RESUMO Antecedentes: A doença crítica é um importante ônus permanente da assistência médica em todo o mundo e está associada a altas taxas de mortalidade. Os inibidores do cotransportador de sódio-glicose do tipo 2 têm demonstrado consistentemente benefícios nos desfechos cardiovasculares e renais. Os efeitos dos inibidores do cotransportador de sódio-glicose do tipo 2 em doenças agudas ainda não foram devidamente investigados. Métodos: O DEFENDER é um estudo de iniciativa do investigador, multicêntrico, randomizado, aberto, desenhado para avaliar a eficácia e a segurança da dapagliflozina em 500 participantes adultos com disfunção orgânica aguda hospitalizados na unidade de terapia intensiva. Os participantes aptos serão randomizados 1:1 para receber 10mg de dapagliflozina e o tratamento padrão por até 14 dias ou apenas o tratamento padrão. O desfecho primário é um composto hierárquico de mortalidade hospitalar, início de terapia renal substitutiva e tempo de internação na unidade de terapia intensiva, até 28 dias. O monitoramento da segurança será rigoroso durante todo o estudo. Conclusão: O DEFENDER é o primeiro estudo desenvolvido para investigar o uso de um inibidor do cotransportador de sódio-glicose do tipo 2 em pacientes de unidade de terapia intensiva geral com disfunção orgânica aguda. O estudo fornecerá informações relevantes sobre o uso de medicamentos dessa classe promissora em pacientes críticos. ABSTRACT Background: Critical illness is a major ongoing health care burden worldwide and is associated with high mortality rates. Sodium-glucose cotransporter-2 inhibitors have consistently shown benefits in cardiovascular and renal outcomes. The effects of sodium-glucose cotransporter-2 inhibitors in acute illness have not been properly investigated. Methods: DEFENDER is an investigator-initiated, multicenter, randomized, open-label trial designed to evaluate the efficacy and safety of dapagliflozin in 500 adult participants with acute organ dysfunction who are hospitalized in the intensive care unit. Eligible participants will be randomized 1:1 to receive dapagliflozin 10mg plus standard of care for up to 14 days or standard of care alone. The primary outcome is a hierarchical composite of hospital mortality, initiation of kidney replacement therapy, and intensive care unit length of stay, up to 28 days. Safety will be strictly monitored throughout the study. Conclusion: DEFENDER is the first study designed to investigate the use of a sodium-glucose cotransporter-2 inhibitor in general intensive care unit patients with acute organ dysfunction. It will provide relevant information on the use of drugs of this promising class in critically ill patients. ABSTRACT The SISBIOTA-BRASIL was a three-year multimillion-dollar research program of the Brazilian government to document plants and animals in endangered/understudied areas and biomes in Brazil. Distributional patterns and the historical events that generated them are extensively unknown regarding Brazilian fauna and flora. This deficiency hinders the development of conservation policies and the understanding of evolutionary processes. Conservation decisions depend on precise knowledge of the taxonomy and geographic distribution of species. Given such a premise, we proposed to research the diversity of Diptera of the Brazilian western arc of Amazon, Cerrado, and Pantanal in the states of Mato Grosso, Mato Grosso do Sul, and Rondônia. Three important biomes of the South American continent characterize these Brazilian states: Amazon forest, Cerrado (Brazilian Savannah), and Pantanal. Besides their ecological relevance, these biomes historically lack intensive entomological surveys. Therefore, they are much underrepresented in the Brazilian natural history collections and in the scientific literature, which is further aggravated by the fact that these areas are being exponentially and rapidly converted to commercial lands. Our project involved over 90 collaborators from 24 different Brazilian institutions and one from Colombia among researchers, postdocs, graduate and undergraduate students, and technicians. We processed and analyzed nearly 300,000 specimens from ~60 families of Diptera collected with a large variety of methods in the sampled areas. Here, we provide a detailed overview of the genera and species diversity of 41 families treated. Our results point to a total of 2,130 species and 514 genera compiled and identified for the three states altogether, with an increase of 41% and 29% in the numbers of species and genera known for the three states combined, respectively. Overall, the 10 most species-rich families were Tachinidae, Cecidomyiidae, Tabanidae, Psychodidae, Sarcophagidae, Stratiomyidae, Bombyliidae, Syrphidae, Tephritidae, and Asilidae. The 10 most diverse in the number of genera were Tachinidae, Stratiomyidae, Asilidae, Mycetophilidae, Syrphidae, Tabanidae, Muscidae, Dolichopodidae, Sarcophagidae, and Chloropidae. So far, 111 