Valoración de un programa de telemedicina en obstetricia durante la pandemia por la COVID-19

ABSTRACT Introduction: Telemedicine has positioned itself as a strategy to improve the quality and coverage of services, but its implementation represents a challenge for health workers. Objective: To determine the perception of the use of the Telemedicine program during the COVID-19 pandemic among midwifery health professionals. Methods: Descriptive cross-sectional study carried out on 40 obstetrics professionals working in a telemedicine program in health centers of the Ministry of Health and who were selected non-randomly. A content-validated and reliable questionnaire was used. Frequencies and percentages were estimated. Results: 45% agreed that the trainings were timely and efficient, as well as the determination of roles and profiles for a commitment to the program (50%). However, 47.5% pointed out the lack of articulation of activities in the different areas. 85% found the platform user-friendly and 70% that it had an efficient information registration system. Also, that the program provides timely access to users (60%), optimizes consultation time and number of users (52.5%), as well as more flexible timetables (55%). Conclusions: Obstetrics professionals highly value the telemedicine program, recognizing it as a useful and technologically appropriate tool for improving healthcare access and quality. Nonetheless, there is a need for better organization and articulation of this technology in professional activities.
RESUMEN Introducción: La telemedicina se ha posicionado como estrategia para mejorar la calidad y cobertura de los servicios, no obstante, su implementación representa un desafío para el personal de salud. Objetivo: Determinar la percepción del uso del programa de telemedicina durante la pandemia por la COVID-19 en profesionales de salud de obstetricia. Métodos: Estudio descriptivo de corte transversal desarrollado en 40 profesionales de obstetricia que trabajaban en un programa de telemedicina de centros sanitarios del Ministerio de Salud y que fueron seleccionados de manera no aleatoria. Se utilizó un cuestionario validado en contenido y confiable. Se estimaron frecuencias y porcentajes. Resultados: El 45 % estuvo de acuerdo con que las capacitaciones fueron oportunas y eficientes, así como la determinación de roles y perfiles para un compromiso con el programa (50 %). No obstante, el 47,5 % señalaron la falta de articulación de las actividades en las distintas áreas. Al 85 % le pareció amigable la plataforma y al 70 % que tenía un sistema de registro de información eficiente. También que el programa brinda un acceso oportuno a los usuarios (60 %), optimiza el tiempo de consulta y cantidad de usuarios (52,5 %), y horarios más flexibles (55 %). Conclusiones: El programa de telemedicina es bien valorado por los profesionales de obstetricia, la reconocen como una herramienta útil y tecnológicamente adecuada para mejorar el acceso y la calidad de la atención en salud. Sin embargo, es necesario perfeccionar la organización y la articulación de esta tecnología en las actividades profesionales.

