ABSTRACT Introduction: The minimal residual disease is the permanence of residual leukemic cells at subclinical levels after remission of the disease. This condition increases the risk of relapse and mortality. Objective: To characterize the clinical and molecular factors of patients with acute leukemias and minimal residual disease detected by flow cytometry in a highly complex institution in the city of Medellín, Colombia during the years 2015 - 2017. Methodology: This is a retrospective descriptive observational study, which included patients with leukemia diagnosed by flow cytometry. A non-probabilistic sampling of consecutive cases was carried out. The information collected was entered into a database in Excel, and the analysis was carried out through the IBM SPSS Version 24 program, using absolute and relative frequencies, average and standard deviation or median and interquartile ranges, according to the nature of each variable and its distribution. Results: 60 patients were included in which male sex predominated with 63.3% (38). The most frequent diagnosis was lymphoid leukemia with 78.3% (47). Of the total patients included, 36.6% (22) were positive for minimal residual disease; 28.3% received a bone marrow transplant and 10% (6) had a cerebrospinal fluid compromise. In the second cytometry of the patients with minimal residual disease, 90.9% (20) expressed CD45 +. 31.8% (7) of the patients with minimal residual disease relapsed. Conclusion: Minimal residual disease is a frequent pathology in patients with acute leukemias that requires follow-up and constitutes a relevant prognostic factor.

RESUMEN Introducción: La enfermedad mínima residual es la permanencia de células leucémicas residuales en niveles subclínicos luego de la remisión de la enfermedad. Esta condición incrementa el riesgo de recaída y mortalidad. Objetivo: Caracterizar factores clínicos y moleculares de pacientes con leucemias agudas y enfermedad mínima residual detectada por citometría de flujo en una institución de alta complejidad de la ciudad de Medellín, Colombia durante los años 2015 - 2017. Metodología: Este es un estudio descriptivo retrospectivo, que incluyó pacientes con leucemia diagnosticada por citometría de flujo. Se realizó un muestreo no probabilístico de casos consecutivos. La información recolectada fue digitada en una base de datos en Excel, y el análisis se realizó a través del programa IBM SPSS Versión 24, empleando según la naturaleza de cada variable frecuencias absolutas y relativas, promedio y desviación estándar o mediana y rangos intercuartílicos según su distribución. Resultados: Se incluyó un total de 60 pacientes con predominio del sexo masculino 63,3 % (38). El diagnóstico más frecuente fue la leucemia linfoide 78,3 % (47). Del total de pacientes incluidos, 36,6% (22) fue positivo para enfermedad mínima residual; 28,3 % recibió trasplante de médula ósea y el 10 % (6) presentó compromiso de líquido cefalorraquídeo. En la segunda citometría en pacientes con enfermedad mínima residual, 90,9 % (20) expresaba CD45+. El 31,8 % (7) de los pacientes con enfermedad mínima residual presentó recaída. Conclusión: La enfermedad mínima residual es una condición frecuente en pacientes con leucemias agudas que requiere seguimiento y constituye un factor pronóstico relevante.

Objectives: To design and to validate a scale for nursing students, that allows to identify the conceptions on nursing care competences and determine any possible discrepancies that may arise between their current experiences in the acquisition of competence and the standards or guidelines established by the own profession. Methods: Cross-sectional study with a sample of 349 first- and fourth-year students of Nursing. A first version of the scale according statistical criteria and expert judges was refined. The final 24-item scale provides nursing care competences. Results: It provides a scale with adequate psychometric properties. It includes two subscales with two domains independent from each other (actual self, ought self). Discussion: There is a line of work, which introduces a new way of analysis to determine the well-being and the academic and professional success of nursing students, based on their actual and normative conception of nursing care competences.