scientific papers were published regarding taxonomic, phylogenetic, and biogeographical aspects of the studied families, with the description of 101 new species and three new genera. We expect that additional publications will result from this investigation because several specimens are now curated and being researched by specialists. Abstract Fusariosis (Fusarium guttiforme) is the most destructive disease of the pineapple crop in Brazil. The aim of this study was to evaluate the reaction to an isolate in pineapple genotypes selected for crownless fruit weight (CFW) and total soluble solids (TSS). A total of 1,125 genotypes ('BRS Imperial'×'Pérola') were evaluated. Selection for greatest CFW and TSS was performed during the sexual cycle. The isolate was inoculated on slip-type plantlets using the immersion technique. Plantlets were evaluated for disease intensity in the stem, attributing scores from 0 to 5. Eighteen genotypes were selected with values from 1.514 to 3.307 g (CFW) and from 14.6 to 19.4 °Brix (TSS). Six genotypes reached the highest level of survival against the IT-01 isolate. Selected genotypes have agronomic traits superior to those of their parents, which qualify them to be cycled back into the plant breeding program of the Instituto Agronômico de Pernambuco - IPA. ABSTRACT Objective The incidence of thrombotic events and acute kidney injury is high in critically ill patients with COVID-19. We aimed to evaluate and compare the coagulation profiles of patients with COVID-19 developing acute kidney injury versus those who did not, during their intensive care unit stay. Methods Conventional coagulation and platelet function tests, fibrinolysis, endogenous inhibitors of coagulation tests, and rotational thromboelastometry were conducted on days 0, 1, 3, 7, and 14 following intensive care unit admission. Results Out of 30 patients included, 13 (43.4%) met the criteria for acute kidney injury. Comparing both groups, patients with acute kidney injury were older: 73 (60-84) versus 54 (47-64) years, p=0.027, and had a lower baseline glomerular filtration rate: 70 (51-81) versus 93 (83-106) mL/min/1.73m2, p=0.004. On day 1, D-dimer and fibrinogen levels were elevated but similar between groups: 1780 (1319-5517) versus 1794 (726-2324) ng/mL, p=0.145 and 608 (550-700) versus 642 (469-722) g/dL, p=0.95, respectively. Rotational thromboelastometry data were also similar between groups. However, antithrombin activity and protein C levels were lower in patients who developed acute kidney injury: 82 (75-92) versus 98 (90-116), p=0.028 and 70 (52-82) versus 88 (78-101) µ/mL, p=0.038, respectively. Mean protein C levels were lower in the group with acute kidney injury across multiple time points during their stay in the intensive care unit. Conclusion Critically ill patients experiencing acute kidney injury exhibited lower endogenous anticoagulant levels. Further studies are needed to understand the role of natural anticoagulants in the pathophysiology of acute kidney injury within this population. COVID19. Abstract Leaf spinescence in pineapple plants was evaluated in a total of 2.014 F1 individuals as follows: 1.125 plants of the progeny of the Imperial BRS cv. × Pérola cv. cross, and 889 plants of the progeny of the Pico de Rosa cv. × BRS Imperial cv. cross. The genetic hypothesis with two loci and epistatic interaction of the piping (P) over spiny (S) trait was tested. The chi-square test was performed on deviations between the observed and expected numbers based on the genetic hypotheses formulated. The “BRS Imperial” × “Pérola” and “Pico de Rosa” × “BRS Imperial” progenies segregated into 1:0:1 (spineless:spiny tip:spiny), expressing phenotypes of their respective parents. The spineless leaf (“BRS Imperial”) is determined by the “Ppss” genotype, with the locus “P” in heterozygosity and the locus “S” in recessive homozygosity. The spiny leaf margin (“Pérola” and “Pico de Rosa”) is controlled by the “ppss” genotype.

RESUMO Em dezembro de 2019, uma série de pacientes com pneumonia grave foi identificada em Wuhan, província de Hubei, na China. Esses pacientes evoluíram para síndrome respiratória aguda grave e síndrome do desconforto respiratório agudo. Posteriormente, a COVID-19 foi atribuída a um novo betacoronavírus, o coronavírus da síndrome respiratória aguda grave 2 (SARS-CoV-2). Cerca de 20% dos pacientes com diagnóstico de COVID-19 desenvolvem formas graves da doença, incluindo insuficiência respiratória aguda hipoxêmica, síndrome respiratória aguda grave, síndrome do desconforto respiratório agudo e insuficiência renal aguda e requerem admissão em unidade de terapia intensiva. Não há nenhum ensaio clínico randomizado controlado que avalie potenciais tratamentos para pacientes com infecção confirmada pela COVID-19 no momento da publicação destas recomendações de tratamento. Dessa forma, essas recomendações são baseadas predominantemente na opinião de especialistas (grau de recomendação de nível C).
