ABSTRACT We conducted a systematic review and meta-analysis of randomized and non-randomized controlled trials that compared pure endoscopic with microscopic transsphenoidal surgery (TSS) in the resection of pituitary tumors. Embase, PubMed, Lilacs, and Central Cochrane were used as our data sources. The outcomes were total tumor resection, achievement of biochemical control of functioning adenomas, hospital stay and surgery complications. The randomized trials were analyzed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach. Two randomized and three prospective controlled non-randomized studies were included. Two studies, including 68 patients, evaluated total tumor resection and the meta-analysis did not show differences between the groups [RR: 1.45 (95% CI: 0.87, 2.44)]. Three studies involving 65 patients analyzed the achievement of biochemical control and no statistical difference was found [RR: 0.94 (95% CI: 0.7, 1.26)]. All five studies compared the frequency of postoperative complications between intervention and control group and meta-analysis favored for a low rate of postoperative complications in the endoscopic TSS group [(RR: 0.37 (95% CI: 0.16, 0.83)]. Due to the low evidence level and low number of observations, the results of our meta-analysis should not be viewed as a final proof of inferiority or superiority of one approach in relation to the other. More data including higher numbers of observations are needed.

OBJETIVO: Analisar os efeitos da terapêutica adotada para o canal arterial (CA) em recém-nascidos (RN) < 1.000gadmitidos em unidades neonatais (UN) da Rede Brasileira de Pesquisas Neonatais (RBPN), sobre os desfechos: óbito, displasia broncopulmonar (DBP), hemorragia intraventricular grave (HIVIII/IV), retinopatia da prematuridade cirúrgica (ROPcir), enterocolite necrosante cirúrgica (ECNcir) e o desfecho combinado óbito e DBP. MÉTODOS: Estudo multicêntrico, de coorte, coleta de dados retrospectiva, incluindo RN de 16 UN da RBPN de 01/01/2010 a 31/12/2011, PN < 1.000 g, idade gestacional (IG) < 33 semanas e diagnóstico ecocardiográfico de PCA. Excluídos: óbitos ou transferências até o terceiro dia de vida, infecções congênitas ou malformações. Grupos:G1 - conservadora (sem intervenção medicamentosa ou cirúrgica), G2 - farmacológica (indometacina ou ibuprofeno) e G3 - cirúrgico (com ou sem tratamento farmacológico anterior). Analisou-se: uso de esteroide antenatal, parto cesárea, PN, IG, Apgar5' < 4, sexo masculino, SNAPPE II, síndrome do dDesconforto respiratório (SDR), sepse tardia, ventilação mecânica (VM), surfactante < 2 horas de vida, tempo de VM e os desfechos: óbito, dependência de oxigênio com 36 semanas (DBP36s), HIV III/IV, ROPcir, ECNcir e óbito/DBP36s. Estatística: Teste t-Student, Qui-Quadrado ou teste Exato de Fisher. Testes de Regressão Binária Logística e Regressão Múltipla Stepwise Backward. MedCalc (Medical Calculator) software, versão 12.1.4.0.p < 0,05. RESULTADOS: Foram selecionados 1.097 RN e 494 foram incluídos: G1-187 (37,8%), G2-205 (41,5%) e G3-102 (20,6%). Verificou-se: maior mortalidade (51,3%) no G1 e menor no G3(14,7%); maior frequência DBP36s (70,6%) e ROPcir (23,5%) no G3; maior frequência de óbito/DBP36s no G2 (58,0%). As abordagens farmacológica (OR-0,29; 95%, IC-0,14-0,62) e conservadora (OR-0,34; 95%, IC- 0,14-0,79) foram protetoras somente para o desfecho óbito/DBP36sem. CONCLUSÃO: Em RN com PCA, a abordagem conservadora relacionou-se à maior mortalidade, a cirúrgica à ocorrência de DBP36s e ROPcir., enquanto o tratamento farmacológico mostrou-se protetor para o desfecho óbito/DBP36sem.