Objetivos: Diseñar y validar una escala para estudiantes de enfermería que identifica las concepciones en las competencias de cuidado y determina las discrepancias que puedan derivarse entre las experiencias actuales en la adquisición de competencias y los estándares o guías establecidos por la propia profesión. Métodos: Estudio transversal con una muestra de 349 estudiantes de primero y cuarto de Enfermería. Se elaboró una primera versión de la escala que fue depurada siguiendo criterios estadísticos y de jueces expertos. La escala definitiva contempla 24 ítems de competencias de cuidado. Resultados: Se aporta una escala con adecuadas propiedades psicométricas. Incluye dos subescalas con dos dominios independientes entre sí (Yo real, Yo debería). Discusión: Se plantea una línea de trabajo que introduce nuevos elementos para el análisis del bienestar y el éxito académico y profesional de los estudiantes de enfermería en función de la concepción real y normativa de las competencias de cuidado.

Background: The estimation by standardized tests on belief in personal capacity to practice regular physical activity (self-efficacy for physical activity) allows us to make predictions about the actual practice of this behavior. We do not have measuring instruments suitable for this purpose, therefore, we propose to construct and validate a scale to determine levels of self-efficacy of adult users of the Primary Health Care Services for regular physical activity. Methods: Phases: 1) Develop and review the scale by expert judges. 2) A pilot study. 3) Determination of reliability and validity of the test. The sample was 388 adult users (18-65 years) of health centres from the province of Seville selected by cluster sampling (health centres) and quotas (town size, age and sex). Concurrent validity was found by the International Physical Activity Questionnaire (IPAQ) and discriminant validity by Beck Depression Inventory (BDI). Results: The results reported adequate content validity and high reliability (Cronbach Alpha > 0,9 in all factors and β = 0,96). Exploratory factor analysis showed three factors explaining 52,569% of the variance. The scale was sensitive to activity levels assessed by the IPAQ (F (1)= 4.131; p <0.05) and negatively correlated with BDI (r = -0,127; p <0,05). Conclusions: The psychometric properties of the scale are appropriate for measuring the efficacy of adult users of Health Services to practice regular physical activity.

Fundamento: La estimación con pruebas estandarizadas de la creencia en la capacidad personal para practicar con regularidad actividad física (autoeficacia para la actividad física) nos permite establecer predicciones sobre la práctica real de esta conducta. El objetivo del trabajo es validar una escala que determine los niveles de autoeficacia de los usuarios adultos de los servicios de salud de atención primaria para realizar actividad física regular. Métodos: Fases: 1) Construcción de la escala y revisión por expertos, 2) Estudio piloto, 3) Determinación de su fiabilidad y validez. La muestra estuvo compuesta por 388 personas de 18 a 65 años usuarias de Centros de Salud de la provincia de Sevilla seleccionadas mediante muestreo por conglomerados (centros de salud) y cuotas (tamaño del municipio, edad y sexo). La validez concurrente fue hallada con el Cuestionario Internacional de Actividad Física (IPAQ) y la discriminante con el Inventario de Depresión de Beck (BDI). Resultados: Validez de contenido y fiabilidad: α de Cronbach>0,9 en todos los factores y β = 0,96. El análisis factorial mostró tres factores que explicaron el 52,569% de la varianza. La escala fue sensible a los niveles de actividad medidos con el IPAQ F (1)= 4.131; p <0,05 y correlacionó negativamente con el BDI (r=-0,127; p <0,05). Conclusiones: Las propiedades psicométricas de la escala son adecuadas para medir la autoeficacia de las personas adultas usuarias de los servicios de salud para practicar actividad física.

Objective: To determine the impact of a 1-month telephone care program after hospital discharge from a trauma surgery unit on health services utilization and patient anxiety and to perform a budgetary analysis. Methods: We carried out an experimental study in 604 patients who formed an experimental and a control group. The experimental group was offered telephone care to resolve doubts during the first month after discharge. After this period, data were collected from both groups on the following outcome variables: visits to the emergency department or family physician, hospital readmissions, and the results of an anxiety test. Data analyses included logistic and linear multivariate analyses and calculation of the budgetary impact of the program on the hospital, the Andalusian Health Service, and the National Health System. Results: A total of 73 telephone consultations were conducted with 60 patients, almost half for doubts about the therapeutic regimen. For the outcome variable "visit to emergency department", the group without telephone care had an odds ratio of 1.8 in the multivariate analysis adjusted for the other independent variables: days of hospital stay, patient anxiety and comprehension of discharge indications. No differences between groups were found in the remaining outcome variables. The budgetary analysis demonstrated the possibility of implementing the program at a cost of 1.65 € per patient. Conclusions: This program proved effective in reducing visits to the emergency department at a low cost.

