A transmissão do vírus Oropouche (OROV) para novas regiões, juntamente com um aumento de casos e o surgimento de formas graves, anteriormente não suspeitas, expressas preocupações de saúde pública. Uma fazendeira grávida de 40 anos, com 30 semanas de gestação, desenvolvimento de febre, mialgia e dor de cabeça, com infecção por OROV confirmada por RT-qPCR. as avaliações maternas e fetais não foram inicialmente concluídas. No entanto, na semana seguinte, um paciente notou diminuição dos movimentos fetais, e o ultrassom confirmou a morte fetal. O diagnóstico molecular detectou o RNA do OROV em vários espécimes fetais. Este caso de transmissão vertical ressalta a necessidade urgente de proteger as mulheres grávidas, incorporar o OROV no diagnóstico diferencial de doenças febris e investigar mais profundamente os potenciais mecanismos patogênicos do vírus.

Uma granjera embarazada de 40 anos em 30 semanas de gestação apresentou febre, mialgia e dor de cabeça, e a infecção por OROV foi confirmada por RT-qPCR. As avaliações maternas e fetais não foram avaliadas inicialmente. No entanto, na semana seguinte, o paciente notou uma diminuição dos movimentos fetais e a ecografia confirmada a morte fetal. Os diagnósticos moleculares detectam ARN de OROV em múltiplas amostras fetais. Este caso de transmissão vertical justifica a necessidade urgente de proteger as mulheres embaraçadas, incorporar o OROV no diagnóstico diferencial de doenças febris e investigar mais o fundo dos possíveis mecanismos patogênicos do vírus.

Abstract Background Cutaneous melanoma is a neoplasm with a high mortality rate and risk of metastases to distant organs. The Breslow micrometric measurement is considered the most important factor for evaluating prognosis and management, measured from the granular layer to the deepest portion of the neoplasm. Despite its widespread use, the Breslow thickness measurement has some inaccuracies, such as not considering variations in the thickness of the epidermis in different body locations or when there is ulceration. Objective To evaluate the applicability of a modified Breslow measurement, measured from the basal membrane instead of from the granular layer, in an attempt to predict sentinel lymph node examination outcome and survival of patients with melanoma. Methods A retrospective and cross-sectional analysis was carried out based on the evaluation of slides stained with hematoxylin & eosin from 275 cases of melanoma that underwent sentinel lymph node biopsy from 2008 to 2021 at a reference center in Brazil. Results Analysis of the Cox model to evaluate the impact of the Breslow measurement and the modified Breslow measurement on survival showed that both methods are statistically significant. Logistic regression revealed a significant association between both measurements and the presence of metastasis in sentinel lymph nodes. RESUMO
ABSTRACT The hydrodynamic conditions to which hydraulic structures such as spillways and stilling basins of dams are submitted under given conditions favor the occurrence of cavitation, damaging the concrete surfaces. This damage may be intensified by the cumulative effect of the successive hydrologic events that characterize the operational regime of these structures. Thus, the purpose of the present article is to identify the propensity for damage caused by cavitation by using SpillwayPro software to estimate the hydraulic parameters of flow (mean velocities and cavitation indices) based on information on smooth chute spillways at the Furnas, Luiz Carlos Barreto de Carvalho and Batalha hydroelectric power plants. The results shown were compared to the critical limits of cavitation defined in the literature on photographic records of inspections performed in the field. The highest mean velocities estimated coincided with the zones where the worst potential damage of the structures was identified. Abstract Antimicrobial photodynamic therapy (aPDT) is an adjuvant treatment to scaling and root planing (SRP) which improves periodontal health. It may be beneficial to patients with systemic diseases, such as type 1 diabetes mellitus. Objective This randomized clinical trial evaluated the adjunctive effect of aPDT on the periodontal treatment of patients with