This article presents the results of a qualitative research that aimed to analyze the implementation of the National Policy for Comprehensive Men's Health Care (PNAISH) in the Amazonian territory, taking the city of Belém (Pará) as a reference. Interviews were conducted with managers, workers, users and potential users of the Brazilian National Health System (SUS). Based on theoretical-methodological framework of the discursive practices, in social psychology, the analysis of the material identified the little institutionality in the actions carried out, the intense turnover in management positions, reduced teams, scarcity of financial resources and reports of discrimination against men from the LGBTQIA+ population and indigenous people. It is concluded that the ways to overcome the challenges in the implementation of PNAISH involve the need for an intersectional reading that considers the particularities of the Amazon territory, in addition to a greater relevance of the policy in the dynamics of services and management. Abstract Objectives: To identify somatic mutations in tumors from young women with triple-negative or luminal breast cancer, through targeted sequencing and to explore the cancer driver potential of these gene variants. Methods: A customized gene panel was assembled based on data from previous sequencing studies of breast cancer from young women. Triple-negative and luminal tumors and paired blood samples from young breast cancer patients were sequenced, and identified gene variants were searched for their driver potential, in databases and literature. Additionally, the authors performed an exploratory analysis using large, curated databases to evaluate the frequency of somatic mutations in this gene panel in tumors stratified by age groups (every 10 years). Results: A total of 28 young women had their tumoral tissue and blood samples sequenced. Using a customized panel of 64 genes, the authors could detect cancer drivers in 11/12 (91.7 %) TNBC samples and 11/16 (68.7 %) luminal samples. Among TNBC patients, the most frequent cancer driver was TP53, followed by NF1, NOTCH1 and PTPN13. In luminal samples, PIK3CA and GATA3 were the main cancer drivers, and other drivers were GRHL2 and SMURF2. CACNA1E was involved in both TN and luminal BC. The exploratory analysis also indicated a role for SMURF2 in luminal BC development in young patients. Conclusions: The data further indicates that some cancer drivers are more common in a specific breast cancer subtype from young patients, such as TP53 in TNBC and PIK3CA and GATA3 in luminal samples. These results also provide additional evidence that some genes not considered classical cancer-causing genes, such as CACNA1E, GRHL2 and SMURF2 might be cancer drivers in this age group. RESUMO O artigo teve como objetivo relatar a experiência de imersão em campo de prática de estudante do Curso de Psicologia da Universidade Federal do Pará, enquanto participante do Projeto Multicampi Saúde: Atenção à Saúde da Criança, durante a realização do atendimento à criança-guia, à família e à comunidade de acordo com atividades que tiveram por finalidade a capacitação de estudantes e profissionais do serviço, diretamente na Unidade Básica de Saúde (UBS) e território adjacente. Delineou-se a trajetória percorrida pela estudante realizada no município de Cametá, Pará, durante o mês de junho de 2019, relatando a experiência em seis atividades principais: acompanhamento do uso da Caderneta da Criança com os profissionais da UBS; construção do Plano Terapêutico Singular para a criança/família-guia; mapeamento da rede de saúde mental do município; ações de educação em saúde; grupos de escuta; e capacitações para os profissionais da UBS. Resumo Este artigo opinião apresenta reflexões sobre masculinidades e construções de gênero - a partir do fenômeno global da pandemia do novo coronavírus -, produzidas por pesquisadores/as que integram a equipe nacional de uma pesquisa sobre política de atenção integral aos homens na saúde, no Brasil. A partir de leituras baseadas em gênero, o artigo argumenta que é preciso atentarmos que a socialização masculina cisheteronormativa se orienta a partir de três eixos: 1) a abjeção às práticas de cuidado de si e dos outros; 2) a rejeição às práticas preventivas em saúde, dada uma distorcida matriz de percepção de risco (e certo sentimento de “invulnerabilidade”); 3) a dinâmica doméstica marcada por posições de comando, ordenamento e honra. Essas dimensões da vida cotidiana foram profundamente provocadas nesta primeira fase da epidemia, em que o confinamento se tornou a alterativa mais recomendável. Esses eixos se configuram como repertórios recorrentes (embora não recentes) que reificam o modelo central de uma ordem masculina que precisa se tornar objeto de reflexão, na medida em que colocam em risco a saúde de homens e mulheres e mais amplamente dos pactos civilizatórios e da ordem social.

