Abstract Background Patients with immune-mediated rheumatic diseases (IMRDs) have been prioritized for COVID-19 vaccination to mitigate the infection severity risks. Patients with rheumatoid arthritis (RA) are at a high risk of severe COVID-19 outcomes, especially those under immunosuppression or with associated comorbidities. However, few studies have assessed the safety of the COVID-19 vaccine in patients with RA. Objective To evaluate the safety of vaccines against SARS-CoV-2 in patients with RA. Methods This data are from the study “Safety and Efficacy on COVID-19 Vaccine in Rheumatic Diseases,” a Brazilian multicentric prospective phase IV study to evaluate COVID-19 vaccine in IMRDs in Brazil. Adverse events (AEs) in patients with RA of all centers were assessed after two doses of ChAdOx1 (Oxford/AstraZeneca) or CoronaVac (Sinovac/Butantan). Stratification of postvaccination AEs was performed using a diary, filled out daily and returned at the end of 28 days for each dose. Results A total of 188 patients with RA were include, 90% female. CoronaVac was used in 109 patients and ChAdOx1 in 79. Only mild AEs were observed, mainly after the first dose. The most common AEs after the first dose were pain at the injection (46,7%), headache (39,4%), arthralgia (39,4%), myalgia (30,5%) and fatigue (26,6%), and ChAdOx1 had a higher frequency of pain at the injection (66% vs 32 %, p < 0.001) arthralgia (62% vs 22%, p < 0.001) and myalgia (45% vs 20%, p < 0.001) compared to CoronaVac. The more common AEs after the second dose were pain at the injection (37%), arthralgia (31%), myalgia (23%), headache (21%) and fatigue (18%). Arthralgia (41,4% vs 25%, p = 0.02) and pain at injection (51,4% vs 27%, p = 0.001) were more common with ChAdOx1. No serious AEs were related. With Regard to RA activity level, no significant difference was observed between the three time periods for both COVID-19 vaccines. Conclusion In the comparison between the two immunizers in patients with RA, local reactions and musculoskeletal symptoms were more frequent with ChAdOx1 than with CoronaVac, especially after the first dose. In summary, the AE occurred mainly after the first dose, and were mild, like previous data from others immunizing agents in patients with rheumatoid arthritis. RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials. Resumo O uso de coagulantes naturais é uma alternativa promissora para substituir ou auxiliar coagulantes químicos devido às suas inúmeras vantagens. Este artigo avalia a eficácia no uso de um coagulante natural no tratamento de água. O coagulante foi extraído da semente de Killip Zygia cauliflora (Willd.) em soluções salinas e desengordurada com etanol a 95%. Os testes foram conduzidos utilizando diferentes concentrações do coagulante (0,1, 0,5, 1,0, 2,0, 3,0 e 4,0 g L-1) e de NaCl (0,0 M, 1,0 M e 5,0 M). Os resultados mostraram que o uso de 2 g L-1 de coagulante e 1,0 M de sal alcançou uma eficácia de 20% e 70%, respectivamente, na remoção de cor e turbidez. A análise mostrou uma eficácia de 70% e 30%, respectivamente, na remoção do composto UV254 e DOM (matéria orgânica dissolvida). Além disso, o mecanismo de remoção da turbidez pelos coagulantes preparados com solução aquosa e com solução salina a 1,0 M de NaCl é suposto ser adsorção e neutralização de carga, respectivamente seguindo os modelos de Freundlich e Langmuir. No entanto, o coagulante preparado com solução salina a 5,0 M de NaCl tende a formar uma estrutura semelhante a uma rede seguida pela remoção da turbidez por mecanismo de coagulação por varredura. Portanto, trabalhos futuros devem se concentrar no uso de Zygia cauliflora (Willd.) Killip como uma alternativa para substituição ou uso como coagulante químico auxiliar.
Abstract The use of natural coagulants is a promising alternative to replace or assist chemical coagulants due to its numerous advantages. This paper evaluates the effectiveness of a natural coagulant in water treatment. The coagulant was extracted from the Zygia cauliflora (Willd.) Killip seed in saline solutions and defatted using 95% ethanol. The tests were conducted using different concentrations of the coagulant (0.1, 0.5, 1.0, 2.0, 3.0, and 4.0 g L-1) and of NaCl (0.0 M, 1.0 M, and 5.0 M). The results showed that the use of 2 g L-1 of coagulant and 1.0 M of the salt achieved an effectiveness of 20% and 70%, respectively, of color and turbidity removal. The analysis showed an effectiveness of 70% and 30%, respectively, when removing the UV254 compound and DOM (dissolved organic matter). The mechanism for turbidity removal by the coagulants prepared with aqueous solution and with saline solution at 1.0 M of NaCl is supposed to be adsorption and charge neutralization, respectively, following the Freundlich and Langmuir models. However, the coagulant prepared with saline solution at 5.0 M of NaCl tends to form a netlike structure followed by turbidity removal through a sweep coagulation mechanism. Future papers should therefore focus on the use of Zygia cauliflora (Willd.) Killip as an alternative for replacement or use as an auxiliary chemical coagulant.