OBJECTIVE: To analyze the effects of treatment approach on the outcomes of newborns (birth weight [BW] < 1,000 g) with patent ductus arteriosus (PDA), from the Brazilian Neonatal Research Network (BNRN) on: death, bronchopulmonary dysplasia (BPD), severe intraventricular hemorrhage (IVH III/IV), retinopathy of prematurity requiring surgical (ROPsur), necrotizing enterocolitis requiring surgery (NECsur), and death/BPD. METHODS: This was a multicentric cohort study, retrospective data collection, including newborns (BW < 1000 g) with gestational age (GA) < 33 weeks and echocardiographic diagnosis of PDA, from 16 neonatal units of the BNRN from January 1, 2010 to Dec 31, 2011. Newborns who died or were transferred until the third day of life, and those with presence of congenital malformation or infection were excluded. Groups: G1 - conservative approach (without treatment), G2 - pharmacologic (indomethacin or ibuprofen), G3 - surgical ligation (independent of previous treatment). Factors analyzed: antenatal corticosteroid, cesarean section, BW, GA, 5 min. Apgar score < 4, male gender, Score for Neonatal Acute Physiology Perinatal Extension (SNAPPE II), respiratory distress syndrome (RDS), late sepsis (LS), mechanical ventilation (MV), surfactant (< 2 h of life), and time of MV. Outcomes: death, O2 dependence at 36 weeks (BPD36wks), IVH III/IV, ROPsur, NECsur, and death/BPD36wks. Statistics: Student's t-test, chi-squared test, or Fisher's exact test; Odds ratio (95% CI); logistic binary regression and backward stepwise multiple regression. Software: MedCalc (Medical Calculator) software, version 12.1.4.0. p-values < 0.05 were considered statistically significant. RESULTS: 1,097 newborns were selected and 494 newborns were included: G1 - 187 (37.8%), G2 - 205 (41.5%), and G3 - 102 (20.6%). The highest mortality was observed in G1 (51.3%) and the lowest in G3 (14.7%). The highest frequencies of BPD36wks (70.6%) and ROPsur were observed in G3 (23.5%). The lowest occurrence of death/BPD36wks occurred in G2 (58.0%). Pharmacological (OR 0.29; 95% CI: 0.14-0.62) and conservative (OR 0.34; 95% CI: 0.14-0.79) treatments were protective for the outcome death/BPD36wks. CONCLUSION: The conservative approach of PDA was associated to high mortality, the surgical approach to the occurrence of BPD36wks and ROPsur, and the pharmacological treatment was protective for the outcome death/BPD36wks.

OBJETIVO: Determinar a frequência de câncer de cólon, hiperparatireoidismo primário, tumores de tireoide e pele em todos os acromegálicos em seguimento no Hospital de Clínicas da Faculdade de Medicina de Botucatu de 2005 a 2011. SUJEITOS E MÉTODOS: Esses pacientes foram avaliados retrospectivamente quanto a presença de câncer de cólon, hiperparatiroidismo primário, tumores da tiroide e pele. RESULTADOS: Dos 29 pacientes incluídos no início do estudo, dois foram excluídos. Dentre os 19 pacientes submetidos à colonoscopia, um apresentou adenocarcinoma de cólon (5%). Nódulos de tireoide estiveram presentes em 65% dos pacientes e carcinoma papilífero, em dois deles (7,7%). Quatro pacientes apresentaram hiperparatireoidismo primário (15%). A maioria das lesões de pele foram: espessamento (100%), acrochordons (64%), cistos epidérmicos (50%), pseudoacantose nigricans (50%) e apenas um paciente apresentou carcinoma basocelular. CONCLUSÃO: O tamanho da amostra é pequeno, mas nossos achados confirmam a alta frequência da neoplasia da tireoide e hiperparatireroidismo neste grupo de pacientes.

OBJECTIVE: To determine the frequency of colon cancer, primary hyperparathyroidism, thyroid tumor, and skin cancer in all acromegalic patients in follow-up at the Clinics Hospital - Botucatu Medical School, from 2005 to 2011. SUBJECTS AND METHODS: These patients were evaluated retrospectively for colon cancer, primary hyperparathyroidism, dermatological, and thyroid tumors. RESULTS: Of 29 patients included at the beginning of the study, two were excluded. Among 19 patients submitted to colonoscopy, one presented colon adenocarcinoma (5%). Thyroid nodules were present in 63% of patients, and papilliferous carcinoma was confirmed in two patients (7,7%). Four patients were confirmed as having primary hyperparathyroidism (15%). The most common dermatologic lesions were thickened skin (100%), acrochordons (64%), epidermal cysts (50%), and pseudo-acanthosis nigricans (50%). Only one patient presented basal cell carcinoma. CONCLUSION: Although a small number of acromegalic patients was studied, our findings confirm the high frequency of thyroid neoplasias and primary hyperparathyroidism in this group of patients.

