ABSTRACT Despite being subject to lower AIDS-related mortality rates and having a higher life expectancy, patients with HIV are more prone to develop non-AIDS events. A low CD4+/CD8+ ratio during antiretroviral therapy identifies people with heightened immune senescence and increased risk of mortality. In clinical practice, finding determinants of a low CD4+/CD8+ ratio may be useful for identifying patients who require close monitoring due to an increased risk of comorbidities and death. We performed a prospective study on the evolution of the CD4+/CD8+ ratio in 60 patients infected with HIV (80% males), who were subjected to two different antiretroviral regimens: early and deferred therapy. The initial CD4+/CD8+ ratio was ≤1 for 70% of the patients in both groups. Older age, CD4+ cell count at inclusion, Nadir CD8+T-cell count, and Initial CD4+/CD8+ ratio ≤ 1 were risk factors for lack of ratio recovery. In the multivariate analysis, a CD4+/CD8+ ratio > 1 at the start of the treatment was found to be a determinant factor in maintaining a CD4+/CD8+ ratio > 1. The nadir CD4+T-cell count was lower in the deferred therapy group (p=0.004), and the last CD4+/CD8+ ratio ≤1 was not associated with comorbidities. Ratio recovery was not associated with the duration of HIV infection, time without therapy, or absence of AIDS incidence. A greater improvement was observed in patients treated early (p=0.003). In contrast, the slope of increase was slower in patients who deferred treatment. In conclusion, the increase in the CD4+/CD8+ ratio occurred mostly for patients undergoing early strategy treatment and its extension did not seem to be related to previous HIV-related factors. ABSTRACT Objective To estimate vaccination coverage and analyze sociodemographic factors associated with non-vaccination in children born in 2017 and 2018 in the state capitals of Northeast Brazil. Methods A household survey using cluster sampling was conducted from 2020-2022 to estimate vaccination coverage and hesitancy. Factors associated with non-vaccination were analyzed using logistic regression to calculate Odds Ratios (OR) and their Confidence Intervals (95%CI). Results Natal was the capital with the lowest vaccination coverage, below 75.0% for most immunizers. Teresina had rates equal to or greater than 90.0% for all vaccines. Among those interviewed, 99.1% (95%CI 98.9;99.3) believe that vaccines are important for health; 95.4% (95%CI 95.0;95.8) trust immunobiologicals and 79.6% (95%CI% 78.8;80.3) are not afraid of reactions. Belonging to the highest socioeconomic stratum (adjusted OR: 1.34 – 95%CI 1.20;1.50) was as a factor associated with non-vaccination. Conclusion Low coverage highlights the need for a better understanding of regional specificities and social inequalities.
RESUMO Objetivo Estimar a cobertura vacinal e analisar fatores sociodemográficos associados à não vacinação em crianças nascidas vivas em 2017 e 2018 nas capitais do Nordeste brasileiro. Métodos Realizou-se inquérito domiciliar com amostragem por conglomerados, entre 2020 e 2022, para estimar cobertura e hesitação vacinal. Fatores associados à não vacinação foram analisados usando-se regressão logística para calcular odds ratio (OR) e seus intervalos de confiança (IC95%). Resultados A capital com menores coberturas vacinais foi Natal, com < 75,0% para a maioria dos imunizantes; Teresina apresentou coberturas ≥ 90,0% em todos os imunizantes. Dos entrevistados, 99,1% (IC95% 98,9;99,3) acreditavam que vacinas são importantes para a saúde; 95,4% (IC95% 95,0;95,8) confiavam nos imunobiológicos distribuídos pelo governo; e 79,6% (IC95% 78,8;80,3) não tinham medo de reações adversas. Residir em estrato socioeconômico mais alto (OR ajustado: 1,34 – IC95% 1,20;1,50) foi fator associado à não vacinação. Conclusão As baixas coberturas destacam a necessidade de entender melhor as especificidades regionais e as desigualdades sociais.

