ABSTRACT Purpose: To evaluate the peritoneal histopathological changes and culture after the use of intravenous meropenem and intra-abdominal inoculation of 10% aqueous extract of anacardiaceae, in elderly rat model after autogenous fecal peritonitis induced. Methods: Thirty 18-month-old Wistar rats received induction of autogenous fecal peritonitis and then were stratified into two groups: study I, treated with meropenem (40 mg/kg); and study II, treated with meropenem (40 mg/kg) and intraperitoneal 10% aqueous extract of anacardiaceae. Animals were monitored for 15 days until euthanasia. Peritoneal fragments were collected for histopathological and culture. The study was approved by Ethics Committee. Results: None study-II animals died, while in study I, one died before euthanasia. In study II, 20% of the animals showed histopathological changes, none positive peritoneal culture, but one blood culture was positive (10%). In study I, 50% of the animals presented histopathological changes, 40% positive peritoneal cultures, and 50% positive blood cultures. All results when evaluated in the morbidity score showed better outcome for study-II group (p = 0,175). Conclusion: The use of meropenem associated with intraperitoneal 10% aqueous anacardiaceae extract after induction of autogenous fecal peritonitis in elderly rats showed better outcome in the set of histopathological changes, negative peritoneal and blood cultures, when compared with the use of meropenem isolated. Caracterización de apendicitis aguda en pacientes pediátricos. Cienfuegos, 2013-2018

ABSTRACT Foundation: The acute appendicitis’ behavior study offers a general view of its etiology and severe forms; guarantees the action protocols adequacy in order to improve specialized medical care. Objective: To characterize the acute appendicitis behavior in pediatric patients. Methods: A descriptive study was carried out at the Paquito González Cueto University Pediatric Hospital, in Cienfuegos, from 2013 to 2018. For the total cases corresponding to the studied period (N= 933), the analyzed variables were: age, sex, year of diagnosis, municipality of origin, type of acute appendicitis and complications presence. Descriptive statistics and epidemiological indicators were applied. Results: Patients between the ages of 10 and 14 years (57.87%) predominated, as well as males (54.34%), for an incidence rate of 17.65 cases per 10.000 inhabitants. The Cienfuegos municipality was the most representative, with 377 patients (40.41%). The cases with acute catarrhal appendicitis (75.34%) and uncomplicated forms (91.63%) stood out. Conclusions: Acute appendicitis behaved variably depending on the temporal relationship; in correspondence with the ages and sex of highest incidence. It manifested itself mainly in its milder stages, related to uncomplicated forms.
RESUMEN Fundamento: El estudio del comportamiento de la apendicitis aguda ofrece una visión general sobre su etiología y formas graves; garantiza la adecuación de los protocolos de actuación en aras de mejorar la asistencia médica especializada. Objetivo: Caracterizar el comportamiento de la apendicitis aguda en pacientes pediátricos. Métodos: Se realizó un estudio descriptivo, en el Hospital Pediátrico Universitario Paquito González Cueto, de Cienfuegos, en el periodo 2013-2018. Para el total de casos correspondiente al periodo de estudio (N= 933), se analizaron las variables: edad, sexo, año del diagnóstico, municipio de procedencia, tipo de apendicitis aguda y presencia de complicaciones. Se aplicó la estadística descriptiva e indicadores epidemiológicos. Resultados: Predominaron los pacientes de edades entre 10 y 14 años (57,87 %), así como los de sexo masculino (54,34 %), para una tasa de incidencia de 17,65 casos por 10 000 habitantes. El municipio de Cienfuegos fue el más representativo, con 377 pacientes (40,41 %). Destacaron los casos con apendicitis aguda catarral (75,34 %), y las formas no complicadas (91,63 %). Conclusiones: La apendicitis aguda se comportó de manera variable según la relación temporal; en correspondencia con las edades y sexo de mayor incidencia. Se manifestó fundamentalmente en sus etapas más leves, relacionadas con las formas no complicadas.

ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT Background: This study aimed to evaluate the effectiveness and safety of direct-acting antivirals (DAAs) for hepatitis C treatment by measuring sustained virologic response (SVR) and serious adverse events to help design effective interventions for reducing disease prevalence. Methods: This was a retrospective, observational, real-life study of patients with chronic hepatitis C receiving DAA treatment in the state of Ceará, Brazil. Data were collected in REDCap and analyzed using R® software by the Student's t, chi-square, and Fisher’s exact tests, with a significance level of 5%. Results: In this study, 1075 patients who were diagnosed with hepatitis C infection between October 2015 and October 2023 were included. The mean age of the participants was 56.6 ± 11 years and 60.2% were men. The sample included 51 HIV-infected patients (6.6%), 166 (15,4%) liver transplant recipients, 34 (3,1%) kidney transplant recipients, and 446 patients with cirrhosis (41.4%). The overall SVR rate was 96.4%. The sofosbuvir/daclatasvir/ribavirin regimen used in 354 (32.9%) patients achieved an SVR of 96%. The cure rate was 96.5%, with a lower SVR in patients with cirrhosis (93.4%) than in those with less severe fibrosis (97.9%) (p=0.0015). Serious adverse events associated with ribavirin use occurred in 3.5% of patients. Conclusions: DAA treatment for hepatitis C achieved SVR in real life in all patient profiles, including transplant recipients, HIV carriers, and patients with cirrhosis. RESUMO Objetivo: A Oftalmologia é a área médica dedicada a cuidar da saúde ocular dos indivíduos e está presente em todos os meios de atenção à saúde. Sua correta compreensão é imprescindível para o correto manejo das afecções oculares e visuais. O objetivo do estudo foi analisar o nível do conhecimento oftalmológico geral dos acadêmicos de Medicina que já cursaram a disciplina de Oftalmologia de uma universidade do Sul de Santa Catarina. Métodos: O estudo foi do tipo transversal. A coleta de dados se deu por meio da plataforma Formulários Google. Foram coletados 242 formulários de acadêmicos do 6° ao 12° semestre. Resultados: Verificou-se uma média geral de acerto de 32,7%, nenhuma questão obteve mais de 80% de acerto, e não houve relação entre o semestre do acadêmico com seu desempenho. O percentual de acerto entre o gênero feminino foi de 34,1% (143), contra 30,6% (99) do gênero masculino. Conclusões: Concluiu-se que o nível de conhecimento oftalmológico dos acadêmicos foi insatisfatório em todos os semestres analisados, e que medidas para subsidiar melhorias curriculares podem ser empregadas, visando um melhor conhecimento em Oftalmologia aos futuros médicos. ABSTRACT Objective: Ophthalmology is the medical area dedicated to taking care of the eye health of individuals and is present in all means of health care. Its correct understanding is essential for the correct management of ocular and visual disorders. The objective of the study was to analyze the level of general ophthalmological knowledge of medical students who have already attended the course of ophthalmology at a university in the South of Santa Catarina. Methods: This is a cross-sectional study. Data collection took place through the Google Forms platform. Two hundred forty-two forms of students from the 6th to the 12th semester were collected. Results: There was a general average of 32.7% correct answers, no question obtained more than 80% correct answers, and there was no relation between the student's semester and their performance. The percentage of correct answers among women was 34.1% (143), against 30.6% (99) for men. Conclusions: It was concluded that the level of ophthalmological knowledge of academics was unsatisfactory in all semesters analyzed, and that measures to support curriculum improvements can be used, aiming at better knowledge in ophthalmology for future doctors. Resumen La carga no controlada de vehículos eléctricos plantea un gran desafío para los operadores de redes de distribución y los planificadores de sistemas de potencia. En lugar de focalizarse en el control de esta carga no controlada, se propone un modelo que utiliza variables de análisis de contingencias para calcular la capacidad de potencia necesaria en el sistema de potencia. Se emplea la variable de potencia no servida para evaluar la cantidad de potencia de carga no cubierta en cada barra del sistema, seguido del cálculo de la capacidad de potencia adicional requerida, utilizando un sistema fotovoltaico y de almacenamiento y otra alternativa de generación constante en el sistema de potencia IEEE de 14 barras con información sobre algunos vehículos eléctricos y la carga diaria en el sistema de potencia de Perú. Los resultados obtenidos en el sistema de potencia con generación distribuida muestran que no hay presencia de potencia no servida, corroborando el éxito de la metodología utilizada. Este modelo brinda herramientas tanto a los operadores de redes de distribución como a los planificadores de sistemas de potencia, reduciendo el impacto en el sistema de potencia de los vehículos eléctricos y aportando una metodología aplicable a otros sistemas de distribución eléctrica con cargas no controladas.