A osteomalacia oncogênica é um diagnóstico clínico desafiador, caracterizado pela perda renal de fosfato e baixos níveis de 1,25-di-hidroxivitamina D3, ocorrendo na presença de um tumor produtor de altos níveis de fator de crescimento de fibroblasto 23. No entanto, é possível que se trate muito mais de uma falha de diagnóstico clínico do que propriamente uma doença rara. Os autores relatam o caso de um homem de 42 anos com histórico de fraqueza muscular progressiva por cinco anos e restrição à cadeira de rodas, sem diagnóstico. Seus exames laboratoriais evidenciavam baixos níveis de fósforo. A remoção cirúrgica de um hemangiopericitoma detectado previamente em cavidade nasal levou à resolução completa dos sintomas. Os autores enfatizam que, mesmo com a etiologia já evidenciada, o paciente consultou diversos clínicos no decorrer dos cinco anos até que fossem instituídos o diagnóstico e o tratamento adequados. Diagnosing oncogenic osteomalacia is still a challenge. The disorder is characterized by osteomalacia caused by renal phosphate wasting and low serum concentration of 1,25-dihydroxyvitamin D3 occurring in the presence of a tumor that produces high levels of fibroblast growth factor 23. However, it is possible that the disease is much more misdiagnosed than rare. We present the case of a 42-year-old man with a long-term history of undiagnosed progressive muscle weakness. His laboratory results mainly showed low serum phosphate. Surgical removal of a nasal hemangiopericytoma that had been diagnosed five years earlier, brought him to a symptom-free condition. Even though knowing the underlying etiology would explain his osteomalacia, the patient sought medical help from countless physicians for five consecutive years, and only after adequate treatment a rewarding outcome was achieved. OBJETIVO: O presente estudo teve como objetivo avaliar os genes PROP1 e HESX1 em um grupo de pacientes com displasia septo-óptica (DSO) e deficiência hormonal hipofisária (combinada - DHHC; ou deficiência isolada de GH - DGH). Onze pacientes com apresentação clínica e bioquímica consistente com DHHC, DGH ou DSO foram avaliados. SUBJECTS AND METHODS: Em todos os pacientes, o gene HESX1 foi analisado pelo sequenciamento direto e, nos casos de DHHC, o gene PROP1 foi também sequenciado. RESULTADOS: Um polimorfismo no gene HESX1 (1772 A > G; N125S) foi identificado em um paciente com DSO. Foram encontrados três pacientes portadores da variação alélica 27 T > C; A9A e 59 A > G; N20S no éxon 1 do gene PROP1. Mutações no gene PROP1 e HESX1 não foram identificadas nesses pacientes com DGH, DHHC e DSO esporádicos. CONCLUSÃO: Alterações genéticas em um ou diversos outros genes ou mecanismos não genéticos devem estar implicados nesse processo patogênico.

OBJECTIVE: The present study aimed at evaluating the PROP1 and HESX1 genes in a group of patients with septo-optic dysplasia (SOD) and pituitary hormone deficiency (combined - CPHD; isolated GH deficiency - GHD). Eleven patients with a clinical and biochemical presentation consistent with CPHD, GHD or SOD were evaluated. SUBJECTS AND METHODS: In all patients, the HESX1 gene was analyzed by direct sequence analysis and in cases of CPHD the PROP1 gene was also sequenced. RESULTS: A polymorphism (1772 A > G; N125S) was identified in a patient with SOD. We found three patients carrying the allelic variants 27 T > C; A9A and 59 A > G; N20S in exon 1 of the PROP1 gene. Mutations in the PROP1 and HESX1 genes were not identified in these patients with sporadic GHD, CPHD and SOD. CONCLUSION: Genetic alterations in one or several other genes, or non-genetic mechanisms, must be implicated in the pathogenic process.

