In the clinical practice it is not always easy the differentiation between bacterial and aseptic meningoencephalitis, causing the raise of the treatment price in cases of aseptic meningoencephalitis, the increase of the potential risk of complications, and also a bigger familiar impact. We made a retrospective study of the patients admitted with the diagnosis of meningoencephalitis in the Infantile Hospital Eliseo Noel Caamaño during a 5-years period (377 patients), applying them the BMS (bacterial meningoencephalitis score). Our objective was describing the score at patients' admittance, and classifying them as presenting high or low risk for bacterial meningoencephalitis, and also comparing the diagnoses at the admittance and discharge. As a result, 100 % of the patients with bacterial meningoencephalitis bacteriologically tested showed scores of 2 or higher (high risk); there were also identified 9 low risk patients (score 0 for bacterial meningoencephalitis), who were firstly considered as bacterial positive, and discharged as aseptic meningoencephalitis. The bacterial meningoencephalitis score could be a useful tool in the initial evaluation of the patients with the infectious Neurological Syndrome.

Introducción: En la práctica clínica pediátrica no siempre resulta fácil la diferenciación entre las meningoencefalitis bacterianas y asépticas, lo cual genera encarecimiento del tratamiento en aquellos casos de meningoencefalitis asépticas, incremento del riesgo potencial de complicaciones, así como mayor impacto familiar. Método: Se realizó un estudio retrospectivo de los pacientes ingresados con el diagnóstico de meningoencefalitis en el Hospital Provincial Pediátrico Docente Eliseo Noel Caamaño, de la ciudad de Matanzas, durante un período de 5 años (377 pacientes), a quienes se les aplicó el score para meningoencefalitis bacteriana. Objetivo: Describir el puntaje al ingreso en los pacientes y clasificarlos en bajo o alto riesgo para meningoencefalitis bacteriana, así como compararlos con los diagnósticos al ingreso y egreso. Resultados: El 100 % de los pacientes con meningoencefalitis bacteriana comprobadas bacteriológicamente mostraron puntaje de 2 o mayor (alto riesgo); también identificó 9 pacientes de bajo riesgo (puntaje de 0) para meningoencefalitis bacteriana, los cuales fueron considerados inicialmente como bacterianas y egresados como meningoencefalitis asépticas. Conclusión: El score para meningoencefalitis bacteriana pudiera ser una herramienta útil en la valoración inicial de los pacientes con síndrome neurológico infeccioso.

To choice the right time to initiate an antibiotic therapy is a daily clinical question mark since if there are evidences that in diverse types of infections, the early use of antibiotics is related to an improvement in diagnosis, also is associated with the potential increment of antibiotic resistance. The aim of present review is to discuss on features as which are the entities where the early use of antibiotics is related to a better prognosis, what type or types of antibiotics be used, the onset time and which is the better time to discontinue them.

La elección del momento más oportuno para el inicio de una terapéutica antibiótica es una interrogante clínica diaria, pues si bien existen evidencias de que en muchos tipos de infecciones el uso temprano de antibióticos se relaciona con una mejoría del pronóstico, también se asocia con el potencial incremento de la resistencia antibiótica. Esta revisión tiene como propósito discutir aspectos como cuáles son las entidades donde el uso temprano de antibióticos se relaciona con un mejor pronóstico, qué tipo o tipos de antibióticos utilizar, en qué momento comenzar y cuál es el mejor momento para descontinuarlos.

Sepsis is the main cause of children death around the world, and spends many health resources. The objective of this study is finding out the doctors´ knowledge on sepsis in paediatric patients. We carried out a prospective descriptive study in the period from January to February 2009 applying an inquiry to 103 doctors of the province of Matanzas working full time or part time in settings receiving paediatric patients suspected or with evidences of sepsis, severe sepsis or septic shock. The inquiry states the characterization of the doctors and explores the comprehension of the problem, the knowledge on the diagnosis, the predisposing facts and the initial sepsis management. 67 % of all the participants considered to have a medium knowledge, 21,3 % high and 11,7 % low. The best results were in comprehending the entity and its initial management, with fewer achievements in definitions. The study shows that professionals diagnosing and treating sepsis have knowledge and perception on it, but they are neither even nor sufficient.