RESUMO Objetivo Analisar a cobertura vacinal contra sarampo, caxumba e rubéola em crianças até 24 meses de idade e os fatores associados à não vacinação em coorte de nascidos-vivos em 2017-2018, em capitais e municípios de grande porte populacional do interior do Nordeste brasileiro. Métodos Inquérito domiciliar populacional com análise de cobertura vacinal e fatores sociodemográficos por regressão logística. Resultados Em 12.137 crianças, cobertura vacinal de 79,3% (IC95% 76,5;81,8) e taxa de abandono de 10,6%. Associação à não vacinação: estrato socioeconômico A (OR-a 1,29; IC95% 1,10;1,50), residir no interior (OR-a 1,22; IC95% 1,07;1,39), não acesso ao Programa Bolsa Família (OR-a 1,19; IC95% 1,05;1,34), renda familiar ≤ R$ 1.000,00 (OR-a 1,17; IC95% 1,03;1,31), mãe sem trabalho remunerado (OR-a 1,28; IC95% 1,15;1,42), > 1 filho por mãe (OR-a 1,12; IC95% 1,08;1,17), sem caderneta de vacinação (OR-a 10,69; IC95% 6,27;18,20). ABSTRACT Objective To analyze measles, mumps, and rubella vaccination coverage among children up to 24 months old and factors associated with non-vaccination in a 2017−2018 live birth cohort, in state capitals and large interior region cities in Northeast Brazil. Abstract Background Hepatocellular carcinoma (HCC) is a prevalent tumor with high mortality rates. Computed tomography (CT) is crucial in the non-invasive diagnosis of HCC. Recent advancements in artificial intelligence (AI) have shown significant potential in medical imaging analysis. However, developing these AI algorithms is hindered by the scarcity of comprehensive, publicly available liver imaging datasets. Objectives This study aims to detail the tools, data organization, and database structuring used in creating HepatIA, a medical imaging annotation platform and database at a Brazilian tertiary teaching hospital. HepatIA supports liver disease AI research at the institution. Material and methods The authors collected baseline characteristics and CT scans of 656 patients from 2008 to 2021. The database, designed using PostgreSQL and implemented with Django and Vue.js, includes 692 CT volumes from a four-phase abdominal CT protocol. Radiologists made segmentation annotations using the OHIF medical image viewer, incorporating MONAI Label for pre-annotation segmentation models. The annotation process included detailed descriptions of liver morphology and nodule characteristics. Results The HepatIA database currently includes healthy individuals and those with liver diseases such as HCC and cirrhosis. The database dashboard facilitates user interaction with intuitive plots and histograms. Key patient demographics include 64% males and an average age of 56.89 years. The database supports various filters for detailed searches, enhancing research capabilities. Conclusion A comprehensive data structure was successfully created and integrated with the IT systems of a teaching hospital, enabling research on deep learning algorithms applied to abdominal CT scans for investigating hepatic lesions such as HCC. Abstract Objectives To present the Brazilian Psychiatric Association’s Consensus on the Management of Acute Intoxication. Methods A group of experts selected by the Brazilian Psychiatric Association searched for articles on the MEDLINE (by PubMed) and Cochrane databases, limited to human studies and acute intoxication. Working groups reviewed these materials for appropriateness to the topic and the quality of the work. A survey was conducted using the Delphi method to produce a table of agreed recommendations presented at the end of the systematic review. Three survey rounds were held to reach consensus. Results Support for intoxication should start with Initial Management: Resuscitation/Life Support/Differential Diagnosis. For this, the group proposed the following sequence of assessments: A (airway), B (breathing), C (circulation), D.1 (disability), D.2 (differential diagnosis), D.3 (decontamination), D.4 (drug antidotes), E (enhanced elimination). The group of experts then presented specific interventions for the main drugs of abuse. Conclusions Management of intoxication with drugs of abuse is complex and requires systematic protocols. The group suggests adoption of the A-B-C-D-E technique first, with constant investigation. Then, specific conduct and support until remission of intoxication. The literature is still scarce in evidence on the subject. Therefore, this consensus was necessary. We believe that at present this document can help psychiatric, general, and emergency physicians deal with emergency psychiatric episodes due to acute intoxication. This work could stimulate future studies on the topic. ABSTRACT Serra da Saudade Formation corresponds to the upper part of the stratigraphic column of the Bambuí Group. Few studies have addressed the soil properties and pedogenesis of the pelitic rocks rich in potassium minerals of this formation. This study analyzed siltstone-derived soils, some of which are glauconitic (green