RESUMO: A Doença de Aujeszky (DA) é uma enfermidade de suínos altamente infecciosa, causada pelo herpesvírus suíno 1 (SuHV 1). O presente estudo objetivou avaliar a biosseguridade das granjas comerciais capixabas de suínos e realizar o primeiro inquérito sorológico para Doença de Aujeszky no Estado do Espírito Santo, Brasil. Foram selecionadas 55 granjas em 27 municípios do Estado, onde se coletou 416 amostras de soro de suínos para serem submetidos ao diagnóstico sorológico para DA. Por fim, foi realizado inquérito, mediante questionário para obter informações sobre a biosseguridade das granjas. Os resultados pela técnica de ELISA indicaram ausência de anticorpos para DA. Em relação à biosseguridade das granjas, 56,4% das granjas não atenderam o requisito de cerca de isolamento interno, 67,3% não possuíam vestiários, 72,7% não disponibilizavam roupas específicas para funcionários e visitantes, 85,5% não exigiam a prática de banho dos visitantes para acesso aos galpões e somente 10,9% das granjas faziam o controle do fluxo de visitantes e veículos que acessam a propriedade. Outro fator destacado é que apenas 25,5% das granjas apresentavam área para realizar quarentena dos animais de reposição e 69,1% possuíam o embarcador localizado próximo às baias dos animais. Com os resultados, concluiu-se que há necessidade de melhoria das medidas de biosseguridade nas granjas comerciais no Estado do Espírito Santo e que não houve presença da Doença de Aujeszky nos animais avaliados. ABSTRACT: Aujeszky’s Disease (AD) is a highly infectious swine disease caused by Suid herpesvirus 1 (SuHV 1). The present study evaluated the biosafety of commercial swine farms and carried out the first serological survey for Aujeszky’s Disease in Espírito Santo State, Brazil. A total of 55 farms in 27 municipalities of the state were selected, where 416 swine serum samples were collected to be submitted to serological diagnosis for AD. Finally, a survey was carried out using a questionnaire to obtain information on the biosecurity of the farms. The results by the ELISA technique indicated the absence of antibodies for AD. Regarding the biosecurity of the farms, 56.4% of the farms did not meet the requirement of internal isolation, 67.3% did not have changing rooms, 72.7% did not provide specific clothing for employees and visitors, 85.5% did not require visitors bathe to access the sheds and only 10.9% of the farms controlled the flow of visitors and vehicles accessing the property. Another noteworthy factor is that only 25.5% of the farms had an area to quarantine replacement animals and 69.1% had an access ramp located close to the animal pens. With the results, it was concluded that there is a need to improve biosecurity measures on commercial farms in the state of Espírito Santo and that there was no presence of Aujeszky’s Disease in the evaluated animals. ABSTRACT This study analyzes the operational efficiency of Brazil’s 30 largest airports from 2014 to 2018, using a two-stage model. Operational efficiency is defined as an airport’s capacity to effectively utilize resources, such as facilities, staff, and technology, to cater to air traffic and passenger needs. The first stage involved measuring operational efficiency through data envelopment analysis. The second stage used a three-level hierarchical linear model to identify influencing variables. Key findings reveal that location significantly impacts airport efficiency, which generally declined during the study period. The interest rate, the only notable economic factor, had a negative effect on efficiency. Factors like the number of aircraft parking positions, years of airport operation, and the number of airlines positively influenced efficiency. Conversely, governance structure, airport size, commercial establishment count, and vehicle parking lot numbers didn’t notably affect efficiency variation. This methodological approach provided more accurate predictions than traditional regression models. ABSTRACT Objective: To evaluate the association of TSH, free T3 (FT3), free T4 (FT4), and conversion (FT3:FT4) ratio values with incident hypertension. Materials and methods: The study included data from participants of the ELSA-Brasil study without baseline hypertension. Serum TSH, FT4 and FT3 levels, and FT3:FT4 ratio values were assessed at baseline, and incident hypertension (defined by blood pressure levels ≥ 140/90 mmHg) was estimated over a median of 8.2 years of follow-up. The risk of incident hypertension was evaluated considering a 1-unit increase in TSH, FT4, FT3, and conversion ratio values and after dividing these variables into quintiles for further analysis using Poisson regression with robust variance. The results are presented as relative risks (RR) and 95% confidence intervals (CIs) before and after adjustment for multiple variables. Results: The primary analysis incorporated data from 5,915 euthyroid individuals, and the secondary analysis combined data from all euthyroid individuals, 587 individuals with subclinical hypothyroidism, and 31 individuals with subclinical hyperthyroidism. The rate of incident hypertension was 28% (95% CI: 27%-29.3%). The FT4 levels in the first quintile (0.18-1.06 ng/dL) were significantly associated with incident hypertension (RR: 1.03, 95% CI: 1.01-1.06) at follow-up. The association between FT4 levels in the first quintile and incident hypertension was also observed in the analysis of combined data from euthyroid individuals and participants with subclinical thyroid dysfunction (RR: 1.04, 95% CI: 1.01-1.07). The associations were predominantly observed with systolic blood pressure levels in euthyroid individuals. However, in the combined analysis incorporating euthyroid participants and individuals with subclinical thyroid dysfunction, the associations were more pronounced with diastolic blood pressure levels. Conclusion: Low FT4 levels may be a mild risk factor for incident hypertension in euthyroid individuals and persons with subclinical thyroid dysfunction. ABSTRACT Schools appear to be a key place to develop healthy eating habits. Currently in Portugal there is no mandatory legislation regarding school mid-morning snacks, allowing the children to bring all types of foods from home. This study aims to assess the effects of a school-based intervention in the dietary and nutritional quality of mid-morning snacks among children of São Miguel Island, Azores. The protocol describes the design of a quasi-experimental study that will include children attending second grade. The intervention involves children, school and family and will consist of two main components: i) a dietary education component through a storybook; and ii) dietary evaluations of mid-morning snacks brought from home during the intervention. The effects will be evaluated through assessment of the dietary quality of mid-morning snacks converted into a nutritional traffic-light label. To measure children’s nutrient intake an electronic database, including the Portuguese Food Composition Table will be used. Data on mid-morning snacks foods will be collected pre- and post-intervention. Through the implementation of this school-based intervention that involves children, their parents and teachers we expect to improve children’s food choices in mid-morning snacks. Ultimately, it is expected that these findings will contribute to the short- and long- term development of future dietary interventions in schools.

