Abstract This work aimed to show which treatments showed efficacy against coronavirus disease 2019 (COVID-19); therefore, the results of 37 clinical trials started in 2020 and completed in 2021 are reviewed and discussed here. These were selected from databases, excluding vaccines, computational studies, in silico, in vitro, and those with hyperimmune sera from recovered patients. We found 34 drugs, one vitamin, and one herbal remedy with pharmacological activity against symptomatic COVID-19. They reduced mortality, disease progression, or recovery time. For each treatment, the identifier and type of trial, the severity of the disease, the sponsor, the country where the trial was conducted, and the trial results are presented. The drugs were classified according to their mechanism of action. Several drugs that reduced mortality also reduced inflammation in the most severe cases. These include some that are not considered anti-inflammatory, such as Aviptadil, pyridostigmine bromide, anakinra, imatinib, baricitinib, and bevacizumab, as well as the combination of ivermectin, aspirin, dexamethasone, and enoxaparin. Nigella sativa seeds with honey have also been reported to have therapeutic activity. On the other hand, tofacitinib, novaferon with ritonavir, and lopinavir were also effective, as well as in combination with antiviral therapies such as danoprevir with ritonavir. The natural products colchicine and Vitamin D3 were only effective in patients with mild-to-moderate COVID-19, as was hydroxychloroquine. Drug repositioning has been the main tool in the search for effective therapies by expanding the pharmacological options available to patients.

Resumen El objetivo del presente trabajo fue conocer qué tratamientos mostraron efectividad contra COVID-19, para lo cual se revisan y discuten los resultados de 37 estudios clínicos iniciados durante 2020 y concluidos en 2021. Estos fueron seleccionados de bases de datos, excluyendo vacunas, estudios computacionales, in silico, in vitro y con sueros hiperinmunes de pacientes recuperados. Se documentaron 34 fármacos, una vitamina y un remedio herbolario, con actividad farmacológica ante COVID-19 sintomático. Estos redujeron la mortalidad, el progreso de la enfermedad, o el tiempo de recuperación. Para cada tratamiento se presenta identificador y tipo de estudio, la gravedad de la enfermedad, patrocinador, país donde se realizó, así como sus resultados. Los fármacos se clasificaron de acuerdo con su mecanismo de acción. Varios fármacos que redujeron la mortalidad también disminuyeron la inflamación en los casos más graves. Esto incluyendo algunos no considerados antiinflamatorios, como el aviptadil, el bromuro de piridostigmina, el anakinra, el imatinib, el baricitinib y el bevacizumab, así como la combinación de ivermectina, aspirina, dexametasona y enoxaparina. También se reportaron con actividad terapéutica las semillas de Nigella sativa con miel. Además, resultaron efectivos el tofacitinib, el novaferón con ritonavir y lopinavir, así como los antivirales en terapias combinadas como el danoprevir con ritonavir. Los productos naturales colchicina y vitamina D3, solo tuvieron actividad en los pacientes en estado leve a moderado de la COVID-19, así como la hidroxicloroquina. El reposicionamiento de fármacos fue la principal herramienta para buscar terapias efectivas ampliando las opciones farmacológicas accesibles a los pacientes.

Resumen Objetivo: Evaluar un dispositivo automatizado para la asistencia ventilatoria basado en un AMBU manufacturado en Marzo del 2020. Métodos: El ESSI-1 INC fue evaluado por medio de parámetros fisiológicos y mecánica pulmonar a través de pruebas de espirómetro compensatorios (TISSOT); pulmón artificial (Modelo 5600i Dual Adult PNEU VIEW SYSTEM); así como su desempeño comparado a la máquina de anestesia (AEONMED 7500) en modelos porcinos, midiendo criterios ventilatorios, hemodinámicos y gasométricos. Resultados: Este equipo (ESSI-1 INC) fue exitosamente probado por modelos mecánicos y biológicos, tales como cerdos donde su desempeño fue evaluado en términos de la variabilidad del volumen tidal, frecuencia ventilatoria, y relación I/E versus el desempeño manual de dos médicos. Todos los resultados finalizaron como se esperaba de forma satisfactoria. Conclusiones: Es un equipo seguro y efectivo, el cual debería ser probado y usado en distintas condiciones clínicas para estandarizar la seguridad ventilatoria y cuidado de pacientes que lo requieran.

