RESUMEN La disponibilidad de medicamentos para producir un aborto, sobre todo en contextos de acceso restringido, transformó las prácticas y permitió que las propias mujeres y/o sus organizaciones comunitarias ayuden a otras mujeres a abortar, interactuando o no con el sistema de salud. Este estudio recupera la experiencia de una organización feminista de la comunidad que, desde la provincia de Neuquén, se extiende a todo el país, generando una red de cuidados comunitarios. Se realizó un estudio exploratorio descriptivo, con enfoque cualitativo con el propósito de analizar las experiencias de las mujeres que facilitan el acceso al aborto permitido en Argentina. A través de entrevistas en profundidad a tres líderes de la colectiva feminista La Revuelta y de entrevistas semiestructuradas a 33 integrantes de las grupas socorristas, realizadas entre noviembre de 2019 y diciembre de 2020, describimos su historia y los procesos de trabajo y crecimiento; exploramos sus motivaciones y sentimientos y caracterizamos las interacciones de dichas organizaciones con los sistemas de salud público y privado. Los resultados de este trabajo coinciden con la conversación y la producción bibliográfica internacional acerca de estas organizaciones y sus particularidades y con la necesidad de incorporar estos cuidados a los sistemas de salud institucionales.

ABSTRACT The availability of medications to induce abortion, especially in contexts of restricted access, has transformed practices and allowed women and/or their community organizations to assist other women in obtaining abortions, whether or not they interact with the healthcare system. This study recovers the experience of a feminist community organization that, from the province of Neuquén, extends throughout the country, creating a network of community care. An exploratory descriptive study with a qualitative approach was conducted to analyze the experiences of women who facilitate access to permitted abortion in Argentina. Through in-depth interviews with three leaders of the feminist collective La Revuelta and semi-structured interviews with 33 members of the socorrista groups, conducted between November 2019 and December 2020, we describe their history and processes of work and growth; we explore their motivations and feelings and characterize the interactions of these organizations with public and private health systems. The results of this work align with the international conversation and bibliographic production about these organizations and their particularities, and with the need to incorporate these forms of care into institutional health systems.

RESUMEN INTRODUCCIÓN: La anticoncepción es un derecho, y es obligación del Estado garantizar el acceso a métodos anticonceptivos efectivos, seguros y de calidad. Se realizó una evaluación de tecnología sanitaria sobre los parches anticonceptivos transdérmicos. MÉTODOS: Un equipo multidisciplinario e independiente designado por el Comité Provincial de Biotecnologías de Neuquén buscó información epidemiológica, regulatoria y evidencias científicas sobre eficacia, seguridad y adherencia. Se analizó y sistematizó siguiendo metodología GRADE (Grading of Recommendations Assessment, Development and Evaluation) y CASPe (Critical Appraisal Skills Programme Español) . RESULTADOS: El único parche autorizado en Argentina para su comercialización libera 33,9 μg/día de etinilestradiol y 203 μg/día de norelgestromina. Su prospecto en Argentina, EE.UU. y Europa lo asocia al doble de riesgo de enfermedad tromboembólica venosa si se compara con las píldoras anticonceptivas que provee el Estado. Esto coincide con resultados de estudios de cohortes de alta calidad. Los parches proveen similar eficacia anticonceptiva a corto plazo, pero con altas tasas de abandono en el seguimiento. La Organización Mundial de la Salud no los ha incluido en su listado de medicamentos esenciales. Los parches son más costosos que otros métodos disponibles . DISCUSIÓN: Sobre la base de los principios de beneficencia, no maleficencia, de precaución y de proporcionalidad, no se recomienda la incorporación de parches.

ABSTRACT INTRODUCTION: Contraception is a right, being an obligation of the State to guarantee access to effective, safe and quality contraceptive methods. A health technology assessment was carried out on transdermal contraceptive patches . METHODS: A multidisciplinary and independent team appointed by the Provincial Biotechnology Committee of Neuquén searched for epidemiological and regulatory information and scientific evidence on efficacy, safety and adherence. It was analyzed and systematized following the GRADE (Grading of Recommendations Assessment, Development and Evaluation) and CASPe (Critical Appraisal Skills Programme Español) methodology . RESULTS: The only patch authorized for commercialization in Argentina releases 33.9 μg/day of ethinylestradiol and 203 μg/day of norelgestromin. Its package insert in Argentina, the US and Europe highlights that the risk of venous thromboembolic disease is twice as high compared to the contraceptive pills provided by the State. This is consistent with results from high-quality cohort studies. Patches provide similar short-term contraceptive efficacy, but with high dropout rates at follow-up. The World Health Organization has not included them in its list of essential medicines. Patches are more expensive than other available methods . DISCUSSION: Based on the principles of beneficence, non-maleficence, precaution and proportionality, the incorporation of patches is not recommended.

