Abstract Introduction Spinocerebellar ataxias (SCAs) are a heterogeneous group of neurodegenerative diseases. Objective To evaluate the living standard of patients with SCA, by applying the Vestibular Disorders Activities of Daily Living Scale (VADL) and Activitiesspecific Balance Confidence Scale (ABC) questionnaires. Methods An uncontrolled clinical trial study was conducted with 28 patients who underwent anamnesis, ENT evaluation, and vestibular assessment and the application of questionnaires VADL and ABC before and after rehabilitation with virtual reality. Results The vestibular exam was altered in 64.3% of the cases. The result between the correlation of the VADL and ABC questionnaires showed significant results in all cases (p < 0.005). The correlation between the ages and disease length with the VADL and ABC questionnaires was significant in the T3 assessment (p = 0.015). The correlation between the disease length and the VADL questionnaire was significant in all cases (p < 0.005). The comparison of the vestibular rehabilitation result (T1 to T2) showed a significant difference for all the applied games, except for the ski slalom. Abstract Introduction: Autoimmune hepatitis (AIH) has a spectrum of symptoms ranging from asymptomatic disease to acute severe hepatitis, chronic hepatitis, and decompensated cirrhosis. The acute presentation is not rare and could represent genuine acute AIH (GAAIH) or acute exacerbation of chronic autoimmune hepatitis. We aimed to identify the prevalence, clinical features, and prognostic factors associated with GAAIH and compare these cases with acute exacerbation of chronic AIH. Methods: This cross-sectional observational study evaluated patients with acute AIH presentation, defined as total bilirubin >5 times the upper limit of normality (xULN) and/or alanine aminotransferase >10 xULN, and no prior history of liver disease. Histology findings of acute disease defined GAAIH. Bivariate analyses were performed to identify factors associated with the GAAIH, when compared with acute exacerbation of chronic AIH. Results: Seventy-two patients with acute presentation of AIH were included and six (8.3%) of them presented GAAIH. Comparative analysis between patients with GAAIH and patients with acute exacerbation of chronic AIH revealed that prothrombin activity (96% [74-100] vs. 61% [10-100]; p = 0.003) and albumin levels (3.9 ± 0.2 g/dL vs. 3.4 ± 0.5 g/dL; p < 0.001) were higher in patients with GAAIH. The International Autoimmune Hepatitis Group score was higher in patients with acute exacerbation of chronic AIH (18.5 [8-23] vs. 16.5 [15-17]; p = 0.010). Compared to 15.2% of acute exacerbation of chronic AIH, complete therapeutic response to treatment was achieved in 67.7% of cases with GAAIH (p = 0.018). Conclusions: GAAIH was rare (8.3%), and patients with this presentation exhibited more preserved liver function tests, suggesting that most cases presenting with loss of function are acute exacerbation of chronic AIH. Additionally, patients with GAAIH had a better complete therapeutic response, suggesting a more preserved liver function at presentation, and early diagnosis has a positive therapeutic implication.
Resumo Introdução: A hepatite autoimune (HAI) apresenta um espectro de sintomas que varia de doença assintomática a hepatite aguda grave, hepatite crónica e cirrose descompensada. A apresentação aguda não é rara e pode representar hepatite autoimune aguda genuína (HAIAG) ou exacerbação aguda de hepatite autoimune crónica (EAHAIC). O nosso objetivo foi identificar a prevalência, caraterísticas clínicas e fatores prognósticos associados à HAIAG, e comparar esses casos com EAHAIC. Métodos: Estudo observacional, transversal, incluindo doentes com apresentação aguda de HAI, definida como bilirrubina total > 5 vezes o limite superior da normalidade (xLSN) e/ou ALT > 10 xLSN, e sem história prévia de doença hepática. HAIAG foi definida pela presença de achados histológicos de doença aguda. Análises bivariadas foram realizadas para identificar fatores associados à HAIAG, quando comparado com o EAHAIC. Resultados: Foram incluídos setenta e dois doentes com apresentação agu-da de HAI, dos quais seis (8.3%) com HAIAG. A análise comparativa entre doentes com HAIAG e doentes com