Abstract The detection of Amaranthus palmeri in Aragon has encouraged efforts to identify the best strategies for its control in infested plots and to prevent its spread to other areas. Together with the different greenhouse and field trials carried out at CITA, the Center for Plant Health and Certification developed an important survey work that entails considerable costs for the administration. This paper collects all this information with the aim of estimating the losses associated with the invader in the affected areas, including yield losses depending on the density of A. palmeri, the costs for the farmer associated with the different control measures in his plot and the costs related to the survey and dissemination tasks that are being assumed by the administration. For this purpose, the data from the field trials and prospections carried out in the first two years of testing are incorporated.

Resumen La detección de Amaranthus palmeri en Aragón ha impulsado los esfuerzos por identificar cuáles son las mejores estrategias para su control en las parcelas infestadas y para evitar su expansión a otras zonas. Junto con los diferentes ensayos en invernadero y en campo desarrollados en el CITA, el Centro de Sanidad y Certificación Vegetal lleva a cabo una importante labor de prospección que conlleva considerables costes para la administración. En este trabajo se recopila toda esta información con el objetivo de estimar las pérdidas asociadas a la invasora en las zonas afectadas, incluyendo las pérdidas de rendimiento en función de la densidad de A. palmeri, los costes para el agricultor asociados a las diferentes medidas de control en su parcela y los costes relacionados con las tareas de prospección y divulgación que está asumiendo la administración. Para ello se incorporan los datos de los ensayos y prospecciones realizados en los dos primeros años de ensayos.

ABSTRACT Background: The use of mobile immunization teams has been proposed as a strategy to increase influenza vaccination (IV) coverage among healthcare workers (HCW), but has not been evaluated in Primary Healthcare (PHC). The objective of this work was to determine if the use of mobile immunization teams increases IV coverage among HCW of a basic health area in the Valencian Community. Methods: Community intervention trial that included all HCWs from a basic health area in the Valencian Community. The assignment was by conglomerates in one stage, with the HCWs of a health center as an intervention group and that of the rest of the centers as a control group. The intervention group was visited by a team consisting of a doctor and a nurse who offered on-site IV, while the control group did not receive such a visit. The independent variable was the visit of the mobile immunization team and the dependent variable was IV in the study season (2015-2016). Data analysis was done both for the total IV coverage and separately for sex, age, professional category and history of IV in the previous seasons. A McNemar test was used to compare frequency distributions of paired data. Results: The control group went from 14 (31.8%) vaccinated in the 2014-2015 season to 19 (45.2%) in the 2015-2016 season, while the intervention group went from 19 (30.6%) to 34 (54.8%). Among the total of the WHCs it went from 33 (31.3%) vaccinated to 53 (50.0%). This increase was significant. By groups, the increase was significant among those who received the visit of the mobile team (p = 0.0003), but not in the control group (p=0.18). Conclusions: The visit of a mobile immunization team is a significant factor favorable to IV among HCW in our setting.

RESUMEN Fundamentos: El uso de equipos móviles ha sido propuesto como estrategia para aumentar la cobertura de vacunación antigripal (VAG) entre el personal sanitario (PS), pero no ha sido evaluado en Atención Primaria (AP). El objetivo de este trabajo fue determinar si el uso de equipos móviles aumenta la cobertura de la VAG entre el PS de una zona básica de salud de AP de la Comunidad Valenciana. Métodos: Ensayo de intervención comunitaria que incluyó a todo el PS de AP de una zona básica de salud de la Comunidad Valenciana. La asignación fue por conglomerados en una etapa, con el PS de un centro de salud como grupo de intervención y el del resto de centros como grupo control. El PS del grupo de intervención recibió la visita de un equipo formado por un médico y un enfermero que ofreció in situ la vacunación antigripal, mientras que el grupo control no recibió tal visita. La variable independiente fue la visita del equipo móvil y la variable dependiente fue la VAG en la temporada de estudio (2015-2016). El análisis de los datos se realizó tanto para las coberturas de VAG totales como separadamente por sexo, edad, categoría profesional y antecedentes de VAG en las temporadas anteriores. Para comparar las distribuciones de frecuencia de datos apareados se usó la prueba de McNemar. Resultados: El grupo control pasó de 14 (31,8%) vacunados en la temporada 2014-2015 a 19 (45.2%) en la 2015-2016, mientras que el grupo de intervención pasó de 19 (30,6%) a 34 (54,8%). Entre el total del PS se pasó de 33 (31,3%) vacunados a 53 (50,0%). Este aumento fue significativo. Por grupos, el aumento fue significativo entre los que recibieron la visita del equipo móvil (p=0,0003), pero no en el grupo de control (p=0,18). Conclusiones: La visita de un equipo móvil es un factor significativo favorable a la VAG entre el PS de AP en nuestro medio.

