RESUMO A pandemia de covid-19 se impôs de modo mais agudo e dramático aos grupos políticos e socialmente mais vulnerabilizados, como a População em Situação de Rua (PSR). A literatura sobre as respostas às demandas de saúde e de assistência social dos mais vulnerabilizados, durante a pandemia, ainda é escassa. Neste artigo, descreve-se a rede de cuidado à PSR, considerando os caminhos formalmente instituídos e aqueles produzidos durante a pandemia de covid-19, no município de Belo Horizonte. Para tanto, realizaram-se análise documental, visitas técnicas a serviços assistenciais, entrevistas em profundidade e grupos focais com atores que compõem a rede de cuidado. Os resultados mostraram que a rede é composta por quatro componentes com interseções: pela rede da saúde, da assistência social, da Pastoral Nacional do Povo da Rua (sociedade civil organizada) e a rede criada para cuidado de sintomáticos respiratórios. As mudanças de fluxos impostas pela pandemia repercutiram em dificuldades no acesso aos serviços de cuidado da PSR. ABSTRACT The COVID-19 pandemic has imposed itself in a acutely and dramatically way, particularly on politically and socially vulnerable groups, such as the homeless population (HP). The literature on health emergency and response and social care needs of the most vulnerable during the pandemic is scarce. This article describes the network of care for HP, including formally instituted pathways of care and those produced during the pandemic of COVID-19, in the city of Belo Horizonte. The methodological path used documentary analysis, technical visits to care services, in-depth interviews and focus groups with the care network stakeholders. The results showed that the network is composed of four components with intersections: the health network, the social care network, the ‘National Pastoral Care of the Homeless People’ (non-governmental organization) and the network created to care for people with respiratory symptoms. The workflow changes imposed by the pandemic resulted in access barriers in different centers offering care for HP. However, it was observed the potentiality of the integral care offer, through the connections built during the pandemic between health, social care, and the non-governmental organizations. ABSTRACT OBJECTIVE The transgender population in Brazil faces marginalization and difficulties in accessing education and health, leading many individuals to self-medicate. This study aimed to evaluate the impact of the implementation of Specialized Centers in the Transsexualizing Process (SCTP) on the use of cross-sex hormone therapy (CSHT) without medical prescription, as well as the level of education and mental health profile of these individuals. METHODS This is a cross-sectional study with data from physical and electronic medical records between September 2017 and February 2023 regarding the use of CSHT before and after the implementation of two SCTP in the state of Bahia, Brazil, in addition to data on education level, previous diagnosis of anxiety and depression of patients. RESULTS A total of 219 participants, 127 transgender men (TM) and 92 transgender women and travestis (TrTW), were assessed. A significant reduction in the prevalence of self-medication was observed in both TrTW (92.98% before and 51.43% after, p<0.001), and TM (47.17% before and 25.67% after, p = 0.010) with the implementation of SCTP. Transgender individuals who used CSHT before accessing the service were found to have a lower prevalence of depression. Self-medication was not significantly associated with education or anxiety in our sample. CONCLUSION The results indicate the need for the expansion of SCTP, as they were associated with lower rates of self-medication in the transgender population.

Abstract Associations between the WNT5A rs566926 variant and non-syndromic orofacial cleft (NSOC) have been reported in different populations. Objective This study aimed to investigate the role of the rs566926 single nucleotide polymorphism (SNP) in WNT5A and its interactions with SNPs in BMP4, FGFR1, GREM1, MMP2, and WNT3 in the occurrence of NSOC in a Brazilian population. Methodology A case-control genetic association study was carried out involving participants from four regions of Brazil, totaling 801 patients with non-syndromic cleft lip with or without cleft palate (NSCL±P), 273 patients with cleft palate only (NSCPO), and 881 health volunteers without any congenital condition (control). Applying TaqMan allelic discrimination assays, we evaluated WNT5A rs566926 in an ancestry-structured multiple logistic regression analysis, considering sex and genomic ancestry as covariates. Interactions between rs566926 and variants in genes involved in the WNT5A signaling pathway (BMP4, FGFR1, GREM1, MMP2, and WNT3) were also explored. Results WNT5A rs566926 