Objetivo: Determinar el impacto, sobre la utilización de servicios sanitarios y la ansiedad del paciente, de un programa de atención telefónica durante el mes siguiente al alta hospitalaria de una unidad de cirugía traumatológica, junto con análisis presupuestario. Método: Estudio experimental en 604 pacientes, que constituyeron los grupos experimental y control. El grupo experimental dispuso de atención telefónica para solventar dudas durante el mes siguiente al alta. Al finalizar se registraron, en ambos grupos, si habían acudido a urgencias, al médico de familia o reingresado, y el resultado de un test de ansiedad, que constituyeron las variables resultado. El análisis de datos incluyó análisis multivariados de regresión logística y lineal, y cálculo del impacto presupuestario del programa en tres escenarios (Hospital, Servicio Andaluz de Salud, Sistema Nacional de Salud). Resultados: Se atendieron 73 consultas telefónicas de 60 pacientes, casi la mitad de ellas por dudas respecto al régimen terapéutico. Para la variable resultado «asistencia a los servicios de urgencias», en el análisis multivariado los pacientes sin atención telefónica tenían una odds ratio de 1,8, ajustada por las variables «días de estancia hospitalaria», «ansiedad del paciente» y «comprensión de las indicaciones al alta». Para el resto de las variables resultado no hubo diferencias entre ambos grupos. El análisis de costes demostró la posibilidad de implantar el programa con un coste de 1,65 euros por paciente intervenido. Conclusiones: El programa fue eficaz para disminuir las visitas a urgencias, a un coste reducido.

The theoretical and practical aspects regulating the evaluation process are examined when the object is made up of social organizations collaborating with the Government Agencies for the social services. In this way, it responds to the evaluation needs regarding following: 1) the knowledge of the characteristics of the object being evaluated; 2) the formulation of the criteria that will be used for their assessment; 3) the procedure through which the relevant data needed for assessment is obtained; and 4) the use of the results of the evaluation. All of these are directed to the formulation of a model that guides the evaluation practice of the social organizations.

Se examinan los aspectos teóricos y prácticos que regulan el proceso de la evaluación cuando el objeto de la misma lo conforman las organizaciones sociales que colaboran con la Administración Pública para la prestación de servicios sociales. De este modo, se pretende dar respuesta a las necesidades de evaluación en lo que se refiere a: 1) el conocimiento de las características del objeto de la evaluación; 2) la formulación de los criterios que serán empleados para su valoración; 3) el procedimiento a través del cual se obtendrá información relevante de cara a la valoración; y 4) la utilización de los resultados de la evaluación. Todo ello encaminado a la formulación de un modelo que pueda orientar la práctica de la evaluación de las organizaciones sociales.

Objetivo: validar la entrevista diagnóstica para estudios genéticos (DIGS 3.0) en Colombia. Métodos: se hicieron dos traducciones del inglés al español del DIGS y se hizo traducción en sentido inverso (al inglés) de cada una. Un comité de revisión verificó la equivalencia translingüística y transcultural. Se evaluó la confiabilidad examen-reexamen e interevaluador del DIGS 3.0 en 65 y 91 pacientes, respectivamente, mediante el cálculo de kappa de Cohen. Resultados: el DIGS 3.0 mostró ser comprensible, con validez de apariencia y de contenido. La confiabilidad interevaluador fue excelente para esquizofrenia (kapa=0,81, IC95%: 0,68-0,93), trastorno bipolar (kapa=0,87, IC95%: 0,75-0,99), trastorno depresivo mayor (kapa=0,86, IC95%: 0,7- 1) y ausencia de trastorno psiquiátrico (kapa=0,88, IC95%: 0,71-1); fue buena para otro diagnóstico psiquiátrico (kapa=0,65, IC95%: 0,41-0,89) y pobre para trastorno esquizoafectivo (kapa=0,37, IC95%: -0,02-0,76). La confiabilidad examen-reexamen fue excelente para todos los diagnósticos (kapa>0,8), excepto para otro diagnóstico psiquiátrico (kapa=0,64, IC95%: 0,31-0,96), donde fue buena. Conclusiones: la versión en español del DIGS para Colombia mostró comprensibilidad, validez de apariencia y de contenido, y confiabilidad examen-reexamen e interevaluador. Es una herramienta útil para estudios genéticos en esquizofrenia y en trastornos afectivos.