RESUMO A escola tem sido considerada um local privilegiado para a aquisição de hábitos alimentares saudáveis. Atualmente, em Portugal, não existe legislação obrigatória referente ao lanche da manhã realizado na escola, possibilitando que as crianças consumam qualquer tipo de alimento trazido de casa. Este estudo tem como objetivo avaliar os efeitos de um programa de educação alimentar na qualidade alimentar e nutricional dos lanches da manhã em crianças da ilha de São Miguel, Açores. O protocolo descreve o desenho de um estudo quase-experimental que irá incluir crianças do segundo ano de escolaridade. A intervenção envolverá as crianças, a escola e as suas famílias, e consiste em dois eixos principais: i) inclusão da educação alimentar no currículo escolar, através da leitura de um livro de histórias; e ii) avaliações qualitativas dos lanches da manhã durante a intervenção. A avaliação da qualidade alimentar dos lanches será realizada utilizando um semáforo nutricional. A avaliação da qualidade nutricional será realizada através de uma base de dados eletrónica, que incluiu a Tabela Portuguesa de Composição dos Alimentos. A recolha de dados será realizada nos momentos pré- e pós-intervenção. Espera-se que a implementação deste programa de educação alimentar possibilite a melhoria das escolhas alimentares das crianças no lanche da manhã, em resposta a uma intervenção que envolverá as crianças, os seus pais e professores. Em última instância, espera-se que os resultados possam contribuir para a definição de futuras políticas alimentares e nutricionais, em contexto escolar, a curto e longo prazo.

