Abstract Introduction: The World Health Organization (WHO) points out that infection by enteroparasites can affect ~3.5 billion people around the world. Hemodialysis (HD) patients may be more susceptible to infections by opportunistic pathogens due to impaired immune function. We evaluated enteroparasite infection in a sample of HD-patients from two dialysis centers and in a control group. Methods: Fecal samples were processed using the Hoffmann-Pons-Janner, Ritchie, Willis, and Rugai techniques. Patients with kidney failure from two dialysis centers undergoing HD for more than 3 months were included. The control group consisted of relatives of the patients without overt CKD. The TaqMan PCR and multiplex real-time PCR were carried out for detection of Cryptosporidium spp. and C. parvum and to differentiate the Entamoeba (E.) histolytica/E. dispar complex, respectively Results: A total of 97 HD patients and 42 controls were enrolled in the study. Fifty (51.5%) fecal samples from the HD group were positive for enteroparasites, as were 26 (61.9%) from the control group (P = 0.260). S. stercoralis was the single helminth detected and was only present in HD-patients. Coproscopy detected seven positive samples for the E. histolytica/E. dispar complex, three from HD patients and four from controls: by PCR, all samples were positive for the non-pathogenic E. dispar. Safranin-stained fecal smear slides were all negative for Cryptosporidium spp. However, by PCR, amplification for Crypstosporidium spp. was seen in six samples, all from the HD patients. Two of the species were classified as C. hominis by PCR-RFLP Conclusions: Enteroparasite infection as detected by traditional techniques were not more prevalent in HD patients, but S. stercoralis was only found in these patients. It is noteworthy that Cryptosporidium spp. infection, also affecting only HD patients, could only be detected by molecular biology techniques. Resumo Introdução: A OMS aponta que infecções por enteroparasitos podem afetar ~3,5 bilhões de pessoas globalmente. Pacientes em hemodiálise (HD) podem ser mais suscetíveis a infecções por patógenos oportunistas devido à função imunológica prejudicada. Avaliamos a infecção por enteroparasitos em pacientes em HD de dois centros de diálise e um grupo controle Métodos: Amostras fecais foram processadas pelas técnicas de Hoffmann, Pons&Janner, Ritchie, Willis e Rugai. Incluímos pacientes com insuficiência renal, de dois centros de diálise, em HD por mais de três meses. O grupo controle consistiu em familiares dos pacientes sem DRC evidente. Resumo Introdução: O Censo Brasileiro de Diálise (CBD) é uma pesquisa nacional anual sobre pacientes em diálise crônica que contribui para as políticas de saúde. Objetivo: Informar os dados epidemiológicos de 2022 do CBD da Sociedade Brasileira de Nefrologia (SBN). Métodos: Foi realizada uma pesquisa em centros brasileiros de diálise por meio de um questionário online que incluiu aspectos clínicos e epidemiológicos de pacientes em diálise crônica, dados da terapia dialítica e características do centro de diálise. Resultados: No total, 28% (n = 243) dos centros de diálise ativos cadastrados na SBN responderam ao questionário. Em julho de 2022, o número total estimado de pacientes em diálise era de 153.831. As taxas estimadas de prevalência e incidência de pacientes por milhão (ppm) de habitantes foram 758 e 214, respectivamente. Dos pacientes prevalentes, 95,3% estavam em hemodiálise (HD; 4,6% desses em hemodiafiltração) e 4,7% em diálise peritoneal (DP). Apenas 1,3% dos pacientes não foram vacinados contra a COVID-19. A prevalência de anemia (Hb < 10g/dL) foi de 27% e de hiperfosfatemia (P > 5,5mg/dL) alcançou 30%. A taxa bruta total anual de mortalidade estimada foi de 17,1%. Conclusões: O número absoluto e a taxa de prevalência de pacientes em diálise crônica continuam a aumentar. Um número crescente de pacientes estava em hemodiafiltração. Abstract Introduction: The Brazilian Dialysis Survey (BDS) is an annual national survey about patients on chronic dialysis that contributes to health policies. Objective: To report the 2022 epidemiological data from the BDS of the Brazilian Society of Nephrology (BSN). Methods: A survey was carried out in Brazilian chronic dialysis centers