RESUMO Objetivo Analisar os gastos públicos em saúde destinados aos serviços ambulatoriais em Fonoaudiologia nas cinco Regiões Federativas do Brasil. Métodos Trata-se de um estudo ecológico realizado por meio da busca de dados secundários disponíveis em uma plataforma virtual de domínio público, DATASUS (Departamento de Informática do Sistema Único de Saúde). Os dados coletados referiram-se aos valores aprovados para os procedimentos da Fonoaudiologia no período de 2009 a 2018 nas cinco Regiões Federativas. Adotou-se análise descritiva dos dados e exposição dos resultados em valores absolutos, relativos e taxas de crescimento. Resultados Verificou-se que o investimento médio em reais (R$), por ano, nos serviços de Fonoaudiologia no Brasil foi de, aproximadamente, R$ 223.952.639.232,00; sendo 47,2% dos recursos destinados à Região Sudeste. Dentre as grandes áreas de especialização, a Audiologia foi responsável por 95,4% do investimento, seguida pelas áreas da Linguagem (4,0%), Motricidade Orofacial (0,5%) e Voz (0,1%). Conclusão Os serviços fonoaudiológicos demandam considerável parcela dos recursos públicos, sendo a área da Audiologia responsável por quase metade desses gastos, seguida pelas grandes áreas da Linguagem, Motricidade Orofacial e Voz.

ABSTRACT Purpose To analyze public spending on health services to outpatient services in Speech Therapy in the five federative regions of Brazil. Methods This is an ecological study carried out through the search for secondary data available on a public domain virtual platform, DATASUS. The collected data refer to the values approved for the Speech Therapy procedures in the period from 2009 to 2018 in the five federative regions. Descriptive analysis of data and exposure of results in absolute and relative values and growth rates were adopted. Results It was found that the average investment in reais, per year, in speech therapy services in Brazil was approximately R$ 223,952,639,232.00, with 47.2% of the resources destined for the Southeast region. Among the major areas of specialization, Audiology accounted for 95.4% of the investment, followed by Language (4.0%), Orofacial Motricity (0.5%) and Voice (0.1%). Conclusion The findings indicate that speech therapy services demand a considerable portion of public resources, with the area of Audiology being responsible for almost half of these expenses, followed by the large area of Language, Orofacial Motricity and Voice.

Abstract The aim of this study was to evaluate whether the modification in the silver component is capable of providing GuttaFlow 2 with antibacterial activity against Enterococcus faecalis compared with epoxy resin-based (AH Plus) and zinc oxide and eugenol-based (Endofill) sealers. The antibacterial activity was evaluated using a reference strain of E. faecalis (ATCC 29212). Freshly mixed sealers were subjected to the agar diffusion test (ADT), while the direct contact test (DCT) was performed after materials setting. ADT results were obtained through measurements, in millimeters, of the inhibition zones promoted by the materials, using a digital caliper. In DCT, values of CFU/mL promoted by the three sealers were compared in three experimental periods (1 min, 1 h, and 24 h). The data were analyzed using Kruskal-Wallis and Dunn post-hoc tests (p < 0.05). In both ADT and DCT, GuttaFlow 2 presented no effect against E. faecalis, while Endofill and AH Plus showed similar inhibition zones. Endofill was the only material capable of reducing bacterial growth in DCT. In conclusion, modifications in the silver particle of GuttaFlow 2 did not result in a sealer with antibacterial effect against E. faecalis.