ABSTRACT In December 2019, a series of patients with severe pneumonia were identified in Wuhan, Hubei province, China, who progressed to severe acute respiratory syndrome and acute respiratory distress syndrome. Subsequently, COVID-19 was attributed to a new betacoronavirus, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Approximately 20% of patients diagnosed as COVID-19 develop severe forms of the disease, including acute hypoxemic respiratory failure, severe acute respiratory syndrome, acute respiratory distress syndrome and acute renal failure and require intensive care. There is no randomized controlled clinical trial addressing potential therapies for patients with confirmed COVID-19 infection at the time of publishing these treatment recommendations. Therefore, these recommendations are based predominantly on the opinion of experts (level C of recommendation).

Abstract This study aimed to evaluate the effect of two Embryo Manipulation Solutions (EMS and EMS supplemented) in maintenance of the viability of embryos, initially using structures derived from mice (first phase). Next, the efficiency of these solutions in routines of bovine embryo transfer was evaluated (second stage). Mice embryos were used in the stages of early blastocyst, and compact morula grades I and II. These embryos were initially randomly distributed and maintained for four hours in three solutions: Modified phosphate buffered saline (PBS; Control); EMS (treatment 1), and EMS supplemented (treatment 2). Subsequently, they were cultured in TCM 199 medium and evaluated in terms of total number of cells, morphometric characteristics, ultra structural aspects, detection of cell apoptosis, and quantification of Hsp70.3 gene expression. In the second phase, these same solutions were tested in the transfer of quality I and II bovine embryos (excellent and good). These embryos were transferred fresh to 58 recipients. The results showed that the total number of cells in embryos expanded blastocyst (ExB), the number of apoptotic cells, the cell, nuclear, nucleolar diameter and the nucleus/nucleolus ratio was similar among the treatments. The pregnancy rate shown on second phase was also similar. However, the EMS supplemented expressed more Hsp70.3 than EMS. The expression of Hsp70.3 was also greater for embryos in EMS than that of EMS supplemented. The McII embryos, EMS and EMS supplemented samples also expressed more Hsp70.3 compared to control embryos. In conclusion, the tested solutions can be used in routine embryo transfer techniques, replacing modified PBS solution as an effective media in maintaining embryo viability.

Resumo Justificativa e objetivos A estenose aórtica é o tipo mais comum de doença valvular cardíaca. A substituição percutânea de válvula aórtica tornou-se a alternativa para pacientes cirúrgicos considerados de alto risco. A ventilação mecânica controlada com intubação traqueal tem sido a escolha para esse tipo de procedimento; porém, o uso de ventilação não invasiva em pacientes cardíacos mostrou ser benéfico. Janus é uma nova máscara facial que permite a aplicação de suporte à VNI durante a anestesia. Nosso objetivo primário foi avaliar a viabilidade da substituição transcateter de valva aórtica com monitoração ecocardiográfica transesofágica prolongada sob sedação inalatória profunda através de uma nova máscara para ventilação não invasiva. Métodos Estudo observacional de série de casos que incluiu cinco pacientes com estenose aórtica em fase crítica, submetidos à anestesia inalatória com sevoflurano para substituição transcateter de valva aórtica em uma sala híbrida de um hospital universitário. Monitores padrão e índice bispectral foram usados, seguidos de indução inalatória e colocação da máscara Janus. A anestesia foi mantida com sevoflurano. Os pacientes foram transferidos para a unidade de terapia intensiva após o procedimento. As complicações relacionadas ao uso da máscara, a acessibilidade da ecocardiografia transesofágica e as implicações respiratórias para os pacientes foram registradas. Resultados Todos os procedimentos transcorreram sem incidentes e não foram observadas complicações maiores no intraoperatório. Um paciente apresentou retenção de CO2 (50 mmHg) e vazamento de sevoflurano em torno da abertura central da máscara, ambos sem significância clínica. Conclusões O uso de anestesia inalatória com a máscara facial Janus é uma opção segura e eficiente à anestesia geral com intubação traqueal para substituição transcateter de valva aórtica e pode facilmente acomodar o uso de ecocardiografia transesofágica no intraoperatório.
