RESUMO Introdução: A carreira do cirurgião é desafiadora, pesquisas científicas identificaram uma prevalência de burnout em cerca de 1/3 dos cirurgiões plásticos. Estudos científicos anteriores sobre esse tópico têm se concentrado nos aspectos negativos do trabalho na área médica. Os dados de bem-estar e dos fatores associados à maior felicidade disponíveis, específicos para cirurgia plástica, são inconsistentes e limitados. O objetivo é avaliar a felicidade do cirurgião plástico do estado de São Paulo e quais fatores estão associados à maior felicidade. Método: Estudo primário, observacional, descritivo e transversal. Foi realizada uma pesquisa on-line utilizando um instrumento validado, a Escala de Felicidade Subjetiva (EFS), que foi enviado aos membros da Regional São Paulo da Sociedade Brasileira de Cirurgia Plástica (SBCP-SP) entre dezembro de 2020 e julho de 2021. Características sociodemográficas e ocupacionais foram relacionadas ao grau de felicidade mensurado. Resultados: A taxa de resposta foi de 12,18%, n=268, sendo 70,1% do sexo masculino e 29,9% do feminino. O escore obtido através da EFS foi de 5,51±0,13 e a média do escore para o sexo masculino foi de 5,49 e para o sexo feminino de 5,57. 143 (53,36%) dos participantes são membros associados e 125 (46,64%) membros titulares da SBCP. 177 (66,04%) afirmaram que, caso pudessem voltar atrás, escolheriam novamente a cirurgia plástica como especialidade, 62 (23,13%) que talvez, e 27 (10,82%) que não. Conclusão: A cirurgia plástica no estado de São Paulo, Brasil, possibilita aos profissionais da especialidade uma carreira com altos índices de felicidade, inclusive para o sexo feminino. ABSTRACT Introduction: The career of the surgeon is challenging, and scientific research has identified a prevalence of burnout in approximately 1/3 of plastic surgeons. The data on well-being and factors associated with greater happiness available specifically to plastic surgery are inconsistent and limited. Objective: To evaluate the happiness of plastic surgeons in São Paulo and which factors are associated with greater happiness. Method: This was a primary, observational, descriptive, and cross-sectional study. An online survey was conducted using a validated instrument, the Subjective Happiness Scale (SHS), which was sent to members of the Brazilian Society of Plastic Surgery-São Paulo (SBCP-SP) from December 2020 to July 2021. Sociodemographic and occupational characteristics were related to the degree of happiness measured. Results: The response rate was 12.18%, n = 268, with 70.1% males and 29.9% females. The score obtained using the SHS was 5.51 ± 0.13, and the mean score for males was 5.49 and for females was 5.57. A total of 143 (53.36%) of the participants were associate members, and 125 (46.64%) were full members of the SBCP. A total of 177 (66.04%) stated that if they could go back in time, they would choose plastic surgery again as a specialty, 62 (23.13%) perhaps, and 27 (10.82%) said that they would not. Conclusion: Plastic surgery in the state of São Paulo, Brazil, allows professionals in the specialty to have a career with high levels of happiness, including for females. ■ RESUMO Introdução: Esta revisão de escopo tem por objetivo analisar a qualidade das informações sobre lipoaspiração para o público leigo. Método: Foi realizada busca na literatura no período de 18 de novembro a 12 de dezembro de 2021 nas seguintes plataformas de base de dados: Medline, Cochrane, LILACS, Embase e BVS. A estratégia de busca envolveu a combinação de vários descritores. Três investigadores independentes leram o resumo dos estudos que foram obtidos usando a estratégia de busca para avaliar aqueles que preenchiam os critérios de elegibilidade. Resultados: Inicialmente, foram levantados 33 artigos utilizando a estratégia de busca. Dentre esses, 23 estudos foram excluídos após a leitura dos resumos e avaliação dos critérios de elegibilidade, por não possuírem desfechos de interesse ao tema proposto. Assim, dez estudos preenchiam os critérios de inclusão, sendo nove estudos transversais e uma revisão de literatura. Dentre os dez artigos incluídos, nove relatam que as informações sobre lipoaspiração são precárias e imprecisas. ■ ABSTRACT Introduction: This scoping review aims to analyze the quality of information about liposuction for the lay public. Method: A literature search was carried out from November 18 to December 12, 2021, on the following database platforms: Medline, Cochrane, LILACS, Embase, and VHL. The search strategy involved the combination of several descriptors. Three independent investigators read the abstract of studies obtained using the search strategy to evaluate those that met the eligibility criteria. Results: Initially, 33 articles were collected using the search strategy. Among these, 23 studies were excluded after reading the abstracts and evaluating the eligibility criteria, as they did not have outcomes of interest to the proposed topic. Thus, ten studies met the inclusion criteria, nine of which were cross-sectional and one literature review. Among the ten articles included, nine report that information about liposuction is poor and inaccurate. Conclusion: The content on liposuction made available to the lay public via the Internet is, for the most part, unsatisfactory. Abstract COVID-19 can converge with the pro-inflammatory immunoregulatory mechanisms of chronic lung diseases. Given the disorders inherent to lung transplantation and the inexistence of other definitive therapeutic alternatives, Adipose tissue-derived Stem Cells (ASCs) presented themselves as a therapeutic hope. The purpose of this review is to assess the basis for the potential use of ASCs in lung diseases unresponsive to conventional therapy, relating to their possible use in COVID-19 ARDS. 35 studies comprised this review, 14 being narrative reviews, 19 preclinical trials and two proofs of concept. COVID-19 can converge with the pro-inflammatory immunoregulatory mechanisms of chronic lung diseases. In view of the disorders inherent to lung transplantation and the inexistence of definitive therapeutic alternatives, Adipose tissue-derived Stem Cells (ASCs) presented themselves as a therapeutic hope. Its detailed reading indicated the absence of serious adverse effects and toxicity to the administration of ASCs and suggested possible effectiveness in reducing lung damage, in addition to promoting the recovery of leukocytes and lymphocytes with its immunomodulatory and anti-apoptotic effects. The revised clinical data suggests optimism in the applicability of ASCs in other immunoinflammatory diseases and in severe COVID-19 ARDS. However, further studies are needed to develop a consensus on the methods of collection of ASCs, the ideal dosage schedule, the most effective time and route of administration, as well as on the definition of indications for the administration of ASCs in cases of COVID-19 for conducting clinical trials in near future. RESUMO Introdução: A ferida cirúrgica apresenta altos níveis de radicais livres em resposta ao dano cutâneo, o que gera a hipótese de um possível benefício do uso de antioxidantes no reparo destas feridas, tal como a aplicação tópica do ácido ascórbico. No entanto, pesquisas recentes obtiveram conclusões discrepantes para este tipo de tratamento. O objetivo é avaliar o efeito do ácido ascórbico tópico na cicatrização cutânea por meio de uma revisão de escopo. Métodos: A revisão de escopo foi realizada na base de dados Medline, Lilacs e Cochrane, com os descritores: ácido ascórbico, creme para a pele e cicatrização de feridas. Foram definidos como critérios de inclusão: ensaios clínicos randomizados, observacionais e revisões sistemáticas, em humanos, com data de publicação de até 5 anos, nas línguas inglesa, portuguesa ou espanhola. Foram excluídas: revisões narrativas, dissertações, teses, editoriais, estudos in vitro e em animais. Por fim, foi realizada a classificação dos estudos através da metodologia GRADE. Resultados: Foram encontrados 83 estudos e, após triagem, seis artigos foram selecionados. Destacou-se o uso do ácido ascórbico na concentração de 5 a 20% e de seus derivados (0,075% a 9,55%). Apresentaram a qualidade GRADE moderada os desfechos: aumento da firmeza cutânea e redução da vermelhidão, e alta qualidade: melhora na hidratação, elasticidade, colorometria das manchas e melhora do fechamento das feridas. Conclusão: O ácido ascórbico promove melhor elasticidade cutânea, diminuição do eritema e melhor fechamento das feridas. Apesar destes fortes indícios, ensaios clínicos randomizados com menor risco de viés de aferição e com maior casuística ainda se fazem necessários.