Caesalpinia ferrea Mart. (jucá ou pau-ferro) é uma espécie da família Leguminosae cuja ocorrência estende-se da região Nordeste ao Estado do Rio de Janeiro. Trata-se de uma espécie bastante utilizada na medicina popular pelas suas inúmeras propriedades terapêuticas tais como antiinflamatória, analgésica, antimicrobiana e antitérmica as quais indicam a presença de compostos de interesse farmacológico. Contudo, muitos estudos em plantas também investigam a presença de compostos de interesse industrial. Com base nas propriedades terapêuticas e atividades já descritas para essa espécie, esse trabalho objetivou pesquisar atividades biológicas no extrato de sementes de C. ferrea na busca por compostos de interesse industrial e farmacológico. Os resultados indicaram a presença das atividades celulásica, amilásica, anticoagulante e larvicida contra A. aegypti no extrato aquoso das sementes de C. ferrea, entretanto, não foram observadas as atividades tóxica aguda, hemolítica, heparinásica, antibacteriana e antifúngica.

Caesalpinia ferrea Mart. is a species belonging to Leguminosae family commonly known in Brazil as "jucá" or "pau-ferro". It occurs in Brazil from the Northeast Region to the State of Rio de Janeiro and it is widely utilized in folk medicine due to its several therapeutic properties such as anti-inflammatory, analgesic, antimicrobial and antithermic, which indicate the presence of compounds of pharmacological interest. Besides, many studies with plants look for the presence of compounds with industrial applications. Based upon the therapeutic and bioactive properties described for this species so far, this work aimed to investigate several biological activities in the water extract of C. ferrea seeds. The results indicated the presence of the following activities: cellulase, amylase, anticoagulant and larvicide against A. aegypti in the water extract of C. ferrea seeds. Nevertheless, the extract did not show the other activities assayed: acute toxic activity, hemolytic, heparinasic, antibacterial and antifungal activities.

Complexo de Carney (CNC) e neoplasia endócrina múltipla tipo 1 (MEN1) são formas de neoplasias endócrinas múltiplas de herança autossômica dominante. O diagnóstico do CNC ocorre quando dois critérios maiores (lentiginose, doença nodular pigmentosa primária das adrenais, mixomas cardíacos e cutâneos, acromegalia, neoplasia testicular, carcinoma de tireóide) são observados e/ou um critério maior associado a um critério suplementar (familiar afetado, mutação do gene PRKAR1A) ocorre. Por outro lado, o diagnóstico de MEN1 dá-se pela detecção de dois ou mais tumores localizados na glândula hipofisária, paratireóide e/ou células pancreáticas. O presente caso descreve um homem de 55 anos, com diagnóstico de acromegalia, hiperparatireoidismo primário e carcinoma papilífero de tireóide, exibindo critérios diagnósticos para as duas condições descritas. Embora possa ter ocorrido apenas uma associação esporádica, ou a acromegalia per se tenha predisposto ao carcinoma papilífero, novos mecanismos moleculares podem estar envolvidos.

Carney Complex (CNC) and Multiple Endocrine Neoplasia type 1 (MEN1) are forms of multiple endocrine neoplasia of dominant autosomal inheritance. Diagnosis of CNC occurs when two major criteria (lentiginoses, primary pigmented nodular adrenocortical disease, cardiac and cutaneous myxomas, acromegaly, testicular neoplasias, thyroid cancer) are observed and/or a major criterion associated with a supplementary criterion (affected relative, PRKAR1A gene mutation) occurs. On the other hand, diagnosis for MEN1 occurs through detection of two or more tumors located at the pituitary gland, parathyroid and/or pancreatic cells. The present case describes a 55 year-old male patient, diagnosed with acromegaly, primary hyperparathyroidism and papillary thyroid cancer, exhibiting components that meet the diagnostic criteria of both conditions described. Despite the occurrence of only one sporadic association or the acromegaly per se being responsible for the papillary cancer, new molecular mechanisms may not be ruled out.