Uma das formas atuais para se avaliar o grau de hipnose e depressão do sistema nervoso central durante a anestesia é o índice biespectral (BIS), que fornece um valor de 0 a 100. Objetivou-se avaliar as alterações do BIS de cães durante a estimulação elétrica e mecânica supra-máxima e sob diferentes concentrações de isofluorano, a fim de verificar se existe uma relação entre este índice e a resposta motora frente à nocicepção. Utilizaram-se sete cães da raça Beagle, dois machos e cinco fêmeas, anestesiados com isofluorano nas concentrações de 2,0; 1,8; 1,6; 1,4 e 1,2%. Os diferentes tipos de estímulo foram utilizados em procedimentos anestésicos diferentes nos mesmos animais, com intervalo mínimo de 15 dias. Os valores de BIS foram anotados por 60 segundos em cada avaliação, derivando médias antes (AE) e depois (DE) de cada estímulo. Os resultados foram analisados por meio de teste t pareado ou Wilcoxon pareado (P<0,05). Houve diferença significativa entre os valores AE e DE em todas as concentrações de isofluorano, exceto 2,0% no estímulo mecânico. Também foi significativa entre a subtração desses valores, quando comparados entre estímulos, em 1,8 e 2,0%. Conclui-se que a resposta frente a estímulos supra-máximos modifica os valores de BIS em cães por meio da superficialização do plano anestésico. Sugere-se o emprego do modelo mecânico para evitar possíveis interferências de corrente elétrica e perda da confiabilidade dos dados.

One of the current methods to assess hypnosis and central depression during anesthesia is the bispectral index (BIS), which generates a number from 0 to 100. The purpose of the present study was to evaluate the changes in BIS during electrical or mechanical supra-maximal stimulation in order to investigate whether there is a relation between this index and motor response to nociception. Seven beagle dogs were included, two males and five females, which were anesthetized with isoflurane at 2.0, 1.8, 1.6, 1.4 and 1.2%. Different types of stimuli were employed at different procedures in the same animals with at least 15 days of interval. The BIS values were recorded during 60 seconds each time and mean values were obtained before (AE) and after (DE) the stimulus. Results were compared through paired t test or Wilcoxon's paired test (P<0.05). Significant differences were found between AE and DE values in almost every isoflurane concentration, as well as between the subtraction of these values, when compared between types of stimuli at 1.8 and 2.0%. In conclusion, the response to supra-maximal stimulation does change BIS values in dogs by decreasing the depth of anesthesia. The use of mechanical stimulation is suggested in order to avoid possible interferences of the electrical current and loss of data reliability.

OBJETIVO: Identificar os fatores de risco para morte em pacientes com asma grave. MÉTODOS: Estudo caso-controle aninhado a uma coorte de pacientes acompanhados no Ambulatório Central de Referência do Programa para o Controle da Asma na Bahia, em Salvador (BA). No período entre dezembro de 2002 e dezembro de 2010, foram selecionados todos os pacientes com asma grave que foram a óbito e pacientes asmáticos graves vivos como controles na relação 1:4. As informações foram coletadas nos prontuários do serviço e complementadas por meio de visitas domiciliares e atestados de óbitos. RESULTADOS: Foram selecionados 58 óbitos e 232 controles. Os óbitos, na sua maioria, foram atribuídos a causas respiratórias e ocorreram dentro de uma unidade de saúde. Idade avançada, inatividade laboral, presença de rinite, sintomas de doença do refluxo gastroesofágico, tempo prolongado de doença e obstrução ao fluxo aéreo persistente foram aspectos comuns em ambos os grupos. A análise multivariada mostrou que o gênero masculino, VEF1 pré-broncodilatador < 60% do previsto e a ausência de controle dos sintomas da asma foram fatores de risco significativamente e independentemente associados à mortalidade nessa amostra de asmáticos graves. CONCLUSÕES: Nesta coorte ambulatorial de pacientes com asma grave, os óbitos ocorreram predominantemente por causas respiratórias em unidades de saúde. A falta de controle da asma e o gênero masculino foram os fatores de risco para óbito.