EAHAIC mostrou que a atividade de protrombina (96%(74-100) versus 61% (10-100; p=0.003) e os níveis de albumina (3,9 ± 0,2 g/dL vs. 3,4 ± 0,5 g/dL; p < 0,001) foram significativamente mais elevados em pacientes com HAIAG. O score do Grupo Internacional de Hepatite Autoimune foi mais elevado em doentes com EAHAIC (18.5 (8-23) versus 16.5 (15-17); p=0.010). A resposta terapêutica completa ao tratamento foi alcançada em 66.7% dos casos de HAIAG (vs. 15,2% na EAHAIC, p=0,018). Conclusões: A HAIAG é rara (8.3%), e os doentes com esta apresentação mostraram testes de função hepática mais preservados, sugerindo que a maioria dos casos com perda de função são EAHAIC. Além disso, os doentes com HAIAG tiveram maior taxa de resposta terapêutica completa, sugerindo que uma função hepática mais preservada na apresentação e o diagnóstico precoce tem uma implicação terapêutica positiva.

Abstract Patient Blood Management (PBM) is a holistic approach to managing blood as a resource of each patient; it is a multimodal strategy that is implemented using a set of techniques that can be applied in individual cases. In fact, the overall result of the implementation of PBM cannot be fully appreciated or explained by simply summing up the effects of the individual strategies and techniques used, since they can only produce the expected ideal result if combined. Abstract Patient Blood Management (PBM) is a multidimensional approach that seeks to optimize the use of blood and its components in patients. This matter emerged as a response to the need to reduce unnecessary exposure to blood transfusions and their potential risks. In the past, blood transfusion was often overused resulting in complications and high costs. The advent of Patient Blood Management has caused a paradigm shift, highlighting anemia prevention, bleeding control and maximizing the production of blood cells by the organism itself. Patient Blood Management guidelines include the early identification of anemia, strategies to minimize blood loss during surgery, intraoperative blood conservation techniques, preoperative hemoglobin optimization and evidence-based approaches to the rational use of blood transfusions. Aiming to improve clinical outcomes, decrease transfusion-related complications and reduce associated costs, this multidisciplinary approach counts on doctors, nurses, pharmacists and other healthcare professionals. Based on research and clinical evidence, Patient Blood Management continues to evolve thereby promoting safer, more effective patient-centered practices. Its implementation has proven beneficial in various medical contexts thereby contributing to improvements in the quality of care provided to patients. Our goal with this Consensus is to present readers with a broad and diverse view of Patient Blood Management so that they have the building blocks to implement this new technique. Abstract Background Systemic sclerosis (SSc) is a rare chronic autoimmune disease with heterogeneous manifestations. In the last decade, several clinical trials have been conducted to evaluate new treatment options for SSc. The purpose of this work is to update the recommendations of the Brazilian Society of Rheumatology in light of the new evidence available for the pharmacological management of SSc. Methods A systematic review including randomized clinical trials (RCTs) for predefined questions that were elaborated according to the Patient/Population, Intervention, Comparison, and Outcomes (PICO) strategy was conducted. The rating of the available evidence was performed according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE) methodology. To become a recommendation, at least 75% agreement of the voting panel was needed. Results Six recommendations were elaborated regarding the pharmacological treatment of Raynaud’s phenomenon, the treatment (healing) and prevention of digital ulcers, skin involvement, interstitial lung disease (ILD) and gastrointestinal involvement in SSc patients based on results available from RCTs. New drugs, such as rituximab, were included as therapeutic options for skin involvement, and rituximab, tocilizumab and nintedanib were included as therapeutic options for ILD. Recommendations for the pharmacological treatment of scleroderma renal