Objective: Atypical antipsychotics (AAPs) promote obesity and insulin resistance. In this regard, the main objective of this study was to present potential mechanisms and evidence concerning side effects of atypical antipsychotics in humans and rodents. Method: A systematic review of the literature was performed using the MEDLINE database. We checked the references of selected articles, review articles, and books on the subject. Results: This review provides consistent results concerning the side effects of olanzapine (OL) and clozapine (CLZ), whereas we found conflicting results related to other AAPs. Most studies involving humans describe the effects on body weight, adiposity, lipid profile, and blood glucose levels. However, it seems difficult to identify an animal model replicating the wide range of changes observed in humans. Animal lineage, route of administration, dose, and duration of treatment should be carefully chosen for the replication of the findings in humans. Conclusions: Patients undergoing treatment with AAPs are at higher risk of developing adverse metabolic changes. This increased risk must be taken into account when making decisions about treatment. The influence of AAPs on multiple systems is certainly the cause of such effects. Specifically, muscarinic and histaminergic pathways seem to play important roles.

Lúpus eritematoso neonatal é uma doença auto-imune produzida pela passagem de anticorpos maternos antinucleares e anticorpos contra antígenos extraíveis nucleares através da placenta. No momento do diagnóstico, as mães são assintomáticas em 40 a 60% dos casos. As manifestações mais comuns são lesões cutâneas e bloqueio cardíaco congênito. Os achados cutâneos são variáveis e geralmente começam nas primeiras semanas ou meses de vida. Lúpus eritematoso congênito é uma variante do lúpus eritematoso neonatal. Apresentaremos um caso de lúpus eritematoso congênito e um caso de lúpus eritematoso neonatal, mostrando a variabilidade da doença.

Neonatal lupus erythematosus is an autoimmune disease produced by the passage of maternal antinuclear antibodies and extractable nuclear antigen antibodies through the placenta. At the moment of the diagnosis, the mothers are asymptomatic in 40 to 60% of cases. The most common manifestations are cutaneous lesions and congenital heart block. The cutaneous findings are variable and usually begin within the first weeks or months of life. Congenital lupus erythematosus is a congenital variant of neonatal lupus erythematosus. We present one case of congenital lupus erythematosus and one case of neonatal lupus erythematous, showing the variability of this disease.

O Streptococcus agalactie é um importante micro-organismo causador de doenças em gestantes, neonatos, idosos (maiores de 65 anos de idade), e portadores de doenças crônicas debilitantes, sendo um patógeno incomum em pacientes que não se enquadrem nestas faixas etárias ou perfil clínico (1-5), e, raramente, é descrito como agente causador de doenças sexualmente transmissíveis. Descrevemos o caso de um adulto jovem hígido de 19 anos, apresentando lesões ulceradas genitais e oral, assim como corrimento uretral e ocular, sugestivas de terem sido causadas pelo Streptococcus agalactie, e adquiridas através do contato sexual (doenças sexualmente transmissíveis).

Streptococcus agalactiae is an important microorganism involved in a number of conditions in pregnant women, newborns, elderly people (over 65 years of age) and individuals with chronic disabling illnesses. This pathogen is infrequently found among patients outside this age range or clinical profile(1-5) and is rarely reported in the etiology of sexually transmitted diseases. Here we describe a case of an otherwise healthy 19 year-old male, who presented with ulcerative genital and oral lesions in association with urethral and ocular discharge, suggestive of Streptococcus agalactiae infection acquired through sexual contact.

CONTEXTO E OBJETIVO: As interconexões entre qualidade de vida e estado de saúde não têm sido investigadas aprofundadamente. O objetivo deste estudo é investigar possível correlação entre construtos de estado geral de saúde e qualidade de vida mediante versões em português de dois questionários recentemente adaptados e testados no Brasil. TIPO DE ESTUDO E LOCAL: Estudo transversal com emprego de dois questionários auto-administráveis. Esta pesquisa foi conduzida em serviços de saúde associados à Universidade de Caxias do Sul, Brasil. MÉTODOS: Este estudo apresenta dados de uma amostra de 120 voluntários que completaram as versões em português da Escala de Saúde Pessoal e do Índice Multicultural de Qualidade de Vida. Regressão linear bivariada e coeficientes de correlação de Pearson foram gerados a partir de escores de ambos questionários. RESULTADOS: Significativa correlação entre os conceitos de qualidade de vida e estado de saúde tais como avaliados pelas versões em português de ambos questionários foi observada. Quase todas as questões relacionadas à saúde da Escala de Saúde Pessoal apresentaram uma forte correlação com o conceito global de qualidade de vida. A magnitude desta significativa correlação foi responsável por quase metade da variação observada. CONCLUSÕES: Estes achados indicam que, nesta amostra, aspectos relacionados à saúde são essenciais para experiência global de bem-estar e qualidade de vida. Estas similaridades nos diferentes grupos indicam que a inter-relação entre estado de saúde e qualidade de vida é homogênea independentemente da presença e/ou tipo de doença.