was significantly associated with an increased risk of NSCL±P, particularly due to a strong association with non-syndromic cleft lip only (NSCLO), in which the C allele increased the risk by 32% (OR: 1.32, 95% CI: 1.04–1.67, p=0.01). According to the proportions of European and African genomic ancestry, the association of rs566926 reached significant levels only in patients with European ancestry. Multiple interactions were detected between WNT5A rs566926 and BMP4 rs2071047, GREM1 rs16969681 and rs16969862, and FGFR1 rs7829058. Conclusion The WNT5A rs566926 polymorphism was associated with NSCL±P, particularly in individuals with NSCLO and high European ancestry. Epistatic interactions involving WNT5A rs566926 and variants in BMP4, GREM1, and FGFR1 may contribute to the risk of NSCL±P in the Brazilian population. Resumo Esta revisão visa identificar e descrever intervenções realizadas para promover o clima ético em instituições de saúde. Foi feita revisão de escopo nas bases PubMed, Biblioteca Virtual em Saúde, EBSCOhost e Google Acadêmico, entre outubro e dezembro de 2022. Os dados foram analisados mediante síntese numérica e temática. Foram incluídos três estudos: dois quase-experimentais e um experimental. Médicos, enfermeiros e outros membros da equipe participaram da pesquisa. São descritas intervenções realizadas e resultados obtidos. As intervenções foram workshops, rodadas de ética e protocolo de ação precoce. A duração variou de duas semanas a seis meses. Houve diferença do clima ético para todos os participantes em um estudo; apenas para enfermeiros em outro; e não houve diferença em uma das pesquisas. Poucos artigos implementam e avaliam intervenções para promover o clima ético. Abstract This review aims to identify and describe interventions to promote an ethical climate in health institutions. Method: a scope review carried out on PubMed, Virtual Health Library, EBSCOhost, and Google Scholar from October to December 2022. Data were analyzed using numerical and thematic synthesis and included two quasi-experimental and one experimental studies. Physicians, nurses, and other team members participated in this review, which describes the carried out interventions and obtained results. The interventions included workshops, ethics rounds, and an early action protocol that lasted from two weeks to six months. This review found a difference in the ethical climate for all participants in one study, only for nurses in another study, and no significant differences in the last studies. Few studies implement and evaluate interventions to promote an ethical climate. Los datos se analizaron mediante síntesis numérica y temática. Se incluyeron tres estudios: dos cuasiexperimentales y uno experimental. Abstract The objective was to evaluate the physicochemical characteristics, nutritional composition and fatty acid profile of eleven exotic fruits in Brazil Northeast. The fruits, except pequi, presented acid pH, high moisture, low protein, low lipid and low energetic contents. Pequi is highlighted by its high protein content (2.79 g.100 g-1), lipid (13.6 g.100 g-1), carbohydrates (28.71 g.100 g-1), titratable acidity (2.75 g citric acid.100 g-1) and pH (2.54-5.19). Unpeeled and peeled jenipapo presented higher ash composition (1.26-1.38g.100 g-1), soluble solids (20.29-21.17 ºBrix) and carbohydrates (22.55-23.66 g.100 g-1) compared to others fruits. Fourteen fatty acids were quantified and classified as saturated fatty acids, monounsaturated fatty acids and polyunsaturated fatty acids. The concentrations of total fatty acids ranged from 1.92 to 1293.21 mg.100 g-1, being palmitic acid and oleic acid more prevalent. Resumo Atualmente, Myloplus schomburgkii representa a espécie mais facilmente reconhecida entre os serrasalmídeos, por apresentar uma barra preta vertical no meio do corpo. No entanto, através de uma ampla revisão taxonômica, incluindo DNA barcoding e análises morfológicas, conseguimos identificar e descrever duas novas espécies que também compartilham a barra vertical escura no flanco. Além disso, redescrevemos M. schomburgkii, designando um neótipo e restringindo a localidade-tipo ao rio Negro, Barcelos, Estado do Amazonas, Brasil. As três linhagens de pacus de barra preta apresentam altas divergências moleculares (7,9–11%) e podem ser morfologicamente distinguidas por diferenças na forma da barra vertical, forma da nadadeira anal nas fêmeas, número de vértebras totais, número de raios ramificados na nadadeira dorsal, entre outros caracteres. Embora a existência dessas duas novas espécies tenha sido ocultada devido a grandes semelhanças morfológicas, principalmente pela presença da barra vertical preta, as três linhagens não compõem um grupo monofilético, com uma das novas espécies sendo recuperada como irmã de Ossubtus xinguense. Este resultado reforça