Spatial analysis of probable cases of dengue fever, chikungunya fever and zika virus infections in Maranhao State, Brazil

ABSTRACT Dengue fever, chikungunya fever, and zika virus infections are increasing public health problems in the world, the last two diseases having recently emerged in Brazil. This ecological study employed spatial analysis of probable cases of dengue fever, chikungunya fever, and zika virus infections reported to the National Mandatory Reporting System (SINAN) in Maranhao State from 2015 to 2016. The software GeoDa version 1.10 was used for calculating global and local Moran indices. The global Moran index identified a significant autocorrelation of incidence rates of dengue (I=0.10; p=0.009) and zika (I=0.07; p=0.03). The study found a positive spatial correlation between dengue and the population density (I=0.31; p<0.001) and a negative correlation with the Performance Index of Unified Health System (PIUHS) by basic care coverage (I=-0.08; p=0.01). Regarding chikungunya fever, there were positive spatial correlations with the population density (I=0.06; p=0.03) and the Municipal Human Development Index (MHDI) (I=0.10; p=0.002), and a negative correlation with the Gini index (I=-0.01; p<0.001) and the PIUHS by basic care coverage (I=-0.18; p<0.001). Lastly, we found positive spatial correlations between Zika virus infections and the population density (I=0.13; p=0.005) and the MHDI (I=0.12; p<0.001), as well as a negative correlation with the Gini index (I=-0.11; p<0.001) and the PIUHS by basic care coverage (I=-0.05; p=0.03). Our results suggest that several socio-demographic factors influenced the occurrence of dengue fever, chikungunya fever, and zika virus infections in Maranhao State.

Resumo Neste estudo, uma membrana polimérica combinada com carvão ativado granular (CAG) impregnado com nanopartículas de óxido de cobre foi avaliada em um processo híbrido com membrana. A avaliação do sistema gravitacional proposto visando a melhoria da qualidade da água destinada ao consumo humano em países em desenvolvimento foi realizada avaliando-se o fluxo permeado, pH, remoção de Escherichia coli, cor, turbidez e a remoção de cloro livre. Técnicas instrumentais tais como adsorção de N2 a 77 K, microscopia eletrônica de varredura (MEV), microscopia eletrônica de transmissão (MET) e análises de espectroscopia de raios-x por dispersão em energia (EDX) foram utilizadas para caracterizar o sistema proposto de membrana e adsorvente. O CAG garantiu maiores remoções de cloro, e também maior fluxo permeado. Além disso, o CAG impregnado com nanopartículas de óxido de cobre exibiu maior remoção de E. coli. Portanto, o sistema de membrana gravitacional híbrido utilizando CAG impregnado com óxido de cobre pode ser considerado como um tratamento potencial alternativo pontual para melhorar a qualidade da água destinada ao consumo humano.
Abstract This study evaluates the application of a polymeric microfiltration membrane in a gravitational filtration module and its combination with granular activated carbon (GAC) impregnated with copper, resulting in a hybrid process. The proposed system would be used to improve the quality of water for human consumption in developing countries. Permeate flux, pH, Escherichia coli removal, color, turbidity and free chlorine removals were evaluated in the applied process. Instrumental techniques, such as N2 adsorption at 77 K, scanning electron microscopy (SEM), transmission electron microscopy (TEM) and energy dispersive X-ray spectroscopy (EDX) analyses were used to characterize the proposed membrane and adsorbent. The GAC ensured higher chlorine removals, as well as higher permeate flux. Furthermore, the GAC impregnated with copper oxide nanoparticles exhibited higher Escherichia coli removal.
