the project receives collaborative support from the GBD Brazil Network and an International Committee (GAR, PP, and TAG) from both the IHME/University of Washington (GAR) and the World Heart Federation (PP and TAG).&nbsp;

Regarding the Program’s effects, there was a rapid expansion in primary healthcare coverage, an improvement in comprehensiveness and humanization of healthcare, as well as significant impact on hospitalizations for primary healthcare sensitive conditions, which resulted in an approximate reduction of 23 thousand hospitalizations throughout three years, saving R$30 million for SUS. Critical points that undermined the Program’s potential impact were also identified: focus deviations; changes in priority criteria and undue replacement of hired doctors by PMM physicians, in addition to the disruption caused by the departure of 8,500 Cuban doctors in November 2018. It is estimated that the relaunch of the Mais Médicos Program, in 2023, especially based on existing evidence, will promote the continued progress of the Program.

Quanto aos efeitos do Programa, verificaram-se rápida expansão na cobertura da APS, melhoria na integralidade e humanização da atenção à saúde e impacto significativo nas internações por condições sensíveis à APS, gerando aproximadamente 23 mil internações ao longo de três anos, economizando R$ 30 milhões para o SUS. identificaram-se também pontos críticos que prejudicaram o potencial de impacto do Programa: desvios na focalização; mudanças nos critérios de prioridade e substituição indevida de médicos já contratados por outros do PMM, além da ruptura causada pela saída de 8.500 médicos cubanos em novembro de 2018. Estima-se que o relançamento do Programa Mais Médicos em 2023, principalmente a partir de evidências já existentes, promoverá a continuidade dos progressos do Programa.

Introdução: O programa de Agentes Comunitários de Saúde (ACS) foi institucionalizado em 1991 pelo Ministério da Saúde brasileiro. Em 2003, foi organizado o perfil de competências desses profissionais. Objetivos: Verificar os conhecimentos e perceções dos Agentes Comunitários de Saúde sobre a atuação na atenção à saúde do idoso. Métodos: Estudo descritivo, exploratório e com abordagem mista, realizado no município de Picos, Piauí, Brasil, com a participação de 98 ACS. Os ACS foram identificados como a população-alvo deste estudo por serem profissionais estratégicos na Atenção Básica e com atuação na promoção da saúde em todo o ciclo de vida. Para coleta de dados utilizou-se um questionário na plataforma Google Forms com questões sobre o perfil socioeconômico e a perceção dos Agentes Comunitários de Saúde sobre o processo de formação para trabalhar na saúde do idoso. Resultados: Após a análise de conteúdo das respostas dadas pelos participantes, ressaltaram duas temáticas: dificuldades na realização das visitas, nomeadamente problemas com o autocuidado, negligência e abandono por parte da família e do cuidador, solidão, diminuta comunicação, carência emocional e condições socioeconômicas precárias; e medidas necessárias para melhorar a atuação dos ACS na atenção à saúde do idoso como formação para cuidar do idoso, capacitação de familiares e de cuidadores, melhoria das condições de trabalho, facilitação do acesso do idoso ao serviço, criação de programa e espaço para o idoso, ampliação de visitas, entre outros. Conclusões: O ACS desempenha um papel importante nos cuidados ao idoso uma vez que conhece as reais condições do contexto onde o idoso se encontra inserido e respetivas necessidades nos domínios pessoal, familiar, social e comunitário, mas que almeja formação, capacitação e reconhecimento pelo trabalho que presta.