Abstract Objective: The objectives of this study were to evaluate an automated device for ventilatory support based on AMBU manufactured in March 2020. Methods: The ESSI-1 INC was evaluated through pulmonary mechanics and physiology parameters through compensatory spirometer tests (TISSOTs), and an artificial lung Model5600i Dual Adult PNEU VIEW SYSTEM; it was also compared to the anesthetic ventilatory support equipment (AEONMED 7500) in porcine models, measuring ventilatory, hemodynamic and gasometric parameters. Results: This equipment (ESSI-1 INC) was successfully tested by mechanical and biological models, such as pigs in which its performance was evaluated in terms of variability of tidal volume, ventilation frequency, and I/E relationship versus the manual performance of two medical interns. All the results turned out as expected and were satisfactory. Conclusions: It is safe and effective equipment and should be tested and used in diverse clinical conditions to standardize the ventilatory safety and care of patients who require it.

Abstract Introduction: In Mexico, cardiac rehabilitation (CR) as an interdisciplinary intervention with therapeutic impact in patients with heart disease is growing. There is the need to know actual conditions of CR in our country. Objectives: The objective of this National Registry is to follow-up those existing and new CR units in Mexico through the comparison between the two previous registries, RENAPREC-2009 and RENAPREC II-2015 studies. This is a descriptive study focused on diverse CR activities such as assistance training, and certification of health professionals, barriers, reference, population attended, interdisciplinarity, permanence over time, growth prospects, regulations, post-pandemic condition, integrative characteristics, and scientific research. Results: Data were collected from 45 CR centers in the 32 states, 75.5% are private practice units, 67% are new, 33% were part of RENAPREC II-2015, and 17 have continued since 2009. With a better distribution of CR units along the territory, the median reference of candidates for CR programs is 9% with a significant reduction into tiempo of enrollment to Phase II admission (19 ± 11 days). Regarding to previous registries, the coverance of Phases I, II, and III is 71%, 100%, and 93%, respectively; and a coverance increases in evaluation, risk stratification, and prescription, more comprehensive attendance and prevention strategies. Conclusions: CR in Mexico has grown in the past 7 years. Even there is still low reference and heterogeneity in specific processes, there are strengths such as interdisciplinarity, scientific professionalization of specialists, national diversification, and an official society that are consolidated over time.

Resumen Antecedentes: Mesoamérica es uno de los hotspots de biodiversidad más importantes del planeta. A pesar de importantes esfuerzos realizados durante más de dos siglos para contribuir al conocimiento florístico de esta región, el conocimiento de su flora todavía es disperso y desigual. Preguntas: ¿Cuál es la magnitud de la riqueza de plantas vasculares de la cuenca del río Usumacinta? Sitio y años de estudio: Cuenca del río Usumacinta (Guatemala y México), 1838-2018. Metodos: Se integró y sistematizó información florística a partir de diferentes fuentes de información derivadas de estudios florísticos y ecológicos. Resultados: Se registraron 6,977 especies, 1,892 géneros y 274 familias. El mayor número de especies (5,746) y registros (58,859) fueron de la porción mexicana, en comparación con su contraparte guatemalteca (4,445 especies y 19,952 registros). Las familias con mayor riqueza de especies fueron Orchidaceae (598), Fabaceae (512), y Asteraceae (476). La prevalencia de estas y todas las demás familias con contribuciones significativas a la flora varió entre los tres sectores altitudinales en los que se subdividió la cuenca (cuenca baja, cuenca media y cuenca alta). Conclusiones: La cuenca del río Usumacinta es una región estratégica para la conservación de la biodiversidad vegetal, con casi una tercera parte de las especies de plantas vasculares conocidas para Mesoamérica y ca. de 6 % de la flora del continente americano. Los esfuerzos de exploración botánica deben centrarse en áreas de la cuenca en donde la información disponible es escasa o nula a fin de obtener una mejor apreciación de su flora.