using an online questionnaire that included clinical and epidemiological aspects of patients on chronic dialysis, dialysis therapy data, and dialysis center characteristics. Results: Overall, 28% (n = 243) of the centers answered the questionnaire. In July 2022, the estimated total number of patients on dialysis was 153,831. The estimated prevalence and incidence rates of patients per million population (pmp) were 758 and 214, respectively. Of the prevalent patients, 95.3% were on hemodialysis (HD, 4.6% of these on hemodiafiltration) and 4.7% on peritoneal dialysis (PD). Only 1.3% of patients were not vaccinated against COVID-19. The prevalence of anemia (Hb < 10g/dL) was 27% and hyperphosphatemia (P > 5.5mg/dL) reached 30%. The estimated overall crude annual mortality rate was 17.1%. ABSTRACT In Brazil, the COVID-19 burden was substantial, and risk factors associated with higher in-hospital mortality rates have been extensively studied. However, information on short-term all-cause mortality and the factors associated with death in patients who survived the hospitalization period of acute SARS-CoV-2 infection is limited. We analyzed the six-month post-hospitalization mortality rate and possible risk factors of COVID-19 patients in a single center in Brazil. This is a retrospective cohort study focused on a six-month follow-up. The exclusion criteria were death during hospitalization, transference to another hospital, and age under 18. We collected data from the charts of all hospitalized patients from March 2020 to December 2020 with a positive RT-PCR test for SARS-CoV-2, resulting in a sample size of 106 patients. The main outcome was death after hospitalization, whereas comorbidities and demographics were evaluated as risk factors. The crude post-hospitalization death rate was 16%. The first 30 days of follow-up had the highest mortality rate. In a Cox regression model for post-hospitalization mortality, previous chronic kidney disease (HR, 4.06, 95%CI 1.46 – 11.30) and longer hospital stay (HR 1.01, 95%CI 1.00 – 1.02) were the only factors statistically associated with death. In conclusion, a high six-month all-cause mortality was observed. Within the six-month follow-up, a higher risk of death was observed for patients who had prior CKD and longer hospital stay. These findings highlight the importance of more intensive medical surveillance during this period. Resumo Objetivo Analisar o índice de fragilidade modificado de 11 itens (mFI-11) como preditor de mortalidade entre pessoas idosas em hemodiálise crônica. Método Estudo observacional prospectivo realizado nas unidades de diálise do município de Niterói (RJ). Um total de 124 pacientes em hemodiálise (HD) por pelo menos 3 meses, que iniciaram terapia de substituição renal aos 65 anos ou mais, foram acompanhados por 24 meses. A fragilidade foi medida pelo mFI-11, composto por 9 comorbidades, 1 item funcional e 1 item cognitivo. As comorbidades foram obtidas por anamnese e revisão de prontuário. A dependência funcional foi determinada pela presença de 2 ou mais dependências na escala de Katz, e o deficit cognitivo foi medido pelo Miniexame do Estado Mental (MEEM). O ponto de corte para fragilidade foi mFI-11 ≥3. Resultados A idade média no início do estudo foi de 76 anos, e 55,6% eram homens. Dos 124 participantes, 56,5% tinham diabetes, 21% tinham dependência funcional e 52,9% apresentavam deficit cognitivo. Abstract Objective To analyze the 11-item modified frailty index (mFI-11) as a predictor of mortality among older adults on chronic hemodialysis. Method A prospective observational study conducted in dialysis units in the municipality of Niterói (RJ, Brazil). A total of 124 patients on hemodialysis (HD) for at least 3 months, who began renal replacement therapy at the age of 65 or older, were followed for 24 months. Frailty was measured using the mFI-11, which comprises 9 comorbidities, 1 functional item, and 1 cognitive item. The comorbidities were obtained through anamnesis and medical record review. Functional dependency was determined by the presence of 2 or more dependencies on the Katz scale, and cognitive deficit was measured by the Mini-Mental State Examination (MMSE). The cutoff point for frailty was defined as mFI-11 ≥3. Results The mean age at the