siltstone; “verdete”), to understand the role of the main pedogenetic factors and processes in the landscape of the Central-West region of the Minas Gerais State, covered by Cerrado vegetation. Nine soil profiles were described, and their morphological, physical, chemical and mineralogical properties were analyzed. Soils were classified as Neossolos Litólicos (P1, P3, P6, P8 and P9), Cambissolos Háplicos (P2 and P5), Argissolo Vermelho-Amarelo (P4) and Argissolo Acinzentado (P7). The main active pedogenetic processes identified in the study area are melanization, goethization, argiluviation and elutriation. These are essentially controlled by the nature of the parent material and position of the soil in the relief. Soils are typically shallow, dystrophic, highly Al-saturated and contain essentially quartz and micas in the coarse fractions (sand and silt) and illite/glauconite and kaolinite in the clay fraction. In soil environments with siltstone and green siltstone under “dry forests”, the soil water pH was higher and high levels of exchangeable calcium and magnesium, a eutrophic character and high-activity clay were observed. Barium, chromium, lead and zinc contents were high in all studied soils. Green siltstone-derived soils have peculiar physical and chemical properties, divergent from those developed from other glauconitic rocks on the Earth surface. However, greenish tones in horizons are common in all these soils. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). Abstract Cereus jamacaru DC. (Cactaceae) is used in folk medicine for treating pain and inflammation. However, the therapeutic potential of C. jamacaru extracts has not been assessed experimentally. This study aimed to characterize the effect of both aqueous (CAECJ) and ethanolic (CEECJ) extracts of C. jamacaru in mouse models of pain and inflammation. CAECJ and CEECJ were chemically characterized by qualitative methods. Toxicity of extracts was evaluated in vitro against J774.A1 macrophages and in vivo by monitoring mice for 14 days following a single oral treatment at 2000 mg/kg. The antinociceptive activity of the extracts was assessed in models of pain in mice: acetic acid-induced writhing test, formalin test, hot plate test, and glutamate-induced nociception assay. The anti-inflammatory activity of the extracts was assessed following zymosan-induced peritonitis and in the model of arthritis induced by Complete Freund’s Adjuvant (CFA). Phytochemical analyses revealed alkaloids, condensed tannins, flavonoids, and anthraquinones in both extracts; saponins were present in CEECJ only. Neither extract was cytotoxic in vitro or induced toxicity in mice. Both extracts reduced nociception in all models of nociception without impairing motor function. Both extracts reduced leucocyte migration during experimental peritonitis and reduced paw edema during experimental arthritis. The effects of the extracts can be both due to anti-inflammatory and centrally-mediated mechanisms. This paper corroborates the traditional use of C. jamacaru extracts and demonstrates their therapeutic potential in the development of new analgesic and anti-inflammatory drugs with a good safety profile. RESUMO Objetivo: Analisar as experiências e significados atribuídos por homens trans ao processo de amamentação à luz da Teoria das Representações Sociais. Método: Estudo qualitativo, descritivo e exploratório, realizado em âmbito nacional, com uma amostra intencional de cinco homens trans que experienciaram a amamentação e com idade igual ou superior aos 18 anos. Aplicou-se o roteiro de entrevista semiestruturado como instrumento de coleta de dados, utilizando a plataforma Google Meet®, de setembro a outubro de 2021. Os dados foram descritos, integrando-os à análise de conteúdo, resultando em duas categorias analíticas. Resultados: Os homens trans parturientes se dispuseram a amamentar sua criança, ancorando suas representações sobre a amamentação na ideia de que se trata de um fenômeno desempenhado, exclusivamente, por mulheres e reconhecendo, também, esse fenômeno como um acontecimento que constitui sua masculinidade, além de descrevê-la como uma experiência pedagógica e de aprendizado com o objetivo de garantir o desenvolvimento saudável da criança. ABSTRACT Objective: To analyze the experiences and meanings attributed by trans men to the breastfeeding process in light of the Theory of Social Representations. ABSTRACT Currently, COVID-19 and tuberculosis (TB) are the deadliest infectious diseases worldwide. Their synergy, form of presentation, morbidity, and mortality are data that have been scarcely explored. Thus, this study aimed to characterize the clinical, epidemiological, and laboratory factors of this co-infection and to analyze the factors associated with the active TB among COVID-19 cases. A case-control study was conducted with a retrospective survey of 21 laboratory-confirmed COVID-19/TB co-infected patients (case group) and 21 COVID-19 patients (control group). The