type 1 diabetes (T1D). Methodology 38 patients were included in the study and divided into four groups: DSRP – T1D patients treated with SRP; CSRP – normoglycemic patients treated with SRP; DPDT – T1D patients treated with SRP + aPDT (methylene blue and red laser); CPDT – normoglycemic patients treated with SRP + aPDT. , Periodontal clinical parameters and inflammatory cytokines in crevicular fluid were recorded at baseline and then after 1, 3 and 6 months. The clinical endpoint for treatment was evaluated after 6 months. Results Adjuvant aPDT treatment resulted in reduction of probing depth after 3 months (0.38 mm - p<0.05) on T1D patients and in control group after 6 months (0.66 mm - p<0.05). Reduction of clinical attachment levels was similar for both treatments in control patients (p>0.05). There was a significant reduction of TNF-α in crevicular fluid in both groups treated with aPDT (p<0.05). The T1D (65%) and normoglycemic (72%) groups achieved the clinical endpoint after both treatments (p>0.05). Conclusions Adjuvant aPDT provided additional benefits in improving periodontal clinical parameters and reducing inflammatory cytokines in both T1D and normoglycemic patients. Objective: To report on suicide plans and attempts among Brazilian physicians and to investigate the associated risk factors. Methods: From January 2018 to January 2019, a nationwide online survey was conducted among Brazilian physicians using the Tool for Assessment of Suicide Risk and the Satisfaction with Life Scale. Multivariate exploratory associations of demographic, psychological, and work-related factors were performed on reports of suicide plans and attempts. Results: Of the 4,148 participants, 1,946 (53.5%) were male, 2,527 (60.9%) were 30 to 60 years old, 2,675 (64.5%) had two to four jobs, and 1,725 (41.6%) worked 40 to 60 hours a week. The overall prevalence of suicide plans was 8.8% (n=364), and suicide attempts were reported by 3.2% (n=133) of participants. Daily emotional exhaustion (ORadj = 7.857; 95%CI 2.282-27.051, p = 0.002), weekly emotional exhaustion (ORadj = 7.953; 95%CI 2.403-26.324, p = 0.001), daily frustration at work (ORadj = 3.093; 95%CI 1.711-5.588, p < 0.001), and bisexuality (ORadj = 5.083; 95%CI 2.544-10.158, p < 0.001) were significantly associated with higher odds of suicide. Extremely dissatisfied physicians reported suicide plans and attempts in 38.3% of cases, whereas extremely satisfied physicians reported suicide plans and attempts in only 2.8% of cases (p < 0.001). Conclusion: Brazilian physicians with a history of suicide plans and attempts express emotional exhaustion and frustration at work. There is an urgent need for actions to promote professional safeguards and resilience. Resumo Pesquisadores devem decidir sobre como modelar a resposta não linear do material em uma simulação com Elementos Finitos para avaliar a vulnerabilidade sísmica. Esse trabalho pretende auxiliar nessa tomada de decisão ao comparar os modelos de Fibra e de Plasticidade Concentrada em Elementos Finitos em análises não lineares estáticas e dinâmicas de um pórtico em concreto armado. A metodologia é baseada na engenharia de desempenho sísmico (PBEE) para determinar o dano nas estruturas. Os resultados indicam que os dois modelos apresentam uma boa correspondência em uma análise estática, e ao considerar os Estados Limite de Dano Extensivo e Completo na análise dinâmica. Abstract Researchers must decide on how they will model the non-linear material response in a Finite Element simulation to assess seismic vulnerability. This paper aims at giving an insight into these modelling decisions by comparing Fiber and Lumped Plasticity Finite Element Models in static and dynamic non-linear analysis in a RC frame. The methodology is based on the performance-based earthquake engineering to determine the expected damage on structures. The results indicate that both models are in good agreement with the static analysis, and when considering Extensive and Complete Damage Limit States on the dynamic analysis. Abstract The authors aim to study Religiosity/Spirituality (R/S) and Quality of Life (QoL) in patients with Crohn's disease and their correlation with the disease phenotypes. Methods Prospective