start of the study was 76 years, and 55.6% were men. Of the 124 participants, 56.5% had diabetes, 21% had functional dependency, and 52.9% had cognitive deficits. The prevalence of frailty was 67.7%, and an mFI-11 score of ≥3 was significantly associated with an increased risk of death (HR 2.39, 95% CI 1.21-4.72). Resumo O primeiro caso de covid-19 foi detectado em dezembro de 2019, na China. A doença rapidamente evoluiu para uma pandemia e trouxe um fardo social e de saúde sem paralelo para a humanidade. As formas graves de covid-19 afetam principalmente adultos, especialmente idosos com comorbidades. Relatamos um caso grave de covid-19 em uma paciente de 12 anos de idade, previamente saudável, que deu entrada no pronto-socorro em 26/05/2020 com febre, dores abdominais, vômitos e diarreia. Durante a investigação, apresentou teste positivo para SARS-CoV-2 e evoluiu com falência múltipla de órgãos e eventos trombóticos catastróficos, resultando em amputação bilateral de pernas e quirodáctilos. Ela recebeu alta hospitalar para acompanhamento ambulatorial após 107 dias. Abstract The first case of COVID-19 was detected in Dec 2019, in China. The disease shortly evolved into a pandemic and imposed an unparalleled health and social burden on mankind. Severe forms of COVID-19 mainly affect adults, especially the elderly and those with comorbidities. We report a severe case of COVID-19 in a previously healthy 12-year-old female who was admitted to the emergency room on May 26, 2020, with fever, abdominal pain, vomiting, and diarrhea. During the hospital stay, she tested positive for SARS-CoV-2 and developed multiple organ failure and catastrophic thrombotic events resulting in bilateral amputation of legs and fingers. She was discharged from the hospital for outpatient follow-up after 107 days. By the time this report was written, the patient was undergoing prosthesis prescription and training and regaining her independence to walk. Introdução: O Brasil possui um vasto território dividido em cinco regiões geográficas com importantes diferenças nos índices sociodemográficos. Nosso objetivo foi apresentar e comparar características sociodemográficas, resultados bioquímicos e prescrição de medicamentos de pacientes em tratamento de hemodiálise crônica (HD) nas cinco regiões geográficas. Métodos: Avaliamos os dados do Registro Brasileiro de Diálise de todos os pacientes adultos submetidos à HD crônica em 2021. As variáveis incluíram características sociodemográficas, níveis séricos de fosfato, cálcio e albumina, hemoglobina, taxa de redução de ureia e prescrição de quelantes de fosfato, eritropoietina e ferro intravenoso. Os dados das regiões Norte e Nordeste foram combinados em um único grupo. Resultados: Foi analisado um total de 13.792 pacientes (57,9 ± 16,0 anos, 58,5% do sexo masculino, mediana de tempo de HD de 31 (11–66) meses) de 73 centros de diálise. A distribuição regional foi de 59,5% dos pacientes provenientes do Sudeste; 21,7% do Sul; 5,9% do Centro-Oeste; e 12,9% do Norte/Nordeste. As características sociodemográficas, os resultados bioquímicos e as prescrições de medicamentos diferiram entre as regiões. A prevalência de pacientes idosos foi menor nas regiões Centro-Oeste e Norte/Nordeste. A região Sul apresentou a maior prevalência de hiperfosfatemia (41,2%) e taxa de redução de ureia < 65% (24,8%), enquanto a anemia e a hipoalbuminemia foram mais prevalentes no Sudeste, 32,7% e 11,6%, respectivamente. ABSTRACT Introduction: Brazil has a vast territory divided into five geographic regions with important differences in sociodemographic indices. We aimed to present and compare socio-demographic characteristics, biochemical results, and drug prescription of patients on chronic hemodialysis (HD) treatment in the five geographic regions. Methods: We evaluated data from the Brazilian Dialysis Registry of all adult patients undergoing chronic HD in 2021. Variables included sociodemographic characteristics, serum levels of phosphate, calcium, and albumin, hemoglobin, urea reduction rate, and prescription of phosphate binders, erythropoietin, and intravenous iron. Data from the North and Northeast regions were combined into one group. Results: A total of 13,792 patients (57.9 ± 16.0 years old, 58.5% male, median HD vintage