Abstract: This article presents a case study, based on the social systems theory, about how the communicative process influences the configuration of labor expectations upon graduation from the university in two groups of engineering students of the Electronics and Mechatronics careers of the Benemérita Universidad Autónoma de Puebla. The methodology, which has an explanatory scope, has a quantitative approach, and makes use of a survey technique and functional method in order to carry out a comparative analysis of the degree of informational interdependencies between the educational, economic and family systems. The results highlight that continuous school paths are possible with economic support from the family and that the work experiences, which consist of communicative situations that influence the type and degree of resonance of the information to condense, are predominantly work alternatives oriented to the automotive industry and activities linked with the design of system. In addition, few intentions for business entrepreneurship and graduate studies can be observed.

Resumen: Este artículo presenta un estudio de caso, basado en la teoría de sistemas sociales, sobre la influencia del proceso comunicativo en la configuración de las expectativas laborales al egresar de la universidad en dos grupos de estudiantes de ingeniería de las carreras de Electrónica y Mecatrónica de la Benemérita Universidad Autónoma de Puebla. La metodología tiene un alcance explicativo, utiliza un enfoque cuantitativo y, para realizar un análisis comparativo del grado de las interdependencias informativas entre los sistemas educativo, económico y familiar, se usaron la técnica de la encuesta y el método funcional. Los hallazgos resaltan trayectorias escolares continuas con apoyo económico de la familia y experiencias laborales, las cuales consisten en situaciones comunicativas que influyen en el tipo y grado de resonancia de la información para condensar, predominantemente, alternativas de trabajo orientadas a la industria automotriz y actividades en diseño de sistemas. Además, hay pocas intenciones de emprendimiento de negocios y estudios de posgrado.

Resumen El paracetamol es una droga analgésica y antipiré tica comúnmente utilizada, que ha experimentado un aumento en su consumo en los últimos años en nuestro medio. También se ha observado un incremento en el número de sobredosis accidentales e intencionales que fueron atendidas por el sistema de salud. Su toxicidad es dosis dependiente y puede causar falla hepática fulminante, convirtiéndose en una de las principales razones de trasplante hepático en países angloparlan tes. Se presenta el caso de una mujer de 28 años con antecedentes de depresión mayor y cinco intentos de suicidio previos, quien ingirió deliberadamente una cantidad significativa de comprimidos de paracetamol. Desarrolló una falla hepática fulminante y acidosis metabólica, por lo que fue sometida a un trasplante hepático de emergencia debido a la gravedad de su condición evolucionando favorablemente. La decisión de realizar un trasplante hepático en casos graves como este y bajo una condición de vulnerabilidad psiquiátrica grave, es un desafío y debe considerarse cuidadosamente. Este caso en particular ilustra la im portancia de la atención multidisciplinaria incluyendo la evaluación psiquiátrica en pacientes con intoxicación por paracetamol.

Abstract Acetaminophen is a commonly used analgesic and antipyretic drug, which has experienced an increase in its consumption in recent years in our environment. There has also been an increase in the number of accidental and intentional overdoses that were treated by the health system. Its toxicity is dose-dependent and can cause ful minant liver failure, becoming one of the main reasons for liver transplantation in English-speaking countries. The case of a 28-year-old woman with a history of major depression and five previous suicide attempts, who de liberately ingested a significant amount of paracetamol tablets, is here presented. She developed fulminant liver failure and metabolic acidosis, for which she underwent an emergency liver transplant due to the severity of her condition, from which she evolved favorably. The decision to perform a liver transplant in serious cases like this and under a condition of severe psychiatric vulnerability is challenging and must be carefully considered. This particular case illustrates the importance of multidisci plinary care including psychiatric evaluation in patients with acetaminophen poisoning.

