Abstract The puma population is constantly decreasing, and cloning by somatic cell nuclear transfer can be used to conserve the species. One of the factors determining the success of the development of cloned embryos is the cell cycle stage of the donor cells. We evaluated the effects of full confluency (~100%), serum starvation (0.5% serum), and roscovitine (15 µM) treatments on the cell cycle synchronization in G0/G1 of puma skin-derived fibroblasts by flow cytometric analysis. Also, we assessed the effects of these synchronization methods on morphology, viability, and apoptosis levels using microscopy tools. The results showed that culturing the cells to confluence for 24 h (84.0%), 48 h (84.6%), and 72 h (84.2%) and serum starvation for 96 h (85.4%) yielded a significantly higher percentage of cells arrested in the G0/G1 (P < 0.05) phase than cells not subjected to any cell cycle synchronization method (73.9%). Nevertheless, while serum starvation reduced the percentage of viable cells, no difference was observed for the full confluence and roscovitine treatments (P > 0.05). Moreover, roscovitine for 12 h (78.6%) and 24 h (82.1%) was unable to synchronize cells in G0/G1 (P > 0.05). In summary, full confluency induces puma fibroblast cell cycle synchronization at the G0/G1 stage without affecting cell viability. RESUMO OBJETIVO Estimar a soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares de quatro a 14 anos de idade residentes no município de São Paulo, segundo variáveis clínicas, demográficas, epidemiológicas e sociais, durante o período de fechamento das escolas como medida de controle da covid-19. MÉTODOS Realizou-se um inquérito sorológico em setembro de 2020 com amostra aleatória estratificada por tipo de rede de ensino (pública municipal, pública estadual e privada). Foi coletada amostra de sangue venoso utilizando-se o teste de imunoensaio de fluxo lateral da fabricante Wondfo para detecção de anticorpos totais contra o vírus SARS-CoV-2. Aplicaram-se questionários semiestruturados para o levantamento de dados clínicos, demográficos, sociais e epidemiológicos. RESULTADOS A soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares foi de 16,6% (IC95% 15,4–17,8). O estudo encontrou soroprevalências mais elevadas na rede pública municipal (18,5%; IC95% 16,6–20,6) e estadual (16,2%; IC95% 14,4–18,2) em relação à rede privada (11,7; IC95% 10,0–13,7) e entre escolares da raça/cor preta e parda (18,4%; IC95% 16,8–20,2) e no estrato social mais vulnerável (18,5%; IC95% 16,9–20,2). A pesquisa identificou menores soroprevalências nos escolares que relataram seguir as medidas recomendadas de proteção contra a covid-19. CONCLUSÃO A soroprevalência de anticorpos contra o vírus SARS-CoV-2 atinge principalmente os escolares socialmente mais vulneráveis. ABSTRACT Polycystic liver disease, a hereditary pathology, usually manifests as autosomal dominant polycystic kidney disease. The many cysts in the liver cause massive hepatomegaly, majorly affecting the patient’s quality of life. In cases of refractory symptoms, liver transplantation is the only treatment choice. A 43-year-old woman was followed up as a hepatology outpatient in August 2020, with a progressive increase in abdominal volume, lower limb edema, and cachexia. The patient was diagnosed with polycystic renal and liver disease with massive hepatomegaly in March 2021, a combined kidney-liver transplant. Liver size represented 13% of the patient’s corporal composition, weighing 8.6kg. The patient was discharged on the 7th postoperative day with no complications. Only 10-20% of patients with polycystic liver disease have clinical manifestations, most of which result from hepatomegaly. An increase in liver volume deteriorates liver function until the condition becomes end-stage liver disease, as kidney function is already compromised; liver-kidney transplantation remains the only treatment choice. RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. ABSTRACT OBJECTIVE To estimate the evolution of the prevalence of SARS-CoV-2 virus infection among residents aged 18 years or over in the municipality of São Paulo. METHODS This is a population-based household survey conducted every 15 days, between June and September 2020, and January and February 2021. In total, the study comprised 11 phases. The presence of antibodies against SARS-CoV-2 was identified in venous blood using a lateral flow test, Wondfo Biotech. In the last phase, the researchers combined it with an immunoenzymatic test, Euroimmun. The participants also answered a semi-structured questionnaire on sociodemographic and economic factors, and on social distancing measures. Prevalence estimates and the 95% confidence interval were estimated according to regions, Human Development Index, sex, age group, ethnicity, education, income, and variables associated with risk or prevention of infection. To compare the frequencies among the categories of each variable, the chi-square test with Rao-Scott correction was used, considering a significance level of 5%. RESULTS In total, 23,397 individuals were interviewed and had their samples collected. The estimated prevalence of antibodies against SARS-CoV-2 ranged from 9.7% (95%CI: 7.9–11.8%) to 25.0% (95%CI: 21.7–28.7). The prevalence of individuals with antibodies against the virus was higher among black and brown people, people with lower schooling and income, and among residents of regions with lower Human Development Index. The lowest prevalences were associated with recommended measures of disease protection. The proportion of asymptomatic infection was 45.1%. CONCLUSION The estimated prevalence of the infection was lower than the cumulative incidence variation, except for the last phase of the study. The differences in prevalence estimates observed among subpopulations showed social inequality as a risk of infection. The lower prevalence observed among those who could follow prevention measures reinforce the need to maintain social distancing measures as a way to prevent SARS-CoV-2 infection.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