a necessidade da redefinição dos gêneros de Myleini. Abstract Presently, Myloplus schomburgkii is the most easily recognized species among the serrasalmids by having a vertical black bar in the middle of the body. However, through a broad taxonomic review, including DNA barcoding and morphological analyses, we were able to identify and describe two new species that also share a dark vertical bar on the flank. In addition, we redescribe M. schomburgkii, designating a neotype and restricting the type-locality to rio Negro in Barcelos, Amazonas State, Brazil. The three lineages of black-barred pacus present high molecular divergences (7.9–11%) and can be distinguished by differences in the shape of the vertical bar, shape of females’ anal fin, number of total vertebrae, number of total branched dorsal-fin rays, among other characters. Although the existence of these two new species has been hidden due to many morphological similarities, mainly the presence of the black bar, the three lineages do not compose a monophyletic group, with one of the new species being recovered as sister to Ossubtus xinguense. This result reinforces the necessity of the redefinition of the Myleini genera. ABSTRACT Objective To evaluate deaths, hospitalizations, and persistence of symptoms in patients with COVID-19 after infection in an outpatient setting during the first COVID-19 wave in Brazil. Methods This prospective cohort was between April 2020 and February 2021. Hospitalized or non-hospitalized COVID-19 patients until five days after symptom onset were included. The outcomes measured were incidence of death, hospitalization, and persistence of more than two symptoms 60 days after discharge. Results Out of 1,198 patients enrolled in the study, 66.7% were hospitalized. A total of 289 patients died (1 [0.3%] non-hospitalized and 288 [36%] hospitalized). At 60 days, patients non-hospitalized during admission had more persistent symptoms (16.2%) compared to hospitalized (37.1%). The COVID-19 severity variables associated with the persistence of two or more symptoms were increased age (OR= 1.03; p=0.015), respiratory rate at hospital admission (OR= 1.11; p=0.005), length of hospital stay of more than 60 days (OR= 12.24; p=0.026), and need for intensive care unit admission (OR= 2.04; p=0.038). Conclusion COVID-19 survivors who were older, tachypneic at admission, had a hospital length of stay >60 days, and were admitted to the intensive care unit had more persistent symptoms than patients who did not require hospitalization in the early COVID-19 waves. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo No Brasil, a infeção por anemia infecciosa equina (AIE) está disseminada por diversas regiões, com níveis de prevalência variáveis. O objetivo deste trabalho foi determinar as características epidemiológicas que poderiam contribuir para a disseminação da AIE na região oeste do Rio Grande do Sul, Brasil. Para isso, foi aplicado um questionário epidemiológico aos proprietários de equinos que tiveram animais positivos para AIE em suas propriedades no período de 2009 a 2019. Os entrevistados foram contatados por meio de ligações telefônicas, e as perguntas visavam identificar padrões entre as propriedades, sobre o manejo dos animais, a sanidade e o conhecimento do proprietário sobre a infeção. As informações primárias foram obtidas nos arquivos do Serviço Veterinário Oficial (SVO), compiladas e tabuladas. Adicionalmente, o mesmo questionário foi aplicado a propriedades sem registo de infeção, designadas controle. Os resultados foram analisados por meio de regressão logística forward para explorar potenciais associações de risco. De um total de 123 propriedades focais, 28 entrevistas foram completadas, seis foram interrompidas ou recusadas, 55 não responderam aos telefonemas e 34 tinham informação de contato desatualizada. Entre as 30 propriedades controle, 15 concordaram em participar do inquérito. Os resultados sugerem que as práticas de criação, gestão e controle sanitário são semelhantes entre as propriedades foco e as controle. Apenas dois fatores de risco puderam ser distinguidos das propriedades positivas e negativas, sendo estes o trânsito de animais da propriedade e o contato com outros cavalos. Além disso, observou-se que os registos oficiais dos produtores está desatualizado para um número significativo de propriedades. Abstract In Brazil, equine infectious anemia (EIA) infection is widespread in various regions, with variable prevalence levels. The objective of this work was to determine the epidemiological characteristics that could contribute to its dissemination on the western border of Rio Grande do Sul, Brazil. To accomplish this, an epidemiological questionnaire was administered to equine owners who had experienced EIA-positive animals on their properties from 2009 to 