of 31 (11–66) months) from 73 dialysis centers were analyzed. Regional distribution was 59.5% in the Southeast; 21.7% in the South; 5.9% in the Midwest; and 12.9% in the North/Northeast. Sociodemographic features, biochemical results, and medication prescriptions differed across regions. The prevalence of elderly patients was lower in the Midwest and North/Northeast. The South region had the highest prevalence of hyperphosphatemia (41.2%) and urea reduction rate <65% (24.8%), while anemia and hypoalbuminemia were more prevalent in the Southeast, 32.7% and 11.6%, respectively. Conclusion: We found differences in socio-demographics, clinical features, and drug prescriptions across Brazilian geographic regions. Resumo Introdução: O Brasil possui o maior sistema público e universal de saúde do mundo, mas pouco se sabe sobre os desfechos dos pacientes em hemodiálise (HD) no país de acordo com a fonte de financiamento do tratamento. Objetivo: Comparar o perfil e a sobrevida dos pacientes que têm o tratamento de HD custeado pelo Sistema Único de Saúde (SUS) com aqueles com convênio privado. Métodos: Análise retrospectiva dos adultos incidentes em HD entre 2012 e 2017 em 21 centros de diálise no Brasil que atendiam tanto pelo SUS quanto por convênios privados. Os participantes, independentemente da fonte pagadora, receberam tratamento dialítico semelhante. Os dados foram censurados com 60 meses de acompanhamento ou ao final de 2019. Resultados: Foram incluídos 4945 pacientes, sendo 59,7% financiados pelo SUS. Os pacientes financiados pelo SUS, em comparação aos que tinham convênio privado, eram mais jovens (58 vs 60 anos; p < 0,0001) e com menor prevalência de diabetes (35,8% vs 40,9%; p < 0,0001). As taxas de sobrevida, em 60 meses nesses grupos foram de 51,1% e 52,1%, respectivamente (p = 0,85). ABSTRACT Introduction: Brazil has the largest public and universal healthcare system in the world, but little is known about the outcomes of patients on hemodialysis (HD) in the country according to the source of funding for the treatment. Objective: To compare the profile and survival of patients under HD treatment funded by the Public Healthcare System (SUS) to those with private insurance. Methods: Retrospective analysis of adults undergoing HD between 2012 and 2017 in 21 dialysis centers in Brazil that provided both by the SUS and private health insurance. Participants, regardless of the paying source, received similar dialysis treatment. Data were censored after 60 months of follow-up or at the end of 2019. Results: 4,945 patients were included, 59.7% of which were financed by the SUS. Patients financed by SUS, compared to those with private insurance, were younger (58 vs. 60 years; p < 0.0001) and with a lower prevalence of diabetes (35.8% vs. 40.9%; p < 0.0001). The 60-month survival rates in these groups were 51.1% and 52.1%, respectively (p = 0.85). In the analysis of the subdistribution proportional hazard ratio by the Fine-Gray model, including adjustment for concurrent outcomes, a significant increase in the risk ratio for death was found (1.22 [95% confidence interval 1.04 to 1.43]) in patients with treatment funded by the SUS. Conclusions: Patients on HD with treatment funded by the SUS have a higher adjusted risk of death when compared to those with private insurance, despite similar dialysis treatment. RESUMO Introdução: O Censo Brasileiro de Diálise (CBD) constitui importante fonte de dados nacionais sobre pessoas em diálise crônica que contribui para a formulação de políticas de saúde. Objetivo: Relatar dados epidemiológicos de 2021 do CBD da Sociedade Brasileira de Nefrologia (SBN). Métodos: Realizou-se pesquisa em centros brasileiros de diálise crônica utilizando questionário online abrangendo aspectos clínicos e epidemiológicos de pacientes em diálise crônica, dados sobre terapia dialítica, características dos centros de diálise e o impacto da pandemia COVID-19 em 2021. Resultados: Trinta por cento (n = 252) dos centros responderam ao questionário. Em Julho de 2021, o número total estimado de pacientes em diálise foi 148.363. ABSTRACT Introduction: The Brazilian Dialysis Survey (BDS) is an important source of national data about people on chronic dialysis that contributes to the formulation of health policies