ABSTRACT Introduction: The surgical wound has high levels of free radicals in response to skin damage, which raises the hypothesis of a possible benefit from using antioxidants in repairing these wounds, such as the topical application of ascorbic acid. However, recent research has found conflicting conclusions about this type of treatment. The objective is to evaluate the effect of topical ascorbic acid on skin healing through a scope review. Methods: The scope review was carried out in the Medline, Lilacs and Cochrane databases, with the descriptors: ascorbic acid, skin cream, and wound healing. Inclusion criteria were defined as randomized clinical trials, observational and systematic reviews, in humans, with a publication date of up to 5 years, in English, Portuguese or Spanish. The following were excluded: narrative reviews, dissertations, theses, editorials, in vitro and animal studies. Finally, the studies were classified using the GRADE methodology. Results: 83 studies were found, and six articles were selected after screening. The use of ascorbic acid in the concentration of 5 to 20% and its derivatives (0.075% to 9.55%) stood out. The outcomes presented a moderate GRADE quality: increased skin firmness and reduced redness, and high quality: improved hydration, elasticity, colorimetry of the stains and improved wound closure. Conclusion: Ascorbic acid promotes better skin elasticity, reduced erythema and better wound closure. Despite these strong indications, randomized clinical trials with a lower risk of measurement bias and greater casuistry are still necessary.