Resumen Introducción : En el consultorio de atención inmediata de clínica se concentra la atención de pacientes ambu latorios con enfermedades que requieren diagnóstico precoz, como la adenitis tuberculosa (AT). El objetivo fue describir las características clínicas, estudios com plementarios y procedimientos realizados a pacientes con diagnóstico confirmado por bacteriología o anatomía patológica de AT. Métodos : Estudio observacional, descriptivo, retros pectivo. Período: 2017-2023. Resultados : Se incluyeron 49 pacientes, con una mediana de edad de 31 años; 59% de sexo femenino, 22% con comorbilidades El 40% presentó adenopatías localizadas, la mayoría cervicales. La serología para HIV era positiva en 3 (6.1%). Al 73% se le ingresaron muestras para bacteriología, con aislamiento de M. tuberculosis en 71%. Al 79% se le realizó punción aspiración con aguja fina (PAAF) ganglionar; en el 48% los resultados de la citología fueron sugestivos de tuberculosis (TB). Al 77% se le realizó biopsia ganglionar, resultando en el 62% adenitis granulomatosa. Desde la primera consulta hasta el diagnóstico transcurrieron una mediana de 40 días. La mayoría de los tratamientos se iniciaron luego del resultado de la biopsia, seguido de cultivos, baciloscopia, PAAF y GeneXpert. Un paciente falleció. Discusión : La AT predominó en el sexo femenino en el grupo estudiado, coincidente con la experiencia local, la edad promedio de presentación fue 30 a 40 años. Puede afectar cualquier cadena ganglionar, aunque predomina la localización cervical, que coincide con los hallazgos de este trabajo. En nuestra serie, la demora diagnóstica desde la primera consulta fue menor a la referida en la bibliografía.

Abstract Introduction : In the clinical medicine’s immediate assistance unit, care is focused on outpatients with dis eases that require early diagnosis, such as tuberculous adenitis (TA). The aim was to describe clinical features, complementary studies and procedures performed in patients with a diagnosis confirmed by bacteriology or pathological anatomy of TA. Methods : Observational, descriptive, retrospective. Period: 2017-2023. Results : Fourty nine patients were included, with a median age of 31 years, 59% were female, 22% with comorbidities. 40% had localized lymphadenopathy, most of them cervical. HIV serology was positive in 3 cases (6.1%). Samples for bacteriology were submitted in 73%, with isolation of M. tuberculosis in 71%. Nodal fine needle aspiration (FNA) was performed in 79%, and in 48% the cytology results were suggestive of tuberculosis. Nodal biopsy was performed in 77%, with granulomatous adenitis as result in 62%. The term between admis sion and diagnosis ranged from a median of 40 days. Most treatments were started after the biopsy result, followed by culture, bacilloscopy, FNA, and GeneXpert. One patient died. Discussion : TA predominates in the female sex in the studied group, coinciding with the local experience, the average age of presentation is 30 to 40 years, can affect any lymph node region, although the cervical location predominates, which coincides with the findings of this work. In our series, the diagnostic delay from the first consultation was shorter than reported in the literature.

Abstract Background: Dysphagia and cognitive impairment are common in older people. It is linked to alterations in brain areas related to swallowing. Objective: To explore the characteristics of dysphagia using fluoroscopy in patients with mild cognitive impairment (MCI) and dementia. Material and methods: 158 participants from a memory clinic. Specific criteria for MCI and dementia were used. Dysphagia was assessed with EAT-10 (≥3). Those who accepted completed a fluoroscopy study with a barium swallow to evaluate the phases of swallowing; the presence of bronchoaspiration supported the diagnosis of dysphagia. A logistic regression analysis was performed to test the probability between dysphagia and MCI. Results: According to EAT-10, 86 (54.4 %) were at risk of dysphagia, confirmed in 84 by fluoroscopy, 53.8 % MCI group and 46.2 % dementia. An inverse association was observed in the MCI oral phase group (OR 0.23, p < 0.025), and a positive association; in protection phase against regurgitation (OR 3.76, p < 0.056) and laryngeal muscle contraction (OR 3.22, p < 0.045) versus dementia group. Conclusions: The study showed a high frequency of dysphagia in patients with MCI, this highlights the importance of its early detection to improve complications associated with dysphagia.