An interview tool, Diagnostic Interview for Genetic Studies (DIGS 3.0), was translated into Spanish for application in studies of psychiatric disorders in Colombia. Two Spanish translations of the original English version of DIGS were prepared and backtranslated into English. A review committee verified the linguistic and cultural equivalence of the translations. The evaluator and test-retest reliability were assessed calculating Cohen’s kappa for samples of 65 and 91 patients respectively. DIGS proved valid in both appearance and content. The confidence interval (C.I.) was excellent for schizophrenia (kappa=0.81, C.I. 95% = 0.68-0.93), bipolar disorder (kappa=0.87, C.I. 95% = 0.75-0.99), major depressive disorder (kappa=0.86, C.I. 95% = 0.70-1.00), and for a normal diagnosis (kappa=0.65, C.I. 95% = 0.41-0.89); it was good for other psychiatric diagnosis (kappa=0.65, C.I. 95% = 0.41-0.89) and poor for schizoaffective disorder (kappa=0.37, C.I. 95% = -0.02-0.76). Test-retest reliability was excellent for all diagnoses (kappa>0.8), except for "other psychiatric diagnoses" (kappa=0.64, C.I. 95% = 0.31-0.96). The Spanish translation of the DIGS was comprehensible, with face and content validity, and good test-retest and evaluator reliability. This translation will be a useful tool for genetic studies of psychiatric disorders in Latin America, particularly where schizophrenia and affective disorders are involved.

Background: We describe the investigation of an acute gastroenteritis outbreak with an explosive beginning, probably waterborne, that ocurred in an nursing home in Albacete, in November 1999 and affected 104 inmates and 35 employees. The dominant symptoms were diarrhea and vomiting. Methods: A case-control study was designed. We carried out a descriptive analysis of facts and crossing of variables with the help of simple tables. A multivariant analysis, by models of logistic regression, was conducted both for the explosive phase and for that of transmission person-to-person. Results: The attack rate for inmates was 45,8% and for employees,33,7%. An association was found between the use of the main dining-room of the nursing home and the explosive beginning of the outbreak and between the previous presence of a case-patient in the shared bedrooms and the person-to-person transmission. Norwalk-like virus was isolated from faeces samples of four cases. Conclusions: The analysis of the clinical and epidemiological findings as well as the laboratory results proved the implication of a Norwalk-like virus in this outbreak.

Fundamento: Se describe el estudio de un brote de gastroenteritis aguda de inicio explosivo y probable origen hídrico, en una residencia de ancianos de Albacete, en el mes de noviembre de 1999 y que afectó a 104 residentes y a 35 trabajadores. El cuadro clínico se caracterizó por la presentación de vómitos y diarrea como síntomas dominantes. Métodos: Se diseñó un estudio de casos y controles. Se realiza un análisis descriptivo de datos y cruce de variables mediante tablas simples. El análisis multivariante se realiza mediante modelos de regresión logística para la fase explosiva y de transmisión de persona a persona. Resultados: La tasa de ataque fue del 45,8% para residentes y de 33,7% en trabajadores. Se encontró asociación entre la utilización del comedor principal de la residencia y el inicio explosivo del brote así como entre la presencia inicial de un caso en habitaciones compartidas y la transmisión de persona a persona. Se aisló virus Norwalk-like en heces de 4 enfermos. Conclusiones: El análisis de las características clínicas y epidemiológicas, así como los resultados de laboratorio confirman la implicación del virus Norwalk-like como agente causal en este brote.