RESUMO Introdução: Avaliar o impacto da monitoria em Cirurgia Plástica da Universidade Federal de São Paulo no conhecimento e nas habilidades práticas em estudantes de Medicina. Métodos: Seis acadêmicos participantes da monitoria e oito acadêmicos não participantes foram selecionados aleatoriamente e avaliados antes e depois do programa, por meio de testes de múltipla escolha e através do instrumento Surgical Skills Assessment (SSA), para avaliação das habilidades práticas. Resultados: O Grupo estudo obteve aumento significativo nas notas de prova teórica, entre o período antes do programa (média 15,8 e desvio padrão de 2,7) e após (média 21,8 e desvio padrão 1,3) (p = 0,02). Resultado semelhante foi encontrado com o instrumento SSA entre o período antes (média 9,8 e desvio padrão 1,4) e após o programa (média de 13,5 e desvio padrão 1,6) (p = 0,02). O grupo controle não obteve mudança significativa nos testes teóricos em agosto de 2010 ( média de 14,0 e desvio padrão 4,1) e junho de 2011 ( média de 13,8 e desvio padrão de 2,6 ) (p = 0,40 ). ABSTRACT Introduction: To assess the impact of extracurricular activities in plastic surgery on the knowledge and practical skills of medical students at the Federal University of São Paulo. Methods: Six students participating in extracurricular activities and eight students who did not participate were randomly selected and evaluated before and after the program by using multiple-choice tests and a Surgical Skills Assessment (SSA) instrument to assess their practical skills. Results: The study group obtained higher scores in the theoretical tests, when the scores before starting the program (average, 15.8; standard deviation, 2.7) were compared with those received after their participation in extracurricular activities (average, 21.8; standard deviation, 1.3) (p = 0.02). Similar results were obtained with the SSA instrument for the period before the start of (average, 9.8; standard deviation, 1.4) and after (average, 13.5; standard deviation, 1.6) (p = 0.02) the program. INTRODUÇÃO: Transfusões de uma ou mais unidades de concentrado de hemácias é estratégia amplamente utilizada em cirurgia cardíaca, mesmo após várias evidências de aumento de morbimortalidade. A escassez de sangue no mundo também já é evidente. OBJETIVO: Avaliar se o risco de mortalidade é dose-dependente do número de unidades de concentrado de hemácias transfundidas após cirurgia de revascularização miocárdica. MÉTODOS: Entre junho 2009 e julho 2010, foram analisados 3010 pacientes: transfundidos e não transfundidos. Pacientes hemotransfundidos foram divididos em seis grupos conforme receberam uma, duas, três, quatro, cinco e seis ou mais unidades concentrado de hemácias e, após um ano da cirurgia de revascularização miocárdica, avaliamos o risco de mortalidade em cada grupo. Para obtenção do odds ratio foi utilizado modelo de regressão logística multivariado. RESULTADOS: Transfusão crescente de unidades de concentrado de hemácias resulta em risco também crescente de mortalidade, evidenciando uma relação dose-reposta. Os valores do odds ratio aumentam com o acréscimo do número de unidades de hemácias alogênicas transfundidas. O risco de ocorrência de óbitos pelo odds ratio bruto foi 1,42 (P=0,165); 1,94 (P=0,005); 4,17; 4,22; 8,70, 33,33 (P<0,001) e o risco de mortalidade pelo odds ratio ajustado foi 1,22 (P=0,43); 1,52 (P=0,08); 2,85; 2,86; 4,91 e 17,61 (P<0,001), conforme receberam transfusão de uma, duas, três, quatro, cinco, seis ou mais unidades concentrado de hemácias, respectivamente. CONCLUSÃO: O risco de mortalidade é diretamente proporcional ao número de unidades de concentrado de hemácias transfundidas em cirurgia de revascularização miocárdica. Quanto mais sangue alogênico transfundido, maior o risco de mortalidade. A prática transfusional atual precisa ser reavaliada.