Abstract The uncontrolled charging of electric vehicles poses a great challenge for distribution network operators and power system planners. Instead of focusing on controlling this uncontrolled load, a model that uses contingency analysis variables to calculate the power capacity needed in the power system is proposed. The unserved power variable is used to evaluate the amount of uncovered load power at each bus of the system, followed by the calculation of the additional power capacity required using a photovoltaic and storage system and another constant generation alternative in the 14-bus IEEE power system with information on some electric vehicles and daily load in the power system of Peru. The results obtained in the power system with distributed generation, the absence of unserved power, corroborate the success of the methodology used. This model provides tools to both distribution network operators and power system planners, reducing the impact on the power system of electric vehicles and providing a methodology applicable to other electric distribution systems with uncontrolled loads.

Abstract Objectives: To evaluate the impact of hearing aids (HA) on the quality of life of patients with presbycusis and identify the causes for not using them. Study design: Retrospective cohort. Materials and methods: The Hearing Handicap Inventory for the Elderly (HHIE) was applied to patients with presbycusis, with indication for HA, followed in the ENT Service of a tertiary hospital between 2021 and 2022. Patients who did not use HA answered the closed question “Why don't you use HA?”. Results: Ninety-nine patients were included in the study, 47% of which were HA users. In the group of HA users, the average score for HHIE decreased from 24 to 7 points after use. The group of patients who did not use HA pointed out economic problems (46%) as the main reason for not using it. Conclusion: Our study confirmed the benefit of HA on quality of life and that limited financial resources are the main reason for non-use.
Resumo Objetivos: Avaliar o impacto da utilização de próteses auditivas (PA) na qualidade de vida dos doentes com presbiacusia e identificar as causas da sua não utilização. Desenho do estudo: Coorte retrospetivo. Material e Métodos: Foi aplicado a escala Hearing Handicap Inventory for the Elderly(HHIE) em doentes com presbiacusia, com indicação para o uso de PA, seguidos no Serviço de ORL de um hospital terciário entre 2021 e 2022. Os doentes que não usavam PA responderam adicionalmente à pergunta fechada “Por que não usa PA?”. Resultados: Foram incluídos 99 doentes no estudo, 47% dos quais utilizadores de PA. No grupo de doentes utilizadores de PA a pontuação média do HHIE foi de 24 pontos, com redução para 7 pontos após a utilização de PA. No grupo dos doentes não utilizadores de PA, razões económicas (46%) foram o motivo principal razão para a sua não utilização. Conclusão: O nosso estudo confirmou o benefício das PA na qualidade de vida da população idosa, sendo a limitação de recursos financeiros o principal motivo para a sua não utilização.

Hypertension is one of the primary modifiable risk factors for morbidity and mortality worldwide, being a major risk factor for coronary artery disease, stroke, and kidney failure. Furthermore, it is highly prevalent, affecting more than one-third of the global population.