Abstract Background and objectives Aortic stenosis is the most common type of heart valve disease. Percutaneous aortic valve replacement has become the alternative for patients considered at high risk for surgery. Controlled mechanical ventilation with tracheal intubation has been the choice for this type of procedure, however the use of noninvasive ventilation in cardiac patients has shown to be beneficial. Janus is a novel full-face mask that allows application of noninvasive ventilation support during anesthesia. Our main objective was to evaluate the feasibility of transcatheter aortic valve replacement with prolonged transesophageal echocardiographic monitoring under deep inhalational sedation delivered through a new mask for noninvasive ventilation. Methods A case series observational study that included five patients with critical aortic stenosis that underwent inhalational anesthesia with sevoflurane for transcatheter aortic valve replacement in a hybrid room of a teaching hospital. Standard monitors and bispectral index were used, followed by inhalational induction and placement of the Janus mask. Anesthesia was maintained with sevoflurane. Patients were transferred to intensive care unit after the procedure. Complications related to the mask use, transesofageal echocardiography accessibility and respiratory implications to the patients were recorded. Results All procedures were uneventful and no major complications were observed intraoperatively. One patient presented CO2 retention (50 mmHg) and sevoflurane leak around the central opening of the mask, both without clinical significance. Conclusions The use of inhalational anesthesia with the facial mask Janus is a safe and efficient alternative to general anesthesia with tracheal intubation for transcatheter aortic valve replacement and can easily accommodate the use of transesophageal echocardiography intraoperatively.

Resumo O objetivo do trabalho foi avaliar o efeito da somatotropina recombinante bovina (rbST) e da tiroxina (T4) sobre a sobrevivência e o crescimento de folículos ovarianos pré-antrais (FOPA) bovinos cultivados in vitro. Fragmentos ovarianos foram coletados em abatedouros locais e imediatamente fixados para histologia clássica e microscopia eletrônica de transmissão (controle não cultivado). Os demais fragmentos foram cultivados in situ por sete dias em meio essencial mínimo sozinho (MEM+ - controle cultivado), ou na presença de 1.000 ng/mL de rbST e 20 ng/mL de T4, isoladamente ou em associação. Após sete dias, houve redução (P<0,05) na percentagem de folículos normais no MEM+ sozinho ou quando adicionado T4. No diâmetro oocitário, houve redução no MEM+ sozinho. Não houve influência (P>0,01) do meio utilizado, no diâmetro folicular, dos FOPA cultivados por sete dias. A análise ultraestrutural demonstrou danos celulares. Conclui-se que, apesar da presença da rbST manter a taxa de folículos morfologicamente normais durante o cultivo por sete dias (observado por microscopia óptica), o uso de rbST e T4 em meio de cultivo in vitro de FOPA, nas concentrações utilizadas no presente experimento, não exerce efeitos benéficos quanto à integridade ultraestrutural e aos crescimentos oocitário e folicular.
Abstract The aim of the study was to evaluate the effect of recombinant bovine somatotropin (rbST) and thyroxine (T4) on survival and growth of bovine preantral ovarian follicles (PAOF) cultured in vitro. Ovarian fragments were collected in local abattoirs and immediately fixed for classical histology and transmission electron microscopy (non-cultured control). The other fragments were then cultured in situ for seven days in minimum essential medium alone (MEM+ - cultured control) or in the presence of 1,000 ng/mL rbST and 20 ng/mL T4, isolated or associated. After seven days, there was a reduction (P<0.05) in the percentage of normal follicles in MEM+ alone or with T4. In oocyte diameter, there was a reduction in MEM+ alone. There was no influence (P>0.01) of the medium used on the follicular diameter of the PAOF cultured for seven days. Ultrastructural analysis showed cell damage. In conclusion, the presence of rbST maintains the rate of morphologically normal follicles during the culture for seven days (observed by optical microscopy), but it does not exert beneficial effects on its ultrastructural integrity and oocyte and follicular growth.