2019. The interviewees were contacted via phone calls, and the questions aimed to identify patterns among properties, on animal management, health, and owner’s knowledge about the infection. The primary information was acquired from the data of the Official Veterinary Service (OVS) through data compilation and tabulation of the official forms. Additionally, the same questionnaire was applied to properties without records of the infection, as control. The results were analyzed using forward logistic regression to explore potential risk associations. Out of a total of 123 focal properties, 28 interviews were completed, six were interrupted or declined, 55 did not respond to phone calls, and 34 had outdated contact information. Among the 30 control properties, 15 agreed to participate in the survey. Abstract Objective: To describe the speech pattern of patients with hereditary Spastic Paraplegia type 4 (SPG4) and correlated it with their clinical data. Methods: Cross-sectional study was carried out in two university hospitals in Brazil. Two groups participated in the study: the case group (n = 28) with a confirmed genetic diagnosis for SPG4 and a control group (n = 17) matched for sex and age. The speech assessment of both groups included: speech task recording, acoustic analysis, and auditory-perceptual analysis. In addition, disease severity was assessed with the Spastic Paraplegia Rating Scale (SPRS). Results: In the auditory-perceptual analysis, 53.5% (n = 15) of individuals with SPG4 were dysarthric, with mild to moderate changes in the subsystems of phonation and articulation. On acoustic analysis, SPG4 subjects’ performances were worse in measurements related to breathing (maximum phonation time) and articulation (speech rate, articulation rate). The articulation variables (speech rate, articulation rate) are related to the age of onset of the first motor symptom. Conclusion: Dysarthria in SPG4 is frequent and mild, and it did not evolve in conjunction with more advanced motor diseases. This data suggest that diagnosed patients should be screened and referred for speech therapy evaluation and those pathophysiological mechanisms of speech involvement may differ from the length-dependent degeneration of the corticospinal tract. RESUMEN Objetivo analizar la percepción de los enfermeros en relación a la teleconsulta de enfermería en la atención inicial de salud a través de la caracterización de los flujos de trabajo, potenciales, desafíos y la viabilidad de la teleconsulta en enfermería. Método estudio exploratório-descriptivo, con abordaje cualitativa. Los datos son colectados con 19 enfermeros que actúan en cuatro distritos sanitarios de una capital del sur del Brasil, utilizando un cuestionario digital. La colecta de los datos fue realizada entre los meses de mayo a junio de 2021 de forma totalmente online; los datos fueron sistematizados en el Microsoft Excel y analizados según los criterios del Discurso del sujeto colectivo, que utiliza las representaciones sociales y el senso común retratado la manifestación o posicionamento de cada individuo y posibilita agrupamientos de pensamientos y/o opiniones individuales sobre determinado asunto. Ese proceso fue realizado con el apoyo del Programa DSCsoft. Resultados los resultados fueron organizados en cinco categorías temáticas: Flujo de la teleconsulta de enfermería en la Atención Inicial de Salud; Proceso de realización de la teleconsulta; Instrumentos utilizados a lo largo de la realización de la teleconsulta; Potencialidades y desafíos identificados a lo largo de la teleconsulta en la Atención Inicial de Salud; y, Aplicabilidad: atendimientos pasibles de resoluciones por medio de la teleconsulta. ABSTRACT Objective analyze the nurse’s perception about nursing teleconsultation in Primary Health Care through the workflows characterization, potential, challenges and teleconsultation in nursing feasibility. Method exploratory-descriptive study with a qualitative approach. Data were collected, between the months of May and June 2021, with 19 nurses from four health districts of a capital in south of Brazil, using digital questionnaire. Data were systematized in Microsoft ExcelⓇ and analyzed following the criteria of the Discourse of the Collective Subject. This process was carried out with the support of the DSCsoftⓇ Program. Results the results were organized into five thematic categories: Nursing teleconsultation flow in Primary Health Care; Teleconsultation process; Instruments used during the teleconsultation; Potentialities and challenges identified during the teleconsultation in the Primary Health Care; and, Applicability: services that can be resolved through teleconsultation. RESUMO: Crianças e adolescentes com Transtorno do Espectro Autista (TEA) apresentam características clínicas que podem levá-las a