INTRODUCTION: Transfusions of one or more packed red blood cells is a widely strategy used in cardiac surgery, even after several evidences of increased morbidity and mortality. The world's blood shortage is also already evident. OBJECTIVE: To assess whether the risk of mortality is dose-dependent on the number of packed red blood cells transfused after coronary artery bypass graft. METHODS: Between June 2009 and July 2010, were analyzed 3010 patients: transfused and non-transfused. Transfused patients were divided into six groups according to the number of packed red blood cells received: one, two, three, four, five, six or more units, then we assess the mortality risk in each group after a year of coronary artery bypass graft. To calculate the odds ratio was used the multivariate logistic regression model. RESULTS: The increasing number of allogeneic packed red blood cells transfused results in an increasing risk of mortality, highlighting a dose-dependent relation. The odds ratio values increase with the increased number of packed red blood cells transfused. The death's gross odds ratio was 1.42 (P=0.165), 1.94 (P=0.005), 4.17; 4.22, 8.70, 33.33 (P<0.001) and the adjusted death's odds ratio was 1.22 (P=0.43), 1.52 (P=0.08); 2.85; 2.86; 4.91 and 17.61 (P<0.001), as they received one, two, three, four, five, six or more packed red blood cells, respectively. CONCLUSION: The mortality risk is directly proportional to the number of packed red blood cells transfused in coronary artery bypass graft. The greater the amount of allogeneic blood transfused the greater the risk of mortality. The current transfusion practice needs to be reevaluated.

INTRODUÇÃO: As Testemunhas de Jeová são um grupo religioso que não aceita as transfusões de sangue. RELATO DO CASO: Os autores descrevem o caso de uma criança, Testemunha de Jeová, que apresentava alopecia cicatricial no couro cabeludo e defeito da calota craniana, resultantes de politraumatismo. O tratamento foi iniciado com a colocação de expansor no couro cabeludo e uso de eritropoetina e sulfato-ferroso para aumento dos níveis de hemoglobina. A retirada do expansor e a correção do defeito da calota craniana com placa de acrílico foram realizadas com sucesso e sem o emprego de transfusões de sangue.

INTRODUCTION: Jehovah's witnesses are a religious group that refuses blood transfusion. CASE REPORT: The authors describe a case of a child who was a Jehovah's Witness and presented cicatricial alopecia in the scalp and cranial bone defect caused by a polytraumatism. The treatment was initiated with the introduction of tissue expander in the scalp and the use of erythropoietin and ferrous sulfate to increase the hemoglobin levels. The removal of the tissue expander and the correction of the cranial bone defect with an acrylic plate were done with success and without blood transfusion.

O estudo anatômico do encéfalo em cortes é facilitado empregando-se métodos de coloração para substância cinzenta. Os métodos mais freqüentemente empregados são os de Barnard, Robert e Brown, Mulligan e Green. O objetivo deste estudo foi determinar qual dessas técnicas apresenta melhores resultados com relação à diferenciação entre substâncias branca e cinzenta. Trinta cortes coronais de hemisfério cerebral humano foram submetidos às três técnicas, comparados entre si e analisados de acordo com três parâmetros estabelecidos: grau de diferenciação entre as substâncias branca e cinzenta; presença de linha única e contínua separando a substância branca do córtex cerebral; grau de impregnação da coloração em outros locais de substância branca. Atribuíram-se pontuações de 0 a 3 conforme a presença destes parâmetros, cada corte recebendo pontuação total que variava de 0 a 9. Após análise estatística, a técnica de Barnard, Robert e Brown apresentou média 8,33; a de Green 7,93 e a de Mulligan, 7,5, com diferença estatisticamente significativa.

Studing neuroanatomy at brain slices with gray matter staining techniques has several advantages. More often, the models described by Barnard, Robert and Brown, Mulligan, and Green are used. The aim of this study was to identify which of them achieves the best results on differentiation between the gray and the white matter. Thirty coronal slices of human brains underwent staining by the three techniques, and thus compared and analysed according this three parameters: degree of differentiation between white and gray matter, presence of a single and uninterrupted line dividing the white matter from the brain cortex; and degree of impregnation of the color staining in the white matter; scores from 0 to 3 have been given for the three parameters, with total score from 0 to 9. After statistic analysis, the Barnard, Robert and Brown model showed the best results, followed by Green's and Mulligan's.