O objetivo do presente estudo foi analisar o efeito de programas de treinamento com videogames ativos (VGAs) estruturados e não estruturados sobre a composição corporal e desempenho nos testes de aptidão física durante seis semanas. Participaram do estudo 20 homens, os quais foram divididos homogeneamente em três grupos: Grupo controle - GC (n=7; 20.6 ± 2.8 anos), Grupo Experimental Estruturado -GET (n=6; 20.2 ± 2.1 anos) e Grupo Experimental Não estruturado GEN (n=7; 19,1 ± 1,2 anos). O programa com VGAs foi realizado três sessões por semana (30 minutos cada) durante seis semanas. Houve melhoras significativas nos testes de apoio de frente (GET p=0.02), flexibilidade (GEN: p=0.04 e GET: p=0.01) e preensão manual (GEN: p=0.05). E apenas o GEN aumentou significativamente o percentual de gordura (p=0.05) e massa gorda (p=0.01). Conclui-se que após seis semanas de treinamento com VGAs, os dois grupos experimentais apresentaram manutenção na massa magra e aumento no nível de aptidão física.
The aim of this study was to analyze the effect of training programs with active video games (AVGs) structured and unstructured on body composition and performance in the physical fitness tests during six weeks. Study participants were 20 men who were evenly divided into three groups: Control Group - CG (n = 7; 20.6 ± 2.8 years), Experimental Structured Group -GET (n = 6; 20.2 ± 2.1 year) and Experimental Unstructured Group - GEN (n = 7; 19.1 ± 1.2 years). The AVGs program was performed three times a week (thirty minutes each) during six weeks. There were significant improvements in the front support testing (GET p = 0.02), flexibility (GEN: p = 0.04 and GET: p = 0.01) and handgrip (GEN p = 0.05). And only GEN increased significantly the fat percentage (p = 0.05) and fat mass (p = 0.01). After six weeks of each parallel AVGs training, these two groups presented maintenance in lean mass and increase in the level of physical fitness.

RESUMO A avaliação do potencial produtivo do pasto não é o único fator que pode ser usado para determinar a eficiência de manejo do pastejo, também é necessário considerar as características estruturais do dossel. Neste estudo, objetivou-se avaliar a influência de dois critérios para definição do período de descanso do pasto de capim-marandu sobre a massa de forragem, a composição morfológica e as características estruturais do pasto durante a estação chuvosa. Os períodos de descanso estudados foram: fixo de 30 dias (PD30) e variável, conforme a interceptação de 95% da radiação fotossinteticamente ativa (IL95). O delineamento experimental utilizado foi em blocos inteiramente casualizados, com duas repetições de área e três repetições dentro de bloco. Os animais (vacas mestiças) foram colocados nos piquetes, logo que os pastos atingiam os critérios de manejo estabelecidos e permaneciam dentro dos mesmos durante três dias, de forma a atingir a altura de resíduo de 20-25 cm. Menor intervalo entre pastejo (22,8 vs. 30 dias), menor altura pré-pastejo (35,9 vs. 42,3 cm) e menor massa de forragem verde (4.411 vs. 5.290 kg/ha.ciclo) foram observados no pasto sob IL95. Contudo, o pasto sob IL95 apresentou maior percentual de lâmina foliar (48,3 vs. 41,1) e menor percentual de material morto (19,0 vs. 25,4) na massa de forragem pré-pastejo em relação ao pasto com PD30. O manejo do pasto de capim-marandu, baseado em período de descanso fixo de 30 dias, ao longo da estação chuvosa, compromete a estrutura do pasto, reduzindo a participação de folhas e aumentando os percentuais de colmo e material senescente na forragem disponível. A menor massa de forragem em pré-pastejo observada sob IL95 é compensada pelo maior número de ciclos de pastejo dentro da estação chuvosa.