Abstract Background: Mesoamerica is one of the most important biodiversity hotspots on the planet. Despite significant efforts made over two centuries to contribute to the floristic knowledge of this region, our understanding of its flora is still scattered and uneven. Questions: What is the magnitude of the vascular plant species richness in the Usumacinta River Basin? Study site and dates: Usumacinta River Basin (Guatemala and Mexico), 1838-2018. Methods: We compiled the checklist by systematizing the floristic information acquired from various sources derived from numerous floristic and ecological studies. Results: We recorded 6,977 species, 1,892 genera and 274 families. The largest numbers of species (5,746) and records (58,859) correspond to the Mexican portion of the Usumacinta River Basin, compared to its Guatemalan counterpart (4,445 species and 19,952 records). The most species-rich families were Orchidaceae (598 species), Fabaceae (512), and Asteraceae (476). The prevalence of these and all other families with significant contributions to the flora varied among three elevation-defined sectors into which the Usumacinta River Basin was subdivided (lower, middle, and upper basin). Conclusions: The Usumacinta River Basin is a strategic region for plant biodiversity conservation as it hosts almost one third of all vascular plant species known for Mesoamerica and ca. 6 % of the entire flora in the Americas. Further botanical exploration should focus on those areas of the basin for which little or no information is available in order to gain a better appreciation of its flora.

ABSTRACT Background Reduced or null expression of E-cadherin protein is a frequent cause of diffuse gastric cancer (DGC). More than 50% of patients with DGC present somatic variants in CDH1 gene. Objectives The objectives of this study were to study E-cadherin expression and identify variants in the CDH1 gene in gastric tumors of patients with DGC. Methods We studied 18 Mexican DGC patients who attended a hospital of the Mexican Social Security Institute; E-cadherin expression was determined by immunohistochemistry, and variants were identified by Sanger sequencing in promoter and coding regions. Predictive analysis was performed using PolyPhen-2 and HOPE software. Results We found that 56% of DGC patients showed reduced expression of E-cadherin. All patients carried CDH1 variants; overall, 12 different CDH1 variants were identified. Predictive analysis revealed that the rs114265540 variant was probably damaging, with a value of 0.985, indicating a functional impact on the E-cadherin protein. Variants rs34939176 and rs33964119 were identified as risk factors for DGC (odds' ratios [OR] = 31.3, 95% CI 6.3-154.0, p < 0.001; OR = 6.1, 95% CI 2.0-19.0, p < 0.001, respectively) given their elevated frequency and by comparing it with those reported for MXL population in the 1000 Genomes Project database. Conclusions In this Mexican population, the percentage of diffuse gastric tumors with reduced expression of E-cadherin was similar to that reported in other populations. All gastric tumors of DGC patients studied had somatic CDH1 gene variants; however, the rs114265540, rs34939176, and rs33964119 variants were importantly related to DGC.