regarding kidney failure. Objective: To report the 2021 epidemiological data from the BDS of the Brazilian Society of Nephrology (BSN). Methods: A survey was carried out in Brazilian chronic dialysis centers using an online questionnaire covering clinical and epidemiological aspects of patients in chronic dialysis, data on dialysis therapy, characteristics of dialysis centers, and the impact of the COVID-19 pandemic of 2021. Results: Thirty percent (n = 252) of the centers answered the questionnaire. In July 2021, the estimated total number of patients on dialysis was 148,363. The estimated prevalence and incidence rates of patients per million population (pmp) were 696 and 224, respectively. Resumo Introdução: A hipercalemia é uma condição multifatorial comum em pessoas em diálise crônica e está associada à mortalidade. Nosso objetivo foi informar e discutir a prevalência de hipercalemia em uma grande população de pacientes em diálise crônica no Brasil e diferenças entre as regiões geográficas. Métodos: A prevalência de hipercalemia (potássio sérico ≥6,0 mEq/L) foi avaliada por meio do Censo Brasileiro de Diálise (CBD) em Julho de 2019, uma pesquisa online de participação voluntária na qual foram convidados todos os centros de diálise registrados na Sociedade Brasileira de Nefrologia. Abstract Introduction: Hyperkalemia is a common multifactorial condition of people on chronic dialysis and is associated with mortality. We aimed to inform and discuss the prevalence of hyperkalemia in a large population of chronic dialysis patients in Brazil and its geographic regions. Methods: Prevalence of hyperkalemia (serum potassium ≥6.0 mEq/L) was assessed in the Brazilian Dialysis Survey (BDS) in July 2019, an online survey of voluntary participation in which all dialysis centers registered at the Brazilian Society of Nephrology were invited. Results: Approximately one-third (n=263 of 805) of the Brazilian dialysis clinics participated. The prevalence of hyperkalemia in the whole population was 16.1% (n=7,457 of 46,193; 95%CI=15.8-16.5%,), and varied from 12.1% in the North to 18.7% in the Northeast. Objetivo: Comparar o perfil e a sobrevida dos pacientes que têm o tratamento de HD custeado pelo Sistema Único de Saúde (SUS) com aqueles com convênio privado. Métodos: Análise retrospectiva dos adultos incidentes em HD entre 2012 e 2017 em 21 centros de diálise no Brasil que atendiam tanto pelo SUS quanto por convênios privados. Os participantes, independentemente da fonte pagadora, receberam tratamento dialítico semelhante. Os dados foram censurados com 60 meses de acompanhamento ou ao final de 2019. Resultados: Foram incluídos 4945 pacientes, sendo 59,7% financiados pelo SUS. Os pacientes financiados pelo SUS, em comparação aos que tinham convênio privado, eram mais jovens (58 vs 60 anos; p < 0,0001) e com menor prevalência de diabetes (35,8% vs 40,9%; p < 0,0001). Resumo Objetivo: Avaliar joelhos de indivíduos com falência renal em hemodiálise por meio de ressonância magnética, em comparação com um grupo controle sem doença renal crônica. Materiais e Métodos: Estudo transversal, observacional, controlado, realizado entre agosto/2018 e fevereiro/2020. Os 15 casos consistiram de pacientes com cinco anos ou mais em hemodiálise, sem ruptura do tendão do quadríceps. Os 15 controles, sem doença renal crônica, foram pareados (1:1) por sexo, idade e nível de atividade física. Resultados: A média de idade dos casos foi 50 ± 15 anos e a dos controles, 49 ± 14 anos. A mediana do tempo em hemodiálise foi 11 anos (variação: 10-14 anos). Nos casos, os níveis séricos de paratormônio, ferritina, fosfatase alcalina, fósforo e creatinina estavam mais altos e os de albumina e hemoglobina, mais baixos (p < 0,05). Hipersinal no tendão foi demonstrado em 11 casos e três controles (p = 0,009). Derrame articular foi observado em nove casos e três controles (p < 0,05). A espessura, o comprimento e a largura do tendão não diferiram entre os grupos. Hipersinal no tendão do quadríceps não mostrou associação com o tempo em hemodiálise ou com os níveis de paratormônio intacto, hemoglobina e fosfatase alcalina. Conclusão: Pacientes em hemodiálise sem episódios de ruptura já apresentam hipersinal no tendão do quadríceps nas imagens de ressonância magnética.