Resumen Antecedentes: El deterioro cognitivo y la disfagia son comunes en personas mayores. Objetivo: Explorar las características de la disfagia mediante fluoroscopia en pacientes con deterioro cognitivo leve (DCL) y demencia. Material y métodos: Se analizaron 158 adultos de una clínica de memoria. Para evaluar el DCL y la demencia, se emplearon criterios específicos; y para la disfagia, la escala EAT-10. Se realizó fluoroscopia con trago de bario para evaluar las fases de la deglución; la presencia de broncoaspiración apoyó el diagnóstico de disfagia. Se realizó análisis de regresión logística para establecer la relación entre disfagia y DCL. Resultados: Según EAT-10, 86 pacientes (54.4 %) tenían riesgo de disfagia, la cual fue confirmada en 84 mediante fluoroscopia (53.8 % tenía DCL y 46.2 %, demencia). En el grupo con DCL se observó asociación inversa con alteraciones en la fase oral (RM = 0.23, p < 0.025) y asociación positiva con alteraciones en la protección contra la regurgitación (RM = 3.76, p < 0.056) y contracción del músculo laríngeo (RM = 3.22, p < 0.045) versus grupo con demencia. Conclusiones: El estudio mostró alta frecuencia de disfagia en pacientes con DCL, lo que resalta la importancia de su detección temprana para mejorar las complicaciones asociadas.

SUMMARY: Although almost all studies exclude lip conditions because they would affect furrow patterns, some authors maintain that they should be considered temporary or permanent disabilities, all of which require further research in this context.We present a study in which we associate lip conditions with lip print patterns to associate both morphological elements for fieldwork purposes. Fifty-seven women and 48 men aged between 19 and 38 years who resided in Temuco (Chile) were included. The lip conditions and their prints were recorded, and an analysis, comparison, evaluation, and verification protocol (ACE-V) for lip prints was applied. Of the participants, 27.4 % had healthy lips, while 71.7 % had some type of condition. Although patterns related to temporary and permanent lip conditions were recognized in the lip prints in a non-significant way, the diagnosis of "healthy" or "altered" lip status could be made significantly by a calibrated examiner. Although these conditions do not represent identifying variables without empirical studies to validate them, they can affect the quality of the evaluated lip print; therefore, they should be recognized during the analyses considering the prevalence of these conditions.

RESUMEN: Aunque casi todos los estudios excluyen las afecciones labiales pues afectarían los patrones de sus surcos y huellas, algunos autores sostienen que deben ser consideradas alteraciones temporales o permanentes, por lo que se requiere mayor investigación en este aspecto. Se presenta un estudio en el que se asocian afecciones labiales con sus patrones de huellas para evaluar conjuntamente ambos elementos morfológicos con fines de trabajo forense de campo. Se incluyeron 57 mujeres y 48 hombres con edades entre 19 y 38 años residentes en Temuco (Chile). Se registraron las afecciones de los labios y sus huellas, y se aplicó un protocolo de análisis, comparación, evaluación y verificación (ACE-V) para las huellas labiales. De los participantes, el 27,4 % tenía labios sanos, mientras que el 71,7 % presentaba algún tipo de afección. Aunque los patrones relacionados con afecciones labiales temporales y permanentes se reconocieron en las huellas labiales de manera no significativa, un examinador calibrado podría realizar significativamente el diagnóstico de estado labial "sano" o "alterado". Si bien estas condiciones no representan variables de identificación sin estudios empíricos que las validen, pueden afectar la calidad de la impresión labial evaluada; por lo tanto, deben ser reconocidos durante los análisis considerando la prevalencia de estas condiciones.

SUMMARY: Tissue engineering aims to fabricate a scaffold that exhibits a suitable surface topography for a desired cellular response. Therefore, a study analyzing the characteristics of bone grafts is important for future research directions. This work aims to analyze the physical-chemical characteristics of commercially available bone grafts of human and bovine origin for dental use, using morphological analysis of the surface and chemical composition by variable pressure scanning electron microscope (VP-SEM) and energy-dispersive x-ray (EDX) spectrometry. In addition, pore diameter and surface area were analyzed by degassing method using a porosimeter, and particle size by laser diffraction. The analyzed allograft and xenograft particles differ in morphological characteristics and chemical composition. The allograft particles present a cuboidal and prismatic geometric morphology with angled edges and the absence of macropores. On the contrary, the xenograft particles present an irregular morphology with macropores in their structure. There is a statistically significant difference in C, P, and Ca between the xenograft and allografts (p < 0,05). The analyzed composition of allografts showed mainly the presence of C and O. In contrast, the composition of the xenograft was mainly Ca. These differences could influence the osteogenic properties of allografts and xenografts. This analysis provides basic information to understand the physicochemical properties of allografts and xenografts that facilitate cell-graft interaction.

