Resumen El uso de ácido poliláctico (PLA) en la tecnología por deposición fundida (FDM) más conocida como impresión 3D para crear todo tipo de piezas, desde prototipos y piezas simples hasta productos finales altamente técnicos es un gran un competidor debido a su amplio rango de propiedades frente a otros materiales compuestos de origen petroquímico. El objetivo de este trabajo fue evaluar el uso de ácido poliláctico (PLA) para la construcción de vehículos aéreos no tripulados (UAV) por medio de impresión 3D. Se realizó el cálculo de la potencia y la fuerza del UAV, un análisis estructural en el software SolidWorks para calcular los esfuerzos máximos y mínimos y determinar los parámetros óptimos de elaboración. Así mismo, se realizó un análisis aerodinámico mediante el software ANSYS para determinar el coeficiente de arrastre y las fuerzas que intervienen en el UAV. Se encuentra que la fuerza que ejercen los motores sobre los bazos es de 13,63 N y la potencia es de 536N; en el análisis estructural se encontró que los esfuerzos mínimos se encuentran en los brazos del chasis y los esfuerzos máximos se encuentra donde se ajusta el chasis con un valor de 6,604 MPa y un factor de seguridad de 3,23; en el análisis aerodinámico se encontró una fuerza de arrastre de 1N y una potencia de vuelo de 108,44 N. El UAV alcanzó una altura de vuelo de 48 m y una autonomía de vuelo de 17 minutos concluyendo que la tecnología de impresión 3D es una alternativa viable y de bajo costo.

Abstract The use of polylactic acid (PLA) in the technology of fused deposition modeling (FDM), better known as 3D printing, to create all kinds of parts, from prototypes and simple pieces to highly technical end products, is a significant contender due to its wide range of properties compared to other petrochemical-based composite materials. The aim of this study was to evaluate the use of polylactic acid (PLA) for the construction of unmanned aerial vehicles (UAVs) through 3D printing. Power and force calculations for the UAV were performed, and a structural analysis using SolidWorks software was conducted to calculate maximum and minimum stresses and determine optimal manufacturing parameters. Additionally, an aerodynamic analysis using ANSYS software was carried out to determine the drag coefficient and the forces acting on the UAV. It was found that the force exerted by the motors on the arms is 13,63 N, and the power is 536 N. In the structural analysis, it was determined that the minimum stresses are located in the chassis arms, while the maximum stresses are found where the chassis is attached, with a value of 6,604 MPa and a safety factor of 3,23. In the aerodynamic analysis, a drag force of 1 N and a flight power of 108,44 N were calculated. The UAV achieved a flight height of 48 m and a flight autonomy of 17 minutes, concluding that 3D printing technology is a viable and cost-effective alternative.

Abstract Background: Cardiac resynchronization therapy (CRT) has been established as an effective therapy for heart failure with reduced ejection fraction. Randomized clinical trials have shown its impact on mortality and HF hospitalizations, as well as improvement of symptoms and quality of life. Objectives: Finding clinical, electrocardiographic, and echocardiographic variables that may predict the response to cardiac resynchronization therapy (CRT). Methods: We performed a single-center, observational, analytic, and retrospective study that included 102 patients with heart failure (HF) diagnosis who underwent CRT according to guideline-directed therapy from January 2010 to April 2020 in a third-level center. CRT response was defined as an improvement of New York Heart Association functional class in at least 1 category associated with a recovery of ≥ 5% in the left ventricular ejection fraction (LVEF). Results: Our study population was 102 patients of which 61 (59.8%) were men. The mean age at HF diagnosis was 54 ± 18.7 years. Ischemic heart disease was the etiology in 37 (36.3%) cases. Fifty-one (50%) patients were classified as responders. Responders had wider QRS, and lower LVEF and right ventricular fractional area change at baseline. After CRT, responders had a greater reduction of QRS duration, and improvement in LVEF, global longitudinal strain, and echocardiographic dyssynchrony parameters. Multivariate regression analysis showed that left bundle branch block (LBBB), left ventricular end-diastolic volume (LVEDV), tricuspid annular plane systolic excursion (TAPSE), and baseline difference of pre-ejection periods were predictors of a positive response to CRT in this population. Conclusions: LBBB, TAPSE, LVEDV, and pre-ejection time difference are independent variables that can predict adequate response to CRT.