Introduction: The Community Health Agents (ACS) program was institutionalized in 1991 by the Brazilian Ministry of Health. In 2003, the skills profile of these professionals was organized. Goals: Verify the knowledge and perceptions of the Community Health Agents about the action in the health care of the elderly. Methods: Descriptive, exploratory study with a mixed approach carried out in the municipality of Picos, Piauí, Brazil, with the participation of 98 CHAs. The CHAs were identified as the target population of this study because they are strategic professionals in Primary Care and active in health promotion throughout the life cycle. For data collection, a questionnaire on the Google Forms platform was used with questions about the socioeconomic profile and the perception of Community Health Agents about the training process to work in the health of the elderly. Results: After analyzing the content of the responses given by the participants, two themes were highlighted: difficulties in carrying out the visits, namely problems with self-care, negligence and abandonment by the family and caregiver, loneliness, poor communication, emotional deprivation and precarious socioeconomic conditions; and necessary measures to improve the performance of the ACS in the health care of the elderly, such as training to care for the elderly, training of family members and caregivers, improvement of working conditions, facilitating access of the elderly to the service, creation of a program and space for the elderly, expansion of visits, among others. Conclusions: The CHA plays an important role in caring for the elderly, as they are aware of the real conditions of the context in which the elderly person is inserted and their needs in the personal, family, social and community domains, but who aim for training, qualification and recognition for the work they provide. format.