La sepsis representa la principal causa de muerte de niños a nivel mundial, y es consumidora de sustanciales recursos de salud. El objetivo del presente estudio es describir el conocimiento de los médicos sobre la sepsis en el paciente pediátrico. Se realizó un estudio descriptivo prospectivo en el periodo de Enero a Febrero del 2009 p ara lo cual se aplicó una encuesta a 103 médicos de la provincia de Matanzas, los cuales se desempeñan total o parcialmente en sitios donde se reciben pacientes pediátricos con sospecha o evidencia de sepsis, sepsis severa o shock séptico. La encuesta define la caracterización de los practicantes y explora la comprensión del problema, los conocimientos acerca del diagnóstico, los factores predisponentes y el manejo inicial de la sepsis. Del total de los encuestados, el 67% consideró poseer un conocimiento medio, el 21,3% alto y el 11,7% bajo. Los mejores resultados correspondieron a la comprensión de la entidad y a su manejo inicial, con menos logros en relación a las definiciones. El estudio muestra que existe conocimiento y percepción de la sepsis por parte del profesional que la diagnóstica y trata, pero estos no son uniformes ni suficientes.

The Pelger-Hüet anomaly was described firstly in 1928 by the Holland physician Pelger and its genetic origin was discovered later by the pediatrician Hüet. It is transmitted with a dominant autosomal character and it is a mutation of the gene codifying the plate B receptor. Beginning from this mutation, the alteration of white cells core took place, mainly in neutrophils, with affectations of the nuclear segmentation and chromatin disturbances. We present a patient entering our hospital as a cause of an animal bite, and there were discovered hypolobulated neutrophils in the periferal plate. The familiar character was confirmed with the finding of this alteration in the patient's mother.

La anomalía de Pelger Hüet fue descrita inicialmente en 1928 por el médico holandés Pelger y su origen genético fue descubierto más tarde por el pediatra Hüet. Se transmite con un carácter autosómico dominante y consiste en una mutación del gen que codifica el receptor de la lámina B. A partir de esta mutación se producen alteraciones en el núcleo de los leucocitos, fundamentalmente en los neutrófilos, con afectación de la segmentación nuclear y trastornos en la cromatina. Presentamos a un paciente que fue ingresado en el hospital por una mordedura de animal y se describió en la lámina periférica la presencia de neutrófilos hipolobulados. El carácter familiar se confirmó por el hallazgo de esta alteración en la madre del paciente.

We present three inpatients with the diagnosis of meningoencephalitis to Salmonella (Non-typhi D in two and C2 in one) attended in the Intensive Care Unit of our hospital from 1989 to 2006. The evolution of the patients infected with the kind non-typhi D was more severe; one of the patients presented osteomyelitis of the lower third of the humerus to the same germen after a 14-days treatment with cefotaxime, and other had a relapse of the meningoencephalitis after 21 days of treatment with cefotaxime and ampicillin. Like other authors, we observed a more aggressive evolution of the infection in these patients in comparison with other bacterial meningoencephalitis, and recommend the treatment with third generation cephalosporin alone at least during 4 weeks or in association with ciprofloxacin.

Se presentan tres pacientes con el diagnóstico de meningoencefalitis a Salmonella (D no typhi en dos y C2 en uno) ingresados en la Unidad de Terapia Intensiva de nuestro hospital desde 1989 a 2006. La evolución de los infectados con el tipo D no typhi fue más severa, presentando uno de los pacientes osteomielitis de tercio inferior del húmero al mismo germen luego de 14 días de tratamiento con cefotaxima y otro con recaída de la meningoencefalitis luego de 21 días de tratamiento con cefotaxima y ampicillín. Al igual que otros autores observamos una evolución más agresiva de la infección en estos enfermos comparados con otras meningoencefalitis bacterianas y se recomienda tratamiento con cefalosporina de tercera generación al menos por 4 semanas sola o en asociación con ciprofloxacina.