Resumen Antecedentes: La terapia de resincronización cardíaca (TRC) se ha establecido como una terapia efectiva para la insuficiencia cardíaca con fracción de eyección reducida. Ensayos clínicos aleatorizados han demostrado su impacto en la mortalidad y hospitalizaciones por insuficiencia cardíaca, así como la mejora de los síntomas y la calidad de vida. Objetivos: Determinar las variables clínicas, electrocardiográficas y ecocardiográficas que puedan predecir la respuesta a la terapia de resincronización cardíaca (TRC). Método: Estudio unicéntrico, observacional, analítico, retrospectivo, que incluyó 102 pacientes con diagnóstico de IC sometidos a TRC y terapia dirigida por guías, de enero de 2010 a abril de 2020, en un centro de tercer nivel. La respuesta a TRC fue definida como mejoría de la clase funcional de la New York Heart Association en al menos 1 categoría, asociado con una recuperación ≥ 5% en la fracción de expulsión del ventrículo izquierdo (FEVI). Resultados: Incluimos a 102 pacientes, 61 (59.8%) fueron hombres. El promedio de edad al diagnóstico de IC fue 54 ± 18.7 años. La cardiopatía isquémica fue la etiología en 37 (36.3%) pacientes. 51 (50%) pacientes, fueron clasificados como respondedores. Los respondedores presentaron QRS amplio, menor FEVI y menor fracción de acortamiento del ventrículo derecho al inicio del estudio. Después de la TRC, los respondedores tuvieron una mayor reducción en la duración del QRS, mejoría en la FEVI, strain longitudinal global y parámetros de disincronía ecocardiográfica. El análisis de regresión multivariado mostró que el bloqueo de rama izquierdo (BRI), el volumen telediastólico del ventrículo izquierdo (VTDVI) la excursión sistólica del plano anular tricuspídeo (TAPSE) y la diferencia basal del período expulsivo fueron predictores de respuesta positiva a TRC. Conclusiones: BRI, TAPSE, VTDVI y la diferencia basal de períodos preexpulsivos son variables independientes que predicen respuesta adecuada a TRC.

Abstract Aneurysms are abnormal vessel dilatations that usually occur in the arterial system. The presence of these in the veins is very uncommon, finding only a few reports in the literature. They can be classified into two types: primary and secondary. Clinical presentation in most cases is with thrombosis in situ. A case of 23-year-old man with an 8-month history of a growing mass in the left groin with a history of bilateral hernioplasty, mastectomy due to gynecomastia, and several esthetic procedures, has a history of repetitive trauma from bodybuilding. The patient was attended for pain, finding an image suggesting the presence of a fusiform aneurysm in the proximal third of the left common femoral vein measured 3 × 3 cm in diameter. The patient was taken to the operating room to perform the aneurysmectomy.

Resumen Los aneurismas son dilataciones anormales de los vasos que se presentan normalmente en el sistema arterial. Su presencia en las venas es poco común, reportándose muy poco en la literatura. Estos se pueden clasificar en primarios y secundarios. El cuadro clínico inicial en la mayoría de los casos es trombosis in situ y en ocasiones a distancia. Se presenta el caso de un paciente de 23 años con aumento de volumen en región inguinal izquierda de ocho meses de evolución, antecedente de hernioplastia bilateral, mastectomía por ginecomastia, así como de múltiples procedimientos estéticos, y cuenta con historial de trauma repetitivo por fisiculturismo. El paciente fue valorado por dolor, encontrando una imagen sugestiva de un aneurisma fusiforme en el tercio proximal de la vena femoral común izquierda y medidas de 3 × 3 cm de diámetro. El paciente fue sometido a aneurismectomía.

Abstract This work aimed to show which treatments showed efficacy against coronavirus disease 2019 (COVID-19); therefore, the results of 37 clinical trials started in 2020 and completed in 2021 are reviewed and discussed here. These were selected from databases, excluding vaccines, computational studies, in silico, in vitro, and those with hyperimmune sera from recovered patients. We found 34 drugs, one vitamin, and one herbal remedy with pharmacological activity against symptomatic COVID-19. They reduced mortality, disease progression, or recovery time. For each treatment, the identifier and type of trial, the severity of the disease, the sponsor, the country where the trial was conducted, and the trial results are presented. The drugs were classified according to their mechanism of action. Several drugs that reduced mortality also reduced inflammation in the most severe cases. These include some that are not considered anti-inflammatory, such as Aviptadil, pyridostigmine bromide, anakinra, imatinib, baricitinib, and bevacizumab, as well as the combination of ivermectin, aspirin, dexamethasone, and enoxaparin. Nigella sativa seeds with honey have also been reported to have therapeutic activity. On the other hand, tofacitinib, novaferon with ritonavir, and lopinavir were also effective, as well as in combination with antiviral therapies such as danoprevir with ritonavir. The natural products colchicine and Vitamin D3 were only effective in patients with mild-to-moderate COVID-19, as was hydroxychloroquine. Drug repositioning has been the main tool in the search for effective therapies by expanding the pharmacological options available to patients.