study included participants from eight hospitals in Campo Grande city, capital of Mato Grosso do Sul State, Brazil, from March 2020 to March 2022. Association analysis and binomial logistic regression were employed with statistical significance set at p≤0.05. From the 21 identified cases of COVID-19/TB co-infection, we found a more frequent association with HIV infection than the control-group, without worsening the outcome. COVID-19/TB patients had less dyspnea and less need for mechanical ventilation compared to the cases with COVID-19 only. On the other hand, COVID-19/TB patients had higher levels of C-reactive protein and lower hemoglobin levels, the latter variable was independently associated with COVID-19/TB. Among the clinical differences presented among COVID-19/TB co-infected patients, despite the association with HIV and lower clinical repercussions, only lower hemoglobin levels were associated with COVID-19/TB. RESUMO Objetivo Estimar a cobertura vacinal e analisar fatores sociodemográficos associados à não vacinação em crianças nascidas vivas em 2017 e 2018 nas capitais do Nordeste brasileiro. Métodos Realizou-se inquérito domiciliar com amostragem por conglomerados, entre 2020 e 2022, para estimar cobertura e hesitação vacinal. Fatores associados à não vacinação foram analisados usando-se regressão logística para calcular odds ratio (OR) e seus intervalos de confiança (IC95%). Resultados A capital com menores coberturas vacinais foi Natal, com < 75,0% para a maioria dos imunizantes; Teresina apresentou coberturas ≥ 90,0% em todos os imunizantes. Dos entrevistados, 99,1% (IC95% 98,9;99,3) acreditavam que vacinas são importantes para a saúde; 95,4% (IC95% 95,0;95,8) confiavam nos imunobiológicos distribuídos pelo governo; e 79,6% (IC95% 78,8;80,3) não tinham medo de reações adversas. Residir em estrato socioeconômico mais alto (OR ajustado: 1,34 – IC95% 1,20;1,50) foi fator associado à não vacinação. ABSTRACT Objective To estimate vaccination coverage and analyze sociodemographic factors associated with non-vaccination in children born in 2017 and 2018 in the state capitals of Northeast Brazil. Methods A household survey using cluster sampling was conducted from 2020-2022 to estimate vaccination coverage and hesitancy. Factors associated with non-vaccination were analyzed using logistic regression to calculate Odds Ratios (OR) and their Confidence Intervals (95%CI). Results Natal was the capital with the lowest vaccination coverage, below 75.0% for most immunizers. Teresina had rates equal to or greater than 90.0% for all vaccines. Among those interviewed, 99.1% (95%CI 98.9;99.3) believe that vaccines are important for health; 95.4% (95%CI 95.0;95.8) trust immunobiologicals and 79.6% (95%CI% 78.8;80.3) are not afraid of reactions. RESUMO Objetivo Analisar a cobertura vacinal e os fatores associados ao esquema incompleto das vacinas contra a poliomielite em coorte de nascidos vivos de 2017-2018, em capitais e municípios do interior do Nordeste brasileiro. Métodos Inquérito domiciliar em crianças com idade ≤ 24 meses, entre 2020 e 2022. Estimaram-se a cobertura vacinal, a taxa de abandono e fatores associados ao esquema de vacinação incompleto, analisados pela razão de chances (odds ratio, OR) e intervalos de confiança de 95% (IC95%). Resultados Em 12.137 crianças, a cobertura vacinal (quatro doses) foi de 80,9% (IC95% 78,4;83,1); 8,4% não eram vacinadas. Não possuir caderneta de vacinação (OR = 14,05; IC95% 8,54;23,12) e uso de serviço privado (OR = 1,46; IC95% 1,23;1,74) estiveram associados à vacinação incompleta. Taxas de abandono mais altas foram observadas na dose de reforço, principalmente no estrato alto. Not having a vaccination card (OR=18.06; 95%CI 10.01;32.61) and use of private services (OR=1.46; 95%CI 1.23;1.74) were associated with incomplete vaccination. Higher dropout rates were found for the booster dose, especially in the highest stratum. ABSTRACT Objective To analyze measles, mumps, and rubella vaccination coverage among children up to 24 months old and factors associated with non-vaccination in a 2017−2018 live birth cohort, in state capitals and large interior region cities in Northeast Brazil. Methods Population-based survey analyzing vaccination coverage and sociodemographic factors through logistic regression. Results For 12,137 children, vaccination coverage was 79.3% (95%CI 76.5;81.8), and the dropout rate was 10.6%. Association with non-vaccination: socioeconomic stratum A (OR-a 1.29; 95%CI 1.10;1.50), living in the interior region (OR-a 1.22; 95%CI 1.07;1.39), no access to the Bolsa Família Program (OR-a 1.19, 95%CI 1.05;1.34), family income ≤BRL 1,000 (OR-a 1.17, 95%CI 1.03;1.31), mother not working (OR-a 1.28, 95%CI 1.15;1.42), >1 child per mother (OR-a 1.12, 95%CI 1.08;1.17), and no vaccination card (OR-a 10.69, 95%CI 6.27;18.20). Conclusion Low vaccination coverage and a high dropout rate in state capitals and municipalities in the interior region of Northeast Brazil.