ABSTRACT INTRODUCTION : Health Technology Assessment (HTA) is the systematic process of assessing the properties, effects and impact of the technology used. The institutionalization of the HTA is a fundamental tool to carry out health policies and achieve better results with more efficiency and equity. The objective of the study was to describe current state of the institutionalization of HTA in Argentina and to know the characteristics of these organizations. METHODS : A descriptive cross-sectional exploratory study was conducted through a self-administered survey to identify organizations and to describe their characteristics, resources, production andprocesses for the development of HTA. RESULTS : A total of 70 surveys were received, 39 of those institutions carry out HTA reports. The lack of trained human resources and the lack of specific financing were among the limitations for the implementation of HTA. According to 80% of respondents, it is important to increase organizational support, streamline decision-making structures and foster the established communication channels and networks in order to improve the impact of the recommendations. DISCUSSION : There is a significant institutional capacity to develop HTA in Argentina. The data obtained can serve as a reference to promote the use of HTA in decision-making, strengthen the links between the organizations identified, increase information exchange and coordinate efforts.

RESUMEN INTRODUCCIÓN : La Evaluación de Tecnologías Sanitarias (ETS) es el proceso sistemático de valorización de las propiedades, los efectos y el impacto de la tecnología utilizada. La institucionalización de la ETS es una herramienta fundamental para llevar adelante las políticas de salud y conseguir mejores resultados con mayor eficiencia y equidad. El objetivo del estudio fue describir el estado actual en la institucionalización de la ETS a nivel nacional y conocer las características de estas organizaciones. MÉTODOS : Se efectuó un estudio exploratorio transversal descriptivo a través de una encuesta autoadministrada para identificar organizaciones que realizan ETS y describir sus características, recursos, producción y procesos. RESULTADOS : Se recibieron encuestas de 70 instituciones, de las cuales 39 realizan informes de ETS. Entre las limitaciones referidas para su implementación se señaló la falta de recursos humanos capacitados y de financiamiento específico. Según el 80%, para mejorar el impacto de las recomendaciones es importante incrementar el apoyo organizacional, dinamizar las estructuras de toma de decisiones y estimular los canales de comunicación y redes establecidas. DISCUSIÓN : Existe una considerable capacidad institucional para desarrollar ETS en Argentina. Los datos obtenidos permiten guiar el esfuerzo para fomentar el uso de la ETS en la toma de decisiones, fortalecer los vínculos entre las organizaciones identificadas, incrementar el intercambio de información y articular esfuerzos.

INTRODUCTION: The intrauterine device (IUD) is the most cost-efficient reversible contraceptive method. Low utilization is reported in Argentina and the region under study. Knowledge, attitudes and practices (KAP) of providers can strongly influence the choice of method among users. OBJECTIVES: To describe the KAP of healthcare providers in the urban conglomerate Neuquen/Plottier/Centenario as regards IUD in 2013, in order to characterize the determinants of promotion and possible barriers. METHODS: A descriptive observational cross-sectional study was performed with a non-probabilistic sample of 335 gynecologists, general practitioners and midwives, using an anonymous online survey RESULTS: The response rate was 31% (N = 103), 80 of them working in the public sector. The knowledge was correct, and the opinions of providers were favorable to the method. In 9 out of 12 situations, more than 50% said they would not recommend or insert IUDs. Half of the people surveyed had inserted less than 10 IUDs in the previous year, and 12% had not inserted any Two out of ten were concerned about some of the issues. CONCLUSIONS: The right knowledge and the favorable opinions do not correspond to the low number of IUDs inserted. There are barriers to eligibility and access, as well as little use of interventions validated by the evidence.

INTRODUCCIÓN: El dispositivo intrauterino (DIU) es el método anticonceptivo más costo-eficiente entre los reversibles. Se reporta una baja utilización en Argentina y la región. Los conocimientos, actitudes y prácticas (CAP) de los proveedores pueden influenciar fuertemente la elección del método entre las usuarias. OBJETIVOS: Describir los CAP de los proveedores de salud en el conglomerado urbano Neuquén/Plottier/Centenario en relación con el DIU en 2013, para caracterizar los determinantes de su promoción y las posibles barreras. MÉTODOS: Se realizó un estudio observacional descriptivo de corte transversal, con una muestra no probabilística de 335 ginecólogos, médicos generalistas y obstétricas. Se utilizó una encuesta en línea anónima. RESULTADOS: La tasa de respuesta fue del 31% (N = 103), con un 80% perteneciente al sector público. Los conocimientos fueron correctos, y hubo opiniones favorables al método. En 9 de 12 situaciones habituales planteadas, más de la mitad contestó que no lo recomendaría/insertaría. La mitad había insertado menos de 10 DIU en el año, y un 12% no había puesto ninguno. Dos de cada diez estaban preocupados por alguna de las razones indagadas. CONCLUSIONES: Los conocimientos y opiniones favorables al uso no se correspondieron con la baja cantidad de DIU insertados. Se visualizó poco aprovechamiento de intervenciones validadas por la evidencia. Se constataron barreras en la elegibilidad y el acceso.