ABSTRACT The genus Bartonella encompasses 38 validated species of Gram-negative, facultative intracellular bacteria that colonize the endothelial cells and erythrocytes of a wide spectrum of mammals. To date, 12 Bartonella species have been recorded infecting humans, causing diseases of long historical characterization, such as cat scratch fever and trench fever, and emerging bartonellosis that mainly affect animal health professionals. For this reason, this study aimed to report a documented case of Bartonella bovis infecting a veterinarian from Mexico by the amplification, sequencing and phylogenetic reconstruction of the citrate synthase (gltA) and the RNA polymerase beta-subunit (rpoB) genes, and to report the natural course of this infection. Resumen Objetivo: Analizar los cambios en la frecuencia y la incidencia de la enfermedad del ojo seco (EOS) y los tiempos de exposición a pantallas al inicio de cursos en línea, y comparar estos cambios entre sexos. Secundariamente, correlacionar los tiempos de exposición con la gravedad de los síntomas. Método: Se aplicaron cuatro encuestas idénticas durante 6 semanas, las cuales contenían el Ocular Surface Disease Index (OSDI) que cuantifica la gravedad de los síntomas de EOS y preguntas sobre tiempos de exposición a pantallas. Se incluyeron universitarios que transicionaron de cursos presenciales a plataformas en línea. Resultados: La frecuencia de EOS entre los 97 participantes (54 mujeres y 43 hombres) alcanzó su máximo en la semana 4 (82.47%). Las puntuaciones OSDI aumentaron significativamente (p < 0.0001) entre la situación basal (27.01 ± 17.55) y la semana 6 (37.17 ± 24.64), reflejando un empeoramiento sintomatológico y una incidencia del 8.5%. Este empeoramiento se observó en las mujeres (p < 0.0001), pero no en los hombres (p = 0.11); se encontraron diferencias significativas entre sexos en la situación basal (p = 0.01), la semana 2 (p = 0.02) y la semana 6 (p = 0.008), pero no en la semana 4 (p = 0.11). El inicio de los cursos en línea aumentó el tiempo frente a la pantalla (p < 0.0001). Las horas basales fueron 25.52 ± 11.33 y alcanzaron su máximo en la semana 2, con 34.62 ± 10.90 horas. Las puntuaciones OSDI y los tiempos de exposición de los cursos en línea se correlacionaron significativamente (semana 2, R = 0.265; semana 4, R = 0.262; semana 6, R = 0.205). Conclusiones: Los universitarios sufren síntomas graves de EOS que se correlacionaron con el inicio de los cursos en línea. Las instituciones educativas deberían fomentar la salud ocular.

Abstract Objective: To analyze changes in the frequency and incidence of dry eye disease (DED) and screen exposure times during the onset of online courses; moreover, compare these changes between sexes. Secondarily, correlate online courses screen exposure times with the severity of DED symptoms. Method: Four identical surveys, each containing the Ocular Surface Disease Index (OSDI) which quantifies DED symptom severity, and questions which categorized screen exposure times, were applied throughout 6 weeks. University students who had transitioned from face-to-face courses to online platforms were included. Results: DED frequency among the 97 subjects (54 women, 43 men) peaked on week 4 (82.47%). OSDI scores significantly increased throughout the study (p < 0.0001), from baseline (27.01 ± 17.55) to week 6 (37.17 ± 24.64), reflecting symptom worsening along an incidence of 8.5%. This worsening of symptoms occurred in women (p < 0.0001), while in male subjects it did not (p = 0.11). Significant differences between sexes were found during the baseline (p = 0.01), week 2 (p = 0.02), and week 6 (p = 0.008), but not on week 4 (p = 0.11). Online courses onset significantly increased screen exposure time (p < 0.0001). The baseline hours were 25.52 ± 11.33 and peaked on week 2, being 34.62 ± 10.90 hours. OSDI scores and online courses exposure times correlated significantly (week 2, R = 0.265; week 4, R = 0.262; week 6, R = 0.205). Conclusions: University students suffer from severe DED symptoms, which correlate with online courses onset. Educational institutions should foster ocular health.

Abstract Objective: The objective is to describe the role of leadership in the training of the general surgeon and to point out its relevance. Method: A search of the medical literature was carried out to identify studies that reported on topics related to leadership in the context of medical education and surgery. We used the search engine of Medline/PubMed databases including related terms such as “leadership” AND “medicine” OR “surgery”. Results: Leadership is a valued characteristic in surgery, it implies professionalism, technical competence, motivation, innovation, teamwork, communication skills, decision making, emotional competence, resilience, and effective teaching. Leadership is measurable and can be developed through experience, observation, and education. Conclusions: Priority must be given to actions that improve leader traits. This has been shown to create a work environment where efficient work is performed, and failures are minimized. The current vision of the leader must be expanded to reduce the gender gap between the positions as leader occupy in this moment.