Resumen El objetivo del presente trabajo fue conocer qué tratamientos mostraron efectividad contra COVID-19, para lo cual se revisan y discuten los resultados de 37 estudios clínicos iniciados durante 2020 y concluidos en 2021. Estos fueron seleccionados de bases de datos, excluyendo vacunas, estudios computacionales, in silico, in vitro y con sueros hiperinmunes de pacientes recuperados. Se documentaron 34 fármacos, una vitamina y un remedio herbolario, con actividad farmacológica ante COVID-19 sintomático. Estos redujeron la mortalidad, el progreso de la enfermedad, o el tiempo de recuperación. Para cada tratamiento se presenta identificador y tipo de estudio, la gravedad de la enfermedad, patrocinador, país donde se realizó, así como sus resultados. Los fármacos se clasificaron de acuerdo con su mecanismo de acción. Varios fármacos que redujeron la mortalidad también disminuyeron la inflamación en los casos más graves. Esto incluyendo algunos no considerados antiinflamatorios, como el aviptadil, el bromuro de piridostigmina, el anakinra, el imatinib, el baricitinib y el bevacizumab, así como la combinación de ivermectina, aspirina, dexametasona y enoxaparina. También se reportaron con actividad terapéutica las semillas de Nigella sativa con miel. Además, resultaron efectivos el tofacitinib, el novaferón con ritonavir y lopinavir, así como los antivirales en terapias combinadas como el danoprevir con ritonavir. Los productos naturales colchicina y vitamina D3, solo tuvieron actividad en los pacientes en estado leve a moderado de la COVID-19, así como la hidroxicloroquina. El reposicionamiento de fármacos fue la principal herramienta para buscar terapias efectivas ampliando las opciones farmacológicas accesibles a los pacientes.

Resumen: La explotación de los recursos costero-marinos en nuestras costas ha ocurrido desde hace unos 12 000 años; esta actividad ha sido intensa y con menosprecio por el estado del recurso que se explota; como consecuencia, muchos de los recursos han colapsado a través de los siglos: las perlas, el carey, la madreperla, las tortugas, las chuchecas y muchas otras especies han dejado de ser comercialmente viables. Actualmente, los recursos pesqueros, que antes se creían inagotables, también muestran señales inequívocas de sobreexplotación. Lejos de haber mejorado nuestra relación con los recursos marinos ha empeorado; el aumento de la explotación, combinada con la contaminación y el calentamiento de nuestros mares, los ha deteriorado. Los imprescindibles análisis de biomasa y estado de las poblaciones explotadas siguieron sin realizarse, aún después de la fundación del INCOPESCA en 1994. Los desembarcos totales, entre 2000 y 2015, disminuyeron en un 45 % y la flota artesanal ha sido la más golpeada. Las embarcaciones pesqueras han pasado de 5 000 en el 2010 a menos de 2 000 cuatro años después y el número de empleos en el sector ha disminuido en un 50 %. Es urgente que tengamos una institucionalidad capaz de generar información que permita determinar el nivel y la forma de explotación adecuados. Este manejo debe ser independiente de intereses económicos y políticos y debe venir acompañado de una administración eficiente y efectiva, así como de un control y vigilancia estrictos de la actividad.

Abstract: The exploitation of coastal-marine resources has occurred in our coasts since about 12 000 years ago. This activity has been intense and careless regarding to the well-being of the resource being exploited. Consequently, many of the resources have collapsed throughout the centuries. Pearls, tortoiseshell, mother-of-pearl, turtles, mangrove-cockle, and many other species have stopped being commercially viable. Presently, fisheries, formerly believed inexhaustible, also show unequivocal signs of overexploitation. Far from improving, our relationship with marine resources has exacerbated. The exploitation increment, combined with contamination and ocean warming, have deteriorated these resources. Essential biomass and population status analyses continue to be neglected, even after the founding of INCOPESCA in 1994. Total ship landings, between 2000-2015 have decreased 45 % impacting particularly the artisanal fleet. The number of fishing vessels have gone from 5 000 in 2010 to less than 2 000 four years later and the number of jobs in the sector has decreased by 50 %. It is urgent to have an institution able to generate information that allow us to determine the level and manner of this exploitation. Such management needs to be independent of economic and political interests and should be coupled with an efficient and effective administration, together with strict control and surveillance of this activity.

Abstract Introduction: In Mexico, cardiac rehabilitation (CR) as an interdisciplinary intervention with therapeutic impact in patients with heart disease is growing. There is the need to know actual conditions of CR in our country. Objectives: The objective of this National Registry is to follow-up those existing and new CR units in Mexico through the comparison between the two previous registries, RENAPREC-2009 and RENAPREC II-2015 studies. This is a descriptive study focused on diverse CR activities such as assistance training, and certification of health professionals, barriers, reference, population attended, interdisciplinarity, permanence over time, growth prospects, regulations, post-pandemic condition, integrative characteristics, and scientific research. Results: Data were collected from 45 CR centers in the 32 states, 75.5% are private practice units, 67% are new, 33% were part of RENAPREC II-2015, and 17 have continued since 2009. With a better distribution of CR units along the territory, the median reference of candidates for CR programs is 9% with a significant reduction into tiempo of enrollment to Phase II admission (19 ± 11 days). Regarding to previous registries, the coverance of Phases I, II, and III is 71%, 100%, and 93%, respectively; and a coverance increases in evaluation, risk stratification, and prescription, more comprehensive attendance and prevention strategies. Conclusions: CR in Mexico has grown in the past 7 years. Even there is still low reference and heterogeneity in specific processes, there are strengths such as interdisciplinarity, scientific professionalization of specialists, national diversification, and an official society that are consolidated over time.