Blood pressure measurement is a MANDATORY procedure in any medical care setting and is carried out by various healthcare professionals. However, it is still commonly performed without the necessary technical care. Since the diagnosis relies on blood pressure measurement, it is clear how important it is to handle the techniques, methods, and equipment used in its execution with care.
It should be emphasized that once the diagnosis is made, all short-term, medium-term, and long-term investigations and treatments are based on the results of blood pressure measurement. Therefore, improper techniques and/or equipment can lead to incorrect diagnoses, either underestimating or overestimating values, resulting in inappropriate actions and significant health and economic losses for individuals and nations.
Once the correct diagnosis is made, as knowledge of the importance of proper treatment advances, with the adoption of more detailed normal values and careful treatment objectives towards achieving stricter blood pressure goals, the importance of precision in blood pressure measurement is also reinforced.
Blood pressure measurement (described below) is usually performed using the traditional method, the so-called casual or office measurement. Over time, alternatives have been added to it, through the use of semi-automatic or automatic devices by the patients themselves, in waiting rooms or outside the office, in their own homes, or in public spaces. A step further was taken with the use of semi-automatic devices equipped with memory that allow sequential measurements outside the office (ABPM; or HBPM) and other automatic devices that allow programmed measurements over longer periods (HBPM).
Some aspects of blood pressure measurement can interfere with obtaining reliable results and, consequently, cause harm in decision-making. These include the importance of using average values, the variation in blood pressure during the day, and short-term variability. These aspects have encouraged the performance of a greater number of measurements in various situations, and different guidelines have advocated the use of equipment that promotes these actions. Devices that perform HBPM or ABPM, which, in addition to allowing greater precision, when used together, detect white coat hypertension (WCH), masked hypertension (MH), sleep blood pressure alterations, and resistant hypertension (RHT) (defined in Chapter 2 of this guideline), are gaining more and more importance.
Taking these details into account, we must emphasize that information related to diagnosis, classification, and goal setting is still based on office blood pressure measurement, and for this reason, all attention must be given to the proper execution of this procedure.
La hipertensión arterial (HTA) es uno de los principales factores de riesgo modificables para la morbilidad y mortalidad en todo el mundo, siendo uno de los mayores factores de riesgo para la enfermedad de las arterias coronarias, el accidente cerebrovascular (ACV) y la insuficiencia renal. Además, es altamente prevalente y afecta a más de un tercio de la población mundial.
La medición de la presión arterial (PA) es un procedimiento OBLIGATORIO en cualquier atención médica o realizado por diferentes profesionales de la salud. Sin embargo, todavía se realiza comúnmente sin los cuidados técnicos necesarios. Dado que el diagnóstico se basa en la medición de la PA, es claro el cuidado que debe haber con las técnicas, los métodos y los equipos utilizados en su realización.
Debemos enfatizar que una vez realizado el diagnóstico, todas las investigaciones y tratamientos a corto, mediano y largo plazo se basan en los resultados de la medición de la PA. Por lo tanto, las técnicas y/o equipos inadecuados pueden llevar a diagnósticos incorrectos, subestimando o sobreestimando valores y resultando en conductas inadecuadas y pérdidas significativas para la salud y la economía de las personas y las naciones.
Una vez realizado el diagnóstico correcto, a medida que avanza el conocimiento sobre la importancia del tratamiento adecuado, con la adopción de valores de normalidad más detallados y objetivos de tratamiento más cuidadosos hacia metas de PA más estrictas, también se refuerza la importancia de la precisión en la medición de la PA.
La medición de la PA (descrita a continuación) generalmente se realiza mediante el método tradicional, la llamada medición casual o de consultorio. Con el tiempo, se han agregado alternativas a través del uso de dispositivos semiautomáticos o automáticos por parte del propio paciente, en salas de espera o fuera del consultorio, en su propia residencia o en espacios públicos. Se dio un paso más con el uso de dispositivos semiautomáticos equipados con memoria que permiten mediciones secuenciales fuera del consultorio (AMPA; o MRPA) y otros automáticos que permiten mediciones programadas durante períodos más largos (MAPA).