crisis and musculoskeletal involvement were elaborated based on the expert opinion of the voting panel, as no placebo-controlled RCTs were found. Conclusion These guidelines updated and incorporated new treatment options for the management of SSc based on evidence from the literature and expert opinion regarding SSc, providing support for decision-making in clinical practice. Duplex stainless steels are widely used due to their good mechanical properties and corrosion resistance. However, they are susceptible to microstructural fragility related to harmful transformations taking place in intermetallic phases, such as the sigma phase. These transformations reduce mechanical properties like toughness and corrosion resistance. The current study aims to analyze the effect of different thermodynamic- and electrochemical-reactive phases based on using both linear sweep voltammetry tests as non-destructive tests, as well as the concept of microelectrodes. This technique is capable of detecting small amounts of intermetallic phases at the UNS-S31803 duplex stainless steel surface. Results have evidenced the influence of reduced electrode-solution exposure area on improving sensitivity for deleterious phase analyses. Peak potential and current peak recorded direct correlation to sigma phase concentration and it enabled detecting very low sigma-phase surface concentration, at the order of 0.04%. RESUMO Objetivo: compreender a percepção dos idosos quanto à violência física, à psicológica e ao abandono. Método: estudo transversal, descritivo, de abordagem qualitativa, fundamentado na Teoria das Representações Sociais. Desenvolvido com idosos cadastrados em uma Unidade Básica de Saúde do município de Campina Grande-PB, Brasil, entre os meses de abril e maio de 2022 através da coleta de dados a partir de uma entrevista semiestruturada. Resultados: participaram do estudo 14 idosos, sendo 86% (12) do sexo feminino com idade entre 61 e 83 anos. Os resultados demonstraram o conhecimento que os idosos possuem sobre as tipologias abordadas, sendo a violência psicológica a menos entendida por eles; em relação ao abandono, os idosos descreveram como solidão e asilo. Conclusão: este estudo permitiu a compreensão do conceito de violência pela pessoa idosa, o que contribui substancialmente para ações de saúde, em especial pelos profissionais da enfermagem na identificação e intervenção assertiva. ABSTRACT Objective: To understand the perception of the elderly regarding physical violence, psychological violence, and abandonment. RESUMEN Objetivo: conocer la percepción de las personas mayores sobre la violencia física, la violencia psicológica y el abandono. Método: estudio descriptivo transversal con un enfoque cualitativo, basado en la Teoría de las Representaciones Sociales. Desarrollado con personas mayores registradas en una Unidad Básica de Salud del municipio de Campina Grande-PB, Brasil, entre los meses de abril y mayo de 2022, a través de la recogida de datos basada en una entrevista semiestructurada. Resultados: Participaron en el estudio 14 ancianos, de los cuales el 86% (12) eran del sexo femenino y tenían entre 61 y 83 años. Los resultados mostraron que los ancianos son conscientes de las tipologías discutidas, siendo la violencia psicológica la menos comprendida por ellos; en relación con el abandono, los ancianos lo describieron como soledad y asilo. Resumo Fundamento: A adesão à uma alimentação adequada em macronutrientes é fundamental para a prevenção secundária de doenças cardiovasculares. Objetivo: Avaliar a prevalência de adesão às recomendações de consumo de ácidos graxos para prevenção e tratamento de doenças cardiovasculares, e estimar se a presença de determinados fatores de risco cardiovascular estaria associada à adesão. Métodos: Estudo transversal com os dados de linha de base de 2358 participantes do estudo "Brazilian Cardioprotective Nutritional Program Trial". Dados de consumo alimentar, e fatores de risco cardiovascular foram avaliados. Foi considerada, de acordo com a Sociedade Brasileira de Cardiologia, uma ingestão adequada de ácidos graxos poli-insaturados (AGPI) ≥10% do consumo total de energia diária, para ácidos graxos monoinsaturados (AGM), 20% e para ácidos graxos saturados (AGS), <7%. Na análise