cross-sectional cohort study with 151 consecutive patients enrolled from March 2021 to October 2021 at the Colorectal IBD Outpatient of Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP). Sociodemographic, Religiosity/Spirituality (Duke University Religion Index - Durel) questionnaires and QoL (Inflammatory Bowel Disease Questionnaire - Short IBDQ-S) were applied. When necessary, qualitative variables were evaluated using the chi-square or Fisher's exact test. The Mann-Whitney and Kruskall-Wallis tests were used to analyze quantitative variables and compare more than two groups, both non-parametric statistical techniques. Results The most frequent location was Ileocolonic followed by Ileal and colonic (41.1 %, 27.2 %, and 25.2 %); only 6.6 % of subjects had a perianal presentation. Inflammatory, stenosing, and penetrating behaviors showed 36.4 %, 19.1 %, and 44.4 % respectively. The majority of the population is Catholic, Evangelical, or Spiritualist (92.4 %). QoL score showed no significant difference in the phenotypes. The scores for DUREL domains were 61.4 % for organizational religiosity, 75 % for non-organizational religiosity, 98.6 %, 93.6 % and 89.3 % for intrinsic spirituality, with high results in all disease phenotypes. Conclusions The studied population presented homogeneous sociodemographic results and high religious and spiritual activity. R/S in a positive context were not associated with better QoL or phenotype. R/S is present in the patients’ lives and could be seen as an important tool for adherence to treatment and the professional relationship between doctor and patient. The homogeneity of the sample difficult for an appropriate evaluation, which leads us to suggest new studies with more heterogeneous groups. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Introduction This article describes an unusual case of perianal Paget's Disease in a 42-year-old man, highlighting distinct characteristics about more typical cases. Case report 42-year-old male patient with persistent perianal lesion, recurrent abscesses, and discomfort. Magnetic resonance imaging indicated a possible fistulous path. Biopsy revealed secondary anal Paget's disease and adenocarcinoma. Chemotherapy and radiotherapy treatment demonstrated a positive response. Discussion Perianal Paget's Disease is a rare condition of the anal canal that is part of the Extramammary Paget's Disease (EMPD) group. Symptoms include itching, pain, perianal bleeding, and skin lesions. Diagnosis can be challenging and usually involves performing a histopathological biopsy associated with an immunohistochemical study. Treatment varies and may involve surgery, radiotherapy, and chemotherapy. Due to the rarity of the disease, the best therapeutic approach is not yet well defined, and follow-up is essential due to the risk of recurrence. Conclusion Perianal Paget's Disease is rare and requires differentiation from other conditions. Early detection is essential to prevent malignancy. Abstract Objective: To assess the predictive value of selected growth phenotypes for neonatal morbidity and mortality in preterm infants < 30 weeks and to compare them with INTERGROWTH-21st (IG21). Methods: Retrospective analysis of data from the Brazilian Neonatal Research Network (BNRN) database for very low birth weight (VLBW) at 20 public tertiary-care university hospitals. Outcome: the composite neonatal morbidity and mortality (CNMM) consisted of in-hospital death, oxygen use at 36 weeks, intraventricular hemorrhage grade 3 or 4, and Bell stage 2 or 3 necrotizing enterocolitis. Selected growth phenotypes: small-for-gestational-age (SGA) defined as being < 3rd (SGA3) or 10th (SGA10) percentiles of BW, and large-for-gestational-age (LGA) as being > 97th percentile of BW. Stunting as being < 3rd percentile of the length and wasting as being < 3rd percentile of BMI. Single and multiple log-binomial regression models were fitted to estimate the relative risks of CNMM, comparing them to IG21. Results: 4,072 infants were included. The adjusted relative risks of CNMM associated with selected growth phenotypes were (BNRN/IG21): 1.45 (0.92–2.31)/1.60 (1.27–2.02) for SGA; 0.90 (0.55–1.47)/1.05 (0.55–1.99) for LGA; 1.65 (1.08–2.51)/1.58 (1.28–1.96) for