Algunos aspectos en la medición de la PA pueden interferir en la obtención de resultados confiables y, en consecuencia, causar daños en las decisiones a tomar. Estos incluyen la importancia de usar valores promedio, la variación de la PA durante el día y la variabilidad a corto plazo. Estos aspectos han alentado la realización de un mayor número de mediciones en diversas situaciones, y diferentes pautas han abogado por el uso de equipos que promuevan estas acciones. Los dispositivos que realizan MRPA o MAPA, que además de permitir una mayor precisión, cuando se usan juntos, detectan la hipertensión de bata blanca (HBB), la hipertensión enmascarada (HM), las alteraciones de la PA durante el sueño y la hipertensión resistente (HR) (definida en el Capítulo 2 de esta guía), están ganando cada vez más importancia.
Teniendo en cuenta estos detalles, debemos enfatizar que la información relacionada con el diagnóstico, la clasificación y el establecimiento de objetivos todavía se basa en la medición de la presión arterial en el consultorio, y por esta razón, se debe prestar toda la atención a la ejecución adecuada de este procedimiento.

Além disso, é altamente prevalente e atinge mais de um terço da população mundial.
A medida da PA é procedimento OBRIGATÓRIO em qualquer atendimento médico ou realizado por diferentes profissionais de saúde. Contudo, ainda é comumente realizada sem os cuidados técnicos necessários. Como o diagnóstico se baseia na medida da PA, fica claro o cuidado que deve haver com as técnicas, os métodos e os equipamentos utilizados na sua realização.
Deve-se reforçar que, feito o diagnóstico, toda a investigação e os tratamentos de curto, médio e longo prazos são feitos com base nos resultados da medida da PA. Assim, técnicas e/ou equipamentos inadequados podem levar a diagnósticos incorretos, tanto subestimando quanto superestimando valores e levando a condutas inadequadas e grandes prejuízos à saúde e à economia das pessoas e das nações.
Uma vez feito o diagnóstico correto, na medida em que avança o conhecimento da importância do tratamento adequado, com a adoção de valores de normalidade mais detalhados e com objetivos de tratamento mais cuidadosos no sentido do alcance de metas de PA mais rigorosas, fica também reforçada a importância da precisão na medida da PA.
A medida da PA (descrita a seguir) é habitualmente feita pelo método tradicional, a assim chamada medida casual ou de consultório. Ao longo do tempo, foram agregadas alternativas a ela, mediante o uso de equipamentos semiautomáticos ou automáticos pelo próprio paciente, nas salas de espera ou fora do consultório, em sua própria residência ou em espaços públicos. Um passo adiante foi dado com o uso de equipamentos semiautomáticos providos de memória que permitem medidas sequenciais fora do consultório (AMPA; ou MRPA) e outros automáticos que permitem medidas programadas por períodos mais prolongados (MAPA).
Alguns aspectos na medida da PA podem interferir na obtenção de resultados fidedignos e, consequentemente, causar prejuízo nas condutas a serem tomadas. Entre eles, estão: a importância de serem utilizados valores médios, a variação da PA durante o dia e a variabilidade a curto prazo. Esses aspectos têm estimulado a realização de maior número de medidas em diversas situações, e as diferentes diretrizes têm preconizado o uso de equipamentos que favoreçam essas ações. Ganham cada vez mais espaço os equipamentos que realizam MRPA ou MAPA, que, além de permitirem maior precisão, se empregados em conjunto, detectam a HA do avental branco (HAB), HA mascarada (HM), alterações da PA no sono e HA resistente (HAR) (definidos no Capítulo 2 desta diretriz).
Resguardados esses detalhes, devemos ressaltar que as informações relacionadas a diagnóstico, classificação e estabelecimento de metas ainda são baseadas na medida da PA de consultório e, por esse motivo, toda a atenção deve ser dada à realização desse procedimento.