Abstract In the Bajío, bean halo blight [Pseudomonas syringae pv. phaseolicola (Psph)] causes severe damage to varieties of the Azufrado and Cacahuate Bola types. To identify resistant genotypes, various bean materials were inoculated with two isolates, races 2 and 6. Likewise, readings of the reaction to natural infection by Psph in the field were recorded in two sets of Azufrado-type lines evaluated under rainfed conditions in Celaya, Guanajuato, in 2018, one with 10 and another with 16 lines established in June and July, respectively. In the marker laboratory, primers B10, W13, and T8, associated with genes of resistance against the bacterium in various bean genotypes, were used to determine the association between the presence of the markers and resistance to Psph. The Azufrado-type genotypes established in June were severely damaged (reaction from 5 to 8 on a scale of 1 to 9) by Psph, while those of July were damaged to a lesser degree. In the first trial, environmental conditions favored the attack of the bacterium, conditions that did not occur in the second trial. Most of the genotypes inoculated with race 6 in the greenhouse were susceptible. Genotypes positive for the presence of the three markers used were susceptible to races 2 and 6; that is, the marked resistance factors do not provide resistance to those races. All evaluated genotypes of the Azufrado-type of Nueva Granada race were susceptible to Psph races 2 and 6. Among the inoculation-resistant genotypes are Flor de Mayo M38, San Rafael, Pinto Laguna 80, and Pinto Saltillo.

Resumen En el Bajío el tizón de halo del frijol [(Pseudomonas syringae pv. phaseolicola (Psph)] causa severo daño a variedades de los tipos Azufrado y Cacahuate Bola. Para identificar genotipos resistentes se inocularon diversos materiales de frijol con dos aislados, razas 2 y 6. Asimismo, se registraron lecturas de la reacción a la infección natural por Psph en campo en dos juegos de líneas de tipo Azufrado evaluadas bajo temporal en Celaya, Guanajuato en 2018, uno con 10 y otro con 16 líneas establecidas en junio y julio, respectivamente. En el laboratorio de marcadores se usaron los iniciadores B10, W13 y T8 asociados a genes de resistencia contra la bacteria en diversos genotipos de frijol para determinar la asociación entre la presencia de los marcadores y la resistencia a Psph. Los genotipos de tipo azufrado establecidos en junio fueron severamente dañados (reacción de 5 a 8 en escala de 1 a 9) por Psph, mientras que los de julio lo fueron en menor grado. En el primer ensayo las condiciones ambientales favorecieron el ataque de la bacteria, condiciones que no se registraron en el segundo ensayo. La mayoría de los genotipos inoculados con la raza 6 en invernadero, resultaron susceptibles. Genotipos positivos para la presencia de los tres marcadores utilizados fueron susceptibles a las razas 2 y 6; es decir, los factores de resistencia marcados no proporcionan resistencia a esas razas. Todos los genotipos evaluados del tipo azufrado de raza Nueva Granada, fueron susceptibles a Psph razas 2 y 6. Entre los genotipos resistentes a la inoculación se encuentran, Flor de Mayo M38, San Rafael, Pinto Laguna 80 y Pinto Saltillo.

Resumen Antecedentes: Mesoamérica es uno de los hotspots de biodiversidad más importantes del planeta. A pesar de importantes esfuerzos realizados durante más de dos siglos para contribuir al conocimiento florístico de esta región, el conocimiento de su flora todavía es disperso y desigual. Preguntas: ¿Cuál es la magnitud de la riqueza de plantas vasculares de la cuenca del río Usumacinta? Sitio y años de estudio: Cuenca del río Usumacinta (Guatemala y México), 1838-2018. Metodos: Se integró y sistematizó información florística a partir de diferentes fuentes de información derivadas de estudios florísticos y ecológicos. Resultados: Se registraron 6,977 especies, 1,892 géneros y 274 familias. El mayor número de especies (5,746) y registros (58,859) fueron de la porción mexicana, en comparación con su contraparte guatemalteca (4,445 especies y 19,952 registros). Las familias con mayor riqueza de especies fueron Orchidaceae (598), Fabaceae (512), y Asteraceae (476). La prevalencia de estas y todas las demás familias con contribuciones significativas a la flora varió entre los tres sectores altitudinales en los que se subdividió la cuenca (cuenca baja, cuenca media y cuenca alta). Conclusiones: La cuenca del río Usumacinta es una región estratégica para la conservación de la biodiversidad vegetal, con casi una tercera parte de las especies de plantas vasculares conocidas para Mesoamérica y ca. de 6 % de la flora del continente americano. Los esfuerzos de exploración botánica deben centrarse en áreas de la cuenca en donde la información disponible es escasa o nula a fin de obtener una mejor apreciación de su flora.