ABSTRACT CONTEXT: Complex karyotypes in acute myeloid leukemia (AML) are characterized by an overall low response rate with frequent relapses after clinical treatment. CASE REPORT: Here, we describe the case of a 61-year-old obese female with clinically diagnosed AML who presented a complex karyotype involving an uncommon abnormality: ring chromosome 11. Immunophenotypic analysis confirmed the diagnosis. Classical and molecular cytogenetic analyses, using GTG banding and FISH (fluorescence in situ hybridization), revealed the presence of complex structural rearrangement involving r(11), add(12)(p13), der(5) and der(13). CONCLUSIONS: Molecular cytogenetic analysis is suitable for better identification and characterization of chromosomal rearrangements in AML. Case reports like this, as well as population-based studies, are necessary for understanding the karyotypic changes that occur in humans.

RESUMO Introdução Alterações do feedback auditivo, em geral, propiciam o aumento da fluência da fala de pessoas com gagueira, porém, poucos estudos investigaram os efeitos na fala de pessoas com taquifemia. Objetivo Comparar a velocidade de fala e a frequência das disfluências da fala espontânea e da leitura em adultos com e sem taquifemia, com duas formas diferentes de retroalimentação auditiva. Métodos Participaram deste estudo 16 adultos, divididos em dois grupos: grupo pesquisa (G1), composto por oito adultos com taquifemia e grupo controle (G2), com oito adultos fluentes, pareados por gênero e idade. Os participantes do G1 deveriam apresentar 10%, ou mais, de disfluências comuns e velocidade de fala maior do que os padrões esperados para a idade e gênero. Os procedimentos utilizados foram: avaliação audiológica, avaliação da fluência (fala espontânea e leitura), em duas condições de escuta: inicialmente sem alteração na retroalimentação auditiva – Retroalimentação Auditiva Habitual (RAH) – e, posteriormente, com a Retroalimentação Auditiva Atrasada (RAA). Resultados A RAA ocasionou redução do fluxo de sílabas por minuto e da frequência das disfluências comuns no G1, na tarefa de fala espontânea. Na leitura, houve diminuição da descontinuidade de fala e do fluxo de sílabas por minuto, no G2, sob o efeito da RAA. Conclusão O efeito da Retroalimentação Auditiva Atrasada nos adultos taquifêmicos foi positivo na fala espontânea, devido à redução da velocidade de fala e das disfluências comuns, que são as principais manifestações do distúrbio.
ABSTRACT Introduction Auditory feedback changes, in general, provide the increase of the speech fluency of people who stutter, but few studies have investigated the effects on speech of people who clutter. Purpose: To compare the speech rate and frequency of speech disruption in spontaneous speech and reading of adults with and without cluttering, with two different auditory feedbacks. Methods Participants were 16 adults, divided into two groups: Research Group (G1) composed by eight cluttering adults; Control Group (G2), composed by 8 fluent adults, paired by age and gender to participants of G1. The participants of G1 should present at least 10% of common disfluencies, and speech rate higher than the standards expected for age and gender. The procedures used were audiology assessment and fluency evaluation (spontaneous speech and reading) in two listening conditions, with Non-Altered Auditory Feedback (NAF) and after with Delayed Auditory Feedback (DAF). Results DAF caused reduction of flow of syllables per minute and frequency of common disfluencies in G1 during spontaneous speech task. In reading, there was a decrease in speech disruption and flow of syllables per minute, in G2, influenced by the effect of DAF. Conclusion The delayed auditory feedback effect in adults who clutter was positive in spontaneous speech due the reduction of speech rate and common disfluencies that are the main manifestations of this disorder.