OBJECTIVE: To identify risk factors for death among patients with severe asthma. METHODS: This was a nested case-control study. Among the patients with severe asthma treated between December of 2002 and December of 2010 at the Central Referral Outpatient Clinic of the Bahia State Asthma Control Program, in the city of Salvador, Brazil, we selected all those who died, as well as selecting other patients with severe asthma to be used as controls (at a ratio of 1:4). Data were collected from the medical charts of the patients, home visit reports, and death certificates. RESULTS: We selected 58 cases of deaths and 232 control cases. Most of the deaths were attributed to respiratory causes and occurred within a health care facility. Advanced age, unemployment, rhinitis, symptoms of gastroesophageal reflux disease, long-standing asthma, and persistent airflow obstruction were common features in both groups. Multivariate analysis showed that male gender, FEV1 pre-bronchodilator < 60% of predicted, and the lack of control of asthma symptoms were significantly and independently associated with mortality in this sample of patients with severe asthma. CONCLUSIONS: In this cohort of outpatients with severe asthma, the deaths occurred predominantly due to respiratory causes and within a health care facility. Lack of asthma control and male gender were risk factors for mortality.

A enzima purina nucleosídeo fosforilase do parasita Schistosoma mansoni (SmPNP) é um alvo molecular atrativo para o desenvolvimento de candidatos a novos fármacos para o tratamento da esquistossomose, doença tropical negligenciada que afeta mais de 200 milhões de pessoas em todo mundo. No presente trabalho, estudos de cinética enzimática foram conduzidos para a determinação da potência e do mecanismo de inibição de uma série de inibidores da enzima SmPNP. Além das investigações bioquímicas, estudos cristalográficos e de modelagem molecular revelaram importantes bases moleculares para a afinidade de ligação frente à enzima alvo, levando ao desenvolvimento de relações entre a estrutura e atividade (SAR).

The enzyme purine nucleoside phosphorylase from Schistosoma mansoni (SmPNP) is an attractive molecular target for the development of novel drugs against schistosomiasis, a neglected tropical disease that affects about 200 million people worldwide. In the present work, enzyme kinetic studies were carried out in order to determine the potency and mechanism of inhibition of a series of SmPNP inhibitors. In addition to the biochemical investigations, crystallographic and molecular modeling studies revealed important molecular features for binding affinity towards the target enzyme, leading to the development of structure-activity relationships (SAR).

OBJETIVO: Avaliar o efeito da veloplastia intravelar sobre o fechamento velofaríngeo de pacientes com fissura de palato reparada e insuficiência velofaríngea (IVF), por meio de avaliação aerodinâmica (técnica fluxo-pressão). MÉTODOS: Quarenta e sete indivíduos de ambos os gêneros, com idade entre seis e 48 anos, que apresentavam fissura de palato±lábio reparada e IVF, submetidos à veloplastia intravelar. Os pacientes foram avaliados, por meio da técnica fluxo-pressão, aferindo a medida da área do orifício velofaríngeo antes e, em média, 12 meses após a cirurgia, durante a produção do fonema /p/ inserido na palavra "rampa". O fechamento velofaríngeo foi classificado como adequado (0 a 0,049 cm²), marginal (0,050 a 0,199 cm²) ou inadequado (>0,200 cm²). RESULTADOS: Após a cirurgia, verificou-se melhora considerável no grau de fechamento velofaríngeo em 66% dos casos. Do total de pacientes analisados, 47% passaram a apresentar fechamento adequado, 23%, fechamento marginal e 30% permaneceram com fechamento inadequado. CONCLUSÃO: Estes resultados mostraram que a veloplastia intravelar levou à melhora do fechamento velofaríngeo na maioria dos pacientes analisados repercutindo na redução dos sintomas de fala da IVF.