Abstract Objective: To evaluate the knees of individuals with renal failure who are on hemodialysis, using magnetic resonance imaging (MRI), comparing them with those of a group of individuals with normal renal function. Materials and Methods: This was a cross-sectional, observational, controlled study conducted between August 2018 and February 2020. The cases consisted of 15 patients who had been on hemodialysis for ≥ 5 years and did not have a quadriceps tendon rupture. The controls consisted of 15 individuals with normal renal function who were matched (1:1) to the cases for sex, age, and physical activity level. The subjects in both groups underwent MRI of the right knee only. Results: The mean ages of the cases and controls were 50 ± 15 years and 49 ± 14 years, respectively. The median time on hemodialysis was 11 years (range, 10-14 years). Serum levels of parathyroid hormone, ferritin, alkaline phosphatase, phosphorus, and creatinine were higher among the cases than among the controls, whereas serum albumin and hemoglobin were lower (p < 0.05 for all). The MRI study showed a hyperintense signal in the quadriceps tendon in 11 of the cases and in three of the controls (p = 0.009). Knee joint effusion was observed in nine of the cases and in three of the controls (p < 0.05). The thickness, length, and width of the tendon did not differ between the groups. A hyperintense signal in the tendon was not associated with the time on hemodialysis; nor with the levels of intact parathyroid hormone, hemoglobin, or alkaline phosphatase. Conclusion: Patients on chronic hemodialysis, even those without a tendon rupture, show a hyperintense signal in the quadriceps tendon on MRI.

Objetivo: Relatar dados epidemiológicos do Censo Brasileiro de Diálise 2020, patrocinado pela Sociedade Brasileira de Nefrologia. Métodos: Realizou-se pesquisa em centros brasileiros de diálise crônica utilizando questionário online relativo ao ano, abrangendo aspectos clínicos e epidemiológicos de pacientes em programa de diálise crônica, dados sobre terapia dialítica, características das unidades de diálise e impacto da pandemia COVID-19. Resultados: 235 (28%) centros responderam ao questionário. Em julho de 2020, o número total estimado de pacientes em diálise foi 144.779. As taxas estimadas de prevalência e incidência de pacientes por milhão da população (pmp) foram 684 e 209, respectivamente. Dos pacientes prevalentes, 92,6% estavam em hemodiálise (HD) e 7,4% em diálise peritoneal (DP); 23% estavam na lista de espera para transplante. Cateter venoso central foi utilizado por um quarto dos pacientes em HD. A taxa de incidência de COVID-19 confirmada entre fevereiro e julho de 2020 foi 684/10.000 pacientes em diálise, a taxa de letalidade foi 25,7%. As taxas estimadas de mortalidade geral e mortalidade bruta anual por COVID-19 foram 24,5 e 4,2%, respectivamente. Conclusão: O número absoluto de pacientes em diálise crônica e a taxa de prevalência continuaram a aumentar. O baixo uso de DP como terapia de diálise foi mantido e o uso de cateteres de longa permanência para HD aumentou. A pandemia COVID-19 contribuiu para o aumento da taxa de mortalidade geral.

Of the prevalent patients, 92.6% were on hemodialysis (HD) and 7.4% were on peritoneal dialysis (PD); 23% were on the transplant waiting list. A central venous catheter was used by a quarter of patients on HD. The incidence rate of confirmed COVID-19 between February and July 2020 was 684/10,000 dialysis patients, and the lethality rate was 25.7%. The estimated overall mortality and COVID-19 crude annual mortality rates were 24.5 and 4.2%, respectively. Conclusion: The absolute number of patients on chronic dialysis and prevalence rate continued to increase. The low use of PD as dialysis therapy was maintained and the use of long-term catheters for HD increased. The COVID-19 pandemic contributed to the increase in the overall mortality rate.

