Resumo Objetivou-se avaliar a eficácia analgésica pós-operatória da dexmedetomidina isolada e associada à morfina em pacientes submetidas à anestesia geral inalatória com isoflurano e submetidas à ovariohisterectomia eletiva (OH). Vinte cadelas saudáveis foram selecionadas a partir de exames físico e laboratoriais. Previamente ao estudo, foram submetidas a um período de 24 horas de adaptação ao ambiente e aos observadores. Ato contínuo, foram divididas aleatoriamente em dois grupos: grupo dexmedetomidina, na dose de 10 μg/Kg (GD) e grupo dexmedetomidina (10 μg/Kg) associado com morfina, na dose de 0,3 mg/Kg (GDM), administrados pela via intramuscular (IM). Ato contínuo, as pacientes foram induzidas com propofol e mantidas com isoflurano diluído em oxigênio 100% e administrado através de vaporizador calibrado. Para verificação da analgesia pós-operatória foram realizadas avaliações de dor por meio de escalas de Glasgow Modificada (EGM) e de Melbourne (EM). Para avaliação de sedação, por meio da escala de Dobbins (ED), em diferentes tempos: antes da administração da MPA (T0), e em mais 6 tempos no período pós-operatório, uma (T1), duas (T2), quatro (T3), oito (T4), 12 (T5) e 24 (T6) horas após a extubação orotraqueal. Não foram observadas diferenças estatísticas entre os grupos nas escalas de acordo com o teste de Kruskal-Wallis post-hoc de Dunn; e entre os tempos notou-se diferença estatística pelo teste de Friedman (p<0,05) para EGM e EM, porém não houve necessidade de resgate analgésico. Portanto, conclui-se que a dexmedetomidina isolada e associada a morfina produziu efeito analgésico adequado no período pós-operatório de cadelas submetidas a OH.

Abstract This study aimed to evaluate the postoperative analgesic efficacy of dexmedetomidine alone and when associated with morphine in patients under general inhalational anesthesia with isoflurane and undergoing elective ovariohysterectomy (OH). Twenty healthy bitches were selected via physical and laboratory examinations. Prior to the study, they underwent a 24-h period of adaptation to the environment and observers. They were then randomly divided into two groups: the dexmedetomidine group receiving a dose of 10 μg/kg, and dexmedetomidine group (10 μg/kg) associated with morphine receiving a dose of 0.3 mg/kg, administered via the intramuscular route. Thereafter, patients were induced with propofol and maintained with isoflurane diluted in 100% oxygen administered through a calibrated vaporizer. To verify postoperative analgesia, pain assessments were performed using the modified Glasgow (EGM) and Melbourne (EM) scales. For sedation assessment, the Dobbins scale was used at different times: before the administration of pre-anesthetic medication (T0) and at another six times in the postoperative period, 1 (T1), 2 (T2), 4 (T3), 8 (T4), 12 (T5), and 24 (T6) h after orotracheal extubation. No statistical differences were observed between groups in the scales according to Dunn’s Kruskal-Wallis post hoc test, and between the times a statistical difference was noticed by the Friedman test (p<0.05) for the EGM and EM scale scores, but there was no need for analgesic rescue. Therefore, we found that isolated dexmedetomidine and morphine produced adequate analgesic effects in the postoperative period of bitches submitted for OH.

Foi realizada falha segmentar de 5cm na região diafisária do fêmur de 14 cães adultos, sendo utilizado implante ósseo cortical alógeno conservado em mel para a sua reconstrução. O implante foi estabilizado no leito receptor por meio de uma placa de compressão dinâmica e oito parafusos corticais. Foram realizadas avaliações clínicas e radiográficas periódicas por até 360 dias. A porcentagem de incorporação das interfaces foi de 79,2%, e o tempo médio necessário para a incorporação foi de 67,1 dias, variando entre 45 e 90 dias. Não foi verificada diferença significativa entre o tempo de incorporação das interfaces proximal e distal. As principais complicações foram não-união, fratura e reabsorção intensa do implante. Apesar das complicações, é possível concluir que o implante ósseo cortical alógeno conservado em mel é opção viável para a reconstrução óssea.