resumo A arbitragem no futebol exige alto preparo físico. As federações aplicam testes antropométricos para classificar os árbitros como aptos ou não para a função. O presente trabalho teve como objetivo avaliar a validade do percentual de gordura corporal (%GC) aferido por meio de diferentes métodos de avaliação referenciado em um modelo de quatro compartimentos (4C). O %GC foi estimado por seis métodos: antropometria; bioimpedância elétrica (BIA); absortometria dupla de raios-X (DXA); pletismografia por deslocamento de ar (ADP); modelo de três e quatro compartimentos (3 e 4C). Bland-Altman e correlações intraclasse (ICC) foram realizadas para determinar a validade de todos os métodos em comparação com o modelo de referência 4C. Os resultados da ANOVA revelaram que não houve diferença significativa (F = 1,541; p = 0,182) entre o %GC analisado pelo modelo 4C (15,98 ± 6,20), antropometria (média ± DP, 18,46 ± 7,03), ADP (16,19 ± 6,24), BIA (16,67 ± 5,30), DXA (20,33 ± 6,56) e 3C (16,92 ± 5,53). Segundo Bland-Altman todos os métodos superestimam o %GC em comparação com o 4C. A melhor concordância foi obtida na ADP (viés= -0,2), seguida da BIA (bias = -0,6), 3C (viés = -0,9), antropometria (viés = -2,4) e DXA (viés = -4,3). O ICC foi excelente (ICC≥0,90) na maioria dos métodos, exceto para antropometria (ICC = 0,80) e DXA (ICC = 0,71). Os resultados sugerem que ADP, BIA e 3C foram os melhores métodos para avaliação do %GC. No entanto, a antropometria continua sendo um método válido para monitorar o %GC.

abstract The arbitration exercise in a soccer game requires high physical fitness and all federations apply physical tests to referees, including anthropometric tests, classifying them as fit or not for the role. The aim of this study was to assess the validity of the total body fat percentage (%BF) through different evaluation methods of body composition referenced in a four-compartment (4C) model. Cross-sectional study performed in 2018 with 21 elite male referees. %BF was estimated by 4 methods: anthropometry; bioelectrical impedance analysis (BIA); Dual X-ray absorptiometry (DXA) and air displacement plethysmography (ADP). Moreover, three and four-compartment (3 and 4C) models were calculated. Bland–Altman and intraclass correlations (ICC) analysis were performed to determine validity of all methods compared to a 4C reference. The results of one-way ANOVA revealed that there was no significant difference (F=1.541; p=0.182) between %BF analyzed by 4C model (15.98 ± 6.20), anthropometry (mean ± SD, 18.46 ± 7.03), ADP (16.19 ± 6.24), BIA (16.67 ± 5.30), DXA (20.33 ± 6.56) and 3C (16.92 ± 5.53). The Bland–Altman analysis showed that all methods analyzed overestimate %BF compared to the 4C model. The best agreement was obtained from the ADP evaluation (bias=-0.2), followed by BIA (bias=-0.6), 3C (bias=-0.9), anthropometry (bias=-2.4) and DXA (bias=-4.3). Validation assessed by ICC was excellent (ICC≥0.90) in most methods, except for anthropometry (ICC=0.80) and DXA (ICC=0.71). Overall, the results suggest that ADP, BIA and 3C were the best method to %BF evaluation. Nevertheless, anthropometry remains as a feasible method to monitor %BF of elite soccer referees.

O artigo analisa o processo de desigualdades sociais do período recente do capitalismo histórico, com ênfase no acirramento da iniquidade e como ela impacta nos meios de luta dos movimentos sociais. Primeiro, apresentamos o debate teórico-metodológico sobre desigualdade, a lógica das expulsões, os novos riscos sociais e suas consequências para a democracia contemporânea. Em seguida, avaliamos como os movimentos sociais têm lutado contra a desigualdade e a retirada de direitos por meio de novas formas de articulação, manifestação e formação de movimentos antissistêmicos. Partindo do debate agência-estrutura, demonstramos como o local e o global se entrelaçam na dinâmica das desigualdades e a luta dos diferentes movimentos sociais. E, por fim, apontamos os principais desafios para que os movimentos recuperem sua capacidade de promover a emancipação social.