Resumen Objetivo: Describir el papel de liderazgo en la formación del cirujano general y puntualizar su relevancia en nuestro entorno. Método: Se realizó una búsqueda en la literatura médica para identificar estudios que informaran sobre temas relacionados al liderazgo en el contexto de la enseñanza médica y en cirugía. Se utilizó el motor de búsqueda de las bases de datos Medline/PubMed incluyendo términos relacionados como “leadership” AND “medicine” OR “surgery”. Resultados: El liderazgo es una característica valorada en cirugía, que implica profesionalismo, competencia técnica, motivación, innovación, trabajo en equipo, habilidades comunicativas, toma de decisiones, competencia emocional, resiliencia y enseñanza eficaz. Es medible y susceptible de desarrollarse a través de la experiencia, la observación y la educación. Conclusiones: En la formación del cirujano general se deben priorizar las acciones que mejoren los rasgos de líder. Se ha demostrado que esto genera un ambiente de trabajo donde se realiza una labor eficiente y se minimizan las fallas. Se debe expandir la visión actual que se tiene de líder y permitir que se disminuya la brecha de género entre los puestos que hoy se ocupan.

Resumen Introducción: El propósito del estudio es describir las características clínicas y quirúrgicas de los pacientes con enfermedad hemorroidal, sangrado activo y anemia con necesidad de cirugía. Metodología: Se trata de un estudio descriptivo y retrospectivo de 510 pacientes de ambos sexos diagnosticados con enfermedad hemorroidal entre 2015 y 2019. Se revisaron los expedientes de los pacientes sometidos a cirugía urgente con presencia de sangrado, anemia, choque y otros problemas clínicos. El análisis estadístico fue realizado con el software SPSS version 24. Resultados: El 10.3% (53) de los pacientes fueron diagnosticados con sangrado hemorroidal activo al momento de la revisión y con datos clínicos y bioquímicos de anemia. El promedio de hemoglobina de estos fue de 7.5 g/dl. La enfermedad hemorroidal grado II fue la más frecuente. Se realizó hemorroidectomia tipo Ferguson en el 92.5% (49) de los casos; 5.7% (3) fueron operados con energía avanzada y solo 1.9% (1) con la técnica Milligan-Morgan. Conclusiones: La hemorroidectomia de urgencia es el tratamiento de elección en el sangrado activo severo por enfermedad hemorroidal.

Abstract Introduction: The purpose of the study is to describe the clinical and surgical characteristics of patients with hemorrhoidal disease (HD), anemia, and active bleeding in need of urgent surgery. Methods: It is a descriptive, retrospective study of 510 patients between 2015 and 2019. Male and female patients diagnosed with HD. The records of these patients were reviewed, and we identified those who underwent emergency surgery due to active bleeding, anemia, shock, and other clinical problems. Statistical analysis was carried out with the SPSS version 24 software. Results: Fifty-three (10.3%) patients met the criteria for presenting active hemorrhoidal bleeding at the time of review and clinical and biochemical data of anemia. Patients presented a mean hemoglobin of 7.5 g/dl. The most frequent degree of HD was Grade II. The treatment received in 92.5% of the cases was hemorrhoidectomy with the Ferguson technique; in 5.7% (3), a hemorrhoidectomy was performed with an advanced energy device, and 1.9% (1) underwent the Milligan-Morgan technique. Conclusions: Emergency hemorrhoidectomy is the treatment of choice for patients with severe active bleeding from HD.