terem desafios em compreender e participar do novo contexto mundial e estarem sujeitas a sofrerem os impactos causados pela pandemia da covid-19. Nesse sentido, o objetivo deste estudo foi analisar se a pandemia trouxe impactos para crianças e adolescentes com TEA. Trata-se de uma revisão sistemática realizada nas bases de dados eletrônicas PubMed, DOAJ, BVS, SciELO e Oásis. Foram incluídos apenas estudos que tiveram como público crianças e adolescentes com TEA. Após as buscas, um total de 28 artigos foram incluídos para a análise da revisão. Os resultados apontam que aspectos relacionados ao comportamento, à saúde mental, à rotina, à interrupção dos atendimentos presenciais e ao contexto familiar e escolar apresentaram maiores impactos negativos, enquanto comunicação e socialização tiveram impactos positivos. Não foram identificados muitos estudos em relação aos impactos da pandemia sobre os aspectos motores e a mudança dos atendimentos para telemonitoramento. Concluiu-se que a pandemia causou impactos significativos em crianças e adolescentes com TEA; assim sendo, com a retomada das atividades presenciais, esse público precisará de maior atenção. ABSTRACT: Children and teenagers with Autism Spectrum Disorder (ASD) have clinical characteristics that can lead them to challenges in understanding and participating in the new world context and they are more likely to suffer the impacts caused by the COVID-19 pandemic. In this sense, the goal of this study was to analyze whether the pandemic has had an impact on children and teenagers with ASD. This is a systematic review carried out in the electronic databases PubMed, DOAJ, BVS, SciELO and Oásis. Only studies that had children and teenagers with ASD were included. After the searches, a total of 28 articles were included for review analysis. The results show that aspects related to behavior, mental health, routine, interruption of face-to-face attendance and family and school context had greater negative impacts, while communication and socialization had positive impacts. Not many studies were identified regarding the impacts of the pandemic on the motor aspects and the change of calls to telemonitoring. It is concluded that the pandemic caused significant impacts on children and adolescents with ASD, so, with the resumption of face-to-face activities, this audience will need more attention. Abstract Background Rheumatoid arthritis is an autoimmune inflammatory disease that often leads patients to muscle impairment and physical disability. This study aimed to evaluate changes in the activity of proteasome system in skeletal muscles of mice with collagen-induced arthritis (CIA) and treated with etanercept or methotrexate. Methods Male DBA1/J mice were divided into four groups (n = 8 each): CIA-Vehicle (treated with saline), CIA-ETN (treated with etanercept, 5.5 mg/kg), CIA-MTX (treated with methotrexate, 35 mg/kg) and CO (healthy control group). Mice were treated two times a week for 6 weeks. Clinical score and hind paw edema were measured. Muscles were weighted after euthanasia and used to quantify proteasome activity, gene (MuRF-1, PMSα4, PSMβ5, PMSβ6, PSMβ7, PSMβ8, PSMβ9, and PSMβ10), and protein (PSMβ1, PSMβ5, PSMβ1i, PSMβ5i) expression of proteasome subunits. Results Both treatments slowed disease development, but only CIA-ETN maintained muscle weight compared to CIA-MTX and CIA-Vehicle groups. Etanercept treatment showed caspase-like activity of 26S proteasome similar to CO group, while CIA-Vehicle and CIA-MTX had higher activity compared to CO group (p: 0.0057). MuRF-1 mRNA expression was decreased after etanercept administration compared to CIA-Vehicle and CO groups (p: 0.002, p: 0.007, respectively). PSMβ8 and PSMβ9 mRNA levels were increased in CIA-Vehicle and CIA-MTX compared to CO group, while CIA-ETN presented no difference from CO. PMSβ6 mRNA expression was higher in CIA-Vehicle and CIA-MTX groups than in CO group. Protein levels of the PSMβ5 subunit were increased in CO group compared to CIA-Vehicle; after both etanercept and methotrexate treatments, PSMβ5 expression was higher than in CIA-Vehicle group and did not differ from CO group expression (p: 0.0025, p: 0.001, respectively). The inflammation-induced subunit β1 (LMP2) was enhanced after methotrexate treatment compared to CO group (p: 0.043). Conclusions The results of CIA-Vehicle show that arthritis increases muscle proteasome activation by enhanced caspase-like activity of 26S proteasome and increased PSMβ8 and PSMβ9 mRNA levels. Etanercept treatment was able to maintain the muscle weight and to modulate proteasome so that its activity and gene expression were compared to CO after TNF inhibition. The protein expression of inflammation-induced proteasome subunit was increased in muscle of CIA-MTX group but not following etanercept treatment.