This article analyzes the process of social inequalities in the recent historical capitalism, highlighting its intensification and how it impacts the means of struggle used by social movements. First, we present the theoretical and methodological debate on inequality, the logic of expulsions, the new social risks, and their consequences for contemporary democracy. Then, we evaluate how social movements have fought against inequality and the stripping of rights by new forms of articulation, manifestation and formation of anti-systemic movements. Based on the agency-structure debate, we show how the local and the global are intertwined in the dynamics of inequality and the struggle of different social movements. Finally, we point out the main challenges for movements to recover their capacity to promote social emancipation.

L’article analyse la démarche des inégalités sociales dans la période récente du capitalisme historique, en soulignant son intensification et son impact sur les moyens de lutte des mouvements sociaux. Dans la première partie, nous présentons le débat théorico-méthodologique sur les inégalités, la logique des expulsions, les nouveaux risques sociaux et leurs conséquences sur la démocratie contemporaine. Ensuite, nous évaluons comment les mouvements sociaux ont lutté contre les inégalités et le retrait des droits à travers de nouvelles formes d’articulation, de manifestation et de formation des organisations anti-systémiques. En s’appuyant dur le débat agence-structure, nous démontrons comment le local et le global s’entremêlent dans la dynamique des inégalités et la lutte des différents mouvements sociaux. Enfin, nous soulignons les principaux défis que les mouvements doivent relever pour retrouver leur capacité à promouvoir l’émancipation sociale.

RESUMO Objetivo: Avaliar a capacidade do Teste de Caminhada de 6 Minutos para predizer a melhora do estado funcional físico em longo prazo de pacientes sobreviventes à unidade de terapia intensiva. Métodos: Foram avaliados, de forma prospectiva, entre fevereiro de 2017 e agosto de 2018, em um ambulatório pós-unidade de terapia intensiva, 32 sobreviventes à unidade de terapia intensiva. Foram inscritos consecutivamente os pacientes com permanência na unidade de terapia intensiva acima de 72 horas (para admissões emergenciais) ou acima de 120 horas (para admissões eletivas) que compareceram ao ambulatório pós-unidade de terapia intensiva 4 meses após receberem alta da unidade de terapia intensiva. A associação entre a distância percorrida no Teste de Caminhada de 6 Minutos realizado na avaliação inicial e a evolução do estado funcional físico foi avaliada durante 8 meses, com utilização do Índice de Barthel. Resultados: A distância média percorrida no Teste de Caminhada de 6 Minutos foi significantemente mais baixa nos sobreviventes à unidade de terapia intensiva do que na população geral (405m versus 557m; p < 0,001). A idade (β = -4,0; p < 0,001) e a fraqueza muscular (β = -99,7; p = 0,02) se associaram com a distância percorrida no Teste de Caminhada de 6 Minutos. A distância percorrida no Teste de Caminhada de 6 Minutos se associou com melhora do estado funcional físico no período de 8 meses de acompanhamento desses pacientes (razão de chance para cada 10m: 1,07; IC95% 1,01 - 1,16; p = 0,03). A área sob a curva Característica de Operação do Receptor para predição da melhora funcional física pelo Teste de Caminhada de 6 Minutos foi de 0,72 (IC95% 0,53 - 0,88). Conclusão: O Teste de Caminhada de 6 Minutos, realizado 4 meses após a alta da unidade de terapia intensiva, predisse com precisão moderada a melhora do estado funcional físico de sobreviventes à unidade de terapia intensiva.