PURPOSE: To evaluate the effect of intravelar veloplasty on velopharyngeal closure of patients with repaired cleft palate and velopharyngeal insufficiency (VPI), using the pressure-flow technique. METHODS: Forty-seven individuals of both genders, with ages between six and 48 years, with repaired cleft palate±lip and residual VPI, submitted to intravelar veloplasty. The pressure-flow technique was used to measure the velopharyngeal orifice area during the production of the phoneme /p/ in the word "rampa", before and, in average, 12 months after surgery. Velopharyngeal closure was classified as adequate (0 to 0.049 cm²), borderline (0.050 to 0.199 cm²) or inadequate (>0.200 cm²). RESULTS: After surgery, there was considerable improvement in the velopharyngeal closure degree in 66% of the cases. From the patients analyzed, 47% presented adequate closure, 23%, borderline closure and 30% remained with inadequate closure. CONCLUSION: These results showed that intravelar veloplasty led to improvement in velopharyngeal closure in most of the patients studied, consequently decreasing the speech symptoms of VPI.

CONTEXTO: O lítio é utilizado como medicamento de escolha para o tratamento dos transtornos de humor bipolar. Mostra-se eficaz em reduzir as crises maníacas e depressivas do transtorno do humor bipolar, além de exercer efeito anti-suicida. No entanto, a efetividade do lítio é comprometida pela baixa adesão ao tratamento, por sua vez influenciada por características dos pacientes e pelas características farmacológicas do lítio, que dificultam seu uso. OBJETIVO: Revisar alguns aspectos farmacológicos clínicos do lítio e sua importância para a monitorização terapêutica do lítio, por meio de dosagens e de escalas de auto-relato em português para avaliar a falta de adesão ao lítio. MÉTODO: Foi realizada revisão da literatura básica e clínica sobre lítio disponível em artigos científicos em língua inglesa e portuguesa acessados pelo PUBMED e SCIELO, usando as palavras "lítio", "monitoramento", "cooperação" OU "adesão". RESULTADOS: O lítio é um fármaco de muito baixo índice terapêutico e farmacocinética muito individual, fato que justifica a necessidade de monitorização terapêutica intensiva, por meio de determinação da concentração sérica e hemática. As taxas de adesão ao lítio em pacientes brasileiros que freqüentam os ambulatórios específicos para transtornos bipolares estão entre 60 e 85%, sendo mais elevadas que a de pacientes tratados sem acompanhamento psicoeducativo (adesão em torno de 46%). Os pacientes não aderentes apresentavam um menor nível de conhecimento sobre o lítio e sobre a doença, e mais atitudes negativas dos pacientes ao uso de medicamentos, além de receio dos efeitos adversos. CONCLUSÃO: A monitorização laboratorial e por escalas auxilia na adesão ao tratamento, por auxiliar na individualização terapêutica e ajuste de doses de lítio, além de detectar as interações medicamentosas e as atitudes dos pacientes em relação a sua doença e tratamentos.

BACKGROUND: Lithium has been used as the treatment of choice for bipolar disorder. It is efficient in reducing depressive and maniac episodes and has an anti-suicidal effect. However, the effectiveness of lithium is decreased by low compliance to treatment, influenced by patient's characteristics and the pharmacological properties of lithium. OBJECTIVE: To review some of the clinical pharmacological aspects of lithium and its importance for the therapeutic monitoring of lithium, through concentration measurements and through translated to Portuguese self-report scales of non-compliance to lithium treatment. METHOD: Relevant basic and clinical scientific literature available in English and in Portuguese were searched in PUBMED and SCIELO with the words "lithium", "monitoring", "compliance" OR "adherence". RESULTS: Lithium is a drug with very low therapeutic index and difficult individual pharmacokinetic properties, leading to the necessity of intensive therapeutic monitoring, through lithium measurements in serum and in red blood cells. The rates of compliance to lithium prescription by Brazilian bipolar patients who attend outpatient treatment are between 60 and 85%, being higher than those of patients not receiving psychoeducational programs (compliance around 46%). The non-compliant patients present lower knowledge about lithium and their disorder and more negative attitudes toward the use of medication, besides the fear of side-effects. CONCLUSION: Biological monitoring helps in treatment adherence, through the improvement of therapeutic individualization and lithium dose adjustments, besides being helpful in the detection of drugs interactions.