Fourteen adult mongrel dogs were used to evaluate the honey preserved cortical allografts in the repair of diaphyseal femoral defect. The allografts were inserted into a 5cm segmental defect created in the mid-diaphysis of the right femur in each dog. The bones were stabilized with a dynamic compression plate and eight bone screws. Healing was followed clinically and femora were evaluated radiographically, periodically. Nineteen (79.2%) of the twenty-four host-graft interfaces were radiographically incorporated. Average time to allograft incorporation was 67.1 days (range 45 days to 90 days). There was no statistical difference in the allograft incorporation time between proximal and distal host-graft interfaces. Complications observed were nonunion, allograft fracture, and allograft resorption. The conclusion is that despite the complications, honey preserved cortical allografts are a viable option to bone reconstruction.

Neste experimento, foi utilizado um retalho do músculo grande dorsal, com pedículo dorso-caudal, para o reparo de defeitos diafragmáticos iatrogênicos em 18 cães adultos. O acesso ao diafragma foi através de toracotomia no décimo espaço intercostal, sendo criado um defeito diafragmático, onde o retalho foi suturado. O retalho muscular pediculado cobriu um terço do hemidiafragma direito. Os animais tiveram boa recuperação pós-cirúrgica, sem sinais de comprometimento respiratório. Foram feitas necropsias aos 15, 30 e 60 dias do pós -operatório. O local submetido ao implante apresentou algumas aderências com vísceras adjacentes e consolidação da sutura macro e microscopicamente. Conclui-se que o retalho pediculado do grande dorsal pode ser útil para o reparo de grandes defeitos diafragmáticos em cães.

In this study, the Latissimus dorsi muscular flap was experimentally used for the repair of iatrogenic diaphragmatic defects in 18 adult dogs. The flap was elevated maintaining its dorso-caudal attachment. The diaphragm was accessed through a right thoracotomy in the tenth intercostal space, a defect in the diaphragm was created and the flap was sutured in the margins of the defect. The flap was capable of covering one third of the right hemidiaphragm. All the animals had good post-surgery recoveries. Respiratory function was not compromised. Necropsies were taken at 15, 30 and 60 days post-surgery. Macro and microscopically, the muscle showed few adhesions with adjacent viscera and excellent healing. The Latissimus dorsi muscular flap can be useful for reconstruction of large diaphragmatic defects in dogs.

Foram utilizados 30 frangos com 20 semanas de idade, pesando em média 2,90 ± 0,65kg divididos aleatoriamente em três grupos. A indução da anestesia foi realizada com máscara facial artesanal conectada ao sistema de Maggil Modificado, utilizando entre 3,0 e 3,5 vezes a dose anestésica mínima (DAM) de cada agente e fluxo diluente de O2 de 2l/min, sendo que posteriormente os animais foram intubados e mantidos com valores de aproximadamente 1,7DAM durante 65 minutos. O isofluorano causou maior depressão respiratória e hipotensão; o halotano proporcionou maiores valores de pressão arterial e temperatura corporal e o sevofluorano, menor depressão respiratória e hipotensão em relação ao grupo do isofluorano, sendo considerado o agente mais indicado para a utilização em aves. A indução e recuperação foram mais rápidas com o sevofluorano, embora sem diferença significativa estatisticamente.

For the comparison 30 twenty-week-old chicken were used. They weigh na averdage of 2.90 ± 0.65kg aleatorily divided into 3 groups. The anesthetic induction was accomplished with a home-made facial mask connected with the modified circuit of Maggil using values between 3.0 e 3.5 a minimal anesthetic dose (MAD) for each agent. Posteriorly, the animals were intubated and anesthesia was maintained with values approximately 1.7DAM during 65 minutes. Isoflurane caused further respiratory depression and hypotension when it is compared to the other two anesthetic agents. The group of halothane presented higher values of blood pressure and body temperature and the group of sevoflurane, lower respiratory depression and hypotension in relation to the group of isoflurane. The induction and recuperation were faster with the sevoflurane, although without expressive diference statistically. The sevoflurane is the most indicated anesthetic agent.