Abstract Background: Mesoamerica is one of the most important biodiversity hotspots on the planet. Despite significant efforts made over two centuries to contribute to the floristic knowledge of this region, our understanding of its flora is still scattered and uneven. Questions: What is the magnitude of the vascular plant species richness in the Usumacinta River Basin? Study site and dates: Usumacinta River Basin (Guatemala and Mexico), 1838-2018. Methods: We compiled the checklist by systematizing the floristic information acquired from various sources derived from numerous floristic and ecological studies. Results: We recorded 6,977 species, 1,892 genera and 274 families. The largest numbers of species (5,746) and records (58,859) correspond to the Mexican portion of the Usumacinta River Basin, compared to its Guatemalan counterpart (4,445 species and 19,952 records). The most species-rich families were Orchidaceae (598 species), Fabaceae (512), and Asteraceae (476). The prevalence of these and all other families with significant contributions to the flora varied among three elevation-defined sectors into which the Usumacinta River Basin was subdivided (lower, middle, and upper basin). Conclusions: The Usumacinta River Basin is a strategic region for plant biodiversity conservation as it hosts almost one third of all vascular plant species known for Mesoamerica and ca. 6 % of the entire flora in the Americas. Further botanical exploration should focus on those areas of the basin for which little or no information is available in order to gain a better appreciation of its flora.

Resumen El comportamiento de los componentes de las plantas proteicas es de vital importancia para los animales que los consumen en su dieta. La presente investigación tiene como objetivo evaluar algoritmos de regresión para determinar la conducta de las expresiones que mejor se adaptan a los procedimientos de un laboratorio tradicional y estimar los componentes químicos de plantas proteicas, en este sentido, se ha utilizado la biblioteca MULAN de java, que contiene algoritmos de aprendizaje automático capaces de adaptarse a disímiles problemas. Para ello, se crearon tres conjuntos de datos para cada especie estudiada en este trabajo; cada uno de estos incluye los elementos principales a ser evaluados en cada experimento, que están delimitados por: Metabolitos secundarios, componentes de la pared celular y digestibilidad para los ficheros de entrenamiento uno, dos y tres, respectivamente. Posteriormente, fueron evaluados por medio del aprendizaje supervisado y una validación cruzada de cada uno para determinar el mejor ajuste por aRMSE (Error cuadrático medio de la raíz). Los resultados del aprendizaje fueron comparados con experimentos anteriores, donde se tenía una variante de aprendizaje que contenía en un solo dataset todos los componentes a evaluar en una sola predicción. El resultado de la comparación muestra que los algoritmos vagos basados en instancias tienen un mejor comportamiento en el aprendizaje que los otros evaluados.

Abstract The behavior of components of protein plant is of vital importance for animals that consume them in their diet. The objective of this research is to evaluate regression algorithms, to determine the behavior of the expressions that best adapt to the procedures of a traditional laboratory and to estimate the chemical components of protein plants, in this sense the MULAN library of java has been used, that contain automatic learning algorithms capable of adapting to dissimilar problems. Three data set were created for each species treated in this study; each of these include the main elements to be evaluate in each experiment, these are delimitings by: secondary metabolites, cell wall components and digestibility element for training files one, two and three, respectively; subsequently, they were evaluated through learning supervised and cross-validation of each to determine the best fit by aRMSE (Average Root Mean Square Error). The learning results were compare with previous experiments, where there was a learning variant that contained in a single dataset all the components to be evaluates in a single prediction. The result of the comparison shows that the lazy algorithms based on instances have a better learning behavior than the others evaluate.

ABSTRACT Introduction: Lung injury and STAT1 deficit can induce EGFR overexpression in SARS-CoV2 infected cells. Methods: Considering the role of EGFR in inflammation and fibrosis, a Phase I/II trial was done to evaluate the safety and preliminary effect of nimotuzumab, a humanized anti-EGFR antibody, in severe and high-risk moderate patients with COVID-19. Patients were able to receive other drugs included in the national protocol. Treatment consisted on nimotuzumab intravenous infusions, administered every 72 hours. Before launching the study, EGFR expression was confirmed by immunohistochemistry in 20 lung samples from COVID-19 deceased subjects. Results: They were included forty-one patients (31 severe/10 moderate) in the trial. The median age was 62 years and the main comorbidities were hypertension, diabetes, cardiovascular disease and obesity. Seven patients received one dose of nimotuzumab, 29 received 2 infusions while 5 subjects required 3 doses. The antibody was very safe. There were only 4 related adverse events in 2 subjects. Eight patients required invasive mechanical ventilation. The 14 day recovery rate was 82,9%. 90% in moderate patients and 80,64 % in severe patients. Inflammatory markers decreased overtime and interleukin-6 concentration diminished from 46,5 pg/ml to 14,51 pg/ml at day 7. None of the evaluated patients showed signs of fibrosis in the follow-up evaluation. After the use of nimotuzumab in 1,536 patients in the real-world scenario, a high recovery rate was achieved in severe and moderate patients (89,4% and 79.4%, respectively), which compares very favorably with the phase I/II trial data. Conclusions: our results suggest that nimotuzumab is a safe antibody that can reduce inflammation and avoid fibrosis in severe and moderate COVID-19 patients at high risk of aggravation.