Objetivou-se verificar o conhecimento de profissionais da Estratégia Saúde da Família sobre ações de prevenção e controle da transmissão vertical da sífilis. Estudo transversal, em Fortaleza-CE, Brasil, de agosto a outubro de 2009. Utilizou-se questionário autoaplicado, considerando respostas corretas aquelas consoante às recomendações do Ministério da Saúde. Utilizou-se análise univariada, com distribuições de frequências, médias e desvios padrão. Participaram 269 profissionais, 44,7% com seis a dez anos de atuação na saúde da família; 60% das perguntas foram respondidas corretamente sobre o conhecimento destes da seguinte forma: 75,8% conheciam o período de solicitação do exame Venereal Disease Research Laboratory; 78,1%, a droga alternativa para tratamento da gestante alérgica à penicilina; 55,1% a periodicidade de solicitação do VDRL para controle de cura; e 50,2%, a conduta diante do parceiro sexual. Os profissionais de saúde pesquisados não detinham conhecimento adequado acerca das ações preventivas e do controle da sífilis congênita.
The aim of this study was to investigate the knowledge of family health strategy professionals regarding the prevention and control of the vertical transmission of syphilis. This cross-sectional study was carried out in Fortaleza, Ceará, Brazil, from August to October 2009. A questionnaire was used, with its correct answers being in line with the recommendations of the Ministry of Health. Univariate analysis was carried out, with frequency distribution, mean and standard deviation. A total of 269 professionals participated, 44.7% had 6 to 10 years of experience in the FHS, and 60% of the questions regarding their knowledge were answered correctly: 75.8% were aware of the request period of the Venereal Disease Research Laboratory (VDRL) test; 78.1% of the alternative drug for the treatment for pregnant women allergic to penicillin, 55.1% of the request period for VDRL for cure and control, and 50.2% of the conduct with a sexual partner. It was concluded that healthcare professionals are unaware of the appropriate actions for the prevention and control of congenital syphilis.

Farmácia: a porta de entrada para o acesso a medicamentos para idosos residentes em Santos

Este estudo teve como objetivos conhecer o perfil dos medicamentos utilizados pelos idosos, com ênfase na associação entre aqueles de uso contínuo mais utilizados e outros fármacos sem indicação médica e avaliar a função social do farmacêutico nas farmácias privadas. O método utilizado foi a pesquisa qualitativa de natureza compreensiva e explicativa, realizada no município de Santos, em 2008, em bairros com maior concentração de idosos (IBGE,2000). Nesses bairros, foram identificadas três farmácias privadas, que autorizaram a realização da pesquisa por um período de uma semana, quando foram realizadas as entrevistas, a partir de um roteiro estruturado.Os resultados evidenciaram um maior consumo dos idosos para as classes farmacológicas dos medicamentos de uso contínuo (os anti-hipertensivos, antidiabéticos, diuréticos e o antiagregante plaquetário). Dentre os medicamentos adquiridos sem prescrição médica, destacaram-se a compra de: analgésicos/antitérmicos, antigripais, antiinflamatórios e fitoterápicos. Alguns dos que compõem esse conjunto são tarjados e exigiriam a apresentação do receituário. As evidências obtidas apontam para o fato de que o consumo autoindicado, associado ao uso de medicamentos de uso contínuo, é uma prática rotineira entre os idosos.
This study aimed to profile the consumption of drugs by seniors,with emphasis on the association between the most commonly purchased drugs of continueduse and other drugs without medical prescription,and to evaluate the social role of the pharmacist in private pharmacies.The method was qualitative research, of the comprehensive and explanatory type, carried outin the city of Santos in 2008, inneighborhoods with high concentration of elderly people(IBGE 2000). In these neighborhoods, three private pharmacieswere identified.They authorized the conductionof the research for one week. During this period,interviews were performedbasedon a structured script.The results showed that the elderly mostly consumethe pharmacological classes of drugs ofcontinuous use: antihypertensives, antidiabetic drugs, diuretics and the antiplatelet agent.The main drugs acquired without medical prescription were: analgesics and antipyretics, anti-flu drugs, anti-inflammatory drugs and herbal medicines. Some of those who comprise this set could not have been acquired without prescription. The evidences point to the fact that self-prescribedconsumption, associated with the utilizationof drugs of continuous use, is a common practice among the elderly.