ABSTRACT Objective: To evaluate the ability of the 6-Minute Walk Test to predict long-term physical functional status improvement among intensive care unit survivors. Methods: Thirty-two intensive care unit survivors were prospectively evaluated from February 2017 to August 2018 in a post-intensive care unit outpatient clinic in Brazil. Individuals with intensive care unit stays > 72 hours (emergency admissions) or > 120 hours (elective admissions) attending the post-intensive care unit clinic four months after intensive care unit discharge were consecutively enrolled. The association between the 6-Minute Walk Test distance at baseline and physical functional status was assessed over 8 months using the Barthel Index. Results: The mean 6-Minute Walk Test distance was significantly lower in intensive care unit survivors than in the general population (405m versus 557m; p < 0.001). Age (β = -4.0; p < 0.001) and muscle weakness (β = -99.7; p = 0.02) were associated with the 6-Minute Walk Test distance. A 6-Minute Walk Test distance was associated with improvement in physical functional status over the 8-month follow-up (odds ratio for each 10m of 1.07; 95%CI 1.01 - 1.16; p = 0.03). The area under the Receiver Operating Characteristic curve for the 6-Minute Walk Test prediction of physical functional status improvement was 0.72 (95%CI 0.53 - 0.88). Conclusion: The 6-Minute Walk Test performed 4 months after intensive care unit discharge predicted long-term physical functional status among intensive care unit survivors with moderate accuracy.

Abstract Objectives To evaluate the impact of invasive mechanical ventilation associated with two serum inflammatory cytokines and clinical indicators, on the second day of life, as predictors of bronchopulmonary dysplasia in very low birth weight preterm infants. It was hypothesized that the use of invasive mechanical ventilation in the first hours of life is associated with biomarkers that may predict the chances of preterm infants to develop bronchopulmonary dysplasia. Methods Prospective cohort of 40 preterm infants with gestational age <34 weeks and birth weight <1500 g. The following were analyzed: clinical variables; types of ventilator support used (there is a higher occurrence of bronchopulmonary dysplasia when oxygen supplementation is performed by long periods of invasive mechanical ventilation); hospitalization time; quantification of two cytokines (granulocyte and macrophage colony stimulating factor [GM-CSF] and eotaxin) in blood between 36 and 48 h of life. The preterm infants were divided in two groups: with and without bronchopulmonary dysplasia. Results The GM-CSF levels presented a significantly higher value in the bronchopulmonary dysplasia group (p = 0.002), while eotaxin presented higher levels in the group without bronchopulmonary dysplasia (p = 0.02). The use of continuous invasive mechanical ventilation was associated with increased ratios between GM-CSF and eotaxin (100% sensitivity and 80% specificity; receiver operating characteristic area = 0.9013, CI = 0.7791–1.024, p < 0.0001). Conclusions The duration of invasive mechanical ventilation performed in the first 48 h of life in the very low birth weight infants is a significant clinical predictor of bronchopulmonary dysplasia. The use of continuous invasive mechanical ventilation was associated with increased ratios between GM-CSF and eotaxin, suggesting increased lung injury and consequent progression of the disease.

ABSTRACT Strongyloidiasis is a helminthiasis of neglected condition that has no gold standard parasitological diagnosis due to the intermittent release of larvae in feces. This study aimed to use an scFv (single chain variable fragment) obtained by Phage Display, previously validated to detect immune complexes in serum samples from individuals infected with Strongyloides stercoralis by enzyme-linked immunosorbent assay (ELISA). Now the ability of scFv to detect the immune complexes was verified by immunofluorescence, flow cytometry using magnetic beads and surface plasmon resonance (SPR). As ELISA, the SPR, immunofluorescence and flow cytometry demonstrated the ability of scFv to detect immune complexes in sera from individuals with strongyloidiasis and discriminate them from sera of individuals with other parasitic diseases and healthy individuals. Besides de conventional ELISA, the novel approaches can also be promptly applied as auxiliary diagnostic tools to the existing parasitological method for accurate diagnosis of human strongyloidiasis.

RESUMO: Campilobacteriose Genital Bovina (CGB) é uma doença venérea e subclínica que causa problemas reprodutivos em rebanhos, causada por Campylobacter fetus subsp. venerealis. Este trabalho teve como objetivo selecionar peptídeos miméticos ao agente da CGB de uma biblioteca de fagos. Phage display é uma técnica que aplica bibliotecas de bacteriófagos que expõem peptídeos fusionados ao capsídeo viral em seleções biológicas contra proteínas alvo. Biopannings foram realizados para seleção biológica na biblioteca de fagos por meio de soro hiperimune de coelho e extrato proteico de C. fetus subsp. venerealis. Cinco heptapeptídeos selecionados foram considerados miméticos para Cfv-NCTC 10354 a partir de análises de bioinformática e ensaios com soro hiperimune e muco cérvico-vaginal de novilhas. ALASLPL e LSYLFPP foram os peptídeos mais reativos e considerados promissores como possíveis imunógenos miméticos para C. fetus subsp. venerealis.