RESUMEN Introducción: La lesión pulmonar y el déficit del transductor de señal y el activador de la transcripción 1 (STAT1) inducen la sobreexpresión de receptor del factor de crecimiento epidérmico (EGFR) en células infectadas con SARS-CoV-2. Métodos: Teniendo en cuenta el papel del EGFR en la inflamación y la fibrosis, se realizó un ensayo de fase I/II para evaluar la seguridad y el efecto del nimotuzumab, un anticuerpo anti-EGFR humanizado, en pacientes con COVID-19 graves y moderados. Los pacientes recibieron otros medicamentos incluidos en el protocolo nacional. El tratamiento consistió en infusiones intravenosas de nimotuzumab, administradas cada 72 h. Antes de iniciar el estudio, la expresión de EGFR se confirmó mediante inmunohistoquímica en 20 muestras de pulmón de sujetos fallecidos por COVID-19. Resultados: En el ensayo se incluyeron 41 pacientes (31 graves/10 moderados). La media de edad fue de 62 años y las principales comorbilidades fueron hipertensión, diabetes, enfermedad cardiovascular y obesidad. Recibieron una dosis de nimotuzumab 7 pacientes, 29 recibieron 2 infusiones mientras que 5 sujetos requirieron 3 dosis. El anticuerpo fue seguro. Solo se detectaron 4 eventos adversos en 2 sujetos. Requirieron ventilación mecánica invasiva 8 pacientes. La tasa de recuperación a los 14 días fue del 82,9 %: pacientes moderados 90 % y 80,64 % en pacientes graves. Los marcadores inflamatorios disminuyeron en el tiempo y la concentración de interleucina-6 disminuyó de 46,5 pg/ml a 14,51 pg/ml el día 7. Ninguno de los pacientes evaluados mostró signos de fibrosis en el seguimiento. Tras el uso del nimotuzumab en 1536 pacientes en las condiciones del mundo real, se logró una alta tasa de recuperación en los pacientes graves y moderados (89,4 y 79,4 %, respectivamente), lo cual se compara muy favorablemente con los datos del ensayo fase I/II. Conclusiones: estos resultados sugieren que el nimotuzumab es un anticuerpo seguro que puede reducir la inflamación y evitar la fibrosis en pacientes con COVID-19 graves y moderados.

Resumen Objetivo: Presentar los resultados de nuestra serie de nefrectomía laparoscópica en pielonefritis xantogranulomatosa (PXG). Método: Se realizó un estudio retrospectivo que incluyó 143 pacientes tratados con nefrectomía laparoscópica por exclusión renal, de los cuales 15 fueron por PXG, en el periodo comprendido de 2011 a 2019. Se recolectaron datos demográficos y clínicos, resultados transoperatorios, complicaciones y días de estancia hospitalaria. Resultados: Se realizó nefrectomía laparoscópica transperitoneal de forma exitosa en 15 pacientes con PXG, sin necesidad de conversión. El tiempo transoperatorio promedio fue de 124.4 minutos (rango: 70-240). El sangrado transoperatorio fue de 148.5 ml (rango: 30-550), sin requerimiento de transfusión sanguínea. No se reportaron complicaciones transoperatorias; se presentó una complicación en el posoperatorio (6.6%) clasificada como Clavien-Dindo I (infección de la herida quirúrgica). La estancia hospitalaria promedio fue de 2.85 días (rango: 2-7). Conclusiones: El manejo definitivo de la PXG es la nefrectomía, y el abordaje laparoscópico debe ser considerado como una modalidad de tratamiento a pesar de ser una patología que presenta un proceso inflamatorio grave y crónico, obteniéndose beneficios como disminución en el tiempo quirúrgico, menor sangrado, menor tasa de complicaciones y menos días de estancia hospitalaria cuando es realizado por un cirujano experimentado.