ABSTRACT The present study assessed the diagnostic performance of the Xpert®Xpress SARS-CoV-2 test in comparison with the Charité protocol real-time RT PCR for the detection of SARS-CoV-2 in Peruvian patients. This was a diagnostic test study that included 100 nasal and pharyngeal swab samples. We obtained an overall concordance of 98.70% (95%CI: 92.98-99.97), with a kappa coefficient of 0.97 (95%CI: 0.86-1.00) and sensitivity and relative specificity rates of 100% and 96.15%, respectively. Additionally, the percentage of the area under the ROC curve was 98.08% in both cases, and an analytical specificity rate of 100% was obtained for the different respiratory viruses evaluated. In conclusion, the Xpert®Xpress SARS-CoV-2 test, by using nasal and pharyngeal swab samples, was highly sensitive and specific, and the kappa coefficient showed an excellent correlation when compared to the reference test.
RESUMEN En el presente estudio se estimó el rendimiento diagnóstico de la prueba Xpert®Xpress SARS-CoV-2 en comparación con la RT PCR en tiempo real-protocolo Charité, para la detección de SARS-CoV-2 en pacientes peruanos. Se trató de un diseño de prueba diagnóstica que incluyó 100 muestras de hisopado nasal y faríngeo. Se obtuvo una concordancia global de 98,70% (IC95%: 92,98-99,97), con un coeficiente kappa de 0,97 (IC95%: 0,86-1.00); se estimó una sensibilidad y especificad relativa de 100% y 96,15%, respectivamente. Adicionalmente, el porcentaje del área bajo la curva ROC fue 98,08% en ambos casos y se obtuvo una especificidad analítica del 100% para los diferentes virus respiratorios evaluados. Abstract Background Extracellular vesicles (EVs) and their cargoes, including MicroRNAs (miRNAs) play a crucial role in cell-to-cell communication. We previously demonstrated the upregulation of bta-mir-148b in EVs from oviductal fluid of cyclic cows. This miRNA is linked to the TGF-β pathway in the cell proliferation. Our aim was to verify whether miR-148b is taken up by embryos through gymnosis, validate its target genes, and investigate the effect of miR-148b supplementation on early embryo development and quality. Methods Zygotes were cultured in SOF + 0.3% BSA (Control) or supplemented with: 1 μM miR-148b mimics during: D1-D7 (miR148b) or D1-D4 (miR148b-OV: representing miRNA effect in the oviduct) or D4-D7 (miR148b-UT: representing miRNA effect in the uterus) or 1 μM control mimics was used during: D1-D7 (CMimic). Embryos at ≥ 16-cells and D7 blastocysts (BD7) were collected to examine the mRNA abundance of transcripts linked to the TGF-β pathway (TGFBR2, SMAD1, SMAD2, SMAD3, SMAD5, BMPR2, RPS6KB1, POU5F1, NANOG), total cell number (TC), trophectoderm (TE), and inner cell mass (ICM) were also evaluated. One-way ANOVA was used for all analyses. Results We demonstrated that miR-148b can be taken up in both 16-cell embryos and BD7 by gymnosis, and we observed a decrease in SMAD5 mRNA, suggesting it's a potential target of miR-148b. Cleavage and blastocysts rates were not affected in any groups; however, supplementation of miR-148b mimics had a positive effect on TC, TE and ICM, with values of 136.4 ± 1.6, 92.5 ± 0.9, 43.9 ± 1.3 for miR148b and 135.3 ± 1.5, 92.6 ± 1.2, 42.7 ± 0.8, for miR148b-OV group. Furthermore, mRNA transcripts of SMAD1 and SMAD5 were decreased (P ≤ 0.001) in 16-cell embryos and BD7 from miR148b and miR148b-OV groups, while POU5F1 and NANOG were upregulated (P ≤ 0.001) in BD7 and TGFBR2 was only downregulated in 16-cell embryos. pSMAD1/5 levels were higher in the miR148b and miR148b-OV groups. Conclusions Our findings suggest that supplementation of bta-miR-148b mimics during the entire culture period (D1 - D7) or from D1 - D4 improves embryo quality and influences the TGF-β signaling pathway by altering the transcription of genes associated with cellular differentiation and proliferation. This highlights the importance of miR-148b on embryo quality and development.