ABSTRACT: Bovine genital campylobacteriosis (BGC) is a venereal and subclinical disease that affects the fertility of cattle herds, and it is caused by Campylobacter fetus subsp. venerealis . This study selected peptides mimetic to the BGC-causing agent from a phage library. Phage display is a technique that applies bacteriophage libraries that reveal peptides fused to the viral capsid in biological selections against target proteins. Biopannings were performed for biological selection in the phage library using rabbit hyperimmune serum and C. fetus subsp. venerealis protein extract. Five selected heptapeptides were considered mimetic to Cfv-NCTC 10354 based on the results of bioinformatics analysis and assays with hyperimmune serum and cervicovaginal mucus obtained from heifers. ALASLPL and LSYLFPP were the most reactive peptides and considered promising as possible mimetic immunogens for C. fetus subsp. venerealis.

RESUMO Objetivo: Descrever o relato de experiência de um enfermeiro no cuidado ao paciente com a COVID-19 em hemodiálise e posição prona. Método: Relato de experiência. O estudo foi realizado em um hospital filantrópico de uma capital brasileira, no período de maio a agosto de 2020, durante a pandemia do Coronavírus. Resultados: O enfermeiro enfrentou diversos desafios estruturais, materiais, de recursos humanos e assistenciais para o cuidado ao paciente com a COVID-19 em hemodiálise e posição prona. A gestão fundamentada no diálogo e no compartilhamento de informações baseadas em evidências e a implementação de um novo protocolo assistencial foram o alicerce para a reorganização assistencial da equipe de Enfermagem para o cuidado ao paciente com a COVID-19. Conclusão: Diante dessa experiência, destaca-se a importância da atenção de enfermagem às respostas dos pacientes, respaldando suas ações em protocolos assistenciais e fortalecendo as relações humanas. Faz-se necessária a condução de outros delineamentos de estudos de modo a contribuir para a melhoria da assistência de enfermagem e a sobrevida de pacientes com complicações da COVID-19.

ABSTRACT Objective: To describe the experience report of a nurse providing care to COVID-19 patients on hemodialysis and in prone position. Method: Experience report. This study was conducted in a philanthropic hospital in a Brazilian capital from May to August 2020 during the Coronavirus pandemic. Results: The nurse has faced several challenges concerning structure, material, human resources, and care when treating COVID-19 patients on hemodialysis and in prone position. Management based on dialogue and shared evidence-based information and implementation of a new care protocol were a foundation for care reorganization of the Nursing team for providing care to COVID-19 patients. Conclusion: This experience emphasizes the importance of nursing care to patients’ responses, with actions based on care protocols, strengthening human relations. The conduction of different study designs is necessary to contribute to an improved nursing care and survival of patients experiencing COVID-19 complications.

RESUMEN Objetivo: Describir el informe de experiencia de un enfermero en el cuidado al paciente con COVID-19 en hemodiálisis y posición prona. Método: Informe de experiencia. El estudio fue realizado en un hospital filantrópico de una capital brasileña de mayo a agosto de 2020, durante la pandemia de Coronavirus. Resultados: El enfermero enfrentó varios retos estructurales, materiales, de recursos humanos y de asistencia para el cuidado del paciente con COVID-19 en hemodiálisis y posición prona. La gestión basada en el diálogo y en el intercambio de información basada en evidencias y la implementación de un nuevo protocolo asistencial fueron el fundamento para la reorganización asistencial del equipo de Enfermería para el cuidado del paciente con COVID-19. Conclusión: A partir de esta experiencia, se destaca la importancia de la atención de enfermería a las respuestas de los pacientes, respaldando sus acciones en los protocolos de asistencia y fortaleciendo las relaciones humanas. Es necesaria la realización de otros diseños de estudio para contribuir a la mejora de la asistencia de enfermería y la supervivencia de los pacientes con complicaciones de la COVID-19.