A prática da anestesia inalatória em aves vem ganhado grande popularidade principalmente na clínica de animais exóticos. Os programas de preservação de aves exóticas, principalmente da fauna brasileira, vem aumentando bastante, bem como, o número de problemas cirúrgicos e não cirúrgicos que, muitas vezes, requerem anestesia geral. Existe uma carência muito grande em relação à literatura sobre a anestesia dessas espécies, com pequeno número de trabalhos publicados com diversidade de protocolos anestésicos. O objetivo desta revisão é relatar os efeitos dos principais agentes anestésicos utilizados na medicação pré-anestésica e na manutenção anestésica de aves após uma revisão de literatura dos últimos 20 anos.

The preservation programs in exotics birds, mainly those the brazilian fauna, has increased as well the nunber of surgery and not surgery problems those required general anesthesia. There are few articles in the last years that talk about bird’s anesthesia, especially in inhalation anesthesia. The goal this review paper is discusses the effects of main anesthetic protocols used in birds in the last 20 years.

Neste experimento, foram utilizados 6 cães, com o objetivo de avaliar a viabilidade do músculo reto do abdome pediculado para correção de defeito no diafragma. O músculo foi dissecado até próximo sua inserção e deslizado para a cavidade abdominal através de uma incisão paracostal. Após a formação de um defeito no diafragma de aproximadamente 4 x l0cm, o músculo reto do abdome foi fixado com pontos de Wolff com fio de seda 2-0. Os animais foram observados por um período de tempo determinado de 15, 30 e 60 dias, quando foram operados novamente para observação macroscópica e microscópica. Tecido conetivo e áreas de hemorragia entre o retalho e diafragma, com tecido de granulação de maturidade média, foram observados no material das biópsias aos 15 dias. Aos 30 dias, foi notado pouco tecido de granulação que, aos 60 dias, era maduro. Na região do implante, foi verificada integração tecidual entre o músculo reto do abdome e diafragma com total oclusão do defeito diafragmático, formação de tecido de granulação, onde foram observadas aderências do fígado, lobo pulmonar caudal, estômago e omento. O músculo reto do abdome pediculado pode ser indicado para correção de defeitos no músculo diafragma, havendo uma completa integração tecidual na região do implante.

With the objective of analyzing the use Rectus abdominis muscle flaps for repair of large diaphagmatic defects, when direct suture is impossible, six dogs underwent surgery. The Rectus ahdominis muscle was dissected up to its insertion and was introduced into the abdominal cavity through a paracostal incision. A defect of 4 x l0cm was created in the diaphragmatic muscle and the flap was sutured to the edges of the defect with horizontal mattres suture with 2-0 silk. Animais were clinically evaluated every day at the post-operative period. Biopsies and macroscopic observation were done at the 15th, 30th and 60th day. Connective tissue and hemorrhagic areas among the flap and the diaphragm, and granulation tissue with median maturity were observed in the biopsy material at the 15th day. At the 30th day, few granulation tissue among the muscles were observed. At the 60th day, there was mature granulation tissue. Adherence ocurred in the region of the implant with the liver, caudal pulmonar lobe, stomach and omentun. The Rectus abdominis muscle can be used to correct defects in the diaphragm with a complete tissue integration in the implanted region.

A visibilização e a identificação dos vasos que irrigam os tecidos é facilitada quando são utilizadas substâncias com poder corante e de distensão vascular ou meios radiopacos. Nesta pesquisa, em 22 cães foi realizada toracotomia lateral intercostal esquerda no 8º espaço intercostal e clampeamento da aorta torácica a ± 1 cm cranial ao diafragma. Em 10 animais, foi perfundida solução de gelatina/anilina e, no restante, injetado contraste vascular positivo e acompanhamento radiográfico. Foram identificadas artérias que vascularizavam o diafragma, mas não o fígado quando é efetuado o clampeamento aórtico no tórax.

The visual observation and identification of the vessels that irrigate the corporal tissues is greatly facilitated by using special staining dyes and vessel distenders, or by using a positive vascular contrast solution. In this research, lateral intercostal thoracotomy was performed on the eighth intercostal space in the left side of 22 dogs followed by clamping of thoracic aorta ± 1 cm cranial to the diaphragm. Ten dogs were then injected with the gelatin/aniline mixture, and the other 12 dogs were given a positive vascular contrast solution. In summary, following clamping of the thoracic aorta, arteries that irrigate the diaphragm, but not the liver, were identified.