CONTEXT AND OBJECTIVE: The interconnections between quality of life and health status as assessed via questionnaires have not been thoroughly investigated. The objective of this study was to investigate a possible correlation between the constructs of general health status and quality of life as assessed by the Portuguese versions of two questionnaires recently adapted and tested in Brazil. DESIGN AND SETTING: This was a cross-sectional study in which two self-administered questionnaires were used. This investigation was conducted at healthcare services associated with the Universidade de Caxias do Sul, Brazil. METHODS: This study presents data from a sample of 120 volunteers who completed the Portuguese versions of the Personal Health Scale and the Multicultural Quality of Life Index questionnaires. Bivariate linear regression analysis and Pearson correlation coefficients were generated from the scores of the two questionnaires. RESULTS: A significant correlation between the concepts of quality of life and health status as evaluated by the Portuguese versions of both questionnaires was observed. Almost all of the health-related questions displayed strong correlations with the overall concept of quality of life. The magnitude of this correlation accounted for almost half of the observed variance. CONCLUSIONS: These findings indicate that, within this sample, health-related issues were key factors for the overall experience of wellbeing and quality of life. The similarities observed across the different groups indicate that the interrelation between health status and quality of life was homogenous, regardless of presence and/or type of ailments.

As metástases ósseas que acometem o segmento torácico e lombar da coluna vertrebal têm como principais complicações o fenômeno doloroso e a compressão medular ou radicular. O tratamento adotado dependerá das condições clínicas e neurológicas do paciente e do grau de invasão tumoral. Foram analisados, prospectivamente, 32 pacientes acometidos por metástases do segmento torácico e lombar da coluna vertebral, sendo tratados com cirurgia de descompressão e estabilização interna seguida por radioterapia ou irradiação com mobilização externa. A seleção dos grupos foi estabelecida de acordo com a sensibilidade do tumor a radioterapia, condições clínicas, grau de estabilidade da coluna, grau de compressão medular/radicular e opção do paciente. A escala de Frankel e o teste visual de dor foram aplicados no momento do diagnóstico e após 1 e 6 meses. O grupo cirúrgico apresentou melhores resultados, mantendo a deambulação por mais tempo, pois manteve a força muscular nos pacientes Frankel E, propiciou a reversão do déficit neurológico pré-operatório em 61,5% dos casos e propiciou melhora significativa da dor.

Bone metastases at the thoracic and lumbar segment of the spine are usually presented with painful sensation and medullar compression. The treatment is based on the clinical and neurological conditions of the patient and the degree of tumor invasion. In the present study, 32 patients with spinal metastasis of thoracic and lumbar segment were prospectively analyzed. These patients were treated by decompression and internal stabilization followed by radiotherapy or irradiation with external immobilization. The election of the groups was in accordance with the tumor radiotherapy sensitivity, clinical conditions, spinal stability, medullar or nerve compression and patient's decision. The Frankel scale and pain visual test were applied at the moment of diagnosis and after 1 and 6 months. The surgical group had better results with preserving the ambulation longer and significant reduction of pain.

A Escala de Saúde Pessoal é um instrumento conciso para a avaliação do estado geral de saúde e bem-estar que considera os aspectos culturais e a visão pessoal do entrevistado. Ela é composta por 10 questões que avaliam diferentes dimensões de saúde obtidas a partir da literatura internacional, incluindo aspectos que variam de domínios somáticos e psicológicos até juízo critico e funcionamento social. OBJETIVO: Nesta investigação, os resultados de um estudo conduzido no Sul do Brasil para testar e validar a versão em português da Escala de Saúde Pessoal são apresentados. MÉTODO: Este estudo analisa dados de uma amostra de 120 voluntários brasileiros (90 pacientes e 30 profissionais da saúde). Todos pacientes completaram a versão em português da Escala de Saúde Pessoal sob orientação mínima de examinadores treinados que seguiram procedimentos padronizados de instrução. RESULTADOS: A consistência interna da PHS-Pt atingiu um índice á de Cronbach de 0.75 entre pacientes e de 0.69 entre profissionais da saúde. O coeficiente de correlação da validade teste-reteste indicou um escore de 0.82. Ademais, a PHS-Pt foi capaz de detectar uma significativa validade descriminante entre as duas amostra avaliada (P < .001). CONCLUSÕES: A versão original em língua inglesa da Escala de Saúde Pessoal foi adaptada com sucesso para o idioma português como demonstrado metodologicamente neste estudo. A PHS-Pt constitui um instrumento de pesquisa confinável e fidedigno para avaliar o estado de saúde no Brasil, já que é apropriadamente estruturada para distinguir diferentes grupos de voluntários de acordo com seus estados de saúde.

The Personal Health Scale is a concise instrument for comprehensive culture-informed and self-rated assessment of general health status and well-being. It is composed of 10 questions that appraise different health dimensions collated from the international literature, including aspects ranging from somatic and psychological domains to social functioning and insight. PURPOSE: In this investigation, results of a study conducted in Southern Brazil to test and validate the Portuguese version of the Personal Health Scale (PHS-Pt) are presented. Method: This study analyzes data from a sample of 120 Brazilian volunteers (90 patients and 30 health care professionals). All patients completed the Portuguese version of the Personal Health Scale under a minimal guidance by trained examiners, who followed standardized instructional procedures. RESULTS: The internal consistency of the PHS-Pt attained a Cronbach's a of 0.75 among patients and of 0.69 among health care professionals. The test-retest reliability correlation coefficient yielded a score of 0.82. Furthermore, the PHS-Pt was able to detect a significant discriminating validity between the 2 evaluated samples (P < .001). CONCLUSIONS: The original English version of the Personal Health Scale was successfully adapted to Portuguese as methodologically demonstrated herein. The PHS-Pt constitutes a reliable and trustworthy research instrument for evaluating health status in Brazil, since it is appropriately designed to distinguish different groups of volunteers regarding their health status.