RESUMEN: La ingeniería de tejidos tiene como objetivo fabricar un andamio que muestre una topografía de superficie adecuada para una respuesta celular deseada. Por tanto, un estudio que analice las características de los injertos óseos es importante para futuros enfoques de investigación. Este trabajo tiene como objetivo analizar las características físico-químicas de injertos óseos de origen humano y bovino disponibles comercialmente para uso odontológico, mediante análisis morfológico de la superficie y composición química mediante microscopio electrónico de barrido de presión variable (VP-SEM) y x-dispersivo de energía. espectrometría de rayos (EDX). Además, el diámetro de los poros y el área superficial se analizaron mediante el método de desgasificación utilizando un porosímetro y el tamaño de las partículas mediante difracción láser. Las partículas de aloinjerto y xenoinjerto analizadas difieren en características morfológicas y composición química. Las partículas del aloinjerto presentan una morfología geométrica cúbica y prismática con bordes angulados y ausencia de macroporos. Por el contrario, las partículas de xenoinjerto presentan una morfología irregular con macroporos en su estructura. Existe una diferencia estadísticamente significativa en C, P y Ca entre el xenoinjerto y los aloinjertos (p < 0,05). La composición analizada de los aloinjertos mostró principalmente la presencia de C y O. Por el contrario, la composición del xenoinjerto fue principalmente Ca. Estas diferencias podrían influir en las propiedades osteogénicas de los aloinjertos y xenoinjertos. Este análisis proporciona información básica para comprender las propiedades fisicoquímicas de aloinjertos y xenoinjertos que facilitan la interacción célula-injerto.

SUMMARY: In the context of an active learning process, this study investigated the short-term and long-term academic performance of students in two practical sessions of the Histology course, which included an escape room activity. The impact of this approach on motivation and knowledge retention was also assessed. Students were divided into a control group, which followed a traditional teaching methodology, and an experimental group, which participated in the escape room activity. The results revealed significant improvements in post-intervention grades in the experimental group. Additionally, the study assessed students' perceptions of the escape room experience, which showed highly satisfactory evaluations.

RESUMEN: En el marco de un proceso de aprendizaje activo, se investigó el rendimiento académico a corto y a largo plazo de los estudiantes en dos sesiones prácticas de la asignatura de Histología a las que se incorporó un escape room. También se evaluó su impacto en la motivación y la retención del conocimiento. Los estudiantes fueron clasificados en un grupo control, que siguió una metodología de enseñanza tradicional, y un grupo experimental, que participó en la actividad del escape room. Los resultados revelaron mejoras significativas en las calificaciones posteriores a la intervención en el grupo experimental. El estudio también evaluó la percepción estudiantil de la experiencia del escape room que demostró valoraciones muy satisfactorias.