Abstract This article presents reflections on masculinity and the social construction of gender - based on the global phenomenon of the new coronavirus pandemic - produced by researchers who are part of the national research team on comprehensive health care policy for men in Brazil. From a gender-based standpoint, the article contends that it is necessary to note that cis heteronormative male socialization is guided by three core issues: 1) the submission to practices of care of self and others; 2) the rejection of preventive health practices, due to a distorted matrix of risk perception (and a certain sense of “invulnerability”); 3) the domestic dynamics marked by postures of command, order, and honor. These dimensions of everyday life were profoundly upset in this first phase of the epidemic, in which confinement became the most recommended alternative. These issues are configured as recurring (though not recent) repertoires that glorify the central model of a male order that needs to become an object of reflection, insofar as they endanger the health of men and women and, more broadly, of the status quo of the accepted tenets of domestic and social order.

RESUMO Este estudo teve o intuito de conhecer de que forma o tema da medicalização da educação tem sido abordado e debatido entre profissionais da educação, mais especificamente as/os docentes. Dez escolas públicas e particulares de Belém participaram da pesquisa-intervenção, na qual utilizamos aulas-teatro como disparadoras do debate junto às professoras e professores. A partir dessa experiência, sistematizamos três categorias de análise: a) A ausência de discussão crítica sobre a medicalização da educação; b) A exigência do laudo; c) A falta de estrutura e investimento na formação continuada. Discute-se a carência de políticas que favoreçam práticas não medicalizantes no contexto educacional, bem como a necessidade de maior engajamento das escolas nesse debate.

Abstract This study aimed to find out how the topic of medicalization of education has been approached and debated among education professionals, more specifically teachers. Ten public and private schools in Belém participated in the intervention-research, in which we used theater-based classes as triggers for the debate with teachers. Based on this experience, we have systematized three categories of analysis: a) The absence of critical discussion about the medicalization of education; b) The requirement of the report; c) The lack of structure and investment in teacher training. The lack of policies that favor non-medicalizing practices in the educational context is discussed, as well as the need for greater involvement of schools in this debate.

RESUMEN En este estudio se tuvo como intuito conocer de qué forma el tema de la medicalización de la educación ha sido abordada y debatida entre profesionales de la educación, más específicamente las/los docentes. Diez escuelas públicas y privadas de Belém participaron de la investigación-intervención, en la cual utilizamos clases-teatro como disparadoras del debate junto a profesoras y profesores. A partir de esa experiencia, sistematizamos tres categorías de análisis: a) La ausencia de discusión crítica sobre la medicalización de la educación; b) La exigencia del laudo; c) La falta de estructura e investimento en la formación continuada. Se discute la carencia de políticas que favorezcan prácticas no medicalización en el contexto educacional, así como la necesidad de más participación de las escuelas en ese debate.

OBJECTIVES: In breast cancer (BC) patients, the frequency of germline BRCA mutations (gBRCA) may vary according to the ethnic background, age, and family history of cancer. Phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit alpha (PIK3CA) is the second most common somatic mutated gene in BC; however, the association of mutations in both genes with cancer has not been thoroughly investigated. Thus, our aims were to investigate gBRCA mutation frequency in a cohort of postmenopausal Brazilian BC patients and the association of gBRCA1/BRCA2 and PIK3CA somatic mutations. METHODS: Forty-nine postmenopausal (>55 years) and forty-one young (≤35 years) BC patients were included in this study. The postmenopausal group included patients who reported a positive family history of cancer. For these patients, gBRCA1/BRCA2 were sequenced using next-generation sequencing (NGS) or Sanger sequencing. Data for gBRCA in young patients were already available from a previous study. DNA from formalin-fixed, paraffin-embedded (FFPE) tumors was obtained from 27 postmenopausal and 41 young patients for analyzing exons 9 and 20 of PIK3CA. The association between gBRCA1/BRCA2 and somatic mutations in PIK3CA was investigated. RESULTS: The overall frequency of gBRCA1/BRCA2 among the 49 postmenopausal patients was 10.2%. The frequencies of somatic mutations in PIK3CA in the postmenopausal and young patients were 37% and 17%, respectively (ns). The most common PIK3CA mutation was found to be E454A. Nonsense and frameshift mutations, which may counteract the oncogenic potential of PIK3CA were also detected. Regardless of age, 25% of BRCA1/BRCA2 mutation carriers and non-carriers , each, had PIK3CA somatic mutations. CONCLUSIONS: Data obtained indicate that BRCA1/BRCA2 gene testing may be considered for postmenopausal patients with BC who have a family history of cancer. Although some of them are not considered pathogenic, somatic variants of PIK3CA are frequently observed in BC patients, especially in postmenopausal patients.