RESUMO Objetivo Analisar a cobertura vacinal contra sarampo, caxumba e rubéola em crianças até 24 meses de idade e os fatores associados à não vacinação em coorte de nascidos-vivos em 2017-2018, em capitais e municípios de grande porte populacional do interior do Nordeste brasileiro. Métodos Inquérito domiciliar populacional com análise de cobertura vacinal e fatores sociodemográficos por regressão logística. Resultados Em 12.137 crianças, cobertura vacinal de 79,3% (IC95% 76,5;81,8) e taxa de abandono de 10,6%. Associação à não vacinação: estrato socioeconômico A (OR-a 1,29; IC95% 1,10;1,50), residir no interior (OR-a 1,22; IC95% 1,07;1,39), não acesso ao Programa Bolsa Família (OR-a 1,19; IC95% 1,05;1,34), renda familiar ≤ R$ 1.000,00 (OR-a 1,17; IC95% 1,03;1,31), mãe sem trabalho remunerado (OR-a 1,28; IC95% 1,15;1,42), > 1 filho por mãe (OR-a 1,12; IC95% 1,08;1,17), sem caderneta de vacinação (OR-a 10,69; IC95% 6,27;18,20). Conclusão Baixa cobertura e alta taxa de abandono vacinal em capitais e municípios do interior do Nordeste.

RESUMEN: Objetivo: Este estudio tiene como objetivo evaluar la progresión del valor angular de individuos no tratados diagnosticados con Escoliosis Idiopática del Adolescente (EIA), que están en espera de un procedimiento quirúrgico. Métodos: Se trata de un estudio observacional y descriptivo. Se recogieron datos de edad de indicación quirúrgica inicial, fecha inicial y ángulo de Cobb, fecha y ángulo de Cobb de la visita de seguimiento, tiempo transcurrido entre la visita inicial y de seguimiento y tipo de curva. Todos los ángulos de Cobb registrados fueron revisados por los autores. Resultados: El 86,1% de los individuos eran mujeres, la edad media de indicación del tratamiento quirúrgico fue de 13,34 años. El tipo de curva más común fue Lenke 3 y el que más progresó fue Lenke 4. El promedio general de progresión anual fue de 9,89 grados para las curvas primarias y 12,32 para las curvas secundarias y el seguimiento fue en promedio de 35,77 meses. Conclusión: La progresión de la magnitud de la curva aumenta durante la espera del tratamiento definitivo de la EIA, independientemente del grupo de la clasificación de Lenke al que pertenezca la curva. Las curvas secundarias presentan una tasa de progresión de 12,32º/año, superior a la curva principal que presenta una tasa de 9,89º/año. El tiempo de espera ha ido aumentando a lo largo de los años, lo cual es evidente al compararlo con publicaciones más antiguas. Nivel de Evidencia IV; Tipo de Estudio: Estudio Pronostico. ABSTRACT Objective: This study aims to evaluate angular progression of patients with a diagnosis of Adolescent Idiopathic Scoliosis (AIS), that await surgical treatment. Methods: This is an observational and descriptive study. Data were collected for age at initial surgical indication, initial date and Cobb angle, date and Cobb angle of the follow-up visit, time elapsed between the initial and follow-up visit, and type of curve. All recorded Cobb angles were reviewed by the authors. Results: 86.1% of the individuals were women, the mean age of indication for surgical treatment was 13.34 years. The most common type of curve was Lenke 3 and the one that progressed the most was Lenke 4. The general average of annual progression was 9.89 degrees for the primary curves and 12.32 for the secondary curves, and the follow-up was, on average, 35.77 months. Conclusion: The progression of the magnitude of the curve increases during the wait for the definitive treatment of AIS, no matter which group of the Lenke classification the curve belongs to. The secondary curves present a progression rate of 12.32º/year, higher than the main curve, which presents a rate of 9.89º/year. The waiting time has been increasing over the years, which is evident compared to older publications. Level of Evidence IV; Type of Study: Prognostic Study.