ABSTRACT Polymers based on glycidyl azide polymer (GAP) and isocyanate present molecular structures dependent on NCO/OH molar ratio and diisocyanate reactivity. In this study, GAP polymers are obtained from a reaction with aromatic (toluene diisocyanate, TDI) or aliphatic (isophoranediisocyanate, IPDI) diisocyanates, varying the NCO/OH molar ratio from equimolar to 2.5. The increment in NCO/OH molar ratio increases the gel fraction in GAP/TDI polymers up to 90 wt%, along with a progressive growth in their glass transition temperature (Tg), which rises 10 °C from NCO/OH equimolar to 2.5. In opposition, in the GAP/IPDI polymers, the maximum gel fraction is 20 wt%, and the Tg value practically does not change in NCO excess. Infrared spectroscopy shows the predominant presence of urethane groups in polymers containing up to 2.0 NCO/OH molar ratio; however, at 2.5, urethane and allophanate characteristic bands are present in both polymers. That reactivity is controlled by chemical kinetics since the activation barrier of the reaction between the GAP and TDI is 10 kcal.mol-1 lower than in the corresponding reaction with the IPDI. ABSTRACT BACKGROUND: Adolescence is characterized by complex and dynamic changes, often involving experimentation, including the use of psychotropic substances. Although it is well-established that recreational psychotropic drugs are associated with suicide ideation in adults, evidence of this association in adolescents remains limited. OBJECTIVE: To investigate the relationship between suicide ideation and psychotropic recreational drug use among adolescents. DESIGN AND SETTING: Systematic review with meta-analysis developed at Universidade Federal de Uberlândia (UFU) and Universidade Estadual de Campinas (UNICAMP), Brazil. METHODS: A search across eight electronic databases for observational studies, without language or publication year restrictions, was conducted. The Joanna Briggs Institute tool was used to assess the risk of bias. Random-effects meta-analyses and odds ratios were used to measure the effects. RESULTS: The search yielded 19,732 studies, of which 78 were included in the qualitative synthesis and 32 in the meta-analysis. The findings indicated that suicidal ideation was 1.96 times more likely (95% confidence interval, CI = 1.47; 2.61) for adolescents who used some drug recurrently and 3.32 times more likely (95%CI = 1.86; 5.93) among those who abused drugs. Additionally, adolescents who used cannabis were 1.57 times more likely (95%CI = 1.34; 1.84) to experience suicide ideation compared with non-users, while cocaine users had 2.57 times higher odds (95%CI = 1.47; 4.50). CONCLUSIONS: Psychotropic recreational drug use is associated with suicidal ideation among adolescents regardless of current or previous use, abuse, or type of substance used. ABSTRACT Congenital heart disease (CHD) affects eight to ten out of every 1,000 births, resulting in approximately 23,057 new cases in Brazil in 2022. About one in four children with CHD requires surgery or other procedures in the first year of life, and it is expected that approximately 81% of these children with CHD will survive until at least 35 years of age. Professionals choosing to specialize in CHD surgery face numerous challenges, not only related to mastering surgical techniques and the complexity of the diseases but also to the lack of recognition by medical societies as a separate subspecialty. Furthermore, families face difficulties when access to services capable of providing treatment for these children. To address these challenges, it is essential to have specialized hospitals, qualified professionals, updated technologies, sustainable industry, appropriate financing, quality assessment systems, and knowledge generation. The path to excellence involves specialization across all involved parties. As we reflect on the importance of Pediatric Cardiovascular Surgery and Congenital Heart Diseases establishing themselves as a subspecialty of Cardiovascular Surgery, it is essential to look beyond our borders to countries like the United States of America and United Kingdom, where this evolution is already a reality. This autonomy has led to significant advancements in research, education, and patient care outcomes, establishing a care model. Fentanyl coadministration reduces the catecholaminergic response to orotracheal intubation allowing propofol dose reduction. The aim of this study is to determine whether the hemodynamic response is improved by increasing the time between fentanyl and propofol administration and reducing the dose of the latter without increasing the time to achieve optimal hypnosis. Methods: After approval by the Research Ethics Committee, patients undergoing non-cardiac surgery with endotracheal intubation were randomized by a computer-generated table into six time-dose groups (1 or 2 minutes/1, 1.5, or 2mg.kg-1 of propofol). Patients with high bronchoaspiration risk, a difficult airway, hemodynamic instability, or anesthetic allergies were excluded. After giving intravenous fentanyl (2 μg.kg-1 ), each group received different doses of propofol after 1 or 2 minutes. Non-invasive blood pressure (BP) and heart rate (HR) were measured at pre-induction, pre-intubation, and post-intubation. Time to hypnosis (bispectral index < 60) was also recorded. Results: Of the 192 recruited patients, 186 completed the study (1 min group n = 94; 2 min group n = 92). It was observed that HR and BP decreased after propofol administration and increased after intubation in all groups (p<0.0001). In patients over 55 years, the 2 min – 2 mg.kg-1 group showed the greatest systolic BP reduction (36 ± 12%) at pre-intubation, while the 1 min – 1.5 mg.kg-1 group showed the least hemodynamic alteration between pre- and post-intubation (-4 ± 13%). No significant differences were found in younger patients or in the time to reach hypnosis between the six groups. While no cases of severe bradycardia were recorded, 5,4% of the sample required vasopressors. Resumo Diante da incerteza quanto à eficácia das vacinas contra a Covid-19 e diante da possibilidade de viver ou morrer, é importante conhecer a percepção e a expectativa de sua aceitação por parte da população. O objetivo principal do presente estudo é analisar a como a vacina contra a Covid-19 é vista pela população estudada e quais são as expectativas que famílias sul-americanas de uma área urbana de Pasco têm em relação a esta vacina. Este é um estudo descritivo, transversal, usando uma amostragem aleatória simples de 197 famílias. Os participantes foram selecionados digitalmente através do contato com uma liderança local e uma pesquisa online foi aplicada com o consentimento prévio dos indivíduos. A análise de regressão logística foi realizada no EPIDAT 4.1 com nível de significância de 5%. Acerca do desejo de se vacinar, é válido enfatizar que há falecimentos de familiares devido ao coronavírus e, portanto, é necessário que a equipe assistencial continue com o trabalho preventivo de incentivo à vacinação para obter resultados favoráveis para toda a população. Abstract Faced with the uncertainty of whether the vaccines against Covid-19 are effective or not and faced with living or dying, it is important to know the perception and expectation of their acceptance. The main aim of the study to analyze the perception and expectation of the vaccine against Covid- 19 that South American families have in an urban area of De Pasco. Descriptive, cross-sectional study, simple random sampling of 197 families. The participants were recruited digitally through a neighborhood leadership and an online survey was applied with prior consent. The logistic regression analysis was performed in EPIDAT 4.1 with a significance level of 5%. Abstract Background There is no consensus on the most effective strategy for Postoperative Pulmonary Complication (PPC) reduction. This study hypothesized that a Goal-Directed Fluid Therapy (GDFT) protocol of infusion of predetermined boluses reduces the occurrence of PPC in patients undergoing elective open abdominal surgeries when compared with Standard of Care (SOC) strategy. Methods Randomized, prospective, controlled study, conducted from May 2012 to December 2014, with ASA I, II or III patients undergoing open abdominal surgeries, lasting at least 120 min, under general anesthesia, randomized into the SOC and the GDFT group. In the SOC, fluid administration was according to the anesthesiologist's discretion. In the GDFT, the intervention protocol, based on bolus infusion according to blood pressure and delta pulse pressure, was applied. Patients were postoperatively evaluated by an anesthesiologist blinded to the group allocation regarding PPC incidence, mortality, and Length of Hospital Stay (LOHS). Results Forty-two patients in the SOC group and 43 in the GDFT group. Nineteen patients (45%) in the SOC and 6 in the GDFT (14%) had at least one PPC (p = 0.003). There was no difference in mortality or LOHS between the groups. Among the patients with PPC, four died (25%), compared to two deaths in patients without PPC (3%) (p = 0.001). The LOHS had a median of 14.5 days in the group with PPC and 9 days in the group without PPC (p = 0.001). Abstract Background: Members of the genus Cupiennius Simon, 1891 are categorized as wandering spiders and are part of the family Trechaleidae. The genomics and proteomics of Cupiennius spiders from North America remain uncharacterized. The present study explores for the first time molecular data from the endemic species Cupiennius chiapanensis Medina, 2006, and also presents new data for Cupiennius salei (Keyserling, 1878), both collected in southern Mexico. Methods: In total, 88 Cupiennius specimens were collected from southern Mexico and morphologically identified. DNA was extracted and the mitochondrial COI fragment was amplified. COI sequences were analyzed, and a phylogenetic tree was inferred for species from the Americas. Genetic diversity was analyzed using haplotype networks and gene distances. Venom was obtained from C. chiapanensis and C. salei by electrostimulation. The venom was separated by HPLC, visualized using SDS-PAGE, and quantified for use in toxicity bioassays in mice and insects. Results: Analysis of COI sequences from C. chiapanensis showed 94% identity with C. salei, while C. salei exhibited 94-97% identity with sequences from Central and South American conspecifics. The venom from C. chiapanensis exhibited toxic activity against crickets. Venoms from C. chiapanensis and C. salei caused death in Anastrepha obliqua flies. Analysis of venom fractions from C. salei and C. chiapanensis revealed molecular masses of a similar size as some previously reported toxins and neurotoxic components. We determined the amino acid sequences of ChiaTx1 and ChiaTx2, toxins that are reported here for the first time and which showed toxicity against mice and insects. Conclusion: Our work is the first to report COI-based DNA barcoding sequences from southern Mexican Cupiennius spiders. Compounds with toxic activity were identified in venom from both species.