Resumen En este artículo se identificaron y relacionaron los significados de las sucesiones matemáticas en los números naturales a través de su estructura conceptual, representaciones y modos de uso en el currículo mexicano de matemáticas de secundaria y en catorce libros de texto utilizados para la enseñanza y aprendizaje de las sucesiones en este nivel escolar. Para ello, se realizó un análisis didáctico enfocado en la dimensión de contenido matemático (RICO et al., 2008; RICO, 2012; RICO, MORENO, 2016). Los resultados evidencian que en México el tema de sucesiones matemáticas se aborda desde la básica primaria hasta la secundaria y las propuestas de enseñanza abordan el uso de múltiples representaciones y aplicaciones en otras disciplinas. Abstract In this article, the meanings of mathematical sequences in natural numbers were examined through their conceptual structure, representations, and modes of use in the middle school mathematics curriculum in Mexico and in fourteen textbooks used for teaching and learning mathematical sequences in this school level. For this purpose, we carried out a didactic analysis focused on the dimension of mathematical content (RICO et al., 2008; RICO, 2012; RICO, MORENO, 2016). The results show that, in Mexico, the topic of mathematical sequences is addressed from elementary to middle school and the teaching proposals address the use of multiple representations and applications in other disciplines. Resumen El propósito de esta investigación fue identificar y conectar medidas y patrones en tipos de siembra de palma de coco, construidos por un campesino mexicano. El estudio se fundamentó en la Etnomatemática y se desarrolló bajo una metodología de tipo cualitativa-etnográfica en tres etapas: una observación participante para lograr la familiarización con el campesino, entrevistas semiestructuradas para recolectar la información, y un análisis temático etnomatemático de los datos. Los resultados muestran que el campesino construye dos sistemas de siembra: marco real y tres bolillos, usando medidas no convencionales como el paso y el cordel, y, como unidad de medida convencional, el metro. Además, aplicó razones trigonométricas y el teorema de Pitágoras para hallar la altura del triángulo, usado en la siembra tres bolillos e implementarlo como patrón de medida. Los hallazgos tienen potencial matemático para la enseñanza y aprendizaje de tópicos matemáticos, conectados al contexto sociocultural de los estudiantes y profesores.
Abstract Research’s goal is to identify and connect measurements and patterns in coconut palm planting types constructed by a Mexican farmer. The study was theoretically based on Ethnomathematics and was developed under a qualitative-ethnographic methodology in three stages: first, a participant observation was carried out to achieve familiarization with the peasant. Second, to collect the data, semi-structured interviews were implemented, and third, an ethnomathematical thematic analysis of the data was followed. The results show that the farmer builds two sowing systems (real frame and three bobbins) using non-conventional measures such as step and string, and, as a conventional unit of measurement, the meter. In addition, he applied trigonometric ratios and the Pythagorean theorem to find the height of the triangle, used in the triangular planting system, and implement it as a measurement standard. This work has mathematical potential for the teaching and learning of mathematical topics connected to the sociocultural context of students and teachers.

Abstract This study evaluated the microbiological quality and prevalence of Escherichia coli and diarrheagenic E. coli pathotypes (DEP) among fresh-squeezed orange juice from street vendors and their hygienic practices. Sixty orange juice stalls in Mexico City were visited, and 60 samples were purchased. Most (65%) of the street vendors had poor hygienic practices. Twelve (20%) prepared orange juice at the time of purchase, while the other stalls sold already-squeezed orange juice. All samples analyzed were positive for aerobic mesophilic bacteria, with limits ranging from 1 to 6.8 log CFU/mL. A total of 35 (58.3%), 27 (45%), and 5 (8.3%) samples were positive for total coliforms, fecal coliforms, and E. coli, respectively, present at concentrations ranging from < 3 to > 1100, < 3 to > 1100, and < 3 to 53 MPN/mL, respectively. Two samples harbored DEP at concentrations of 3.6 and 9.2 MPN/mL. Both samples harbored Shiga toxin-producing E. coli strains, positive for the Shiga toxin 2 (stx2) locus. Our results suggest that consumption of fresh-squeezed street-vended orange juice may pose a health risk for consumers, revealing the necessity to implement adequate hygiene and safety practices to protect the health of consumers.

Abstract Conventional orthodontic treatment with the use of stainless steel may be detrimental to oral health by promoting demineralizing lesions appearance and increasing adhesion and formation of bacterial biofilm, inducing the development of cavities. An alternative that has been researched to reduce the side effects of orthodontic treatment is the coating of materials with antimicrobial nanoparticles. Nanometric- sized particles increase their surface area and contact with the microbial membrane, consequently intensifying their bacteriostatic and bactericidal effect. In this work, hydrothermal synthesis, a “green” process was used to attach silver nanoparticles (AgNPs) to the surface of two different brands of orthodontic wires. The coated materials were analyzed for their physicochemical properties by scanning electron microscopy (SEM), X-ray diffraction (XRD) and differential scanning calorimetry (DSC), which showed the distribution of AgNPs along the wires without modifying their properties. In the microbiological test, one of the brands showed a statistically significant difference in microbial adhesion and biofilm formation by Staphylococcus aureus and Streptococcus mutans. Results lead to the conclusion that antimicrobial orthodontic wires coated with silver nanoparticles through hydrothermal synthesis is a promising material for the improvement of orthodontic treatment.