stunting; and 1.48 (1.02–2.17) for wasting. Agreement between the two references was variable. The growth phenotypes had good specificity (>95%) and positive predictive value (70-90%), with poor sensitivity and low negative predictive value. Conclusion: The BNRN phenotypes at birth differed markedly from the IG21 standard and showed poor accuracy in predicting adverse neonatal outcomes. resumo: Investigações experimentais têm sido comumente usadas para melhorar o conhecimento existente sobre o projeto estrutural, apresentando conclusões precisas sobre o comportamento da estrutura. No entanto, considerando as limitações dos estudos experimentais, como quantidade restrita de extensômetros, patologias inesperadas, dificuldades de previsão do correto posicionamento das fissuras ou simples economia de custos, o uso da análise numérica pode apresentar algumas abordagens inovadoras para a compreensão do processo de falha em elementos de concreto, apresentando resultados mais fáceis onde programas experimentais dificilmente podem relatar. Este estudo apresenta a análise numérica de oito vigas experimentadas por Sherwood [26], com dois tamanhos diferentes e agregados de tamanhos variados, buscando-se entender a influência do tamanho do agregado na resistência ao cisalhamento de vigas sem estribos. A abordagem numérica foi usada para avaliar a influência de cada mecanismo de cisalhamento interno e para identificar a quantidade específica de força de pino, transferência de cisalhamento na zona de compressão não fissurada e a parcela de intertravamento de agregados. Os resultados mostraram que resultados razoáveis podem ser obtidos por abordagens de fissuras distribuídas 2D, permitindo a identificação da maior porção de intertravamento do agregado em vigas com maiores tamanhos de agregado graúdo. A comparação do efeito de escala em vigas permitiu concluir que uma maior contribuição do intertravamento de agregados pode ser obtida em vigas grandes, com quase 64% da contribuição total, enquanto as vigas menores tiveram apenas 43%. Os resultados de Sherwood [26] foram comparados com as normas Model Code [23], CSA-A.23.3 [24], NBR 6118 [38] e ACI 318 [39] para avaliar a precisão das normas estudadas. Com relação a previsão da ruptura dos elementos por meio dos mecanismos internos do concreto, as instruções normativas Model Code [23] e CSA A.23.3 [24] resultaram valores mais precisos.
abstract: Experimental investigations have been commonly used to improve the existing knowledge about structural design, presenting accurate conclusions regarding structure behavior. However, considering the limitations of experimental studies, such as restricted amount of strain gauges, unexpected pathologies, difficulties to foresee the correct position of cracks, or simply saving in costs, the use of numerical analysis can present some innovative approaches to understand the process of failure in concrete elements, presenting easier results where experimental programs can hardly report. This study presents the numerical analysis of eight beams experimented by Sherwood [26], with two different sizes and variable aggregate sizes, seeking to understand the influence of coarse aggregate size in shear strength of beams without stirrups. The numerical approach was used to derive the influence of each internal shear mechanism and to identify the specific amount of dowel force, shear transfer by uncracked compression zone and aggregate interlock portion. The results showed that fair results can be obtained by 2D smeared crack approaches, enabling the identification of major aggregate interlock portion in beams with bigger coarse aggregate sizes. Comparing the size effect in beams allowed us to conclude that a higher contribution of aggregate interlock contribution can be obtained in large beams, with almost 64% of the total contribution, whereas smaller beams had only 43%. To evaluate the accuracy of the studied mechanisms, the results of Sherwood [26] were compared with the standards Model Code [23], CSA-A.23.3 [24], NBR 6118 [38], and ACI 318 [39]. Regarding the prediction of element rupture by concrete internal mechanisms, the normative instructions Model Code [23] and CSA A.23.3 [24] were the closest to the experimental results.