Objective: To report the 2021 epidemiological data from the BDS of the Brazilian Society of Nephrology (BSN). Methods: A survey was carried out in Brazilian chronic dialysis centers using an online questionnaire covering clinical and epidemiological aspects of patients in chronic dialysis, data on dialysis therapy, characteristics of dialysis centers, and the impact of the COVID-19 pandemic of 2021. Results: Thirty percent (n = 252) of the centers answered the questionnaire. In July 2021, the estimated total number of patients on dialysis was 148,363. The estimated prevalence and incidence rates of patients per million population (pmp) were 696 and 224, respectively. Of the prevalent patients, 94.2% were on hemodialysis (HD) (1.8% of these on hemodiafiltration), and 5.8% on peritoneal dialysis (PD); 21% were on the transplant waiting list. The incidence rate of confirmed COVID-19 between January and July 2021 was 1,236/10,000 dialysis patients, and the case-fatality rate reached 25.5%. Up to July 2021, 88.6% of patients had received at least one dose of the anti-SARS-CoV-2 vaccine. The estimated overall and COVID-19 crude annual mortality rates were 22.3% and 5.3%, respectively. Conclusion: The absolute number and the prevalence rate of patients on chronic dialysis continue to increase. Most dialysis patients were vaccinated against COVID-19 during the year. The COVID-19 pandemic was associated to the overall mortality rate.

RESUMO As rupturas do tendão do quadríceps são lesões incomuns, frequentemente associadas a doenças crônicas, incluindo a doença renal em estágio de falência funcional. O papel do hiperparatireoidismo secundário como fator predisponente para rupturas tendinosas nesse grupo de pacientes está bem documentado, e o enfraquecimento da junção tendão-osso faz parte desse contexto. O tratamento de escolha para rupturas do tendão do quadríceps em pacientes com falência renal é a cirurgia, e deve ser realizado o mais rápido possível. Existem várias técnicas cirúrgicas a serem utilizadas, mas a falta de estudos comparativos não nos permite concluir qual é a melhor opção. Publicações mais recentes têm preferido a associação de técnicas, com destaque para o uso de enxerto tendíneo autólogo como ferramenta de reforço, procedimento de escolha do autor. Estudos recentes relatam o uso de agentes biológicos para estimular a cicatrização e aloenxertos, mas as informações parecem preliminares para serem rotineiramente recomendadas. ABSTRACT Quadriceps tendon tears are uncommon injuries often associated with chronic diseases, including end-stage renal disease (ESRD). The role of secondary hyperparathyroidism as a predisposing factor for tendon tears in this group of patients is well documented, and the weakening of the tendon-bone junction is part of this context. The treatment of choice for quadriceps tendon ruptures in patients with ESRD is surgery, which should be performed as soon as possible. There are several surgical techniques to be used, but the lack of comparative studies does not allow us to conclude which one is the best option. More recent publications have preferred the association of techniques, with emphasis on the use of autologous tendon grafts as a reinforcement tool, which is the author’s procedure of choice. Recent studies reported the use of biological agents to stimulate healing and allografts, but the information seems preliminary to be routinely recommended. Abstract Background: Cryptosporidium spp. are pathogenic protozoans that play an important role in developing diseases in the elderly, children, and immunosuppressed individuals. Methods: The objective of this study was to detect and genetically characterize Cryptosporidium spp. in kidney transplanted patients (n = 97 samples; group 1) and immunosuppressed individuals from an outpatient clinic suspected of having Cryptosporidium infection (n = 53 samples; group 2). All fecal samples were analyzed by parasitological stool examination, immunochromatographic test, and real-time polymerase chain reaction (real-time PCR). Cryptosporidium-positive samples were tested using nested PCR for the gp60 gene, followed by sequencing for subtype determination. Results: Parasitological examination was negative in all Group 1, and positive in four Group 2 samples. Real-time PCR revealed Cryptosporidium in 13 samples: four in Group 1 (three C. hominis and one C. parvum) and nine in Group 2 (seven C. hominis, one C. parvum, and one mixed C. hominis/C. parvum). The immunochromatographic test was reactive in 11 samples (four in Group 1 and seven in Group 2). All 11 C. hominis isolates were identified as subtype IbA10G2 and one C. parvum as subtype IIbA15G2R1. All C. hominis belonged to subtype IbA10G2, which is recognized as the most prevalent and pathogenic subtype. Conclusions: This study showed, for the first time, that the presence of Cryptosporidium subtypes is considered more virulent in Brazilian transplanted kidney patients.