Abstract Objective: To identify the incidence of bruxism in children with obstructive sleep-disordered breathing (OSRD) and to evaluate the impact of adenotonsillectomy on the resolution of bruxism. Study Design: Retrospective observational study of 54 children with OSRD who underwent adenotonsillectomy. Materials and Methods: Questionnaire applied to caregivers of children aged 2-12 years, who underwent adenotonsillectomy for OSRD between January and November 2021, at Hospital Beatriz Ângelo. Results: 54 children were evaluated and it was found that 40,7% of the children had bruxism, with only 9.3% after surgery. Conclusion: According to our study, as it has been already described in the literature, there is a relationship between adenotonsillectomy and bruxism, with improvement or resolution in 77% of children that presented bruxism before surgery.
Resumo Objetivo: Identificar a prevalência de bruxismo em crianças com distúrbios respiratórios do sono de causa obstrutiva (DRSO) e avaliar o impacto da adenoamigdalectomia na resolução do bruxismo. Desenho do Estudo: Estudo observacional retrospectivo de 54 crianças com DRSO submetidas a adenoamigdalectomia. Materiais e Métodos: Questionário aplicado aos cuidadores de crianças com idades entre os 2-12 anos, que realizaram adenoamigdalectomia por DRSO entre Janeiro e Novembro de 2021, no Hospital Beatriz Ângelo. Resultados: Foram avaliadas 54 crianças e verificou-se que 40,7% das crianças apresentavam bruxismo, sendo que apenas 9,3% apresentava bruxismo após a cirurgia. Conclusão: Tal como descrito na literatura, o nosso trabalho mostra a existência de uma relação entre adenoamigdalectomia e o bruxismo, verificando-se a melhoria ou resolução em 77% das crianças com bruxismo.

ABSTRACT. Banana (Musa spp.) is one of the main fruits consumed worldwide. However, finger drop, is a physiological disorder that causes many postharvest problems, which eventually reduces market value and consumer acceptance. Therefore, the objective of the study was to evaluate the anatomical changes that occur in the pedicel rupture area (drop zone) of bananas diploids (BB França) and tetraploid (BRS Pioneira) in three ripening stages. The levels of gene expression involved in the natural ripening process and in the development of finger drop, was also investigated. The accumulation of their mRNAs and those of expansin (EXP1), pectate lyase (PEL1) and xyloglucan endotransglucosylase/hydrolase protein (XTH4) genes already isolated from bananas were measured by quantitative polymerase chain reaction in three ripening stages. BB França presented a higher resistance to finger drop due to the presence of some specific morphoanatomical characteristics, such as larger parenchymal cells and greater deposition of lignin. In contrast, there was degeneration of the pedicel parenchymal tissue of the BRS Pioneira genotype, forming large empty spaces during the ripening of the fruits, mainly in stage 6, which contributed to the finger drop. The diploid BB França is a strong candidate for use in banana breeding programs aimed at fruit drop resistance. This will certainly improve the quality of banana varieties. ABSTRACT Objective: To assess actual data on the safety, effectiveness, and hemodynamic performance of Bovine Pericardium Organic Valvular Bioprosthesis (BVP). Methods: The BIOPRO Trial is an observational, retrospective, non-comparative, non-randomized, and multicenter study. We collected data from 903 patients with symptomatic, moderate, or severe valve disease who underwent BVP implants in the timeframe from 2013 to 2020 at three Brazilian institutions. Death, valve-related adverse events (AEs), functional recovery, and hemodynamic performance were evaluated at the hospital, at discharge, and six months and one year later. Primary analysis compared late (> 30 days after implant) linearized rates of valve-related AEs, such as thromboembolism, valve thrombosis, major hemorrhage, major paravalvular leak, and endocarditis, following objective performance criteria (OPC). Analysis was performed to include at least 400 valve-years for each valve position (aortic and mitral) for complete comparisons to OPC. Kaplan-Meier survival and major adverse cardiovascular and cerebrovascular event analyses were also performed. Results: This retrospective study analyzed