RESUMO Esta pesquisa objetivou conhecer o nível de satisfação e a importância do trabalho para a equipe multiprofissional de Atenção Primária à Saúde. Trata-se de pesquisa quantitativa e de caráter exploratório, transversal. O nível de satisfação dos profissionais foi classificado no escore bom, porém, algumas questões indicaram baixo grau de satisfação. O grau de importância do trabalho destacou-se em nível mais elevado nas questões relacionadas à qualidade do trabalho desenvolvido e à comunicação com a equipe. Conclui-se que existe a necessidade de mudanças na gestão organizacional, especialmente no que diz respeito à ampliação de recursos humanos e materiais, pois identificam-se como indicadores expressivos que contribuem para menor satisfação com o trabalho e alto impacto sobre a saúde do trabalhador.

Resumo A insuficiência renal crônica é um problema da saúde pública e a hemodiálise é um tratamento invasivo e desgastante, do qual os pacientes relatam sentimentos de impotência e dependência. A pesquisa teve como objetivos propor atividades lúdicas e relaxamento durante sessões de hemodiálise e investigar opinião dos pacientes sobre o trabalho, bem como conhecer vivências e percepções quanto à doença e tratamento. Método Trata-se de uma pesquisa qualitativa realizada em um hospital geral da região da Baixada Santista – litoral sul de São Paulo – com grupos de pacientes durante as sessões de hemodiálise, na qual se realizaram atividades lúdicas, relaxamento e visualização, entrevista em profundidade e aplicação de escala Likert. As entrevistas foram categorizadas pelo método de análise de conteúdo do tipo temática. Resultados Foram realizados quatro grupos envolvendo um total de 13 participantes, com idades entre 20 e 70 anos, de ambos os sexos, com tempo de hemodiálise que variou de um mês a dois anos. Conclusão Relato dos participantes sobre as atividades propostas indicam que houve melhora relativa do humor e que elas contribuíram para melhor enfrentamento da situação de imobilidade e ociosidade durante as sessões de hemodiálise, além de permitirem a expressão de necessidades subjetivas singulares.
Abstract Chronic renal failure is a public health problem and hemodialysis is considered an invasive and exhausting long-term process. Patients usually report feelings of helplessness and dependence, and ludic activities might provide involvement, joy and pleasure. Objectives To propose ludic activities, to investigate patients’ opinions about these activities, and to understand patients’ experience regarding disease and treatment. Method This was a qualitative study conducted in a general hospital in the metropolitan area of Santos, Brazil, with groups of patients during hemodialysis sessions, where ludic activities, relaxation and visualization, in-depth interviews and application of a Likert scale were performed. The speeches were categorized by the method of content thematic analysis. Results We had four groups involving 13 participants, aged between 20 and 70 years, of both sexes. Conclusion The proposed activity is a positive factor for mood improvement and contributed to better cope with the situation of immobility and idleness, mobilizing objectively and subjectively, and in addition, allowing the expression of emotions in a constructive way, revealing patients’ unique needs and idiosyncrasies.
Resumen La enfermedad renal crónica es un problema de salud pública y la hemodiálisis es un tratamiento invasivo y agotador del que los pacientes reportan sentimientos de impotencia y dependencia. La investigación tuvo como objetivo ofrecer actividades lúdicas y de relajación durante las sesiones de hemodiálisis, investigar la opinión de los pacientes acerca de la propuesta de trabajo y conocer las experiencias y percepciones acerca de la enfermedad y el tratamiento. Método se realizó un estudio cualitativo en un hospital general en la región de Santos - costa sur de São Paulo - con grupos de pacientes durante las sesiones de hemodiálisis, en las que se realizaron actividades lúdicas, de relajación y de visualización, entrevistas en profundidad y la aplicación de la escala de Likert. Las entrevistas fueron categorizadas por el análisis de contenido temático. Resultados cuatro grupos con un total de 13 participantes, con edades comprendidas entre 20 y 70 años, de ambos sexos, con una duración de hemodiálisis que osciló entre un mes y dos años. Conclusión la opinión de los participantes sobre las actividades propuestas fue que produjeron una relativa mejora del estado de ánimo y ayudaron a enfrentar la situación de inmovilidad e inactividad durante las sesiones de hemodiálisis, permitiendo la expresión de necesidades individuales subjetivas.