estatística foi considerando nível de significância de 5%. Resultados: Nenhum participante aderiu a todas as recomendações de forma simultânea e mais da metade (1482 [62,9%]) não aderiu a nenhuma recomendação. A adesão exclusivamente à recomendação de AGS foi a mais prevalente, sendo cumprida por 659 (28%) dos participantes, seguida da adesão exclusivamente à recomendação de AGP (178 [7,6%]) e de AGM (5 [0,2%]). Não houve associação entre o número de comorbidades e a adesão às recomendações nutricionais (p =0,269). Os participantes da região Nordeste do país apresentaram maior proporção de adesão às recomendações para consumo de AGS (38,42%), e menor para ingestão de AGPI (3,52%) (p <0,001) em comparação às demais. Background: Adhering to a diet adequate in macronutrients is crucial for the secondary prevention of cardiovascular diseases. Objective: To assess the prevalence of adherence to recommendations for the consumption of dietary fatty acids for the prevention and treatment of cardiovascular diseases and to estimate whether the presence of certain cardiovascular risk factors would be associated with adherence. Methods: Cross-sectional study using baseline data from 2,358 participants included in the "Brazilian Cardioprotective Nutritional Program Trial". Dietary intake and cardiovascular risk factors were assessed. Adequate intake of polyunsaturated fatty acids (PUFA) was considered as ≥10% of total daily energy intake; for monounsaturated fatty acids (MUFA), 20%; and for saturated fatty acids (SFA), <7% according to the Brazilian Society of Cardiology. A significance level of 5% was considered in the statistical analysis. Results: No participant adhered to all recommendations simultaneously, and more than half (1,482 [62.9%]) did not adhere to any recommendation. Adherence exclusively to the SFA recommendation was the most prevalent, fulfilled by 659 (28%) participants, followed by adherence exclusively to the PUFA (178 [7.6%]) and MUFA (5 [0.2%]) recommendations. There was no association between the number of comorbidities and adherence to nutritional recommendations (p = 0.269). Participants from the Brazilian Northeast region showed a higher proportion of adherence to SFA consumption recommendations (38.42%) and lower adherence to PUFA intake (3.52%) (p <0.001) compared to other regions. Conclusions: Among the evaluated sample, there was low adherence to nutritional recommendations for dietary fatty acid consumption. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo As áreas de reserva legal (ARLs) são parte fundamental da estratégia brasileira de conservação, juntamente com as áreas de preservação permanente. As ARLs são destinadas à manutenção da biodiversidade e podem ser manejadas de forma sustentável. Quando essas áreas abrigam ecossistemas dependentes de fogo e pastejo, como os campos nativos e as savanas, devem ser adotadas práticas de manejo adequadas à sua conservação e ao enfrentamento dos efeitos das mudanças climáticas globais. No entanto, esse assunto ainda é pouco discutido no Brasil, e as políticas públicas não são claras a esse respeito. Este artigo de revisão descreve os ecossistemas campestres e savânicos no Brasil, os aspectos legais relacionados com o manejo das ARLs, os cenários climáticos atuais e futuros, e a relação entre clima e risco de incêndios. Também apresenta uma revisão sobre o uso do fogo e do pastejo em ecossistemas campestres e savânicos, os desafios legais relativos à sua aplicação nas ARLs e o uso de geotecnologias no monitoramento destas práticas. Conclui-se que o pastejo e o fogo, como instrumentos de manejo, são adequados às funções das ARLs, desde que praticados segundo normas legais e cientificamente embasadas para evitar os efeitos negativos do seu uso equivocado. Abstract Legal reserve areas (LRAs) are a fundamental part of the Brazilian conservation strategy, together with permanent preservation areas. The LRAs are intended to maintain biodiversity and can be managed sustainably. When these areas are home to ecosystems that depend on fire and grazing, such as native grasslands and savannas, management practices that are suitable for their conservation and for dealing with the effects of global climate change should be adopted. However, this subject is still