ABSTRACT An evaluation of productive potential is not the only factor used to determine grazing management efficiency; it is also necessary to know the structural characteristics of the canopy. The objective of this study was to evaluate the effects of two management styles on Marandu palisade grass pastures under rotational stocking with respect to the herbage mass, morphological composition, and structural characteristics of the pasture. The treatments under consideration were a) a fixed rest period of 30 days (RP30) or b) a rest period based on the time necessary for the canopy to reach 95% light interception (LI95). The experimental design was a completely randomized block with two blocks and three replicates within the blocks. Animals were placed in the paddocks when the grass reached the established management criteria, which were maintained for three days to reach 25 cm in stubble height. Shorter grazing intervals (22.8 vs. 30 days), lower pre-grazing heights (35.9 vs. 42.3 cm), and lower forage masses (4,411 vs. 5,290 kg/ha.cycle) were observed in LI95 pastures. The LI95 treatment had a higher leaf percentage (48.3 vs. 41.1) and the lowest percentage of dead forage (19.0 vs. 25.4) in the pre-grazing forage mass of the pasture in relation to that of RP30. Performing pasture management based on RP30 throughout the season is harmful to the pasture structure because it reduces the leaf percentage and increases the stem and senescent material in the available forage. The lower pre-grazing forage mass observed under LI95 management is offset by more grazing cycles within the rainy season.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

A melhoria da qualidade dos cuidados pré-natais e dos cuidados intensivos neonatais resultaram, nas últimas décadas, numa continuada diminuição da morbilidade e da mortalidade perinatais e infantis, particularmente de recém-nascidos muito pré-termo. No entanto, há uma grande variabilidade entre unidades de saúde na decisão sobre procedimentos e intervenções, quer a nível nacional quer internacional, que se pode refletir em desigualdades nos resultados em saúde e que importa identificar e compreender. Este artigo tem como objetivos descrever: (1) o projeto europeu “Effective Perinatal Intensive Care in Europe” (EPICE); (2) o estudo piloto desenvolvido na Região Norte de Portugal para testar o protocolo e o instrumento de recolha de dados do estudo de coorte; (3) o recrutamento e amostra final da coorte EPICE-Portugal. O projeto EPICE desenvolve-se em 19 regiões de 11 Estados Membros da União Europeia e tem como objetivo investigar como o conhecimento científico é efetivamente aplicado no acompanhamento de recém-nascidos muito pré-termo, usando metodologias quantitativas e qualitativas. Em Portugal, o estudo integra todas as unidades públicas de obstetrícia e neonatologia da região Norte e de Lisboa e Vale do Tejo (LVT), bem como três unidades privadas de LVT. Os procedimentos do estudo (recrutamento e questionários) foram testados em quatro unidades hospitalares da região Norte de Portugal, através da recolha de informação do processo clínico de 21 nados-vivos, entre maio e dezembro de 2010, sobre a gravidez e os cuidados pré-natais, o parto, os cuidados neonatais e a alta hospitalar, usando um formulário estruturado. O estudo piloto permitiu testar o instrumento de recolha de dados e verificar que, em geral, a informação registada nos processos era suficiente para assegurar a implementação do projeto EPICE em Portugal. O recrutamento da coorte EPICE-Portugal decorreu entre 1 de junho de 2011 e 31 de maio de 2012, tendo sido identificados 724 nados-vivos muito pré-termo, bem como 95 interrupções médicas da gravidez e 155 mortes fetais com idade gestacional entre as 22+0 e as 31+6 semanas de gestação. Dos 724 nados-vivos, 607 tiveram alta hospitalar, tendo sido obtidos 544 (89,6%) consentimentos para as avaliações de seguimento.
In the last decades, the improvement of antenatal and neonatal care led to a continued decrease in perinatal and infant morbidity and mortality, particularly for very preterm infants. However, there is a great variability in medical procedures and interventions across health units, both at national and international levels, which can result in avoidable inequalities in health outcomes. This study intends to describe (1) the European project “Effective Perinatal Intensive Care in Europe” (EPICE), designed to identify and understand such variation; (2) the pilot study conducted in the Northern Region of Portugal to test the protocol and the questionnaire for data collection and (3) the recruitment and final sample of the EPICE-Portugal cohort. The EPICE project includes 19 regions from 11 EU Member states and aims to explore how scientific knowledge is effectively applied to monitor very preterm infants, using both quantitative and qualitative methodologies. In Portugal, this study involves all the public maternity and neonatal intensive care units from Northern and Lisbon and Tagus Valley (LVT) regions, along with 3 private units from LVT. The study procedures (recruitment and questionnaires) were tested in 4 hospitals in the Northern region of Portugal, by collecting information from the clinical records of 21 live births, born between May and December 2010. Data on pregnancy and prenatal care, childbirth, neonatal care and discharge was collected using a structured questionnaire. The pilot study tested data collection instruments and showed that, in general, the information documented in clinical records was enough to answer the questionnaire items, guaranteeing the feasibility of the EPICE project in Portugal. The EPICE-Portugal recruitment occurred between 1st june 2011 and 31st May 2012, identifying 724 very preterm live-births, as well as 95 terminations of pregnancy and 155 fetal deaths, from 22+0 to 31+6 weeks of gestation. The 724 live births resulted in 607 babies discharge alive, and written informed consent for the follow-up evaluations was obtained for 544 (89.6%) infants.