Desenvolveram-se estudos sobre flebotomíneos em área de leishmaniose tegumentar, fazenda Boa Sorte, Município de Corguinho, Mato Grosso do Sul, Brasil, com vistas a incriminar vetor dessa parasitose. No início dos estudos, encontravam-se bem preservados vários tipos da cobertura vegetal primitiva, com predomínio de cerrado e cerradão, denominado localmente de "croa". Decorridos quatro meses, parte significativa da "croa" e do cerrado foi queimada para transformação em áreas de pastagens. Durante julho/1991 a junho/93, realizaram-se coletas semanais das 18:00 às 6:00 horas, com armadilha CDC (Center on Disease Control), em floresta-galeria, floresta de encostas, cerrado, "croa", peridomicílio (chiqueiro e poleiro) e no interior de uma tulha; coletas mensais com armadilha de Shannon das 18:00 às 24:00 horas em floresta-galeria e "croa". De junho/91 a setembro de 1992, capturas mensais com isca humana, por 24 horas, em floresta-galeria. Investigou-se infecção natural por flagelados em flebotomíneos coletados com armadilha de Shannon e isca humana. As coletas com CDC resultaram 24 espécies de Lutzomyia e duas de Brumptomyia. A "croa" foi o ambiente que mais contribuiu com espécimens e que apresentou a maior diversidade, juntamente com a floresta de encostas. Nas coletas com CDC, L. whitmani revelou-se a mais abundante, índice de abundância padronizado = 0,991; porém, esteve muito pouco representada no interior do anexo domiciliar; apresentou prevalência de 96,0% nas armadilhas de Shannon e isca humana, respectivamente com 3.265 e 516 espécimens. Sua maior freqüência deu-se em épocas frias e secas. Dotada de atividade quase que exclusivamente noturna, exibiu pico de ocorrência das 18:00 às 19:00 horas. A taxa de infecção natural por flagelados, em 680 fêmeas de flebotomíneos dissecadas, foi de 0,15% e, entre 613 fêmeas de L. whitmani, de 0,16%. Com base em seu comportamento, L. whitmani foi incriminada como provável vetora da leishmaniose tegumentar na área. A segunda espécie mais abundante, L. lenti, não demonstrou antropofilia. Apresenta-se também a fauna flebotomínica por ambientes.

Studies of the phlebotomine sandflies on the Boa Sorte farm, Corguinho county, State of Mato Grosso do Sul, Central-West region of Brazil, were carried out, with the object of identifying local fauna and a cutaneous leishmaniasis vector. At the beginning of the studies, several types of primitive vegetation covering: gallery forest, forest slopes and the cerrados: s. str. and tropical xeromorphic semideciduous broadleaf forest, locally denominated "croa", existed. Four months after the beginning of the studies, a fire destroyed a significant part of the cerrados. Captures were made during the interval from July/91 to June/93, with a CDC trap, weekly, at 10 ecotopes: in the soil of forest slopes; in the soil and canopy of cerrado s. str., "croa" and gallery forest; in the peridomicile, in hen house and pigpen and in a storage shed. A Shannon's trap was used, monthly, from 18:00-24:00 hours, in the gallery forest and "croa". Human bait was used, monthly, for 24 hours, from June/91 to September/92. An investigation into natural infection in female phlebotomines was made through the dissection of specimens captured in the Shannon's trap and on human bait. The captures with CDC totalled in 2,281 specimens of 26 species: 2 of Brumptomyia and 24 of Lutzomyia. The "croa" was the environment that contributed with the greatest number of specimens and presented the largest diversity, together with the forest slope. L. withmani was the most abundant species captured with CDC, in all the ecotopes (Standardized abundance index = 0.991). However, in the storage shed its frequence was the lowest. This species presented a prevalence of 96.0% in the Shannon's trap and on human bait (3,265 and 516 specimens, respectively). It was the most frequent in the cold and dry periods. It presented almost exclusively nocturnal activity, with its peak at 18:00-19:00 hours and an infection rate by flagellates of 0.16% (613 females dissected). On the basis of its behavior, this species was incriminated as the probable vector of the cutaneous leishmaniasis in the area, which had extradomicilary transmission. L. lenti, the second most abundant species, is not anthropophilic. The phlebotomine fauna is presented by environment.

Com o intuito de acrescentar parâmetros à taxonomia do gênero Sirthenea Spinola, 1837, realizamos uma análise morfológica comparativa da genitália externa dos machos de quatro espécies: S. carinata (Fabricius, 1798), S. stria (Fabricius, 1794), S. flavipes (Stal, 1855) e S. amazona Stal, 1866. Foram evidenciadas 10 estruturas (8º esternito, processo do pigóforo, parâmeros, falosoma e seus quatro processos e os dois processos do endosoma) que permitem diferenciar as quatro espécies. The comparative morphology of the external genitalia of four species of the genus Sirthenea Spinola, 1837 were studied: S. carinata (Fabricius, 1798), S. stria (Fabricius, 1794), S. flavipes (Stal, 1855) and S. amazona Stal, 1866.Taxonomic data from ten structures (8º sternite, process of pygophore, parameres, phallosoma and its four processes and two endosoma processes) were diagnosed among these species. A brief description of the general morphology of each species is included.