Abstract Objective: To present the results of our case series on laparoscopic nephrectomy in xanthogranulomatous pyelonephritis (XGP). Methods: A retrospective study was conducted that included 143 patients treated with laparoscopic nephrectomy for non-functioning kidney, of whom 15 had XGP, within the time frame of 2011 to 2019. The demographic and clinical data were collected, along with the intraoperative results, complications, and days of hospital stay. Results: Transperitoneal laparoscopic nephrectomy was successfully performed on 15 patients with XGP, with no need for conversion. Mean intraoperative time was 124.4 minutes (range 70-240) and intraoperative blood loss was 148.5 ml (range 30-550), with no blood transfusion required. No intraoperative complications occurred but there was one postoperative complication (6.6%), classified as Clavien-Dindo I (surgical wound infection). Mean hospital stay was 2.85 days (range 2-7). Conclusions: Nephrectomy is the definitive management for XGP, and the laparoscopic approach should be considered a treatment modality, despite the fact that the pathology involves a severe chronic inflammatory process. Its benefits are reduced surgery duration, less blood loss, a lower complication rate, and fewer days of hospital stay, when performed by a skilled and experienced surgeon.

ABSTRACT Background Reduced or null expression of E-cadherin protein is a frequent cause of diffuse gastric cancer (DGC). More than 50% of patients with DGC present somatic variants in CDH1 gene. Objectives The objectives of this study were to study E-cadherin expression and identify variants in the CDH1 gene in gastric tumors of patients with DGC. Methods We studied 18 Mexican DGC patients who attended a hospital of the Mexican Social Security Institute; E-cadherin expression was determined by immunohistochemistry, and variants were identified by Sanger sequencing in promoter and coding regions. Predictive analysis was performed using PolyPhen-2 and HOPE software. Results We found that 56% of DGC patients showed reduced expression of E-cadherin. All patients carried CDH1 variants; overall, 12 different CDH1 variants were identified. Predictive analysis revealed that the rs114265540 variant was probably damaging, with a value of 0.985, indicating a functional impact on the E-cadherin protein. Variants rs34939176 and rs33964119 were identified as risk factors for DGC (odds' ratios [OR] = 31.3, 95% CI 6.3-154.0, p < 0.001; OR = 6.1, 95% CI 2.0-19.0, p < 0.001, respectively) given their elevated frequency and by comparing it with those reported for MXL population in the 1000 Genomes Project database. Conclusions In this Mexican population, the percentage of diffuse gastric tumors with reduced expression of E-cadherin was similar to that reported in other populations. All gastric tumors of DGC patients studied had somatic CDH1 gene variants; however, the rs114265540, rs34939176, and rs33964119 variants were importantly related to DGC.

ABSTRACT Artemisia ludoviciana contains several medically important metabolites, but few studies have been reported on its effects on insects of agricultural importance. This article aimed to know the biological effects of a methanolic extract of A. ludoviciana on neonatal larvae of Spodoptera frugiperda, where column chromatography was obtained to fractionate the crude oil and mass gas chromatography to identify metabolites, the extract, and the 90:10 mobile phase showed low mortality of about 30% at a concentration of 1 mg/mL, but a weight loss in neonatal larvae of more than 50% was lost, confirming a significant antifeedant activity. GC-MS analysis revealed the presence of terpenoids such as limonene, thujone, camphor, borneol, and borneol acetate, which are associated with an insecticidal and antifeedant activity. Therefore, the methanolic extracts of A. ludoviciana can be a friendly alternative for insect pest control.

RESUMEN Artemisia ludoviciana contiene varios metabolitos de importancia médica, sin embargo, se han informado pocos estudios sobre sus efectos en insectos de importancia agrícola. El objetivo de este trabajo fue conocer los efectos biológicos de un extracto metanólico de A. ludoviciana sobre larvas neonatas de Spodoptera frugiperda en donde se utilizó cromatografía de columna para fraccionar el crudo y cromatografía de gases masas para identificación de metabolitos, dando como resultados que el extracto y la fase móvil 90:10 mostraron una baja mortalidad de alrededor del 30 % a una concentración de 1 mg/mL, sin embargo, se observó una pérdida de peso en las larvas neonatas de más de 50 %, corroborando una actividad antialimentaria significativa, y el análisis GC-MS reveló la presencia de terpenoides como: limoneno, tujona, alcanfor, borneol y acetato de borneol, asociados con el efecto insecticida y la actividad antialimentaria. Por lo tanto, los extractos metanólicos de A. ludoviciana, pueden ser una alternativa amigable para el control de insectos plagas.