Abstract Background: Pulmonary involvement in juvenile systemic sclerosis (JSSc) is rare in children and contributes to morbimortality. This study aimed to describe the pulmonary function and clinical, radiologic, and tomographic findings in JSSc. Methods: Patients with JSSc between 5-14 years of age were included. Clinical, functional, and imaging characteristics were assessed. Patients were excluded if they showed lung disease not associated with JSSc: mixed connective tissue disease, overlap syndrome, or acute cardiopulmonary failure at the time of the study. All patients underwent physical examination, electrocardiogram, spirometry, chest X-ray, high-resolution computed tomography (HRCT) of the chest, echocardiography, lung function tests, and the 6-minute walk test (6-MWT). Descriptive statistics were employed for data analysis. Results: We studied 15 patients with the following characteristics: median age, 11 years; median since symptoms onset, 6 years; median since JSSc diagnosis and the finding of pulmonary involvement, 2 years. Lung disease was detected in 73%, interstitial lung disease (ILD) the most common affection (67%); pulmonary hypertension was found in 6.6%. 6-MWT was positive in 26.6%, forced vital capacity (FVC) was abnormal in 26.6%. No pulmonary involvement was found in four patients. Conclusions: The most frequent pulmonary affection in JSSc was ILD. Thus, early JSSc detection and periodic lung monitoring are mandatory to avoid further complications once JSSc is diagnosed.

Resumen Introducción: La afección pulmonar en la esclerosis sistémica juvenil (ESJ) es rara en niños y contribuye a la morbimortalidad. El objetivo de este estudio fue describir los hallazgos de función pulmonar, clínicos, radiológicos y tomográficos en la ESJ. Métodos: Se incluyeron pacientes con ESJ de 5-14 años de edad. Se evaluaron las características clínicas, funcionales y de imagen. No se incluyeron pacientes con enfermedades pulmonares no asociadas con ESJ en el momento del estudio: enfermedad mixta del tejido conectivo, síndrome de superposición o insuficiencia cardiopulmonar aguda. Se realizaron exploración física, electrocardiograma, espirometría, radiografía de tórax, tomografía computarizada de alta resolución de tórax, ecocardiografía, pruebas de función pulmonar y prueba de caminata de 6 minutos (PC6M). Se utilizó estadística descriptiva para el análisis de los datos. Resultados: Se estudiaron 15 pacientes con las siguientes características: mediana de edad, 11 años; mediana desde el inicio de los síntomas, 6 años; y mediana desde el diagnóstico de ESJ y hallazgo de afección pulmonar, 2 años. Se detectó enfermedad pulmonar en el 73%. La enfermedad pulmonar intersticial (EPI) fue la afección más común (67%) y se encontró hipertensión pulmonar en el 6.6%. La PC6M fue positiva en el 26.6%, y la capacidad vital forzada resultó anormal en el 26.6%. Cuatro pacientes no presentaron afección pulmonar. Conclusiones: La afección pulmonar más frecuente en la ESJ fue la EPI. La detección temprana de ESJ y la monitorización pulmonar periódica son obligatorias para evitar más complicaciones una vez diagnosticada la ESJ.

Background: Healthcare-associated infections (HAIs) are important adverse events that must be prevented. Objective: The objective of the study was to report and study possible changes in HAI rates as well as their causes after the COVID-19 hospital surge capacity response (HSCR) in an academic referral center. Methods: This was a before-after observational study. The Infection Prevention and Control (IPC) program (prospective surveillance, prevention bundles, antibiotic stewardship, continuing education, and feedback) was transiently disrupted after the start of HSCR (March 2020). HAI rates were compared before (January 2019-February 2020) and after (April-July 2020) HSCR, and plausible predisposing factors in affected patients were compared. Results: An increase in the HAI rate from 6.2 to 11.8 cases/1000 patient-days was noted between periods due to increases in ventilator-associated pneumonia and bloodstream infection (BSI) rates. More critically ill patients were admitted during HSCR, and use of invasive devices increased. Prone positioning and infusion of muscle relaxants became commonplace. The nurse-to-patient ratio in the intensive care unit decreased, and 4 h shifts were introduced to avoid fatigue. The BSI rate decreased after the IPC program with additional measures was reintroduced in May 2020. Conclusions: The strain on the workforce and modifications to the IPC program very possibly underlay the findings. IPC programs continue to be essential during the pandemic.