poorly discussed in Brazil, and public policies are not clear on this matter. This review article describes the grassland and savanna ecosystems in Brazil, the legal aspects related to the management of LRAs, the current and future climate scenarios, and the relationship between climate and fire risk. It also presents a review about the use of fire and grazing in grassland and savanna ecosystems, the legal challenges related to their application in LRAs, and the use of geotechnologies to monitor these practices. The conclusion is that grazing and fire, as management tools, are adequate for LRA functions, as long as they are practiced in accordance with legal and scientifically based standards to avoid the negative effects of their incorrect use. Abstract This study aimed to analyze the physical features and effect of hydroxyapatite (HAp) and Niobium/HAp CaNb2O6/Ca3(PO4)2 composite on critical size defect in rat calvaria. Biomaterials were characterized by X-ray diffraction (XRD), X-ray fluorescence (XRF), Particle size distribution (PSD) and Scanning Electron Microscopy (SEM). Critical size (8 mm) calvarial defects were created in Wistar rats (n = 24) distributed in three groups (90 days): Control group (CG): critical defect no treatment; HAp group: Hydroxyapatite; CNb/CPO group: CaNb2O6/Ca3(PO4)2 composite. After 90 days, calvaria samples were subjected to histological, and immunohistochemical reactions for osteopontin and osteocalcin, and Micro-CT analyses. XRD analysis confirmed the formation of a crystalline structure of CNb/CPO. XRF measurements confirmed the presence of Nb added in the sample CNb/CPO. Fracture toughness from CNb/CPO composite was 0.82 MPa.m1/2. Micro-CT showed increased trabecular number (p˂0.001), decreased porosities (%) (p˂0.001) and decreased trabecular separation (mm) (p˂0.001) in the CNb/CPO compared to CG group. In CNb/CPO group showed presence of new bone in which connective tissue differentiates to form or indicate a bone matrix score 3 (3-3.75, p˂0.05) and strong immunoexpression of osteopontin (p<0.05). The findings indicate that CNb/CPO showed resistance to the propagation of cracks and stimulated bone formation in calvaria defects in rats. ABSTRACT Acute rheumatic fever (ARF) is a complex disease with several clinical manifestations. Its most significant long-term complication is valvular heart damage, commonly referred to as chronic rheumatic heart disease. The risk of ARF varies globally, with over 80% of cases occurring in low- and middle-income countries, highlighting the role of socioeconomic factors. A comprehensive understanding of the risk factors associated with ARF and its clinical, genetic, and sociodemographic mediators can help clinicians identify high-risk individuals, develop effective management strategies, enhance target screening and active case-finding initiatives, and ultimately improve patient outcomes. Abstract The production of the enzymes α-amylase, glucoamylase, and xylanase by Aspergillus niger DR02 was studied using waste from the Brazilian cassava flour industry. Cassava wastewater (manipueira) at 30% (v/v) was used, either supplemented (M30F10) or not (M30) with 10 g/L of cassava peel flour as a carbon source. Variables such as chemical oxygen demand (COD), proteins, reducing sugars, glucose, nitrate, ammonia, and enzymatic activities were monitored. The results for M30F10 and M30 at 30°C and pH 5.00 showed similar COD removals of 82.88%±12.00 and 82.41%±7.00, respectively. Despite the significant reduction in organic load, the final effluent still requires post-treatment before being discharged into the receiving water body. The addition of cassava peel flour favored the maximum production of 18.40 U/mL±0.15 α-amylase in 72 hours of cultivation, 14.80 U/mL±0.91 xylanase in 96 hours, and 4.50 U/mL±0.29 pectinase in 24 hours. Without medium supplementation, the highest glucoamylase activity (19.90 U/mL±1.01) was obtained in 96 hours. The kinetic adjustment to a first-order model (R²: 0.9998) confirmed the highest efficiency for α-amylase production when the medium was supplemented with peel flour (M30F10), with a production rate of 7.07 x 10⁻² h⁻¹, while the highest glucoamylase production rate was obtained in M30 (3.251 x 10⁻² h⁻¹; R²: 0.9975).