follow-up data collected from 903 patients (834.2 late patient-years) who have undergone surgery for 455 isolated aortic valve replacement (50.4%), 382 isolated mitral valve replacement (42.3%), and 66 combined valve replacement or other intervention (7.3%). The linearized rates of valve-related AEs were < 2 × OPC. One-year survival rates were 95.1% and 92.7% for aortic and mitral valve replacement, respectively. This study demonstrated an improvement in the New York Heart Association classification from baseline and hemodynamic performance within an expected range. In many applications, such as fuel storage tanks and fuel lines, galvanized steel is used as a construction material which comes into contact with biodiesel. Considering that biodiesel is more corrosive than petroleum diesel, studies that aim to increase the corrosion resistance of galvanized steel in this environment are required. The present work investigates how the chemical conversion treatment in the oxalic acid solution affects the corrosion resistance of galvanized steel in soybean biodiesel (B100) in the absence and presence of the natural additives the extract the turmeric, clove, ginger, stone breaker, and rosemary. ABSTRACT Objective: To set cutoff points for the triglyceride and glucose index (TyG) as a marker of insulin resistance (IR) for the pediatric population. Subjects and methods: This was a cross-sectional study with schoolchildren population-based data using data of 377 schoolchildren age 10 to 17 years of both sexes. We studied metabolic variables associated with IR indicators, such as fasting insulin and blood glucose, to calculate the homeostatic model assessment (HOMA-IR), and we studied triglycerides (TG) to determine the TyG index. We obtained TyG cutoff values for IR using the receiver operation characteristic (ROC), with definitions of sensitivity (Sen), specificity (Spe), and area under the ROC curve (AUC), with the HOMA-IR as reference. Results: The cutoff points of the TyG index for IR in adolescents are 7.94 for both sexes, 7.91 for boys, and 7.94 for girls, indicating moderate discriminatory power. When we also considered anthropometric variables of excess weight [TyG-BMI (body mass index)] and visceral fat [TyG-WC (waist circumference)], these indexes reached AUC values higher than 0.72, enhancing their potential use for a good diagnosis. Conclusion: TyG has proven to be a useful instrument for identifying IR in adolescent health screening, with high discrimination capacity when added to anthropometric variables, making it a feasible and inexpensive option. ABSTRACT Acute scrotum syndrome is a surgical emergency, which if an adequate evolution is not made and there are delays in the diagnosis, the patient can have irreversible damage in the testicular viability. The Clinical Practice Guidelines on acute scrotum syndrome is presented with the aim of updating the subject and providing a care and teaching instrument in the pediatric surgery services of the country. Several diseases can be the cause of the onset of acute scrotum syndrome, including testicular torsion, testicular hydatid torsion, and orchiepididymitis. The fundamental symptom is testicular pain and usually requires surgical treatment. The timely treatment of these patients avoids complications such as the need for orchiectomy, sterility, and that way the patient is more quickly reincorporated into his social activity.
RESUMEN El síndrome de escroto agudo es una urgencia quirúrgica, que si no se hace una evolución adecuada y existen demoras en el diagnostico puede el paciente tener daños irreversibles en la viabilidad testicular. Se presenta la Guía de Práctica Clínica sobre el síndrome de escroto agudo con el objetivo de hacer una actualización sobre el tema y proporcionar un instrumento asistencial y docente en los servicios de cirugía pediátrica del país. Varias enfermedades pueden ser la causa de aparición del síndrome de escroto agudo, entre ellas, la torsión testicular, la torsión de hidátides testiculares y la orquiepididimitis. El síntoma fundamental es el dolor testicular y requiere generalmente tratamiento quirúrgico. El tratamiento oportuno de estos pacientes evita complicaciones como la necesidad de orquiectomia, la esterilidad y reincorpora más rápidamente al paciente a su actividad social.