Resumen Objetivo: El índice de eficiencia miocárdica (IEM) correlaciona el consumo miocárdico de oxígeno (MVO2) con el consumo máximo de oxígeno, el cual proporciona información sobre la eficiencia cardiovascular (EfCV). En deportistas, el IEM mejora posterior a un microciclo de entrenamiento, en el paciente con enfermedad cardiovascular sometido a un programa de rehabilitación cardiaca y prevención secundaria PRHCyPS, el comportamiento del IEM podría resultar un estimador relacionado con mejoría derivado de un periodo de entrenamiento. El objetivo del estudio fue determinar el comportamiento del IEM posterior a un PRCyPS en pacientes con cardiopatías y riesgo cardiovascular alto (RCVA). Métodos: Estudio de cohorte ambilectivo, descriptivo, analítico, no aleatorizado. Se seleccionaron pacientes con cardiopatías de etiología mixta con RCVA ingresados a un PRCyPS durante 4-6 semanas. A todos los pacientes se les realizó una prueba de ejercicio máximo en banda antes y después del PRCyPS. Se determinaron umbrales de consumo de oxígeno (VO2) pico, equivalentes metabólicos-carga, doble producto e IEM. Se estableció un punto de corte del IEM mediante una curva ROC con un valor de 7.37 con un área bajo la curva de 0.68 (IC 95%: 0.61-0.76; p < 0.001), sensibilidad 0.60 y 1-especificidad de 0.35. Resultados: Se incluyeron 193 pacientes con una media de edad de 62.3 años, en su mayoría del sexo masculino (66.2%). Se observaron porcentajes de cambio en el IEM –27.1% (p < 0.001), MET 43.1% (p < 0.001), doble producto 5.7% (p < 0.01) y MVO2: 8.3% (p < 0.01) al término del PRCyPS. Conclusiones: Se observó un cambio significativo en el IEM posterior a un PRCyPS, lo cual se asoció a una mejoría en la EfCV, sugiriendo que este pueda considerarse como un parámetro clínico que evaluar en los programas de rehabilitación cardiaca.

Abstract Objective: The myocardial efficiency index (MEI) correlates the Myocardial Oxygen Consumption (MVO2) with the Maximum Oxygen Consumption (VO2max), this index provides information about the cardiovascular efficiency (CVEf). In athletes, the MEI improves after a micro-cycle training, however in patients with cardiovascular disease undergoing Cardiac Rehabilitation Program (CRP), IEM behavior could be a good estimator related to the improvement training period. The objective of this study was to determine the myocardial efficiency index behavior in patients with heart disease and high cardiovascular risk (HCVR) after a CRP. Methods: Ambilective, descriptive, analytical, non-randomized cohort study was conducted. Patients with heart disease of mixed etiology and HCVR admitted to a CRP for 4-6 weeks were selected. All patients performed a maximal exercise test in band before and after the CPR. Thresholds of VO2 peak, METs-load, Double product (DP) and MEI were determined. A cut-off point for the MEI was established using a ROC curve with a value of 7.37, area under the curve: 0.68 (95% CI 0.61 - 0.76, p < 0.001), sensitivity 0.60 and 1-specificity 0.35. Results: 193 patients with a mean age of 62.3 years were included, predominantly men (66.2%). Percentages changes in the MEI-27.1% (p < 0.001),METs-43.1% (p < 0.001),DP 5.7% (p < 0.01), and MVO2: 8.3% (p < 0.01) were observed at the end of CRP. Conclusions: Significant change in the MEI were observed after CRP associated to CVEf improvement, suggesting that this parameter could be considered as a good clinical tool in the CRP care programs.

Abstract Objective: The objective is to describe the role of leadership in the training of the general surgeon and to point out its relevance. Method: A search of the medical literature was carried out to identify studies that reported on topics related to leadership in the context of medical education and surgery. We used the search engine of Medline/PubMed databases including related terms such as “leadership” AND “medicine” OR “surgery”. Results: Leadership is a valued characteristic in surgery, it implies professionalism, technical competence, motivation, innovation, teamwork, communication skills, decision making, emotional competence, resilience, and effective teaching. Leadership is measurable and can be developed through experience, observation, and education. Conclusions: Priority must be given to actions that improve leader traits. This has been shown to create a work environment where efficient work is performed, and failures are minimized. The current vision of the leader must be expanded to reduce the gender gap between the positions as leader occupy in this moment.

Resumen Objetivo: Describir el papel de liderazgo en la formación del cirujano general y puntualizar su relevancia en nuestro entorno. Método: Se realizó una búsqueda en la literatura médica para identificar estudios que informaran sobre temas relacionados al liderazgo en el contexto de la enseñanza médica y en cirugía. Se utilizó el motor de búsqueda de las bases de datos Medline/PubMed incluyendo términos relacionados como “leadership” AND “medicine” OR “surgery”. Resultados: El liderazgo es una característica valorada en cirugía, que implica profesionalismo, competencia técnica, motivación, innovación, trabajo en equipo, habilidades comunicativas, toma de decisiones, competencia emocional, resiliencia y enseñanza eficaz. Es medible y susceptible de desarrollarse a través de la experiencia, la observación y la educación. Conclusiones: En la formación del cirujano general se deben priorizar las acciones que mejoren los rasgos de líder. Se ha demostrado que esto genera un ambiente de trabajo donde se realiza una labor eficiente y se minimizan las fallas. Se debe expandir la visión actual que se tiene de líder y permitir que se disminuya la brecha de género entre los puestos que hoy se ocupan.