Abstract Background Dental anesthetic management in implantable cardioverter defibrillator (ICD) recipients with cardiac channelopathies (CCh) can be challenging due to the potential risk of life-threatening arrhythmias and appropriate ICD therapies during procedural time. Objectives The present study assessed the hypothesis that the use of local dental anesthesia with 2% lidocaine with 1:100,000 epinephrine or without a vasoconstrictor can be safe in selected ICD and CCh patients, not resulting in life-threatening events (LTE). Methods Restorative dental treatment under local dental anesthesia was made in two sessions, with a wash-out period of 7 days (cross-over trial), conducting with a 28h - Holter monitoring, and 12-lead electrocardiography, digital sphygmomanometry, and anxiety scale assessments in 3 time periods. Statistical analysis carried out the paired Student’s t test and the Wilcoxon signed-rank test. In all cases, a significance level of 5% was adopted. All patients were in stable condition with no recent events before dental care. Results Twenty-four consecutive procedures were performed in 12 patients (9 women, 3 men) with CCh and ICD: 7 (58.3%) had long QT syndrome (LQTS), 4 (33.3%) Brugada syndrome (BrS), and 1 (8.3%) Catecholaminergic polymorphic ventricular tachycardia (CPVT). Holter analysis showed no increased heart rate (HR) or sustained arrhythmias. Blood pressure (BP), electrocardiographic changes and anxiety measurement showed no statistically significant differences. No LTE occurred during dental treatment, regardless of the type of anesthesia. Conclusion Lidocaine administration, with or without epinephrine, can be safely used in selected CCh-ICD patients without LTE. ABSTRACT Objective: To analyze the use of Information and Communication Technologies and the interaction in a health education group with women on WhatsApp. Methodology: Descriptive-exploratory, qualitative research with mothers assisted in two Basic Health Units in Sinop - MT, part of a University Extension Project that used three health education strategies (“G-Day”, “Health Tips”, and “Educational Video”) and project team members. Data were collected between October and December 2021 from messages in the WhatsApp group and submitted to Thematic Content Analysis. Results: Two categories were identified, showing the information provided by the team and specific information requested by the mothers; the doubts expressed by the mothers mobilized the communication-interaction among the participants and little to no interaction was provoked by the health information contained in the educational strategies used. Abstract Degenerative aortic stenosis is currently a public health problem. Affecting the elderly population, this pathology has been showing an increasing prevalence as a direct result of the population aging. In this context, women have a greater life expectancy, corresponding to most of the population with degenerative aortic stenosis. Specific characteristics of this pathology in females are present in the diagnosis, pathophysiology, anatomical aspects, imaging and in therapeutic approach. Women present a more severe disease with less valve calcification than men, more concentric ventricular remodeling, higher transvalvular gradients, and less myocardial fibrosis. Less evident symptoms mean that these patients are referred later for surgical or percutaneous therapeutic treatment. The greater comorbidity presented by females and possibly due to the smaller body surface, bring specific aspects that affect the surgery results, leading to higher mortality rates and, more often, the prosthesis-patient mismatch. Percutaneous valve implantation is a good alternative, with better results in females, when compared to surgery, both in the treatment of native valves and in the treatment of a previously implanted bioprosthesis’ dysfunction. ABSTRACT. This study aimed to produce mechanically separated meat (MSM) from tilapia obtained at the Teresina fish market in the state of Piauí, Brazil. Waste production and disposal and yields were estimated, and physico-chemical and microbiological MSM qualities were determined. A questionnaire was applied to the fish market sellers, followed by tilapia carcass sampling for MSM production and assessments concerning yields, microbiological (Escherichia coli and Staphylococcus coagulase positive counts and the detection of Salmonella spp.) and physicochemical (water activity, pH, reaction to hydrogen sulfide and ammonia) quality and proximate composition (moisture, ash, proteins, lipids, carbohydrates and total energy value). The questionnaire results indicated that tilapia is highly in demand and thats processing leads to significant waste. The yield analysis indicated that tilapia MSM presents adequate residual meat extraction rates from tilapia carcasses. All microbiological analyses were in compliance with Brazilian standards. Physicochemical results were as follows: protein (14.9 to 16.1%), lipids (16.6 to 22.1%), ash (1.3 to 2.8%), moisture (60.6 to 64 .1%), carbohydrates (0.6 to 1.5%) and calories (216.5 to 263.1 kcal 100-1). It is, therefore, clear that MSM constitutes a safe and nutritious foodstudd and comprises a viable alternative for the development of co-products. Resumo Objetivou-se determinar a resistência de nematoides gastrintestinais aos anti-helmínticos levamisol, ivermectina e albendazol em caprinos e ovinos da região metropolitana da Ilha de São Luís, Maranhão, Brasil. Foram coletadas amostras de fezes de 150 animais de quatro propriedades diferentes, sendo 2 com caprinos e 2 com ovinos, e aleatoriamente distribuídos de três a quatro grupos de 10 animais: Grupo I: grupo controle, sem tratamento. Grupo II: tratado com anti-helmíntico à base de ivermectina, administrado oralmente na dose de 200 mcg/kg; Grupo III: tratado com anti-helmíntico à base de levamisol, administrado oralmente na dose de 7,5mg/kg e Grupo IV: tratado com anti-helmíntico à base de albendazol administrado oralmente na dose de 3mg/kg. Amostras de fezes foram colhidas da ampola retal um dia antes do tratamento e 10 dias após o tratamento anti-helmíntico. Foram feitos exames coproparasitológicos individuais, pela técnica de McMaster modificada, no Laboratório de Sanidade Animal, do Instituto Federal do Maranhão (IFMA), Campus São Luís-Maracanã. A ivermectina, na espécie caprina, mostrou eficácia de 14,28%, enquanato para ovina, 13,6 e 52,2%. Considerando o levamisol na espécie caprina, não apresentou eficácia contra os nematoides gastrintestinais, enquanto para ovinos, apresentou eficácia de 79,4%. Já o albendazol, apresentou eficácia de 59,8% para caprinos, e 3,43% para ovinos. Os nematoides gastrintestinais demonstraram resistência múltipla (RAM), visto que a percentagem de redução da contagem de ovos foi inferior a 95% e o limite inferior do intervalo de confiança menor do que 90%. Abstract The aim of this study was to determine the resistance of gastrointestinal nematodes in goats and sheep to the anthelmintic drugs levamisole, ivermectin, and albendazole in the metropolitan region of São Luís Island, Maranhão, Brazil. Fecal samples were collected from 150 animals across four different farms; two farms had goats, and the other two had sheep. The samples were then randomly divided into three to four groups of 10 animals: Group I: control, without treatment; Group II: ivermectin treatment; Group III: levamisole treatment; and Group IV: albendazole treatment. Stool samples were collected from the rectal ampulla one day before treatment and 10 days after anthelmintic treatment. Individual coproparasitological examinations were performed using the modified McMaster technique at the Animal Health Laboratory of the Federal Institute of Maranhão, Campus São Luís-Maracanã.