Objective. To determine the proportion of healthcare professionals who were vaccinated against Swine flu, and their reasons for doing so. Design. Cross-sectional, descriptive observational study. Setting. Primary care and hospital pneumology department. Participants. A total of 138 doctors of whom 88 were family doctors, 30 house physicians in this speciality, 9 paediatricians and 11 pneumologists. Results. A total of 59.4% (95% CI: 50.8-68.0) of doctors had been vaccinated against seasonal flu and 31.2% (95% CI: 23.1-39.3) against swine flu. 15.2% considered that this vaccine should be compulsory. As regards reasons for not being vaccinated, 16.5% were afraid of possible side effects, 37% considered there were not enough clinical trials, 31.2% stated that the information was not clear enough, 9.4% thought the strain could have mutated and 15.9% did not consider themselves likely to transmit the virus. 67.4% of the doctors thought that the health authorities recommended the vaccine to health professionals in order to avoid infecting the patients, although 63.8% thought that it would avoid absenteeism. Conclusion. Vaccination against Swine flu was much less than against seasonal flu. Amongst other factors, this could be attributed to the lack of confidence in the vaccine amongst the health professionals.

Objetivo. Conocer la proporción de profesionales sanitarios vacunados frente a gripe estacional y gripe A, así como las motivaciones para hacerlo. Diseño del estudio. Estudio observacional, descriptivo, transversal. Emplazamiento. Atención primaria y servicio de neumología hospitalario. Participantes. 138 facultativos, de los que 88 eran médicos de familia, 30 residentes de esta especialidad, 9 pediatras y 11 neumólogos. Resultados. Se había vacunado frente a gripe estacional el 59,4% (IC95%: 50,8-68,0) y frente a gripe A el 31,2% (IC95%: 23,1-39,3). El 15,2% consideraba que esta vacuna debería ser obligatoria. En cuanto a las razones para no vacunarse, 16,5% mostraba miedo a posibles efectos secundarios, 37% consideraba que no había suficientes ensayos clínicos, 31,2% manifestaba que la información era poco clara, 9,4% pensaba que la cepa podría haber mutado, y un 15,9% no se consideraba especial transmisor. Un 67,4% de los médicos pensaba que las autoridades sanitarias recomiendan la vacuna a los profesionales para evitar el contagio a los pacientes, aunque para un 63,8% evitaría el absentismo laboral. Conclusión. El seguimiento de la vacunación frente a la gripe A ha sido mucho menor que frente a la gripe estacional. Entre otros factores habría que atribuirlo a la desconfianza que la vacuna generaba entre los profesionales sanitarios.

Background: Human Immunodeficiency Virus (HIV) is an occupational hazard among healthcare professionals accidentally contaminated with HIV-positive blood. This study is aimed at describing the characteristics of the accidents involving blood of HIV-positive patients recorded over a sixteen-year period at a general hospital. Methods: Epidemiological study of the accidents reported in 2001 involving biological material from an HIV-positive source by the healthcare personnel of a general hospital throughout the 1986-2001 period entailing the presence of biological material from HIV-positive serology individuals. Individual, time and place-related variables, in addition to the initial serologies and those throughout the protocolized follow-up were studied for those individuals involved in these accidents. Results: A total 550 accidents entailing an HIV-positive source were reported. The average number of accidents was 34.4/year. The accidental exposure rate for the period under study was 7.5 /1000 workers /year. The professional group showing the highest accident rate was the nursing staff (54.4%). Percutaneous injuries were the most frequent (80.2%). The mean exposure rate was 2.6/100 beds/year. The anatomical areas involved to the greatest degree were the fingers (75.6%). A total 53.4% of those injured completed the serological follow-up without having shown any seroconversion. Conclusions: Throughout the sixteen-year period under study, the annual incidence of accidents involving an HIV-positive source increased from the 27 accidents reported in 1986 to the 60 accidents reported in 1990, there having been a downward trend as of that point in time, to the point of 12 accidents having been recorded in 2001.

Fundamento: La contaminación por el virus de la inmunodeficiencia humana (VIH) es un riesgo ocupacional para los profesionales sanitarios accidentados contaminados con sangre positiva al VIH. El objetivo de nuestro estudio es describir las características de los accidentes registrados durante dieciséis años en un hospital general con sangre o fluidos biológicos procedentes de pacientes VIH positivos, determinar las tasas de exposición accidental en las diferentes categorías profesionales y comprobar el seguimiento realizado por los trabajadores del protocolo de accidentes establecido. Métodos: Estudio descriptivo en un hospital general de Madrid durante el período 1986 a 2001, de los accidentes con presencia de material biológico procedente de personas con serología VIH positiva. Se han estudiado variables del profesional accidentado relacionadas con persona, lugar y tiempo, además de las serologías al inicio y durante el seguimiento protocolizado. Resultados: Durante el período de estudio se declararon 550 accidentes con fuente VIH positiva. La media fue de 34,4 de accidentes por año. La tasa de exposición fue de 7,5 por cada 1.000 trabajadores-año. El colectivo profesional con mayor frecuencia de accidentes fue el personal de enfermería (54,4%). Las lesiones percutáneas fueron las más frecuentes (80,2%). La tasa media de exposición ha sido de 2,6 por cada 100 camas-año. Las zonas anatómicas más afectadas fueron los dedos de la mano (75,6%). El 53,6% de los accidentados finalizó el seguimiento serológico, sin registrarse ninguna seroconversión. Conclusiones: Durante los dieciséis años de estudio, la incidencia anual de accidentes con fuente VIH positiva fue en aumento desde los 27 accidentes declarados en 1986 hasta los 60 accidentes en 1990, con una tendencia descendente desde ese momento hasta contabilizarse 12 accidentes en el año 2001.