Resumen Introducción: El gateo representa la primera forma de locomoción autónoma. Se han mencionado las implicaciones de la adquisición del gateo para lograr la marcha independiente y el control motor en el niño, pero son pocos los estudios relacionados con el gateo y sus efectos en el niño de alto riesgo biológico. Por eso se intentó conocer la relación entre la adquisición del gateo y la marcha independiente en una población de niños nacidos de alto riesgo en un programa de seguimiento pediátrico. Material y métodos: Estudio observacional, retrospectivo y descriptivo de una cohorte de niños de alto riesgo que acuden al seguimiento pediátrico, en el cual se revisó el periodo de adquisición del patrón de gateo y la marcha independiente. Resultados: Se integraron cuatro grupos: gateo normal, gateo limítrofe, gateo con retraso y gateo nulo. Se estudió a 558 lactantes; los grupos se integraron con gateo normal, 238 niños; gateo limítrofe, 96 lactantes; retraso en la adquisición del gateo, 207 niños; y gateo nulo, 17 niños. Por género, las niñas gatean mejor, con peso y edad gestacional mayores y predominio en los gateadores. La escala de Bayley señala mejores puntuaciones para los gateadores. En los niños con gateo normal, la marcha independiente se adquirió en el periodo normal a diferencia del grupo con retraso en el gateo en el cual la marcha independiente apareció con retraso. Conclusiones: En este estudio se identificó una relación entre la edad de inicio del gateo con la edad de adquisición de la marcha independiente, con mejor evolución en los niños gateadores.

Abstract Background: Crawling represents the first form of autonomous human locomotion. Much has been said about the implications that an adequate acquisition of crawling has on development in order to achieve independent gait in the short term and the child’s motor control in the long term. There are few studies related to crawling and its implications in children who were high biologic risk newborns. Therefore, we wanted to know the relation between crawling acquisition and independent gait in a population of children who were high risk at birth in our Pediatric Follow up clinic. Material and Methods: An observational, retrospective, and analytical study of a cohort of children who were high risk at birth, and attended our pediatric follow-up clinic was done. The period between crawling acquisition and independent gait was reviewed. Results: 4 groups were integrated; normal crawling, borderline crawling, delayed crawling and null crawling. 558 infants were studied; the groups were integrated by: normal crawling 238 children; borderline crawling with 96 infants; delayed crawling with 207 children and null crawling with 17 children. By gender distribution, girls achieved better crawling. Weight and higher gestational age predominated in children with normal crawling. Crawlers had the best scores in the Bayley Scale. In children with normal crawling, independent gait was acquired within the normal period unlike the group with delay in crawling where independent gait was behind. Conclusions: In this study, we found a relation between the age of onset of crawling with the age of acquisition of the independent gait, with better skill in children who crawled.

Abstract Background: Wallenberg syndrome (WS) is a classic neurologic disorder secondary to vascular pathology of the vertebrobasilar circulation. Methods: Consecutive patients > 18 years with WS and isolated lateral medullary infarction syndrome were included from two Mexican hospitals. Risk factors, initial signs, symptoms, radiological findings, and acute treatment were evaluated; the prognosis was assessed using the Glasgow Outcome Scale. Results: Twenty-six patients were studied in a 26-year period (1988-2014); 17 patients were men (67%); and the average age was 45 years (range 19-77). Fourteen patients were under 45 years old (54%). The most common risk factors were dyslipidemia (41%) and hypertension (37%). The main initial symptoms were vertigo-dizziness (89%) gait ataxia (70%), and crossed-sensory deficit; and the main signs were crossed-sensitive deficit (93%), Horner syndrome (85%), and nystagmus (82%). In the bivariate analysis, age under 45 years was associated with a vertebral arterial dissection (p = 0.001), and age > 45 with atherothrombotic etiology (p = 0.01). About 96% of patients presented good recovery at an average of 17 months follow-up. Conclusion: Non-atherosclerotic vasculopathy was the main cause of WS-ILBI in young people. The clinical characteristics were similar to those reported in other series with a usually benign prognosis.