Nested-PCR multiplex test with increased sensitivity for detection of allogeneic cells transplanted from male to female mice

A terapia celular traz perspectivas encorajadoras à medicina humana e veterinária. Experimentalmente, a manipulação genética permite a marcação e a localização de células alogênicas. Porém, isso torna seu genótipo/fenótipo diferente daquelas usadas clinicamente, sem marcação. Alternativamente, a presença do cromossomo Y possibilita detectar células de doadores machos no organismo de fêmeas. Todavia, a concentração de células transplantadas pode ser mínima em certos tecidos, pela distribuição sistêmica. Neste estudo, foi desenvolvida uma nested-PCR multiplex, visando a aumentar a sensibilidade do diagnóstico de presença/ausência de células derivadas do tecido adiposo (CDTA-Y) e derivadas da fração mononuclear da medula óssea (CFMO-Y) de camundongos machos, em amostras de sangue e de pulmões de camundongos fêmeas, após transplante endovenoso. Quatro fêmeas receberam placebo; quatro fêmeas receberam CDTA-Y de dois machos; e quatro fêmeas receberam CFMO-Y de dois machos. A primeira fase da PCR teve dois pares de primers (multiplex): um para amplificação de fragmento do cromossomo Y (SRYout; 300pb); outro para amplificação de fragmento do cromossomo X (gene DXNds3). Na segunda fase da PCR, foi usado um par de primers para amplificação de fragmento de 110pb (SRYinn) interno ao produto amplificado pelo SRYout. O controle interno da reação (gene DXNds3) foi detectado em todas as amostras de DNA testadas, enquanto que o fragmento externo do gene SRY (300pb) foi detectado apenas nas amostras puras de DNA de CDTA-Y e CFMO-Y. O fragmento interno do gene SRY (110pb) foi detectado no sangue e nos pulmões de 100% das receptoras de CDTA-Y e CFMO-Y. A técnica de nested-PCR aumentou a sensibilidade e a segurança do diagnóstico molecular de presença ou ausência de células de camundongos machos em fluidos e tecidos de receptoras fêmeas após transplante endovenoso.
Cell therapy has shown encouraging perspectives for human and veterinary medicine. Experimentally, genetic manipulation allows to mark and locate allogeneic cells. However, this makes their genotype/phenotype different from non-marked cells used clinically. Alternatively, the presence of the Y-chromosome enables male donor cells detection in female organisms. However, the concentration of engrafted cells may be minimal in tissues, due to systemic distribution. In this study, a nested-PCR multiplex test was developed, aiming to increase the sensitivity of the presence/absence diagnosis of male mice adipose-derived (ADSC-Y) and bone marrow mononuclear (BMNC-Y) cells in samples of blood and lungs from females, after endovenous transplantation. Four females received placebos; four females received ADSC-Y from two males; and four females received BMNC-Y from two males. The PCR first-step included two primer sets (multiplex): one for amplification of a Y-chromosome fragment (SRYout; 300bp); the other for amplification of an X-chromosome (DXNds3 gene) fragment. In the PCR second-step, one primer set (SRYinn) was used for amplification of a 110bp fragment, restrained in the SRYout amplification product. The PCR internal control (DXNds3 gene) was detected in all DNA samples, whereas the SRY gene external fragment (300bp) was detected exclusively in ADSC-Y and BMNC-Y pure DNA samples. The SRY gene internal fragment (110bp) was detected in 100% of the blood and lung samples from the ADSC-Y and BMNC-Y female recipients. The nested-PCR technique increased sensitivity and reliability for molecular diagnostic of presence or absence of male mice cells in body fluids and tissues of female recipients after endovenous transplantation.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.