Resumen Antecedentes: El síndrome de Wallenberg (SW) es un diagnóstico clásico en neurología, generalmente secundario a patología vascular de la circulación vertebro-basilar. Métodos: Se incluyeron a pacientes mexicanos consecutivos mayores de 18 años de edad con síndrome de SW e infarto bulbar lateral aislado (SW-IBLA) en dos hospitales mexicanos. Se evaluaron los factores de riesgo, signos y síntomas iniciales, hallazgos radiológicos, tratamiento agudo y pronóstico mediante el Glasgow Outcome Scale (GOS). Resultados: Se estudiaron 26 pacientes en un periodo de 26 años (1988-2014); Diecisiete pacientes fueron hombres (67%); el promedio de edad fue de 45 años (rango 19-77). Catorce pacientes fueron menores de 45 años (54%). Los factores de riesgo más frecuentes fueron dislipidemia (41%) e hipertensión arterial (37%). Los principales síntomas al inicio fueron vértigo-mareo (89%) y ataxia para la marcha (70%), así como déficit sensorial cruzado (93%), síndrome de Horner (85%) y nistagmo (82%) a la exploración neurológica. En el análisis bivariado, la edad menor a 45 años se asoció a disección arterial vertebral (p = 0.001), y la edad > 45 a etiología aterotrombótica (p = 0.01). El 96% de los pacientes presentaron buena recuperación en un seguimiento promedio de 17 meses. Conclusión: La vasculopatía no ateroesclerosa fue la principal causa de SW-IBLA en jóvenes. Las características clínicas para esta localización fueron similares a lo reportado en otras series con un pronóstico habitualmente benigno.

Abstract Phthalimide is considered a scaffold for the development of new anticancer agents. In this work, the antiproliferative activity of forty-three phthalimide derivatives was evaluated against cervical (HeLa), liver (HepG2), breast (4T1) cancer cell lines, and a normal cell line of murine fibroblasts (3T3). Finally, a molecular docking analysis of phthalimide derivatives on the active site of the enzymes DNA methyltransferase 1 (DNMT1) and vascular endothelial growth factor receptor 2 (VEGR2) as potential drug targets was performed. The compounds, C16, E11, and E16 showed the best antiproliferative activity against the cell lines HeLa and 4T1. Only, the compound H16 decreased 32% cell proliferation against HepG2 cell line. The compounds H5, H16, E2, E16, and C1 did not affect the proliferation of the 3T3 cell line. The molecular docking analysis showed that phthalimide derivatives have a greater affinity for DNMT1 than S-adenosyl-l-homocysteine, a potent DNMT1 inhibitor. However, molecular docking results do not correlate with their antiproliferative effects, suggesting another potential mechanism of action for the active compounds.

Resumen Laestructura de la ftalimida esconsiderada un bloque de construcción para el desarrollo de nuevos agentes anticancerígenos. En este trabajo, se evaluó la actividad antiproliferativa de cuarenta y tres derivados de ftalimida contra las líneas celulares cancerígenas de cérvix (HeLa), hígado (HepG2), mama (4T1), y la línea celular normal de fibroblastos murinos (3T3). Por último, se realizó un análisis de acoplamiento molecular de los derivados de la ftalimida en el sitio activo de la enzima metiltransferasa 1 de DNA (DNMT1, por sus siglas en inglés) y el receptor del factor de crecimiento endotelial vascular 2 (VEGR2, por sus siglas en inglés) como posibles blancos farmacológicos. Los compuestos C16, E11 y E16 mostraron la mejor actividad antiproliferativa contra las líneas celulares HeLa y 4T1. Solamente, el compuesto H16 disminuyó 32% la proliferación celular de la línea HepG2. Los compuestos H5, H16, E2, E16 y C1 no afectaron la proliferación celular de la línea 3T3. El análisis de acoplamiento molecular demostró que los derivados de la ftalimida tienen una mayor afinidad que la S-adenosil-l-homocisteína, un potente inhibidor de la metiltransferasa 1 de DNA. Sin embargo, los resultados del acoplamiento molecular no se correlacionan con los efectos antiproliferativos; lo cual sugiere que los compuestos activos tienen otro mecanismo de acción.
